Abstract:Six of 56 patients (10.7 per cent) surgically treated for anterior habitual and recurrent dislocation of the shoulder joint exhibited congenital habitual dislocation. Surgery revealed agenesis of the anterior and upper parts of the capsular wall. The lower part of the capsular wall was used for reconstruction.

Abstract:The starting point of the retrospective analysis of 177 primary dislocations was the question of frequency of habitual dislocations, the correlation between habitual dislocation and time of early immobilisation, as well as the search for additional statements which would result in a differentiated therapy. 18.1% habitual dislocations were seen to develop, mostly after immobilisation for less than three weeks. It is an interesting fact that the technique of immobilisation made no difference. We found that after the first even there was generally no loss in function, whereas 14.5% of the men and 34.3% of the women complained of severe subjective symptoms. The 12 patients who underwent an operation because of habitual dislocation suffered neither from dysfunction nor from painful movement, and no dislocation was found. The rate of habitual dislocation in our patients showed a lower rate of dislocation of the shoulder joint. We found 18.1% dislocations, whereas the literature showed an average quota of 30%. To improve the results we recommend a special X-ray technique following the first reposition. In the age group between 20 and 30 years, which is most susceptible, we also recommend arthrography and arthroscopy of the shoulder joint. To improve typical lesions, such as osseous rupture of the capsule or what is known as Hill-Sachs lesions, primary operation is discussed as an alternative treatment.

[Concomitant osseous and ligamentous injuries of traumatic shoulder dislocation and its significance for the pathogenesis of habitual dislocation]

Abstract:161 consecutive patients with traumatic shoulder luxation between 1975 and 1983 are followed, concerning their evolution to recurrent shoulder dislocation. In 26 patients there was a recurrent shoulder dislocation in a mean time of 19 months after the first luxation. In literature the common level of recurrent dislocation is higher than our 16.1%, despite of the shorter immobilising time after the first shoulder luxation in our center. When there is no evolution to recurrent dislocation, a stable shoulder and a normal shoulder function without pain can be expected in 95%. The compression fracture of the humeral head and the avulsion of the glenoid margin were made responsible for the recurrent dislocation, mostly appearing in adult men, younger than 35 (55% of all recurrent dislocations). Because avulsion fractures of the tuberculum majus don't lead to an unstable fracture neither the existence of a compression defect in the humeral head is pathognomonic for an unstable shoulder, nor a lack of such radiological appearance excludes a recurrent dislocation. In first instance rotatory cuff injuries could be responsible for the instability of the shoulder joint. All patients with this invalidating injuries should be stimulated to an operative procedure, because after correction of an unstable shoulder by a derotation osteotomy of Weber or the elevation of the anterior margin of glenoid in the technique of Trillat good results with normal functional capacities of the shoulder can be expected.
Clinical Pathology of Recurrent Anterior Dislocation of Shoulder Joint

Abstract:Objective To comprehensively know clinical pathological change of recurrent anterior dislocation of shoulder joint by arthroscopy. Methods 52 patients with recurrent anterior shoulder dislocation were inspected by arthroscopy in our institute, 38 cases were male, 14 cases were female; the mean age were 24.7, from 16 to 49 years old. 2 epilepsy patients were excluded, the other 50 cases were divided into two groups, the first group included 32 shoulder dislocated patients accompanied with shoulder hyper-laxity. The other group included 18 purely traumatic shoulder dislocated patients. All patients were explored by arthroscopy and all pathological changes were recorded and the difference of pathological change between the two groups were contrasted. Results 51 cases showed anterior-inferior labrum injury, 50 cases showed Hill-Sachs injury, 32 cases showed biceps-tendon inflammation or injury, 30 cases showed bone or cartilage injury of anterior-inferior glenoid, 29 cases showed SLAP injury, 7 cases showed rotator cuff injury. Anterior-inferior labrum injury: labrum abraded disappear was more occurred in pure traumatic group(P=0.055) and ALPSA injury was more in hyper-laxity group (59.4% to 38.9%, P=0.164). Bone or cartilage injury of anterior-inferior glenoid were more occurred in purely traumatic group(P=0.083, P=0.052). Hill-Sachs injury was more superficial or only cartilage injury in hyper-laxity group but wider and deeper in purely traumatic group. Conclusion The general pathological changes of recurrent anterior shoulder dislocation sucessively are: anterior-inferior labrum injury, Hill-Sachs injury, biceps-tendon inflammation or injury, bone or cartilage injury of anterior-inferior glenoid, SLAP injury and rotator cuff injury. The pathological changes of the hyper-laxity group were lighter than purely traumatic group.
High-energy injuries, combined ligamentous injuries, and joint incongruity as predictors of postoperative shoulder dysfunction in AC joint dislocation surgery

