Abstract:Acute myeloid leukemia (AML) is a malignant hematological tumor disease. Chromosomal abnormality is an independent prognostic factor in AML. AML with t(8:21) (q22; q22)/AML1-ETO (AE) is an independent disease group. In this research, a new method based on Raman spectroscopy is reported for label-free single-cell identification and analysis of AE fusion genes in clinical AML patients. Raman spectroscopy reflects the intrinsic vibration information of molecules in a label-free and non-destructive manner, the fingerprint Raman spectrum of cells characterizes intracellular molecular types and relative concentration information, so as to realize the identification and molecular metabolism analysis of different kinds of cells. We collected the Raman spectra of bone marrow cells from clinically diagnosed AML M2 patients with and without the AE fusion gene. Through comparison of the average spectra and identification analysis based on multivariate statistical methods such as principal component analysis and linear discriminant analysis, the distinction between AE positive and negative sample cells in M2 AML patients was successfully achieved, and the single-cell identification accuracy was more than 90%. At the same time, the Raman spectra of the two types of cells were analyzed by the multivariate curve resolution alternating least squares decomposition method. It was found that the presence of the AE fusion gene may lead to the metabolic changes of lipid and nucleic acid in AML cells, which was consistent with the results of genomic and metabolomic multi-omics studies. The above results indicate that single-cell Raman spectroscopy has the potential for early identification of AE-positive AML.

Abstract:Leukemia, a highly malignant form of cancer, relies on early diagnosis for effective treatment and patient survival. Currently, leukemia is diagnosed by manually identifying blood cells through recognizing morphological features using staining techniques. In this work, we achieve rapid, accurate, label-free identification and categorization of four important leukemia cell lines to a subclass level using Raman spectroscopy and an identification model. The flat gold film-covered glass substrates ensure cell intact, good morphology and high-quality Raman spectra. Although the shapes of unstained erythrocytes, K562, U937 and Raji B cells are similar, their spectra are different. The sensitivity, the specificity and the accuracy of the unsupervised identification model (PCA-KMCA) of these four cell lines were 93.75%, 96.67%, and 97.50%, respectively. The accuracy of this model is higher than that of the supervised identification model (LDA-KNN) with an accuracy of 90.62%. Further, the unsupervised identification (PCA-KMCA) of Raji B and Jurkat cells could be greatly achieved, with sensitivity of 96.25%, specificity of 97.50% and accuracy of 96.88%. This method has great potential in the early diagnosis of leukemia, which can help doctors determine treatment plans faster and improve the prognosis of patients.

Abstract:BACKGROUND AND OBJECTIVES:The diagnosis of acute myeloid leukemia (AML) is purely dependent on counting the percentages of blasts (>20%) in the peripheral blood or bone marrow. Manual microscopic examination of peripheral blood or bone marrow aspirate smears is time consuming and less accurate. The first and very important step in blast recognition is the segmentation of the cells from the background for further cell feature extraction and cell classification. In this paper, we aimed to utilize computer technologies in image analysis and artificial intelligence to develop an automatic program for blast recognition and counting in the aspirate smears.METHODS:We proposed a method to analyze the aspirate smear images, which first performs segmentation of the cells by k-means cluster, then builds cell image representing model by HMRF (Hidden-Markov Random Field), estimates model parameters through probability of EM (expectation maximization), carries out convergence iteration until optimal value, and finally achieves second stage refined segmentation. Furthermore, the segmentation results are compared with several other methods using six classes of cells respectively.RESULTS:The proposed method was applied to six groups of cells from 61 bone marrow aspirate images, and compared with other algorithms for its performance on the analysis of the whole images, the segmentation of nucleus, and the efficiency of calculation. It showed improved segmentation results in both the cropped images and the whole images, which provide the base for down-stream cell feature extraction and identification.CONCLUSIONS:Segmentation of the aspirate smear images using the proposed method helps the analyst in differentiating six groups of cells and in the determination of blasts counting, which will be of great significance for the diagnosis of acute myeloid leukemia.

