Abstract:We developed an upper-limb power-assist exoskeleton actuated by pneumatic muscles. The exoskeleton included two metal links: a nylon joint, four size-adjustable carbon fiber bracers, a potentiometer and two pneumatic muscles. The proportional myoelectric control method was proposed to control the exoskeleton according to the user's motion intention in real time. With the feature extraction procedure and the classification (back-propagation neural network), an electromyogram (EMG)-angle model was constructed to be used for pattern recognition. Six healthy subjects performed elbow flexion-extension movements under four experimental conditions: (1) holding a 1-kg load, wearing the exoskeleton, but with no actuation and for different periods (2-s, 4-s and 8-s periods); (2) holding a 1-kg load, without wearing the exoskeleton, for a fixed period; (3) holding a 1-kg load, wearing the exoskeleton, but with no actuation, for a fixed period; (4) holding a 1-kg load, wearing the exoskeleton under proportional myoelectric control, for a fixed period. The EMG signals of the biceps brachii, the brachioradialis, the triceps brachii and the anconeus and the angle of the elbow were collected. The control scheme's reliability and power-assist effectiveness were evaluated in the experiments. The results indicated that the exoskeleton could be controlled by the user's motion intention in real time and that it was useful for augmenting arm performance with neurological signal control, which could be applied to assist in elbow rehabilitation after neurological injury.
Combined surface and intramuscular EMG for improved real-time myoelectric control performance

Abstract:The four main functions that are available in current clinical prostheses (e.g. Otto Bock DMC Plus®) are power grasp, hand open, wrist pronation and wrist supination. Improving the control of these two DoFs is therefore of great clinical and commercial interest. This study investigates whether control performance can be improved by targeting wrist rotator muscles by means of intramuscular EMG. Nine able-bodied subjects were evaluated using offline metrics and during a real-time control task. Two intramuscular (targeted) and four surface EMG channels were recorded concurrently from the right forearm. The control was derived either from the four surface sources or by combining two surface channels combined with two intramuscular channels located in the pronator and supinator muscles (combined EMG). Five metrics (Throughput, Path Efficiency, Average Speed, Overshoot and Completion Rate) were used to quantify real-time performance. A significant improvement of 20% in Throughput was obtained with combined EMG (0.90±0.12bit/s) compared to surface EMG alone (0.75±0.10bit/s). Furthermore, combined EMG performed significantly better than the surface EMG in terms of Overshoot, Path Efficiency and offline classification error. No significant difference was found for Completion Rate and Average Speed. The results obtained in this study imply that targeting muscles that are involved in the rotation of the forearm could improve the performance of myoelectric control systems that include both wrist rotation and opening/closing of a terminal device.

Abstract:BackgroundMyoelectric pattern recognition systems can decode movement intention to drive upper-limb prostheses. Despite recent advances in academic research, the commercial adoption of such systems remains low. This limitation is mainly due to the lack of classification robustness and a simultaneous requirement for a large number of electromyogram (EMG) electrodes. We propose to address these two issues by using a multi-modal approach which combines surface electromyography (sEMG) with inertial measurements (IMs) and an appropriate training data collection paradigm. We demonstrate that this can significantly improve classification performance as compared to conventional techniques exclusively based on sEMG signals.MethodsWe collected and analyzed a large dataset comprising recordings with 20 able-bodied and two amputee participants executing 40 movements. Additionally, we conducted a novel real-time prosthetic hand control experiment with 11 able-bodied subjects and an amputee by using a state-of-the-art commercial prosthetic hand. A systematic performance comparison was carried out to investigate the potential benefit of incorporating IMs in prosthetic hand control.ResultsThe inclusion of IM data improved performance significantly, by increasing classification accuracy (CA) in the offline analysis and improving completion rates (CRs) in the real-time experiment. Our findings were consistent across able-bodied and amputee subjects. Integrating the sEMG electrodes and IM sensors within a single sensor package enabled us to achieve high-level performance by using on average 4-6 sensors.ConclusionsThe results from our experiments suggest that IMs can form an excellent complimentary source signal for upper-limb myoelectric prostheses. We trust that multi-modal control solutions have the potential of improving the usability of upper-extremity prostheses in real-life applications.

