Abstract:BackgroundIn China, the reported incidence of workplace violence in hospitals has been increasing. Most research has focused on secondary and tertiary care facilities. We aimed to determine the incidence of workplace violence and its aftermath in county and township hospitals and community health centres in Zhejiang Province.MethodsA cross-sectional survey using stratified cluster sampling was carried out at 18 hospitals (eight county hospitals and ten township hospitals or community health centres) in Zhejiang Province from July 27, 2016 to Jan 20, 2017. Questionnaires were distributed to all health workers (including physicians, nurses, medical technicians, and administrative staff) present on the day of the survey. Ethical approval was obtained from Zhejiang University School of Public Health.Findings2018 out of 2303 participants completed questionnaires from 596 men and 1417 women. In the past year, 140 (7%) respondents had experienced physical attacks and 384 (19%) physical threats. Men experienced around twice as many physical attacks (59 [10%] vs 80 [6%] of women; p=0·001), and significantly more threats than females (132 [22%] vs 252 [18%]; p=0·025). Doctors were most likely to be victims of physical attacks, and the commonest location was the emergency department. Most respondents did not report the physical attack (117 [85%] respondents), mainly because they regarded it as pointless (70 [52%] respondents) or didn't know who to report to (32 [24%] respondents). Of all those attacked or threatened, 312 (61%) wanted to quit direct patient care. In terms of impact, 413 [80%] victims said they had repeated, disturbing memories of the attack, 498 [97%] said they had become highly vigilant, and 386 [75%] were fearful dealing with difficult cases. Only four [3%] were satisfied with the way the incident was handled.InterpretationWorkplace violence is common at lower-level health facilities. The lack of a formal response to workplace violence implies an institutional tolerance which is of great concern. Safety During Night Duty: Survey of 3885 Doctors Across India

Abstract:Abstract: Background: Reports of violence against doctors at the workplace are on the rise. In August 2024, a young female doctor was raped and murdered during night duty at her workplace in Kolkata, India. This incident prompted nationwide protests and a service shutdown by doctors' organizations advocating for improved workplace safety. Aim: This survey was undertaken by the Indian Medical Association to evaluate safety concerns during night shifts among doctors. With 3,885 individual responses, it is the largest study from India on this topic. Methods: An online survey was sent to doctors across India through a Google form. There were 3885 responses within 24 hours. In addition to seeking suggestions to improve safety, three separate parameters were assessed: 1. 0-10 numeric rating scale of perception of safety 2. Availability of duty room 3. Access to bathroom while on night duty Results: 1. Respondents were from several states. 85% were under 35 years. 61% were interns or postgraduate trainees. Women constituted 63%, aligning with the gender ratio in some MBBS courses. 2. Several doctors reported feeling unsafe (24.1%) or very unsafe (11.4%), totalling one-third of the respondents. The proportion of those feeling unsafe was higher among women. 3. A duty room was not available to 45% of respondents during night shifts. 4. Those with access to a duty room had greater sense of safety. 5. Duty rooms were often inadequate due to overcrowding, lack of privacy and missing locks, forcing doctors to find alternative rest areas. 6. One-third of available duty rooms did not have an attached bathroom. 7. In more than half the instances (53%), duty room was located far from the ward/casualty area. 8. Suggestions to enhance safety included increasing the number of trained security personnel, installing CCTV cameras, ensuring proper lighting, implementing the Central Protection Act (CPA), restricting bystander numbers, installing alarm systems, and providing basic amenities such as secure duty rooms with locks. For detailed information, refer to Table 13, the synopsis and the verbatim comments section. Conclusions: Doctors across the country, particularly women, report feeling unsafe during night shifts. There is substantial scope for improving security personnel and equipment in healthcare settings. Modifications to infrastructure are essential to ensure safe, clean, and accessible duty rooms, bathrooms, food, and drinking water. Adequate staffing, effective triaging, and crowd control in patient care areas are also necessary to ensure that doctors can provide the required attention to each patient without feeling threatened by their work environment. Workplace Violence and Its Aftermath in China's Health Sector: Implications from a Cross-Sectional Survey Across Three Tiers of the Health System