Abstract:Acute myeloid leukemia (AML) is a life‐threatening disease with low survival rates. In most cases, disease relapse is observed due to induced drug resistance. Here, we propose a rapid machine learning‐assisted Raman spectroscopy method to discriminate drug‐sensitive AML cell line HL‐60 from drug resistant HL‐60/MX2 cell line which can help to identify development of resistance in early stages. Linear discriminant analysis (LDA) and logistic regression (LR) models were developed for stratification of cellular Raman spectra obtained from these two groups with accuracies over 94%. The misclassified Raman spectra were investigated in detail, and the reasons for misclassification were elucidated at the biomolecular level making the models clearly discernable. Drug resistance plays a vital role in both cancer treatment and prognosis. Especially, early insights into such drug‐induced resistance in acute myeloid leukemia (AML) can help to improve treatment plans, reduce costs, and bring overall positive outcomes for patients. Raman spectroscopy provides precise biomolecular information and can provide all these necessities effectively. In this study, we employed machine learning (ML) discrimination of Raman micro‐spectroscopic data of myelocytic leukemia cell line HL‐60 from its drug‐resistant counterpart HL‐60/MX2. Principal component analysis (PCA), linear discriminant analysis (LDA), and logistic regression (LR) methods were evaluated for their ability to identify and discriminate drug resistance in AML cells. Our study demonstrates the power of ML to classify drug‐induced resistance in AML cells utilizing subtle variations in biomolecular information contained in molecular spectroscopic data by obtaining 94.11% and 97.05% classification accuracies by LDA and LR models, respectively. We also showed that the ML methods are discernable. Our findings depict the importance of automation and its optimal usage in cancer study and diagnosis. The results of our study are expected to take ML‐assisted Raman spectroscopy one step closer to making it a generalized tool in medical diagnosis in the future.

Abstract:As a label-free, nondestructive, and in situ detection method, Raman spectroscopy analysis of single cells has potential application value in biomedical fields such as cancer diagnosis. In this study, the Raman spectral characteristics of nucleophosmin (NPM1)-mutant acute myeloid leukemia (AML) cells and nonmutated AML cells were investigated, and the reasons for the differences in spectral peaks were explained in combination with transcriptomic analysis. Raman spectra of two AML cell lines without NPM1 mutation (THP-1 and HL-60) and the OCI-AML3 cell line carrying the NPM1 mutant gene were cultured and collected experimentally. It was found that the average Raman spectra of NPM1 mutant and nonmutated cells had intensity differences in multiple peaks corresponding to chondroitin sulfate (CS), nucleic acid, protein, and other molecules. The differentially expressed genes were identified by quantitative analysis of the gene expression matrix of the two types of cells, and their roles in the regulation of CS proteoglycan and protein synthesis were analyzed. The results showed that the differences between the two types of cells expressed by the single-cell Raman spectral information were consistent with the differences in transcriptional profiles. This research could advance the application of Raman spectroscopy in cancer cell typing.

Abstract:Acute lymphoblastic leukaemia (ALL) is a complex disease in pediatric oncology, necessitating accurate diagnostic strategies for effective treatment planning. The ability to differentiate between B-cell ALL (B-ALL) and T-cell ALL (T-ALL) is crucial for targeted interventions. However, current diagnostic methods are time-consuming and require rapid, dependable tests. This study explores the potential of label-free Raman imaging coupled with chemometrics for rapid blast phenotyping of B-ALL and T-ALL. Our findings demonstrate the efficacy of Raman spectroscopy in sensitively and specifically screening and classifying ALL, as well as its rapidity and reliability. The obtained molecular information allows for label-free and precise leukaemia diagnosis at the single-cell level, surpassing the capabilities of traditional diagnostic techniques. Raman spectra of cancer cells reveal distinctive molecular signatures, specifically heightened protein and nucleic acid content, revealing molecular signatures unique to leukemic phenotypes. Based on that, they could be distinguished from each other and their normal B and T lymphocyte counterparts. This research underscores the analytical power of Raman spectroscopy, positioning it as a valuable tool for identifying and classifying pediatric ALL subtypes. The potential translational applications in clinical practice offer a promising avenue for an expedited and accurate leukaemia diagnosis, paving the way for more targeted and personalised therapeutic approaches.

