Abstract:The characteristics of shock wave have a great potential for medical applications. Extracorporeal Shock Wave Lithotripsy is one such medical application. Recently, it is suggested that angiogenesis, Drug Delivery System and Gene Transfer using low-level shock wave. We developed small shock wave device for minimally invasive surgery. In this device, shock waves are generated by electrical discharge. We evaluated this device with visualization of shock wave and measurement of pressure. The results of visualization showed that shock wave converged toward the second focal point after 15 microsecond of electrical discharge, and this device could produce an average pressure of 2.32 MPa with standard deviation of 0.89 MPa. Then we had two experiments to investigate the effects of shock wave on biological cells by our device. In first experiment, cells exposed to shock wave were stained by Calcein-AM and PI. Cells were stained red in the small region; the size was about 1.5mm. This result showed that our device could provide effects locally. In another experiment, we investigate the variations of calcium ion concentration during shock wave irradiation. We observed calcium-ion elevation depending on shock wave irradiation.

Abstract:Shock waves have recently attracted attention for their effects on biological soft tissues, such as angiogenesis. When shock waves are reflected at some surfaces of biological tissues, an expansion wave with as short duration as shock wave can be generated because of a change in acoustic impedance. We hypothesized that the expansion wave affects soft tissues, inducing bioeffects. In this study, we developed an expansion wave generator consisting of an ellipsoidal reflector and a polydimethyl siloxane (PDMS) unit. A shock wave (compression wave) was generated by underwater electric discharge and focused by reflection from an ellipsoidal mirror. The shock wave propagated in the water and was transmitted at the water-PDMS boundary surface because they have almost the same acoustic impedance (1.5 x 10(6) kg/m(2 center dot)s for water, approximately 1.4 x 10(6) kg/m(2)center dot s for PDMS). Next, the shock wave was reflected at the PDMS-air boundary surface. At this surface, the shock wave was converted to an expansion wave because the acoustic impedance of PDMS is much higher than that of air (4.3 x 10(2) kg/m(2)center dot s). After these reflections, the expansion wave was focused. At the focal point, the peak pressure measured by a hydrophone was - 5.52 +/- 1.23 MPa (n = 10) when the discharge voltage was 4 kV. Moreover, the half-width of the expansion wave was 0.55 +/- 0.16 mu s (n = 10). These values were the same as those of the shock wave. These results indicate that our expansion wave generator is able to produce expansion waves and is useful for studying their bioeffects.
Shock wave treatment enhances cell proliferation and improves wound healing by ATP release-coupled extracellular signal-regulated kinase (ERK) activation

Abstract:Shock wave treatment accelerates impaired wound healing in diverse clinical situations. However, the mechanisms underlying the beneficial effects of shock waves have not yet been fully revealed. Because cell proliferation is a major requirement in the wound healing cascade, we used in vitro studies and an in vivo wound healing model to study whether shock wave treatment influences proliferation by altering major extracellular factors and signaling pathways involved in cell proliferation. We identified extracellular ATP, released in an energy- and pulse number-dependent manner, as a trigger of the biological effects of shock wave treatment. Shock wave treatment induced ATP release, increased Erk1/2 and p38 MAPK activation, and enhanced proliferation in three different cell types (C3H10T1/2 murine mesenchymal progenitor cells, primary human adipose tissue-derived stem cells, and a human Jurkat T cell line) in vitro. Purinergic signaling-induced Erk1/2 activation was found to be essential for this proliferative effect, which was further confirmed by in vivo studies in a rat wound healing model where shock wave treatment induced proliferation and increased wound healing in an Erk1/2-dependent fashion. In summary, this report demonstrates that shock wave treatment triggers release of cellular ATP, which subsequently activates purinergic receptors and finally enhances proliferation in vitro and in vivo via downstream Erk1/2 signaling. In conclusion, our findings shed further light on the molecular mechanisms by which shock wave treatment exerts its beneficial effects. These findings could help to improve the clinical use of shock wave treatment for wound healing.
A Convolution Method for Folding Systematic Uncertainties into Likelihood Functions

Abstract:We describe a general convolution method for incorporating systematic uncertainties into likelihood functions for analyses that seek to extract upper limits or two-sided intervals on some parameter of interest. The method is derived as an approximation to the correct Bayesian approach and therefore inherits the latter’s statistical interpretation and to some degree its mathematical properties. In particular, the form of the convolution kernel is fully determined, in contrast with commonly used approaches that rely on intuition and guesswork. A simple example is analyzed in detail.