Abstract:Objectives To determine the prevalence of physical violence and threats against health workers and the aftermath in tertiary, secondary and primary care facilities in China. Design A cross-sectional questionnaire study. Setting 5 tertiary hospitals, 8 secondary hospitals and 32 primary care facilities located in both urban and rural areas of Zhejiang Province, China, were chosen as the study sites. Participants A total of 4862 health workers who have contact with patients completed a survey from July 2016 to July 2017. Outcome measures The prevalence of physical violence, threats and Yi Nao, specific forms of physical violence and their aftermath were measured by a self-designed and verified questionnaire. Multivariable logistic regression models were used to examine the association between perceived organisational encouragement of reporting workplace violence (WPV) and physical violence, threats and Yi Nao after controlling for age, sex, level of facility, professional ranking and type of health worker. Results Among all respondents, 224 (4.6%) were physically attacked and 848 (17.4%) experienced threats in the past year. Respondents in secondary hospitals were more likely to experience physical violence (AOR=3.29, 95%CI 2.21 to 4.89), threats (AOR=1.61, 95%CI 1.32 to 1.98) and Yi Nao (AOR=2.47, 95%CI 2.10 to 2.91), compared with primary care providers. Lack of organisational policies to report WPV was associated with higher likelihood of physical violence (AOR=3.64, 95%CI 2.57 to 5.18) and threats (AOR=2.21, 95%CI 1.76 to 2.78). Among physical violence cases, only 29.1% reported the attack to police mainly because most felt it useless to do so (58.8%). Only 25.7% were investigated and 72.4% of attackers received no punishment. Of all those attacked or threatened, 59.4% wanted to quit current post and 76.0% were fearful of dealing with urgent or severe cases. Conclusions Proper management of the aftermath of violence against health workers is inadequate. Formal guidelines for reporting and managing WPV are urgently needed.
Safety measures to prevent workplace violence in emergency primary care centres--a cross-sectional study

Abstract:Background: Employees in emergency primary care centres (EPCC) have raised personal safety as an issue. Despite a high risk of experiencing workplace violence at EPCCs in Norway, knowledge regarding applied preventive measures is limited. The description of existing safety measures is an important prerequisite to evaluate and make guidelines for the improvement of preventive practices on a national level. The objective of this study was to investigate to which extent general practitioners work alone in EPCCs in Norway, and to estimate the prevalence of other preventive measures against workplace violence. Methods: A survey was sent to the managers of all 210 registered EPCCs in Norway. The questionnaire included 22 items on safety measures, including available staff, architecture and outfitting of the reception and consulting rooms, and the availability of electronic safety systems and training or monitoring systems. The data were analysed using descriptive statistics. Differences between EPCCs staffed by one general practitioner alone and EPCCs with more health personnel on duty were explored. Results: Sixty-one (30%) of the 203 participating EPCCs had more than one person on duty round-the-clock. These EPCCs reported the application of a significantly higher number of safety measures compared to the EPCCs with only one general practitioner on duty during some or part of the 24 hours. Examples of safety measures being more common in highly staffed EPCCs were automatic door locks (p < 0.001), arrangement of furniture in the consulting room ensuring that the patient is not seated between the clinician and the exit (p = 0.014), the possibility of bringing an extra person on emergency call-outs or home visits when needed for security reasons (p = 0.014), and having organised training regarding violence (p < 0.001). Conclusion: This study shows considerable differences between Norwegian EPCCs regarding applied preventive measures, and a higher prevalence of such measures in EPCCs staffed with several health personnel around-the-clock. More research is needed to understand the reasons for, and the effects of, these differences.