Abstract:<p>Acute lymphoblastic leukemia (ALL) is one of the most common malignancies that account for nearly one-third of all pediatric cancers. The current diagnostic assays are time-consuming, labor-intensive, and require expensive reagents. Here, we report a label-free approach featuring diffraction phase imaging and Raman microscopy that can retrieve both morphological and molecular attributes for label-free optical phenotyping of individual B cells. By investigating leukemia cell lines of early and late stages along with the healthy B cells, we show that phase images can capture subtle morphological differences among the healthy, early, and late stages of leukemic cells. By exploiting its biomolecular specificity, we demonstrate that Raman microscopy is capable of accurately identifying not only different stages of leukemia cells but also individual cell lines at each stage. Overall, our study provides a rationale for employing this hybrid modality to screen leukemia cells using the widefield QPI and using Raman microscopy for accurate differentiation of early and late-stage phenotypes. This contrast-free and rapid diagnostic tool exhibits great promise for clinical diagnosis and staging of leukemia in the near future.</p>

Classifying Raman Spectra of Colon Cells Based on Machine Learning Algorithms

Abstract:Colorectal cancer is very widespread in developed countries. Its diagnosis partly depends on pathologists' experience and their laboratories' instrumentation, producing uncertainty in diagnosis. The use of spectroscopic techniques sensitive to the cellular biochemical environment could aid in achieving a reliable diagnosis. So, we used Raman micro-spectroscopy, combined with a spectral analysis by means of machine learning methods, to build classification models, which allow colon cancer to be diagnosed in cell samples, in order to support such methods as complementary tools for achieving a reliable identification of colon cancer. The Raman spectra were analyzed in the 980–1800 cm−1 range by focusing the laser beam onto the nuclei and the cytoplasm regions of single FHC and CaCo-2 cells (modelling healthy and cancerous samples, respectively) grown onto glass coverslips. The comparison of the Raman intensity of several spectral peaks and the Principal Component Analysis highlighted small biochemical differences between healthy and cancerous cells mainly due to the larger relative lipid content in the former cells with respect to the latter ones and to the larger relative amount of nucleic acid components in cancerous cells compared with healthy ones. We considered four classification algorithms (logistic regression, support vector machine, k nearest neighbors, and a neural network) to associate unknown Raman spectra with the cell type to which they belong. The built machine learning methods achieved median values of classification accuracy ranging from 95.5% to 97.1%, sensitivity values ranging from 95.5% to 100%, and specificity values ranging from 93.9% to 97.1%. The same median values of the classification parameters, which were estimated for a testing set including unknown spectra, ranged between 93.1% and 100% for accuracy and between 92.9% and 100% for sensitivity and specificity. A comparison of the four methods pointed out that k nearest neighbors and neural networks better perform the classification of nucleus and cytoplasm spectra, respectively. These findings are a further step towards the perspective of clinical translation of the Raman technique assisted by multivariate analysis as a support method to the standard cytological and immunohistochemical methods for diagnostic purposes.

Abstract:Leukemia comprises a diverse group of bone marrow tumors marked by immature cell proliferation. Current diagnosis involves identifying leukemia subtypes through visual assessment of blood and bone marrow smears, a subjective and time-consuming method. Our study introduces a novel approach for the characterization of different leukemia subtypes using a global clustering approach of Raman hyperspectral maps of cells. We analyzed bone marrow samples from 19 patients with nine distinct subtypes, conducting high-resolution Raman imaging on 319 cells, generating over 1.3 million spectra in total. A nine-step automated pre-processing pipeline and global clustering identified relevant cellular components, enabling the creation of high-quality pseudostained images at the single cell level. This approach provides a semi-quantitative analysis of cellular component distribution, and multivariate analysis of clustering results reveals the potential of Raman imaging in leukemia research, highlighting both advantages and challenges associated with global clustering.

