Abstract:Objective To improve the safety and feasibility of the intra-hospital transport of critically ill patients.Methods A mobile intensive care unit(MICU) was set up in 2012,equipped with a professional transport team,monitoring and life support instruments,transport process,emergency project and strict quality control system,for transportation of critically ill patients.Results The critically ill patients were successfully transported 214 times.Three patients returned to the intensive care unit because the pulse oxygen saturation was below 90% in the initial stage of transport,and were transported safely later when they were in stable condition.No accidental death or dislocation of artificial airway occurred during transport.Conclusion The MICU provides a continuous monitoring environment for the critically ill patients,reduces the adverse events during transport and ensures the safety of transport.

Abstract:Nuclear Medicine examinations are integral in the management of children with cancer. Imaging with conventional gamma camera systems, hybrid gamma camera systems and radiopharmaceuticals; and with positron emission tomography (PET) and PET/ CT systems and positron emitters can be used for staging, metastatic work-up, surveillance follow-up, therapeutic response assessment and treatment. Tumors that are routinely evaluated include, osteogenic sarcoma, Ewing’s sarcoma, rhabdomyosarcoma, neuroblastoma and lymphoma. Leukemia, brain tumors and thyroid tumors, renal and hepatic tumors may also be imaged using scintigraphic techniques.

Abstract:Neuroimaging in conjunction with a neurologic examination has become a valuable resource for today's intensive care unit (ICU) physicians. Imaging provides critical information during the assessment and ongoing neuromonitoring of patients for toxic-metabolic or structural injury of the brain. A patient's condition can change rapidly, and interventions may require imaging. When making this determination, the benefit must be weighed against possible risks associated with intrahospital transport. The patient's condition is assessed to decide if they are stable enough to leave the ICU for an extended period. Intrahospital transport risks include adverse events related to the physical nature of the transport, the change in the environment, or relocating equipment used to monitor the patient. Adverse events can be categorized as minor (e.g., clinical decompensation) or major (e.g., requiring immediate intervention) and may occur in preparation or during transport. Regardless of the type of event experienced, any intervention during transport impacts the patient and may lead to delayed treatment and disruption of critical care. This review summarizes the commentary on the current literature on the associated risks and provides insight into the costs as well as provider experiences. Approximately, one-third of patients who are transported from the ICU to an imaging suite may experience an adverse event. This creates an additional risk for extending a patient's stay in the ICU. The delay in obtaining imaging can negatively impact the patient's treatment plan and affect long-term outcomes as increased disability or mortality. Disruption of ICU therapy can decrease respiratory function after the patient returns from transport. Because of the complex care team needed for patient transport, the staff time alone can cost $200 or more. New technologies and advancements are needed to reduce patient risk and improve safety.

Abstract:Background Intrahospital transport (IHT) is often performed by nurse anaesthetists and specialist intensive care nurses. Studies have shown that IHT increases the risk of mortality and morbidity, with up to 71% negative incidents. Using checklists when preparing for an IHT is important. Several international guidelines exist to ensure IHT safety and reduce the risk of complications. However, existing guidelines are often problematic in clinical practice. Aim This study aimed to describe the experiences of nurse anaesthetists and specialized intensive care nurses during the IHT of adult patients with critical illnesses. Study Design This study adopted a mixed‐methods approach. Methods Data were collected through a questionnaire completed by 66 nurses with specialist education in anaesthesia or intensive care. The data were analysed with qualitative content analysis, and the quantitative data were analysed with descriptive statistics. Results Two categories with two subcategories each emerged from the analysis of the responses of nurse anaesthetists and specialist intensive care nurses regarding their IHT experiences: creating good circumstances (subcategories: being risk‐conscious and the importance of meticulous preparations) and the importance of routines and education (subcategories: following guidelines and having adequate training). Conclusion IHT was described as a high risk for patient safety and complications. Routines with good compliance and education can positively impact patient safety during IHT. Checklists and scenario training can better prepare nurse anaesthetists and specialist intensive care nurses to manage complications that may arise during IHT, resulting in safer patient care. Relevance for Clinical Practice The findings underscore the importance of written guidelines for IHT, emphasizing awareness and adherence by the entire team. Careful pre‐IHT preparations, coupled with an understanding of potential risks, are vital for ensuring patient safety. Clinical training and discussions following incidents during IHT play a crucial role in raising the collective awareness of patient safety within the entire team.