Abstract:Since the 1930's the scientific literature on cow's milk protein allergy (CMPA) has accumulated. Over the last decade new diagnostic tools and treatment approaches have been developed. The diagnosis of reproducible adverse reactions to cow's milk proteins (CMP), i.e. CMPA, still has to be confirmed by controlled elimination and challenge procedures. Advanced diagnostic testing using epitope and microarray technology may in the future improve the diagnostic accuracy of CMPA by determination of specific IgE against specific allergen components of cow's milk protein. The incidence of CMPA in early childhood is approximately 2-3% in developed countries. Symptoms suggestive of CMPA may be encountered in 5-15% of infants emphasizing the importance of controlled elimination/milk challenge procedures. Reproducible clinical reactions to CMP in human milk have been reported in 0.5% of breastfed infants. Most infants with CMPA develop symptoms before 1 month of age, often within 1 week after inter introduction of CMP-based formula. The majority has two or more symptoms from two or more organ systems. Approximately 50-70% have cutaneous symptoms, 50-60% gastrointestinal symptoms and 20-30% respiratory symptoms. Symptoms may occur within 1 hour after milk intake (immediate reactions) or after 1 hour (late reactions). The prognosis of CMPA is good with a remission rate of approximately 45 to 50% at 1 year, 60 to 75% at 2 years and 85 to 90% at 3 years. Associated adverse reactions to other foods develop in up to 50% and allergy against inhalants in 50 to 80%. The basic treatment of CMPA is avoidance of CMP. In early childhood a milk substitute is needed. Documented extensively hydrolysed formulas are recommended, whereas partially hydrolysed formulas should not be used because of a high degree of antigenicity and allergenicity associated with adverse reactions. In case of intolerance to extensively hydrolysed formulas and multiple food allergies a formula based on aminoacids is recommended. Alternative milk substitutes such as sheep's and goat's milk should not be used because of a high degree of cross reactivity with CMP. Milk from other mammals such as mare and donkey may be tolerated by some children with CMPA. Soy protein is as allergenic as CMP and soy formula is not recommended for young children with CMPA because of a great risk of development of allergy to soy, whereas soymilk is normally tolerated in older children with CMPA. Recent treatment modalities are oral immunotherapy (OIT) involving the ingestion of increasing amounts of milk allergen on a regular basis to desensitize and potentially permanently tolerize patients to CMP. OIT can increase the reaction thresholds to CMP, but questions about safety and long-term efficacy remain. Anti-IgE therapy with Omalizumab may improve the safety and efficacy of OIT and may provide benefit in monotherapy.

Abstract:As doctors, we find it disturbing to read the paper "Burnout in physicians who are exposed to workplace violence" (January 2020).1 Workplace violence experienced by doctors has been documented both in developed and developing countries. Reports show that Chinese medical professionals are being injured, disabled, or even killed by patients or their family members.2,3 The killing of a doctor in Beijing in 2019 attracted national attention. China's top legislative body has approved the first fundamental and comprehensive law to protect health-care professionals. The bill will take effect on June 1, 2020. However, the law alone will not solve the problem. We need to treat it from the root and address the issues that have led to this health-care-related violence. A multi-faceted action must be launched to reduce violence against doctors in China urgently.

Abstract:The mean serum calcium of 13 hyperthyroid patients was found to be significantly higher than that of controls matched for sex and age, though none of the patients' values were outside the normal range. Nevertheless, these patients responded very promptly to hypercalcaemia (induced by an intravenous calcium load), and their serum calcium returned to normal much more rapidly compared with the matched controls. There was also increased retention of intravenous calcium load, possibly owing to increased calcitonin production. Calcium infusion may be useful in treating bone diseases in which increased bone resorption exceeds bone accretion.
Management of Domestic Violence by Primary Care Physicians in Hong Kong: Association With Barriers, Attitudes, Training, and Practice Background