Automated Cytomorphological Analysis of Bone Marrow Samples: A Proof-of-Principle Study for AI-Based Classification on a Real-Life Data Set of 979 Unselected Cases

Abstract:BACKGROUND: Cytomorphological analysis of bone marrow (BM) samples is a cornerstone of hematological diagnostics. It requires microscopical cell counting by a human specialist, resulting in a differential cell count and a morphological description/diagnosis. The process is susceptible to interobserver variability and may be limited by the areas examined. AIM: Clinical implementation of AI-based recognition of pathological patterns in BM smears to support cytomorphological diagnostics. METHODS: We evaluated unselected BM samples over a three-week period (n= 979), including cases with various hematological diseases. We analyzed both shipped unstained slides (n=522, 53%) and smears prepared in-house from BM aspirates (n=457, 47%) with variable shipping time (0-10d, median = 1d), reflecting diagnostic reality in an international reference laboratory. After conventional cytomorphological assessment, each sample was further analyzed by immunophenotyping, cytogenetics, FISH or molecular genetics, according to diagnostic guidelines. BM slides were then scanned using a Metasystems Metafer System (Altlussheim, GER), yielding up to 52 fields of view (FOVs) (2048x1496px each). Cells were automatically identified, segmented, and imaged resulting in 2636 cells per case on average (range = 0 - 10,072, SD = 2164), which were classified into 24 distinct cell classes using an AI-classifier based on the ResNext-architecture. After eliminating 107 samples (10.9%) due to insufficient scanning/sample quality or low cell counts (<100), 872 samples were further analyzed. We defined 7 relevant pathological patterns extractable from human differential counts which served as ground truth for comparison with AI-based classifications: elevated blast fraction (three-tiered, ≥5%, ≥10%, ≥20%), eosinophilia (≥ 20%), plasmacytosis (≥10%), lymphocytosis (≥20%) and monocytosis (≥5%). For each case, we calculated the median prediction probability (MPP) for each pathological pattern, based on the sum of single cell predictions for pattern-specific cell type and cell fraction (Figure 1). RESULTS: Out of 872 analyzed cases, 341 cases showed one pathological pattern (39.1%), 85 cases showed two patterns (9.7%), and 8 showed three patterns (0.9%) as ground truth. MPP for the pathological patterns were: 0.60 for blasts ≥20% (indicating acute leukemia), 0.52 for blasts ≥10% (e.g. suggesting MDS-IB2), 0.61 for blasts ≥5% (e.g. suggesting MDS-IB1), 0.90 for Eosinophilia (suggesting ML-Eo, HES), 0.87 for plasmacytosis (suggesting plasma cell myeloma), 0.87 for lymphocytosis (suggesting lymphoma) and 0.79 for monocytosis (suggesting MDS or CMML). In cases presenting a blast fraction ≥20% (ground truth) and an MPP for blasts below the median of 0.61, MPP for lymphocytosis and monocytosis was substantially higher (0.81 and 0.71 respectively). This points to the known challenge of differentiating these 3 cell types in AI-supported as well as in human cell classification. Of note, pathological patterns of high clinical relevance (i.e. elevated blast fractions) had consistently low MPP in true negative cases (0.03 for blasts ≥20%, 0.07 for blasts ≥10%, 0.15 for blasts ≥5%). CONCLUSION: We successfully implemented automated scanning and AI-based recognition of pathological patterns in a high-throughput routine workflow on unselected BM smears with variable pathologies, preanalytical parameters and quality. AI detected 7 relevant pathological patterns extracted from human differentials at acceptable prediction probability, even if present concurrently. Confusion of mononuclear cells was noted and requires further optimization of image acquisition and AI-based quality control (e.g., upstream binary classifiers or pre-selection of FOVs) to enhance robustness. This preliminary study proves the technical feasibility of AI-based recognition of pathological BM patterns, to support human investigators in decision making for further diagnostic processes.

