Abstract:<p>In this paper, we propose an approximate dynamic programming (ADP) algorithm to solve a Markov decision process (MDP) formulation for the admission control of elective patients. To manage the elective patients from multiple specialties equitably and efficiently, we establish a waiting list and assign each patient a time-dependent dynamic priority score. Then, taking the random arrivals of patients into account, sequential decisions are made on a weekly basis. At the end of each week, we select the patients to be treated in the following week from the waiting list. By minimizing the cost function of the MDP over an infinite horizon, we seek to achieve the best trade-off between the patients' waiting times and the over-utilization of surgical resources. Considering the curses of dimensionality resulting from the large scale of realistically sized problems, we first analyze the structural properties of the MDP and propose an algorithm that facilitates the search for best actions. We then develop a novel reinforcement-learning-based ADP algorithm as the solution technique. Experimental results reveal that the proposed algorithms consume much less computation time in comparison with that required by conventional dynamic programming methods. Additionally, the algorithms are shown to be capable of computing high-quality near-optimal policies for realistically sized problems.</p>

Abstract:We study the Patient Assignment Scheduling (PAS) problem in a random environment that arises in the management of patient flow in the hospital systems, due to the stochastic nature of the arrivals as well as the Length of Stay distribution. We develop a Markov Decision Process (MDP) which aims to assign the newly arrived patients in an optimal way so as to minimise the total expected long-run cost per unit time over an infinite horizon. We assume Poisson arrival rates that depend on patient types, and Length of Stay distributions that depend on whether patients stay in their primary wards or not. Since the instances of realistic size of this problem are not easy to solve, we develop numerical methods based on Approximate Dynamic Programming. We illustrate the theory with numerical examples with parameters obtained by fitting to data from a tertiary referral hospital in Australia, and demonstrate the application potential of our methodology under practical considerations.

Abstract:Hospitals typically lack effective enterprise level strategic planning of bed and care resources, contributing to bed census levels that are statistically “out of control.” This system dysfunction manifests itself in bed block, surgical cancelation, ambulance diversions, and operational chaos. This is the classic hospital admission scheduling and control (HASC) problem, which has been addressed in its entirety only through inexact simulation-based search heuristics. This paper develops new analytical models of controlled hospital census that can, for the first time, be incorporated into a mixed-integer programming model to optimally solve the strategic planning/scheduling portion of the HASC. Our new solution method coordinates elective admissions with other hospital subsystems to reduce system congestion. We formulate a new Poisson-arrival-location model (PALM) based on an innovative stochastic location process that we developed and call the patient temporal resource needs model. We further extend the PALM approach to the class of deterministic controlled-arrival-location models (d-CALM) and develop linearizing approximations to stochastic blocking metrics. This work provides the theoretical foundations for an efficient scheduled admissions planning system as well as a practical decision support methodology to stabilize hospital census.

Abstract:To mitigate outpatient care delivery inefficiencies induced by resource shortages and demand heterogeneity, this paper focuses on the problem of allocating and sequencing multiple medical resources so that patients scheduled for clinical care can experience efficient and coordinated care with minimum total waiting time. We leverage highly granular location data on people and medical resources collected via Real-Time Location System technologies to identify dominant patient care pathways. A novel two-stage Stochastic Mixed Integer Linear Programming model is proposed to determine the optimal patient sequence based on the available resources according to the care pathways that minimize patients' expected total waiting time. The model incorporates the uncertainty in care activity duration via sample average approximation.We employ a Monte Carlo Optimization procedure to determine the appropriate sample size to obtain solutions that provide a good trade-off between approximation accuracy and computational time. Compared to the conventional deterministic model, our proposed model would significantly reduce waiting time for patients in the clinic by 60%, on average, with acceptable computational resource requirements and time complexity. In summary, this paper proposes a computationally efficient formulation for the multi-resource allocation and care sequence assignment optimization problem under uncertainty. It uses continuous assignment decision variables without timestamp and position indices, enabling the data-driven solution of problems with real-time allocation adjustment in a dynamic outpatient environment with complex clinical coordination constraints.

