Abstract:Background Inflammatory bowel disease (IBD) is rising in China, and the tendency for lifelong recurrence decreases patients' quality of life. However, no studies on treatment decision-making in Chinese patients with IBD exist. Thus, this study aimed to determine the actual and ideal decision-making, as well as factors affecting decision-making in Chinese IBD patients. Methods A multicenter online questionnaire was distributed among patients diagnosed with IBD. To assess factors that influence treatment decision-making, univariate and multivariate logistic regression analyses were performed. Results From March 20, 2018, to May 20, 2018, a total of 866 patients completed the questionnaires, including 222 patients with ulcerative colitis, 588 patients with Crohn's disease, and 56 patients with unclassified IBD. There was a significant difference between ideal and actual decision-making in Chinese IBD patients (P < .005). The factors affecting ideal decision-making included income, education, illness severity, religiosity, the importance of the treatment decision, the employment situation, and occupation area. The factors affecting actual decision-making included age, illness severity, religiosity, the employment situation, economic anxiety, concern about the side effects, and the importance of the treatment decision. Conclusions There is a significant difference between ideal and actual decision-making in IBD patients in China. That is, the economy, religiosity, illness severity, and concern about the side effects of treatment are the most important factors affecting treatment decisions in Chinese IBD patients.
Exploring the medical decision-making patterns and influencing factors among the general Chinese public: a binary logistic regression analysis

Abstract:Abstract Objective With the ongoing evolution of the healthcare system and shifts in cultural paradigms, there is a pressing need to delve into the medical decision-making behaviors of general Chinese public and understand their underlying motivations. This research seeks to elucidate the prevailing tendencies in these decision-making processes and to empirically validate the pivotal factors that shape their choices, offering valuable insights for healthcare policymakers and institutions. Method A comprehensive survey was administered to 2,696 Chinese residents to examine their medical decision-making patterns. These patterns were classified into two primary categories: Unilateral Decision-making (Doctor-dominant, Family-centric, and Patient-driven subtypes) and Collaborative Decision-making (Doctor-led, Doctor-Patient, Patient-Family, and Doctor-Patient-Family subtypes). Binary logistic regression analysis was employed to empirically pinpoint the significant factors influencing these decision-making frameworks. Results The study's analysis reveals distinct preferences in medical decision-making among Chinese residents. In the Collaborative Decision-making category, chosen by 70.81% of participants, the subtypes are as follows: Doctor-led (29.90%), Doctor-Patient (13.54%), Patient-Family (2.93%), and Doctor-Patient-Family (24.44%). The Unilateral Decision-making, preferred by 29.19%, includes Doctor-dominant (23.22%), Family-centric (1.74%), and Patient-driven (4.23%) models. The preference for Collaborative Decision-making is associated with higher educational levels, specific marital statuses (particularly married but childless), and choices of rural residents' basic medical insurance or occupational basic medical insurance. In contrast, Unilateral Decision-making correlates with males, individuals with religious beliefs, certain occupational roles (like civil servants), and holders of commercial or publicly funded medical insurance. Conclusion This study elucidates the complex interplay of socio-cultural and individual determinants shaping medical decision-making in China. The findings reveal a marked inclination towards collaborative models, closely linked to educational level, marital status, and specific insurance types, reflecting an evolving trend towards participatory healthcare. Simultaneously, the persistence of unilateral models, influenced by gender, religious beliefs, and occupational roles, highlights the heterogeneity within Chinese healthcare preferences. These insights are crucial for policymakers and healthcare practitioners, underscoring the need for adaptable, culturally attuned healthcare frameworks that cater to this diversity, thereby enhancing patient engagement and healthcare efficacy.

