Abstract:OBJECTIVE To study the effects of ultraviolet (UV) light-treatment on the physicochemical properties and bioactivity of micro-arc oxidation (MAO) titanium surfaces in vitro. METHODS The pure titanium were prepared using MAO. MAO titanium samples were treated with 15 W bactericidal lamp UVC [λ = (250 ± 20) nm] or 15 W mercury lamp [λ = (360 ± 20) nm] for 24 h under ambient conditions. Three sample groups were prepared: MAO, UVA treated after MAO (MAO + UVA), UVC treated after MAO (MAO + UVC). The surface physicochemical properties were evaluated using scanning electron microscopy (SEM), contact angle measuring device, energy dispersive X-ray spectrometer (EDX), and X-ray diffraction (XRD). Bicinchoninic acid (BCA) based colorimetric detection was used to quantify the percentage of albumin adsorption after 2 h, 6 h, and 24 h incubation on the titanium surfaces. The rates of MG-63 cells attached to each group titanium surfaces were calculated by nucleus immunofluorescence using Hoechst 33342 after 1 h, 2 h, and 4 h incubation. SEM was used to observe cell morphology on titanium surfaces in each group. RESULTS No obvious differences in surface topography, TiO(2) crystal and elemental composition were detected on titanium surfaces with or without UV treatment. Statistically significant difference in contact angles among MAO + UVC group (65.34 ± 1.16)°, MAO + UVA group (44.64 ± 1.28)°, and MAO group (3.41 ± 0.48)° were found (P < 0.001). The percentage of albumin adsorption reached the plateau after 2 h incubation on MAO + UVC titanium surfaces (48.16 ± 1.24)%, which was higher than those in MAO [(8.22 ± 2.99)%] and MAO + UVA groups [(5.29 ± 2.27)%, P < 0.001]. The rates of cells attached to the surfaces of MAO + UVC titanium was greater than that on MAO surfaces and MAO + UVA surfaces after 1 h [(40.71 ± 4.08)%], 2 h [(53.72 ± 2.38)%], 4 h [(70.32 ± 2.85)%] incubation (P < 0.05). The MAO + UVC surfaces remarkably enhanced the spread of MG-63 cells, however, there was no significant difference between the group of MAO and MAO + UVA. CONCLUSIONS Pretreatment of micro-arc oxidation titanium with UVC light considerably improved the surface bioactivity to MG-63 cells, which showed an increase in cellular attachment and spread.
Stillbirth - transitions and rituals when birth brings death: Data from a danish national cohort seen through an anthropological lens

Abstract:Background: Many parents bereaved of a stillborn baby spend time with the child. In this time frame, different acts with the child in focus may occur. Some parents invite others to see the child too. Parents who suffer the loss of a newborn are vulnerable, and understanding acts and practices surrounding the dead newborn is important knowledge for caretakers. Aims: This article aims to enlighten the amount of time Danish parents spend with their stillborn in hospital settings that encourage this practice. Furthermore, it aims to transcend the mere quantitative numbers through a theoretical approach that frames the analysis and discussion of possible layers of meaning imbedded in time spent with a dead newborn. Study design: Data from a Danish cohort of bereaved parents were collected using web-based questionnaires. These numbers were successively interpreted through an anthropological lens within the perspective of transition and ritualisation. Knowledge from existing empirical literature was also fused. Results from the cohort: Danish parents spend hours or days with their stillborn child. They feel supported in this by healthcare professionals. Mainly close relatives join the parents while admitted to the hospital to see the stillborn child, followed by other family members and friends. Conclusion: Danish parents engage to a very high degree in contact with their dead baby. The analysis points out that 'Time' and 'Others' are needed to create a socially comprehensible status for parents and child when birth brings death. In liminal space during the transition, healthcare professionals act as ritual experts, supporting parents and their relatives to ascribe social status to the dead body of the child through ritualised acts. Instead of only thinking of this period as 'memory-making', we suggest regarding it as a time of ontological clarification as well.