Abstract:Objective. Multivariate logistic analysis was employed to explore the risk factors of safety risks in the transport of critically ill patients with ICU and the improvement of nursing strategies. Methods. Two hundred critical transport patients with ICU treated in our hospital from January 2019 to April 2021 were enrolled. According to the occurrence of unsafe events in transit, the patients were assigned to the control group (165 cases without unsafe events, n = 165 ) and the study group (35 cases with safety incidents, n = 35 ). Multivariate logistic analysis was employed to explore the risk factors of safety risks in the transport of critically ill patients with ICU and to enhance nursing strategies. Results. (1) General data of the subjects: among the 200 critically ill patients with ICU who needed in-hospital transport, the age ranged from 18 to 85 years with an average age of 52.48 ± 3.31 , including 89 males and 111 females. There were 35 cases of gastrointestinal bleeding, 16 cases of respiratory failure, 23 cases of heart failure, 43 cases of myocardial infarction, 26 cases of cerebrovascular accident, 14 cases of ectopic pregnancy, 25 cases of severe injury, and 18 cases of mechanical ventilation. There were 35 cases in the study group with accidents and 45 cases in group B without accidents. (2) Among the 200 patients, 35 patients had complications during the transit process in the intermediate people's court, with an incidence rate of 17.5%. It included blood pressure fluctuation ( n = 6 ), artificial airway obstruction ( n = 6 ), decrease in blood oxygen saturation ( n = 10 ), dyspnea ( n = 5 ), fall pain ( n = 3 ), elevated intracranial pressure ( n = 2 ), and other factors ( n = 3 ). There exhibited no significant difference in blood oxygen saturation at each time point during transport ( P > 0.05 ). There exhibited no significant difference in SpO2 before transport. The comparison of 5 min and 10 min blood oxygen saturation during transit in the study group was lower compared to the control group ( P < 0.05 ). (3) In a univariate analysis of safety risks for critically ill ICU patients, home escorts did not show significant differences in hospital transport for critically ill ICU patients ( P > 0.05 ). There were significant differences in terms of age, patient's condition, transport escort, auxiliary ventilation, means of transport, uncarried drugs and goods, and carrying pipeline ( P < 0.05 ). The results of multivariate logistic regression analysis indicated that age, patient's condition, transport escort, auxiliary ventilation, means of transport, uncarried drugs and goods, and carrying pipeline were the risk factors affecting the safe transport of critically ill patients ( P < 0.05 </ -Abstract Truncated-

Inter-hospital and intra-hospital patient transfer: Recent concepts

Abstract:The intra- and inter-hospital patient transfer is an important aspect of patient care which is often undertaken to improve upon the existing management of the patient. It may involve transfer of patient within the same facility for any diagnostic procedure or transfer to another facility with more advanced care. The main aim in all such transfers is maintaining the continuity of medical care. As the transfer of sick patient may induce various physiological alterations which may adversely affect the prognosis of the patient, it should be initiated systematically and according to the evidence-based guidelines. The key elements of safe transfer involve decision to transfer and communication, pre-transfer stabilisation and preparation, choosing the appropriate mode of transfer, i.e., land transport or air transport, personnel accompanying the patient, equipment and monitoring required during the transfer, and finally, the documentation and handover of the patient at the receiving facility. These key elements should be followed in each transfer to prevent any adverse events which may severely affect the patient prognosis. The existing international guidelines are evidence based from various professional bodies in developed countries. However, in developing countries like India, with limited infrastructure, these guidelines can be modified accordingly. The most important aspect is implementation of these guidelines in Indian scenario with periodical quality assessments to improve the standard of care.
A quality improvement program of intrahospital transport of critically ill patients from emergency department

