Abstract:Aim To investigate the characteristics of workplace violence at primary hospitals in Southeast China and identify associated risk factors. Design A cross-sectional survey design was used for this work. Methods We distributed a workplace violence questionnaire among medical staff at primary hospitals in Southeast Zhejiang Province, China. The data were collected between December 2016 and December 2017. We analysed the categorical data by using the chi-square test and expressed it as frequencies. The risk factors were analysed by using multiple logistic regression analysis. Results Among the 2,560 questionnaires, 1,842 (71.9%) medical staff indicated that they had experienced workplace violence. Verbal assault was the most common type, followed by physical and sexual assault. Furthermore, gender, age, marital status, education, technical position and number of hospital beds' numbers were independent risk factors.

Abstract:BackgroundIn China, the reported incidence of workplace violence in hospitals has been increasing. Most research has focused on secondary and tertiary care facilities. We aimed to determine the incidence of workplace violence and its aftermath in county and township hospitals and community health centres in Zhejiang Province.MethodsA cross-sectional survey using stratified cluster sampling was carried out at 18 hospitals (eight county hospitals and ten township hospitals or community health centres) in Zhejiang Province from July 27, 2016 to Jan 20, 2017. Questionnaires were distributed to all health workers (including physicians, nurses, medical technicians, and administrative staff) present on the day of the survey. Ethical approval was obtained from Zhejiang University School of Public Health.Findings2018 out of 2303 participants completed questionnaires from 596 men and 1417 women. In the past year, 140 (7%) respondents had experienced physical attacks and 384 (19%) physical threats. Men experienced around twice as many physical attacks (59 [10%] vs 80 [6%] of women; p=0·001), and significantly more threats than females (132 [22%] vs 252 [18%]; p=0·025). Doctors were most likely to be victims of physical attacks, and the commonest location was the emergency department. Most respondents did not report the physical attack (117 [85%] respondents), mainly because they regarded it as pointless (70 [52%] respondents) or didn't know who to report to (32 [24%] respondents). Of all those attacked or threatened, 312 (61%) wanted to quit direct patient care. In terms of impact, 413 [80%] victims said they had repeated, disturbing memories of the attack, 498 [97%] said they had become highly vigilant, and 386 [75%] were fearful dealing with difficult cases. Only four [3%] were satisfied with the way the incident was handled.InterpretationWorkplace violence is common at lower-level health facilities. The lack of a formal response to workplace violence implies an institutional tolerance which is of great concern. Workplace Violence and Its Aftermath in China's Health Sector: Implications from a Cross-Sectional Survey Across Three Tiers of the Health System

Abstract:Objectives To determine the prevalence of physical violence and threats against health workers and the aftermath in tertiary, secondary and primary care facilities in China. Design A cross-sectional questionnaire study. Setting 5 tertiary hospitals, 8 secondary hospitals and 32 primary care facilities located in both urban and rural areas of Zhejiang Province, China, were chosen as the study sites. Participants A total of 4862 health workers who have contact with patients completed a survey from July 2016 to July 2017. Outcome measures The prevalence of physical violence, threats and Yi Nao, specific forms of physical violence and their aftermath were measured by a self-designed and verified questionnaire. Multivariable logistic regression models were used to examine the association between perceived organisational encouragement of reporting workplace violence (WPV) and physical violence, threats and Yi Nao after controlling for age, sex, level of facility, professional ranking and type of health worker. Results Among all respondents, 224 (4.6%) were physically attacked and 848 (17.4%) experienced threats in the past year. Respondents in secondary hospitals were more likely to experience physical violence (AOR=3.29, 95%CI 2.21 to 4.89), threats (AOR=1.61, 95%CI 1.32 to 1.98) and Yi Nao (AOR=2.47, 95%CI 2.10 to 2.91), compared with primary care providers. Lack of organisational policies to report WPV was associated with higher likelihood of physical violence (AOR=3.64, 95%CI 2.57 to 5.18) and threats (AOR=2.21, 95%CI 1.76 to 2.78). Among physical violence cases, only 29.1% reported the attack to police mainly because most felt it useless to do so (58.8%). Only 25.7% were investigated and 72.4% of attackers received no punishment. Of all those attacked or threatened, 59.4% wanted to quit current post and 76.0% were fearful of dealing with urgent or severe cases. Conclusions Proper management of the aftermath of violence against health workers is inadequate. Formal guidelines for reporting and managing WPV are urgently needed.