Abstract:Stroke patients often suffer from severe upper limb paresis. Rehabilitation treatment typically targets motor impairments as early as possible, however, muscular contractions, particularly in the wrist and fingers, are often too weak to produce overt movements, making the initial phase of rehabilitation training difficult. Here we propose a new training tool whereby electromyographic (EMG) activity is measured in the wrist extensors and serves as a proxy of voluntary corticomotor drive. We used the Myo armband to develop a proportional EMG controller which allowed volunteers to perform a simple visuomotor task by modulating wrist extensor activity. In this preliminary study six healthy participants practiced the task for one session (144 trials), which resulted in a significant reduction of the average trial time required to move and hold a cursor in different target zones (p < 0.001, ANOVA), indicating skill learning. Additionally, we implemented an EMG based classifier to distinguish between the desired movement strategy and unwanted alternatives. Validation of the classifier indicated that accuracy for detecting rest, wrist extension and unwanted strategies was 92.5 + 6.9% (M + SD) across all participants. When performing the motor task the classification algorithm flagged 4.3 + 3.5% of the trials as 'unwanted strategies', even in healthy subjects. We also report initial feedback from a survey submitted to two chronic stroke patients to inquire about feasibility and acceptance of the general setup by patients.

Abstract:Myocontrol, that is, control of a prosthesis via muscle signals, is still a surprisingly hard problem. Recent research indicates that surface electromyography (sEMG), the traditional technique used to detect a subject's intent, could proficiently be replaced, or conjoined with, other techniques (multi-modal myocontrol), with the aim to improve both on dexterity and reliability. Objective. In this paper we present an online assessment of multi-modal sEMG and force myography (FMG) targeted at hand and wrist myocontrol. Approach. Twenty sEMG and FMG sensors in total were used to enforce simultaneous and proportional control of hand opening/closing, wrist pronation/supination and wrist flexion/extension of 12 intact subjects. Main results and Significance. We found that FMG yields in general a better performance than sEMG, and that the main drawback of the sEMG array we used is not the inability to perform a desired action, but rather action interference, that is, the undesired concurrent activation of another action. FMG, on the other hand, causes less interference.
Multichannel electrotactile feedback for simultaneous and proportional myoelectric control

Abstract:Objective: Closing the loop in myoelectric prostheses by providing artificial somatosensory feedback to the user is an important need for prosthetic users. Previous studies investigated feedback strategies in combination with the control of one degree of freedom of simple grippers. Modern hands, however, are sophisticated multifunction systems. In this study, we assessed multichannel electrotactile feedback integrated with an advanced method for the simultaneous and proportional control of individual fingers of a dexterous hand. Approach: The feedback used spatial and frequency coding to provide information on the finger positions (normalized flexion angles). A comprehensive set of conditions have been investigated in 28 able-bodied subjects, including feedback modalities (visual, electrotactile and no feedback), control tasks (fingers and grasps), systems (virtual and real hand), control methods (ideal and realistic) and range of motion (low and high). The task for the subjects was to operate the hand using closed-loop myoelectric control and generate the desired movement (e.g., selected finger or grasp at a specific level of closure). Main results: The subjects could perceive the multichannel and multivariable electrotactile feedback and effectively exploit it to improve the control performance with respect to open-loop grasping. The improvement however depended on the reliability of the feedforward control, with less consistent control exhibiting performance trends that were more complex across the conditions. Significance: The results are promising for the potential application of advanced feedback to close the control loop in sophisticated prosthetic systems.

Abstract:Robust classification of natural hand grasp type based on electromyography (EMG) still has some shortcomings in the practical prosthetic hand control, owing to the influence of dynamic arm position changing during hand actions. This study provided a framework for robust hand grasp type classification during dynamic arm position changes, improving both the "hardware" and "algorithm" components. In the hardware aspect, co-located synchronous EMG and force myography (FMG) signals are adopted as the multi-modal strategy. In the algorithm aspect, a sequential decision algorithm is proposed by combining the RNN-based deep learning model with a knowledge-based post-processing model. Experimental results showed that the classification accuracy of multi-modal EMG-FMG signals was increased by more than 10% compared with the EMG-only signal. Moreover, the classification accuracy of the proposed sequential decision algorithm improved the accuracy by more than 4% compared with other baseline models when using both EMG and FMG signals.

