Abstract:We consider the inverse problem of determining the spherically symmetric index of refraction $n(r)$ from a knowledge of the corresponding transmission eigenvalues (which can be determined from field pattern of the scattered wave). Abstract:Background: Drains are usually left after thyroid surgery to prevent formation of hematoma and seroma in the thyroid bed. This is done to reduce complications and hospital stay. Objective evaluation of the amount collected in the thyroid bed by ultrasonography (USG) can help in assessing the role of drains. Methods: A randomized prospective control study was conducted on 94 patients undergoing 102 thyroid surgeries, over a period of fifteen months. Patients included in the study were randomly allocated to drain and non-drain group on the basis of computer generated random number table. The surgeon was informed of the group just before the closure of the wound Postoperatively USG neck was done on first and seventh postoperative day by the same ultrasonologist each time. Any swelling, change in voice, tetany and tingling sensation were also recorded. The data was analyzed using two-sample t-test for calculating unequal variance. Results: Both groups were evenly balanced according to age, sex, and size of tumor, type of procedure performed and histopathological diagnosis. There was no significant difference in collection of thyroid bed assessed by USG on D1 & D7 in the two groups (p = 0.313) but the hospital stay was significantly reduced in the non-drain group (p = 0.007). One patient in the drain group required needle aspiration for collection in thyroid bed. No patient in either group required re-operation for bleeding or haematoma. Conclusion: Routine drainage of thyroid bed following thyroid surgery may not be necessary. Not draining the wound results in lesser morbidity and decreased hospital stay.

Abstract:Purpose To describe a case of multiple thoracoabdominal aneurysms of tuberculous origin treated in an endovascular procedure with the Multilayer stent. Case Report A 16-year-old girl had been treated 4 years previously for a ruptured abdominal aortic aneurysm of tuberculous origin. Due to the presence of 4 rapidly evolving saccular aneurysms of the descending thoracic aorta and a fusiform aneurysm of the suprarenal aorta, an endovascular solution was chosen after the patient refused open surgery. Three uncovered Multilayer stents (16×40, 16×80, and 16×80 mm) were successively implanted with a 1-cm overlap from the left subclavian artery to cover the entire aneurysmal segment of the thoracoabdominal aorta to above the renal arteries. At 18 months, serial imaging studies have shown disappearance of some aneurysms and regression of others. Conclusion In this young patient, the endovascular treatment of a thoracoabdominal aneurysm with an uncovered stent made it possible to stabilize the aneurysm process without exposing the patient to the high morbidity and mortality of open surgery.

Abstract:Seventeen normal pre-school children were observed with their mothers in an experimental setting. Various measurements of their behaviour were made by two observers through a one-way screen. There were two 20-min sessions in one of which the mother was interviewed by a stranger. Mothers subsequently completed the Eysenck personality inventory, form A. Items of behaviour which were associated with the presence of a stranger were: remaining longer than 15 min in a circle drawn around the mother, displacement activities and not speaking for the first 5 min of the session. Proximity to mother in the presence of the interviewer was related to an N(A) score of eight or over on the E.P.I.

Abstract:Drainage after thyroid surgery is widely used to prevent postoperative complications by evacuation of blood and fluids. However, to our knowledge no study has shown the benefit of drainage. Therefore, we performed a prospective, randomized study on the rate of complications after drainage or no drainage in thyroid surgery. One hundred fifty patients were allocated to drainage or no drainage. No difference was seen between the two groups according to the experience of the surgeon, type of operation, diagnosis, weight of thyroid specimens, operation time, and hospital stay. All complications were recorded and resulted in two patients receiving reoperation because of bleeding, two permanent laryngeal nerve palsies, one case of permanent hypocalcemia, ten minor hematomas, one wound infection, and one lymphatic leakage. No difference was seen between the groups. This study does not support prophylactic routine drainage after uncomplicated thyroid surgery.

Abstract:Objective To investigate the function of the negative pressure drainage in thyroid gland surgery.Methods 101 examples of the thyroid gland disease which needed surgery were randomly divided into the negative pressure drainage group (57 examples) and the positive pressure drainage group (44 examples) and the complications of wounds fluid-accumulating, fistula, haematoma and skin petal petal necrosis were recorded. The relations between the different drainage ways and post-processing were analyzed.Results The complication rate of the negative pressure drainage group was significantly lower than that of the positive pressure drainage group (P0.01). The negative pressure drainage group could greatly reduce the time of using atropine and arapping-up time, but it did not affect IssueⅠ healing rate. Conclusion This method had some advantoges over traditional positire pressure drainage.

