Abstract:Phototoxicity of pazufloxacin mesilate (PZFX mesilate), a novel parenteral quinolone antimicrobial agent, were evaluated in vitro and in vivo studies. In vitro, phototoxicity for cultured cells of PZFX, which is active principle of PZFX mesilate, was studied, and stability for long-wavelength ultraviolet (UVA) was examined. In vivo, phototoxicity tests in guinea pigs and rats, and photoallergenicity tests in guinea pigs were conducted. In the phototoxicity test on cultured cells, CHL/IU cells were irradiated UVA of 300-3000 mJ/cm2 in the presence of PZFX, ofloxacin (OFLX), lomefloxacin (LFLX) or sparfloxacin (SPFX) at 10 micrograms/mL. Phototoxic potencies for cultured cells of the quinolones tested were SPFX > LFLX > OFLX > PZFX. In addition, changes in ultraviolet absorption spectrum and residual rate of PZFX, OFLX, LFLX and SPFX were examined after UVA irradiation of 300-3000 mJ/cm2 to each solution. PZFX was stable for UVA compared with OFLX and LFLX. In the phototoxicity test of guinea pigs, each quinolone was administered intraperitoneally daily for 7 days, and UVA of about 11 J/cm2 was irradiated at 30 minutes after the last administration. Dose levels of each quinolone were 65 and 130 mg/kg of PZFX mesilate (dose levels converted to PZFX: 50 and 100 mg/kg), 50 and 100 mg/kg of nalidixic acid (NA), 100 mg/kg of OFLX, enoxacin (ENX), ciprofloxacin (CPFX), LFLX and SPFX. Grade of skin reaction (erythema) at 24 hours after UVA irradiation decreased in the order: SPFX > CPFX > NA > ENX = OFLX > LFLX > PZFX mesilate. Thus, PZFX mesilate was found to have the weakest phototoxicity. In the maximum plasma concentration of quinolones from 0.5 to 2.5 hours after administration, corresponding to the time of UVA irradiation, the concentration of the group administered PZFX mesilate was about 4.1 times higher than that of CPFX group, and about 1.3 times higher than that of SPFX group. The area under the blood concentration-time curve (AUC0.5-2.5) of the group administered PZFX mesilate was the same as that of SPFX group, and about 3.2 times larger than that of CPFX group. These data showed that phototoxicity of PZFX mesilate was also weaker than that of CPFX or SPFX in consideration of AUC0.5-2.5. In the phototoxicity test of rats injected intravenously, no phototoxicity was observed at 130 mg/kg of PZFX mesilate. In the photoallergenicity test of guinea pigs, no photoallergenicity was observed by PZFX mesilate. As mentioned above, from in vitro studies PZFX was found to be stable for UVA irradiation compared with OFLX and LFLX, and phototoxicity for cultured cells of PZFX was weaker than that of SPFX, LFLX or OFLX. In addition, from in vivo studies phototoxicity of PZFX mesilate was found to be weaker than that of NA, OFLX, ENX, CPFX, LFLX or SPFX, and no photoallergenicity was observed. Therefore, photosensitive potency of PZFX mesilate might be less than that of other quinolones.

Abstract:✓ The authors report a further 3-year follow-up review of a series of 59 patients suffering with intractable pain due to lumbar spondylosis and treated by multiple-level bilateral laminectomies and facetectomies. Routine postoperative x-ray examination revealed that six of 59 patients had developed spondylolisthesis. Two of the six were symptomatic and required a secondary fusion procedure. Analysis of the patients who had developed vertebral slippage revealed that 6% of the patients with only two levels (L-4 and L-5) removed developed spondylolisthesis, but in those with three or more levels removed 15% showed slipped vertebrae. After an average follow-up period of 6 years, 87% of patients still had a worthwhile result as contrasted with 91% of the same series who had had a good result after a follow-up period averaging 3 years.

Abstract:Competing risks data arise when occurrence of an event hinders observation of other types of events, and they are encountered in various research areas including biomedical research. These data have been usually analyzed using the hazard-based models, not survival times themselves. In this paper, we propose a joint accelerated failure time (AFT) modeling approach to model clustered competing risks data. Times to competing events are assumed to be log-linear with normal errors and correlated through a scaled random effect that follows a zero-mean normal distribution. Inference on the model parameters is based on the h-likelihood. Performance of the proposed method is evaluated through extensive simulation studies. The simulation results show that the estimated regression parameters are robust against the violation of the assumed parametric distributions. The proposed method is illustrated with three real competing risks data sets.

