Abstract:Background The potential impact of diabetes mellitus type 1 (DM1) on male fertility is currently poorly defined. Hyperglycaemia and insulin deficiency may affect spermatogenesis. Some evidence suggests that men with DM1 have a significant reduction in progressive sperm motility, sperm morphology and semen volume, without significant changes in sperm concentration and count, but definite data are lacking. Objectives To evaluate the impact of DM1 on clinical parameters related to male fertility and semen analysis. Materials and methods We compared a court of 42 male DM1 patients with 43 nondiabetic subjects overlapping in age and remaining clinical data in an observational case‐control study. All subjects underwent a comprehensive andrological reproductive evaluation, including medical history, physical examination, and semen analysis. We collected biochemical data in all patients with DM1, while diabetic patients with any alteration in semen parameters underwent sperm culture and scrotal ultrasound. In addition, all men completed the IIEF‐5 questionnaire (International Index of Erectile Function‐5) and the AMS (Aging Male Symptom score) questionnaire. Results Patients with DM1 had a higher prevalence of infertility, erectile dysfunction and worse semen parameters compared with controls. Abstract:Although the prevalence of sub-infertility in diabetic patients in childbearing age is known, the mechanisms by which diabetes mellitus (DM) causes male infertility are not completely explained. This detrimental effect is achieved with a variety of mechanisms that include pre-testicular, testicular, and post-testicular pathogenetic moments and can be different in type 1 diabetes mellitus (DM1) and type 2 diabetes mellitus (DM2) patients because of type of diabetes, duration of disease, and glycemic metabolic compensation. Aim of this study was to evaluate whether diabetic disease can be considered a risk factor for infertility considering the etiopathogenetic differences between DM1 and DM2 on sperm function. We enrolled 38 DM1 patients and 55 DM2 patients with idiopathic infertility history >12 months, and 100 healthy fertile subjects. The following outcomes were evaluated in optical microscopy and flow cytometry: sperm function (by conventional and biofunctional sperm parameters) and signs of urogenital infection/inflammation (by sperm leukocyte concentrations and indices of oxidative stress). Moreover, an andrological evaluation (by didymo-epididymal ultrasound evaluation, serum total testosterone, LH, and FSH measurements) was performed in DM1 and DM2 patients compared to controls. Diabetic patients showed a higher risk of becoming infertile and the pathophysiological mechanisms of damage were different in DM1 and DM2. Conventional sperm parameters of diabetic patients are worse than controls (<i>p</i> < 0.05). The DM2 caused an inflammatory condition with increased oxidative stress resulting in decreased sperm vitality and increased sperm DNA fragmentation. DM1 altered epididymal voiding causing low ejaculate volume and mitochondrial damage resulting in decreased sperm motility. These findings and evidences support the contention that DM could be regarded as cause of male infertility suggesting that the prevention of diabetic disease in DM2 and the follow-up of seminal parameters in DM1 could prevent fertility decline in these categories of patients.

Abstract:The perceptual effects of orthogonal variations in two acoustic parameters which differentiate American English prevocalic /r/ and /l/ were examined. A spectral cue (frequency onset and transition of F2 and F3) and a temporal cue (relative duration of initial steady state and transition of F1) were varied in synthetic versions of "rock" and "lock." Four temporal variations in each of ten stimuli of a spectral-cue continuum were generated. Phonetic identification and oddity discrimination tasks with the four series showed systematic displacement of perceptual boundaries and discrimination peaks, thus reflecting a trading relation between the two cues. The perceptual equivalence of spectral and temporal cues was investigated by comparing the accuracy of discrimination of three types of stimulus comparisons: phonetically facilitating two-cue pairs, one-cue pairs, and phonetically conflicting two-cue pairs. As predicted, discrimination accuracy was ordered: Facilitating cues greater than one-cue greater than conflicting cues, indicating that perceivers discriminated on the basis of an integrated phonetic percept.
Effects of Diabetes Mellitus on sperm quality and fertility outcomes: clinical evidence

