Abstract:Objective: Fenestrated screw fixation with bone cement augmentation has been demonstrated to increase the pullout strength. Bone cement augmentation is performed to prevent screw failure. Although bone cement screw fixation and fenestrated screw fixation have been compared in many studies, there has been no study on the clinical effects and complication in long level percutaneous bone cement screw fixation. The study aimed to investigate the safety and efficacy of multilevel percutaneous fenestrated screw fixation with bone cement augmentation in the adult lumbar spinal deformity.Methods: We performed a retrospective study of 15 patients who underwent multilevel percutaneous fenestrated screw fixation (PFSF) with bone cement augmentation in a single spine surgeon between January 2018 and December 2020. The incidence of screw failure, body mass index (BMI), bone mineral density (BMD), past history, Visual analogue scale (VAS) score, Oswestry disability index (ODI), sagittal vertical axis (SVA), pelvic Tilt (PT), pelvic incidence (PI), sagittal slope (SS), lumbar lordosis (LL) and Cobbs angle were investigated in the patients.Results: All patients underwent percutaneous screw fixation using a bone cement additive. Mean BMD was –2.0±0.8. The mean percutaneous fenestrated screw fixation level was 6. The mean VAS score changed from 7.14 preoperatively to 4.57 postoperatively (1 month after the surgery), to 3.71 at the last follow-up. The mean ODI changed from 45.21 preoperatively to 32.5 postoperatively, to 27.0 at the last follow-up. The mean LL changed from 23.6 preoperatively to 32.96 postoperatively, to 31.67 at the last follow-up. The mean PT changed from 32.67 preoperatively to 28.11 postoperatively, to 28.70 at the last follow-up. The mean PI - LL changed from 35.20 preoperatively to 22.56 postoperatively, to 22.76 at the last follow-up. The mean Cobb’s angle decreased from 18.30 preoperatively to 6.27 postoperatively, to 5.98 at the last follow-up. The mean SVA changed from 76.65 preoperatively to 46.15 postoperatively, to 48.46 at the last follow-up. The bony fusion rate was 73.3%. There were screw loosening in 4 patients, rod fracture in 1 patient and screw fracture in 3 patients. Among the 3 patients with screw fracture, 1 patient required reoperation. Cement leakage occurred towards the anterior body of the vertebrae in 2 patients but no symptoms were observed.Conclusion: Although the current study examined a small sample with relatively short term follow up periods, our study results demonstrate that multilevel PFSF with bone cement augmentation can result in good clinical and radiological outcomes for lumbar spinal deformity. However, larger size screws or smaller through-hole screws are required to prevent screw fracture.

Abstract:Background: The prevalence of osteoporosis is escalating alongside an aging global population, increasing the demand for spinal surgeries, including those necessitating cement augmentation for enhanced construct stability. Objective: This article delves into the nuanced application of cement augmentation techniques for pedicle screws and vertebral body replacements (VBR), aimed at optimizing surgical outcomes in osteoporotic spines. Method: Drawing from a comprehensive literature review according to important clinical and biomechanical studies and the authors' clinical experiences, we elucidate strategies to mitigate complications and improve surgical efficacy. Results: Cement augmentation has shown promise in managing vertebral fractures and in securing pedicle screws within osteoporotic vertebrae, with the advent of polymethylmethacrylate (PMMA) bone cement marking a pivotal advancement in spinal surgery. We highlight intraoperative measures like the choice between pre-injecting cement and utilizing cannulated or fenestrated screws, emphasizing the importance of controlling cement viscosity to prevent leakage and embolism. Through two case reports, we demonstrate the practical application of endplate cementation following VBR. Conclusion: While the use of cement augmentation poses certain risks, its judicious application-supported by evidence-based guidelines and surgical expertise-can substantially enhance the stability of spinal constructs in osteoporotic patients. This allows a reduction in instrumentation length by enhancing biomechanical stability concerning pullout, bending, and rotational forces. Furthermore, the incidence of endplate sintering following VBF can be significantly reduced. Future research, particularly on antibiotic-loaded PMMA, may further expand its utility and optimize its safety profile.
Dipteran standing stock biomass and effects of aquatic bird predation at a constructed wetland