Abstract:Journal of International Medical Research, Volume 52, Issue 11, November 2024. ObjectiveTo identify risk factors associated with postoperative shoulder joint dysfunction in patients who underwent surgical intervention for acromioclavicular (AC) joint dislocation, with the aim of enhancing preoperative counselling, surgical planning, and postoperative management to optimize functional outcomes.MethodsPatients who underwent surgery for AC joint dislocation between January 2018 and January 2023 at a hospital orthopaedic centre were enrolled into this retrospective study. Inclusion criteria were patients aged ≥18 years with a documented AC joint dislocation who underwent surgical treatment and had a minimum follow-up period >1 year. Patients with pre-existing shoulder conditions, prior shoulder surgeries, or incomplete medical records were excluded. Visual Analog Scale pain score ≥5 and Constant-Murley Score ≤70 was considered indicative of shoulder joint dysfunction. Statistical analyses included univariate and multivariate logistic regression to identify independent risk factors for postoperative shoulder joint dysfunction.ResultsAmong 208 included patients, 56 (27%) exhibited shoulder joint dysfunction at the final follow-up. High-energy injury mechanisms, combined coracoclavicular ligament injuries, and lack of coronal plane congruity were identified as significant predictors of postoperative dysfunction. Specifically, high-energy injuries (odds ratio [OR] 5.493, 95% confidence interval [CI] 2.068, 14.590), combined coracoclavicular ligament injuries (OR 0.118, 95% CI 0.032, 0.432), and lack of coronal plane congruity (OR 2.540, 95% CI 1.197, 5.387) were associated with an increased risk of poor shoulder function postoperatively. Other factors, such as age, injury mechanism, and postoperative exercise compliance also contributed to the outcomes. Level of evidence: iv.ConclusionsInjury mechanism, ligamentous injuries, and joint congruity are important in determining postoperative shoulder function. Accurate joint reduction and diligent postoperative rehabilitation are crucial for optimal recovery. These findings provide valuable insights for improving patient care and surgical outcomes in AC joint dislocation treatment. Further research is needed to validate these findings and explore additional factors that may influence postoperative shoulder function.

Arthroscopic stabilization of the shoulder for habitual shoulder dislocation

Abstract:Purpose. To analyze the results of treatment in patients with habitual shoulder dislocation using arthroscopic technique. Materials and Methods. The analysis of the results of treatment of 68 patients with habitual shoulder dislocation presented. The patients underwent arthroscopic stabilization of the shoulder using anchoring fixators (Bankart surgery). The features of surgical technique and postoperative rehabilitation highlighted. Results. Short-term results of treatment (up to one year) evaluated in 35 patients, long-term ones – in 11 patients. Positive results observed in all the patients during the nearest follow-up, and in 10 (90.1%) patients during the long-term follow-up. Dislocation recurrence occurred in one case due to sharp adduction and external rotation of the shoulder (work with cable cutters); the dislocation was single, being corrected by the patient independently. Conclusion. This technique is efficient and advisable for treating the shoulder habitual dislocation. Arthroscopic stabilization of the shoulder for its habitual dislocations allows to achieve the shoulder stability recovery with limb function maintenance in full, minimum invasiveness of surgical intervention, and low recurrence.

Arthroscopic Bankart Repair Versus Conservative Treatment for First-Time Traumatic Anterior Shoulder Dislocation: a Systematic Review and Meta-Analysis