Abstract:Background Recently, decision-making process has become increasingly complex. But there is limited information on Chinese patients’ views of shared decision making (SDM) in inflammatory bowel disease (IBD). This questionnaire investigation aimed to understand Chinese patients’ perspectives and expectations of SDM in IBD and analyze the possible factors that influence their views. Methods An online survey was conducted from July 19th to 24th, 2020. A total of 1118 patients completed the survey. Results One-third of patients were dissatisfied with the current decision-making model, and the satisfaction of inpatients was lower than that of outpatients. 84% of patients preferred to participate in SDM, who were young and had a high education level, high income, commercial insurance, strong learning ability and knowledge of SDM. Most of those who did not want to participate (72%) were worried about the cost. The kind of medicine (948, 84.8%), surgical indications (505, 45.2%) and operation methods (482, 43.1%) were the topics that patients thought most require SDM. Side effects of medicine (837, 74.9%), costs of therapy (675, 60.4%), and surgical risks (563, 50.4%) were considered to be the most influential factors for SDM. 52.7% of all patients hoped experts in different disciplines would participate in SDM. The most desirable amount of time for discussion was 30 to 60 min (562/1118, 50.3%), that were associated with the cost of SDM. Conclusion We can meet the needs of patients by reducing costs and strengthening online patient education and exploring a model suitable for Chinese IBD patients.

Abstract:China is the only country in the world where Western medicine and traditional medicine are practised alongside each other at every level of the healthcare system. Traditional Chinese medicine has a unique theoretical and practical approach to the treatment of disease, which has developed over thousands of years. Traditional treatments include herbal remedies, acupuncture, acupressure and massage, and moxibustion. They account for around 40% of all health care delivered in China. The current government policy of expansion of traditional facilities and manpower is being questioned because many hospitals using traditional Chinese medicine are already underutilized and depend on government subsidies for survival. Research priorities include randomised controlled trials of common treatments and analysis of the active agents in herbal remedies. As more studies show the clinical effectiveness of traditional Chinese medicine, an integrated approach to disease using a combination of Western medicine and traditional approaches becomes a possibility for the future.
Doctor-Patient Communication and Decision-Making Mode in Clinical Application of New Medical Technology in China

Abstract:Objective To analyze the current situation and decision-making mode of communication between doctors and patients in the clinical application of new medical technologies.Methods Based on the two new medical technologies of high-throughput gene sequencing and drug-eluting stent,a survey was conducted among patients and doctors from 19 hospitals in Shanghai,Fujian,and Sichuan by using the cross-sectional design and multi-stage stratified sampling method.Results A total of 950 patients (178 in Cardiology Department,233 in Obstetrics Department and 554 in other departments) and 908 doctors (158 in Cardiology Department,177 in Obstetrics Department and 556 in other departments) from 19 hospitals in 5 cities were investigated.54.9 ％～63.5 ％ of the patients initiatively expressed their demands to doctors,and 52.8％～58.9％ received response from doctors;85.3％～96.7％ of doctors asked opinion from patients,and 41.5％～52.5％ of doctors thought patient's appeal would affect the use of medical technology;83.4％～94.3％ of the patients preferred doctor-patient co-decision,while 85.8％～91.5％ of the patients and 63.9％～91.6％ of the doctors perceived shared decision-making mode.Conclusion The communication between doctors and patients in clinical application of new medical technology needs to be strengthened.Most patients prefered shared decision-making and perceived shared decision-making mode.It was suggested to strengthen the interaction between doctors and patients in term of adoption of new medical technology.

Abstract:Objective Posttraumatic cerebral infarction (CI) is a well-known complication of traumatic brain injury (TBI). However, the causation and apportionment of trauma in patients with CI after TBI is not easy. There is a scoring method, so-called trauma apportionment score (TAS) for CI, consisted with the age, the interval, and the severity of the TBI. We evaluated the reliability of this score. Methods We selected two typical cases of traumatic CI. We also selected consecutive 50 patients due to spontaneous CI. We calculated TAS in both patients with traumatic and spontaneous CI. To enhance the reliability, we revised TAS (rTAS) adding three more items, such as systemic illness, bad health habits, and doctor's opinion. We also calculated rTAS in the same patients. Results Even in 50 patients with spontaneous CI, the TAS was 4 in 44 patients, and 5 in 6 patients. TAS could not assess the apportionment of trauma efficiently. We recalculated the rTAS in the same patients. The rTAS was not more than 11 in more than 70% of the spontaneous CI. Compared to TAS, rTAS definitely enhanced the discriminating ability. However, there were still significant overlapping areas. Conclusion TAS alone is insufficient to differentiate the cause or apportionment of trauma in some obscure cases of CI. Although the rTAS may enhance the reliability, it also should be used with cautions.