Abstract:Purpose: We conducted a prospective, randomized study to evaluate the necessity of drainage after uncomplicated thyroid surgery. Methods: The subjects were 135 patients who underwent thyroid surgery between September 2002 and February 2004. The patients were randomized into two groups according to whether drains were inserted at the time of surgery. Group 1 consisted of 68 patients with drains and group 2 consisted of 67 patients without drains. The indications for surgery, procedures performed, local complications (such as infection, seroma, and bleeding or hematoma), necessity for reoperation, and hospital stay were recorded. Results: There were 110 (81.5%) women and 25 (18.5%) men, with a median age of 46.9 +/- 12.5 years. The mean hospital stay was 2.6 +/- 1.0 days in group 1 and 1.3 +/- 0.7 days in group 2 (P = 0.001). Local complications developed in five (7.3%) patients from group 1, as wound infections in two (2.9%), seroma in one (1.5%), and hemorrhage in two (2.9%); and in two (3%) patients from group 2, as seroma in one (1.5%) and hematoma in one (1.5%). Both of the group 1 patients with postoperative hemorrhage required reoperation within 8 h after initial surgery. The hematoma in the group 2 patient was treated successfully with needle aspiration. Conclusion: These findings suggest that the routine use of drains may be abandoned in uncomplicated thyroid surgery, since serious postoperative bleeding rarely occurs and hematomas can be treated by needle aspiration if drains have not been placed. Furthermore, the use of drains prolongs hospital stay and increases the risk of infection.

Abstract:BACKGROUND:Despite the considerable experience gained thus far using endoscopic technologies, the role of total endoscopic thyroidectomy (ET) for papillary thyroid cancer (PTC) remains controversial. We conducted a systematic review and meta-analysis to investigate the safety and effectiveness of total ET compared with conventional open thyroidectomy (OT) in PTC. METHODS:A systematic search was conducted using the PubMed, Embase and Cochrane Library electronic databases up to March 2018. The quality of included studies was evaluated using the Newcastle-Ottawa Scale. Review Manager software version 5.3 was used for the meta-analysis. RESULTS:Twelve studies including 2,672 patients were ultimately included in the systematic review and meta-analysis. ET was associated with longer operative time (P<0.00001), drainage time (P<0.00001) and hospital stay (P=0.03), higher transient recurrent laryngeal nerve (RLN) palsy rate (P=0.004) and a greater amount of drainage fluid (P<0.0001) compared with OT. Furthermore, no significant differences were detected between ET and OT in terms of retrieved lymph nodes (P=0.17), blood loss (P=0.22), transient hypocalcemia (P=0.84), permanent hypocalcemia (P=0.58), permanent RLN palsy (P=0.14), hematoma or bleeding (P=0.15) and seroma (P=0.54). In addition, the rates of tumor recurrence were comparable (P=0.18), whereas the proportions of stimulated thyroglobulin levels <1 ng/mL measured after completion of thyroidectomy and radioactive iodine therapy were less (P=0.02) in the ET than in the OT group. CONCLUSION:ET is not superior to OT in terms of operation and drainage time, amount of drainage fluid, hospital stay or transient RLN palsy, but is comparable to OT in terms of retrieved lymph nodes and permanent complications. Despite the similar tumor recurrence rates between the two approaches, the level of surgical completeness in ET may not be as good as that for OT.

Abstract:Thyroidectomy is a common surgical procedure. The results of some clinical trials suggest that routine drainage does not offer advantages, but the evidence is not strong either in favor of or against this intervention. The effect of routine drainage compared to no drainage in patients subject to thyroidectomy was measured using a meta-analysis. After an extensive literature review, suitable randomized clinical trials were selected for analysis. Outcome measures included the comparative incidence of neck hematoma or seroma and length of hospital stay. Eleven randomized clinical trials were included. There were no statistically significant differences in the incidence of neck hematoma/seroma (OR 1.03, 95% CI 0.59-1.81) between the groups. The mean length of hospital stay was 1.53 days longer for the drainage group (95% CI 1.39-1.68). There was no difference found between routine drainage and no drainage with regard to the frequency of postoperative hematoma/seroma in patients following thyroidectomy. In addition, the mean length of hospital stay was longer in the routine drainage group.

Abstract:Taste sensation in patients with cancer has previously been studied with the cumbersome method of chemical gustometry, which have produced inconsistent results. In this study taste thresholds were determined with the simple and reliable technique of electrogustometry in 51 patients with cancers of the lung (small cell), ovary or breast, and in 29 matched control patients with nonneoplastic disease. Chemical smell thresholds for phenyl‐methyl‐ethyl‐carbinol were studied in the same group of patients. Electrical taste threshold was higher in patients with cancer compared with control patients (30 [12 to 80] μA versus 9.5 [6.1 to 24] μA; median [interquartile range], P < 0.001). No differences were seen in smell thresholds. Patients who responded to chemotherapy obtained a decreased electrogustometric threshold at time of reevaluation (24 [8 to 64] μA versus 30 [15 to 90] μA; median [interquartile range], P < 0.05). These results suggest an effect of the malignant disease itself on taste thresholds. Cancer 68:2260–2265, 1991.