Abstract:We studied the relationship between benthic invertebrates and aquatic birds at a newly constructed wetland using an avian exclosure experiment combined with counts of aquatic bird use. We measured the standing stock biomass of benthic dipterans both inside and outside exlosures. Chironomidae was the most abundant dipteran family present. Maximum chironomid standing stock biomass per pond ranged from 3.62 to 27.82 g/m2 and was comparable to that found in a number of natural systems. We monitored the abundance of ten aquatic birds species. Abundances of three aquatic bird species [American avocet (Recurvirostra americana), Wilson’s phalarope (Phalaropus tricolor), and cinnamon teal (Anas cyanoptera)] were significantly correlated with chironomid and total dipteran densities. Our experiment, however, found no significant effect of predation on invertebrate densities.
Effect and potential risks of using multilevel cement-augmented pedicle screw fixation in osteoporotic spine with lumbar degenerative disease

Abstract:Abstract Background: The increase of augmented level and bone cement dose are accompanied by the rising incidence of cement leakage (CL) of cement-augmented pedicle screw instrumentation (CAPSI). But the effect and potential risks of the application of CAPSI to osteoporotic lumbar degenerative disease (LDD) have not been studied in the case of multilevel fixation. This study aimed to investigate the effectiveness and potential complications of using multilevel CAPSI for patients with osteoporotic LDD. Methods: A total of 93 patients with multilevel LDD were divided into the CAPSI group (46 subjects) and the conventional pedicle screw (CPS) group (47 subjects), including 75 cases for three levels and 18 cases for four levels. Relevant data were compared between two groups, including baseline data, clinical results, and complications. Results: In the CAPSI group, a total of 336 augmented screws was placed bilaterally. The CL was observed in 116 screws (34.52%). Three cemented screws (0.89%) were found loosened during the follow-up and the overall fusion rate was 93.47%. For perioperative complications, two patients (4.35%) experienced pulmonary cement embolism (PCE), one patient augmented vertebral fracture, and three patients (6.52%) wound infection. And in the CPS group, thirty-three screws (8.46%) suffered loosening in cranial and caudal vertebra with a fusion rate of 91.49%. The operation time and hospital stay of CAPSI group were longer than the CPS group, but CAPSI group has a lower screw loosening percentage ( P <0. 05). And in terms of blood loss, perioperative complications, fusion rate, and VAS and ODI scores at the follow-up times, there were no significant differences between the two groups. Conclusions: Patients with osteoporotic LDD underwent multilevel CPS fixation have a higher rate of screw loosening in the cranial and caudal vertebra. The application of cemented pedicle screws for multilevel LDD can achieve better stability and less screw loosening, but it also accompanied by longer operating time, higher incidence of CL, PCE and wound infections. Selective cement augmentation of cranial and caudal pedicle screws may be a worthy strategy to decrease the complications.

Abstract:STUDY DESIGN:This is a broad, narrative review of the literature.OBJECTIVE:In this review, we describe recent biomechanics studies on cement-augmented pedicle screws for osteoporotic spines to determine which factors influence the effect of cement augmentation.METHODS:A search of Medline was performed, combining the search terms "pedicle screw" and ("augmentation" OR "cement"). Articles published in the past 5 years dealing with biomechanical testing were included.RESULTS:Several factors have been identified to impact the effect of cement augmentation in osteoporotic spines. These include the type of augmentation material, the volume of injected cement, the timing of augmentation, the severity of osteoporosis, the design of the pedicle screw, and the specific augmenting technique, among others.CONCLUSIONS:This review elaborates the biomechanics of cement-augmented pedicle screws, determines which factors influence the augmentation effect, and identifies the risk factors of cement leakage in osteoporotic bone, which might offer some guidance when using this technique in clinical practice. Further, we provide information about newly designed screws and recently developed augmentation materials that provide higher screw stability as well as fewer cement-related complications.