Abstract:The decision-making subjects in China include patients, their family members and doctors, which conform to our current cultural characteristics and medical needs.Taking cancer treatment as an example, this paper explored how to draw clear boundaries among the three parties during shared decision-making in the current medical environment, which expected each party fulfill their respective responsibilities and obligations and do not intervene with the reasonable judgments of other parties.The paper holds that doctors should not only provide patients with sufficient professional guidance but also intervene the inappropriate decisions from patients;Patients should not only express personal wishes actively but also cannot interfere with the professional advices from doctors;Family members need to inform patients with disease information based on their features and assist the patient to make the best treatment decisions.The willingness and degree of participation of patients in decision making is generally not actively enough.Both doctors and family members should encourage and cultivate the autonomy of patients.

Abstract:China is the world's most populous country with total population of more than 1.3 billion by the end of 2006 in Mainland China (Table 1). Over recent years its economic development has been remarkable. In tandem with this and as a result of both this increasing prosperity and the one-child policy, its population is getting generally older, has more chronic disorders, and has higher expectations for better health care. Increasing demand for health care and economic liberalism has, in the most prosperous eastern provinces, led to an explosion in acute care. For example Hangzhou City (Eastern of China) with a population of about 7 million has around 60 hospitals with 25 000 beds. In the longer term, focusing healthcare resources on acute hospitals is an expensive option and is likely to exclude the less advantaged from formal health care. Abstract:Abstract Background Although Chinese government has dedicated the past decades to treating chronic diseases by primary healthcare system, many more residents are apt to choose higher-tier facilities to treat minor chronic diseases. Understanding residents’ preferences for chronic disease management in primary care facilities can bridge the gap between residents’ choices and policy implementation. This study aims to elicit residents’ preferences for chronic disease management in primary care facilities in the hypothetical minor chronic disease scenario. Methods Six hundred eighty residents were administered a discrete choice experiment that elicited preferences for chronic disease management in primary care facilities. Services attributes were service mode, treatment measure, out-of-pocket expenditure (OOP), traveling time to healthcare facility and title of physician. Mixed logit models were used to estimate stated preferences and willingness to pay for attributes. WTP confidence intervals were estimated by the delta method. Results A total of 94.44% of the completed questionnaires were valid (680 of 720 respondents). The participants preferred chronic disease management service with modern medicine, traveling time ≤ 30mins, and less OOP expenditure. Compared with Traditional Chinese Medicine (TCM), residents prefer modern medicine, willing to pay 155.53 CNY ($21.97) to change from TCM to modern medicine. Compensation about 86.02 CNY ($12.15) was needed to enable residents to change the choice of the nearer primary care facility to a further one. Integrated medicine in community clinics by experts was residents’ most preferred scenario while TCM in the tertiary hospital was their least preferred one. Conclusion In order to increase the utilization of primary healthcare services in chronic diseases management, policy makers need to concern more about the services of medical treatment type, price and convenience. Therefore, we advise policy makers to provide nearer primary healthcare services for residents especially for residents in surrounding areas. Furthermore, balancing the resource allocation between Traditional Chinese Medicine and modern medicine is worthy of consideration.

Family roles in informed consent from the perspective of young Chinese doctors: a questionnaire study