Abstract:OBJECTIVE To investigate the relationship of vascular endothelial growth factor C (VEGF-C) and induced nitride oxide synthesizase (iNOS) expression in lymph node micrometastasis of oral squamous cell carcinoma. METHODS Samples were obtained from 47 cases of oral squamous cell carcinoma and 15 cases with normal oral mucosa, VEGF-C and iNOS mRNA expression were detected by RT-PCR method. Lymph node micrometastasis of 10 normal lymph nodes and 355 lymph nodes from 47 cases of oral squamous cell carcinoma was detected with immunohistochemical reaction in cytokeratin antibody. RESULTS The percentages in tumors with higher expression were 57.4% for VEGF-C, 68.1% for iNOS (P < 0.05). They were significantly higher than that of normal groups. Significant positive relationship was found between VEGF-C and iNOS (P < 0.01). The positive rate of cytokeratin (CK) was 48.9%. Significant positive relationship was found between VEGF-C and CK, iNOS and CK (P < 0.01). The expression rates of CK in positive group of VEGF-C and iNOS were 63.0%, 65.6% respectively, and were significant higher than negative groups. CONCLUSION Expression of VEGF-C and iNOS in lymph node micrometastasis of oral squamous cell carcinoma is significant related.
An Alternative Myoelectric Pattern Recognition Approach for the Control of Hand Prostheses: A Case Study of Use in Daily Life by a Dysmelia Subject

Abstract:The functionality of upper limb prostheses can be improved by intuitive control strategies that use bioelectric signals measured at the stump level. One such strategy is the decoding of motor volition via myoelectric pattern recognition (MPR), which has shown promising results in controlled environments and more recently in clinical practice. Moreover, not much has been reported about daily life implementation and real-time accuracy of these decoding algorithms. This paper introduces an alternative approach in which MPR allows intuitive control of four different grips and open/close in a multifunctional prosthetic hand. We conducted a clinical proof-of-concept in activities of daily life by constructing a self-contained, MPR-controlled, transradial prosthetic system provided with a novel user interface meant to log errors during real-time operation. The system was used for five days by a unilateral dysmelia subject whose hand had never developed, and who nevertheless learned to generate patterns of myoelectric activity, reported as intuitive, for multi-functional prosthetic control. The subject was instructed to manually log errors when they occurred via the user interface mounted on the prosthesis. This allowed the collection of information about prosthesis usage and real-time classification accuracy. The assessment of capacity for myoelectric control test was used to compare the proposed approach to the conventional prosthetic control approach, direct control. Regarding the MPR approach, the subject reported a more intuitive control when selecting the different grips, but also a higher uncertainty during proportional continuous movements. This paper represents an alternative to the conventional use of MPR, and this alternative may be particularly suitable for a certain type of amputee patients. Moreover, it represents a further validation of MPR with dysmelia cases.

Abstract:Myoelectric prostheses allow users to recover lost functionality by controlling a robotic device with their remaining muscle activity. Such commercial devices can give users a high level of autonomy, but still do not approach the dexterity of the intact human hand. Here we present a method to control a robotic hand, shared between user intention and robotic automation. The algorithm allows user-controlled movements when high dexterity is desired, but also assisted grasping when robustness is paramount. This combination of features is currently lacking in commercial prostheses and can greatly improve prosthesis usability. First, we design and test a myoelectric proportional controller that can predict multiple joint angles simultaneously and with high accuracy. We then implement online control with both able-bodied and amputee subjects. Finally, we present a shared control scheme in which robotic automation aids in object grasping by maximizing the contact area between the hand and the object, greatly increasing grasp success and object hold times in both a virtual and a physical environment. Our results present a viable method of prosthesis control implemented in real time, for reliable articulation of multiple simultaneous degrees of freedom.

Abstract:Objective. This paper proposes a novel simultaneous and proportional multiple degree of freedom (DOF) myoelectric control method for active prostheses. Approach. The approach is based on non-negative matrix factorization (NMF) of surface EMG signals with the inclusion of sparseness constraints. By applying a sparseness constraint to the control signal matrix, it is possible to extract the basis information from arbitrary movements (quasi-unsupervised approach) for multiple DOFs concurrently. Main Results. In online testing based on target hitting, able-bodied subjects reached a greater throughput (TP) when using sparse NMF (SNMF) than with classic NMF or with linear regression (LR). Accordingly, the completion time (CT) was shorter for SNMF than NMF or LR. The same observations were made in two patients with unilateral limb deficiencies. Significance. The addition of sparseness constraints to NMF allows for a quasi-unsupervised approach to myoelectric control with superior results with respect to previous methods for the simultaneous and proportional control of multi-DOF. The proposed factorization algorithm allows robust simultaneous and proportional control, is superior to previous supervised algorithms, and, because of minimal supervision, paves the way to online adaptation in myoelectric control.