Abstract:Osteoporosis is a common metabolic bone disease in the elderly.It may be one of the factors which contributes to instrumentation failure in spinal surgery.Bone cement augmentation of the pedicle screw is a technique to enhance fixation strength,and it is now mainly applied in patients with osteoporosis and revision surgery,as well as spinal tumor surgery.According to clinical studies,bone cement augmentation can decrease screw loosening rate,prevent correction loss of the spine deformity,and increase the fusion rate.Pedicle screw fracture and pulloutare rare following screw augmentation surgery.The pedicle screw loosening rates following screw augmentation surgery with bone cement are higher in patients with osteoporotic vertebral fracture and lower in patients with spinal degenerative diseases.The main material for bone cement augmentation is Polymethylmethacrylate,and the volume for each screw is about 1-3 ml.Some researchers suggest less volume use in thoracic vertebrae than lumbar vertebrae.Both traditional and cannulated screws can be applied as the augmented pedicle screws,as well as the expandable pedicle screw.Cannulated screws can decrease the risk of bone cement extravasation compared with traditional screws.The mini-mally invasive technique of screw augmentation with bone cement can be used both in primary spinal surgery and augmentation of loosed screws.The main complications of the bone cement augmentation technique are cement leakage and pulmonary cement em-bolism.X-ray fluoroscopy is generally used to detect cement extravasation during the bone cement procedure.The 3D navigation and neurological monitoring are also recommended to reduce cement leakage in some studies.Most local cement leakage events are asymptomatic and need no treatment.Management of pulmonary cement embolism depends on the clinical presentation and the position of the emboli,including regular follow-up,oxygen inhalation,anti-coagulation therapy and embolectomy.
A Biomechanical Study on Cortical Bone Trajectory Screw Fixation Augmented with Cement in Osteoporotic Spines.

Abstract:Study Design A biomechanical study. Objective To evaluate the efficacy and feasibility of cement-augmented cortical bone trajectory (CBT) screw fixation. Methods Forty-nine CBT screws were inserted into lumbar vertebrae guided by three-dimensionally printed templates, and then injected with 0, .5, or 1.0 mL of polymethylmethacrylate. The screw placement accuracy, cement dispersion, and cement leakage rate were evaluated radiologically. Biomechanical tests were performed to measure the axial pull-out strength and torque value. Results Overall, 83.67% of the screws were inserted without pedicle perforation. In the 1.0 mL group, cement dispersed into the pedicle zone and formed a concentrated mass more often than in the .5 mL group, but not significantly more often (P > .05). The total cement leakage rate was 18.75%. Compared with the control group, the torque value was slightly higher in the .5 mL group (P = .735) and significantly higher in the 1.0 mL group (P = .026). However, there was no significant difference between the .5 and 1.0 mL groups (P = .431). The maximal pull-out force (Fmax) was increased by 52.85% and 72.73% in the .5 and 1.0 mL groups, respectively, compared with the control group (P < .05). However, the difference was not significant between the 2 cemented groups (P = .985). Conclusions Cement augmentation is a useful method for increasing CBT screw stability in osteoporotic spines. The cement injection volume is recommended to be 1 mL for each screw, and the cement should disperse into the vertebral body than the pedicle zones.
Effects of pedicle screw cement augmentation on spine tumor surgery