Abstract:Background: Based on the principle of informed consent, doctors are required to fully inform patients and respect their medical decisions. In China, however, family members usually play a special role in the patient's informed consent, which creates a unique "doctor-family-patient" model of the physician-patient relationship. Our study targets young doctors to investigate the ethical dilemmas they may encounter in such a model, as well as their attitudes to the family roles in informed consent. Methods: A questionnaire was developed including general demographic characteristics, the fulfillment of the obligation to fully inform, who will be informed, and the ethical dilemmas in decision-making. We recruited a total of 421 doctors to complete this questionnaire, of which 368 met the age requirements for this study. Cross tabulation and Pearson's chi-squared test were used to analyze the differences between types of patients for categorical variables, and a p-value < 0.05 was considered statistically significant. Results: Our data shows that only 20 doctors (5.40%) stated "informing the patient alone is sufficient" when it comes to informing patients of their serious conditions. The rest of the participants would ensure that the family was informed. When facing elderly patients with decision-making capacity, the data was statistically different (3.8%; P < 0.001) The primary reason for ensuring that family members be informed differs among the participants. In addition, when family members asked doctors to conceal the patient's medical condition for the best interests of patients, 270 doctors (73.4%) would agree and cooperate with the family. A similar proportion (79.6%) would do so when it comes to elderly patients. Conclusions: (1) Chinese doctors pay extra attention to informing the patient's family, which may not be in the patient's best interests. (2) Chinese doctors treat adult (but not elderly) patients and elderly patients differently when it comes to informing family members. (3) When family members request that doctors withhold information from patients "in the best interest of the patient," the majority choose to comply with the request, although this may cause them distress.

Abstract:Background: With progress being made in the treatment of cancer, various clinical and treatment options are being pursued. In China, Traditional Chinese Medicine (TCM) is used widely in the treatment of cancer. Objective: To estimate TCM treatment preferences and SDM mode of physicians in China. Methods: This study was conducted among physicians (n=185) from nine tertiary hospitals in China by discrete-choice experiment (DCE) survey and Shared Decision-Making Questionnaire-physician version (SDM-Q-Doc) survey. The DCE was developed with the inclusion of the most relevant attributes at appropriate levels for the TCM treatment of lung cancer. The empirical data analyses of physicians were performed using mixed logit models. Additionally, subgroup analysis was conducted. Results: In total, 185 respondents completed the questionnaire. All attributes were statistically significant except out-of-pocket costs. Physicians showed the strongest preferences for increasing disease control rate, relieving nausea and vomiting, and reducing the risk of side effects. Most of the physicians (78.38%) self-reported a high willingness to use SDM during the decision-making process. The physicians with a higher SDM-Q-Doc score had more preference for improving all three attributes than those with a lower score. Little variation was found in preferences among the physicians with other sociodemographic characteristics. Conclusion: In China, physicians considered disease control rate as the most essential attribute in the TCM treatment of lung cancer. The physicians in China mainly preferred SDM, and the preference was different according to SDM mode when involving the TCM therapy for patients with lung cancer. The study findings could inform future TCM therapy for lung cancer and promote SDM.
[Applying Confucian Principles to the Resolution of Clinical Ethical Dilemmas in Taiwan]

Abstract:In Taiwan, the family members of patients are commonly involved in medical decision-making, which undermines patient autonomy and presents an ethical dilemma for medical staff. This issue is rooted in traditional Confucian principles of filial piety and strong family cohesion. Medical professionals understand how morality affects the way of life and attitudes of patients, while the interpretation by healthcare workers of patients' moral values is important for providing professional care. Although patients' wishes for their treatment remain important, family intervention in medical treatment decisions is a common phenomenon in Taiwanese culture. Therefore, medical and nursing staff must carefully consider the opinions of patients and their families in order to resolve ethical dilemmas. The following Confucian principles may be applied to clinical interpretation: Ren refers to the empathy felt toward patients who are in pain, which requires that human behavior is in agreement with ethical requirements; Yi refers to the indignity felt toward violations of morality; Ri refers to medical professional norms (standard operating procedures [SOPs]); and Wisdom refers to making appropriate professional judgments regarding a patient's condition. Advanced discussion of the principle of "Flexibility" states that when an SOP does not match the needs / situation of a patient, it should adjusted appropriately. The principle of "Counsel Heaven and the best of all" implies that medical staff should apply professional care practices in order to prolong the life of patients and help them achieve meaning in their life. By understanding these principles, medical staff may perceive themselves as moral practitioners. Because their vocation involves responsibilities for the patient's life, all of their acts must be consistent with the requirements of Ren. When faced with an ethical dilemma, the timely use of Confucian principles may deepen the value and significance of nursing.
The optimal treatment alternatives selection of integrated TCM and Western medicine based on dynamic conflict group decision-making models