Abstract:Objective: Workplace violence (WPV) not only affects workers' physical and mental health but also increases turnover rates and social costs. There are no studies that have consistently followed the prevalence of WPV and the effectiveness of anti-violence measures in cross-sectional studies of the same hospital. The main aim of this study was to understand the prevalence of WPV among medical staff in a hospital in China and the effectiveness of implemented anti-violence measures. Methods: A cross-sectional survey, which was conducted by using a questionnaire developed by the International Labor Organization and the World Health Organization, was administered in a tertiary hospital in China in 2022 to investigate the occurrence of violence in the past year. Valid data were collected from 1,195 respondents via cell phone. WPV was compared between this survey and a baseline survey conducted at the same hospital in 2017 using the same questionnaire. The relationship between years and gender and occupation was explored using the Cochran–Armitage trend test. Descriptive statistics and chi-square tests were used to respond to the effects of the anti-violence interventions. Results: After the implementation of anti-violence measures, the prevalence of WPV decreased from 48.47 in 2017 to 33.95% in 2021. The recognition of safety measures increased from 80.16 in 2017 to 87.70% in 2021( P < 0.001), improvement of the work environment increased from 56.57 in 2017 to 65.10% in 2021( P < 0.001), restricted of public access increased from 34.36 in 2017 to 55.60% in 2021( P < 0.001), the patient protocols increased from 37.42 in 2017 to 38.40% in 2021, patient screening increased from 32.11 in 2017 to 41.90% in 2021( P < 0.001), and strict restrictions on workplace cash activities from 29.65% in 2017 to 36.00% in 2021( P < 0.05). The percentage of health workers who felt that anti-violence measures were not at all helpful to their current work decreased from 2017 to 2021. Conclusions: The prevalence of workplace violence has decreased, recognition of interventions has increased, and the proportion of interventions that are perceived by HWs as not helpful at all has decreased. This study demonstrates that the 12 anti-violence measures recommended by the International Labor Organization are applicable in China. Based on this, hospitals should focus on the improvement of the intervention effect of the following measures to further prevent violence.

Abstract:Objectives Workplace violence (WPV) against healthcare workers (HCWs) has reached significant levels globally, impeding the quality and accessibility of healthcare systems. However, there is limited available knowledge regarding the determinants linked with WPV among HCWs and the discrepancies observed across various levels of hospitals in China. The objective of the present research was to investigate the factors linked to WPV and job satisfaction among HCWs in China. Methods A self-developed questionnaire based on WeChat was employed to collect data. The questionnaire consisted of demographic information as well as occupational factors. To measure WPV, the Chinese version of the Workplace Violence Scale was utilized. Career satisfaction was assessed through two questions regarding career choices. The collected data was analyzed using descriptive analyses, chi-square tests, and multivariate logistic regressions. Results A total of 3,781 valid questionnaires (1,029 doctors and 2,752 nurses) were collected. Among all participants, 2,201 (58.2%) reported experiencing at least one form of WPV in the past year, with emotional abuse being the most frequent occurrence (49.7%), followed by threats (27.9%). The multivariate logistic regression analysis revealed several risk factors associated with WPV, including male gender, shift work, senior professional title, bachelor’s degree education, employment in secondary-level hospitals, and working over 50 h per week ( p < 0.05). Career satisfaction among HCWs who experienced high levels of WPV was low, with only 11.2% remaining confident in their profession, and a mere 2.0% supporting their children pursuing careers in healthcare. Conclusion WPV poses a significant challenge within the Chinese healthcare system. Efforts should be made to address the identified risk factors and promote a safe and satisfying working environment for HCWs.