Abstract:While primary care physicians (PCPs) have opportunities to recognize patients who encounter domestic violence (DV) by intimate partners, warning symptoms such as unexplained physical injury, bruising, anxiety, and depression are often missed during clinic visits. This study investigated the barriers of Hong Kong PCPs toward managing DV, including recognition, management, and referrals of these patients. Four focus group interviews were conducted to explore the in-depth opinions of PCPs on managing DV in Hong Kong. The themes identified were investigated in a questionnaire survey with data from 504 PCPs working in public and private sectors. Factor analysis of the survey data suggested four major barrier factors: (a) worries about the potential harms of intervening in patients’ domestic affairs and DV issues, (b) lack of guidelines and support services, (c) limited skills and time in managing DV, and (d) patients’ reluctance in disclosing DV issues. PCPs with more years of practice had more worries about intervening in domestic issues (Factor 1), while the younger PCPs tended to perceive limited skills and time in managing DV cases as barriers (Factor 3). PCPs working in the public setting were more likely to ask patients about DV ( M = 5.4 vs. 2.9), suspected DV ( M = 3.8 vs. 2.3), and managed DV ( M = 1.7 vs. 1.0) in the past 5 years compared with the private PCPs. Some PCPs in the focus groups expressed the view that DV cases should be handled by social workers instead of doctors. Survey respondents who held such views about doctors’ limited role in DV management also scored higher in all of the barrier factors. The findings suggest that there are boundaries between domestic and medical realms, especially in a Chinese context. Resolving PCPs’ worries by training, provision of DV management guidelines, and referral pathway to social workers may be good starting points to bridge the gaps.

Abstract:A cross sectional survey carried out in three local emergency departments revealed that workplace violence remained a significant concern with a high prevalence rate of 66.7% and low reporting rate. Introduction Workplace violence affects the dignity and well‐being of healthcare workers and poses threats to the efficiency and success of the healthcare system. Healthcare workers in the emergency department (ED) are known to be vulnerable to workplace violence. This study explored the prevalence of workplace violence in the ED in Hong Kong and factors associated with self‐reported workplace violence. Methods An anonymous, voluntary, self‐administered and web‐based cross‐sectional survey was carried out from 1 June to 30 June 2022 among doctors and nurses in three regional EDs. Multivariable logistic regression analyses were conducted to examine the factors associated with workplace violence. Results A total of 162 healthcare workers including 57 doctors and 105 nurses participated in the study, with an overall response rate of 50.8%. The overall prevalence of workplace violence was 66.7%. Verbal abuse (64.2%) was more common than physical violence (20.4%). Nursing group was significantly correlated with physical violence (aOR 7.27, p = 0.005). Reporting rate of workplace violence remained low (18.2% for physical violence and 12.5% for verbal abuse). More than half of the participants with experience of workplace violence reported that the incident affected their mental well‐being. Conclusions Workplace violence was a significant concern in the local ED and under‐reporting was not uncommon. Impact of workplace violence on healthcare workers and the healthcare system should be well recognised. Workplace violence protocol and prevention strategies are crucial.

Abstract:Aggression and violence by patient (and their relatives/friends) is widely acknowledged as a serious occupational hazard, with physicians being particularly susceptible to witnessing and experiencing such incidents within hospitals. Research has shown that the negative consequences of such aggression and violence are not only felt at the individual level, but also at the team and organizational levels. Understanding how to prevent and manage this behavior towards physicians in hospitals is urgent and not fully researched. While there are many potentially effective interventions, it is unclear which ones would be valuable and feasible for Chinese hospitals. Because patient aggression and violence may occur more frequently in Chinese hospitals than in other countries, this suggests that cultural differences play a role and that tailored interventions may be needed.

[Study on the molecular-epidemiological characteristics of HIV-1 in Shenzhen, 1992-2008].

Abstract:OBJECTIVE To investigate the epidemiological characteristics of HIV-1 subtype in Shenzhen from 1992 to 2008. METHODS 489 HIV-1 positive plasma samples were collected from 1992 to 2008 in Shenzhen. HIV-1 env genes were amplified by nested-PCR from RNA. Phylogenetic analysis was performed on data regarding the nucleotide sequence. RESULTS A total of 464 sequences were amplified and genotyped. Data from this study revealed that CRF01_AE was a predominant HIV-1 subtype in Shenzhen (64.4%, 299/464), followed by subtypes CRF_BC (17.5%, 81/464), B' (14.7%, 68/464) and B (2.4%, 11/464). Subtype C (0.4%, 2/464), A1 (0.2%, 1/464), CRF02_AG (0.2%, 1/464) and CRF06_cpx (0.2%, 1/464) were also prevalent in Shenzhen. CRF01_AE and CRF_BC were predominant among heterosexuals, homosexuals and injection drug users, while B' was predominant among blood donors. Results from phylogenetic tree analysis showed that some of the HIV-1 clusters had been defined in CRF01_AE strains at different time or groups with different transmission routes. Cross-infections were also seen. CONCLUSION CRF01_AE was the predominant HIV-1 subtype in Shenzhen while CRF_BC, B, B', C, A1, CRF02_AG and a small amount of CRF06_cpx or recombinant subtypes were prevalent in this city. Different subtypes showed great variation in the process of epidemics.
Prevalence of bullying, discrimination and sexual harassment among trainees and Fellows of the College of Intensive Care Medicine of Australia and New Zealand