Abstract:Background Shoulder is vulnerable to dislocation owing to its anatomical structure and the increasing popularity of contact sports in young population. The management of first-time anterior shoulder dislocation in this group is still controversial and the prognosis are varied. This review aimed to compare the results of arthroscopic Bankart repair and conservative management for first-time traumatic anterior shoulder dislocation in young active patients. Methods Databases were searched till November 2021, and comparative studies between arthroscopic Bankart repair and conservative management for first-time traumatic anterior shoulder dislocation in young population were selected. Methodological quality of the studies was assessed according to the Cochrane Back Review Group 12-item scale. Outcome measures included recurrence of instability, return to play, subsequent instability surgery, and shoulder functional scores. Results The search returned 12 eligible trials with 786 participants. All the trials were of prospective design. After arthroscopic Bankart repair, patients experienced significantly less re-dislocation (7.5% vs. 53.0%, p < 0.00001, I 2 = 0%), subluxation (3.1% vs. 24.2%, p < 0.0001, I 2 = 0%), positive apprehension test (7.3% vs. 25.8%, p = 0.002, I 2 = 11%), and subsequent surgical treatment for instability (5.6% vs. 37.8%, p < 0.00001, I 2 = 0%) when compared with those underwent conservative management. And more patients returned to play (83.5% vs. 66.0%, p = 0.03, I 2 = 81%) after arthroscopic Bankart repair. Outcomes regarding the functional scores did not reach a significant difference between the two cohorts. Conclusions Arthroscopic Bankart repair showed superiority over conservative management in terms of recurrence, return to play, and subsequent instability surgery during the follow-up in young active patients that encountered first episode of dislocation. As long-term prognosis is comparable, an immediate surgical stabilization might not be suitable for everyone.
Bilateral anterior dislocation in the hips: a case report

Abstract:The hip is a stable ball-and-socket joint. Bilateral anterior dislocations of the native hip joints account for fewer than 1% of all dislocations. We present a unique case of a bilateral anterior dislocation in a patient who presented to our institution within 6 hours of trauma. The dislocations were promptly reduced under propofol anesthesia in the operating room. The patient did not suffer a concurrent fracture. After the procedure, we performed regular X-ray examinations for 2 years to rule out the development of avascular necrosis of the head of the femur. The course of the patient was unremarkable.
Prevalence of associated injuries after anterior shoulder dislocation: a prospective study

Abstract:PurposeAnterior shoulder dislocation can be associated with many lesions such as Bankart lesions, rotator cuff tears (RCT), Hill-Sachs lesions or greater tuberosity fractures. It has been documented that early management of the associated injury affords better recovery of shoulder function. The aim of this study was to highlight the incidence of associated injuries after anterior shoulder dislocation.MethodsA total of 240 patients with traumatic anterior glenohumeral dislocations were subjected to complete history taking, neurovascular assessment and pre-reduction plain X-ray. An X-ray was taken immediately after reduction. Ultrasonography (US) and magnetic resonance imaging (MRI) were done within one week after reduction in all patients. Nerve conduction studies were ordered for any patient with suspected nerve injury.ResultsAssociated lesions were reported in 144 (60 %) patients. RCT was the most common injury (67 cases). It was isolated in 34 patients (14.15 %), while it was combined with other lesions in 33 cases (13.75 %). Axillary nerve injury was encountered in 38 patients, of them 8 (3.33 %) were isolated and 30 (12.5 %) were combined. Greater tuberosity fracture was found in 37 patients, of them 15 (6.25 %) were combined with axillary nerve injury, and in the other 22 patients (9.17 %) the fracture was isolated. All cases with Hill-Sachs and Bankart lesion were combined lesions with no isolated cases. There was a significant relation between the incidence of associated injuries and age, mechanism of injury and the affected side.ConclusionsLesions associated with traumatic anterior glenohumeral dislocations are more frequent than expected. Thorough clinical examination and detailed imaging including US and MRI are mandatory to avoid a missed diagnosis.

