Abstract:Sleep, sleepiness, and dreaming are expressed throughout Dante Alighieri's (1265-1321) the Divine Comedy from the start of his journey through the afterlife. In the book, Dante complains that he is "full of sleep," and he experiences sudden wake-dreaming transitions, short and refreshing naps, visions and hallucinations, unconscious behaviors, episodes of muscle weakness, and falls which are always triggered by strong emotions. Taken together these signs are highly reminiscent of narcolepsy, a term coined in 1880 by Gélineau to define a disease consisting of daytime irresistible sleep episodes with remarkable dream mentation, sleep paralysis, hallucinations, and cataplexy (falls triggered by strong emotions). Sleep, sleepiness, and episodes of sudden weakness triggered by emotions are Dante's literary fingerprints from his earliest works, pointing to a lifelong autobiographic trait. In the 19th century, Cesare Lombroso speculated that Dante had epilepsy, as he had suffered from frequent spells and hallucinations. However, the multiple emotionally triggered falls Dante experienced in the Divine Comedy contrast with the epileptic seizure he depicted in one of the damned individuals. It is possible that Dante may have intuitively grasped the main features of narcolepsy, but it also is plausible that Dante's sleep, dreams, hallucinations, and falls are clues to a lifelong pathologic trait and that Dante may have known of or had narcolepsy.
Prevalence of Chlamydia trachomatis Genital Infection Among Persons Aged 14–39 Years — United States, 2007–2012

Abstract:Infection with the bacterium, Chlamydia trachomatis (often termed "chlamydia") is the most frequently reported sexually transmitted infection in the United States. The urethra is the most common site of infection in males, and the urethra and cervix are most commonly infected in females. Ascending infection in females can cause pelvic inflammatory disease, which can lead to infertility and ectopic pregnancy. Genital chlamydial infections are usually asymptomatic, and screening is necessary to identify most infections. Currently, chlamydia screening for sexually active women aged <25 years is recommended by the U.S. Preventive Services Task Force (grade B recommendation). Chlamydia is nationally notifiable; however, if females do not access care or clinicians do not screen, many infections go undiagnosed, unreported, and untreated. CDC monitors population prevalence of genital chlamydial infection through the National Health and Nutrition Examination Survey (NHANES), which tests a sample of the U.S. population aged 14-39 years for genital C. trachomatis and found that the overall chlamydia burden in the United States decreased during 1999-2008. Using data from the most recent cycles of NHANES (2007-2012), CDC estimated chlamydia prevalence among persons aged 14-39 years overall and by demographic characteristics and sexual behaviors. The prevalence of chlamydia among persons aged 14-39 years was 1.7% (95% confidence interval [CI] = 1.4%-2.0%). Chlamydia prevalence varied by age and race/ethnicity, with prevalence highest among non-Hispanic blacks (5.2%). Among sexually active females aged 14-24 years, the population targeted for routine screening, chlamydia prevalence was 4.7% overall and 13.5% among non-Hispanic black females. As chlamydia is common and infections are usually asymptomatic, health care providers should routinely screen sexually active young women aged <25 years for chlamydial infection, provide prompt treatment for infected persons, and ensure that infected patients' sex partners receive timely treatment to prevent reinfection.

Abstract:Abstract Background. Long-term recession of the Japanese economy during the 1990s led to growing social inequalities whilst health inequalities also appeared. The 2007 National Cancer Control Program of Japan targeted “equalisation of cancer medical services”, but the system to monitor health inequalities was still inadequate. We aimed to measure socioeconomic inequalities in cancer survival in Japan. Material and methods. We analysed 13 common invasive, primary, malignant tumours diagnosed from 1993 to 2004 and registered by the population-based Cancer Registry of Osaka Prefecture. An ecological socioeconomic deprivation index based on small area statistics, divided into quintile groups, was linked to patients according to their area of residence at the time of diagnosis. We estimated one-, five-year and conditional five-year net survival by sex, period of diagnosis (1993–1996/1997–2000/2001–2004) and deprivation group. Changes in survival over time, deprivation gap in survival, and change in deprivation gap were estimated at one and five years after diagnosis using variance-weighted least square regression. Results. The deprivation gap in one-year net survival was narrower than in five-year net survival and conditional five-year survival. During the study period, there was no change in deprivation gap, except for reductions for pancreas (men) and stomach (women), and an increase for lung (men) in one-year survival. We observed a linear association between level of survival and deprivation gap at five years and conditional five years, but no association at one-year survival. Conclusion. A wide deprivation gap in survival was observed in most of the adult, solid, malignant tumours, within the universal healthcare system in Japan. Overall, cancer survival improved in Osaka without any widening of inequalities in cancer survival in 1993–2004, shortly after the long-term economic recession and deep modifications in the social and work environments in Japan. The longer term impact of the recession on inequalities in cancer survival needs to be monitored using population-based cancer registry data.

