Abstract:STUDY OBJECTIVES: The first objective was to compare tongue motor skills between patients with obstructive sleep apnea (OSA) and healthy subjects. Secondly, the effect of oropharyngeal myofunctional therapy (MFT) on the tongue muscular qualities of patients with OSA was evaluated.METHODS: Searches were conducted in five electronic databases until July 2023. Risk of bias was assessed via the Joanna Briggs Institute appraisal checklist for cross-sectional studies (aim #1) and the PEDro scale for randomized controlled trials (aim #2). Pooled standardized mean differences (SMD) and 95% confidence intervals were calculated using a random-effects model.RESULTS: Thirteen studies including 520 adults and one study including 92 children addressed aim #1. Compared to healthy adults, individuals with OSA had no significant difference in tongue protrusion strength (9 studies, n=366, SMD [95%CI]; -0.00 [-0.45, 0.44]) or endurance (5 studies, n=125; 0.31 [-0.26, 0.88]) but presented a lower tongue elevation strength (6 studies, n=243; 1.00 [0.47, 1.53]) and elevation endurance (3 studies, n=98; 0.52 [0.11, 0.94]). In pediatric, tongue elevation strength was lower but elevation endurance was higher in children with OSA than healthy children. Two randomized controlled trials (28 adults, 54 children) addressed aim #2 and were of poor methodological quality. In these studies, MFT improved tongue motor skills in patients with OSA.CONCLUSIONS: Tongue elevation motor skills are decreased in adults with OSA whereas tongue protrusion motor skills seem preserved. Very few data are available in children. Failure of NIMV and operator factor: when making things work, sometimes it is hard

Abstract:Despite its well-known advantages in acute respiratory failure (reduces intubation rate and mortality), non invasive mechanical ventilation (NIMV) have failure rates of 5%–60%, and mortality rates have been found to be higher in patients who are intubated after failure of NIV (1). Predictors for NIMV failure were defined to avoid delay in intubation which can cause clinical deterioration and increase morbidity and mortality (2). We read with great interest the article written by Bhatti et al. (3) entitled ‘Operator dependent factors implicated in failure of non-invasive positive pressure ventilation for respiratory failure’ accepted to Clinical Respiratory Journal. The study evaluated retrospectively 111 patients with NIMV failure and investigated operator related factors that may have contributed to failure of NIMV and clinical outcomes. They found three most common reasons for failure as inappropriate indication (33%), progression of underlying disease (30%) and lack of titration (23%). This study definitely requires attention, since it gives valuable information to clinicians to not only prevent failure but also provide protocoled approach to initiation and maintenance of NIMV. However, we think that there are some issues to be commented on. First, the study includes chart review of patients who were initiated and failed NIMV at different places like emergency room, on medical/surgical floors, progressive care units or intensive care units. This means that different operators with different devices and different level of education and training on NIMV. Schettino et al. evaluated 441 patients with NIMV treatment in their prospective one center study. They found entubation and mortality rate in patients only treated in the emergency room as 22.6%, 7.5%, in the intensive care unit as 49.4%, 28.4% and in the ward as 27.3%, 14.9%, respectively. The authors related these diversions with different type and severity of comorbidities. We think that these situations are also valid for the study of Bhatti et al. it is crucial to report failure rates of these different places defined in the study. Also it is necessary to give information about distribution of defined operator based factors among these places. We believe that there may be differences other than operator based factors like disease severity, type of NIMV devices, equipment, and education level of medical stuff and nurse/patient ratio in these different areas. Second, although it was noted in limitations because of the retrospective nature of the study, it does not include variables such as leak; amount and management of secretions that have been proved to be influential in some studies that have evaluated the success or failure of NIMV (4). In addition data of type of respiratory failure whether hypoxic or hypercapnic was not given. It was reported that NIMV success rate of hypoxemic respiratory failure as 50% while in hypercapnic respiratory failure it is %70 (5). We believe that it would be valuable information to give ratio of operator based factors of NIMV failure among hypoxic and hypercapnic respiratory failure. This study gives valuable and useful information about operator based factor for NIMV failure for patients with respiratory failure. Due to multiple factors that affect NIMV success, having heterogeneous places and patients make study hard to give accurate information about only operator based predictors. We consider large randomized clinical based prospective studies for operator based factors of NIMV failure.