Risk Factors for Anterior Shoulder Instability: a Matched Case-Control Study

Abstract:Background: Anatomic skeletal features of the shoulder play important roles in anterior shoulder dislocation. However, studies on the effect of the humeral structure are few. This case-control study aimed to analyze the risk factors of anterior shoulder instability, including glenoid and humeral factors. Methods: Anterior shoulder instability was found in 64 of 10,035 individuals who underwent magnetic resonance imaging. Propensity score matching was used to select controls matched for age, sex, height, and weight. We analyzed the glenoid and humeral structural data using conditional logistic regression analysis and identified cutoff points using receiver operating characteristic curve analysis. Results: Significant differences were found between the control and dislocation groups in the depth-to-width ratio (0.119 +/- 0.034 vs. 0.105 +/- 0.037, P = .021), height-to-width ratio (1.51 +/- 0.13 vs. 1.67 +/- 0.16, P < .001), humeral head diameter-to-glenoid fossa diameter ratio (1.56 +/- 0.11 vs. 1.64 +/- 0.20, P < .001), and humeral containing angle (67.3 degrees +/- 5.9 degrees vs. 60.4 degrees +/- 5.9 degrees, P < .001). The humeral containing angle (odds ratio, 0.95; P = .024) and the glenoid height-to-width ratio (odds ratio, 7.88; P = .002), adjusted for the depth-to-width ratio and diameter ratio, were associated with anterior shoulder instability. The cutoff point for the humeral containing angle was 64 degrees and for the height-to-width ratio was 1.60. Conclusions: This study revealed significant risk factors for shoulder instability in the Chinese Han population. The humeral containing angle and the glenoid height-to-width ratio were risk factors for anterior shoulder instability. (C) 2018 Journal of Shoulder and Elbow Surgery Board of Trustees. 5 months old neglected anterior shoulder dislocation treated with open reduction and laterjet procedure: A case report

Abstract:Introduction: Shoulder dislocation is very common. However, neglected or chronic shoulder dislocations are extremely rare. The position of the humeral head determines the classification of shoulder dislocation. Anterior shoulder dislocation accounts for most cases, while inferior dislocation is rare. Negligence from the patient and misdiagnosis are the leading causes of chronic shoulder dislocation. Treatment of the condition poses a significant challenge for surgeons since there is no widely accepted treatment protocol. Case presentation: A 56-year-old woman presented with a neglected anterior subglenoid dislocation of the shoulder joint. The dislocation occurred due to a cow kick, and initial treatment from a traditional healer and an unqualified health practitioner failed. Clinical examination and X-ray confirmed the dislocation without associated fractures. An initial attempt at closed reduction under sedation was unsuccessful. Subsequently, open reduction and a Latarjet procedure were performed successfully, resulting in a satisfactory outcome. Discussion: Chronic anterior dislocation primarily affects the elderly population. Open reduction and fixation procedures have shown satisfactory outcomes. The choice of treatment depends on several factors, including bone deficiencies, soft tissue damage, and the presence of Hill-Sachs or Bankart lesions. Open reduction and laterjet procedure is a validated treatment. However, early physical therapy contributes to favorable outcomes. Conclusion: The condition's rarity makes it difficult to establish a well-accepted treatment protocol. Early and strict rehabilitation protocols will lead to a favorable outcome.

Abstract:Objective To investigate the effect of the arthroscopic procedure on the patients with recurrent anterior shoulder dislocation. Methods From January 2001 to March 2006, 52 patients with recurrent anterior shoulder dislocation were treated by arthroscopy. Among them 44 patients were followed up for 12 to 54 months(on average of 26 months). The data of the 44 patients was reviewed. Three evaluation systems,University of California at Los Angeles Shoulder Scores(UCLA),Simple Shoulder Test(SST) and Dawson,were used. The study evaluated the effect based on the Dawson system by the factors as:age,course of the disease, frequency of dislocation and relocation methods and the range of shoulder movement.Results The ratio of recurrent dislocation after operation was 4.5%. Assessing through 3 evaluation systems,UCLA,SST and Dawson,results were similar:the follow-up evaluation were extraordinarily different from preoperative assessment,and the rating of good or excellent at the time of the final follow-up reached 91% higher. Based on the Dawson system, the evaluation results had no statistic diversity according to such factors as:age, course of the disease, frequency of dislocation and relocation methods. Conclusions The effects of arthroscopic surgery to the recurrent anterior shoulder dislocation are satisfied and evidenceproved. It is a good option for both common patients and athletes.
Inferior capsular shift operation for multidirectional instability of the shoulder in players of contact sports