Abstract:Background Diabetes mellitus (DM) is a global epidemic characterized by high morbidity and mortality. DM can lead to acute and chronic systemic complications. Among them, a negative effect of DM on sperm quality and male/couple fertility has been suggested. However, available studies in DM men evaluated relatively small cohorts with discordant results. Objectives To evaluate the clinical evidences of the effects of DM on sperm quality and fertility outcomes. Methods An extensive Medline search was performed identifying studies in the English language. Results The prevalence of DM in infertile men ranges from 0.7% to 1.4%, while the prevalence of infertility in DM men, evaluated in a few studies, ranges from 35% to 51%. Male DM seems to play a negative effect on couple fecundity, while being childless or subfertile men might increase the risk of DM. Available cross‐sectional studies investigating semen parameters and male sexual hormones in men with DM are heterogeneous, assessed relatively small cohorts, show often discordant results and frequently are not directly comparable to derive robust conclusions. Two meta‐analyses support a negative effect of DM on sperm normal morphology and no effect on sperm total count, with contradictory results regarding other semen parameters. Considering only studies on type 1 DM (T1DM) men, meta‐analyses support a negative effect of DM on sperm motility and no effect on sperm total count, with contradictory results regarding other semen parameters. The rate of children observed among T1DM men was lower than controls, especially in subjects with a longer DM duration. Couples with a DM male partner undergoing assisted reproduction techniques showed lower pregnancy rates than controls. No study evaluated the impact of DM treatment on semen quality and male fertility. Conclusions Overall, available data show that DM might impair male reproductive health and couple fertility. Abstract:The effects of diabetes mellitus include long-term damages, dysfunctions, and failures of various organs. An important complication of diabetes is the disturbance in the male reproductive system. Glucose metabolism is an important event in spermatogenesis. Moreover, glucose metabolism is also important for maintaining basic cell activity, as well as specific functions, such as motility and fertilization ability in mature sperm. Diabetic disease and experimentally induced diabetes both demonstrated that either type 1 diabetes or type 2 diabetes could have detrimental effects on male fertility, especially on sperm quality, such as sperm motility, sperm DNA integrity, and ingredients of seminal plasma. Epigenetic modifications are essential during spermatogenesis. The epigenetic regulation represents chromatin modifications including DNA methylation, histone modifications, remodeling of nucleosomes and the higher-order chromatin reorganization and noncoding RNAs. If spermatogenesis is affected during the critical developmental window, embryonic gonadal development, and germline differentiation, environmentally-induced epigenetic modifications may become permanent in the germ line epigenome and have a potential impact on subsequent generations through epigenetic transgenerational inheritance. Diabetes may influence the epigenetic modification during sperm spermatogenesis and that these epigenetic dysregulation may be inherited through the male germ line and passed onto more than one generation, which in turn may increase the risk of diabetes in offspring.

Abstract:Abstract Purpose The aim of the present study is to evaluate the association of metabolic and glycemic variables with semen parameters in patients with type 1 diabetes (T1D) with and without erectile dysfunction (ED). Methods The study population included 88 adults with T1D using a continuous glucose monitoring, of whom 28 with ED (ED group) and 60 without it (NO ED group). All men completed the International Index of Erectile Function (IIEF-5) and underwent body composition analysis (BIA) and semen analysis. Results ED group showed worse HbA1c levels [median (IQR), 8.4 (7.7, 9.9) vs 7.4 (7, 8.2) %, P < 0.001)], higher insulin dose [60 (51, 65) vs 45 (38, 56) UI/die, P = 0.004)] and a higher total body water and intracellular water as compared with ED group. Men in the ED group presented higher semen volume [2.8 (2.6, 4.2) vs 2.5 (2.2, 2.7) mL, P < 0.001] and sperm concentration [24 (19, 29) vs 20 (12, 23) mil/mL, P = 0.010], but reduced sperm progressive motility [28 (25, 35) vs 35 (25, 36) %, P = 0.011], higher rate of non-progressive motility [15 (10, 15) vs 10 (5, 10) %, P < 0.001] and higher rate of typical morphology [7(5, 8) vs 5 (4, 5) %, P = 0.001]. Based on multivariate logistic regression analysis performed to assess the association between clinical variables and ED, intracellular water (OR 3.829, 95% CI 1.205, 12.163, P = 0.023) resulted as the only independent predictor of ED. Conclusion Men with T1D and ED showed worse metabolic profile which is associated with poor semen quality, as compared with those without ED.

