Abstract:Background: Frailty is prevalent among older adults with asthma or chronic obstructive pulmonary disease (obstructive lung diseases [OLDs]). Frailty and OLD's co-occurrence is associated with increased hospitalization/mortality. Chemosensory dysfunction is closely connected to both OLD and frailty. We evaluated the utility of olfactory decline as a biomarker of frailty in the setting of OLD. Methods: We performed a prospective, longitudinal, nationally representative study of community-dwelling older US adults in the National Social Life, Health and Aging Project, an omnibus in-home survey. Respondents reported a physician's diagnosis of OLD. Decline in odor identification and sensitivity over 5 years and frailty (adapted fried frailty phenotype criteria) were measured using standard tools. Multivariate logistic regressions evaluated the association between OLD status, olfactory decline, and frailty. Results: We compared individuals with OLD (n = 98; mean age 71.2 years, 59.2% women) and those without OLD (n = 1036; mean age 69.5 years, 58.9% women). Olfactory identification decline was associated with developing frailty over the 5-year follow-up period in individuals with OLD (odds ratio [OR] = 9.1, 95% confidence interval [CI] = 2.1-38.6, p = 0.003). Olfactory decline predicted incidence of frailty in individuals with OLD (identification: OR = 4.8, 95% CI = 1.3-17.5, P = 0.018; sensitivity: OR = 6.1, 95%CI = 1.2-31.0, p = 0.030) but not in those without OLD adjusting for demographics, heavy alcohol use, current smoking, and comorbidity. Results were robust to different thresholds for olfactory decline and frailty development. Conclusions: Older adults with OLD who experience olfactory decline face higher odds of developing frailty. Use of olfactory decline as a biomarker to identify frailty could allow earlier intervention and decrease adverse outcomes for high-risk older adults with OLD.
Olfactory Identification Deficits Are Associated with Cognitive Decline in Chinese Older Adults: the Taizhou Imaging Study

Abstract:AbstractBackgroundOlfaction function has emerged as a potential predictor of cognitive decline and dementia recently. We accessed the association of olfaction and cognitive function in a Chinese cohort.MethodParticipants in this study were part of the community‐based Taizhou Imaging Study. At the first wave of cognitive function follow‐up, 770 individuals underwent neuropsychological testing from July 2018 to August 2019, including four cognitive domains‐memory, executive function and attention, visuospatial function, and language. Mini‐Mental State Examination (MMSE) and Montreal Cognitive Assessment (MoCA) were used to assess participants’ global cognition. Among them, 686 participants underwent olfactory identification testing using 12‐item Sniffin’s Sticks test. Dementia and mild cognitive impairment (MCI) were adjudicated by a committee of experts, according to the Diagnostic and Statistical Manual of Mental Disorders, 4th edition (DSM‐IV) and Petersen RC’s criteria, respectively. After data quality control, 684 participants were included in this analysis (Figure 1). Beta regression was used to evaluate the association between olfaction and cognitive function (continuous variables), logistic regression was used to assess the association between olfaction and cognitive status (categorical variable). Potential confounders were adjusted in two models: Model 1 was adjusted for age, sex, and years of education; Model 2 was additionally adjusted for smoking, alcohol consumption, body mass index, exercise, hypertension, diabetes, and hyperlipidemia.ResultA total of 54 (7.9%) individuals were diagnosed as dementia, 165 (24.1%) individuals were adjudicated as MCI (Table 1). In the fully adjusted Model 2, the associations remained significant (Beta = 0.062 and 0.093, respectively; both P <0.001) (Table 2). Olfactory identification scores were associated with domain‐specific cognitive function scores in Model 2 (visuospatial function: Beta = 0.072, P = 0.003; short delay recall: Beta = 0.120, P <0.001; long delay recall: Beta = 0.057, P = .010; language: Beta = 0.107, P <0.001). Moreover, we found olfactory identification was negatively associated with MCI in Model 2 (odds ration [OR]: 0.88; 95% confidence interval [CI]: 0.81, 0.96) and dementia (OR: 0.76; 95% CI: 0.66, 0.87) (Table 3).ConclusionOlfactory identification are associated with both global and domain‐specific cognitive function and could serve as a protector for cognitive decline.