Abstract:The potential beneficial regenerative and stimulatory extracorporeal shock wave therapy (ESWT) applications to the central nervous system have garnered interest in recent years. Treatment zones for these indications are acoustically shielded by bones, which heavily impact generated sound fields. We present the results of high-resolution tissue-realistic simulations, comparing the viability of different ESWT applicators in their use for transcranial applications. The performances of electrohydraulic, electromagnetic, and piezoelectric transducers for key reflector geometries are compared. Based on density information obtained from CT imaging of the head, we utilized the non-linear wave propagation toolset Matlab k-Wave to obtain spatial therapeutic sound field geometries and waveforms. In order to understand the reliability of results on the appropriate modeling of the skull, three different bone attenuation models were compared. We find that all currently clinically ESWT applicator technologies show significant retention of peak pressures and energies past the bone barrier. Electromagnetic transducers maintain a significantly higher energy flux density compared to other technologies while low focusing strength piezoelectric applicators have the weakest transmissions. Attenuation estimates provide insights into sound field degradation and energy losses, indicating that effective transcranial therapies can readily be attained with current applicators. Furthermore, the presented approach will allow for future targeted in silico development and the design of applicators and therapy plans to ultimately improve therapeutic outcomes.

Abstract:Subtyping of hepatitis B antigen (HBs Ag) in patients with acute hepatitis B revealed subtype ay in 75 percent while subtype ad was much more common in chronic hepatitis B infections: 81 percent of HBs Ag positive blood donors and 76 percent of patients with HBs Ag positive chronic aggressive hepatitis revealed subtype ad. In contrast to blood donors and chronic hepatitis patients, a change of the prevalent subtype was noted in acute hepatitis patients between 1970 and 1974. Before May 1972, subtype ad was found in 67 percent of patients, whereas later subtype ay dominated in 93 percent. An unequal distribution of subtypes between acute and chronic forms of hepatitis B infections has been explained by differences in the virulence of virus strains. Our results suggest that the higher prevalence of subtype ad in longterm carriers may be due to infection during an earlier time when subtype ad was also common in acute infections. The change of the prevalent subtype in acute infections may be attributed to differences in contagiosity rather than differences in virulence of virus strains.

Abstract:Understanding the damage that high intensity compression waves induce in human tissues is critical for developing improved therapies for patients suffering from blast injuries. Experimentally based models of blast injury using live biological samples are needed. In this study we have developed a system to directly assess the effects of dynamic loading conditions on live cells. Here, we describe a confinement chamber designed to subject live cell cultures in a liquid environment to high intensity compression waves using a split Hopkinson pressure bar system. Signals from the strain gauges mounted on the bars and the chamber allow the measurement of parameters such as pressure and duration of the stimulus. The chamber itself also allows recovery of cells subjected to compression for assessment of cellular damage. In these studies we present evidence of increased levels of damage and loss of cellular integrity in cultured mouse mesenchymal stem cells subjected to a high-intensity compression wave with a peak pressure of 7.6 ± 0.8 MPa.

Abstract:We have developed a shock wave forceps for drug delivery and gene transfection. The shock wave is generated by electrical discharge from a shock wave generator and reflected by a spheroid-shaped reflector. The outer diameter of the reflector is 11 mm. The device enables a precision shock wave exposure to the target tissue in the body. We evaluated this device with visualization of shock wave and measurement of shock wave pressure. The visualization experiment demonstrated that the shock wave converged at second focal point. The wave pressure measurement results showed that the device has an average pressure of 2.13 MPa. We conducted an in vitro experiment with the cultured cell to evaluate the feasibility of the device. The experimental results showed that the cells were stained by PI locally after received shock wave exposure. The stained region was about 1.5 mm diameter. The validation experiment showed the possibility of the developed to be used in drug delivery and gene transfection.