Abstract:Objective: To assess the association between self-reported symptoms of pediatric OSA and clinical signs in MB children. Methods: Seventy-three MB children aged 7-14 years answered an interview questionnaire on OSA symptoms in childhood, focusing on chewing, nasal, and sleep disturbances. MB children were checked for changes on the craniofacial, occlusion, TMJ, upper airway, and body posture by a multi-disciplinary team, consisting of medical residents, dental, and psychology postgraduate students. Multiple logistic regression analysis verified the association between clinical signs and self-reported symptoms. Results: Reported symptoms of chewing disturbance as TMD noise, muscle pain, and morning headache were significantly associated with the presence of lip incompetence and Class II malocclusion. Sleep disturbances as snoring, waking up at night, daytime sleepiness, and sleeping with the mouth open were significantly associated with the presence of hypertrophic tonsils and obstructive Mallampati score. Conclusion: Symptoms of pediatric OSA were found in mouth-breathing children, which should be carefully investigated in order to prevent OSA in the future.

Abstract:BACKGROUND: To evaluate the treatment efficacy of a mandibular advancement intraoral appliance (MOA) for treatment of obstructive sleep apnea syndrome (OSAS) in pediatric patients.MATERIAL AND METHODS: Eighteen patients (mean=8.39 years old, women=44.4%) were selected. Sleep disorders, sleep bruxism, and temporomandibular disorders were assessed by the Sleep Disturbance Scale for Children (SDSC), the BiteStrip® (portable SB device), and the Research Diagnostic Criteria for Temporomandibular Disorders, respectively. The clinical diagnosis of OSAS was confirmed with a type 3 portable monitor device (ApneaLinkTM Plus). A silicon-based material MOA was used by patients for 60 days, and the results were compared to baseline.RESULTS: The median RDI was significantly reduced from 10 to 4.5 events/hour. Nadir SpO2 significantly increased from 82.6% to 88.9%. Total snoring events/hour have also significantly decreased from 205.5 to 91.5. Signs and symptoms of TMD remained unaltered. There was also a reduction from moderate to absence of SB in 12 patients. Similarly, all variables measured by the SDSC have had very significant reductions: disorders of initiating and maintaining sleep, sleep disordered breathing, disorders of arousal, nightmares, sleep wake transition disorders, disorders of excessive somnolence, and sleep hyperhidrosis.CONCLUSIONS: In selected cases, OA maybe considered as an alternative for the OSAS treatment.

Abstract:ABSTRACT Purpose: to review, in an integrative manner, studies using surface electromyography in the orofacial and cervical musculature in mouth breathing children aged from three to 11 years and 11 months old. Methods: the survey was conducted in national and international databases, from 1998 to 2018, in Portuguese, English and Spanish. Review articles, dissertations, book chapters, case studies and editorials were excluded. Results: 86 articles were found, 14 of which met the inclusion criteria. Most of these studies used surface electromyography to assess and describe the muscle condition of the mouth breathing population. Only one study addressed the influence of myofunctional speech therapy and two studies included physical therapy treatment, using electromyographic evaluation before and after the intervention. Given the main categories of analysis, the discussion was based on the year, state of publication and journal, sample size, scientific methodology, muscles assessed, assessment protocols used and the results of the publications. Conclusions: surface electromyography has been used mainly in the initial assessment of orofacial and postural myofunctional changes caused by mouth breathing and not as a therapeutic biofeedback, thus, it is important to conduct longitudinal studies using this instrument in mouth breathers.
Multidisciplinary Treatment in Patients with Craniofacial, Neurocognitive, and Neuromuscular Disorders with Obstructive Sleep Apnea: A Systematic Review of the Literature

