Abstract:Objective: We are yet to understand how widely parents seek asthma medication management information for their children, how they are used for health information, how parents engage with them and their influence on parent's decision-making. This study aimed to gauge the current level of asthma knowledge and skills of parents of children with asthma and gain insight into who and what influences their child's asthma medication management decisions. Method: Social network theory was used to map parents' asthma networks and identify the level of influence of each individual/resource nominated. Parents of children with asthma (aged 4-18 years) were interviewed, completed an asthma network map, questionnaires and an inhaler technique assessment. Results: Twenty-six parents participated and had significant gaps in asthma knowledge and inhaler technique skills. The asthma networks of participants ranged from two to ten individuals/resources, with an average number of five. The most commonly nominated individual/resource was general practitioners followed by family members and the internet. Professional connections represented 44% of individuals/resources in networks, personal connections 42% and impersonal connections 14%. When parents were asked about how influential individuals/resources were, professional connections represented 53% of parents influences, personal connections 36% and impersonal connections 11%. Conclusion: This study highlights the priority and co-influence of non-medical sources of information/support on parent's behaviors and decision-making with regards to their child's asthma medicine taking. In further understanding the complexities surrounding these connections and relationships, HCPs are better positioned to assist parents in addressing their needs and better supporting them in the management of their child's asthma.
School-Based Interprofessional Asthma Self-Management Education Program for Middle School Students: A Feasibility Trial

Abstract:BACKGROUND: Asthma is the most common chronic disease among school-aged children under 18 years of age and is a major cause of morbidity, loss of school days, and increased hospitalizations. Asthma disproportionately affects low-income, minority youth in Alabama. The benefits of improving asthma control and self-management have significance for improving health, preventing disease, and reducing health disparities by addressing social, behavioral, environmental, economic, and medical determinants of health.OBJECTIVES: This collaborative community partnership between the Mobile County Public School System, University of South Alabama (USA), and USA Children's and Women's Hospital involved nursing students, respiratory therapy students, and medical residents from three colleges and the hospital. The research question was whether a school-based asthma self-management education program presented by an interprofessional team was feasible for teens with asthma in a medically underserved area (MUA).METHODS: Middle school students with a diagnosis of asthma participated in this institutional review board-approved study. Asthma assessments, one-on-one coaching, and group education were done over five sessions, using Power Breathing for teens curriculum. Instruments were the Childhood Asthma Control Test (ACT), the Asthma Responsibility Questionnaire (ARQ), the Self-Efficacy Scale, the Pediatric Asthma Quality of Life Questionnaire (PAQLQ), peak flow monitoring, and spirometry.RESULTS: Eighteen students with moderate to severe persistent asthma completed the program with reduced asthma symptoms and increased asthma control, medication knowledge/skills, self-efficacy, and asthma responsibility.CONCLUSIONS: Adolescents with asthma need education in medication management, spacer use, peak flow, trigger avoidance, and coaching to take on the responsibility of asthma care. This multidimensional, interprofessional approach can strengthen asthma self-management in a middle school medically underserved community population.

Abstract:trate of T lymphocytes and neutrophils plays a significant pathophysiological role. It is, therefore, not surprising that TNF-a inhibitors are effective in downregulating this process to achieve disease remission. Our three patients had a dramatic and sustained response to adalimumab. Two of the three had failed previous treatment with other biologic therapies, showing that adalimumab can be effective in ACH where other biologics have failed. Adalimumab was well tolerated. One patient developed metastatic lung cancer but it is unclear what role, if any, the drug played. Although it is possible that 5 years of anti-TNF-a treatment with infliximab, etanercept and adalimumab and a multitude of prior immunosuppressive agents contributed to the development of lung cancer, she was an 80-year-old lifelong smoker. This report confirms adalimumab to be an effective agent for ACH.