Abstract:Termination of pregnancy due to a fetal anomaly is a challenging and mentally stressful experience, associated with trauma-related stress, depression and symptoms of complicated grief. It differs from other types of pregnancy loss because of the role of active decision-making. Expectant parents depend on caring and experienced staff to guide them during a time full of ambiguity and contrasting emotions. This study examines how expectant parents experience the process of birth with termination of pregnancy due to fetal anomaly at a specialised unit for prenatal loss. Data was generated from semi-structured interviews with 11 women and 9 male partners, one to five months after termination of pregnancy. Three themes were identified 1) Suspended time/space bubble ; the unit for prenatal loss was described as a time-space bubble, that promoted a sense of meaning, a feeling of togetherness and an awareness of parallel worlds, 2) The midwife matters ; participants emphasised midwives' 'invisible', chronological and individualised practice, making it possible to be present and in the process, and 3) Meeting and spending time with the fetus/baby ; participants' responses and relation to the fetus were very different, with most of them embracing the fetus as a baby, grateful for the opportunity to have time together. The findings from this study suggest a need for specialised midwives dedicated to prenatal loss, in units designed as private and protected spaces to accommodate the requirements of parents undergoing termination of pregnancy due to fetal anomaly.

Abstract:Abstract Background Bereaved parents experience higher rates of depressive and post-traumatic stress symptoms after the stillbirth of a baby than after live-birth. Yet, these effects remain underreported in the literature and, consequently, insufficiently addressed in health provider education and practice. We conducted a participatory based study to explore the experiences of grieving parents during their interaction with health care providers during and after the stillbirth of a baby. Methods This community-based participatory study utilized four focus groups comprised of twenty-seven bereaved parents (44% fathers). Bereaved parents conceptualized the study, participating at all stages of research, analyses, and drafting. Data were reduced into a main theme and subthemes, then broad-based member checked to ensure fidelity and nuances within themes. Results The major theme that emerged centered on provider acknowledgement of the baby as an irreplaceable individual. Subthemes reflected 1) acknowledgement of parenthood and grief, 2) recognition of the traumatic nature of stillbirth, and 3) acknowledgement of enduring grief coupled with access to support. It was important that providers realized how grief was experienced within health care and social support systems, concretized by their desire for long-term, specialized support. Conclusions Both mothers and fathers feel that acknowledgement of their baby as an individual, their parenthood, and their enduring traumatic grief by healthcare providers are key elements required in the process of initiating immediate and ongoing care after the stillbirth of a baby.

Abstract:Qualitative Health Research, Ahead of Print. When an infant dies in a neonatal intensive care unit in Norway, healthcare professionals provide bereaved parents with objects intended to help them processing their loss. Such objects can be clothes, blankets, soft animal toys, hand- and footprints, hair, as well as scrapbooks where the short life is documented through text and photo. By interviewing bereaved parents in three focus groups, we investigated the parents' use of these objects. Applying the method of reflexive thematic analysis, we developed three themes from the data material: (i) the importance of preserving objects, (ii) the approach to the objects, and (iii) the ambivalence concerning the objects. Pertinent to all themes was the parents' feeling of ambivalence toward the objects. On the one hand, the parents experienced the objects to affirm parenthood and manifest that the infant existed as a family member. Further, the objects were important in ritualization while according the child its status as deceased. Also, the objects helped the bereaved establish and keep continuing bonds with the deceased and to integrate their traumatic experience of losing a child. On the other hand, the bereaved parents shared that they were ambivalent toward the objects as they stirred up both good and painful emotions. The objects reminded them of their shocking and traumatic loss and the bereaved did not want to be confronted with this all the time. Therefore, through a preference for some objects and indifference toward others as time passed, the parents worked on transforming their bonds with the lost infant.

