Abstract:What's new? Biliary tract cancer (BTC) is an aggressive malignancy with increasing worldwide incidence. Factors that underlie the recent global increases in BTC incidence remain unclear. In the present study, obesity, dietary, and physical activity factors were investigated in relation to BTC risk in African, East Asian, and European descendants. Body mass index and waist‐to‐hip ratio were positively associated with BTC risk. Meanwhile, physical activity and higher intake of fruit and fish were inversely linked to BTC risk. These findings offer insight into lifestyle factors that may contribute to BTC etiology which could be targets for primary prevention. Biliary tract cancer (BTC) is a rare and aggressive malignancy with increasing incidence. Most BTC cases are diagnosed with metastatic disease which carries a 5‐year survival rate of 35.0 to 18.5–24.9: 1.71 (1.26, 2.31), p‐trend