Abstract:Objective To improve safety of intrahospital transport of critically ill patients from emergency department.Methods Totally,4899 critically ill patients were divided into three groups chronologically.The control group (n=1651) was given routine transport which was guided by the Modified Early Warning System (MEWS),while the two observation groups (n=1568 and 1680 respectively) received improved transport:a transport team was formed of a doctor,a nurse and a worker,and a comprehensive assessment system was developed,including the MEWS,peripheral oxygen saturation (SpO2) and Glasgow score,to guide the assessment and transport.The observation group 2 additionally received team training.Results Except intravenous infusion extravasation or blockage,the incidence rates of three adverse events were significantly lower,whereas the time for transport was significantly longer,in the two observation groups than those in the control group (P＜0.0125,P＜0.05).Conclusion Using the comprehensive assessment system and conducting continuous improvement can effectively improve the safety of intrahospital transport of critically ill patients from emergency department.
Status of Knowledge, Attitude and Practice of Clinical Nurses Towards the Intrahospital Transport of Critically Ill Patients: A Cross‐sectional Study

Abstract:AimsTo explore the knowledge, attitudes and practice status of the intrahospital transport (IHT) of critically ill patients among clinical nurses and their influencing factors.DesignCross-sectional study.MethodsA questionnaire determined the nurses' knowledge, attitudes and practice scores. The questionnaire was used for data collection in a tertiary hospital from 10 January to 17 January 2023. Multivariate regression analysis was also used to evaluate the related factors of IHT of critically ill patients in different dimensions.ResultsOut of 670 distributed questionnaires, 612 nurses returned the completed questionnaire. The scores of KAP were (9.72 +/- 1.61), (42.91 +/- 4.58) and (82.84 +/- 1.61), respectively. Pearson's correlation analysis showed that knowledge, attitude and behaviour scores were positively correlated. Variables that were associated with the scores of transfer knowledge were the scores of transfer practice, different departments and the scores of transfer attitude. The score of practice, number of IHT and received hospital-level training had statistical significance on the nurses' attitude scores. Furthermore, the score of the attitude and transport knowledge had statistical significance on the nurses' practice.ConclusionThe findings indicate a clear need for clinical nurses' knowledge of IHT of critically ill patients, especially in the emergency department (ED) and ICU. In addition, nurses need to be more active in transporting critically ill patients. Managers should enhance nurses' confidence in the IHT of critically ill patients and promote clinical nurses to establish a correct and positive attitude.ImpactThe findings of this study benefit nursing managers in understanding the current situation of IHT of critically ill patients. Managers should apply new training methods to nursing education and develop a multi-level training program that is systematic, comprehensive and demand-oriented.Patient or Public ContributionThe participants of this study were nurses and this contribution has been explained in the Data collection section. Outcomes After Interhospital Critical Care Transfer

Abstract:Objective: Patients who undergo interhospital transfer, particularly for intensive care unit (ICU) care, experience greater length of stay and mortality. There is evidence that patients transferred for surgical ICU care experience higher mortality rates; however, differences in length of stay or mortality across other ICU types remain unclear. The goals of this work were to assess how length of stay and mortality differ by ICU subspecialties. Methods: We conducted a retrospective analysis of an existing critical care transfer data repository. We used multiple and logistic regression to identify significant factors that contribute to differences in length of stay and mortality for surgical ICU patients. Results: There were no differences in length of stay or mortality based on ICU subspecialty. For every 1-year increase in age, mortality odds increased by 8.6% (P = .002). Patients transferred from an ICU had a longer length of stay by 6.3 days (P < .001). Non-Caucasian patients had a shorter length of stay by 3.4 days (P = .012). Conclusion: Length of stay and mortality are not influenced by ICU subspecialty. Further research is needed to determine the mechanism by which sending unit type and race influence length of stay and identify other factors that predict mortality for SICU patients.
Critical care management and intensive care unit outcomes following cytoreductive surgery with hyperthermic intraperitoneal chemotherapy