Abstract:The article examines the famous literary and artistic work of the Renaissance poet Dante Alighieri "Divine Comedy". In the Comedy a special place is occupied by the musical content, which enriches the literary images of the afterlife. Many works of Western researchers pay attention to the "philosophy of music" of Dante's work. The opinions of various scholars who single out the following musical references are considered: songs and musical instruments, liturgical sung texts and psalms, the presence of monophonic and polyphonic sounds, the reflection of the Boethian principles: musica mundana, humana, instrumentalis and assumptions about the poet's own vision of the development of music. Authors saw the relation with words used to designate the music itself. They have focused on moments of structural and literary relations in these sacred texts respect to each other, analysed the "singing" relationship between heroes of Comedy. The considered a set of key cases resonates with music and sound: infernal dissonance, cacophony emphasizes the immorality Inferno; the middle part Purgatorio, is the central part of Comedy is the musical bridge between the “anti-music” of Inferno and the divine music of Paradiso. The music running these canticas throughout psalms and hymns voiced by choirs of souls. Recent scholarship on the literary aspects and musicology enables us as one level up of the meaning of Comedy. This philosophical and metaphysical aspects of Dante`s thought maybe give us information about music life in 14th century. Interesting material is supplemented by quotations from the Ukrainian translation of Maxim Strikha, which preserves the poetics of speech and sound means by which Dante tried to portray his visions.

Abstract:Dante Alighieri’s Divine Comedy presents a large-scale picture of the afterworld that draws on prior tradition of depicting life after death. At the same time, the afterworld of the Divine Comedy is unique, being marked by numerous distinctions with the many similar descriptions in medieval literature: 1. The general nature of the depiction; the summa genre with its striving for universal coverage, classification, scholarship (references to authorities, use of contemporary knowledge), and moral and didactic impact. 2. The close connection of the otherworld with the temporal world – precise description of its location and structure, its connection with the earthly space. The otherworld is inhabited by similar characters, who emphasise their connection with the earthly space (they ask about their friends and relatives and their native city, invite Dante to tell his own story, and beg for his prayers). 3. Personal approach: rather than being illustrations of virtue or sin, the work’s characters are concrete people with their own fates both in the temporal world and in the afterlife. This leads to a powerful dramatic effect that sweeps the reader away and involves him in situations that he applies to his own life. Dante himself plays a double role in the Divine Comedy as both author and character. 4. Combination of “literary” and mystic experience.

Abstract:Giovanni Boccaccio's Decameron contains a novella that details the sudden death of a young man called Gabriotto, including a portrayal of the discomfort that the protagonist experienced and a rudimentary autopsy performed by local physicians. The intriguing description of symptoms and pathologies has made it possible to read a 7-century-old case through the modern clinical lens. Thanks to the medical and philological analysis of the text-despite the vast difference between modern and medieval medicine-2 hypothetical diagnoses have emerged: either an aortic dissection or an atrial myxoma.
Our Experience with Nifedipine and Prednisolone as Expulsive Therapy for Ureteral Stones

Abstract:Objectives: We evaluated the effect of nifedipine associated with prednisolone in ureteral stone passage. Material and Methods: In our department we enrolled 50 patients with radiopaque ureteral stones. Stone size was 15 mm or less. The patients were divided into two groups: group I included 25 patients who received 30 mg oral treatment of slow – release nifedipine (for a maximum of 20 days) and 25 mg of prednisolone (for a maximum of 10 days) daily. Group II was made up of 25 patients who received 25 mg of prednisolone daily. On request, both groups could use non-steroidal anti-inflammatory drugs. Results: The mean expulsion time was 6 days in group I and 10 days in group II. The average stone size was 12 mm in group I and 12.8 mm in group II. Six patients suspended therapy in group I (5 erythema, 1 stomachache), and seven in group II (3 because of intolerable pain, 4 stomachache). The expulsion success rates were 68% in group I and 81% in group II. Conclusions: For ureteral stones that do not cause an emergency situation, such as obstructive uropathy, infection or intolerable pain, we suggest expulsive medical treatment with nifedipine and prednisolone, if there are no contraindications to drug use.