Abstract:Readmission following discharge from an initial hospitalization is a key marker of quality of health care in the United States. For the most part, readmission has been used to study quality of care for patients with acute health conditions, such as pneumonia and heart failure, with analyses typically based on a logistic-Normal generalized linear mixed model. Applying this model to the study readmission among patients with increasingly prevalent advanced health conditions such as pancreatic cancer is problematic, however, because it ignores death as a competing risk. A more appropriate analysis is to imbed such studies within the semi-competing risks framework. To our knowledge, however, no comprehensive statistical methods have been developed for cluster-correlated semi-competing risks data. In this paper we propose a novel hierarchical modeling framework for the analysis of cluster-correlated semi-competing risks data. The framework permits parametric or non-parametric specifications for a range of model components, including baseline hazard functions and distributions for key random effects, giving analysts substantial flexibility as they consider their own analyses. Estimation and inference is performed within the Bayesian paradigm since it facilitates the straightforward characterization of (posterior) uncertainty for all model parameters including hospital-specific random effects. The proposed framework is used to study the risk of readmission among 5,298 Medicare beneficiaries diagnosed with pancreatic cancer at 112 hospitals in the six New England states between 2000-2009, specifically to investigate the role of patient-level risk factors and to characterize variation in risk across hospitals that is not explained by differences in patient case-mix.

Abstract:With explanatory covariates, the standard analysis for competing risks data involves modeling the cause-specific hazard functions via a proportional hazards assumption. Unfortunately, the cause-specific hazard function does not have a direct interpretation in terms of survival probabilities for the particular failure type. In recent years many clinicians have begun using the cumulative incidence function, the marginal failure probabilities for a particular cause, which is intuitively appealing and more easily explained to the nonstatistician. The cumulative incidence is especially relevant in cost-effectiveness analyses in which the survival probabilities are needed to determine treatment utility. Previously, authors have considered methods for combining estimates of the cause-specific hazard functions under the proportional hazards formulation. However, these methods do not allow the analyst to directly assess the effect of a covariate on the marginal probability function. In this article we propose a novel semiparametric proportional hazards model for the subdistribution. Using the partial likelihood principle and weighting techniques, we derive estimation and inference procedures for the finite-dimensional regression parameter under a variety of censoring scenarios. We give a uniformly consistent estimator for the predicted cumulative incidence for an individual with certain covariates; confidence intervals and bands can be obtained analytically or with an easy-to-implement simulation technique. To contrast the two approaches, we analyze a dataset from a breast cancer clinical trial under both models.

Abstract:Survival analysis in the presence of competing risks imposes additional challenges for clinical investigators in that hazard function (the rate) has no one-to-one link to the cumulative incidence function (CIF, the risk). CIF is of particular interest and can be estimated non-parametrically with the use cuminc() function. This function also allows for group comparison and visualization of estimated CIF. The effect of covariates on cause-specific hazard can be explored using conventional Cox proportional hazard model by treating competing events as censoring. However, the effect on hazard cannot be directly linked to the effect on CIF because there is no one-to-one correspondence between hazard and cumulative incidence. Fine-Gray model directly models the covariate effect on CIF and it reports subdistribution hazard ratio (SHR). However, SHR only provide information on the ordering of CIF curves at different levels of covariates, it has no practical interpretation as HR in the absence of competing risks. Fine-Gray model can be fit with crr() function shipped with the cmprsk package. Time-varying covariates are allowed in the crr() function, which is specified by cov2 and tf arguments. Predictions and visualization of CIF for subjects with given covariate values are allowed for crr object. Alternatively, competing risk models can be fit with riskRegression package by employing different link functions between covariates and outcomes. The assumption of proportionality can be checked by testing statistical significance of interaction terms involving failure time. Schoenfeld residuals provide another way to check model assumption.

Abstract:Data analysis methods for the study of treatments or exposures in relation to a clinical outcome in the presence of competing risks have a long history, often with inference targets that are hypothetical, thereby requiring strong assumptions for identifiability with available data. Here data analysis methods are considered that are based on single and higher dimensional marginal hazard rates, quantities that are identifiable under standard independent censoring assumptions. These lead naturally to joint survival function estimators for outcomes of interest, including competing risk outcomes, and provide the basis for addressing a variety of data analysis questions. These methods will be illustrated using simulations and Women's Health Initiative cohort and clinical trial data sets, and additional research needs will be described.