Abstract:The focus of this work is the numerical application of a stochastic decision support model for the patient admission scheduling problem with random arrivals and departures. Here, we discuss the methodology for applying our model to real-world problems. We outline a solution approach for efficient computation, provide a numerical analysis of the model, and illustrate the methodology with examples. A key component of the model is an integer linear program which formulates the patient admission scheduling problem as an optimization of the total expected cost accumulated over a finite planning horizon. We rewrite some of the components of this integer linear program in order to improve numerical efficiency. We use Poisson processes and discrete phase-type distributions to model the random arrivals and departures, respectively. We argue that this stochastic component is essential for an accurate treatment of real-world problems which are stochastic in nature. We support our claim with simple numerical examples, and show that the optimal solutions obtained from deterministic models are inadequate when compared with the solutions of our stochastic model. We also construct more complex numerical examples for large-scale problems using heuristics that approximate the objective function, in order to demonstrate that our model can be efficiently applied to real-world problems, which typically involve large data sets.

Abstract:Various demands of different patients over both medical resource and time domains in health care systems raise requests of strategies for balanced system capacity from an operations perspective. In this paper, a quantitative modeling technique with both patient arrival and associated treatment process integrated are used to characterize health care system performance and evaluate system efficiency. The patient arrival process is described as a dynamic random Poisson process and patient treatments are characterized as consumption processes of various health care resources over time with a view of the "product line" used. The waiting time of patients and usage of health care resources are proposed as system performance measures based on their means, variances, and confidence intervals. A simulation considering patients with several various diseases is given to find a mechanism of conflicting factors in decisions of balanced system capacity, and an operation scheme of "evenly balanced load for bottlenecks" is obtained based on analysis of simulation outputs. Simul8 provides the software environment for the simulation.

Abstract:As pressure on the healthcare system increases, patients that require elective surgery experience longer access times to pre- and post-operative appointments and surgery. Hospitals can control their waiting lists by allocating timeslots to different types of appointments. To allow appointments to be planned timely, this allocation is decided several weeks in advance. However, the consequences of the timeslot allocation are uncertain, as not all patients follow the same treatment pathway. Furthermore, as these planning decisions are made in advance, they are based on an uncertain prediction of future waiting lists. We aim to develop methods that support hospitals in timeslot allocation to reduce access times for patients and ensure that all available capacity is used. The problem is modelled as a Markov decision process (MDP). As the state space is very large, we use least-squares policy iteration to find an approximate solution, formulate an (integer) linear program to solve a deterministic variant of the MDP, and investigate several decision rules. The solution methods are tested on a case study at the Sint Maartenskliniek, a hospital in the Netherlands. Based on a simulation study, we find that all methods improve on the currently used static allocation method, with the (integer) linear program leading to the best results. However, the performance deteriorates with the number of weeks the hospital plans ahead. To counter this, we propose a method in which a percentage of timeslots is statically allocated far in advance, and the remaining timeslots are allocated when enough information is available to effectively deal with variability. For the case study, we find that statically allocating 60% of the timeslots and dynamically allocating the remainder 6 weeks in advance provides the best results in terms of meeting access time targets and efficient resource utilization.

Abstract:<p>This paper addresses the advance scheduling of elective surgeries in an operating theater composed of operating rooms and a downstream surgical intensive care unit (SICU). The arrivals of new patients in each week, the duration of each surgery, and the length-of-stay of each patient in the SICU are subject to uncertainty. At the end of each week, the surgery planner determines the surgical blocks to open in the next week and assigns a subset of the surgeries on the waiting list to open surgical blocks. The objective is to minimize the patient-related costs incurred by performing and postponing surgeries as well as the hospital-related costs caused by utilization of surgical resources. Considering that the pure mathematical programming models commonly used in the literature mostly focus on the short-term optimization of surgery schedules, we propose a novel two-phase optimization model that combines Markov decision process (MDP) and stochastic programming to improve the long-term performance of surgery schedules. Moreover, in order to solve realistically sized problems efficiently, we develop a novel column-generation-based heuristic (CGBH) algorithm, then combine it with the sample average approximation (SAA) approach. The experimental results indicate that the SAA-CGBH algorithm is considerably more efficient than the conventional SAA approach, and that the optimal surgery schedules of the two-phase optimization model significantly outperform those of a pure stochastic programming model.</p>