Abstract:To address pediatric asthma disparities on the South Side of Chicago, a community health worker (CHW) home visiting intervention was implemented collaboratively by academic institutions and community based health centers. This evaluation assessed the effectiveness of this longitudinal quality improvement CHW intervention in reducing asthma morbidity and healthcare utilization. All patients aged 2–18 who met the high-risk clinical criteria in outpatient settings or those who visited the ED due to asthma were offered the program. A within-subject study design analyzed asthma morbidity and healthcare utilization at baseline and follow-up. Multivariable mixed-effects regression models, adjusted for baseline demographic and asthma characteristics, were used to assess changes over time. Among 123 patients, the average age was 8.8 (4.4) years, and 89.3% were non-Hispanic black. Significant reductions were observed in the average daytime symptoms days (baseline 4.1 days and follow-up 1.6 days), night-time symptoms days (3.0 days and 1.2 days), and days requiring rescue medication (4.1 days and 1.6 days) in the past two weeks (all p < 0.001). The average number of emergency department visits decreased from 0.92 one year before to 0.44 one year after program participation, a 52% reduction ( p < 0.001). No significant difference was found in hospital admissions. These results support the use of a collaborative approach to implement the CHW home visiting program as part of standard care for pediatric asthma patients in urban settings. This approach has the potential to reduce asthma disparities and underscores the valuable role of CHWs within the clinical care team.

Abstract:Asthma is one of the most common causes of school absenteeism, and many children are affected by, or encounter, it in the school setting. An integrated curriculum that presents asthma as a real world example can raise all children’s awareness and understanding of asthma, not just those with the condition. A 15-lesson, asthma-based curriculum was developed to integrate with and enhance the core subjects of math, science, and communication arts. A pilot test was performed in fourth- and fifth-grade classes to assess student asthma knowledge gain, teacher acceptance, and grade appropriateness of the curriculum. During the 2006–2007 school year, 15 teachers were recruited from the St. Louis, MO, USA area to assess the curriculum through teaching and administering pre- and post-unit tests and completing a teacher evaluation for each lesson taught. Four additional classrooms served as comparisons. Paired t tests were used for each lesson taught, to evaluate pre-/post-test and classroom differences, and focus groups were used for qualitative evaluation. There was an increase in asthma knowledge between pre- and post-tests in both grades, individually and combined (p < 0.001). Intervention post-test scores were higher than comparison classroom scores (p < 0.001). Teacher feedback indicated that the lessons enhanced previously learned skills and increased students’ overall understanding of asthma. Offering asthma education in the classroom can provide an opportunity for all students to gain asthma knowledge and build health literacy about a leading chronic disease in school-aged children.

Abstract:Abstract Introduction Children who have asthma and live in urban settings experience challenges that can complicate asthma management and compromise sleep outcomes. The goals of this presentation are to 1) describe environmental factors in children’s homes impacting asthma and sleep, 2) examine associations between asthma and sleep outcomes in urban children, and 3) examine the moderating role of environmental and behavioral factors in the association between asthma and sleep outcomes. Methods 140 children 7-9 years old with persistent asthma and their primary caregivers completed a 16-day observational protocol. Asthma and sleep outcomes were evaluated via self/caregiver report (Asthma Control, Sleep Hygiene) and objective (portable handheld spirometer, actigraphy) methods. Home sleep and asthma environment was assessed with a structured interview. Results The most prevalent environmental risk factors for asthma and sleep were tobacco smoke (25% of sample), pets (42%), and pests (e.g., rodents, roaches, etc.; 34%). Sleep hygiene/environmental factors reported by families as challenging included child routinely falling asleep (48% of sample) or sleeping all night (48%) in a room not intended as their sleep space. Shared sleep space (40%) and lack of protective allergy bedding (40%) were also common challenges. Numerous factors causing objectively measured nighttime awakenings and sleep disruption were also reported. On average, children with well-controlled asthma reported more optimal sleep hygiene behaviors (rho = .91, p<.05) relative to those with poorly controlled asthma. Better day-to-day lung function (via spirometry) was related to fewer nighttime awakenings, measured with actigraphy (rho = .20; p’s <.05). Children with poorly controlled asthma tended to have more night awakenings and lower levels of sleep efficiency than those with well controlled asthma. Analyses testing a moderating role of environmental/behavioral processes are underway. Conclusion Urban children's sleep environment contains asthma and sleep-related factors that make it challenging to fall and stay asleep. These disruptors affect both asthma activity and sleep and are areas to target in future multi-component asthma and sleep interventions. Abstract:Background The tendency of sepsis to progress rapidly and the benefit of an early start of treatment emphasise the importance of fast risk stratification in the emergency department (ED). The aim of the present work was to validate the Mortality in Emergency Department Sepsis (MEDS) score as a predictor of 28-day mortality in ED patients with sepsis in The Netherlands, and to compare its performance to C reactive protein (CRP) and lactate. Methods This was a historical cohort study in a secondary and tertiary care university hospital. Patients were included if they were seen by an internist in the ED, fulfilled the clinical criteria for sepsis and were admitted to the hospital. Primary outcome was all-cause in-hospital mortality within 28 days. Results In the 6-month study period, 331 patients were included, of whom 38 (11.5%) died. Mortality varied significantly per MEDS category: ≤4 points (very low risk: 3.1%), 5–7 points (low risk: 5.3%), 8–12 points (moderate risk 17.3%), 13–15 points (high risk: 40.0%), >15 points (very high risk: 77.8%). Receiver operating characteristic (ROC) analysis showed that the MEDS score predicted 28-day mortality better than CRP (area under the curve (AUC) values of 0.81 (95% CI 0.73 to 0.88) and 0.68 (95% CI 0.58 to 0.78), respectively). Lactate was not measured in enough patients (47) for a valid evaluation, but seemed to predict mortality at least fairly (AUC 0.75, 95% CI 0.60 to 0.90). Conclusions The MEDS score is an adequate tool for predicting mortality in patients with sepsis in a Dutch internistic ED population. CRP is less useful in this context. Lactate appears to be at least a fair predictor of mortality, but needs to be investigated more systematically in a larger population.
Interprofessional collaboration for children with physical disabilities: a scoping review