Abstract:Aim: To study the early postoperative intensive care unit (ICU) management and complications in the first 2 wk of patients undergoing cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC). Methods: Our study is a retrospective, observational study performed at Icahn School of Medicine at Mount Sinai, quaternary care hospital in New York City. All adult patients who underwent CRS and HIPEC between January 1, 2007 and December 31, 2012 and admitted to ICU postoperatively were studied. Fifty-one patients came to the ICU postoperatively out of 170 who underwent CRS and HIPEC therapy during the study period. Data analysis was performed using descriptive statistics. Results: Of the 170 patients who underwent CRS and HIPEC therapy, 51 (30%) came to the ICU postoperatively. Mean ICU length of stay was 4 d (range 1-60 d) and mean APACHE II score was 15 (range 7-23). Thirty-one/fifty-one (62%) patients developed postoperative complications. Aggressive intraoperative and postoperative fluid resuscitation is required in most patients. Hypovolemia was seen in all patients and median amount of fluids required in the first 48 h was 6 L (range 1-14 L). Thirteen patients (25%) developed postoperative hypotension with seven requiring vasopressor support. The major cause of sepsis was intraabdominal, with 8 (15%) developing anastomotic leaks and 5 (10%) developing intraabdominal abscess. The median survival was 14 mo with 30 d mortality of 4% (2/51) and 90 d mortality of 16% (8/51). One year survival was 56.4% (28/51). Preoperative medical co morbidities, extent of surgical debulking, intraoperative blood losses, amount of intra op blood products required and total operative time are the factors to be considered while deciding ICU vs non ICU admission. Conclusion: Overall, ICU outcomes of this study population are excellent. Triage of these patients should consider preoperative and intraoperative factors. Intensivists should be vigilant to aggressive postop fluid resuscitation, pain control and early detection and management of surgical complications.

Abstract:Introduction and aim The use of hematopoietic stem cell transplant (HSCT) has become the standard of care for many types of hematologic malignancies. Unfortunately, HSCT is often associated with severe complications, including infection, respiratory failure from multiple etiologies, graft versus host disease, bleeding, sepsis, and multi-organ failure. These complications can occur acutely, especially during the period of neutropenia when patients are profoundly immunocompromised or at a later time after hospital discharge. Such complications often require admission to the intensive care unit (ICU) for higher level of care. The reported range of admission to the ICU is between 11% and 40% of all HSCT recipients. In addition to being medically complex and resource consuming, these patients have the additional stigma of dismal outcomes associated with ICU admission. Patients and methods In our tertiary care Oncology centre, we have a dedicated HSCT care unit where post-transplant patients are being cared until any significant deterioration happens and in the event of any such deterioration needing organ support, the patients get admitted in our intensive care unit (ICU). In this retrospective observational study we want to find the outcome of the HSCT patients coming to ICU primarily from the aspect of mortality and secondarily from the aspect of infection prevalence and associated factors like demography, type and duration of transplant, severity of organ support needed, etc. The study period was five years dating from 01/01/2018 to 31/12/2022. Results A total number of 41 post-HSCT patients got admitted to ICU during immediate and late post-transplant phase. They are being analyzed after retrieving data from the records and after obtaining hospital ethics committee permission. Among those only 8 patients could be survived and sent back successfully. However, one interesting finding was an improving (year-wise) trend in survival among these patients. Other important morbidity and mortality data analysis is ongoing. Conclusions There have been limited Indian data on outcome of HSCT patients getting admitted to ICU. Our aim is not only to analyse the outcome of these patients, but also to continue in future to find out any significant association and feasibility of any possible intervention measures.