Abstract:Objective To evaluate the effectiveness and safety of cement-augmented pedicle screw ifxation on spine tumor with osteoporotic adjacent vertebrae.Methods The medical records of 35 patients ( 22 males, 13 females ) with spine tumors combined with adjacent vertebral osteoporosis were retrospectively analyzed. All underwent operation and were reconstructed by cement-augmented pedicle screw fixation from December 2012 to November 2014. The mean age was ( 58.1 ± 8.5 ) years ( range: 40 - 76 years ). Characteristics of cement distribution, and the location of cement in the vertebrae and with screws were investigated.Results The mean follow-up time was ( 28.3 ± 4.6 ) months ( range: 14 - 37 months ). Scores of American spinal cord injury association ( ASIA ) and visual analogue scale ( VAS ) were all improved signiifcantly at the last follow-up. The mean correction rate of kyphosis and scoliosis were 54.8% and 71% respectively with the average loss of angle 0.6° and 0.3° respectively. Preoperative and postoperative coronal Cobb’s angles were ( 6.0 ± 6.4 ) ° and ( 1.5 ± 2.3 ) °, and preoperative and postoperative sagittal Cobb’s angles were ( 14.1 ± 12.8 ) ° and ( 11.2 ± 8.1 ) °, with statistical signiifcance. Coronal and sagittal Cobb’s angles showed no signiifcant changes 3 months postoperatively and in the latest follow-up. No operative complications, non-union or instrumental failure were detected during the follow-up. 73.4% of cement distributed in the anterior column and the rest 26.6% in the anterior column and the anterior part of the medial column. 95% conifdential interval (CI )of the ratio between the diameter of cement distribution and screw: ( 1.80 - 3.14 ); the depth of cement distribution and screw: ( 64.6% - 92.2% ); the volume of screw and cement distribution: ( 10.1% - 40.6% ); the volume of cement distribution and vertebrae: ( 2.0% - 20.2% ); the depth of cement distribution and the diameter of vertebrae: ( 43.3% - 66.1% ).Conclusions Cement-augmented pedicle screw fixation is an effective and save way to reconstruct the stability of spine after the surgery of spine tumor with osteoporotic adjacent vertebrae. This method can facilitate the relief of pain, recover of neural functions and remain the spinal stability. The key point to ensure the success lies in the appropriate cement distribution around the screw and in the vertebrae.
Comparison of sensitivity and specificity of serum poly-C avid ribonuclease activity and C-reactive protein concentration in detection of mild and severe acute pancreatitis

Abstract:Abstract The aim of this study was to compare diagnostic performance of C-reactive protein (CRP) and poly-C avid ribonuclease (P-RNase) levels in the prediction of a severe clinical course of acute pancreatitis (AP). The study included 36 patients with mild and 20 with severe AP. CRP concentration was measured by an immunonephelometric method and P-RNase activity by the rate of polycytidylate hydrolysis at pH 7.8. At the time of admission, both P-RNase and CRP levels were significantly increased in all patients when compared to healthy subjects (29.2 vs. 18.7 U/l and 91.1 vs. 2.89 mg/l; p<0.001). Up to days 3 and 4 a further increase in P-RNase was observed. On the other hand, the increase in CRP continued only through days 2 and 3 (p<0.001). Severe acute pancreatitis (SAP) and mild acute pancreatitis (MAP) differed significantly with respect to P-RNase levels on all days studied; whereas CRP levels differed significantly on days 2–5 but did not differ at admission. Receiver operating characteristic (ROC) curve function analysis yielded the best sensitivity of SAP detection for P-RNase, equaling 72.2%, at the cut-off point value 65.3 U/l on day 3 after admission. The sensitivity of CRP for detection of SAP was 85.0% at 125.7 mg/l on the 2nd day after admission. Both parameters studied were significantly associated with the severity of the AP clinical course; however, on days 1 and 2 postadmission, P-RNase was more specific for detection of SAP than CRP (94.4% vs. 77.1% on the 1st day and 94.4% vs. 55.5% on the 2nd day). In conclusion, P-RNase has shown an excellent performance for early differentiation of acute necrotizing pancreatitis.

Abstract:Bone cement has been used in spinal surgery for as long as 50 years. In contemporary spinal surgery, cement augmentation of fractured osteoporotic vertebrae in the form of vertebroplasty/kyphoplasty as well as cement augmentation of pedicle screws in instrumented procedures of any etiology are established as standard procedures. Both procedures are very effective, although the benefits of vertebroplasty/kyphoplasty procedures have been controversially discussed in the past. Overall, complications rarely occur. The most relevant complication is cement leakage, which is asymptomatic in the majority of cases but in the worst case might lead to neurological deficits, embolic events and even circulatory collapse. Prevention of cement leakage is therefore crucial. Risk factors for cement leakage and preventive measures are presented in a comprehensive review based on the available literature.
Different pedicle osteosynthesis for thoracolumbar vertebral fractures in elderly patients