Abstract:The abnormal olfactory function has been regarded as a sign in psychosis. To confirm the theory that olfactory and cognitive functions are deficient in Chinese cohort with clinical high risk state of psychosis, 19 individuals at clinical high risk for psychosis (CHRs) and 37 healthy controls (HCs) were recruited. The CHRs scored significantly lower than the HCs in the olfactory identification test and all other cognitive tests. Our findings suggest that olfactory dysfunction may be an early change of psychosis.

Abstract:Objective: The objective of this study is to examine the association of olfactory function and genetic predisposition of Alzheimer's disease (AD) with cognitive performance in adults. Methods: A total of 2049 Chinese adults from Rugao Longevity and Ageing Study (RuLAS, n=1460, mean age 78 years) and Central China Cohort (CCC, n=589, mean age 48 years) were included in this study. A standard interview-based survey, clinical information, and blood samples were collected in both cohorts. Olfactory function in terms of olfactory identification was measured by the brief version of the Chinese Smell Identification Test consisted of 18 full points. Cognitive performance was measured by the Chinese version of the Mini-mental State Examination. A genetic risk score (GRS) was calculated from 5 single nucleotide polymorphisms, which were robustly related to Alzheimer's disease in Caucasians and cognitive performance in our Chinese population. Results: In the pooled analyses, participants at the lowest quartile of olfactory function had significantly higher odds of cognitive impairment (adjusted odds ratio [95% CI] = 1.45 [1.00 to 2.09], P trend = 0.005), and such association was stronger among participants with a stronger genetic predisposition of Alzheimer's disease (beta coefficient +/- SE, -0.06 +/- 0.03 in participants with a lower GRS vs.-0.19 +/- 0.05 in those with a higher GRS, respectively, P-interaction=0.01). Similar associations were observed in RuLAS (P-trend=0.06) and in CCC (P-trend<0.001). Conclusion: In this study, a decreased olfactory function was associated with worse cognitive performance in adults, especially among participants with a higher genetic risk of Alzheimer's disease. Further studies are warranted to evaluate the causal relationship between olfaction and cognitive performance.

Abstract:Decreased olfactory function is very common in the older population, being present in >50% of individuals aged between 65 and 80 years and in 62-80% of those >80 years of age. Smell dysfunction significantly influences physical well-being, quality of life, nutritional status as well as everyday safety and is associated with increased mortality. Multiple factors contribute to age-related olfactory sensory loss, including nasal engorgement, cumulative damage of the olfactory epithelium from environmental insults, a reduction in mucosal metabolizing enzymes, sensory loss of receptor cells to odorants, and changes in neurotransmitter and neuromodulator systems. In addition, structural and functional abnormalities of the olfactory epithelium, olfactory bulb, central olfactory cortex, and basic olfactory circuitry, which are related to the neuronal expression of aberrant proteins in these areas, may result in olfactory sensory impairment in aging and neurodegenerative diseases. Impaired odour identification is associated with a decrease in cognitive abilities and memory decline. A reduction in the sense of smell is considered to potentially represent an early and important warning of neurodegenerative disorders, particularly of Parkinson's disease and Alzheimer's disease, and, in mild cognitive impairment, olfactory impairment may herald progression to dementia. Further investigations of the potential role of olfactory dysfunction in the early diagnosis and treatment of neurodegenerative diseases are warranted.

Poor Odor Identification Predicts Mortality Risk in Older Adults Without Neurodegenerative Diseases: the Shanghai Aging Study

Abstract:Background Irrespective of neurodegeneration, the decline of olfactory function might also reflect a wider range of pathological conditions contributing to mortality. However, the potential explanations and their predictive values have rarely been reported. Methods A total of 1,433 older adults aged ≥ 60 years without neurodegenerative disease were administered a follow-up of 8.6 years on average. Sniffin’ Sticks Screening Test was used to assess the olfactory identification at baseline. Survival status during the follow-up was obtained from the local Center for Disease Control and Prevention. Bidirectional stepwise Cox proportional hazards regression was used to identify variables associated with mortality. Two predictive models were constructed by statistical learning methods. Results All-cause mortality rate was 1.1/100 person-years during the follow-up. Sex (HR = 0.65, 95%CI 0.46 - 0.93), age (HR = 1.12, 95%CI 1.10 - 1.15), chronic kidney disease (HR = 1.88, 95%CI 1.09 - 3.25), low density lipoprotein (HR = 0.81, 95%CI 0.67 - 0.99), anemia (HR = 2.74, 95%CI 1.19 - 6.30), and fail to identify coffee odor (HR = 1.96, 95%CI 1.19 - 3.23) were significantly associated with all-cause mortality. The logistic regression and the random forest predictive models showed similar predictive accuracy (0.91 and 0.90) and area under the receiver operating characteristic curve (0.77 and 0.75). Conclusions The association between poor olfactory and long-term mortality was verified among Chinese older population. Certain odors identification ability may contribute to the prediction of long-term mortality along with other important risk factors.