Abstract:OBJECTIVES: To assess the results of inferior capsular shift for multidirectional instability of the shoulder in athletes.METHODS: Multidirectional instability was surgically corrected in 53 shoulders in 47 athletes who engaged in contact sports. A history of major trauma was found in eight patients, the others having had minor episodes. Before surgery, all patients had complex combinations of instabilities. The surgical approach was selected according to the predominant direction of instability.RESULTS: Anterior inferior capsular shift was carried out in 37 shoulders, and anterior dislocation recurred in three. In one of these, it was anterior alone, one was anterior and inferior, and one was unstable in all three directions. After posterior inferior capsular shift in 16 shoulders, one dislocation occurred anteriorly and one posteriorly. With the anterior approach, four athletes could not return to sport. Two patients treated with the posterior approach could not return to sport. Of these six failures, five patients had had bilateral repairs. Successful repair based on the criteria of the American Shoulder and Elbow Association was achieved in 92% of anterior repairs and 81% of posterior repairs. Successful return to sport was noted in 82% of patients with anterior repairs, 75% with posterior repairs, and 17% with bilateral repairs. Overall, there were five subsequent dislocations, three in the anterior repair group (8%), and two in the posterior repair group (12%).CONCLUSIONS: Inferior capsular shift can successfully correct multidirectional instability in most players of contact sports, but the results in bilateral cases are poor.

Acute irreducible anterior shoulder dislocation due to interposition of the subscapularis muscle and the lesser tuberosity: a case report

Abstract:Efforts to reduce an anterior shoulder dislocation can fail due to numerous mechanical obstructions caused by soft tissue interposition (long head of the biceps, rotator cuff muscles, labrum, musculocutaneous nerve) and/or bony elements (displaced fragment of a greater tuberosity or glenoid fracture, bone impaction such as a Hill-Sachs lesion fixed on the glenoid rim, a bony Bankart lesion). Herein, we report the case of a 35-year-old man who sustained an anterior shoulder fracture-dislocation of his left shoulder after a fall. Despite a postreduction radiological examination that appeared misleadingly reassuring, subtle signs of persistent subluxation raised concerns. A computed tomography (CT) scan revealed subscapularis muscle entrapment along with avulsion of its bony insertion from the lesser tuberosity of the humerus, and a comminuted avulsion fracture of the greater tuberosity of the humerus. The patient underwent surgery using a deltopectoral approach. This involved releasing the entrapped subscapularis muscle and fixing the two fractured fragments. The lesser tuberosity was reduced and secured with two cannulated screws, and the comminuted fragment of the greater tuberosity was reattached using transosseous sutures. At 12-month follow-up, the patient achieved a Constant-Murley score of 85 of 100, with limitation in internal rotation at L3 but no signs of instability or new dislocation episode. This case underscores the importance of confirming shoulder reduction on at least two orthogonal views and paying close attention to the patient's feedback about sensation in their shoulder. Additionally, it highlights the utility of CT or magnetic resonance imaging scans if doubt exists about the integrity of the reduction.
Epidemiological and Clinical Characteristics of Shoulder Dislocations in Emergency Care Settings in Turkey: A Single-Center Experience

Abstract:Objective: This study evaluated the general epidemiological and clinical characteristics of patients admitted to the emergency department with shoulder dislocations. Methods: This is a retrospective cross-sectional study, conducted as a chart review at the Emergency Department of the *** State Hospital between 01 January 2018 to 31 December 2019. Results: A total of 165 patients (median age 50 years and 60% of males) with anterior shoulder dislocations were included. The most common mechanism was traumatic dislocation (65.5%), the primary maneuver of treatment was traction/countertraction (80.6%), and 22.4% were recurrent dislocations. Males were younger and had more spontaneous dislocations. Also, patients with spontaneous dislocations were younger and predominantly males, and tended to be recurrent dislocations than traumatic ones. Conclusions: Epidemiological characteristics of our patients were similar to a previous study conducted in Turkey, but slightly different from the studies in other populations. Further studies evaluating the incidence and long-term follow-up data are also needed to better elucidate the epidemiological features of shoulder dislocations.
Arthroscopic Bankart repair for the acute anterior shoulder dislocation: systematic review and meta-analysis