Abstract:IntroductionPedicle screws’ stability, especially in osteoporotic fractures, is a really problem for spinal surgeons. Nowadays, little is known about the influence of different screw types and amount of cement applied. This single-center retrospective observational study has the aim of evaluating the middle- to long-term mechanical performances of different types of screws in elderly patients with thoracolumbar fractures.Materials and methodsA total of 91 patients (37 males and 54 females), treated between 2011 and 2016, affected by somatic osteoporotic fractures aged over 65 years were treated. We divided patients into three different populations: solid screws, cannulated screws and cannulated screws augmented with poly methyl methacrylate cement (PMMA). Patients were radiologically evaluated with X-rays in pre- and post-surgery and at the follow-up (FU). Clinical evaluations were made with VAS and Oswestry Disability Index.ResultsA total of 636 screws were implanted (222 pedicle screws, 190 cannulated and 224 cannulated screws with PMMA augmentation). At FU, we found significative differences between populations in terms of mechanical performances. We founded five cases of loosening; these were reported in solid screws group and in cannulated screws one. No mechanical failures were reported in cannulated screws with augmentation of PMMA. No rods breakage cases were reported.ConclusionAll stabilization methods showed good clinical results, but cannulated screws augmented with PMMA seem to provide better implant stability with the lowest rate of loosening.Graphical abstractThese slides can be retrieved under Electronic Supplementary Material.

Abstract:OBJECTIVE:To describe the application of polymethylmethacrylate (PMMA) augmentation of cement-injectable cannulated pedicle screws for the treatment of degenerative lumbar scoliosis with osteoporosis.METHODS:Retrospective cohort study was used to compare cement injectable cannulated pedicle screws (CICPs) group with PMMA augmentation and control group with traditional method in the correction surgery for Lenke-silva level III and level IV degenerative scoliosis cases with osteoporosis. Both groups were followed up for 1 year. The clinical results were assessed by visual analog scale (VAS) of pain on lumbar and lower limbers, Oswestry disability index (ODI) score and EuroQol-5 dimensions (EQ-5D) score. The coronal major curve Cobb angel in coronal plane and thoracic kyphosis Cobb angle, lumbar lordosis Cobb angle and sagittal vertical axis (SVA) in sagittal plane were tested in whole long spine X ray. The fusion rates were evaluated by lumbar X ray and dynamic X ray.RESULTS:In this study 34 cases were enrolled, 15 cases in CICPs group and 19 cases in control group. The general characteristics including age, gender, weight, height, BMI and BMD were without statistical difference between the two groups. There were (5.7±2.2)PMMA augmentation screws in CICPs group. The operation time, blood loss and blood transfusion were higher in CICPs group than in control group, but without statistical difference. Lumbar VAS, lower limbers VAS, ODI score and EQ-5D were all better in 1 month postoperation, 6 months postoperation and 1 year postoperation than in preoperation in both groups. lumbar VAS scores of CICPs group in 6 months postoperation (CICPs group 3.1±1.3 vs. control group 4.4±1.4, P<0.01) together with lumbar VAS scores (CICPs group 3.3±1.0 vs. control group 5.2±1.4, P<0.01), ODI scores (CICPs group 22.7±17.2 vs. control group 31.4±18.5, P<0.01) and EQ-5D in 1 year postoperation (CICPs group 2.9±2.0 vs. control group 3.5±2.5, P<0.01)were lower than those of control group. The coronal major curve Cobb angels were all lower in 1 month postoperation, 6 months postoperation and 1 year postoperation than in preoperation in both groups; thoracic kyphosis Cobb angle and lumbar lordosis Cobb angle were all higher in 1 month postoperation, 6 months postoperation and 1 year postoperation than in preoperation in both groups. The coronal major curve Cobb angel was lower in CICPs group than that in control group in 1 year postoperation (CICPs group 17.6°±6.9° vs. control group 21.2°±7.2°, P<0.01)and thoracic kyphosis Cobb angle was higher in CICPs group than that in control group in 6 months postoperation (CICPs group -33.5°±8.8 ° vs. control group -28.9°±8.3°, P<0.01) and 1 year postoperation (CICPs group -33.0°±8.1° vs. control group -26.3°±7.4°, P<0.01) together with lumbar lordosis Cobb angle were higher in CICPs group than that in control group in 1 year postoperation (CICPs group 26.4°±8.1° vs. control group 22.1°±7.3°, P<0.01).CONCLUSION:Polymethylmethacrylate augmentation of bone cement-injectable cannulated pedicle screws for the treatment of degenerative lumbar scoliosis with osteoporosis was effective and safe, the short-term clinical result was good.