Effects of nanobubble collapse on cell membrane integrity

Abstract:Recent studies have shown that ultrasound is used to open drug-carrying liposomes to release their payloads; however, a shockwave energetic enough to rupture lipid membranes can cause collateral damage to surrounding cells. Similarly, a destructive shockwave, which may be used to rupture a cell membrane in order to lyse the cell (e.g., as in cancer treatments) may also impair or destroy nearby healthy tissue. To address this problem, we use dissipative particle dynamic (DPD) simulation to investigate the addition of a cavitation bubble between the shockwave and the model cell membrane to alter the shockwave front, allowing low-velocity shockwaves to specifically damage an intended target. We focus specifically on a spherical lipid bilayer model, and note the effect of shockwave velocity, bubble size, and orientation on the damage to the model cell. We show that a cavitation bubble greatly decreases the necessary shockwave velocity required to damage the lipid bilayer and rupture the model cell. The cavitation bubble focuses the kinetic energy of the shockwave front into a smaller area, inducing penetration at the edge of the model cell. With this work, we provide a comprehensive approach to the intricacies of model cell destruction via shockwave impact, and hope to offer a guideline for initiating targeted cellular destruction using induced cavitation bubbles and low-velocity shockwaves.

Abstract:Mechanical signals play a vital role in cell biology and is a vast area of research. Thus, there is motivation to understand cell deformation and mechanobiological responses. However, the ability to controllably deform cells in the ultrasonic regime and test their response is a noted challenge throughout the literature. Quantifying and eliciting an appropriate stimulus has proven to be difficult, resulting in methods that are either too aggressive or oversimplified. Furthermore, the ability to gain a real-time insight into cell deformation and link this with the biological response is yet to be achieved. One application of this understanding is in ultrasonic surgical cutting, which is a promising alternative to traditional methods, but with little understanding of its effect on cells. Here we present the image based ultrasonic cell shaking test, a novel method that enables controllable loading of cells and quantification of their response to ultrasonic vibrations. Practically, this involves seeding cells on a substrate that resonates at ultrasonic frequencies and transfers the deformation to the cells. This is then incorporated into microscopic imaging techniques to obtain high-speed images of ultrasonic cell deformation that can be analysed using digital image correlation techniques. Cells can then be extracted after excitation to undergo analysis to understand the biological response to the deformation. This method could aid in understanding the effects of ultrasonic stimulation on cells and how activated mechanobiological pathways result in physical and biochemical responses.

Minimizing the expected distortion of two-hop networks using multi-layer source coding

Abstract:This paper concerns minimizing the expected distortion of a Gaussian source, transmitted over a two-hop block fading channel, under the mean square-error measure. It is assumed that there is not a direct link between the source and destination, and a simple Decode and Forward (DF) relay, handles the communication. The channel state information (CSI) associated with each hop is available at the corresponding receiver, while the transmitters are unaware of associated CSIs. It is assumed that the source node performs successive refinement source coding approach, together with multi-layer channel coding to send its information to the relay. Then, the relay re-encodes its received information with a multi-layer channel code, and forwards it to the destination. Accordingly, the optimal power allocation policy across code layers in terms of minimizing the expected achievable distortion are formulated and addressed and then the results are numerically derived for Rayleigh block fading environment, showing the proposed method performs better than the Amplify and Forward (AF) relaying for source-channel mismatch factors greater than one.
Effects of Extracorporeal Shock Waves on Microcirculation and Angiogenesis in the in vivo Wound Model of the Diver Box

Abstract:Introduction: Extracorporeal shock waves (ESWs) have been shown to have a positive effect on skin wound healing; however, little is known on the regeneration of the microcirculation and angiogenesis as well as the different application modes. Methods: A total of 40 BALB/c mice were provided with dorsal skin fold chambers and were divided into 3 therapy groups ( n = 30) and one control group ( n = 10). The 3 therapy groups were treated with shock waves at different pulse rates (500–1,000 pulses/min) and application frequencies (day 0 and day 6 or day 0 only). Photographic documentation and intravital microscopy were carried out on day 1, 2, 4, and 6 after wounding. Results: Using the newly developed Diver Box, shock waves could be applied in vivo without mechanical tissue damage. Shock wave therapy to skin wounds demonstrated to induce faster wound closure rates in the beginning than controls in groups with higher pulse rates and frequencies of the shock waves. Furthermore, the regeneration of microcirculation and perfusion in the healing skin was significantly improved after the application of, in particular, higher pulse rates as given by increased numbers of perfused capillaries and functional vessel density. The study of inflammation showed, especially in high-pulse ESW groups, higher leukocyte counts, and rolling leukocytes over time until day 6 as a response to the induction of inflammatory reaction after ESW application. Angiogenesis showed a marked increase in positive areas as given by sprouts, coils, and recruitments in all ESW groups, especially between days 4 and 6. Conclusion: The major findings of this trial demonstrate that ESW therapy to skin wounds is effective and safe. This is demonstrated by the initially faster wound closure rate, but later the same wound closure rate in the treatment groups than in controls. Furthermore, during the regeneration of microcirculation and perfusion in the healing skin, a significant improvement was observed after the application of, in particular, higher ESW pulse rates, suggesting an ESW-related increase in nutrient and oxygen supply in the wound tissue.