Abstract:Acute myeloid leukemia (AML) is a high mortality and recurrence rates hematologic malignancy. Thus, whatever early detection or subsequent visit are both of high significance. Traditional AML diagnosis is conducted via peripheral blood (PB) smear and bone marrow (BM) aspiration. But BM aspiration is a painful burden for patients especially in early detection or subsequent visit. Herein, the use of PB to evaluate and identify the leukemia characteristics will be an attractive alternative source for early detection or subsequent visit. Fourier transform infrared spectroscopy (FTIR) is a time- and cost-effective approach to reveal the disease-related molecular features and variations. However, to the best of our knowledge, there is no attempts using infrared spectroscopic signatures of PB to replace BM for identifying AML. In this work, we are the first to develop a rapid and minimally invasive method to identify AML by infrared difference spectrum (IDS) of PB with only 6 characteristic wavenumbers. We dissect the leukemia-related spectroscopic signatures of three subtypes of leukemia cells (U937, HL-60, THP-1) by IDS, revealing biochemical molecular information about leukemia for the first time. Furthermore, the novel study links cellular features to complex features of blood system which demonstrates the sensitivity and specificity with IDS method. On this basis, BM and PB of AML patients and healthy controls were provided to parallel comparison. The IDS of BM and PB combined with principal component analysis method revealing that the leukemic components in BM and PB can be described by IDS peaks of PCA loadings, respectively. It is demonstrated that the leukemic IDS signatures of BM can be replaced by the leukemic IDS signatures of PB. In addition, the IDS signatures of leukemia cells are reflected in PB of AML patients with peaks of 1629, 1610, 1604, 1536, 1528 and 1404 cm −1 for the first time as well. To this end, we access the leukemic signatures of IDS peaks to compare the PB of AMLs and healthy controls. It is confirmed that the leukemic components can be detected from PB of AML and distinguished into positive (100%) and negative (100%) groups successfully by IDS classifier which is a novel and unique spectral classifier. This work demonstrates the potential use of IDS as a powerful tool to detect leukemia via PB which can release subjects' pain remarkably.

Abstract:Characterizing the heterogeneity in human leukemias is critical for understanding mechanisms of disease onset, uncovering biomarkers for disease diagnosis, and guiding research into novel treatment approaches. While extensive research focus has been placed on characterizing the molecular heterogeneity in leukemia by modern omics technologies, comparatively less emphasis has been placed on studying the diversity in holistic morphological changes that occur in single cells in the context of disease. Here, we report on a novel approach to understanding and characterizing different hematological malignancies using ghost cytometry, a recently developed high-content flow cytometric method that leverages high-speed, artificial intelligence (AI) driven morphological characterization and analysis of single cells. We performed morphometric profiling on human bone marrow samples from patients with T and B-cell acute lymphoblastic leukemia (ALL, n=5), acute myeloid leukemia (AML, n=5), multiple myeloma (MM, n=5) and healthy bone marrow mononuclear cell (BM-MNC) controls (n=6). Morphological profiles from individual cells were analyzed by unsupervised machine learning (universal manifold approximation and projection, UMAP) and compared within disease subsets and against controls. We identified disease-specific cell populations from a mixture of healthy control and disease samples without any labeling, based on completely label-free information. For validation of the specific cell subsets, we overlaid known cell surface markers (e.g. CD45 for blast cells, CD10/CD19 for B-ALL, CD33/CD34 for AML, CD38/CD138 for MM). The intensity of CD45 was dimmer and disease-specific markers were highly expressed in the disease-specific cell population. We also identified a unique cell subpopulation in the disease-specific cell population that could not be distinguished by existing markers. We also analyzed three human myeloma cell lines (AMO1, KMS, and L363) for comparison to the primary MM samples. In the UMAP analysis using LF-GC data, in all three types of cell lines, Bortezomib-resistant cell lines exhibited different distributions from the control parental cell lines. In addition, we observed distinct morphological profiles between primary myeloma samples and myeloma cell lines. Here we present a novel cell characterization approach for human hematological diseases that leverages AI-based, label-free, high-speed morphological characterization of single cells. We demonstrate that the approach can be used to identify subtle morphological differences in blast cells from a range of blood cancer subtypes. Application of morphological profiling and AI can be used to identify measurable, disease-related changes in these diseases that have diagnostic, drug screening, and therapeutic monitoring potential. Abstract:Invasive damage, poor prognosis and survival are problems for the traditional diagnostic and therapeutic methods of acute myeloid leukemia (AML), especially for the middle-aged and elderly. High sensitivity, less pain and fast speed are demands of AML diagnosis or long-term regular monitoring. Infrared (IR) spectroscopy has the potential to compensate for existing deficiencies and is explored in our work. Spectral differences between AML patients and controls are observed on the IR spectra, which indicate the changes in lipids and proteins in patients. The peak shift at 1390 cm−1 occurs on both peripheral blood (PB) and bone marrow (BM) spectra of AML patients making it possible to use PB rather than BM as the diagnostic source, which can relieve the suffering of diagnosis and monitoring. The logistic regression-based classification model can perfectly identify AML patients and controls with an accuracy of 100 % and the area under curve is equal to 1.00. Besides, some markers for identifying AML patients and controls are found. Statistical analysis shows that the peak area ratio A1451/A1390 has a significant difference, and the decision tree-based classification model finds that on the first derivative spectrum, the position of the largest difference is at 1391 cm−1. Furthermore, the proposed criterion based on markers we found can distinguish AML patients and controls with high accuracy of 95 %. Results show that PB’s IR spectrum combined with classification models can monitor the molecular-level biochemical changes and accurately identify AML patients and controls, which is helpful for the effective and efficient diagnosis and prognosis of AML.