Immunologic defects caused by certain of these mutations are determined at the level of hematopoietic progenitor cells or at the level of hematopoietic cell-stromal cell interactions. The immunological mutants described in this paper serve as experimental tools with which to increase our understanding of the development and regulation of the mammalian immune system.
Generic solution of the heterogeneity-induced competing risk problem in survival analysis

Abstract:Most papers implicitly assume competing risks to be induced by residual cohort heterogeneity, i.e. heterogeneity that is not captured by the recorded covariates. Based on this observation we develop a generic statistical description of competing risks that unifies the main schools of thought. Assuming heterogeneity-induced competing risks is much weaker than assuming risk independence. However, we show that it still imposes sufficient constraints to solve the competing risk problem, and derive exact formulae for decontaminated primary risk hazard rates and cause-specific survival functions. The canonical description is in terms of a cohort's covariate-constrained functional distribution of individual hazard rates of all risks. Assuming proportional hazards at the level of individuals leads to a natural parametrisation of this distribution, from which Cox regression, frailty and random effects models, and latent class models can all be recovered in special limits, and which also generates parametrised cumulative incidence functions (the language of Fine and Gray). We demonstrate with synthetic data how the generic method can uncover and map a cohort's substructure, if such substructure exists, and remove heterogeneity-induced false protectivity and false exposure effects. Application to real survival data from the ULSAM study, with prostate cancer as the primary risk, is found to give plausible alternative explanations for previous counter-intuitive inferences.

Abstract:In the analysis of clustered failure time data, Cox frailty models have been extensively studied by incorporating frailty with a prespecified distribution to address potential correlation of data within clusters. In this paper, we propose a frailty proportional mean residual life regression model to analyze such data. A novel conditional quasi‐likelihood inference procedure is developed, utilizing a stochastic process and the inverse probability of censoring weighting (IPCW) to form estimating equations for regression parameters. Our proposal employs conditional inference based on a penalized quasi‐likelihood to address within‐cluster correlation without need to specify the frailty distribution, bringing the method closer to what suffices for real‐world applications. By adopting the Buckley–James estimator in the IPCW, the method further allows for dependent censoring. We establish asymptotic properties of the proposed estimator and evaluate its finite sample performance via simulation studies. An application to the data from a multi‐institutional breast cancer study is presented for illustration.

Abstract:In the analysis of competing risks data, the cumulative incidence function is a useful quantity to characterize the crude risk of failure from a specific event type. In this article, we consider an efficient semiparametric analysis of mixture component models on cumulative incidence functions. Under the proposed mixture model, latency survival regressions given the event type are performed through a class of semiparametric models that encompasses the proportional hazards model and the proportional odds model, allowing for time-dependent covariates. The marginal proportions of the occurrences of cause-specific events are assessed by a multinomial logistic model. Our mixture modeling approach is advantageous in that it makes a joint estimation of model parameters associated with all competing risks under consideration, satisfying the constraint that the cumulative probability of failing from any cause adds up to one given any covariates. We develop a novel maximum likelihood scheme based on semiparametric regression analysis that facilitates efficient and reliable estimation. Statistical inferences can be conveniently made from the inverse of the observed information matrix. We establish the consistency and asymptotic normality of the proposed estimators. We validate small sample properties with simulations and demonstrate the methodology with a data set from a study of follicular lymphoma.

Abstract:Historically, the primary goal of conventional survival study methods has been to reduce the frequency of failures over time. If the associated observed and unobserved variables are not known when studying such events, this can have detrimental effects. Frailty models offer a tempting solution for investigating the impact of unknown variables in such a case. In this article, we assume that frailty affects the hazard rate. We find that the weighted Lindley frailty models, which use general versions of the Weibull and log-logistic type II distributions as the baseline distributions, are a reliable method for ensuring the influence of endogenous variability. The parameters involved are estimated according to different loss functions using the Bayesian structure as the basis of Markov Chain Monte Carlo. Bayesian evaluation strategies are then implemented to evaluate the models. The results are demonstrated on known data of kidney infections. It is shown that the novel models outperform those based on the inverse Gaussian and gamma frailty distributions.

Abstract:Summary Progression of chronic disease is often manifested by repeated occurrences of disease-related events over time. Delineating the heterogeneity in the risk of such recurrent events can provide valuable scientific insight for guiding customized disease management. We propose a new sensible measure of individual risk of recurrent events and present a dynamic modelling framework thereof, which accounts for both observed covariates and unobservable frailty. The proposed modelling requires no distributional specification of the unobservable frailty, while permitting exploration of the dynamic effects of the observed covariates. We develop estimation and inference procedures for the proposed model through a novel adaptation of the principle of conditional score. The asymptotic properties of the proposed estimator, including the uniform consistency and weak convergence, are established. Extensive simulation studies demonstrate satisfactory finite-sample performance of the proposed method. We illustrate the practical utility of the new method via an application to a diabetes clinical trial that explores the risk patterns of hypoglycemia in type 2 diabetes patients.

