P153. PI Changes in patients with adult spinal deformity following S2AI fixation: is the hip joint involved?

Abstract:BACKGROUND CONTEXT
Pelvic Incidence (PI) is a critical radiographic measurement in optimizing the formulation of surgical strategies for adult spinal deformity (ASD) patients. However, recent studies challenge established theories on surgical strategies formulation by highlighting PI as a variable parameter, leaving the causes of PI changes unclear. Serving as a hinge between the spine and lower extremity, the hip joint exhibits a strong and variable interaction with the sacroiliac joint, compensating for spinal sagittal imbalance, yet its role in PI changes is poorly understood.
PURPOSE
Our study aims to investigate the impact of the relative position of the femoral head and acetabulum on postoperative PI changes.
STUDY DESIGN/SETTING
A retrospective study involving 121 ASD patients following S2AI fixation was conducted to compare the relationship between changes in their hip parameters and alterations in pelvic incidence (PI).
PATIENT SAMPLE
This retrospective case series study was approved by the institutional review board of Nanjing Drum Tower Hospital. Additionally, informed consent was obtained from all patients enrolled in this study. The ASD patients following S2AI fixation between January 2015 and May 2021 were retrospectively reviewed. Inclusion criteria included: Patients (1) diagnosed with ASD who underwent (2) bilateral placement of S2AI screws and (3) were followed up for at least 2 years with (4) complete radiographic data. Exclusion criteria included: (1) History of the previous spine or hip surgeries; (2) spinal deformity secondary to tumors or infections; (3) radiographic evidence of hip dysplasia.
OUTCOME MEASURES
Demographic information such as age, sex, fusion segments, and surgical history were collected. Standing posteroanterior and lateral radiographs of the spine were obtained preoperatively and postoperatively. To avoid subjecting patients to repetitive X-ray exposure, we included only those patients with preoperative supine CT positioning phases to measure supine hip parameters. All the radiographic parameters were measured twice by two experienced spine surgeons independently, and mean values were used for analysis. The spinal radiographic parameters were measured using a validated software (Surgimap, Nemaris, Inc., New York, NY). And we utilized Hip2Norm (Hip2Norm 2007; Switzerland) to measure the parameters of the hip joints. Ten commonly used radiographic spinal parameters and five hip parameters were evaluated. Changes in PI were then calculated by subtracting the preoperative values from the postoperative values. According to the change in PI, ASD patients were divided into two groups: group C (|PI|≥ 10°) and group NC (|PI|< 10°). We further divided the patients who had collected supine radiographic data into two groups, group C2 (|PI|> 10°) and group NC2 (|PI|< 10°). Given the anticipated variation of approximately 5° that may occur during imaging and measurement, we made the decision to employ a more conservative cutoff value of 10°.
METHODS
We performed a retrospective review of 121 consecutive Chinese Han patients following S2AI fixation at our center from January 2015 to May 2021. All patients had complete radiographic data and were categorized into C (change, |PI ≥ 10°|) and NC (no change, |PI < 10°|) groups. The changes in hip parameters postoperatively were compared between group C and group NC. Additionally, we enrolled patients who had supine CT positioning phase from group C and group NC to compare the differences in hip parameter changes during positional changes. Finally, a correlation analysis was performed between changes in PI and changes in hip parameters.
RESULTS
Compared to group NC, patients in group C showed significantly greater postoperative changes in hip parameters (?FHC: 5.9±5.1 vs 2.0±2.0, p=0.021; LCE angle: 3.1±2.7 vs 1.4±1.4, p=0.042; AI: 4.4±3.0 vs 1.3±1.5, p=0.012; SA: 2.6±1.5 vs 1.3±1.7, p=0.041; EI: 4.1±3.0 vs 2.0±1.4, p=0.001). Furthermore, a significant, negative correlation was found between preoperative position-related FHC (Left: r=-0.317, p=0.021; Right: r=-0.320, p=0.031), EI (Left: r=-0.753, p=0.003; Right: r=-0.420, p=0.021), LCE angle (Left: r=-0.532, p=0.021) and postoperative PI. Based on the ROC curve, the optimal threshold of preoperative position-related FHC, LCE angle, EI were 6.65%, 5.40° and 4.55%, respectively.
CONCLUSIONS
The postoperative PI changes in ASD patients following S2AI fixation are closely associated with the relative position and coverage changes of the femoral head. For patients with significant preoperative position-related changes in hip parameters, it is important to consider the potential postoperative reduction in PI when formulating surgical strategies to avoid overcorrection in the sagittal plane.