Abstract:Background: Anecdotal reports about bullying behaviour in intensive care emerged during College of Intensive Care Medicine (CICM) hospital accreditation visits. Bullying, discrimination and sexual harassment (BDSH) in the medical profession, particularly in surgery, were widely reported in the media recently. This prompted the College to formally survey its Fellows and trainees to identify the prevalence of these behaviours in the intensive care workplace. Methods: An online survey of all trainees (n = 951) and Fellows (n = 970) of the CICM. Results: The survey response rate was 51% (Fellows, 60%; trainees, 41%). The overall prevalences of bullying, discrimination and sexual harassment were 32%, 12% and 3%, respectively. The proportions of Fellows and trainees who reported being bullied and discriminated against were similar across all age groups. Women reported a greater prevalence of sexual harassment (odds ratio [OR], 2.97 [95% CI, 1.35-6.51]; P = 0.006) and discrimination (OR, 2.10 [95% CI, 1.39-3.17]; P = 0.0004) than men. Respondents who obtained their primary medical qualification in Asia or Africa appeared to have been at increased risk of discrimination (OR, 1.88 [95% CI, 1.15-3.05]; P = 0.03). Respondents who obtained their degree in Australia, New Zealand or Hong Kong may have been at increased risk of being bullied. In all three domains of unprofessional behaviour, the perpetrators were predominantly consultants (70% overall), and the highest proportion of these was ICU consultants. Conclusions: The occurrence of BDSH appears to be common in the intensive care environment in Australia and New Zealand.
Understanding workplace violence against medical staff in China: a retrospective review of publicly available reports

Abstract:Background: Workplace violence against medical staff in China is a widespread problem that has negative impacts on medical service delivery. The study aimed to contribute to the prevention of workplace violence against medical staff in China by identifying patterns of workplace violence, key risk factors, and the interplay of risk factors that result in workplace violence. Methods: Ninety-seven publicly reported Chinese healthcare violent incidents from late 2013 to 2017 were retrospectively collected from the internet and analysed using content analysis. A modified socio-ecological model guided analysis of the violent incidents focusing on risk. Results: Physical violence, yinao, or a combination of physical and verbal violence were the typical forms of violence reported. The findings identified risk at all levels. Individual level risk factors included service users' unreasonable expectations, limited health literacy, mistrust towards medical staff, and inadequacy of medical staff's communication during the medical encounter. Organisational level risk factors under the purview of hospital management included problems with job design and service provision system, inadequacies with environmental design, security measures, and violence response mechanisms within hospitals. Societal level risk factors included lack of established medical dispute-handling mechanisms, problems in legislation, lack of trust and basic health literacy among service users. Situational level risks were contingent on risk factors on the other levels: individual, organisational, and societal. Conclusions: Interventions at individual, situational, organisational, and societal levels are needed to systematically address workplace violence against medical staff in China. Specifically, improving health literacy can empower patients, increase trust in medical staff and lead to more positive user experiences. Organizational-level interventions include improving human resource management and service delivery systems, as well as providing training on de-escalation and violence response for medical staff. Addressing risks at the societal level through legislative changes and health reforms is also necessary to ensure medical staff safety and improve medical care in China.