Abstract:This study is concerned with the determination of an optimal appointment schedule in an outpatient-inpatient hospital system where the inpatient exams can be cancelled based on certain rules while the outpatient exams cannot be cancelled. Stochastic programming models were formulated and solved to tackle the stochasticity in the procedure durations and patient arrival patterns. The first model, a two-stage stochastic programming model, is formulated to optimize the slot size. The second model further optimizes the inpatient block (IPB) placement and slot size simultaneously. A computational method is developed to solve the second optimization problem. A case study is conducted using the data from Magnetic Resonance Imaging (MRI) centers of Lahey Hospital and Medical Center (LHMC). The current schedule and the schedules obtained from the optimization models are evaluated and compared using simulation based on FlexSim Healthcare. Results indicate that the overall weighted cost can be reduced by 11.6% by optimizing the slot size and can be further reduced by an additional 12.6% by optimizing slot size and IPB placement simultaneously. Three commonly used sequencing rules (IPBEG, OPBEG, and a variant of ALTER rule) were also evaluated. The results showed that when optimization tools are not available, ALTER variant which evenly distributes the IPBs across the day has the best performance. Sensitivity analysis of weights for patient waiting time, machine idle time and exam cancellations further supports the superiority of ALTER variant sequencing rules compared to the other sequencing methods. A Pareto frontier was also developed and presented between patient waiting time and machine idle time to enable medical centers with different priorities to obtain solutions that accurately reflect their respective optimal tradeoffs. An extended optimization model was also developed to incorporate the emergency patient arrivals. The optimal schedules from the extended model show only minor differences compared to those from the original model, thus proving the robustness of the scheduling solutions obtained from our optimal models against the impacts of emergency patient arrivals.

Abstract:Magnetic resonance imaging (MRI) is widely used in diagnostic medicine and contributes significantly to US health care spending. Scheduling MRI jobs involves uncertainties (e.g., patient arrival time, scanning time, and preparation time) that can lead to excessive delays and high costs in MRI operations. This study addresses real-time decision making in use of MRI scanners based on job assignment and sequencing decisions that override the appointment schedule. The decisions are made using real-time information of the waiting patients, the utilization status of the MRI scanners, and the partially revealed uncertainties of scanning times of current patients. A sequential decision-making framework and a simulation-based solution method are proposed to utilize massive real-time information and match the use of MRI rescheduling in practice. The results are then compared with a real case in a large midwestern academic medical center in the US. This study illustrates that the proposed method reduces patient waiting time by 21.7% and improves utilization of MRI scanners by 23.0%. An optimality gap of 13.6% is provided when compared to off line scheduling methods based on a mixed integer programming (MIP) model. The number of simulation replications in this approach uses the ranking and selection method, which not only reduces solution time, but also provides solution quality guarantees wherein the probability of errors in the proposed method for one day is less than 0.1%. In 100 randomly generated workday experiments, all of the scheduling decisions given by the proposed method perform better than current policy, with an average reduction of 17.93 minutes in each patient's waiting time and an improvement of scanner utilization by 7.20%.