Parental Experiences of Grief after Pregnancy Loss: systematic review of qualitative studies

Abstract:Introduction Gestational Loss represents a set of abrupt and unexpected losses throughout pregnancy or after childbirth. Every year, around two million babies die after 28 weeks of gestation, with between 14% and 20% of all pregnancies ending in loss. In most situations, pregnancy loss occurs in a pregnancy without signs of risk or irregularities, something that increases the shock and suffering felt by parents. Objectives The present study aims to understand the relationship between pregnancy loss and parents' grief experiences after spontaneous abortion, stillbirth or neonatal death with qualitative evidence. Methods This review followed the principles of PRISMA, and the search was carried out in the Web of Science and Scopus databases, aiming to find relevant articles about parental grief experiences resulting from pregnancy loss, published between 2012 and 2022. After research and analysis Of the studies, 15 qualitative studies were included. Results The pain and sadness when experiencing the loss of a child was a common point in all the studies found. In this review, the majority of men revealed a duality in wanting to protect, physically and emotionally, their partner, while experiencing their own grief, something that led to the internalization of their emotions and the minimization of their pain. Grieving fathers and mothers report experiencing this process alone, describing the difficulty in expressing what they feel due to the lack of recognition of the loss. It was found that confrontation with other pregnant women leads bereaved parents to reveal jealousy and shame, as well as feelings of guilt. The farewell rituals, the process of writing and talking about their experience helped the women to not feel so alone and to find a purpose: to transform their pain and help other grieving mothers. Fathers and mothers who experienced pregnancy loss stated that the death of their child provided change and growth. Conclusions After Pregnancy Loss, adapting to the new reality is extremely painful, despite the work of mourning being necessary and crucial. This process is a search to integrate and accept the reality of the loss of the baby in a way that has meaning for the mother and father, it is the adaptation to a world without the lost child and to a relationship that had been built during the gestation period, which was violently broken. Abstract:Background: From birth the child has an ability to respond to the environment, which influences the interaction between mother and child. If this attachment is interrupted, the child's emotional development is negatively influenced. When the child needs care in a neonatal intensive care unit (NICU) it is difficult to establish contacts between mother and child. Separation from the child is found to be the most difficult aspect for mothers when their newborn child is hospitalized in a NICU. Aim: The aim of this study was to describe mothers' experiences when their full-term newborn child was cared for in a NICU during the postpartum maternity care period. Method: A phenomenological hermeneutic interview study was performed. Ten mothers were interviewed once, 6 months to 6 years after the experience. Results: The essence of the experience is understood as an alternation between two opposite concepts, exclusion and participation, with emphasis on exclusion. A feeling of exclusion dominates when the new mother feels a lack of interaction and a sense of not belonging to either the maternity care unit or the NICU. This has a negative effect on her maternal feelings. On the contrary, when a feeling of participation dominates, a continuous dialogue exists and the mother is cared for as a unique person with unique needs. This supports her maternal feelings in a positive direction. The implication of the result for nurses is that it is important to decrease mothers' experience of exclusion and to increase their feeling of participation when their child is cared for in a NICU. A return visit to the responsible nurse to go through the treatment and experiences should be offered to all parents whose child has been cared for in a NICU.

Abstract:Objective The study aimed to explore experiences of extremely preterm infant loss in the delivery room and perspectives about antenatal consultation. Study Design Bereaved participants were interviewed, following a semi-structured protocol. Personal narratives were analyzed with a mixed-methods approach. Results In total, 13 participants, reflecting on 17 pregnancies, shared positive, healing and negative, harmful interactions with clinicians and institutions: feeling cared for or abandoned, doubted or believed, being treated rigidly or flexibly, and feeling that infant's life was valued or not. Participants stressed their need for personalized information, individualized approaches, and affective support. Their decision processes varied; some wanted different things for themselves than what they recommended for others. These interactions shaped their immediate experiences, long-term well-being, healing, and regrets. All had successful subsequent pregnancies; few returned to institutions where they felt poorly treated. Conclusion Antenatal consultations can be strengthened by personalizing them, within a strong caregiver relationship and supportive institutional practices. Key Points