Abstract:Financial pressures exerted by managed care organizations toward hospitals to improve efficiencies and to lower total healthcare costs continue to force physicians and administrators to reevaluate operations and practices. This shifting of risk exposure from insurers to providers has resulted in many mergers, acquisitions, and affiliations, so as to form integrated health systems that reduce repetition and duplication of services. Therefore, as these integrated systems develop, along with the emergence of tertiary care, regional referral, and specialty hospitals, the need for patient transfers between such facilities will expand. The decision to move patients between facilities is a multicomponent process comprising health, safety, financial, and legal concerns. Interfacility transportation of patients has been performed over the past 20 to 30 years. Whereas ground transport services were prominent in the 1970s, air medical programs using helicopters and fixed-wing aircraft have recently become widespread. Both hospital-based and private agencies have continued to develop programs for efficiently and expeditiously transporting critically ill or injured patients, many requiring complex life-support devices. The Practice Management Committee of the American College of Emergency Physicians recently updated the 1990 policy statement on interfacility transfers, and two position statements are available from the National Association of EMS Physicians on criteria for air medical transport and medical direction for interfacility transport services. This review provides an overview of transportation systems and services available and assists physicians in understanding the various modes and characteristics of systems available. Personnel configurations and capabilities, physiological limitations, inherent requirements for equipment and patient preparation, and legal issues involved with transferring patients are also outlined.
Impact of transfer status on coronary artery bypass graft surgery outcomes

Abstract:Background There is limited research on how patients fare when they are transferred from one acute care center to another acute care facility for further management. Purpose In this study, we explore the short-term, in-hospital outcomes of patients undergoing coronary artery bypass graft (CABG) surgery who were transferred to acute care hospitals from other acute care centers. Methods This was a retrospective cohort study comparing adult patients who were transferred to the hospital from other acute centers with non-transferred hospital admissions between 2016 to 2018. We queried the National Inpatient Sample database to identify patients admitted with a principal procedural diagnosis of CABG. We analyzed the categorical and continuous variables by Pearson's chi-squared and Student t-test respectively. Multivariable logistic regression, adjusted for age, gender, hospital bed size, hospital location and comorbidities, was used to compare mortality. The comorbidities adjusted for included atrial fibrillation, chronic kidney disease, heart failure, hypertension, obesity and type 2 diabetes mellitus. Results 379,195 patients met our inclusion criteria. CABG patients transferred from acute care centers had 1.34 times higher odds of suffering in-hospital mortality compared to regular hospital admissions (aOR 1.34, 95% CI: 1.17–1.52; p<0.001). When separated by race, acute care transfers had higher mortality rates amongst White patients (2.2% vs 1.4%, p<0.001, Figure 1) as well as longer hospital stays (10.9 vs 8.8 days, p<0.001, Table 1). Acute care transfers consisted of more female patients, had higher average APR-DRG mortality scores and lower median household income (Table 1). These patients also had higher rates of comorbidities such as congestive heart failure and chronic kidney disease, but decreased rates of atrial fibrillation, hypertension, obesity, prior myocardial infarction and type 2 diabetes mellitus (Table 1). In terms of operative complications, acute care transfers were more likely to have intraoperative cardiac arrest (0.2% vs 0.1%, p<0.001) and postoperative cardiac insufficiency (0.2% vs 0.1%, p<0.001, Table 1). There was no significant difference between our two cohorts in postoperative complications such as cardiac arrest, intracerebral infarction or heart failure. Conclusion Patients who were transferred to acute care hospitals from other acute care centers to undergo CABG had higher odds of suffering in-hospital mortality compared to regular hospital admissions. These patients were also at increased risk of intraoperative cardiac arrest and postoperative cardiac insufficiency. Our findings illustrate that acute care transfers undergoing CABG require additional medical support in the operative and postoperative setting. While additional studies are required to better understand this relationship, this study highlights the need for innovative clinical change to address these differences moving forward. Proactive risk assessment of intrahospital transport of critically ill patients from emergency department to intensive care unit in a teaching hospital and its implications