Abstract:Objectives: This study investigated workplace violence (WPV) toward frontline health workers under comprehensive interventions to improve the occupational safety and health management system in a Chinese infectious disease hospital. Methods: The risk assessment of WPV using an international questionnaire was conducted in 2018 and 2021 to compare the perceived levels of exposure to WPV and intervention measures before and after the intensification of anti-violence measures in the hospital context. Additionally, qualitative data were collected in 2021 through semi-structured and unstructured interviews, providing complementary information about WPV toward frontline health workers (HWs). Results: After establishing the occupational safety and health management system (OSHMS), the total incidence rate of WPV decreased from 60.90% in 2018 to 34.44% in 2021. Psychological violence declined significantly from 60.90% in 2018 to 33.89% in 2021. The endorsement of precautionary measures increased significantly from 2018 to 2021, including patient screening recognition, patient protocol, shift or rota changes, etc. A thematic analysis of several subthemes shows that HWs had an in-depth understanding of WPV, recognizing its multifaceted consequences in the context of complex risk factors. Conclusions: This study demonstrates a significant decrease in WPV, psychological violence, verbal abuse, bullying/mobbing, and ethnic discrimination after implementing the comprehensive OSHMS.

Abstract:Abstract Background Workplace violence (WPV) is a serious issue for healthcare workers and leads to many negative consequences. Several studies have reported on the prevalence of WPV in China, which ranges from 42.2 to 83.3%. However, little information is available regarding the correlates of WPV among healthcare workers and the differences across the different levels of hospitals in China. This study aimed to explore the correlates of WPV and career satisfaction among healthcare workers in China. Methods A self-designed WeChat-based questionnaire was used that included demographic and occupational factors. The Chinese version of the Workplace Violence Scale was used to measure WPV. Career satisfaction was assessed using two questions about career choices. Descriptive analyses, chi-square tests and multivariate logistic regressions were used. Results A total of 3706 participants (2750 nurses and 956 doctors) responded to the survey. Among the 3684 valid questionnaires, 2078 (56.4%) reported at least one type of WPV in the last year. Multivariate logistic regressions revealed that male sex, shift work, bachelor’s degree education, a senior professional title, working more than 50 h per week and working in secondary-level hospitals were risk factors associated with WPV. Healthcare workers who had experienced higher levels of WPV were less likely to be satisfied with their careers. Conclusions WPV remains a special concern for the Chinese healthcare system. Interventions to reduce WPV should be implemented by health authorities to create a zero-violence practice environment.

Abstract:Objective To analyze the patient and visitor workplace violence (PVV) toward health workers (HWs) and identify correlations between worker characteristics, measures against violence and exposure to PVV in COVID-19 pandemic. Methods A cross-sectional survey utilizing the international questionnaires in six public tertiary hospitals from Beijing in 2020 was conducted, and valid data from 754 respondents were collected. Multilevel logistic regression models were used to determine the association between independents and exposure to PVV. Results During COVID-19 pandemic and regular epidemic prevention and control, doctors were 5.3 times (95% CI = 1.59~17.90) more likely to suffer from physical PVV than nurses. HWs most frequently work with infants were 7.2 times (95% CI = 2.24~23.19) more likely to suffer from psychological PVV. More than four-fifth of HWs reported that their workplace had implemented security measures in 2020, and the cross-level interactions between the security measures and profession variable indicates that doctors in the workplace without security measures were 11.3 times (95% CI = 1.09~116.39) more likely to suffer from physical PVV compared to nurses in the workplace with security measures. Conclusion Doctors have higher risk of physical PVV in COVID-19 containment, and the security measures are very important and effective to fight against the physical PVV. Comprehensive measures should be implemented to mitigate hazards and protect the health, safety, and well-being of health workers.