Abstract:BackgroundThe ideal treatment of acute anterior shoulder dislocation remains one of the topics that spark debate over the value of primary repair for the first-time anterior shoulder dislocation. The high rate of complications especially in young adults, such as recurrent instability, residual pain, and inability to return to sports, has led to the quest for an ideal management of such injuries.ObjectiveIn this meta-analysis, we compare between the immediate arthroscopic repair and conservative treatment of primary anterior shoulder dislocation as well as arthroscopic reconstruction of recurrent anterior shoulder dislocation. Outcome measures were failure rate (dislocation, subluxation, and instability) and revision rates.MethodsFollowing the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, we searched several database including PubMed, MEDLINE, EMBASE, Web of Science, Cochrane Library, and ClinicalTrials.gov through August 2017 to identify observational and experimental randomized controlled trials comparing the outcomes of immediate arthroscopic repair and conservative treatment of primary shoulder dislocations as well as arthroscopic reconstruction of recurrent dislocation. The primary outcome was failure rate, whereas, secondary outcomes were revision rate and functional outcomes.ResultsOut of the 710 studies identified, 12 were eligible for meta-analysis. The estimated pooled failure rate was 13.7% (7.7%–19.6%), whereas, the pooled revision rate was 7.1% (3.8%–10.4%) in immediate arthroscopic repair of primary shoulder dislocation. The odd ratio (OR) of failure and revision rates were significantly lower in arthroscopic repair of primary shoulder dislocation compared to conservative treatment (OR 0.103, 95% CI [0.052, 0.201]), (OR 0.217, 95% CI [0.078, 0.607]), respectively. The odd ratio (OD) of failure and revision rates were lower in arthroscopic repair of primary shoulder dislocation compared to arthroscopic reconstruction of the recurrent shoulder dislocation; however, the difference was statistically insignificant (OR 0.423, 95% CI [0.117, 1.522]) and (OR 0.358, 95% CI [0.044, 2.920]) respectively.ConclusionThe outcome of immediate arthroscopic repair of primary anterior shoulder dislocation is superior and encouraging with significant reduction in failure and revision rates compared to conservative treatment. Nevertheless, the failure and revision rates are statistically insignificant compared to arthroscopic reconstruction of recurrent dislocation. Hence, there is evidence to support immediate arthroscopic repair option for primary anterior shoulder dislocations over conservative treatment in young active patients, in order to reduce the risk of failure and revision rates. However, the evidence is inconclusive compared to arthroscopic reconstruction of recurrent dislocation.

Clinical Effect Observation of Surgical Treatment of 245 Patients with Habitual Shoulder Dislocation

Abstract:Objective To observe the effect of different surgical methods used in the treatment of habitual shoulder dislocation.Methods During May 1988 and November 2009,we performed Putti-Patt,Weber,Bristow and arthroscopy Bankart repair for 245 patients with habitual dislocation of the shoulder.The postoperative follow-up ranged from 6 months to 4 years.The clinical effect of both groups was observed.Results The excellent rate of Putti-Patt,Weber,Bristow and arthroscopy Bankart surgery were 71.1%,85.3%,80.3% and 95.2% respectively.Conclusion Arthroscopy Bankart repair has the advantages of minimal invasion,optimal and fast functional recovery of the shoulder.It will be used more extensively,but more attention should be paid to surgical indication of arthroscopy Bankart repair.

Abstract:Hypothesis: The purpose of this study is to identify and compare demographic, clinical, historical, and intraoperative variables in patients who have received arthroscopic treatment for single vs. multiple anterior shoulder dislocations. Methods: This is a retrospective chart review of patients who underwent arthroscopic labral repair of the shoulder by six surgeons at a single institution between 2012 and 2020. Patients with a documented anterior shoulder dislocation were included. Patients with pain-only, subluxation-only, multidirectional or posterior instability, and prior shoulder surgeries of any kind were excluded. Studied variables included age, sex, laterality, body mass index, contact/collision sports, Charlson comorbidity index, tobacco use, number of dislocations (1, >1), labral tear size, time from first dislocation to surgery, anchor number, and concomitant procedures. Study groups were compared using student's t-tests and Mann-Whitney U test for continuous variables and chi-square or Fisher's exact tests for discrete variables with a significance of 0.05. Results: Six hundred thirty-three patients were identified, and 351 (85 single dislocators [SDs], 266 multiple dislocators [MDs]) met inclusion criteria (mean age: 27 years; range: 14-71 years). There were no demographic differences between the study groups. SD received surgery significantly sooner at 17 ± 44 months after injury, while MD received surgery 53 ± 74 months postinitial dislocation. SDs (30/85, 35%) were significantly more likely than MDs (56/266, 21%) to receive concomitant posterior labrum repair. MDs (46/266, 17%) were significantly more likely than SDs (5/85, 6%) to receive a remplissage. SDs (11/85, 13%) were significantly more likely than MDs (11/266, 4%) to receive a concomitant biceps tenotomy/tenodesis. There were no other significant differences in injury or surgery characteristics. Conclusion: MDs will have more time between their initial dislocation and arthroscopic labral repair and are more likely to receive a remplissage procedure, yet they are less likely than SDs to receive a concomitant posterior labral repair or biceps tenodesis/tenotomy despite no differences in age, sex, and activity level. Whether the greater extent of labrum injury in SD is due to a more severe initial injury vs. earlier recognition and intervention requires further study.