Abstract:A new agar culture system to clone specifically acute nonlymphocytic leukemia (ANLL) is described, which consists of two phases: an initial liquid phase in the presence of PHA-stimulated lymphocyte conditioned medium (PHA-LCM) and a semi-solid phase. By preincubation of leukemic cells with PHA-LCM in a liquid phase, leukemic cells were found to be more sensitive to forming colonies and clusters in agar medium. In addition, this PHA-two step assay could eliminate colony formation by T cell progenitors. The leukemic origin of the colonies was proven by electron-microscopic analysis, which demonstrated the presence of nuclear blebs and abnormal perinuclear fibril formation. Morphological studies also indicated myeloid differentiation of the cells in the leukemic colonies, although considerable variation was observed among patients. The wide range of linearity between the number of cells plated and the number of colonies grown permits quantitative assay of colony-forming leukemic cells (L-CFU). This assay should be valuable for studies of chemotherapy, growth regulation, and differentiation of L-CFU.

Abstract:Couple infertility is a common condition, defined as being unable to conceive after 12 months of regular unprotected sexual intercourse. Male Factor Infertility (MFI) is responsible, alone or in combination with female factors, for about half of the overall cases of couple infertility. MFI is gradually increasing in prevalence, with a notable decline in semen parameters over the last decades. The aetiologies behind the finding of decreasing sperm counts are difficult to pinpoint but might be due in part to increasing rates of overweight and obesity in men of childbearing age. Diabetes mellitus (DM) is a common and chronic metabolic disease, whose prevalence is also gradually increasing, rising up to 10% of the population. The International Diabetes Federation estimates that there are currently more than 500 million people living with DM worldwide, the vast majority of whom suffering from type 2 DM (T2DM). There is growing awareness of the relationship between unhealthy lifestyle, in particular unhealthy diet, and MFI. Starting from all these premises, the aim of this narrative review is to describe the current evidence on the link between DM and MFI, both in terms of DM as a cause of/a risk factor for MFI and of MFI as a possible predictive marker for T2DM. Finally, we will discuss the risk of DM as a consequence of the therapy of MFI or assisted reproductive techniques.

Abstract:Abstract The effects of mental and physical daytime activities upon REM sleep cycle (REM cycle) during 1 night was studied in five university students (aged 19–25 years). Mental activity with high tension has effects upon the timing of REM sleep periods in the later part of the night. Physical activity has effects upon the timing of REM sleep in the early part of the night. The result suggests that mental and/or physical activities during daytime modulate REM cycle during the night.
Correlation between histological findings on punch biopsy specimens and subsequent excision specimens in cutaneous squamous cell carcinoma.

Abstract:Diagnosis and subsequent treatment of cutaneous squamous cell carcinoma are frequently based on punch biopsies. Regarding the current TNM classification and stage grouping for cutaneous squamous cell carcinoma, it is important to identify the high-risk features (infiltration depth > 4 mm, perineural and/or lymphovascular invasion and poor differentiation). This study investigates the agreement of histological high-risk features and TNM grouping stage on 3 mm punch biopsies and subsequent surgical excision in 105 patients diagnosed with cutaneous squamous cell carcinoma. On punch biopsy, infiltration depth > 4 mm is not identified in 83.3% (30/36), perineural invasion in 90.9% (10/11) and poor differentiation in 85.7% (6/7) of cases. The TNM stage was underestimated on punch biopsy in 15.4% (16/104). This study shows that on a 3 mm punch biopsy, high-risk features in cSCC can remain undetected and that the actual TNM stage is not identified in 1 out of 6 tumours.