Abstract:This study aimed to investigate the understanding and experiences of mothers who have suffered perinatal loss regarding their needs after being discharged from the hospital. Data were collected through semi-structured face-to-face and telephone interviews using purposive sampling with maximum diversity. The sample included 15 mothers and 6 midwives who had experienced fetal loss and perinatal bereavement. The interviews were audio-recorded and transcribed verbatim. Conventional content analysis was used to analyze the data. Sampling was conducted from July 2022 to March 2023, continuing until data saturation was reached. The study identified 2 main categories of needs for mothers who have experienced perinatal loss: continuous healthcare (including the Comprehensive Mother Tracking System, provision of psychological welfare, and educational needs) and provision of a support network, including “spouse and family support” and “peer and colleague support.” The primary theme of this study was “Deliberating Care for Grieving Mothers.” The study results indicate that mothers who experience perinatal loss require continuity of care after being discharged. Psychological screening and counseling support are essential for both parents. Considering the significant impact of spousal, familial, and community support on an individual’s life, it is crucial to prepare the community to comprehend and embrace grieving parents.

Abstract:ABSTRACT The rapid, broad-spectrum, biofilm-eradicating activity of the combination of 0.01% nitroglycerin, 7% citrate, and 20% ethanol and its potential as a nonantibiotic, antimicrobial catheter lock solution (ACLS) were previously reported. Here, a nitroglycerin-citrate-ethanol (NiCE) ACLS optimized for clinical assessment was developed by reducing the nitroglycerin and citrate concentrations and increasing the ethanol concentration. Biofilm-eradicating activity was sustained when the ethanol concentration was increased from 20 to 22% which fully compensated for reducing the citrate concentration from 7% to 4% as well as the nitroglycerin concentration from 0.01% to 0.0015% or 0.003%. The optimized formulations demonstrated complete and rapid (2 h) eradication of methicillin-resistant Staphylococcus aureus (MRSA), vancomycin-intermediate Staphylococcus aureus (VISA), methicillin-resistant Staphylococcus epidermidis (MRSE), vancomycin-resistant enterococci (VRE), multidrug-resistant (MDR) Pseudomonas aeruginosa, MDR Klebsiella pneumoniae, MDR Enterobacter cloacae, MDR Acinetobacter baumannii, MDR Escherichia coli, MDR Stenotrophomonas maltophilia, Candida albicans, and Candida glabrata biofilms. The optimized NiCE lock solutions demonstrated anticoagulant activities comparable to those of heparin lock solutions. NiCE lock solution was significantly more effective than taurolidine-citrate-heparin lock solution in eradicating biofilms of Staphylococcus aureus and Candida glabrata. The optimized, nonantibiotic, heparin-free NiCE lock solution demonstrates rapid broad-spectrum biofilm eradication as well as effective anticoagulant activity, making NiCE a high-quality ACLS candidate for clinical assessment.

Abstract:Objectives We aimed to explore the lived experiences and needs of women after a recent stillbirth event. Design Qualitative phenomenological study. Setting The current study was conducted in a tertiary obstetric hospital in East China between 25 January 2024 and 29 March 2024. Participants 14 women having experienced a stillbirth within the last 6 months. Results Researchers agreed on four key themes including individual variations in emotional reaction and recovery, physical recovery and concerns about future pregnancies, the critical role of social support systems and variations in perceptions of stillbirth as the death of a fetus versus a human being, along with related mourning rituals. These themes collectively highlight the multifaceted nature of the stillbirth experience, underscoring the complex interplay between personal, cultural and medical factors that shape women's emotional and physical responses. Conclusions Post-stillbirth experiences among Chinese women are deeply individualised and influenced by a complex interplay of personal emotions, cultural contexts and medical interactions. It is imperative for healthcare systems to implement tailored care strategies beyond standard protocols to proactively address their varied emotional landscapes and physical concerns with an enhanced awareness of cultural sensitivities. Specialised training for healthcare providers should be devised to recognise and respond to the unique grief processes. Comprehensive support systems should be established to significantly enhance the recovery journey by providing essential resources and community connections.