Abstract:Background: The risk of recurrence after the first episode of anterior shoulder dislocation is high with nonoperative treatment in younger patients. Purpose/Hypothesis: The aim of this study was to compare the results of arthroscopic Bankart repair and nonoperative treatment for shoulder dislocation in patients younger than 25 years, with a minimum of 2 years of follow-up. The hypothesis was that surgery would decrease the risk of recurrence. Study design: Randomized controlled trial; Level of evidence, 1. Methods: We included patients aged between 18 and 25 years after a first episode of anterior shoulder dislocation and divided them into 2 groups. The first group was treated surgically with an arthroscopic Bankart repair within 2 weeks after the dislocation; the second group was treated nonoperatively. Both groups were immobilized for 3 weeks in internal rotation and followed the same physical therapy protocol. Standard radiography and computed tomography were performed immediately after reduction of the dislocation, and follow-up was performed at 3, 6, 12, and 24 months. The primary outcome measure was instability recurrence, defined as another anterior shoulder dislocation requiring closed reduction by another person (the patient was unable to reduce the dislocated joint themselves), a subluxation, or a positive apprehension test. Secondary outcome measures included range of motion, return to sport, and functional scores such as the short version of the Disabilities of the Arm, Shoulder and Hand score the Walch-Duplay score, and the Western Ontario Shoulder Instability Index (WOSI). Results: A total of 20 patients were included in each group. The mean ± SD age was 21 ± 1.8 years, and there were 33 men (82.5%) and 7 women (17.5%) in the total sample. Recurrence of instability was significantly decreased in the surgical treatment group compared with the nonoperative group (2 [10%] vs 14 [70%], respectively; P = .0001). Fewer patients in the surgical treatment group versus the nonoperative group had another episode of dislocation (0 vs 6 [30%], respectively), subluxation (2 [10%] vs 13 [65%], respectively; P = .003), or a positive apprehension test (1 [5%] vs 11 [58%], respectively; P = .0005). The Walch-Duplay score (88.4 vs 70.3 points; P = .046) and WOSI (11.5 vs 17.7 points; P = .035) were significantly better in the surgical group versus the nonoperative group after a 2-year follow-up. Level of sport was the same or better in 89% of the surgical treatment group vs 53% of the nonoperative treatment group ( P = .012). No surgical complication was recorded. We did not find any significant difference in range of motion. Conclusion: In patients with first-time shoulder dislocations, arthroscopic labral repair (Bankart procedure) reduced the risk of secondary shoulder dislocation and improved functional outcome versus nonoperative treatment after a 2-year follow-up. Surgical treatment after a first episode of shoulder dislocation could be offered as a primary treatment option in a younger population if these results are confirmed by larger studies with a longer follow-up. Abstract:Objective To study the cause,treatment and prognosis of recurrent shoulder joint dislocation among soldiers.Methods Clinical data of 27 uniformed patients diagnosed as recurrent shoulder joint dislocation and admitted to the hospital between August 1st,2000 and August 1st,2012 were retrospectively reviewed,and patients sticking to follow-up visit were assessed by Rowe shoulder assessment and Oxford Shoulder Instability Score(OSIS).Results Twenty-five patients received operation.21 patients were followed-up.The following up period ranged from 6-72 months with an average of(46.7 ± 3.76) months.And 15 patients recovered to pre-injury activity levels after operation,3 had a little restricted activity in extorsion and upthrow,2 had recurrent dislocation and 1 suffered from shoulder pain after the procedure.Significant improvement was detected by Rowe score(preop.38.1 ± 14.8 vs postop.85.8 ± 14.6,P < 0.01),with an increase in good and excellent score rating of 76.1%.OSIS was also satisfying(preop.42.7 ±6.4 vs postop.19.7 ±5.8,P <0.01).Conclusion Comprehensive diagnosis after initial injury,adequate fixation,appropriate selection of surgical approach and systematic postoperative rehabilitation training are the key links in prevention and treatment of recurrent shoulder joint dislocation among soldiers.