Abstract:Abstract Background Chronic hyperglycemia is a characteristic of diabetes mellitus (DM). Long-lasting hyperglycemia can generate oxidative stress and reactive oxygen species. The effect of this condition on sperm quality and spermatogenesis leads to male infertility and reproductive dysfunction. Objectives To investigate changes in sperm quality, morphology of testicular structure, and stage of development of seminiferous tubules in a streptozotocin (STZ)-induced rat model of type 1 DM. Methods We divided 15 male Sprague Dawley rats into 2 groups. DM was induced in 7 rats using STZ (60 mg/kg intraperitoneally), while the other 8 were treated with citrate buffer as a vehicle control group. Rat semen was collected for quality measurements including motility, normal morphology, and concentration. Morphological changes in testicular structure and stage of development of seminiferous tubules were investigated by histology with hematoxylin and eosin (HE) staining. Results Significant decreases in all parameters of sperm quality and testicular weight were found in rats with induced DM. Moreover, abnormal morphology of seminiferous tubules including separation of the germinal epithelium, vacuolization, luminal sloughing of germ cells, and tubular atrophy was increased significantly in these rats, while the proportion of their seminiferous tubules at an early stage of development was significantly higher, but was dramatically decreased in the late stage of development when compared with that in vehicle-treated control rats. Conclusions DM has adverse effects on sperm quality, testicular structure, and development of seminiferous tubules. These findings may reflect the male infertility and reproductive dysfunction seen in patients with type 1 DM.

Abstract:The alarming increase in the number of diabetic patients worldwide raises concerns regarding the impact of the disease on global health, not to mention on social and economic aspects. Furthermore, the association of this complex metabolic disorder with male reproductive impairment is worrying, mainly due to the increasing chances that young individuals, at the apex of their reproductive window, could be affected by the disease, further contributing to the disturbing decline in male fertility worldwide. The cornerstone of diabetes management is glycemic control, proven to be effective in avoiding, minimizing or preventing the appearance or development of disease-related complications. Nonetheless, the possible impact of these therapeutic interventions on male reproductive function is essentially unexplored. To address this issue, we have made a critical assessment of the literature on the effects of several antidiabetic drugs on male reproductive function. While the crucial role of insulin is clear, as shown by the recovery of reproductive impairments in insulin-deficient individuals after treatment, the same clearly does not apply to other antidiabetic strategies. In fact, there is an abundance of controversial reports, possibly related to the various study designs, experimental models and compounds used, which include biguanides, sulfonylureas, meglitinides, thiazolidinediones/glitazones, bile acid sequestrants, amylin mimetics, as well as sodiumglucose co-transporter 2 (SGLT2) inhibitors, glucagon-like peptide 1 (GLP1), α-glucosidase inhibitors and dipeptidyl peptidase 4 (DPP4) inhibitors. These aspects constitute the focus of the current review.

Abstract:World Health Organization has stated Infertility as a medical condition in which an individual has the inability to conceive even after one year of regular unprotected active sexual intercourse. Presently, Infertility has been a major condition, which is multifactorial in nature and is supposed to have been affected approximately about 15% to 20% of the couples who are desperate for pregnancy. The contributory factors for male infertility may be of different factors comprising of complex pathogenesis, change in lifestyle, other organic diseases, genetic factors, environmental risk factors and their interactions. The patients with subnormal semen parameters are advised for blood hormonal analysis tests as these hormonal tests are more reliable and valid in the further proper management of the infertility [11]. These hormonal tests include Follicle Stimulation Hormone (FSH), Testosterone, Luteinizing Hormone (LH), Prolactin. This cross section study was done in 100 males, 50 diabetic infertile men and 50 non-diabetic fertile men. Semen samples were obtained by masturbation after 2–7 days of sexual abstinence. The Semen Samples were analyzed as per the instructions given in the WHO manual for semen analysis.