Violence and discrimination are common occurrences in the workplace, especially in health services. Workplace violence has detrimental health impacts and consequences for health care workers. However, this problem is still very little reported so it is largely ignored and unexplored due to the lack of comprehensive data and studies regarding this incident. 2 The study conducted by Faizul Haris Mohd Hatta et al used QAWRF, an instrument that has three components consisting of QAWRF-Administrator, QAWRF-Workers, and QAWRF-Clients and has undergone content validation, face validation, and internal consistency reliability tests. 1 The instrument used is appropriate to the objectives to be achieved, but we would like to recommend a 9-item health survey measure whose overall question is "Have you experienced the following things in the last 12 months at work?", noting that the answers may vary by nine different aspects and specific forms or types of violence and discrimination in the workplace, namely, five items about violence and aggression (verbal violence, threats and insults, physical violence, intimidation or bullying, sexual harassment) and four items about discrimination (treatment that is detrimental and unfair because of age, gender, disability and nationality, ethnicity or skin color). 3 In this research, Faizul Haris Mohd Hatta et al obtained the results that QAWRF Administrators, QAWRF Workers, and QAWRF Clients had acceptable factor loadings, absolute suitability and additional suitability so that QAWRF could identify specific WPV risk factors that were considered prevalent by local stakeholders. 1 Therefore outreach is needed to increase awareness of workplace violence, training of doctors and residents in using workplace violence prevention programs to deal with WPV, video recording, enacting more laws to protect doctors, increasing guards security, increasing staff numbers, limiting visitor access to hospital departments, and a violence reporting system. 4 In conclusion we agree that the QAWRF has good construct validity and reliability that can identify WPV risk factors thereby contributing to data-based, worksite-specific, and targeted WPV interventions in healthcare settings. 1 We therefore recommend that mandatory in-service education on workplace violence should be introduced to all health service staff. The content of the program includes communication skills, service psychology (understanding client needs), service behavior (how to appropriately respond to client needs and personality enhancement) and safety training (handling aggression and de-escalating hostile situations). 5 All authors report no conflicts of interest in this communication. 1. Abstract:Intolerance and grouse against doctors is a global phenomenon but India seems to lead the world in violence against doctors. According to World Health Organization, about 8-38% healthcare workers suffer physical violence at some point in their careers. Many more are verbally abused or threatened. Public is almost behaving like health sector terrorists. The spate of increasing attacks on doctors by damaging their property and causing physical injury is not acceptable by any civilized society. The public is becoming increasingly intolerant to a large number of social issues because of poor governance and vote bank politics. There is a need to arrest the development of further distrust between doctors and their patients/relatives, otherwise it will compromise all achievements of medical science and adversely affect healing capabilities of doctors. Rude and aggressive behavior of the patients or their family members, and arrogant and lackadaisical approach of the doctor, adversely affects the doctor-patient relationship and the outcome of the patient. The doctors, hospital administration and government must exercise "zero tolerance" with respect to acts of violence against healthcare professionals. It is possible to reduce the incidence of intolerance against doctors but difficult to eliminate it completely. The healthcare providers should demonstrate greater compassion and empathy with improved communication skills. The hospitals must have adequate infrastructure, facilities and staff to handle emergencies without delay and with due confidence and skills. The security of healthcare providers, especially in sensitive areas, should be improved by having adequate number of security guards, frisking facilities, extensive CCTV network and availability of "Quick response team" to handle unruly mob. In case of any grievances for alleged mismanagement, the public should handle the situation in a civilized manner and seek redressal through Medical Protection Act and legal avenues. Laws to prevent violence against doctors do exist but they need to be made more stringent and implemented properly.