Understanding workplace violence against medical staff in China: a retrospective review of publicly available reports

Abstract:Background: Workplace violence against medical staff in China is a widespread problem that has negative impacts on medical service delivery. The study aimed to contribute to the prevention of workplace violence against medical staff in China by identifying patterns of workplace violence, key risk factors, and the interplay of risk factors that result in workplace violence. Methods: Ninety-seven publicly reported Chinese healthcare violent incidents from late 2013 to 2017 were retrospectively collected from the internet and analysed using content analysis. A modified socio-ecological model guided analysis of the violent incidents focusing on risk. Results: Physical violence, yinao, or a combination of physical and verbal violence were the typical forms of violence reported. The findings identified risk at all levels. Individual level risk factors included service users' unreasonable expectations, limited health literacy, mistrust towards medical staff, and inadequacy of medical staff's communication during the medical encounter. Organisational level risk factors under the purview of hospital management included problems with job design and service provision system, inadequacies with environmental design, security measures, and violence response mechanisms within hospitals. Societal level risk factors included lack of established medical dispute-handling mechanisms, problems in legislation, lack of trust and basic health literacy among service users. Situational level risks were contingent on risk factors on the other levels: individual, organisational, and societal. Conclusions: Interventions at individual, situational, organisational, and societal levels are needed to systematically address workplace violence against medical staff in China. Specifically, improving health literacy can empower patients, increase trust in medical staff and lead to more positive user experiences. Organizational-level interventions include improving human resource management and service delivery systems, as well as providing training on de-escalation and violence response for medical staff. Addressing risks at the societal level through legislative changes and health reforms is also necessary to ensure medical staff safety and improve medical care in China.
Prevalence of workplace violence against healthcare workers: a systematic review and meta-analysis

Abstract:We aim to quantitatively synthesise available epidemiological evidence on the prevalence rates of workplace violence (WPV) by patients and visitors against healthcare workers. We systematically searched PubMed, Embase and Web of Science from their inception to October 2018, as well as the reference lists of all included studies. Two authors independently assessed studies for inclusion. Data were double-extracted and discrepancies were resolved by discussion. The overall percentage of healthcare worker encounters resulting in the experience of WPV was estimated using random-effects meta-analysis. The heterogeneity was assessed using the I2 statistic. Differences by study-level characteristics were estimated using subgroup analysis and meta-regression. We included 253 eligible studies (with a total of 331 544 participants). Of these participants, 61.9% (95% CI 56.1% to 67.6%) reported exposure to any form of WPV, 42.5% (95% CI 38.9% to 46.0%) reported exposure to non-physical violence, and 24.4% (95% CI 22.4% to 26.4%) reported experiencing physical violence in the past year. Verbal abuse (57.6%; 95% CI 51.8% to 63.4%) was the most common form of non-physical violence, followed by threats (33.2%; 95% CI 27.5% to 38.9%) and sexual harassment (12.4%; 95% CI 10.6% to 14.2%). The proportion of WPV exposure differed greatly across countries, study location, practice settings, work schedules and occupation. In this systematic review, the prevalence of WPV against healthcare workers is high, especially in Asian and North American countries, psychiatric and emergency department settings, and among nurses and physicians. There is a need for governments, policymakers and health institutions to take actions to address WPV towards healthcare professionals globally.
Workplace Violence, Workforce Stability, and Well-being in China's Psychiatric Hospitals