Abstract:Synergistic applications of an electric field combined with nanojet-based mechanical pressure, have recently been shown to help create larger pores and provide control of the aspect ratio in biological membranes. The nanojets are formed by the collapse of nanobubbles in the vicinity of biomembranes upon being subjected to external shockwaves. Here we analyze the effects produced by the collapse of <i>multiple</i> nanobubbles <i>in the presence of an electric field</i>. Our simulations, based on molecular dynamics, show that not only would multiple nanobubbles make it possible to create larger pores, but also increase the pore density on the surface of biological cells. Both aspects could aid in the transport of drugs and genes for bio-medical applications.

Abstract:Understanding the response of solid materials to shock loading is important for mitigating shock-induced damages and failures, as well as advancing the beneficial use of shock waves for material modifications. In this paper, we consider a representative brittle material, BegoStone, in the form of cylindrical bodies and submerged in water. We present a computational study on the causal relationship between the prescribed shock load and the resulting elastic waves and damage in the solid material. A recently developed three-dimensional computational framework, FIVER, is employed, which couples a finite volume compressible fluid solver with a finite element structural dynamics solver through the construction and solution of local, one-dimensional fluid-solid Riemann problems. The material damage and fracture are modeled and simulated using a continuum damage mechanics model and an element erosion method. The computational model is validated in the context of shock wave lithotripsy and the results are compared with experimental data. We first show that after calibrating the growth rate of microscopic damage and the threshold for macroscopic fracture, the computational framework is capable of capturing the location and shape of the shock-induced fracture observed in a laboratory experiment. Next, we introduce a new phenomenological model of shock waveform, and present a numerical parametric study on the effects of a single shock load, in which the shock waveform, magnitude, and the size of the target material are varied. In particular, we vary the waveform gradually from one that features non-monotonic decay with a tensile phase to one that exhibits monotonic decay without a tensile phase. The result suggests that when the length of the shock pulse is comparable to that of the target material, the former waveform may induce much more significant damage than the latter one, even if the two share the same magnitude, duration, and acoustic energy.

Abstract:Wound healing requires the coordinated interaction of dermis cells, the proper deposition of extracellular matrix, re-epithelialization, and angiogenesis. Extracorporeal shock wave (ESW) is a promising therapeutic modality for chronic wounds. This study determined the biological mechanisms activated under ESW, facilitating the healing of pressure ulcers (PUs). A group of 10 patients with PUs received two sessions of radial ESW (300 + 100 pulses, 2.5 bars, 0.15 mJ/mm2, 5 Hz). Histomorphological and immunocytochemical assessments were performed on tissue sections obtained from the wound edges before the ESW (M0) and after the first (M1) and second (M2) ESW. The proliferation index of keratinocytes and fibroblasts (Ki-67), the micro-vessels' density (CD31), and the number of myofibroblasts (α-SMA) were evaluated. The involvement of the yes-associated protein (YAP1) in sensing mechanical strain, and whether the nuclear localization of YAP1, was shown. The increased proliferative activity of epidermal cells and skin fibroblasts and the increased number of myofibroblasts, often visible as integrated cell bands, were also demonstrated as an effect of wound exposure to an ESW. The results indicate that the major skin cells, keratinocytes, and fibroblasts are mechanosensitive. They intensify proliferation and extracellular matrix remodeling in response to mechanical stress. A significant improvement in clinical wound parameters was also observed.