Abstract:Background: The production of specific IgE, which underlies the allergic response, may be a normal correlate of the immune response to a certain class of antigen (allergens), or could represent a unique response driven by regulatory signals that are absent in non‐allergic individuals. If atopic subjects do possess a regulatory environment favoring IgE production, they may display not only allergen‐specific IgE, but also higher levels of total IgE and higher frequencies of IgE‐producing B lymphocytes.

Abstract:The prevalence of type 1 diabetes mellitus has increased significantly among women of reproductive age over the past two decades. Despite improved glycemic control and intensified insulin therapy, patients with diabetes still suffer from many reproductive problems, which often makes this group of patients potential participants in assisted reproductive technology programs under certain conditions. Diabetic nephropathy is one of the most serious complications of type 1 diabetes mellitus. It ranks first in the structure of chronic kidney disease and is a common cause of end-stage renal failure, disability, and mortality. Early diagnosis and identification of specific markers of diabetic nephropathy will allow for timely initiation of nephroprotective therapy to slow the progression of diabetic kidney damage. This review article is based on the results of the PubMed, Frontiers, and ResearchGate search queries from 2016 to 2023. We analyzed worldwide and domestic data on the impact of type 1 diabetes mellitus on kidney function, the influence of sex hormones on diabetic nephropathy, and the importance of the personalized approach to this group of patients at the pre-pregnancy stage, especially those planning treatment within assisted reproductive technology programs.

Abstract:Diabetes mellitus is a global epidemic contributing to the rising male infertility rates. Addressing testicular dysfunction in diabetic patients requires a multimodal strategy encompassing medication, lifestyle changes, early diagnosis, and innovative treatments targeting specific biochemical pathways. This review explores the mechanisms of diabetes-induced testicular dysfunction and potential intervention targets. A comprehensive literature search was conducted using PubMed, Science Direct, Google Scholar, and Web of Science with keywords related to diabetes and testicular dysfunction. Diabetes leads to reduced testosterone synthesis, decreased spermatogenesis, increased germ cell apoptosis, and damage to Leydig and Sertoli cells. Mechanisms involved in the pathogenesis of diabetes-induced testicular dysfunction include: hyperglycaemia oxidative stress, inflammation, apoptosis and disrupted hormone levels among others. Targeting biomolecular regulators involved in the pathogenic pathways offers a promising therapeutic avenue. Additionally, exploring plant-based therapies as alternative treatments shows potential in alleviating testicular dysfunction in diabetes. Implementing a comprehensive approach combining diagnostics, pharmacological interventions, and lifestyle modifications is crucial in managing testicular dysfunction in diabetic individuals. Future research directions suggest the need for large-scale clinical trials, personalized medicine strategies, and innovative technologies to address and mitigate testicular dysfunction in diabetic populations effectively.

Abstract:It is well known that both type 1 and type 2 diabetes mellitus (DM) are related to increased risk for cardiovascular (CV) and chronic kidney disease (CKD). However, besides these prominently presented complications, DM has also been associated with reproductive dysfunctions. It seems that these disorders are met in up to 40% of women with DM and consist of delayed menarche, all types of menstrual disorders, such as amenorrhea, oligomenorrhea, menstrual irregularity, as well as menorrhagia, infertility, characteristics of polycystic ovary syndrome (PCOS) and early (or rarely late) menopause. In type 1 DM (T1DM), insulin treatment, although it has reduced the rates of insulinopenic-induced hypogonadotropic hypogonadism, an entity commonly presented in many women with the disease in the past decades, when it is used in excess it can also promote hyperandrogenism. Regarding type 2 DM (T2DM), insulin resistance (IR) and hyperinsulinemia have mainly been implicated in the pathogenesis of reproductive dysfunctions, as insulin can act as gonadotropin on the theca cells of the ovary and can lead to hyperandrogenism and inhibition of proper ovulation. This review aims to detail the reproductive dysfunctions associated with DM and provide scientific data to enlighten the underlying pathogenetic mechanisms.