Abstract:Obstructive sleep apnea (OSA) is a respiratory disorder that has a high prevalence in patients with craniofacial, neurocognitive, and neuromuscular disorders. Currently, the treatments for this population are diverse and depend on the individual conditions of the patient and the severity of the case. However, there are no multidisciplinary dental treatment guidelines. The aim of the present study was to determine the multidisciplinary dental treatment alternatives in patients with craniofacial, neurocognitive, and neuromuscular disorders with a diagnosis of OSA through evidence-based medicine. A systematic review of the literature has been performed by searching scientific articles in the PubMed, Cochrane, Ovid, ScienceDirect and Scopus databases, through controlled and uncontrolled language. Articles were classified according to the level of evidence and grades of recommendation through the Scottish Intercollegiate Guidelines Network. A total of 19,439 references were identified, of which 15 articles met the predetermined requirements to be included in the investigation. The articles included for this systematic review showed that mandibular distraction osteogenesis and adenotonsilectomy are the first-choice therapies for craniofacial and neurocognitive disorders. However, for neuromuscular disorders, the findings reported were not enough to provide information about surgical or nonsurgical alternatives. Despite the reported high frequency of OSA in those children with craniofacial, neurocognitive, and neuromuscular disorders, the evidence on the surgical and nonsurgical therapeutic success for OSA in these patients is scarce. Abstract:Background: Oral breathing (OB) was considered associated with specific craniofacial structures and same for pediatric obstructive sleep apnea (OSA). This study aimed to investigate the differences of craniofacial structures between OB and OSA. Methods: In this retrospective study, 317 children under age 18 years were recruited and divided into OB group, OSA group, and control group. OSA group (15 boys, 4 girls) were referred from qualified sleep center and diagnosed as pediatric OSA with full-night polysomnography. OB group (10 boys, 10 girls) were mostly referral from pediatric or ENT department, some of whom undertook polysomnography and were not OSA. Control group consisted of orthodontic patients within the same period. Lateral cephalograms were obtained in all groups and their parameters were compared with Chinese normal values and each other. Results: R-PNS of OB group (18.04 +/- 2.49 mm) was greater than OSA group (14.27 +/- 4.36 mm) and even control group (16.22 +/- 3.91 mm) (P < 0.01). U1-NA was also the greatest in OB group (7.15 +/- 2.92 mm), followed by OSA group (4.88 +/- 2.66 mm), while control group was the smallest (5.71 +/- 2.94 mm) (P < 0.05). In addition, OB group presented the smallest adenoids and tonsils among three groups. Bony nasopharynx development, mandibular length and growth direction of mandible of OB group were all better than OSA group. Conclusion: Despite of oral breathing, anatomical morphology (well-developed dentoalveolar structures; mild adenotonsillar hypertrophy) might protect children from developing OSA.
Respiratory outcome of mandibular distraction osteogenesis on obstructive sleep apnea in craniofacial microsomia

Abstract:Purpose Children with craniofacial microsomia (CFM) have a higher risk of airway obstruction. This study aimed to evaluate the effect of mandibular distraction osteogenesis (MDO) on respiratory function in CFM patients with obstructive sleep apnea (OSA) according to polysomnography (PSG). Materials and methods This study retrospectively analyzed patients with CFM who underwent PSG before surgery and after completion of mandible distraction. Patients who met the inclusion criteria were selected. The Pediatric Sleep Questionnaire (PSQ) was used to assess patients' signs and symptoms related to OSA. The obstructive apnea-hypopnea index (OAHI) and lowest oxygen saturation (LSaO 2 ) were imported into SPSS version 26.0. The Wilcoxon signed-rank test was used to assess the differences in PSG before and after MDO. Other data were described using descriptive statistics. A P-value less than 0.05 was considered statistically significant. Results A total of 25 unilateral CFM patients were included in this study. Most patients (72%) had mild OSA; moderate and severe OSA were 12% and 16%, respectively. Snoring (52%) was the most common symptom among these patients. After completion of mandibular distraction, snoring and other OSA-related symptoms were significantly improved. Twelve patients had normalized PSG and the severity of OSA improved significantly in 3 patients. The total effective rate of MDO for OSA was 60%. The statistical results showed that OAHI (P = 0.045) decreased and LSaO 2 (P = 0.009) increased significantly compared to preoperative values. Conclusion MDO can improve OSA-related symptoms in CFM patients. In addition, respiratory function was improved in most patients after MDO, based on PSG. CFM patients, especially those with OSA, can benefit from MDO.

Abstract:MicroRNAs (miRNAs) are short, noncoding RNAs that post-transcriptionally regulate gene expression. While hundreds of mammalian miRNA genes have been identified, little is known about the pathways that regulate the production of active miRNA species. Here we show that a large fraction of miRNA genes are regulated post-transcriptionally. During early mouse development, many miRNA primary transcripts, including the Let-7 family, are present at high levels but are not processed by the enzyme Drosha. An analysis of gene expression in primary tumors indicates that the widespread down-regulation of miRNAs observed in cancer is due to a failure at the Drosha processing step. These data uncover a novel regulatory step in miRNA function and provide a mechanism for miRNA down-regulation in cancer.
Assessment of the gut microbiota of children with obstructive sleep apnea syndrome: A systematic review