Abstract:Background The smell sense reduction was considered to represent the potentially warning of early stage of neurodegenerative disorders. The Shanghai Aging Study provided us a unique opportunity to explore the association between olfactory identification (OI) and cognitive function among community-dwelling elderly in China. Methods OI of each participant was measured by the 12-item identification tests from Sniffin’ Sticks Screening test (SSST-12). Participants with mild cognitive impairment (MCI) were diagnosed by Petersen criteria. We used the logistic regression analysis to explore the association between OI scores and cognitive function by adjusting potential confounders. Results Among 1782 non-demented participants, 345 (19.4 %) participants were diagnosed as MCI. The mean OI score for participants with MCI [7.1 (SD 2.3)] was significantly lower than that for those with normal cognition [8.2 (SD 2.0), P < 0.0001]. After adjusted for age, gender, education, lifestyles, medical history, Apolipoprotein E genotype, lower OI score was found to be an independent influence factor related to MCI (OR 1.19, 95 % CI 1.11–1.27). Conclusions Our study suggests that poor OI may be associated with MCI in elderly population. Further prospective studies may confirm the OI as a reliable and early marker predicting the decline of cognitive function.

Abstract:Background: Olfactory dysfunction is associated with poor physical performance in older adults. However, it remains unknown whether the ability to identify particular olfactory clusters and/or odors is associated with physical performance in physically independent community-dwelling older adults. Methods: This cross-sectional study included 130 community-dwelling older adults (70.1 ± 5.5 years). The Odor Stick Identification Test for Japanese people, consisting of 12 odors in four clusters (wood, grass, herb; sweet; spices; foul-smelling), was used to examine olfaction. Participants also completed physical performance tests (one leg standing with open eyes; aerobic capacity; lower muscle function: five-times chair stand [CS] and vertical jump; mobility: star walking and timed up and go [TUG]) and cognitive function tests. Results: Worse overall olfaction was not significantly associated with any physical performance measure. Worse performance for identifying sweet odors and an inability to identify some specific odors (menthol and rose) were associated with worse mobility and/or lower muscle function-adjusted covariates. Moreover, an inability to identify menthol and rose was associated with worse TUG (odds ratio [OR]: 0.424; 95% confidence interval [CI]: 0.215-0.836), star walking (OR: 0.714; 95% CI: 0.506-0.976), CS (OR: 0.638; 95% CI: 0.470-0.864), and vertical jump (OR: 1.12; 95% CI: 1.001-1.24) performance, even when the analysis was adjusted to exclude menthol and rose score from the overall olfaction score (p < .05 for all). Conclusions: The current study may help to increase awareness of olfactory and physical dysfunction at an earlier stage among physically independent community-dwelling older adults.
Inability to Smell Peppermint Is Related to Cognitive Decline: A Prospective Community-Based Study

Abstract:Background: A few studies have demonstrated the association of poorer olfactory identification (OI) with poorer cognition in population-based cohorts. None of them considered the outcome associated with the inability to smell a certain odor. Objective: To verify the hypothesis that at least one specific odor is associated with incident cognitive decline among older adults. Methods: In the Shanghai Aging Study, a sub-cohort of 948 dementia-free participants who had baseline OI measurements were prospectively followed for 5 years. Results: An inability to smell peppermint (beta = -0.44, p < 0.001), rose (beta = -0.14, p = 0.040), or coffee (beta = -0.37, p = 0.002) was inversely related to the annual rate of change in the Mini Mental State Examination score, and an inability to smell peppermint was associated with a higher risk for incident dementia (hazard ratio 2.67, 95% CI 1.44-4.96) after adjustment for confounders. Conclusion: Our study suggests that some odors, especially peppermint, might be considered as a potential predictor for dementia in older populations.