Abstract:This paper aims to optimise healthcare resource pre-positioning, patient scheduling, and patient transferring under uncertain demands and stochastic resource consumption. We propose a two-stage stochastic programming model that formulates the patient scheduling problem as a Markov decision process. To address complexities and uncertainties, we use Artificial Intelligence (AI) techniques to improve both model formulation and algorithmic performance. To tackle the limited data challenge, we introduce a Wasserstein distance-based ambiguity set and propose a two-stage distributionally robust optimisation (DRO) model, which derives a deterministic equivalent using the Lagrangian dual of non-anticipativity constraints. The solution process is accelerated with a scenario decomposition approach and the K-means clustering method. Both theoretical and numerical results demonstrate the consistency of the two-stage DRO model with the sample average approximation (SAA) method. The potential of AI to improve the model's performance is evident through the significant reduction in computation time achieved with the K-means clustering approach without compromising solution quality. Compared to the SAA method, the DRO model exhibits a considerable reduction in the waiting penalty cost for out-of-sample cases, ranging from 43.06% to 81.23% . Numerical results show that the proposed algorithm outperforms SAA methods and several benchmark policies in terms of computational efficiency and solutions.

Distributed surgical scheduling across collaborating hospitals considering stochastic duration and emergency demand

Abstract:In this paper, we address the need for distributed surgical scheduling across multiple hospitals and multiple days. We develop an effective ex-ante distributed schedule to determine elective service operation and elective patient assignment plan in a hospital alliance with the goal of minimizing the total expected cost considering possible realizations of treatment durations and emergency demands. The problem is formulated into a two-stage stochastic integer programming where uncertainty is characterized by a scenario set. Cancellations of scheduled elective patients and inter-hospital transfers of materialized emergency patients are explicitly modeled as second-stage decisions. To solve the problem, we propose an integer decomposition algorithm that incorporates a novel logic-based optimality cut along with a validity proof. Additionally, we introduce several enhancements to improve the algorithm. Through numerical experiments, we demonstrate that our proposed algorithm outperforms both the commercial solver and the existing integer decomposition algorithm for the proposed problem. It provides effective solutions that closely approach the best upper bounds achieved by the solver in less time, or even surpass the solver's best upper bounds. Furthermore, we provide operational insights into other commonly-used scheduling approaches through numerical comparisons, including the strengths and limitations of the most common way of scheduling elective patients and two strategies for handling emergencies within a hospital alliance.

Abstract:Objective Elective‐emergency admission control referred to allocating available inpatient bed capacity between elective and emergency hospitalization demand. Existing approaches for admission control often excluded several complex factors when making decisions, such as uncertain bed capacity and unknown true probability distributions of patient arrivals and departures. We aimed to create a data‐driven newsvendor framework to study the elective‐emergency admission control problem to achieve bed operational efficiency and effectiveness. Methods We developed a data‐driven approach that utilized the newsvendor framework to formulate the admission control problem. We also created approximation algorithms to generate a pool of candidate admission control solutions. Past observations and relevant emergency demand and bed capacity features were modeled in a newsvendor framework. Using approximation algorithmic approaches (sample average approximation, separated estimation and optimization, linear programing‐LP, and distribution‐free model) allowed us to derive computationally efficient data‐driven solutions with tight bounds on the expected in‐sample and out‐of‐sample cost guaranteed. Results Tight generalization bounds on the expected out‐of‐sample cost of the feature‐based model were derived with respect to the LP and quadratic programing (QP) algorithms, respectively. Results showed that the optimal feature‐based model outperformed the optimal observation‐based model with respect to the expected cost. In a setting where the unit overscheduled cost was higher than the unit under‐scheduled cost, scheduling fewer elective patients would replace the benefit of incorporating related features in the model. The tighter the available bed capacity for elective patients, the bigger the difference of the schedule cost between the feature‐based model and the observation‐based model. Conclusions The study provides a reference for the theoretical study on bed capacity allocation between elective and emergency patients under the condition of the unknown true probability distribution of bed capacity and emergency demand, and it also proves that the approximate optimal policy has good performance.