Abstract:Objective To compare the effects of different types of bone cement augmentation of pedicle screws on the prognosis of osteo -porotic thoracolumbar fractures in elderly patients.Methods From August 2013 to November 2016,184 elderly patients with osteoporotic thoracolumbar fractures in our hospital were selected as the research object,all the patients were randomly divided into observation group and control group,94 cases in each group.Two groups were treated with pedicle screw fixation,the control group received conventional pedicle screw combined with PMMA bone cement augmentation,and the observation group were treated by cannulated pedicle screws combined with PMMA bone cement augmentation,the prognosis of the two groups were recorded.Results All patients were successfully completed the oper-ation without serious complications occurred.There was no significant difference in operation time,intraoperative blood loss,the amount of sin-gle vertebral bone cement,hospitalization time between the two groups(P>0.05).The postoperative 1 month incidence rate of cerebrospinal fluid leakage,bone cement leakage,wound infection,loosening of internal fixation was 4.3% in the observation group,which was lower than 21.3%in the control group,the difference was significant(P<0.05).The postoperative 1 month of vertebral height in the observation group and the control group were respectively(4.53 ±1.11)mm and(5.09 ±1.42)mm that were lower than the preoperative(6.41 ±1.39)mm and(6.38 ±1.31)mm(P<0.05),meanwhile the vertebral height at postoperative 1 month of the observation group was lower than that of the control group,the differences were significant(P<0.05).There were 0 case of Frankle grade A,0 case of grade B,10 cases of grade C, 18 cases of grade D,66 cases of grade E in the observation group at postoperative 1 month,and there were 6 cases,14 cases,20 cases,20 ca-ses and 34 cases of corresponding Frankle grade in the control group,the observation group was significantly better than the control group,the differences were significant(P<0.05).Conclusion The application of cannulated pedicle screw combined with PMMA bone cement in the osteoporotic thoracolumbar fractures does not increase the difficulty of operation,and it can reduce the complications and reduce vertebral height,which is conducive to the recovery of nerve function.

Abstract:Objective:To investigate the clinical effect and safety of applying bone cement to pedicle screw fixation in osteoporotic patients.Method:A series of 20 patients,in whom the pedicle screw fixation was performed with methylmethacrylate(PMMA) injected into the tract,were followed up.By comparing their X-ray expressions at the time of one week after the operation with that at the time of several months after the operation when the patients came back for a further consultation,whether the screws were mobile should be judged.Furthermore,the involved spinal segments were scaned by CT to observe if any bright lines exist around the screws.Result:18 patients(90%) were followed up,the mean follow-up period was 18 months.There was no screw mobility observed in the X-ray expression of all the cases.And there was no bright line found in the CT expression.Conclusion:To applying PMMA bone cement to pedicle screw fixation in osteoporotic patients can immobilize pedicle screw and stabilize vertebrae,and,as a result,gain a satisfactory clinical effect when the operation is carefully done without the leakage of bone cement.
Establishment and validation of nomograms and web calculators for different cement leakage risk types in pedicle screw augmentation for degenerative lumbar stenosis in osteoporotic vertebrae