Abstract:Background: Olfactory dysfunction in Parkinson's disease (PD) is associated with more severe phenotypes, but trajectories of cognitive function, disease severity, and subdomains of quality-of-life measurements in patients with distinct olfactory profiles remain underexplored. Objective: To analyze the influence of olfaction on trajectories of clinical parameters in patients with PD. Design: Retrospective cohort study. Subjects: From October 2016 to May 2021, the study tracked 58 participants over 3 years. Participants completed follow-up assessments using tools including the Chinese version of the University of Pennsylvania's Smell Identification Test (UPSIT), Montreal Cognitive Assessment (MoCA), Movement Disorder Society-sponsored revision of the Unified Parkinson's Disease Rating Scale, and the Chinese translation of the 39-item Parkinson's Disease Questionnaire (PDQ-39). Methods: Participants were divided into anosmia (UPSIT < 19) and non-anosmia (UPSIT ≥ 19) groups based on initial scores. Generalized estimating equations and repeated measures correlations were used to examine longitudinal associations and correlations between olfaction and clinical parameters. Results: Divergent cognitive trajectories were observed between groups. The anosmia group exhibited a faster cognitive decline (adjusted B [beta coefficient] = −1.8, p = 0.012) according to the interaction effect of olfaction and time on the MoCA score. The anosmia group exhibited no longitudinal correlation between cognition and olfactory function but showed correlations with age ( r rm [coefficient of repeated measures correlation] = −0.464, p = 0.004) and disease duration ( r rm = −0.457, p = 0.005). The non-anosmia group's UPSIT scores decreased over time ( B = −2.3, p = 0.005) alongside a significant correlation with motor function ( r rm = −0.479, p = 0.006). Conclusion: The anosmia group's accelerated cognitive decline correlated with age and disease duration, but not olfactory function, suggesting a poor cognitive outcome in this population despite the lack of longitudinal correlation between cognition and olfaction. The non-anosmia group exhibited progressive olfactory degradation and notable correlations between motor function and UPSIT scores, implying pathological accumulation in the olfactory structure and basal ganglia.

Abstract:Background and objectives: Mobility limitations has been linked to cognition. However, little is known about the relationship between mobility decline and cognitive decline. This study investigated the effect of mobility limitations and decline on cognitive decline in a population-based cohort of older adults. Result design and methods: A population-based cohort of 9695 cognitively intact participants (mean age = 65.4 years, standard deviation [SD] = 10.4) was assessed. Mobility limitation scores ranging from 0-10 were assessed at baseline (wave 4) by using self-reporting difficulty in a set of 10 activities, and a higher score indicated worse mobility. A subset of 9250 participants underwent two mobility assessments at waves 3 and 4, and were categorized into normal mobility or mobility decline (defined as wave 4 - wave 3 > 1 SD of wave 3). Linear mixed models were used to assess the longitudinal contribution of mobility limitations and decline to cognitive decline. Results: During a median follow-up period of 9.4 years (SD 1.8), the participants in the highest quartile of mobility scores displayed an accelerated cognitive decline (-0.191 SD/year, 95% CI = -0.223, -0.159) compared with those in the lowest quartile. Notably, individuals experiencing mobility decline exhibited a marked cognitive decline (-0.179 SD/year, 95% CI = -0.220, -0.139), potentially influenced by factors such as physical activity and depression. Discussion and implications: Mobility limitations and decline significantly correlate with cognitive decline in older adults, highlighting that mobility-focused interventions in healthcare strategies to preserve cognition.