Abstract:Poetry elicits emotions, and emotion is a fundamental component of human ontogeny. Although neuroaesthetics is a rapidly developing field of research, few studies focus on poetry, and none address its different modalities of fruition (MOF) of universal cultural heritage works, such as the Divina Commedia (DC) poem. Moreover, alexithymia (AX) resulted in being a psychological risk factor during the Covid-19 pandemic. The present study aims to investigate the emotional response to poetry excerpts from different cantica (Inferno, Purgatorio, Paradiso) of DC with the dual objective of assessing the impact of both the structure of the poem and MOF and that of the characteristics of the acting voice in experts and non-experts, also considering AX. Online emotion facial coding biosignal (BS) techniques, self-reported and psychometric measures were applied to 131 literary (LS) and scientific (SS) university students. BS results show that LS globally manifest more JOY than SS in both reading and listening MOF and more FEAR towards Inferno. Furthermore, LS and SS present different results regarding NEUTRAL emotion about acting voice. AX influences listening in NEUTRAL and SURPRISE expressions. DC’s structure affects DISGUST and SADNESS during listening, regardless of participant characteristics. PLEASANTNESS varies according to DC’s structure and the acting voice, as well as AROUSAL, which is also correlated with AX. Results are discussed in light of recent findings in affective neuroscience and neuroaesthetics, suggesting the critical role of poetry and listening in supporting human emotional processing.

Abstract:Dante Alighieri died in 1321; therefore, 2021 is the 700th anniversary of his death. His best known work is the Divine Comedy, which explores Dante’s journey through the three realms of the underworld. Each realm is associated with three different sensations: Inferno , bodily (pain); Purgatorio, acoustic (music); and Paradiso , visual (light). The progression of the painful experiences and the resolution through music and light mirror the modern understanding of pain and its management. Music has both direct and indirect benefits and can help with coping. Sunlight promotes well-being and self-awareness.

Abstract:With few exceptions, an inverse relationship exists between social disadvantage and disease. However, there are conflicting data for the relationship between socioeconomic status (SES) and BMD. The aim of this study was to assess the association between SES and lifestyle exposures in relation to BMD. In a cross‐sectional study conducted using 1494 randomly selected population‐based adult women, we assessed the association between SES and lifestyle exposures in relation to BMD. BMD was measured at multiple anatomical sites by DXA. SES was determined by cross‐referencing residential addresses with Australian Bureau of Statistics 1996 census data for the study region and categorized in quintiles. Lifestyle variables were collected by self‐report. Regression models used to assess the relationship between SES and BMD were adjusted for age, height, weight, dietary calcium, smoking, alcohol consumption, physical activity, hormone therapy, and calcium/vitamin D supplements. Unadjusted BMD differed across SES quintiles (p < 0.05). At each skeletal site and SES index, a consistent peak in adjusted BMD was observed in the mid‐quintiles. Differences in adjusted BMD were observed between SES quintiles 1 and 4 (3–7%) and between quintiles 5 and 4 (2–7%). At the spine, the maximum difference was observed (7.5%). In a subset of women, serum 25(OH)D explained a proportion of the association between SES and BMD (difference remained up to 4.2%). Observed differences in BMD across SES quintiles, consistent across both SES indices, suggest that low BMD may be evident for both the most disadvantaged and most advantaged.

Abstract:The power of Dante's Divine Comedy is unmistakable, but surprising in view of its theological structure and assumptions that are no longer current among most modern readers. This paper suggests that its power derives from the deep psychological truthfulness with which Dante deals with the painful personal crisis that underlies the poem and is his starting point. It attempts to clarify what may have constituted that crisis, and why the structure of the Comedy, and in particular its use of two guides, Virgil and Beatrice, who might be thought a somewhat incompatible pairing, point significantly to the nature of the solution Dante arrived at. In particular it suggests that the puzzling fictions to do with Statius in the Purgatorio are a clue to Dante's own difficulties in bridging the classical and Christian traditions, and that his highly original solution to these difficulties, by no means conforming to conventional Christian orthodoxy in the 13th/14th century, was needed with special urgency in a time of pervasive civil conflict.

Abstract:Dante Alighieri (1265–1321) was a poet whose work was influenced both by his political experiences and by his study of philosophy. Born in Florence, he was educated in grammar and rhetoric and reports in his Convivio that he studied philosophy at the “schools of the religious,” probably the studia at Florence’s Franciscan and Dominican houses. Until his exile, he was an active participant in civic life, as a guild member, communal councilor, and prior. He was exiled in early 1302 after his faction, the Whites, was replaced by the Blacks (with the not-so-covert support of Boniface VIII). He never returned to Florence, dying in Ravenna in 1321.Except for the early collection of lyric poems, the Vita nuova, most of his works were composed during his exile. Convivio is an unfinished encyclopedic work on human knowledge. Monarchia argues for a single universal ruler, who should be the Roman emperor and not the pope. The Divine Comedy brings together many of these philosophical and political themes, transformed into a masterpiece of imaginative literature.