Abstract:Background The use of cement in pedicle screw augmentation (PSA) enhances the pullout force of pedicle screws in vertebrae affected by osteoporosis. Risks involved in the use of cement for PSA include nerve injury and vascular damage caused by cement leakage. Methods Our study included all individuals who received PSA for degenerative lumbar stenosis (DLS) in osteoporotic vertebrae from January 2014 to May 2022. Postoperative CT scan will be used to assess cement leakage. Correlation analysis and logistic regression analyses were utilized to establish the associated clinical or radiological factors, which were then used to construct Nomograms and web calculators. Results Our study included a total of 181 patients, with 886 screws being inserted into 443 vertebrae. Perivertebral cement leakage was significantly associated with female, decreased BMD, solid screws, and scattered cement distribution. The majority of cement leakage happened through segmental veins (type S, 72.1%), with basivertebral veins (type B, 23.9%) and instrument-related leakage (type I, 13.9%) following suit. Lower BMD and scattered cement distribution were more likely to experience type S or type B leakage. Our analysis data showed that cement augmentation with cannulated and fenestrated screws tended toward concentrated cement distribution. The creation and verification of each Nomogram additionally showcased the prognostic capability and medical significance of the corresponding model. Conclusion Nomograms and web-based calculators can accurately forecast the probability of cement leakage. We should regularly perform the procedure using cannulated and fenestrated screws, along with a moderate amount of high-viscosity cement, while continuously monitoring with fluoroscopy during PSA.

Abstract:Objective The prevalence of osteoporotic vertebral fractures has increased with aging populations, necessitating effective treatments such as percutaneous kyphoplasty combined with posterior screw fixation. However, biomechanical research on the effects of using short screws on fixation stability and bone stress or on the impact of bone cement bonding to screws on structural strength is lacking. This study aimed to optimize short-segment fixation strategies for osteoporotic thoracolumbar burst fractures by analyzing the biomechanical effects of pedicle screw length and bone-cement augmentation. Methods Four models of the thoracolumbar spine were established using computed tomography data of a female volunteer: 1) short screws in the injured vertebra without contact with the bone cement, 2) long screws without contact with the bone cement, 3) long screws in contact with the bone cement, and 4) long screws without the bone cement. The 4 fixation models were simulated under physiological loads. The range of motion, implant stress, and segmental stability were assessed. Results The 3 groups containing the bone cement exhibited similar performances in terms of stability and stress distribution, whereas the group without the bone cement exhibited a poorer biomechanical performance. Incorporation of the bone cement enhanced the biomechanical properties of the structure, and short screws in the injured vertebra without contact with the bone cement did not significantly compromise the biomechanical performance. Conclusions Short screws in injured vertebrae without contact with the bone cement can achieve satisfactory stability and stress distribution. It is feasible to implant short screws in the injured vertebrae, reduce the number of bilaterally injured vertebrae, and inject bone cement through the non-pedicle approach during the surgical procedure, which simplifies the surgical process.

Abstract:Study design: Prospective case study. Purpose: Osteoporotic spine fixation by pedicle screw instrumentation is complicated by screw loosening, migration, or pullout with rates of up to 62% documented in the literature. Contemporary solutions have not adequately addressed these complications. We propose a modified surgical technique of cement augmentation with bicortical pedicle screw fixation to address the issue related to implant failure in osteoporotic spine. Overview of literature: Zindrick and his colleagues described a "windshield wiper" effect owing to the shift of center of rotation to the distal tip of the screw in the bicortical purchase of screws. An increase in pullout strength from 119% to 250% with polymethyl methacrylate augmentation has been documented in the literature. This technique has not been described in the literature. Methods: A prospective study was conducted with 40 patients who underwent surgery by the modified technique. Intraoperative and postoperative complications directly related to the procedure were assessed. Improvement in pain and functional status were assessed. Follow-up radiographs were assessed to check for appreciable screw migration, loosening, or pullout. Results: This technique was used in inserting 364 screws in 40 patients. We did not encounter any difficulty in inserting the screws. A total of 19 screws failed to breach the anterior cortex owing to an error in measurement. There were no complications during the procedure in any of the patients, and the postoperative period was uneventful. The mean follow-up period was 18 months. There were two patients in whom proximal junctional failure with kyphosis was noted during follow-up, who were surgically managed by extension of the fixation levels. Conclusions: Bicortical fixation with cement augmentation is a technically feasible, safe, and effective technique to augment the strength of pedicle screws in osteoporotic spine fixation. It has the potential to be established as a standard of care in osteoporotic spine fixation.