Abstract:An investigation of La and Ca doped Sr4Ru3O10, featuring a coexistence of interlayer ferromagnetism and intralayer metamagnetism, is presented. La doping readily changes magnetism between ferromagnetism and metamagnetism by tuning the density of states. It also results in different Curie temperatures for the c-axis and the basal plane, highlighting a rare spin-orbit coupling with the crystal field states. In contrast, Ca doping enhances the c-axis ferromagnetism and the magnetic anisotropy. La doping also induces a dimensional crossover in the interlayer transport whereas Ca doping exhibits a tunneling magnetoresistance and an extraordinary T 3/2 dependence of the resistivity. The drastic changes caused by the doping demonstrate a rare borderline magnetism that is delicately linked to the interplay of the density of states and spin-orbit coupling.
A comparison of mothers' and fathers' experiences of the attachment process in a neonatal intensive care unit

Abstract:Aim: To compare mothers' and fathers' individual views and experiences of the attachment process in a neonatal intensive care unit within the first week after a premature birth. Background: The attachment between parents and children is a precursor to the consolidation of parenting skills, the growth and development of the infant and the establishment of a bond between parent and child. Premature birth and the resultant hospitalization disrupt the normal attachment process between parent and child. Most of the literature on attachment theory focuses on the mother-child connection and is being criticised for regarding the father's role as supportive and peripheral. Methods: The design of this study was descriptive with a hermeneutic approach. Twelve parents (six mothers and six fathers) in a 13-bed neonatal intensive care unit in a Norwegian regional hospital participated in a field study addressing the encounter between parents and nurses. This paper is based on the semi-structured interviews with the parents at discharge. Results: The interview analysis revealed two main categories. (a) Taken by surprise: For mothers, the premature birth created a feeling of powerlessness and they experienced the immediate postnatal period as surreal and strange. The fathers experienced the birth as a shock, but were ready to be involved immediately. (b) Building a relationship: Mothers experienced a need to regain the temporarily lost relationship with their child, whereas the fathers experienced the beginning of a new relationship. Conclusion: Comparing parents' experiences of the attachment process within the first days after a premature birth reveals a striking contrast between the mother's experience of surrealism and the father's ability to be involved immediately after birth. Relevance to clinical practice. Parents' of premature children's different starting points should be acknowledged as professionals encourage parents to have early skin-to-skin contact with their premature infant.

Abstract:This paper examines the notion of personhood through existing studies of miscarriage. It explores cross‐culturally how the concept of baby developes. One such example is that ‘a baby’ is a product of ‘Western technology’. Therefore, miscarriage may be viewed as a ‘failure of technology’, and the miscarriage may remain unacknowledged. Alternatively, if miscarriage is not a baby but redefined as ‘a foetus ‘ or a collection of cells, it again becomes invisible as the personhood of the baby has been denied. In the UK since the mid‐1980s, personhood in miscarriage has been acknowledged in research and policy in the context of limited resources or broader definitions of preventive health care, where the notion of life has been debated. In health practice, if one argues that life is continuous rather than having definite points, there are different implications for care. Having fixed points in what is a continuous world may be helpful for legal convenience in delimiting hospital policy, such as policy relating to foetal experimentation, in‐vitro fertilisation, brain death or donor organs, but this may be unhelpful at the practice level in care of the woman, the partners or the family, during the resolution of grief. This may present a conflict for the nurse who in the UK is seen as managing ambiguous states: the re‐entry of the person from a state of sickness or abnormality into the social world. The new age of biologically assisted pregnancy and its loss requires thought about how provision is made taking into account the renegotiated family and kinship patterns that are emerging, and how ‘suspended’ potential life in in‐vitro fertilisation both enhances and despoils life.