Abstract:Objectives: Research on olfaction and brain neuropathology may help understand brain regions associated with normal olfaction and dementia pathophysiology. To identify early regional brain structures affected in poor olfaction, we examined cross-sectional associations of microstructural integrity of the brain with olfaction in the Atherosclerosis Risk in Communities Neurocognitive Study. Methods: Participants were selected from a prospective cohort study of community-dwelling adults; selection criteria included: evidence of cognitive impairment, participation in a previous MRI study, and a random sample of cognitively normal participants. Microstructural integrity was measured by two diffusion tensor imaging (DTI) measures, fractional anisotropy (FA) and mean diffusivity (MD), and olfaction by a 12-item odor-identification test at the same visit. Higher FA and MD values indicate better and worse microstructural integrity, respectively, and higher odor-identification scores indicate better olfaction. We used brain region-specific linear regression models to examine associations between DTI measures and olfaction, adjusting for potential confounders. Results: Among 1,418 participants (mean age: 76±5years, 41% male, 21% Black race, 59% with normal cognition), the mean olfaction score was 9±2.3. Relevant to olfaction, higher MD in the medial temporal lobe (MTL) regions, namely, the hippocampus (β: -0.79 (95%CI: -0.94, -0.65) units lower olfaction score per SD higher MD), amygdala, entorhinal area, and some white matter (WM) tracts connecting to these regions, was associated with olfaction. We also observed associations with MD and WM FA in multiple atlas regions that were not previously implicated in olfaction. The associations between MD and olfaction were particularly stronger in the MTL regions among individuals with mild cognitive impairment (MCI) compared to those with normal cognition (e.g., β hippocampus : -0.75 (95%CI: -1.02, -0.49) and -0.44 (95%CI: -0.63, -0.26) for MCI and normal cognition, respectively, p-interaction=0.004). Conclusions: Neuronal microstructural integrity in multiple brain regions, in particular the MTL (the regions known to be affected in early Alzheimer’s), is associated with odor-identification ability. Differential associations in the MTL regions among cognitively normal individuals compared to those with MCI may reflect the earlier vs. later effects of the dementia pathogenesis. It is likely that some of the associated regions may not have any functional relevance to olfaction.

Abstract:Background: Evidence on the association of Olfactory Impairment (OI) with age-related cognitive decline is inconclusive, and the potential influence of allergy remains unclear. Objective: We aimed to evaluate the cross-sectional associations of allergy-related and non-allergy- related OI to cognitive function. Methods: We included 2,499 participants from the Health and Retirement Study (HRS)-Harmonized Cognitive Assessment Protocol (HCAP) sub-study and 1,086 participants from the English Longitudinal Study of Ageing (ELSA)-HCAP. The Olfactory Function Field Exam (OFFE) using Sniffin’ Stick odor pens was used to objectively assess olfactory function and an olfactory score <6/11 indicated OI. Mini-Mental Status Examination (MMSE) was used to assess global cognitive function and define cognitive impairment (<24/30). A neuropsychologic battery was used to assess five cognitive domains. Results: Compared to non-OI participants, individuals with OI had lower MMSE z-score [βHRS = -0.33, 95% Confidence Interval (CI): -0.41 to -0.24; βELSA = -0.31, -0.43 to -0.18] and higher prevalence of cognitive impairment [Prevalence Ratio (PR)HRS = 1.46, 1.06 to 2.01; PRELSA = 1.63, 1.26 to 2.11]. The associations were stronger for non-allergy-related OI (βHRS = -0.36; βELSA = -0.34) than for allergy-related OI (βHRS = -0.26; βELSA = 0.13). Similar associations were observed with domain- specific cognitive function measures. Conclusion: OI, particularly non-allergy-related OI, was related to poorer cognitive function in older adults. Although the current cross-sectional study is subject to several limitations, such as reverse causality and residual confounding, the findings will provide insights into the OI-cognition association and enlighten future attention to non-allergy-related OI for the prevention of potential cognitive impairment.

Olfactory Dysfunction and Balance Dysfunction are Associated with Increased Falls in Older Adults

Abstract:Objective: This study aims to characterize the association between impairments in olfaction and balance, both of which are mediated in part by the cerebellum, and how this relates to prospective incidence of falls in a cohort of aging adults. Methods: The Health ABC study was queried to identify 296 participants with data on both olfaction (measured using the 12-item Brief Smell Identification Test) and balance-related function (measured using the Romberg test). The relationship between olfaction and balance was investigated using multivariable logistic regression. Predictors of performance on a standing balance assessment and predictors of falls were studied. Results: Of 296 participants, 52.7% had isolated olfactory dysfunction, 7.4% had isolated balance dysfunction, and 5.7% had dual dysfunction. Severe olfactory dysfunction was associated with increased odds of balance dysfunction when compared to those without olfactory dysfunction, even when adjusting for age, gender, race, education, BMI, smoking, diabetes, depression, and dementia (OR = 4.1, 95% CI [1.5, 13.7], p = 0.011). Dual sensory dysfunction was associated with worse performance on a standing balance assessment (β = -22.8, 95% CI [-35.6, -10.1], p = 0.0005) and increased falls (β = 1.5, 95% CI [1.0, 2.3], p = 0.037). Conclusion: This study highlights a unique relationship between olfaction and balance, and how dual dysfunction is associated with increased falls. With substantial implications of falls on morbidity and mortality in older adults, this novel relationship between olfaction and balance emphasizes a potentially shared mechanism between olfactory dysfunction and increased fall risk in older adults; however, further study is required to explore the novel relationship of olfaction with balance and future falls. A Prospective Study on Risk Factors for Olfactory Dysfunction in Aging