Abstract:Sleep-disordered breathing promotes not only unfavorable craniofacial changes in untreated pediatric patients but also neurocognitive, metabolic, cardiovascular, and even long-term social alterations. This systematic review evaluated whether children diagnosed with obstructive sleep apnea syndrome (OSAS) have different intestinal microbiota constitutions from healthy children and was based on the PRISMA guidelines (PROSPERO: CRD42022360074). A total of 1562 clinical studies published between 2019 and 2023 were selected from the PubMed/MEDLINE, Embase, Web of Science, Scopus, and Cochrane Library databases, of which five were included in the qualitative analysis, three being randomized and two prospective. The methodological quality was assessed (RoB 2.0 and ROBINS-I) and all studies showed a negative effect of intervention. Sleep deprivation and intermittent hypoxia in children with OSAS seem to trigger a cascade of inflammatory pathways that exacerbate the tissue response to the release of reactive oxygen species and the generation of oxidative stress, leading to a reduction in oxygen supply to the intestinal mucosa and the integral destruction of the intestinal barrier. More evidence-based investigations are needed to optimize the identification of possible alterations in the gut microbiota of pediatric patients, given that its composition may be influenced by the patient's sleep quality and, consequently, by OSAS, showing quantitative and qualitative alterations compared to that found in healthy individuals.

Abstract:Abstract Introduction Obstructive sleep apnea (OSA) is associated with upper airway soft tissue and craniofacial skeletal alterations, but if these abnormalities, using quantitative facial and intraoral photographic analysis, differ between in the general population and clinical samples were not tested yet. This study aimed to evaluate and compare measurements of standardized craniofacial and intraoral photographs between clinical and general population samples, between groups of individuals with an apnea-hypopnea index (AHI) ≥15 and AHI< 15, and the interaction between them, as well as the relationship with the presence and severity of OSA. Methods We used data from 929 participants from Sleep Apnea Global Interdisciplinary Consortium (SAGIC): 309 patients from a clinical setting and 620 volunteers from a general population sample. Results Moderate and severe OSA (AHI≥15) were observed in 30.3% of the total sample and there were some interactions between facial/intraoral measures with OSA and both samples. Mandibular volume (p< 0.01), sternomental distance (p=0.04), and lateral face height (p=0.04) were higher in the AHI≥15 group in the clinical sample compared to the AHI≥15 group in the general population and AHI< 15 group in the clinical sample. Multivariate regression suggests photography-based variables capture independent associations with OSA in both samples. When adjusted for sex and age, greater mandible width (p< 0.01) differed both in the clinical and in the general population samples, reflecting AHI severity and the likelihood of OSA; the measure of smaller tongue curvature (p< 0.01) reflected the severity and probability of OSA in the clinical sample; and the higher posterior mandibular height (p=0.04) showed a relationship with higher AHI and higher risk of moderate to severe OSA in the general population sample. However, when adjusted for sex, age, and BMI, only smaller tongue curvature (p< 0.01) was associated with moderate/severe OSA. Conclusion Quantitative standardized facial photographs indicated that the measures of greater tongue and mandible were associated with increased OSA risk in the clinical sample and higher facial height measurement was associated in the general population sample. Abstract:Study objectives: Early in life impairment of orofacial growth leads to sleep-disordered breathing (SDB). Normal lingual gnosis and praxis are part of this early development related to the normal sensorimotor development of the tongue and surrounding oral musculature. The aim of this retrospective study was to explore if lingual praxia is impaired in both SDB children and adults and if there is an association to craniofacial morphology. Methods: The ability to perform simple tongue maneuvers was investigated in 100 prepubertal SDB children and 150 SDB adults (shown with polysomnography). All individuals had a clinical investigation by specialists to assess any orofacial growth impairment and the elements potentially behind this impairment. In a subgroup of individuals both able and unable to perform the maneuvers, we also performed a blind recognition of forms placed in the mouth. Results: A subgroup of pediatric and adult SDB patients presented evidence not only of orofacial growth impairment, but also apraxia independent of age and severity of OSA. Conclusions: By 3 years of age, children should be able to perform requested tongue maneuvers and have oral form recognition. Abnormal gnosis-praxis was noted, independent of age in SDB children and adults, demonstrating that an abnormal functioning of the tongue in the oral cavity during early development can be detected. Both children and adults with SDB may present similar absences of normal oral development very early in life and a similar presentation of apraxia, suggesting that the distinction of SDB in children versus adults may not be relevant.