Abstract:Interprofessional collaboration is vital in the context of service delivery for children with physical disabilities. Despite the established importance of interprofessional collaboration and an increasing focus on research on this topic, there is no overview of the research. A scoping review was conducted to explore current knowledge on interprofessional collaboration for children with physical disabilities from the point of view of the actors involved. The steps of this review included identifying a research question, developing a protocol, identifying relevant research, selecting studies, summarizing and analyzing the data, and reporting and discussing the results. Through databases and studies from hand-searches, 4,688 records were screened. A total of 29 studies were included. We found that four themes: communication, knowledge, roles, and culture in interprofessional collaboration illustrate current knowledge on the topic. Interprofessional collaboration for children with physical disabilities is shown to be composed of these four themes, depending on the actors involved. Interprofessional collaboration is affected by how these four themes appear; they mainly act as barriers and, to a lesser extent, as facilitators for interprofessional collaboration. Whether and how the themes appear as facilitators need further exploration to support innovation of interprofessional collaboration.

Abstract:To (1) describe primary health care utilization and (2) estimate the effect of primary care early follow-up, continuity, regularity, frequency, and long consultations on asthma hospital readmission, including secondary outcomes of emergency (ED) presentations, asthma preventer adherence, and use of rescue oral corticosteroids within 12 months. An Australian multi-site cohort study of 767 children aged 3–18 years admitted with asthma between 2017 and 2018, followed up for at least 12 months with outcome and primary care exposure data obtained through linked administrative datasets. We estimated the effect of primary care utilization through a modified Poisson regression adjusting for child age, asthma severity, socioeconomic status and self-reported GP characteristics. The median number of general practitioner (GP) consultations, unique GPs and clinics visited was 9, 5, and 4, respectively. GP care was irregular and lacked continuity, only 152 (19.8%) children visited their usual GP on more than 60% of occasions. After adjusting for confounders, there was overall weak indication of effects due to any of the exposures. Increased frequency of GP visits was associated with reduced readmissions (4–14 visits associated with risk ratio of 0.71, 95% CI 0.50–1.00, p = 0.05) and ED presentations (>14 visits associated risk ratio 0.62, 95% CI 0.42–0.91, p = 0.02). Our study demonstrates that primary care use by children with asthma is often irregular and lacking in continuity. This highlights the importance of improving accessibility, consistency in care, and streamlining discharge communication from acute health services.

Balancing our focus: The development of a collaborative approach between child development and children’s mental health services