Abstract:Group conflict decision-making is a crucial research direction in group decision making. Most of the existing group conflict decision theories provide models under the condition of static and complete information, but it is urgent to improve the dynamic conflict group decision-making theory with incomplete information to deal with practical management problems. This paper presents a group optimal alternative selection model with dynamic conflict characteristics under incomplete information. Firstly, a directed graph is used to describe the dynamic decision process with incomplete information, and the directed graph is transformed into a generalized conflict information system with incomplete information. Secondly, a group dynamic conflict decision-making model based on the conflict information system is built. Thirdly, dynamic decision rules for decision makers on alternatives under decision attributes are designed, then the largest coalition and the path dependence measurement of decision makers are calculated. Subsequently, taking the optimization decision-making processes of clinical treatment alternatives of integrated traditional Chinese medicine (TCM) and Western medicine as the research problem, the proposed model is applied to verify the applicability and effectiveness by using the electronic medical record (EMR) of 2481 samples. Finally, the solution of the set of clinical optimal treatment alternatives and the path dependence of decision makers on the set of optimal treatment alternatives are presented. The conclusion shows that the distribution of missing information is generally consistent with the practice of clinical decision-making. In addition, the path dependence of decision makers on the optimal alternative is much higher than that on the worst alternative, the conflicts among decision makers are consistent, and decision makers gradually approach the optimal alternative through repeated comparison and verification.

Abstract:Background: Chinese health insurance system faces resource distribution challenges. A patient-centric approach allows decision-makers to be keenly aware of optimized medical resource allocation. Objective: This study aims to use the discrete choice model to determine the main factors affecting the healthcare preferences of the general Chinese population and their weights in the three scenarios (chronic non-communicable diseases, acute infectious diseases, and major diseases). Methods: This study firstly identified the key factors affecting people's healthcare preferences through literature review and qualitative interviews, and then designed the DCE questionnaire. An online questionnaire produced by Lighthouse Studio (version 9.9.1) software was distributed to voluntary respondents recruited from mainland China's entire population from January 2021 to June 2021. Participants were required to answer a total of 21 questions of three scenarios in the questionnaire. The multinomial logit model and latent class model were used to analyze the collected data. Results: A total of 4,156 participants from mainland China were included in this study. The multinomial logit and latent class model analyses showed that medical insurance reimbursement is the most important attribute in all three disease scenarios. In the scenario of "non-communicable diseases," the attributes that participants valued were, from the most to the least, medical insurance reimbursement (45.0%), hospital-level (21.6%), distance (14.4%), cost (9.7%), waiting time (8.3%), and care provider (1.0%). As for willingness to pay (WTP), participants were willing to pay 204.5 yuan, or 1,743.8 yuan, to change from private hospitals or community hospitals to tertiary hospitals, respectively. Conclusions: This study explores the healthcare preferences of Chinese residents from a new perspective, which can provide theoretical reference for the refinement of many disease medical reimbursement policies, such as developing different reimbursement ratios for various common diseases and realizing rational configuration of medical resources.

Abstract:At present, the mature shared decision-making models can be divided into two types according to practice scenarios:those that do not correspond to specific application scenarios and those that are used in specific clinical practices. To implement the concept of shared decision making, different decision aids have been developed and applied. Scales with different perspectives were also designed to assess the impact and effectiveness of the application of the shared decision-making models.The study suggests that the connotation of the shared decision-making model determines its application level. Meanwhile, the shared decision-making model needs to be constructed according to the characteristics of local health care services and diverse decision aids need to be developed to implement the concept. This provides a reference for further development of shared decision-making models and tools adapting to the healthcare system and medical practice in China.

Abstract:Objective: To review the training mode of clinical pharmacists in the United States for improving the training methods of pharmacy professionals in China thus cultivating clinical pharmacists who can effectively guide clinical rational use of drugs. Methods: Taking the Pharm.D. program of the USC school of pharmacy as an example, this paper briefly introduces how the United States develop Pharm.D. students' clinical practice ability and compare this mode with the current clinical pharmacist training mode in China. Results: China's pharmaceutical college curriculum is not reasonable enough. Due to lack of clinical thinking, the undergraduates are not competent to the job of clinical pharmacists. Conclusions: The traditional pharmacy training mode should be changed into a scientific one through improvement of the curriculum, educational system, continuing education, et al. with the objective of cultivating pharmaceutical professionalswith rich clinical experience.