Abstract:This study proposes the use of semiparametric log-normal shared frailty models to analyze time-to-event data for individuals with similar features referred to as clusters. Shared frailty models are useful for modeling and estimating common risk in the lifetimes of individuals in these clusters. While various methods have been proposed for estimating shared frailty models, few studies have explored the use of the pseudo-full-likelihood method. In this study, the pseudo-full-likelihood and hierarchical likelihood approaches were used to construct and estimate parameter estimates and check for asymptotic properties via simulations. Log-normal semiparametric frailty model was used to obtain cluster-specific frailty based on the semiparametric log-normal shared frailty distribution. The results of both methods were compared, and prediction intervals for a random effect were obtained. To further investigate the existence of shared frailty in diabetes patients and a history of acute coronary syndrome (STEMI and NSTEMI), data from UK Biobank was used. The results suggest the presence of frailty within the clusters and indicate cluster time dependence in the study population. Overall, this study highlights the potential benefits of using the pseudo-full-likelihood method in shared frailty modeling and provides insights into the impact of observed variabilities on hazards within clusters.

Abstract:In this paper, studies of left-truncated and right-censored data with dependent competing risks are discussed. When the latent lifetime of competing failure causes is modeled by a bivariate Marshall–Olkin Weibull distribution, a dependent competing risks model is established for left-truncated and right-censored data with partially observed causes of failure, and classical and Bayesian estimations are presented in consequence. Maximum likelihood estimators along with the associated existence and uniqueness of the model parameters are established, and the asymptotic likelihood theory is used to construct approximate confidence intervals based on asymptotic theory. Further, based on general flexible priors, Bayes estimators and the associated highest posterior density credible intervals of the parameters are also obtained, and an importance sampling technique is used to generate samples from the posterior distribution and in turn to compute the Bayes estimates. Finally, extensive Monte-Carlo simulations are carried out to investigate the performances of our results and two real-life examples are analyzed to show the applicabilities of the proposed methods. The numerical results show that our proposed methods work satisfactory.

Hierarchical Bayesian modeling of heterogeneous outcome variance in cluster randomized trials

Abstract:Clinical Trials, Ahead of Print. Background:Heterogeneous outcome correlations across treatment arms and clusters have been increasingly acknowledged in cluster randomized trials with binary endpoints, where analytical methods have been developed to study such heterogeneity. However, cluster-specific outcome variances and correlations have yet to be studied for cluster randomized trials with continuous outcomes.Methods:This article proposes models fitted in the Bayesian setting with hierarchical variance structure to quantify heterogeneous variances across clusters and explain it with cluster-level covariates when the outcome is continuous. The models can also be extended to analyzing heterogeneous variances in individually randomized group treatment trials, with arm-specific cluster-level covariates, or in partially nested designs. Simulation studies are carried out to validate the performance of the newly introduced models across different settings.Results:Simulations showed that overall the newly introduced models have good performance, reporting low bias and approximately 95% coverage for the intraclass correlation coefficients and regression parameters in the variance model. When variances are heterogeneous, our proposed models had improved model fit over models with homogeneous variances. When used to analyze data from the Kerala Diabetes Prevention Program study, our models identified heterogeneous variances and intraclass correlation coefficients across clusters and examined cluster-level characteristics associated with such heterogeneity.Conclusion:We proposed new hierarchical Bayesian variance models to accommodate cluster-specific variances in cluster randomized trials. The newly developed methods inform the understanding of how an intervention strategy is implemented and disseminated differently across clusters and can help improve future trial design.

Abstract:In this paper, we develop a Bayesian semiparametric accelerated failure time model for survival data with cluster structures. Our model allows distributional heterogeneity across clusters and accommodates their relationships through a density ratio approach. Moreover, a nonparametric mixture of Dirichlet processes prior is placed on the baseline distribution to yield full distributional flexibility. We illustrate through simulations that our model can greatly improve estimation accuracy by effectively pooling information from multiple clusters, while taking into account the heterogeneity in their random error distributions. We also demonstrate the implementation of our method using analysis of Mayo Clinic Trial in Primary Biliary Cirrhosis.

Abstract:Traditional survival analysis techniques focus on the occurrence of failures over the time. During analysis of such events, ignoring the related unobserved covariates or heterogeneity involved in data sample may leads us to adverse consequences. In this context, frailty models are the viable choice to investigate the effect of the unobserved covariates. In this article, we assume that frailty acts multiplicatively to hazard rate. We propose inverse Gaussian (IG) and generalized Lindley (GL) shared frailty models with generalized Weibull (GW) as baseline distribution in order to analyze the unobserved heterogeneity. To estimate the parameters in models, Bayesian paradigm of Markov Chain Monte Carlo technique has been proposed. Model selection criteria have been used for the comparison of models. Three different cancer data sets have been analyzed using the shared frailty models. Better models have been suggested for the data sets.