Abstract:Introduction: Workplace violence against psychiatric professionals is a growing problem, yet nationally representative data in China are lacking. This study examines workplace violence against psychiatrists and psychiatric nurses in China as well as its association with workforce stability and well-being . Methods: Data came from a 2019 national survey of 14,264 participants (including 4,520 psychiatrists and 9,744 nurses) from 41 psychiatric hospitals across China. The occurrence of physical and verbal assaults among psychiatrists and psychiatric nurses was reported. Logistic regression models were constructed to examine the participants' characteristics associated with encountering workplace violence and the association of encountering violence with self-reported quality of life, health status, turnover intention, and career satisfaction. Analyses were performed during 2020. Results: In 2019, among 14,264 psychiatrists and psychiatric nurses in China, 81% reported encountering workplace violence during the past year. Psychiatrists were 0.68 (95% CI=0.55, 0.83) times less likely to report an encounter of violence than nurses. Male and younger nurses were 2.20 (95% CI=1.72, 2.81) and 1.21 (95% CI=1.01, 1.45) times more likely to report violence. Psychiatrists who had a higher educational degree or a higher professional rank were more vulnerable to violence. Encountering violence was significantly associated with poor quality of life, less satisfaction with health status, greater intention to leave the current job, and career dissatisfaction. Conclusions: Workplace violence against psychiatrists and psychiatric nurses are common in China, indicating that China's psychiatric professionals are facing a significant threat to occupational safety. To maintain psychiatric workforce stability, actions are needed to reduce the prevalence of workplace violence at the system, institutional, and individual levels. Media Contribution to Violence Against Health Workers in China: a Content Analysis Study of 124 Online Media Reports

Abstract:BackgroundViolence against health workers has been widely reported by the Chinese media. We aimed to identify the characteristics of the violent events as reported by the media and explore the possible role of media in such violence.MethodsWe used content analysis to study all available online newspaper reports of violence against health workers in China from Jan 1, 2013, to March 31, 2015. Duplicates of the same event were analysed in combination. Two researchers independently coded the data. Ethical approval was obtained from Zhejiang University Research Ethics Committee. Informed consent was not necessary because no patients were involved.FindingsWe found 124 incidents from 26 provinces. 73 occurred in tertiary-care facilities, 35 in secondary care, three in community facilities, and 13 in facilities such as private hospitals. 41 (33%) were reported from the emergency department, 22 (18%) in general medicine, 17 (14%) in general surgery, nine (7%) in obstetrics and gynaecology, eight (6%) in paediatrics, two (2%) in psychiatry, and 25 (20%) in other departments (eg, intensive-care unit, operating rooms, and radiology). 77 victims were doctors and 39 were nurses. In 116 (93·5%) cases the perpetrator was male, and 21 (17%) were reported to be drunk. In 31 (25%) cases the episode followed the death of a patient. The incidents included six (5%) murders and 12 (10%) serious injuries, mostly stabbing, including a doctor who had his throat cut. Two doctors were forced to kneel down before the body. 92 (74%) incidents involved relatives or friends of the patient, and 14 (11%) involved "Yi Nao gangs" who threatened and assaulted hospital personnel and damaged facilities to extort compensation. Two perpetrators were imprisoned, two sentenced to death, and one committed suicide, but the overwhelming majority were unpunished. Most reports attributed the violence to spontaneous justified anger resulting from poor care, unmet demands, or high costs.InterpretationThese reports represent extreme examples and are the tip of the iceberg. But the way in which they are reported and widely disseminated has bred fear in health professionals. Concerns that such reports of compensation, obtained with impunity, might contribute to copycat behaviours have also been expressed on social media.FundingNone.
Workplace violence against healthcare professionals in a multiethnic area: a cross-sectional study in southwest China

Abstract:Objective The purpose of this study is to examine workplace violence (WPV) towards healthcare professionals in a multiethnic area in China, including prevalence, influencing factors, healthcare professionals’ response to WPV, expected antiviolence training measures and content, and evaluation of WPV interventions. Design A cross-sectional study. Setting A grade III, class A hospital in the capital of Yunnan Province, which is the province with the most diverse ethnic minority groups in China. Participants In total, 2036 healthcare professionals participated, with a response rate of 83.79%. Results The prevalence of physical and psychological violence was 5.5% and 43.7%, respectively. Healthcare professionals of ethnic minority were more likely to experience psychological violence (OR=1.54, 95% CI 1.16 to 2.05). Stratified by gender, male healthcare professionals of ethnic minority suffered from more physical violence (OR=3.31, 95% CI 1.12 to 9.79), while female healthcare professionals suffered from psychological violence (OR=1.71, 95% CI 1.24 to 2.36). We also found a unique work situation in China: overtime duty on-call work (18:00–07:00) was a risk factor for psychological violence (OR=1.40, 95% CI 1.02 to 1.93). Healthcare professionals of ethnic minority are less likely to order perpetrators to stop or to report to superiors when faced with psychological violence. They are also more interested in receiving training in force skills and self-defence. Both Han and ethnic minority participants considered security measures as the most useful intervention, while changing the time of shift the most useless one. Conclusion Our study comprehensively described WPV towards healthcare professionals in a multiethnic minority area. More research on WPV conducted in multiethnic areas is needed.