A Machine Learning Tool Using Digital Microscopy (Morphogo) for the Identification of Abnormal Lymphocytes in the Bone Marrow

Abstract:Morphological analysis of the bone marrow is an essential step in the diagnosis of hematological disease. The conventional analysis of bone marrow smears is performed under a manual microscope, which is labor-intensive and subject to interobserver variability. The morphological differential diagnosis of abnormal lymphocytes from normal lymphocytes is still challenging. The digital pathology methods integrated with advances in machine learning enable new diagnostic features/algorithms from digital bone marrow cell images in order to optimize classification, thus providing a robust and faster screening diagnostic tool. We have developed a machine learning system, Morphogo, based on algorithms to discriminate abnormal lymphocytes from normal lymphocytes using digital imaging analysis. We retrospectively reviewed 347 cases of bone marrow digital images. Among them, 53 cases had a clinical history and the diagnosis of marrow involvement with lymphoma was confirmed either by morphology or flow cytometry. We split the 53 cases into two groups for training and testing with 43 and 10 cases, respectively. The selected 15,353 cell images were reviewed by pathologists, based on morphological visual appearance, from 43 patients whose diagnosis was confirmed by complementary tests. To expand the range and the precision of recognizing the lymphoid cells in the marrow by automated digital microscopy systems, we developed an algorithm that incorporated color and texture in addition to geometrical cytological features of the variable lymphocyte images which were applied as the training data set. The selected images from the 10 patients were analyzed by the trained artificial intelligence-based recognition system and compared with the final diagnosis rendered by pathologists. The positive predictive value for the identification of the categories of reactive/normal lymphocytes and abnormal lymphoid cells was 99.04%. It seems likely that further training and improvement of the algorithms will facilitate further subclassification of specific lineage subset pathology, e.g., diffuse large B-cell lymphoma from chronic lymphocytic leukemia/small lymphocytic lymphoma, follicular lymphoma, mantle cell lymphoma or even hairy cell leukemia in cases of abnormal malignant lymphocyte classes in the future. This research demonstrated the feasibility of digital pathology and emerging machine learning approaches to automatically diagnose lymphoma cells in the bone marrow based on cytological-histological analyses.

Abstract:Biochemical information from activated leukocytes provide valuable diagnostic information. In this study, Raman spectroscopy was applied as a label-free analytical technique to characterize the activation pattern of leukocyte subpopulations in an in vitro infection model. Neutrophils, monocytes, and lymphocytes were isolated from healthy volunteers and stimulated with heat-inactivated clinical isolates of Candida albicans, Staphylococcus aureus, and Klebsiella pneumoniae. Binary classification models could identify the presence of infection for monocytes and lymphocytes, classify the type of infection as bacterial or fungal for neutrophils, monocytes, and lymphocytes and distinguish the cause of infection as Gram-negative or Gram-positive bacteria in the monocyte subpopulation. Changes in single-cell Raman spectra, upon leukocyte stimulation, can be explained with biochemical changes due to the leukocyte's specific reaction to each type of pathogen. Raman spectra of leukocytes from the in vitro infection model were compared with spectra from leukocytes of patients with infection (DRKS-ID: DRKS00006265) with the same pathogen groups, and a good agreement was revealed. Our study elucidates the potential of Raman spectroscopy-based single-cell analysis for the differentiation of circulating leukocyte subtypes and identification of the infection by probing the molecular phenotype of those cells.