Abstract:Aims and objectives: To explore the effects of the health failure mode and effect analysis (HFMEA) used in intrahospital transport (IHT) of critically ill patients from emergency department (ED) to the intensive care unit (ICU). Background: Patients who were transported from ED to ICU is highly critical. IHT of these patients is potentially risky, which may associate with adverse events (AEs). The concern of safe IHT can be addressed by performing proactive risk assessments using HFMEA and implementing the findings after the ED of our hospital being reconstructed. Design: A qualitative action research study combined with a quantitative cross-sectional method. Methods: According to the HFMEA method, the failure modes of IHT were identified and analysed, and the effect of alterations was verified. We built a project team, drawn up a IHT flow chart, defined steps of IHT, classified the failure modes, calculated risk priority number and analysed by the decision tree, then formulated an action plan and verified the effects of the alterations. Incidence of AEs of transport was compared before and after HFMEA.SQUIRE 2.0 checklist was chosen on reporting the study process. Results: The HFMEA outlined a total of 5 major steps and 16 sub-steps in the IHT process. From this, 64 potential failure modes were identified, with 17 modes having a RPN score higher than 8. Determined by the decision tree, there were 20 priority control failure modes, of which 16 involved 8 IHT alterations. Notable work-flow alterations included use of a three-stage hierarchical transport strategy based on patients' condition assisted by the intelligent assessment system. Incidence of AEs was significantly decreased from 19.64% to 7.14% after the implementation of HFMEA (p < 0.05). Conclusion: Application of the HFMEA in optimising IHT process can improve the safety of transportation, which is worthy of promotion. Hierarchical transport scheme can reduce the incidence of AEs in IHT of critical emergency patients, which mainly includes the integration and construction of the transport team, equipment configuration and patient information system based on the classification of patients' condition. Relevance to clinical practice: Nurses play a crucial role in the IHT process. HFMEA can be adopted for proactive risk assessment of critically ill patients' IHT from ED to ICU which involves multiple processes. The IHT hierarchical strategy based on the results of failure mode analysis should be more widely used to further verify its clinical effects.
Experience of Performing Hepatic Interventional Radiological Procedures in a Tertiary Care Hospital in Odisha: A Case Series

Abstract:Introduction: During last few decades, radiological interventions have played crucial role in the management of the patients with chronic liver diseases. Various procedures including transjugualar intrahepatic portosystemic shunt (TIPS), transjugular liver biopsy (TJLB), transarterial chemoembilization (TACE)/transarterial radioembolization (TARE), balloon retrograde transvenous obliteration (BRTO) and plug-assisted retrograde transvenous obliteration (PARTO) are being performed safely and have significantly improved clinical outcomes in these patients. The technical and clinical success depend on appropriate patient selection along with thorough knowledge and experience to perform these procedures. On the other hand, few adverse events may also be associated with these procedures. The intervention radiologist and hepatologists should identify and treat these complications at the earliest so as to improve outcome of the patient. Materials and methods: About 25 hepatic intervention radiology procedures were performed in our center from January 2022 to 2023 May. Among these we have selected five patients who underwent TACE/TIPS/DIPS in our institute. We have selected these cases as in each of these cases we encountered some interesting outcomes/complications which were managed successfully. Results: The first case describes 33-year-old male with POEM syndrome and Budd Chiari Syndrome (BCS) who underwent TIPS and immediately had blockade of the stent. The second case is of a 43 years old male having BCS, refractory ascites with umbilical and inguinal hernia. The third case is of a 40 years old female with decompensated cirrhosis who underwent TIPS for portal hypertensive gastropathy. The fourth case is of a 51-years' female with decompensated cirrhosis with sarcopenia. Finally, the fifth case describes 24-year-old female with BCS and hepatocellular carcinoma. In this article we discuss the procedure and clinical course of the patients following the procedure. Conclusion: Hepatic radiological interventions though widely used can be associated with unusual albeit life threatening complications. Appropriate patient selection and thorough knowledge of procedure along with early diagnosis and management of these complications are key to obtain satisfying long term outcomes.
Characteristics and Outcomes of Intrahospital Transfers from Neonatal Intensive Care to Pediatric Intensive Care Units