Abstract:Aim To identify best practices to prevent violence against healthcare workers by patients at risk for aggression in the adult inpatient setting. Design An integrative review. Methods Conducted using the Johns Hopkins Evidence‐based Practice for Nurses and Healthcare Professionals Model. Title and abstract screening on 4186 articles resulted in 156 for full text review. Full text screening yielded 14 articles that met inclusion criteria. Data Sources A search of the databases PubMed, CINAHL, Embase, and JBI from January 2019 to February 2023. Results The review revealed behavioural intervention teams, environmental changes, and coordinated communication plans were the most used strategies, however none demonstrated significant decreases in violence. Conclusions Health systems can implement strategies shown to decrease the incidence of violence in healthcare settings globally. Lack of consistency in the evidence suggests the need for further research to assess mitigating strategies for violence against healthcare workers in inpatient hospital settings. Implications for the Profession and/or Patient Care Patient safety is a cornerstone of nursing practice; however, healthcare workers need to feel safe in their work environment. Violent events are chronically underreported, ill defined, and when reported, do not address change in the practice setting. Identifying strategies to address escalating behaviour before it results in violence is crucial for everyone's safety. Impact This integrative review exposes the scarcity of evidence available to address rising concerns about patients on healthcare provider violence (Type II) in the workplace. Although several assessment tools for identifying violent patients exist, evidence regarding prevention is woefully absent. The review highlights potential interventions for further study to equip healthcare workers to manage patients safely and effectively before an escalation occurs. Prevention of violence in healthcare settings: Australian experience and perspective for China

Abstract:Objective To explore Australian experience of preventing violence in healthcare settings and to discuss on improving Chinese hospital measures including violence reporting,risk screening and management. Methods Literature review and case studies. Results Most violence happened in emergency department and mental/psychiatric services in Australia. Based on report of Australian Incident Monitoring System,causes of violence could be classified as patient,staff and system reasons.Violence in hospital has been recognised as part of nature of medical professional practice,and it is unacceptable as violence both negatively affect clinicians and health consumers. Staff training is found for improving knowledge and skill of preventing violence and protecting medical staff. A risk screening and management tool is used in hospital,which roles as first prevention strategy for safety of staff and patients. Conclusion Violence in Chinese hospital is increasing along with recent health policy implementation for health sector reform and development. Attribution of violence was likely pointed to social and political aspects,such as lack of trust,unsatisfied social harmony,increased social inequality and weakness of social security. Contribution of patients and their families has been under estimated,especially when ‘patient-centred health care’ encouraged.In addition,as lack of information on violence reporting and analysis and weak of mental health service in China,patient/family’s attack under psychiatric reasons are unknown.It is recommended that an information system for incidence monitoring is crucial for evidence-based planning of violence prevention and protection. The nature of violence then could be further discovered.Furthermore,Chinese hospital managers could develop their tool of risk screening and management.
At the crossroads of violence and aggression in the emergency department: perspectives of Australian emergency nurses

Abstract:Objective: Violence is widespread in Australian emergency departments (ED) and most prevalent at triage. The aim of the present study was to identify the causes and common acts of violence in the ED perceived by three distinct groups of nurses. Methods: The Delphi technique is a method for consensus-building. In the present study a three-phase Delphi technique was used to identify and compare what nurse unit managers, triage and non-triage nurses believe is the prevalence and nature of violence and aggression in the ED. Results: Long waiting times, drugs and alcohol all contributed to ED violence. Triage nurses also indicated that ED staff, including security staff and the triage nurses themselves, can contribute to violence. Improved communication at triage and support from management to follow up episodes of violence were suggested as strategies to reduce violence in the ED CONCLUSION :There is no single solution for the management of ED violence. Needs and strategies vary because people in the waiting room have differing needs to those inside the ED. Participants agreed that the introduction and enforcement of a zero tolerance policy, including support from managers to follow up reports of violence, would reduce violence and improve safety for staff. Education of the public regarding ED processes, and the ED staff in relation to patient needs, may contribute to reducing ED violence. What is known about the topic? Violence is prevalent in Australian healthcare, and particularly in emergency departments (ED). Several organisations and government bodies have made recommendations aimed at reducing the prevalence of violence in healthcare but, to date, these have not been implemented consistently, and violence continues. What does this paper add? This study examined ED violence from the perspective of triage nurses, nurse unit managers and non-triage nurses, and revealed that violence is experienced differently by emergency nurses, depending on their area of work. Triage nurses have identified that they themselves contribute to violence in the ED by their style of communication. Nurse unit managers and non-triage nurses perceive that violence is the result of drugs and alcohol, as well as long waiting times. What are the implications for practitioners? Strategies to reduce violence must address the needs of patients and staff both within the ED and in the waiting room. Such strategies should be multifaceted and include education of ED consumers and staff, as well as support from management to respond to reports of violence.