Abstract:Parallel processing is a principle which enables simultaneous implementation of anesthesia induction and operating room (OR) turnover with the aim of improving OR utilization. In this article, we study the problem of scheduling surgeries for multiple ORs and induction rooms (IR) that function based on the parallel processing principle under uncertainty. We propose a two-stage stochastic mixed-integer programming model considering the uncertainty in induction, surgery and turnover durations. We sequence patients and set appointment times for surgeries in the first stage and assign patients to IRs at the second stage of the model. We show that an optimal myopic policy can be used for IR assignment decisions due to the special structure of the model. We minimize the expected total cost of patient waiting time, OR idle time and IR idle time in the objective function. We enhance the model formulation using bounds on variables and symmetry-breaking constraints. We implement a novel progressive hedging algorithm by proposing a penalty update method and a variable fixing mechanism. Based on real data of a large academic hospital, we compare our solution approach with several scheduling heuristics from the literature. We assess the additional benefits and costs associated with the implementation of parallel processing using near-optimal schedules. We examine how the benefits are inflated by increasing the number of IRs. Finally, we estimate the value of stochastic solution to underline the importance of considering uncertainty in durations.

A stochastic optimization model for shift scheduling in emergency departments

Abstract:Excessive waiting time in Emergency Departments (ED) can be both a cause of frustration and more importantly, a health concern for patients. Waiting time arises when the demand for work goes beyond the facility's service capacity. ED service capacity mainly depends on human resources and on beds available for patients. In this paper, we focus on human resources organization in an ED and seek to best balance between service quality and working conditions. More specifically, we address the personnel scheduling problem in order to optimize the shift distribution among employees and minimize the total expected patients' waiting time. The problem is also characterized by a multi-stage re-entrant service process. With an appropriate approximation of patients' waiting times, we first propose a stochastic mixed-integer programming model that is solved by a sample average approximation (SAA) approach. The resulting personnel schedules are then evaluated using a discrete-event simulation model. Numerical experiments are then performed with data from a French hospital to compare different personnel scheduling strategies.
Using stochastic programming to solve an outpatient appointment scheduling problem with random service and arrival times

Abstract:We study a stochastic outpatient appointment scheduling problem (SOASP) in which we need to design a schedule and an adaptive rescheduling (i.e., resequencing or declining) policy for a set of patients. Each patient has a known type and associated probability distributions of random service duration and random arrival time. Finding a provably optimal solution to this problem requires solving a multistage stochastic mixed‐integer program (MSMIP) with a schedule optimization problem solved at each stage, determining the optimal rescheduling policy over the various random service durations and arrival times. In recognition that this MSMIP is intractable, we first consider a two‐stage model (TSM) that relaxes the nonanticipativity constraints of MSMIP and so yields a lower bound. Second, we derive a set of valid inequalities to strengthen and improve the solvability of the TSM formulation. Third, we obtain an upper bound for the MSMIP by solving the TSM under the feasible (and easily implementable) appointment order (AO) policy , which requires that patients are served in the order of their scheduled appointments, independent of their actual arrival times. Fourth, we propose a Monte Carlo approach to evaluate the relative gap between the MSMIP upper and lower bounds. Finally, in a series of numerical experiments, we show that these two bounds are very close in a wide range of SOASP instances, demonstrating the near‐optimality of the AO policy. We also identify parameter settings that result in a large gap in between these two bounds. Accordingly, we propose an alternative policy based on neighbor‐swapping. We demonstrate that this alternative policy leads to a much tighter upper bound and significantly shrinks the gap.