Abstract:Objective: Critically ill children may be transferred from the neonatal intensive care unit (NICU) to the pediatric intensive care unit (PICU) for further critical care, but the frequency and outcomes of this patient population are unknown. The aims of this study are to describe the characteristics and outcomes in patients transferred from NICU to PICUs. We hypothesized that a higher-than-expected mortality would be present for patients with respiratory or cardiovascular diagnoses that underwent a NICU to PICU transition and that specific factors (timing of transfer, illness severity, and critical care interventions) are associated with a higher risk of mortality in the cardiovascular group. Study design: Retrospective analysis of Virtual Pediatric Systems, LLC (2011-2019) deidentified cardiovascular and respiratory NICU to PICU subject data. We evaluated demographics, PICU length of stay, procedures, disposition, and mortality scores. Pediatric Index of Mortality 2 (PIM2) score was utilized to determine the standardized mortality ratio (SMR). Results: SMR of 4,547 included subjects (3,607 [79.3%] cardiovascular and 940 [20.7%] respiratory) was 1.795 (95% confidence interval: 1.62-1.97, p < 0.0001). Multivariable logistic regression analysis demonstrated transfer age (cardiovascular: odds ratio, 1.246 [1.10-1.41], p = 0.0005; respiratory: 1.254 [1.07-1.47], p = 0.0046) and PIM2 scores (cardiovascular: 1.404 [1.25-1.58], p < 0.0001; respiratory: 1.353 [1.08-1.70], p = 0.0095) were significantly associated with increased odds of mortality. Conclusion: In this present study, we found that NICU to PICU observed deaths were high and various factors, particularly transfer age, were associated with increased odds of mortality. While the type of patients evaluated in this study likely influenced mortality, further investigation is warranted to determine if transfer timing is also a factor. Abstract:OBJECTIVE: The purpose of this study was to examine the association between intensive care unit nurse (ICU) staffing and the likelihood of complications for patients undergoing abdominal aortic surgery.DESIGN: The study is a retrospective review of hospital discharge data linked to data on ICU organizational characteristics.SETTING: Research took place in ICUs in non-federal, short-stay hospitals in Maryland.PATIENTS: Study included 2606 patients undergoing abdominal aortic surgery in Maryland between January 1994 and December 1996.OUTCOME MEASURES: Outcome measures included cardiac, respiratory, and other complications.RESULTS: Cardiac complications occurred in 13% of patients, respiratory complications occurred in 30%, and other complications occurred in 8% of patients. Multiple logistic regression revealed a statistically significant increased likelihood of respiratory complications (odds ratio [OR], 2.33; 95% confidence interval [CI], 1.50-3.60) in abdominal aortic surgery patients cared for in ICUs with low- versus high-intensity nurse staffing, an increased likelihood of cardiac complications (OR, 1.78; CI, 1.16-2.72) and other complications (OR, 1.74; CI, 1.15-2.63) in ICUs with medium- versus high-intensity nurse staffing, after controlling for patient and organizational characteristics.CONCLUSIONS: Within the range of ICU nurse staffing levels present in Maryland hospitals, decreased nurse staffing was significantly associated with an increased risk of complications in patients undergoing abdominal aortic surgery.