Abstract:Objective: To evaluate whether no drainage has an advantage over routine drainage in patients with thyroid carcinoma after unilateral thyroid lobectomy and central neck dissection. Methods: A total of 104 patients with thyroid cancer who underwent unilateral thyroid lobectomy and central lymph node dissection were randomly assigned into no drainage tube (n=52) and routine drainage tube (n=52) placement groups. General information of each patient was recorded, including the postoperative drainage volume/residual cavity fluid volume, postoperative complications, incision area comfort, and other data, and the thyroid cancer-specific quality of life questionnaire (THYCA-QoL) and patient and observer scar assessment scale (POSAS) were evaluated after surgery. At the 3–6 month follow-up exam, the differences between the two groups were compared based on univariate analysis. Results: Significant differences were not observed in the general and pathological information (including sex, age, body weight, body mass index (BMI), incision length, specimen volume, Hashimoto's thyroiditis, and number of lymph nodes dissected), operation time, and postoperative complications (postoperative bleeding, incision infection, lymphatic leakage, and temporary hypoparathyroidism) between the two groups. The patients in the non-drainage group had a shorter hospital stay (2.11 ± 0.33 d) than the patients in the drainage group (3.38 ± 0.90 d) (P<0.001). The amount of cervical effusion in patients in the non-drainage group (postoperative 24h: 2.20 ± 1.24 ml/48 h: 1.53 ± 1.07 ml) was significantly less than that in the drainage group (postoperative 24 hours: 22.58 ± 5.81 ml/48 h: 36.15 ± 7.61 ml) (all P<0.001). The proportion of incision exudation and incision numbness in the non-drainage group was lower than that in the drainage group (all P<0.05), and the pain score (VAS) and neck foreign body sensation score (FBST) decreased significantly (P<0.05). During the 3- and 6-month follow-up exams, significant differences were not observed between the THYCA-QoL and drainage groups and the non-drainage group, although the scarring and POSAS values were lower than those in the drainage group. In addition, the length of stay and cost of hospitalization in the non-drainage group were lower than those in the drainage group (P<0.05). Conclusion: Routine drainage tube insertion is not needed in patients with unilateral thyroid lobectomy and central neck dissection.

Abstract:Reported here is the thermotropic behavior of cationic gemini/phospholipid/DNA complexes investigated by synchrotron X-ray diffraction. At low temperature, DNA forms rectangular columnar superlattices between interdigitated bilayers. At temperatures above the lipid main phase transition, the transbilayer DNA ordering is partly preserved and DNA arranged into a simple rectangular lattice. This is the first time that such a predicted superstructure is observed in the liquid-crystalline phase of cationic lipid/DNA complexes.

Abstract:Objective: To determine the need for suction drainage after elective thyroid and parathyroid surgery. Design: Randomised controlled trial. Setting: University teaching hospital. Participants: Patients requiring elective thyroid or parathyroid surgery were recruited and informed consent was obtained (n = 100). Before wound closure, patients were randomised into either group A (to remain without suction drainage) or group B (to receive suction drainage). Excluded patients were those requiring associated neck dissection and those with bleeding diatheses, all of whom would necessarily require drainage in our unit. Main outcome measures: Primary - ultrasound evaluation of any collection in the thyroid bed, performed 1-day postoperatively. Secondary - postoperative complications; length of in-patient stay. Results: One hundred patients completed the study, and groups A and B comprised 50 patients each. Patients in each group exhibited a mean age of 49 years, and a male to female ratio of 1 : 9. Both groups were also well-matched regarding type of operation, size of tumour and histopathological diagnosis. Modal and median postoperative neck collection volume on ultrasound examination was 0 and 0 cm(3) respectively (range 0-16 cm(3)) in group A and was 0 and 0 cm(3) (range 0-70 cm(3)) in group B. This difference was not statistically significant, but three patients with a haematoma were all in the suction drainage group. Difference in complication rates between groups was also not statistically significant. Modal and median length of in-patient stay was 2 and 2 days respectively (range 2-3 days) in group A and 3 and 3 days (range 2-4 days) in group B, and this difference was statistically significant (P = 0.0006). Conclusion: Routine suction drainage after uncomplicated elective thyroid and parathyroid surgery appears unnecessary, and prolongs in-patient stay.

Abstract:Objective: To assess the postoperative complications in patients who underwent elective thyroid or parathyroid surgery without postoperative drainage. Design: During a 6-year period all patients who met study criteria were prospectively evaluated. Setting: General community and tertiary referral center. Patients: Fifty-seven patients undergoing thyroid surgery and eight patients undergoing parathyroid surgery were evaluated. Twenty-four patients were excluded because drains were placed postoperatively. Reasons for exclusion included presence of a large dead space, substernal goiter, extensive neck dissection for malignant neoplasm, and large goiters. Results: Major complications consisted of a hematoma requiring reexploration in one patient, and a recurrent nerve palsy in one patient. Minor complications consisted of temporary hypocalcemia (three), seroma (one), and superior flap edema that resolved in 3 months (20). Conclusion: Routine prophylactic drainage in a select patient population is unnecessary after uncomplicated thyroid or parathyroid surgery.