Abstract:OBJECTIVE Epidemiological studies and randomized clinical trials have demonstrated in various populations that resting heart rate (RHR) was an independent predictor of cardiovascular (CV) risk and all-cause mortality. However, few data specifically evaluated the relationship between RHR and long-term CV and renal complications in a large population of type 2 diabetic (T2D) patients. RESEARCH DESIGN AND METHODS We performed a single-center, prospective analysis in 1,088 T2D patients. RHR was determined at baseline by electrocardiogram. The primary outcome was a composite criterion of CV and renal morbi-mortality (CV death, nonfatal myocardial infarction and/or stroke, hospitalization for heart failure, renal replacement therapy), which was adjusted for death from non-CV cause as a competing event. The secondary outcome was a renal composite criterion (renal replacement therapy or doubling of baseline serum creatinine) adjusted for all-cause death as a competing event. RESULTS During median follow-up of 4.2 years, 253 patients (23%) and 62 patients (6%) experienced the primary and secondary outcomes, respectively. In the subgroup of patients with CV disease history at baseline (n = 336), RHR was found to be associated with the incidence of primary outcome (P = 0.0002) but also with renal risk alone, adjusted for all-cause death as a competing event (secondary outcome; P < 0.0001). In patients without history of CV disease, no relation was found between RHR and the incidence of CV and/or renal events. CONCLUSIONS In the real-life setting, RHR constitutes an easy and less time-consuming factor that would permit identification of CV disease diabetic patients with an increased risk for long-term CV and renal complications.

Abstract:Placing the words Comedy and Africa in the same sentence, is like laying claim to two expansive and complex entities which do not immediately bear relation to another and yet, there is ample opportunity for engagement. The article begins by showing how a young South African’s reading of the Divine Comedy through the lens of her own preoccupation with the body and its theo-performative demeanour can bring fresh perspectives to the fore. A primary instance of the intersection between the body, God and theological performance is the Ethiopian artist Aïda Muluneh’s interpretation of Inferno, canto xx. Muluneh’s performative expression transforms the scope and meaning of tears in the Comedy by bringing to bear her own particularity. Here, tears become central in unveiling the truth that the Comedy speaks. The article explores the significant role that gestures have in giving form to the Divine Comedy. As the logic of relationality, love forms the spine of this article while drawing together the themes of creation and incarnation. The article ends by suggesting that if one has a proper understanding of the relationship between humans and the created order, one might find a theology from below latent in the Comedy.

Abstract:The book examines the ‘poetics of joy’ in Dante’s Divine Comedy across the disciplines of lexicography, language history, philology, and theology, with reliance on an extensive body of research and supplemented with numerous comments to the poem. The book consists of two parts. In the first, the author analyses the vocabulary used by Dante and other medieval authors to express the notion. Dante’s artistic world exists within a broader cultural and religious context of the period. The book’s second part is devoted to the means by which the vocabulary denoting joy is rendered in M. Lozinsky’s now classical translation of the Comedy. The addendum contains the author’s own translations of several cantos from the Paradiso as well as of Jacopone da Todi’s lauda on the meaning of joy. The book will certainly interest Dante scholars and a broader audience of readers who are keen to understand the Divine Comedy — a unique work of world literature that has roused profound curiosity for more than seven centuries.

Abstract:The author discusses a connection between the modern analysand’s experience of depth therapy and the Christian pilgrim’s experience of confession, penitence and transformation as described by Dante in his Divine Comedy. She looks at structure and order in the context of psychotherapy. Taking Adriana Mazzarella’s ‘In Search of Beatrice’ as a guide, she examines a Jungian interpretation of the poem as an analogy of therapeutic initiation. She explores the idea of cultural complexes via the study of the medieval world view as one distinct from our own. Abstract:Dante’s Divine Comedy in Early Renaissance England compares the intellectual, emotional, and religious world of Dante in 13th-century Florence with that of a group of English intellectuals gathered around Humphrey, Duke of Gloucester, uncle of the King, Henry VI. Here, Jonathan Hughes establishes that there was a Renaissance in 15th-century England, encouraged by the discovery and translations of works of Greek philosophers and developments in science and medicine; and that vernacular writers in Gloucester’s circle, such as John Lydgate and Robert Hoccleve, were of fundamental importance in exploring the meaning of the self and man’s relationship with the natural world and the classical past. However, the appearance in 15th-century England of Dante’s ‘Commedia’, the most popular work of the Middle Ages, served to remind writers and readers of the cost of intellectual enquiry: the loss of faith in a harmonious and beautiful world; the redemptive power of the love of a woman; and the tangible presence of an afterlife. Engagingly written and meticulously researched, this innovative study shines a new perspective on Dante scholarship as well as offering a unique anaylsis of intellectual thought and culture in 15th-century England.