This pilot observational study aimed to assess craniofacial values in obese and nonobese subpopulations of patients with OSAS and their correlation and association with the severity of OSAS. The integration of obstructive sleep apnea syndrome (OSAS) diagnosis could influence the choice of orthodontic techniques or their implementation, with the aim of improving ventilatory function. Material and method: The author summarizes the clinical studies using dentofacial orthopedics, alone or in combination with other interventions, in pediatric OSAS or the repercussions of orthodontic interventions on upper airways. Results: For the same orthodontic anomaly, in particular, transverse maxillary deficiency, the temporality and the modality of treatment could be modified by a diagnosis of OSAS. It could be recommended to propose early orthopedic maxillary expansion, seeking to potentiate its skeletal effect, to reduce the severity of OSAS. Class II orthopedic devices have shown interesting results but the evidence value of the studies is not yet sufficient to recommend them widely and as an early treatment. Extractions of permanent teeth do not significantly reduce the upper airway. Discussion: OSAS in children and adolescents includes several endotypes and phenotypes for which orthodontics may or may not be indicated. It is not recommended to orthodontically treat an apneic patient with no significant malocclusion, for the sole purpose of having an effect on the respiratory tract. Conclusion: The orthodontic therapeutic decision is likely to be modified by a diagnosis of sleep-disordered breathing underlining the interest in systematic screening.
Assessment of masticatory performance, bite force, orthodontic treatment need and orofacial dysfunction in children and adolescents

Abstract:Objectives: Few studies have evaluated the relationship between morphological and functional characteristics of the masticatory apparatus in young subjects. Thus, the aim of this study was to evaluate masticatory performance (MP), maximal bite force (BF), orthodontic treatment need and orofacial dysfunction in children and adolescents. Design: The sample consisted of 316 subjects of both genders, with an age range 6-16years divided into 4 groups: early mixed, intermediate mixed, late mixed and permanent dentition. MP was evaluated by the individual's ability to comminute a chewable test material in order to determine median particle size (X(50)) and distribution of particles in different sieves ("b"). BF was determined using a digital gnatodynamometer with fork strength of 10mm. Orofacial function and orthodontic treatment need were screened using the Nordic Orofacial Test-Screening (NOT-S) protocol and Index of Orthodontic Treatment Need (IOTN), respectively. The results were submitted to descriptive statistics, normality test, analysis of variance and stepwise multiple linear regression to test relationship between MP and studied independent variables. Results: Variance of X(50) and b between groups was statistically significant. But evaluation of variables that significantly contributed to MP variation showed that age, body mass index (BMI), BF and the presence of sleep bruxism were negatively related to X(50) and the NOT-S clinical exam scores showed a positive relationship with X(50). Conclusion: In the studied sample, age, BMI, BF and the presence of sleep bruxism were related to better MP; but the increase in NOT-S scores was significantly related to poorer MP.
Brain function in children with obstructive sleep apnea: a resting-state fMRI study

Abstract:Abstract Objective To explore the neural difference between children with obstructive sleep apnea (OSA) and healthy controls, together with the relation between this difference and cognitive dysfunction of children with OSA. Methods Twenty children with OSA (7.2 ± 3.1 years, apnea hypopnea index (AHI): 16.5 ± 16.6 events/h) and 29 healthy controls (7.7 ± 2.8 years, AHI: 1.7 ± 1.2 events/h) were recruited and matched with age, gender, and handedness. All children underwent resting-state fMRI (rs-fMRI) and T1-wighted imaging. Some children were sedated for MRI scanning. We compared amplitude of low frequency fluctuation (ALFF) and regional homogeneity (ReHo) of children with OSA with those of healthy controls. During resting-state, the former reflects the intensity of the spontaneous neural activities, whereas the latter reflects temporal similarity of the spontaneous neural activities within a local brain region. Pearson correlation analysis was performed between these features of rs-fMRI and cognitive scores among children with OSA. Results Compared with controls, children with OSA showed decreased ALFF in the left angular gyrus but increased ALFF in the right insula, and decreased ReHo in the left medial superior frontal gyrus, right lingual gyrus, and left precuneus. Additionally, among children with OSA, the ReHo value in the right lingual gyrus was negatively correlated with FIQ and VIQ, whereas that in the left medial superior frontal gyrus was positively correlated with VIQ. Conclusions Children with OSA presented abnormal neural activities in some brain regions and impaired cognitive functions with the former possibly being the neural mechanism of the latter.