Abstract:Circulation: Cardiovascular Quality and Outcomes, Ahead of Print. BACKGROUND:Wide interhospital variations exist in cardiovascular intensive care unit (CICU) admission practices and the use of critical care restricted therapies (CCRx), but little is known about the differences in patient acuity, CCRx utilization, and the associated outcomes within tertiary centers.METHODS:The Critical Care Cardiology Trials Network is a multicenter registry of tertiary and academic CICUs in the United States and Canada that captured consecutive admissions in 2-month periods between 2017 and 2022. This analysis included 17 843 admissions across 34 sites and compared interhospital tertiles of CCRx (eg, mechanical ventilation, mechanical circulatory support, continuous renal replacement therapy) utilization and its adjusted association with in-hospital survival using logistic regression. The Pratt index was used to quantify patient-related and institutional factors associated with CCRx variability.RESULTS:The median age of the study population was 66 (56–77) years and 37% were female. CCRx was provided to 62.2% (interhospital range of 21.3%–87.1%) of CICU patients. Admissions to CICUs with the highest tertile of CCRx utilization had a greater burden of comorbidities, had more diagnoses of ST–elevation myocardial infarction, cardiac arrest, or cardiogenic shock, and had higher Sequential Organ Failure Assessment scores. The unadjusted in-hospital mortality (median, 12.7%) was 9.6%, 11.1%, and 18.7% in low, intermediate, and high CCRx tertiles, respectively. No clinically meaningful differences in adjusted mortality were observed across tertiles when admissions were stratified by the provision of CCRx. Baseline patient-level variables and institutional differences accounted for 80% and 5.3% of the observed CCRx variability, respectively.CONCLUSIONS:In a large registry of tertiary and academic CICUs, there was a >4-fold interhospital variation in the provision of CCRx that was primarily driven by differences in patient acuity compared with institutional differences. No differences were observed in adjusted mortality between low, intermediate, and high CCRx utilization sites.

Which Method of Transportation Is Associated With Better Outcomes for Patients With Firearm Injuries to the Head and Neck?

Abstract:Background: In firearm injuries (FI), rapid transportation is important for survival. Information regarding different methods of transportation for head and neck FI is limited. Purpose: The purpose of the study was to measure the association between method of transportation and the need for tracheostomy and/or intensive care unit (ICU). Study design, setting, sample: This retrospective cross-sectional study reviewed patients in Trauma Registry at Grady Memorial Hospital (GMH) in Atlanta, Georgia, from January 2016 to June 2021. Patients ≥18 years old who sustained FI to the head and neck and were transported via ground emergency medical services (GEMS) or helicopter emergency medical services (HEMS) were included. Patients who arrived at the hospital by foot, private vehicle, or transported from a different hospital were excluded. Predictor/exposure/independent variable: The primary predictor variable was method of transportation (GEMS: ambulance transportation to GMH vs HEMS: helicopter transportation to GMH helipad). Main outcome variable(s): The primary outcome variables were tracheostomy (yes/no) and ICU admission (yes/no). Covariates: Patient, injury, and hospital-related covariates were collected. Analyses: Univariate analysis, χ2 test for categorical variables, and independent t test for continuous variables were calculated. Statistical significance was P < .05. Results: Of total, 609 patients met the inclusion criteria. There were 560 patients (483 males) with a mean age of 33.6 years old (range, 18 to 90) transported by GEMS. There were 49 patients (40 males) with a mean age of 44 years old (range, 18 to 82) transported by HEMS. Patients transported by HEMS were statistically more likely to have longer transportation time in minutes [13.2 (range, 5 to 132) versus 24.2 (range, 9 to 46), P= <.001], lower Glasgow Coma Scale score [9.9 (range, 3 to 15) versus 6.3 (range, 3 to 15); P= <.001], higher Injury Severity Score [19.3 (range, 3.7 to 98) versus 24.2 (range, 10.3 to 98); P = .007], require transfusion [195 (34.8%); versus 26 (53.1%); P = .013], tracheostomy [46(8.2%) versus 13 (26.5%); P = <.001], and/or admitted to ICU [169, 30.2% versus 24 (49%); P = .007]. Conclusion and relevance: HEMS was positively associated with more tracheostomy and/or ICU admission. Additionally, patients transported by HEMS experienced longer transportation time and severe injuries. HEMS triage criteria specific for FI to the head and neck should be developed.