Abstract:Background: Olfactory dysfunction (OD) refers to a reduced or absent ability to smell. OD negatively impacts health and quality of life and its prevalence increases with advancing age. Since OD may be an early marker of dementia and impending death, more knowledge regarding risk factors of OD in aging is warranted. The objective was therefore to explore longitudinally which demographic, genetic, clinical, lifestyle, and cognitive factors predict the development of OD. Methods: The study included participants aged 60-90 years from the Swedish National Study on Aging and Care in Kungsholmen (SNAC-K), who did not have OD at baseline and were reassessed with an odor identification task at a 6-year follow-up (n = 1,004). Risk factors of OD were assessed with multivariable logistic regression analyses. Results: The percentage of incident OD cases was 14.2% over 6 years in the total sample and this number increased monotonically with age. Increasing age, carrying the ε4 allele of the APOE gene, atrial fibrillation, cerebrovascular disease, and current smoking were found to be risk factors for the development of OD, whereas better olfactory identification and verbal episodic memory proficiency at baseline were identified as protective factors. Conclusions: In addition to nonmodifiable factors (age and genetic risk), several modifiable risk factors of OD were identified. This suggests that it might be possible to reduce OD incidence through the management of vascular risk factors and maintenance of a healthy lifestyle.
Reduced Olfactory Memory Performance Is Associated with Executive Function Deficits in Older Adults

Abstract:Previous research has identified an association between olfactory impairment (reduced odor sensitivity/ identification/ discrimination) and cognitive impairment in older adults. The present study focused on the relationship between olfactory memory performance and cognitive/affective functioning. Recognition performance for olfactory and visual stimuli (control condition) was tested through a matching task in older adults ( n = 44; M age = 76 years) and younger adults ( n = 56; M age = 24 years). Additionally, negative affect (anxiety, depression) and cognitive functioning were assessed via validated questionnaires and a neuropsychological test battery. The older participants performed worse than the younger adults in the olfactory memory task. In older adults, difficulties in remembering odors were associated with reduced odor identification and executive functioning (reduced cognitive flexibility). Affective well-being was not related to olfactory memory performance. Olfactory memory impairment in older adulthood might be a marker for cognitive decline in areas related to executive functions.

Abstract:Olfactory impairment is a potential marker for prodromal dementia, but the underlying mechanisms are poorly understood. This population‐based study included 4214 dementia‐free participants (age ≥65 years). Olfaction was assessed using the 16‐item Sniffin' Sticks identification test. In the subsamples, we measured plasma amyloid beta (Aβ)40, Aβ42, total tau, and neurofilament light chain (NfL; n = 1054); and quantified hippocampal, entorhinal cortex, and white matter hyperintensity (WMH) volumes, and Alzheimer's disease (AD)‐signature cortical thickness (n = 917). Data were analyzed with logistic and linear regression models. In the total sample, mild cognitive impairment (MCI) was diagnosed in 1102 persons (26.2%; amnestic MCI, n = 931; non‐amnestic MCI, n = 171). Olfactory impairment was significantly associated with increased likelihoods of MCI, amnestic MCI, and non‐amnestic MCI. In the subsamples, anosmia was significantly associated with higher plasma total tau and NfL concentrations, smaller hippocampal and entorhinal cortex volumes, and greater WMH volume, and marginally with lower AD‐signature cortical thickness. These results suggest that cerebral neurodegenerative and microvascular lesions are common neuropathologies linking anosmia with MCI in older adults.

Abstract:The human olfactory system is among the oldest limbic structures and has a critical yet underappreciated role in health and behavior. The sense of smell facilitates detection and identification of spoiled food and volatile environmental hazards, influences healthy grooming and personal hygiene, and affects nutritional status through its impact on appetite and feeding behavior. Olfactory regions have reciprocal connections to key brain regions that subserve memory, emotion, and satiety signaling. The olfactory nerve is the only cranial nerve that is directly exposed to the environment, and it is vulnerable to disruption by conditions that contribute to mortality, including medical illness, traumatic brain injury, and neurodegenerative disease.