Abstract:Dear Editor-in-ChiefWorkplace violence has serious consequences for the involved medical workers and the entire health care system (1). It can result in a variety of adverse effects on medical professionals, such as anger, fear, depression, anxiety, job dissatisfaction and demission (2) Research reported that half of abused nurses considered leaving the profession (3). In China, workplace violence against medical workers is more common and serious, especially in recent years (4). A study of 12 hospitals revealed that the incidence of verbal abuse and threats to health professionals was 96% in 2009, and the incidence of physical injuries was 63.7% in 2012 (5). However, the majority of studies were designed to assess the incidence of violence among doctors or nurses who experienced workplace violence. Research on the perception level of risk of violence is rare.The aim of this study was to explore the level of perceived risk of violence in hospitals and the departmental distribution across various levels of hospitals. A cross-sectional survey was conducted between Jun and Jul 2011 in Liaoning Province, which is in northeastern China and comprises three cities and three counties. It concluded 3292 registered health professionals. The survey was answered on a 5-point Likert scale from 1 to 5: 1) never, 2) rarely, 3) sometimes, 4) frequently, and 5) always. The average test-retest reliability of the scale was sufficient.The average score of perceived occupational risk of violence was 3.16±1.31 for all health professionals, 3.05±1.34 for doctors and 3.29±1.25 for nurses. Abstract:Objective: Hospital workplace violence is one of the most frequently reported types of workplace violence in the world, and it harms the mental and physical health of medical staff. Existing research on workplace violence focused more on cross-sectional studies, than longitudinal studies. Therefore, this study examined the dynamic impact of hospital workplace violence on the mental health of medical staff, with the aim of providing appropriate countermeasures and suggestions for hospitals to develop targeted psychological intervention measures in time for medical staff who experience workplace violence. Design: A prospective cohort study. Participants: A total of 112 medical staff who had experienced workplace violence in the hospital for the first time were chosen. Setting: An unconditional latent variable growth curve model was established based on the average value of the general health scale scores of medical staff, and gender and violence types (intimidation threats, physical violence) as control variables. Each medical staff was assessed for depression and anxiety at 4 different time points and the changes in the trajectories was observed. Results: There were significant differences in the incidence of mental health and anxiety among medical staff at different observation times. There were significant individual differences in the initial mental health status, growth rate of anxiety and depression of medical staff. Conclusions: Hospitals should undertake various psychological intervention strategies based on the staff's mental health to safeguard those who have experienced workplace violence.

Abstract:A cross sectional survey carried out in three local emergency departments revealed that workplace violence remained a significant concern with a high prevalence rate of 66.7% and low reporting rate. Introduction Workplace violence affects the dignity and well‐being of healthcare workers and poses threats to the efficiency and success of the healthcare system. Healthcare workers in the emergency department (ED) are known to be vulnerable to workplace violence. This study explored the prevalence of workplace violence in the ED in Hong Kong and factors associated with self‐reported workplace violence. Methods An anonymous, voluntary, self‐administered and web‐based cross‐sectional survey was carried out from 1 June to 30 June 2022 among doctors and nurses in three regional EDs. Multivariable logistic regression analyses were conducted to examine the factors associated with workplace violence. Results A total of 162 healthcare workers including 57 doctors and 105 nurses participated in the study, with an overall response rate of 50.8%. The overall prevalence of workplace violence was 66.7%. Verbal abuse (64.2%) was more common than physical violence (20.4%). Nursing group was significantly correlated with physical violence (aOR 7.27, p = 0.005). Reporting rate of workplace violence remained low (18.2% for physical violence and 12.5% for verbal abuse). More than half of the participants with experience of workplace violence reported that the incident affected their mental well‐being. Conclusions Workplace violence was a significant concern in the local ED and under‐reporting was not uncommon. Impact of workplace violence on healthcare workers and the healthcare system should be well recognised. Workplace violence protocol and prevention strategies are crucial.