Abstract:OBJECTIVE: The conventional myoelectric control scheme of hand prostheses provides a high level of robustness during continuous use. Typically, the electrical activity of an agonist/antagonist muscle pair in the forearm is detected and used to control either opening/closing or rotation of the prosthetic hand. The translation of more sophisticated control approaches (e.g. regression-based classifiers) to clinical practice is limited mainly because of their lack of robustness in real-world conditions (e.g. due to different arm positions). We therefore explore a new hybrid approach, in which a second degree of freedom (DOF) controlled by the myoelectric activity of the posterior auricular muscles is added to the conventional forearm control. With this, an independent, simultaneous and proportional control of rotation and opening/closing of the hand is possible.APPROACH: In this study, we compared the hybrid auricular control system (hACS) to the two most commonly used control techniques for two DOF. Ten able-bodied subjects and one person with transradial amputation performed two standardizes tests in three different arm positions.MAIN RESULTS: Subjects controlled a hand prosthesis significantly more rapidly and more accurately using the hACS. Moreover, the robustness of the system was not influenced by different arm positions.SIGNIFICANCE: The hACS therefore offers an alternative solution for simultaneous and proportional myoelectric control of two degrees of freedom that avoids several robustness issues related to machine learning based approaches.

Abstract:Introduction: Duchenne muscular dystrophy (DMD) is a genetic disorder that induces progressive muscular degeneration. Currently, the increase in DMD individuals' life expectancy is not being matched by an increase in quality of life. The functioning of the hand and wrist is central for performing daily activities and for providing a higher degree of independence. Active exoskeletons can assist this functioning but require the accurate decoding of the users' motor intention. These methods have, however, never been systematically analyzed in the context of DMD. Methods: This case study evaluated direct control (DC) and pattern recognition (PR), combined with an admittance model. This enabled customization of myoelectric controllers to one DMD individual and to a control population of ten healthy participants during a target-reaching task in 1- and 2- degrees of freedom (DOF). We quantified real-time myocontrol performance using target reaching times and compared the differences between the healthy individuals and the DMD individual. Results and Discussion: Our findings suggest that despite the muscle tissue degeneration, the myocontrol performance of the DMD individual was comparable to that of the healthy individuals in both DOFs and with both control approaches. It was also evident that PR control performed better for the 2-DOF tasks for both DMD and healthy participants, while DC performed better for the 1-DOF tasks. The insights gained from this study can lead to further developments for the intuitive multi-DOF myoelectric control of active hand exoskeletons for individuals with DMD.
Proprioceptive Sonomyographic Control: A novel method of intuitive proportional control of multiple degrees of freedom for upper-extremity amputees

Abstract:Technological advances in multi-articulated prosthetic hands have outpaced the methods available to amputees to intuitively control these devices. Amputees often cite difficulty of use as a key contributing factor for abandoning their prosthesis, creating a pressing need for improved control technology. A major challenge of traditional myoelectric control strategies using surface electromyography electrodes has been the difficulty in achieving intuitive and robust proportional control of multiple degrees of freedom. In this paper, we describe a new control method, proprioceptive sonomyographic control that overcomes several limitations of myoelectric control. In sonomyography, muscle mechanical deformation is sensed using ultrasound, as compared to electrical activation, and therefore the resulting control signals can directly control the position of the end effector. Compared to myoelectric control which controls the velocity of the end-effector device, sonomyographic control is more congruent with residual proprioception in the residual limb. We tested our approach with 5 upper-extremity amputees and able-bodied subjects using a virtual target achievement and holding task. Amputees and able-bodied participants demonstrated the ability to achieve positional control for 5 degrees of freedom with an hour of training. Our results demonstrate the potential of proprioceptive sonomyographic control for intuitive dexterous control of multiarticulated prostheses.

Preliminary Assessment of Two Simultaneous and Proportional Myocontrol Methods for 3-DoFs Prostheses Using Incremental Learning

Abstract:Despite progressive developments over the last decades, current upper limb prostheses still lack a suitable control able to fully restore the functionalities of the lost arm. Traditional control approaches for prostheses fail when simultaneously actuating multiple Degrees of Freedom (DoFs), thus limiting their usability in daily-life scenarios. Machine learning, on the one hand, offers a solution to this issue through a promising approach for decoding user intentions but fails when input signals change. Incremental learning, on the other hand, reduces sources of error by quickly updating the model on new data rather than training the control model from scratch. In this study, we present an initial evaluation of a position and a velocity control strategy for simultaneous and proportional control over 3-DoFs based on incremental learning. The proposed controls are tested using a virtual Hannes prosthesis on two healthy participants. The performances are evaluated over eight sessions by performing the Target Achievement Control test and administering SUS and NASA-TLX questionnaires. Overall, this preliminary study demonstrates that both control strategies are promising approaches for prosthetic control, offering the potential to improve the usability of prostheses for individuals with limb loss. Further research extended to a wider population of both healthy subjects and amputees will be essential to thoroughly assess these control paradigms.