Abstract:Child development and children’s mental health services are currently not an integrated service in Brisbane (and many other regions around the world). Children with social-emotional, behavioural and developmental issues present to both child development and children’s mental health services.Without joint collaboration, it is difficult for either team to independently gain a thorough understanding of both the child’s developmental profile and the environmental context in which the development occurs. This can prevent the adequate meeting of the child and family’s needs and can reduce the appropriateness of interventions.Clinicians in Zero to Four Child and Youth Mental Health and the Child Development Service in northern Brisbane have recently begun to work more closely when families require the involvement of both services. A joint formulation, developed collaboratively from the initial assessments of both teams, focusing on identifying need and responding to need, can be an innovative solution to lengthy waiting times and may contribute to earlier intervention.1 The collaboration has been motivated by a biopsychosocial framework and systems theory which emphasise that you cannot separate a child from the family and environment in which they live. Current research highlights the interlinking of developmental and psychological processes and the relational context of development.Balancing both a developmental and mental health focus is important in order to fully meet the needs of the children and families. In order to integrate emotional and developmental schools of thought in clinical formulation and therapy, it is necessary to maintain a reflective stance within teams. This prevents clinicians from merely “seeing what we know” and to consider the most effective treatments.Collaboration during the assessment process has led to greater accuracy in formulating the child’s difficulties while also reassuring parents and carers that the health professionals are “on the same page.” In addition, it has enabled the opportunity for early intervention related to mental health concerns for the child. When a child’s mental health needs are addressed within the family context, this creates a setting which will optimise the progress of the child with developmental challenges, enabling them to be the best that they can be. This presentation will include a discussion of how the child development and children’s mental health services inspire to work together with the family’s best interest in mind. Cases which involved collaboration between the two services will be presented, including a case where there was uncertainty as to whether a diagnosis of autistic spectrum disorder or trauma was more appropriate. Methods used to sensitively gather a detailed history and support the family through the assessment process will be described. Abstract:Mutating or inhibiting genes encoding starch branching enzymes (SBEs) can increase the amylose content (AC) of cereals. We analyzed endosperm starches from three rice cultivars with different ACs and from transgenic lines derived from them. The transgenic lines had simultaneously inhibited SBE I and IIb genes. Compared with the starch from their wild-type parents, the starch from transgenic lines showed significantly increased apparent ACs and lamella size and decreased relative crystallinity, double helix content, and lamellar peak scattering intensity, and altered short-range ordered structure in the external region. These changes were more prominent in the line derived from the high-AC cultivar than in those derived from waxy and low-AC cultivars. Inhibiting both SBE I and IIb changed the crystalline structure of starch from A-type to CA-type in lines derived from waxy and low-AC cultivars, and from A-type to C-type in that derived from the high-AC cultivar.

Abstract:OBJECTIVE To compare different treatment methods for sudden deafness of different blood platelet CD62p levels. METHODS Blood platelet was measured with flow cytometer. Patients with platelet CD62p over or less than 2% were treated with routine treatment (low molecular weight dextran, ATP, CoA and dexamethasone) or routine treatment plus batroxobin (thrombolysis) separately. RESULTS In the group with platelet CD62p > or = 2%, the effective rates of routine treatment was 60.5%, and that of routine treatment plus thrombolysis was 84.6%. There was significant difference between them. In the group with platelet CD62p < 2%, the effective rates of two methods were 65.0% and 71.8% respectively(no significant difference between them). CONCLUSIONS Detecting CD62p in sudden deafness patients makes sense for deciding treatment project.
Student perspectives on asthma management in schools: a mixed-methods study examining experiences, facilitators, and barriers to care

Abstract:Objective: Children spend nearly a third of their day at school, making it a critical point of intervention for those with asthma. This study aimed to illuminate minority students’ experiences with asthma at school and perceptions of facilitators and barriers to care. Methods: A mixed-methods study was conducted with elementary students with asthma. Participants completed a 24-question survey, drawing exercise to depict experiences caring for their asthma at school, and focus group discussion. Drawings were analyzed for narrative and pictorial themes. Discussions were audio-recorded, transcribed, and independently coded for themes. Results: Fifteen children with asthma (8–11 years) participated from two Chicago schools with predominately African-American populations. Most students (79%) indicated they had control of asthma at school, while 85% identified asthma as a problem when exercising. Half (53%) received help with asthma care at school. Drawings depicted cooperative management with adults or peers and the central role of inhalers as part of everyday asthma care in school. Finally, focus groups produced six key themes within the domains of facilitators: 1) support of others, 2) self-efficacy; 3) perception of being normal; and barriers: 4) lack of support from others; 5) difficulty accessing inhaler; 6) perception of being different. Conclusions: This study suggests asthma care plays an important role in students’ school experiences. Stigma around inhaler use, lack of concern by school staff, and limited access to medications remain as barriers to school-based asthma management. Although facilitators, such as support from teachers and peers, do exist, future interventions must address existing barriers.