Type 1 diabetes impairs the activity of rat testicular somatic and germ cells through NRF2/NLRP3 pathway-mediated oxidative stress

Abstract:Background: It is well known that metabolic disorders, including type 1 diabetes (T1D), are often associated with reduced male fertility, mainly increasing oxidative stress and impairing the hypothalamus–pituitary–testis (HPT) axis, with consequently altered spermatogenesis and reduced sperm parameters. Herein, using a rat model of T1D obtained by treatment with streptozotocin (STZ), we analyzed several parameters of testicular activity. Methods: A total of 10 adult male Wistar rats were divided into two groups of five: control and T1D, obtained with a single intraperitoneal injection of STZ. After 3 months, the rats were anesthetized and sacrificed; one testis was stored at -80°C for biochemical analysis, and the other was fixed for histological and immunofluorescence analysis. Results: The data confirmed that T1D induced oxidative stress and, consequently, alterations in both testicular somatic and germ cells. This aspect was highlighted by enhanced apoptosis, altered steroidogenesis and Leydig cell maturity, and impaired spermatogenesis. In addition, the blood–testis barrier integrity was compromised, as shown by the reduced levels of structural proteins (N-cadherin, ZO-1, occludin, connexin 43, and VANGL2) and the phosphorylation status of regulative kinases (Src and FAK). Mechanistically, the dysregulation of the SIRT1/NRF2/MAPKs signaling pathways was proven, particularly the reduced nuclear translocation of NRF2, affecting its ability to induce the transcription of genes encoding for antioxidant enzymes. Finally, the stimulation of testicular inflammation and pyroptosis was also confirmed, as highlighted by the increased levels of some markers, such as NF-κB and NLRP3. Conclusion: The combined data allowed us to confirm that T1D has detrimental effects on rat testicular activity. Moreover, a better comprehension of the molecular mechanisms underlying the association between metabolic disorders and male fertility could help to identify novel targets to prevent and treat fertility disorders related to T1D.

Assessment of ovarian reserve in patients with type 1 diabetes: a systematic review and meta-analysis

Abstract:Purpose Current knowledge about the ovarian reserve in patients with type 1 diabetes is inconsistent and based on studies with small sample size. This meta-analysis aimed to produce a comprehensive evaluation on the ovarian reserve of type 1 diabetes female patients and to analyze the associated factors with the ovarian reserve. Methods Systematic searches were conducted for studies published from the inception to December 2021. Original human observational studies either with case-control, cross-sectional, or longitudinal design evaluating ovarian reserve markers between type 1 diabetes patients and healthy controls were included. Levels of anti-müllerian hormone (AMH), follicle-stimulating hormone (FSH), and estradiol (E2) were extracted. Results It was indicated that women with type 1 diabetes were associated with decreased levels of AMH compared with healthy controls (weighted mean difference [WMD] −0.70 ng/ml, 95% confidence intervals [CI] −1.05 to −0.34 ng/ml, P = 0.0001). Subgroup analyses stratified by age showed that adult patients with type 1 diabetes were associated with decreased levels of AMH (WMD −0.70 ng/ml, 95% CI −1.06 to −0.34 ng/ml, P = 0.0001) and FSH (WMD −1.07 IU/L, 95% CI −1.75 to −0.39 IU/L, P = 0.002) compared with healthy controls. Meta-regression analysis showed no significant correlation between AMH, FSH, and clinical factors, while level of E2 was negatively correlated with daily insulin doses and glycosylated hemoglobin A1c (HbA1c) values. Conclusion According to this meta-analysis, type 1 diabetes might be associated with decreased AMH levels. Further studies using different markers and fertility outcomes focus on the ovarian reserve of women with type 1 diabetes are urgently needed.