Abstract:The disposition of phenytoin was examined in 7 infants with a mean age of 22 days and a mean weight of 3,756 g. Multiple doses of phenytoin were administered intravenously and/or orally as a part of required medical care. Gas chromatography/mass spectrometry was employed for analyses of phenytoin and three metabolites - 5-(4-hydroxyphenyl)-5-phenylhydantoin, methylated 3,4-catechol, and 3,4-dihydrodiol - in blood, urine, and feces. Data from identical studies previously conducted in adults were utilized for comparison with the infants. The mean phenytoin dose (+/- SD) in infants was 8.1 +/- 4.4 mg/kg/day, and the mean serum concentration (+/- SD) was 4.8 +/- 4.6 micrograms/ml. In adults, the mean dose was 5.3 +/- 0.6 mg/kg/day, and the mean serum concentration was 12.0 +/- 1.9 micrograms/ml. No significant differences were found between infants and adults in the pattern of urinary metabolites or in the total recovery of phenytoin and metabolites in 24-hour urine samples. These results indicate that pathways for phenytoin metabolism are the same in infants and adults. Absorption of an oral dose of phenytoin in infants appeared to be completely based on recoveries of drug and metabolites in urine and on the fact that less than 3% of an oral dose could be found in stools. The results of these studies indicate that the low blood concentrations of phenytoin resulting from the relatively high daily dosage of phenytoin in infants, when compared to adults, cannot be explained on the basis of poor oral absorption of phenytoin. These age-related differences must be due to a relatively high metabolic clearance of the drug in infants.

Abstract:Abstract Background Due to their limited dexterity, it is currently not possible to use a commercially available prosthetic hand to unscrew or screw objects without using elbow and shoulder movements. For these tasks, prosthetic hands function like a wrench, which is unnatural and limits their use in tight working environments. Results from timed rotational tasks with human subjects demonstrate the clinical need for increased dexterity of prosthetic hands, and a clinically viable solution to this problem is presented for an anthropomorphic artificial hand. Methods Initially, a human hand motion analysis was performed during a rotational task. From these data, human hand synergies were derived and mapped to an anthropomorphic artificial hand. The synergy for the artificial hand is controlled using conventional dual site electromyogram (EMG) signals. These EMG signals were mapped to the developed synergy to control four joints of the dexterous artificial hand simultaneously. Five limb absent and ten able-bodied test subjects participated in a comparison study to complete a timed rotational task as quickly as possible with their natural hands (except for one subject with a bilateral hand absence), eight commercially available prosthetic hands, and the proposed synergy controller. Each test subject used two to four different artificial hands. Results With the able-bodied subjects, the developed synergy controller reduced task completion time by 177% on average. The limb absent subjects completed the task faster on average than with their own prostheses by 46%. There was a statistically significant improvement in task completion time with the synergy controller for three of the four limb absent participants with integrated prostheses, and was not statistically different for the fourth. Conclusions The proposed synergy controller reduced average task completion time compared to commercially available prostheses. Additionally, the synergy controller is able to function in a small workspace and requires less physical effort since arm movements are not required. The synergy controller is driven by conventional dual site EMG signals that are commonly used for prosthetic hand control, offering a viable solution for people with an upper limb absence to use a more dexterous artificial hand to screw or unscrew objects.

Abstract:In myoelectric control, simultaneous control of multiple degrees of freedom can be challenging due to the dexterity of the human hand. Numerous studies have focused on hand functionality, however, they only focused on a few degrees of freedom. In this paper, a 3DCNN-MLP model is proposed that uses high-density sEMG signals to estimate 20 hand joint positions and grip force simultaneously. The deep learning model maps the muscle activity to the hand kinematics and kinetics. The proposed models' performance is also evaluated in estimating grip forces with real-time resolution. This paper investigated three individual dynamic hand movements (2pinch, 3pinch, and fist closing and opening) while applying forces in 10% and 30% of the maximum voluntary contraction (MVC). The results demonstrated significant accuracy in estimating kinetics and kinematics. The average Euclidean distance across all joints and subjects was 11.01 $\pm$ 2.22 mm and the mean absolute error for offline and real-time force estimation were found to be 0.8 $\pm$ 0.33 N and 2.09 $\pm$ 0.9 N respectively. The results demonstrate that by leveraging high-density sEMG and deep learning, it is possible to estimate human hand dynamics (kinematics and kinetics), which is a step forward to practical prosthetic hands.