Abstract:Phenomenon: Shared decision making (SDM) is a core ideal in the interaction between healthcare providers and patients, but the implementation of the SDM ideal in clinical routines has been a relatively slow process. Approach: In a sociological study, 71 interactions between physicians and simulated patients enacting chronic heart failure were video-recorded in China, Germany, the Netherlands, and Turkey as part of a quasi-experimental research design. Participating physicians varied in specialty and level of experience. The secondary analysis presented in this article used content analysis to study core components of SDM in all of the 71 interactions and a grounded theory approach to observe how physicians responded actively to patients even though they did not actively employ the SDM ideal. Findings: Full realization of the SDM ideal remains an exception, but various aspects of SDM in physician-patient interaction were observed in all four locations. Analyses of longer interactions show dynamic processes of interaction that sometimes surprised both patient and physician. We observed varieties of SDM that differ from the SDM ideal but arguably achieve what the SDM ideal is intended to achieve. Our analysis suggests a need to revisit the SDM ideal-to consider whether varieties of SDM may be acceptable, even valuable, in their own right. Insights: The gap between the SDM ideal and SDM as implemented in clinical practice may in part be explained by the tendency of medicine to define and teach SDM through a narrow lens of checklist evaluations. The authors support the argument that SDM defies a checklist approach. SDM is not uniform, but nuanced, dependent on circumstances and setting. As SDM is co-produced by patients and physicians in a dynamic process of interaction, medical researchers should consider and medical learners should be exposed to varieties of SDM-related practice rather than a single idealized model. Observing and discussing worked examples contributes to the physician's development of realistic expectations and personal professional growth.

Abstract:Background Although the Hierarchical Medical System has been utilized in China for many years, it is inadequate for guiding patients in adopting appropriate diagnose-seeking behaviors in accordance with related policies. This study examined how patients' diagnose-seeking behavior in tertiary hospitals that is dis-accordance with Hierarchical Medical System related policy (“DSB-dis-accordance”) arise and ways to guide patients away from such behaviors, both from the perspective of physicians. Methods A qualitative study based on a mixed method including in-depth interviews and grounded theory. Twenty-seven physicians with more than 2 years of experience serving in tertiary hospitals of Shanghai were involved after reviewing the related purposes and requirements. Patients' “DSB-dis-accordance” was studied from the perspective of physicians. Results Patient-related factors (habits, trust, and knowledge), physician-related factors (conservative preference, risk avoidance), and system-related factors (accessibility, operability) affected patients' diagnose-seeking behavior. Conclusions Patient-related, physician-related, and system-related factors affecting patients' diagnose-seeking behaviors in tertiary hospitals should be addressed by investing more health resources in lower-level hospitals, enhancing dissemination of health-related and policy-related knowledge, refining the classification of diseases, incentivizing physicians, and developing appropriate follow-up measures. Physicians could then become more involved in guiding patients' “DSB-dis-accordance,” thereby benefitting development of the Hierarchical Medical System in China.

Evaluation and Analysis for Chinese Medical Alliance’s Governance Structure Modes Based on Preker-Harding Model

Abstract:Objective: Analyze and evaluate the typical four medical alliance’s governance modes in China, and construct a set of medical alliance’s governance mode that adapt to the current status of medical resource allocation in China. Theory and Methods: We used interview-based case studies to investigate the four most representative medical alliance modes in China, and conducted in-depth analysis and discussion of key incentives affecting medical alliances under the guidance of the Preker-Harding model framework. Results: The results show that the essence of the relationship between the government as the owner and the medical alliance is the entrustment and adjustment of power and responsibility; the government as a regulator has a normative and universal regulation of the medical alliance; the reform of the medical alliance requires the government to clarify the functional positioning of the medical alliance and determine a reasonable compensation system. Conclusion: China should establish the “Positive Triangle” model of medical alliance’s governance, this medical security model provides patients with various types of medical services in a horizontal dimension, covering a variety of difficult disease treatments in a vertical dimension.