Abstract:Objectives Patient-initiated hospital violence is a global problem which threatens the safety of health professionals and is indicative of doctor-patient tensions, impeding health system quality and access. The current study aimed to improve the understanding of medical workplace violence (WPV) in China, using authoritative and nationally representative judgment records, and to approach violence prevention strategies. Methods All litigation records relating to violence against health professionals between 2013 and 2021 were extracted from the China Judgment Online System. Basic case information, victim characteristics, perpetrator characteristics and the nature of the violence were collated. The relationship between different treatment outcomes and violence was also explored. Results Numbers of cases of hospital violence gradually increased from 2013 to a peak in 2016 before gradually decreasing in the following years. The most common perpetrators were patients' relatives (58.2%), followed by patients themselves (38.2%). Only 9 perpetrators had a confirmed history of mental illness and only two were intoxicated with alcohol. More than half of the cases (52.5%) occurred in rural areas and this percentage is even greater for primary health care institutions (71.4%) and secondary hospitals (73.5%). On a departmental level, the highest incidence of medical WPV was found in the emergency (18.9%), pediatrics (13.2%) and obstetrics (11.5%) departments. Violent behaviors, such as stalking, mass occupation of the ward and sharp instrument injury were significantly related to cases not involving patient death ( p < 0.05). Disruptive behavior, such as hanging banners, blocking hospital passages, placing flower wreaths and burning paper money were significantly correlated with cases involving patient death ( p < 0.01). The interval between a patient's death and the ensuing violence was short, happening on the same day in 54.8% of cases. Conclusions A comprehensive overview of medical WPV in China is presented and may have utility for the formulation of prevention strategies.

Workplace violence against COVID-19 front-line healthcare workers versus non-front-line in Hangzhou, China: a cross-sectional study

Abstract:Objectives Workplace violence (WPV) against healthcare workers (HCWs) is a global issue. Our research aimed to elucidate the status and associated factors of WPV among front-line/non-front-line HCWs during the COVID-19 pandemic. Design This cross-sectional study was conducted among HCWs in Hangzhou City through multistage sampling from December 2020 to January 2021. Participants This study included 14 909 valid samples (N=3748 front-line HCWs and N=11 161 non-front-line HCWs). Primary and secondary outcome measures We assessed the WPV status by Chinese version of WPV questionnaire. Binary logistic regression model was established to examine the associated factors of front-line/non-front-line HCWs experiencing WPV. Results The total WPV prevalence equalled 37.25% for front-line HCWs and 27.73% for non-front-line HCWs. Among front-line HCWs, females were less likely to experience WPV (OR 0.837, 95% CI 0.710 to 0.988), while individuals who were undergraduate (OR 1.251, 95% CI 1.061 to 1.541) and had higher professional title (intermediate: OR 1.475, 95% CI 1.227 to 1.772; advanced: OR 1.693, 95% CI 1.294 to 2.216) were more likely to suffer from WPV; for non-front-line HCWs, individuals who aged over 50 years old (OR 0.721, 95% CI 0.563 to 0.969), had worked between 10 and 19 years (OR 0.847, 95% CI 0.749 to 0.958) and worked in the non-graded hospital (OR 0.714, 95% CI 0.614 to 0.832) had less chance to experience WPV, while individuals who had higher educational level (undergraduate: OR 1.323, 95% CI 1.179 to 1.484; ≥graduate: OR 1.519, 95% CI 1.217 to 1.895), were nurse (OR 1.142, 95% CI 1.031 to 1.265), and had higher professional title (intermediate: OR 1.458, 95% CI 1.297 to 638; advanced: OR 1.928, 95% CI 1.607 to 2.313) were more inclined to suffer from WPV (p all<0.05). Conclusions This study indicates that the prevalence of WPV among front-line HCWs is significantly higher than among non-front-line HCWs. Policy-makers should prioritise COVID-19 front-line HCWs, especially those with high educational levels and professional titles.