Abstract:The number of emergency cases or emergency room visits rapidly increases annually, thus leading to an imbalance in supply and demand and to the long-term overcrowding of hospital emergency departments (EDs). However, current solutions to increase medical resources and improve the handling of patient needs are either impractical or infeasible in the Taiwanese environment. Therefore, EDs must optimize resource allocation given limited medical resources to minimize the average length of stay of patients and medical resource waste costs. This study constructs a multi-objective mathematical model for medical resource allocation in EDs in accordance with emergency flow or procedure. The proposed mathematical model is complex and difficult to solve because its performance value is stochastic; furthermore, the model considers both objectives simultaneously. Thus, this study develops a multi-objective simulation optimization algorithm by integrating a non-dominated sorting genetic algorithm II (NSGA II) with multi-objective computing budget allocation (MOCBA) to address the challenges of multi-objective medical resource allocation. NSGA II is used to investigate plausible solutions for medical resource allocation, and MOCBA identifies effective sets of feasible Pareto (non-dominated) medical resource allocation solutions in addition to effectively allocating simulation or computation budgets. The discrete event simulation model of ED flow is inspired by a Taiwan hospital case and is constructed to estimate the expected performance values of each medical allocation solution as obtained through NSGA II. Finally, computational experiments are performed to verify the effectiveness and performance of the integrated NSGA II and MOCBA method, as well as to derive non-dominated medical resource allocation solutions from the algorithms.

Abstract:This paper proposes a new formulation for the dynamic resource allocation problem, which converts the traditional MDP model with known parameters and no capacity constraints to a new model with uncertain parameters and a resource capacity constraint. Our motivating example comes from a medical resource allocation problem: patients with multiple chronic diseases can be provided either normal or special care, where the capacity of special care is limited due to financial or human resources. In such systems, it is difficult, if not impossible, to generate good estimates for the evolution of health for each patient. We formulate the problem as a two-stage stochastic integer program. However, it becomes easily intractable in larger instances of the problem for which we propose and test a parallel approximate dynamic programming algorithm. We show that commercial solvers are not capable of solving the problem instances with a large number of scenarios. Nevertheless, the proposed algorithm provides a solution in seconds even for very large problem instances. In our computational experiments, it finds the optimal solution for $42.86\%$ of the instances. On aggregate, it achieves $0.073\%$ mean gap value. Finally, we estimate the value of our contribution for different realizations of the parameters. Our findings show that there is a significant amount of additional utility contributed by our model.
Surgery Scheduling in Flexible Operating Rooms by using a Convex Surrogate Model of Second-Stage Costs

Abstract:We study the elective surgery planning problem in a hospital with operation rooms shared by elective and emergency patients. This problem can be split in two distinct phases. First, a subset of patients to be operated in the next planning period has to be selected, and the selected patients have to be assigned to a block and a tentative starting time. Then, in the online phase of the problem, a policy decides how to insert the emergency patients in the schedule and may cancel planned surgeries. The overall goal is to minimize the expectation of a cost function representing the assignment of patient to blocks, case cancellations, overtime, waiting time and idle time. We model the offline problem by a two-stage stochastic program, and show that the second-stage costs can be replaced by a convex piecewise linear surrogate model that can be computed in a preprocessing step. This results in a mixed integer program which can be solved in a short amount of time, even for very large instances of the problem. We also describe a greedy policy for the online phase of the problem, and analyze the performance of our approach by comparing it to either heuristic methods or approaches relying on sampling average approximation (SAA) on a large set of benchmarking instances. Our simulations indicate that our approach can reduce the expected costs by as much as 30% compared to heuristic methods and is able to solve problems with $1000$ patients in about one minute, while SAA-approaches fail to obtain near-optimal solutions within 30 minutes, already for $100$ patients.

Recursive modeling of ambulatory health care settings

Abstract:Previous work dealing with the planning of outpatient health care has primarily used either linear programming or simulation. Linear programs are rather severe abstractions of reality. Simulation models are more realistic but are very expensive and do not necessarily identify optimal solutions. This paper discusses a recursive optimization-stimulation approach, which takes advantage of the best features of both methods while minimizing the disadvantages of each method used alone. A mixed integer program generates staffing and facility plans, thus reducing the number of alternatives requiring analysis via simulation. The simulation model then evaluates the acceptability of alternative plans, considering detailed information and complex relationships omitted from the optimization model. The method is used in a hypothetical example based on data from a variety of sources. The results from the example demonstrate the value of the recursive method; however, they are not to be used in a general context.