Abstract:Achieving sufficient mechanical purchase of pedicle screws in osteoporotic or previously instrumented bone is technically and biologically challenging. Techniques using different kinds of pedicle screws or methods of cement augmentation have been used to address this challenge, but are associated with difficult revisions and complications. The purpose of this biomechanical trial was to investigate the use of biocompatible textile materials in combination with bone cement to augment pullout strength of pedicle screws while reducing the risk of cement extrusion. Pedicle screws (6/40 mm) were either augmented with standard bone-cement (Palacos LV + G) in one group (BC, n = 13) or with bone-cement enforced by Vicryl mesh in another group (BCVM, n = 13) in osteoporosis-like saw bone blocks. Pullout testing was subsequently performed. In a second experimental phase, similar experiments were performed using human cadaveric lumbar vertebrae (n = 10). In osteoporosis-like saw bone blocks, a mean screw pullout force of 350 N (±125) was significantly higher with the Bone cement (BC) compared to bone-cement enforced by Vicryl mesh (BCVM) technique with 240 N (±64) (p = 0.030). In human cadaveric lumbar vertebrae the mean screw pullout force was 784 ± 366 N with BC and not statistically different to BCVM with 757 ± 303 N (p = 0.836). Importantly, cement extrusion was only observed in the BC group (40%) and never with the BCVM technique. In vitro textile reinforcement of bone cement for pedicle screw augmentation successfully reduced cement extrusion compared to conventionally delivered bone cement. The mechanical strength of textile delivered cement constructs was more reproducible than standard cementing. © 2017 Orthopaedic Research Society. Published by Wiley Periodicals, Inc. Pedicle Screw Fixation in Single-Level, Double-Level, or Multilevel Posterior Lumbar Fusion for Osteoporotic Spine: A Retrospective Study with a Minimum 2-Year Follow-Up

Abstract:BACKGROUND: Pedicle screw loosening is a common postoperative complication for osteoporotic patients, and several studies have identified the important role of fusion length in internal fixation failure, but the relationship between the number of fusion segments and the potential risks remains unclear. This study aimed to investigate the rate and risk factors of screw loosening in osteoporotic patients with different levels of degenerative lumbar disease.METHODS: The total cohort of 217 patients was divided into 3 groups according to the different fusion levels: single-level (group A; 100 cases), double-level (group B; 73 cases), and multilevel group (group C; 44 cases). Patient baseline demographic characteristics and assessments with a visual analog scale (VAS) and the Oswestry Disability Index (ODI) before operation and at the last follow-up were compared among the 3 groups.RESULTS: Compared with preoperative values, VAS and ODI scores at the last follow-up were significantly improved in all 3 groups. Operative time, blood loss, length of hospital stay, screw loosening rate, fusion rate, and VAS and ODI scores at the last follow-up obviously increased with the increasing number of fusion segments (group C> group B> group A). Of note, all the screw loosening was observed in cranial and caudal vertebra. Furthermore, multivariate logistic regression analysis identified lumbosacral fixation, larger pelvic incidence (PI)-lumbar lordosis (LL) difference (PT-LL), and greater postoperative pelvic tilt (PT) as independent predictors of screw loosening. However, sex, bone mineral density, body mass index, LL, sacral slope, PI, the change in LL, and preoperative PT were not relevant to screw loosening (P > 0.05).CONCLUSIONS: Owing to the high rate of screw loosening in cranial and caudal vertebra, osteoporotic patients with double-level or multilevel pedicle screw fixation benefited less than those with single-level pedicle screw fixation. Larger PI-LL, larger PT, and lumbosacral fixation are other risk factors for screw loosening. An instrument with stronger holding strength at cranial and caudal pedicle screws is recommended for those high-risk patients.