Abstract:Introduction: China has issued and implemented a series of policies aimed at preventing and controlling workplace violence (WPV) against licensed doctors. However, the prevalence of WPV has not been fundamentally curbed. The aim of this study was to present the prevalence of WPV, identify its influencing factors and propose responsive measures. Method: The online Chinese Physician Practice Survey was conducted with purposive sampling method among licensed doctors in China from January 2022 to June 2022. Data covered licensed doctors' sociodemographic characteristics, occupational characteristics, prevalence of WPV, and perception of effective countermeasures. Results: A total of 74,305 licensed doctors participated in this study. A total of 44.88% of respondents had experienced WPV, among them, either physical violence only (1.06%) or non-physical violence only (89.91%) or encountered both of them (9.03%). Age, gender, marital status, education level, professional title and registration type were all associated with WPV, being younger, non-married, more educated, and higher professional title are all risk factors for WPV. Male (OR = 1.396, 95CI%: 1.355 to 1.439), clinicians (OR = 1.342,95%CI: 1.177 to 1.529), who were single (OR = 1.174, 95%CI: 1.111 to 1.241), with master's degree (OR = 2.021, 95%CI: 1.739 to 2.349) and professional title were subsenior (OR = 1.194, 95%CI: 1.125 to 1.267) were most likely to occur WPV. WPV occurred mostly in provincial capitals, public hospitals, primary and community hospitals, and departments of internal medicine, surgery, pediatrics, emergency medicine and mental health. Overall, 44.24% of doctors perceived that strengthening crackdowns on criminal behaviors was the most effective measure to prevent WPV against healthcare staff. Conclusion: The frequency of WPV decreased after the implementation of relevant laws and policies. Future efforts should be made to strengthen crackdowns on illegal and criminal activities and to issue specific legal provisions on the prevention and control of WPV against doctors.

Abstract:Journal of Interpersonal Violence, Ahead of Print. This is the first study in China to investigate the prevalence of workplace violence (WPV) and its long-term association with post-traumatic stress disorder (PTSD) among healthcare workers. Conducted across various time points from March 27, 2020, to April 10, 2023, the research employed a four-wave repeated cross-sectional design involving 14,993 healthcare workers from 32 provincial administrative regions across the country. Nurses accounted for 34.3% of the sample, while doctors accounted for 65.7% of the sample, with a higher concentration in western China. Multivariate logistic regression analysis was employed to identify the risk factors for WPV, and hierarchical logistic regression models were used to examine the association between PTSD and WPV. The findings revealed that 71.4% of healthcare workers reported experiencing or witnessing WPV, with nearly half (47.5%) indicating exposure to violent incidents within the past year. Further analysis revealed significant associations between WPV incidence and various factors, including age, gender, work experience, work setting, and a history of contact with Coronavirus Disease 2019 (COVID-19) patients (the odds ratio [OR] of these influencing factors ranged from 1.10 to 2.03. All the p-values were less than .05). 28.5% of healthcare workers displayed symptoms of PTSD, with WPV identified as a major risk factor contributing to these symptoms (lifetime WPV: OR = 1.61, 95% CI [1.47–1.76]; 1-year WPV: OR = 1.75 [1.61–1.90]). Therefore, it is imperative to prioritize the mental health of healthcare workers and implement effective measures to prevent and mitigate WPV, ensuring their physical and psychological well-being as well as occupational safety.