Asthma self-management in children: a best practice implementation project

Abstract:Introduction: Asthma is one of the most common diseases in children. A self-management program can effectively improve the outcomes for children with asthma and reduce the burden on healthcare services. Objectives: The aim of this project was to integrate the best evidence on asthma self-management with practice in a children's respiratory clinic and to improve compliance with best practice. Methods: Seven audit criteria were developed for the pre- and post-audit based on the best available evidence. The Joanna Briggs Institute (JBI) Practical Application of Clinical Evidence System and Getting Research into Practice audit and feedback tools were used in this project. Results: The baseline audit showed a gap between clinical practice and the best evidence. The only criterion that achieved high compliance was provision of inhaler guidance (100%). After implementation, there were substantial improvements in compliance for many criteria. Training of clinicians increased from 13% at baseline to 67% at follow-up. Education of parents improved, with specific education about asthma triggers increasing from 55% to 100%, education about warning signs from 30% to 85% and education about effective asthma treatment options from 40% to 85%. Use of written asthma action plans increased from 0% to 25%. Conclusion: Strategies developed in this project were effective at providing necessary information for parents and improved the compliance with evidence. Further implementation strategies and audits are still needed to improve the use of asthma action plans and ensure they are reviewed periodically.

Teacher–therapist collaboration in inclusive primary schools: A scoping review

Abstract:Introduction Inclusive school environments require collaboration between teachers and allied health professionals to promote student access and participation. Collaboration is a complex phenomenon with no universally accepted definition or measurement and with many challenges to effective practice. The purpose of this scoping review is to describe what is known about interprofessional collaboration between teachers and therapists in inclusive primary schools. Methods A scoping review of health and education literature was conducted using the Joanna Briggs Institute methodology. Peer‐reviewed articles reporting on empirical studies with a focus on collaboration between teachers and school‐based occupational therapists or speech and language therapists in inclusive primary schools were included. Results Results summarise how collaboration is reported in the literature. Numerical and descriptive summaries describe how collaboration is defined and measured, the challenges to collaborative practice, the structures required to support effective practice, and the outcomes of such practice. Conclusion Definitions vary between studies and disciplines but contain common elements. For effective practice, the purpose of the collaboration must be clear, and the intended outcomes of the collaboration are measured. Measurement of collaboration requires further research using tools developed from robust theoretical frameworks and validated within the educational context and with professionals of different disciplines. Consistent measurement tools would allow cross‐study comparisons. Barriers to collaborative practice are well documented; thus, future research should be directed to examining effective practice, investigating how professionals circumvent obstacles.

Abstract:Introduction Childhood asthma is a highly prevalent chronic disease. A failure to implement patient education programmes may result in increased morbidity, despite the availability of distinct diagnostic and therapeutic approaches. Patients with lower socioeconomic status (SES) tend to have a higher asthma prevalence. Moreover, the progression of asthma is significantly influenced by factors such as health literacy and the children's specific knowledge about the condition. With this trial, the primary objective is to evaluate whether asthma education enhances specific disease understanding in children with asthma (primary outcome). Secondary objectives include evaluating training effects on health literacy, retention rates of information, 'Children Asthma Control Test' (C-ACT) score, frequency of emergency room and physician visits (secondary outcomes) and whether SES influences training effects. Methods and analysis To address the research objectives, this study comprises two projects. The first subproject will investigate the influence of asthma training on the development of disease understanding and health literacy. The second subproject will analyse the influence of SES on the outcome of children participating in asthma training. This research is designed as a comparative, non-randomised study involving two paediatric groups between the ages of ≥7 and < 14 years. After being diagnosed with asthma, the intervention group undergoes standardised psychoeducational asthma training at a certified centre associated with paediatricians in private practice in Germany, following the recommendations of the 'Arbeitsgruppe Asthmaschulung im Kindes- und Jugendalter e.V.', a national association aiming to establish uniform and guideline-based standards for patient education in children and adolescents. The comparison group receives a significantly shorter period of education and instruction on the usage of asthma medication at outpatient clinics. Data will be collected from patients and their parents at three specific survey time points, based on standardised tools. To describe mean differences between the intervention and control group over time (subproject 1), a repeated-measures analysis of variance (ANOVA) will be conducted. In subproject 2, multivariate linear regression analysis will be used to analyse the variables determining the changes in specific disease understanding and health literacy, including SES. The sample size calculation is based on a mixed ANOVA model with two groups and two measurements resulting in a total of 126 participants. Ethics and dissemination All protocols and a positive ethics approval were obtained from the Witten/Herdecke University, Germany (S-159, 2023; application submission: 24 June 2023, final vote: 10 July 2023). Furthermore, the study was registered at the German Clinical Trials Register (DRKS), DRKS00032423. The application submission was on 3 August 2023, and the final approval was on 4 August 2023. The results will be disseminated among experts and participants and will be published in peer-reviewed, international journal with open access. Trial registration number DRKS00032423.