Abstract:AIM To explore the experiences of Taiwanese mothers of stillborn babies, their coping strategies, and the ways in which their subsequent human interactions are shaped by Taiwanese culture. BACKGROUND Giving birth to a male family heir is still considered an important responsibility for Taiwanese women. The ideology of continuity is still at the centre of Chinese family life in which women are expected to deliver babies that will continue their husbands' family lines. Research on the connection between this responsibility and women's reaction to stillbirth in a cultural context is limited. Understanding Taiwanese women's experiences with stillbirth will add to the body of nursing knowledge, especially in terms of giving culturally competent nursing care. METHODS Interpretive ethnographic procedures were used to locate coping strategies. Twenty mothers who had stillbirth experiences were interviewed periodically during the 2 years following their losses. Following Agar's example, data were analysed to identify themes representing coping strategies. FINDINGS The four major themes identified were (a) transforming the meaning of death, (b) doing something for the deceased, (c) anticipating another pregnancy, and (d) rebuilding a social fabric. CONCLUSION The processes that the mothers of stillborn babies experience represent transformations that involve engagement with cultural expectations for woman. The authors suggest that these cultural impacts should be incorporated into nursing assessment and treatment practices.

Abstract:Introduction The death of a child has a tremendous impact on parents' lives. The experience of parents who have lost a child to cancer may differ from other bereavement experiences, including other childhood and adulthood causes of death, because of the uncertainty of the prognosis, the aggressive treatment, and the potential for regret about treatment decisions. Bereavement care remains scarce, and effective interventions to meet the diverse needs of parents have not been defined. Objective To provide insights on bereaved parents' experiences, their needs, and wishes of support following the loss of their child to cancer. Methods We conducted 18 qualitative, in‐depth, semi‐structured interviews with 23 bereaved parents (seven fathers, 16 mothers), and used reflexive thematic analysis to analyze the data. Results Parents received both informal and professional support. Regarding informal support, parents expressed a high level of ambivalence rooted in grief illiteracy. Parents also recognized their own struggles to express what forms of informal support they would have liked to receive. Support provided by healthcare professionals, institutions, and organizations involved in the children's care or in bereavement was inconsistent due to personnel time constraints, interpersonal relationships, or disruptions due to the COVID‐19 pandemic. These factors could result in parents not having access to certain forms of support or not receiving long‐term support. Conclusion Improving grief literacy may strengthen informal support and make discussions of grief and death less taboo. Institutional policies, training, and networking may help to ensure that support provided by healthcare professionals, institutions, and organizations is less vulnerable to inconsistencies.

Separation in Unity: Dialogical Transformation of Maternal Bond

Abstract:The objective of this paper is to explore mothers' psychological adaptation related to separation from their newborn children immediately after birth. Mother and child separation has traditionally received attention from the child's perspective, but given that the bond is dyadic, the mother's wellbeing should also be considered. This qualitative study is based on interview data with mothers of premature infants. From the analysis, three themes emerged: concerns during pregnancy about premature birth; emotional strain caused by separation; and the need to protect and to be close to the newborn. Drawing on the perspectives of dialogical self theory and semiotic regulation model, the paper will focus on intra-psychological dynamics and will analyze the adaptation process in terms of I-positions. Based on the analysis, the maternal bond represents the integration of the I-position I-as-mother and My child, which is integrated during the pregnancy into the core of the self. It can be assumed that rupture of the self-continuity is activated by the birth of the child, which is then compounded if the child leaves. The maternal superordinate viewpoint (meta-I-position) directs a woman's behavior and allows for a sense of coherence under dynamic organization related to the birth of the child. The potential for psychological adaptation is presented as an ability to establish self-continuity.
