Abstract:With the development of 3-dimensional technology it has become increasingly possible to record the facial soft tissue changes that occur with growth and following orthodontic treatment. This article describes the use of the optical surface laser scanner to record and quantify the facial soft tissue changes following orthognathic surgery. Ten skeletal Class III patients were laser scanned, at previously determined time intervals, over a 6-month period following orthognathic surgery. Computer superimposition of the scanned images provided a graphic demonstration of the soft tissue changes as they occurred between the time intervals. Analysis of the results demonstrates a nonuniform, asymmetric resolution of facial swelling and adaptation of the soft tissues to the altered skeletal structure. This occurred principally over the initial 4 months following surgery. However, smaller changes continued to occur throughout the study period. This suggests that the facial soft tissues continue to change over a period of greater than 6 months. This knowledge is important as it is integral for obtaining informed consent from patients.

Abstract:Abstract Objectives To compare the changes of the soft tissue profile in relation to the displacement of the underlying hard structures in maxillary orthognathic surgery and to contribute to the esthetic prediction of the facial profile after surgical procedures. Materials and methods We analyzed the sagittal changes in the facial soft tissue profile related to surgical changes in skeletal structures after maxillary osteotomy in a retrospective study. The study sample comprised 115 adult patients between the ages of 18–50 years who had undergone maxillary orthognathic surgery and interdisciplinary orthodontic treatment at the Department of Orthodontics, Ludwig-Maximilians University of Munich, Germany. LeFort I osteotomy cases in both maxillary monognathic and bignathic osteotomy procedures were included. All subjects had received rigid fixation. A cephalometric analysis of presurgical and postsurgical cephalograms was performed and the correlations between hard tissue and soft tissue change ratios were evaluated using a bivariate linear regression analysis. A vertical line through the landmark sella (S) perpendicular to the nasion-sella line (NSL) served as the reference plane. Results The subnasale (Sn) followed the A point (A) by 57%, the soft tissue A point (A′) followed the A point (A) by 73% and the upper lip, represented by the landmark labrale superius (Ls) followed the upper incisor (Is) by 73%; all three in a linear correlation with a mean prediction error of nearly 2 mm. Conclusion The scatterplots show a linear correlation with a wide spread for all three pairs of reference points. The wide spread and the high prediction error of almost 2 mm indicate low predictability of the expected lip position and Sn.

Abstract:Purpose: The purpose of this study was to perform three-dimensional (3D) assessment of facial soft tissue in patients with skeletal Class III and mandibular asymmetry after orthognathic surgery. Methods: Samples consisted of 3D facial images obtained from five patients with A point-nasion-B point angle less than 2 degrees, and more than 5 mm of menton deviation. All patients had been treated at Gangneung-Wonju National University Dental Hospital from 2009 to 2012. They had undergone orthognathic surgery of Lefort I, and sagittal split osteotomy for correction of skeletal deformity, and orthodontic treatment. Facial scanning was performed before treatment (T1) and post-surgical orthodontic treatment (T2). Linear and angle variables of soft tissue landmarks, antero-posterior facial depth, and facial volume were measured. Results: No significant differences in width of the alar base, mouth width, and nasal canting were observed between T1 and T2. However, lip deviation, menton deviation, alar canting, lip canting, and menton deviation angle were significantly reduced at T2. Antero-posterior facial depth on the axial plane parallel to the left cheilion was significantly reduced on the deviated side and significantly increased on the non-deviated side at T2. Volume of the lower lateral and lower medial parts of the face was reduced on the deviated side, and volume of upper lateral and lower lateral parts on the non-deviated side was significantly increased at T2. Conclusion: After orthognathic surgery, facial asymmetry of soft tissue was improved following skeletal changes, especially the mandibular region. Although the length of the alar base and mouth width did not change, lip and soft tissue menton were displaced to the medial side after treatment. Facial depth also became symmetric after treatment. Facial volume showed a decrease on the lower part of the deviated side and that on lateral parts of the non-deviated side showed an increase after treatment.
Ultra-low background alpha particle counter using pulse shape analysis

Abstract:The need to measure alpha particle emissivities at levels below 0.005 /spl alpha//cm/sup 2/-hr is becoming increasingly important in fundamental physics experiments (e.g. neutrino and rare decay measurements), environmental monitoring, nuclear activities monitoring and semiconductor packaging materials. Present counters can barely reach this level, being limited both by cosmic ray events and by their own alpha emissions. Here we report a detector capable of measurements at 0.00005 /spl alpha//cm/sup 2/-hr using a large electrode ionization chamber with digital pulse shape analysis to locate the point of emission of each alpha particle. Filled with 1 atm N/sub 2/, the counter is essentially blind to both environmental gamma-rays and cosmic ray muon showers, so its background becomes limited by the ability of the pulse shape analysis to distinguish different points of alpha particle origin. The counter's geometry intentionally exaggerates differences between signals originating from its different surfaces, with an inter-electrode separation D over 3 times the alpha particle range L. Since signal risetimes equal charge drift times, anode events have 8-10 /spl mu/s risetimes, while sample event risetimes are 30-35 /spl mu/s and readily distinguished. Integrated charge also increases with drift length, producing a 2 to 1 difference between sample and anode events. Applying both risetime and amplitude cuts distinguishes between sample and anode emitted alpha particles at about 1 part in 1000. A guard electrode surrounding the anode allows alpha particles from the counter's sidewalls to be rejected at a similar ratio, so that essentially only alpha particles emanating from the sample are finally counted.

Abstract:Background: Facial appearance is an important factor, affects social and psychological well-being. The ideal positioning of jaws and soft tissues is crucial during orthognathic surgery for a better outcome, but the response of facial soft tissues does not always reflect the exact movements of the underlying jaws in 1:1 ratio. So, soft tissue changes following orthognathic surgery require utmost attention during surgical correction to make successful treatment. Aims and Objectives: Evaluation of perioral soft tissue changes after orthognathic surgical procedures. The objectives of the study were to assess and compare pre and post-operative perioral soft tissue changes of lip width, nasolabial and mentolabial angle using Three Dimensional Computed Tomography scan (3DCT). Patient and Methods: The study involved ten patients for evaluation requiring orthognathic surgical procedures (maxillary or mandibular anteroposterior excess or deficiency, transverse deformities, vertical maxillary excess and facial asymmetry) presented to the department of oral and maxillofacial surgery during 2014-2016. Pre and post-operative 3DCT scan were taken after 12 months using iCT 256 slice whole body CT scanner and evaluated for changes using Dicom PMS D view. Results: Significant changes were observed in nasolabial angle after maxillary advancement (1.81°) and maxillary setback procedure (2.73°). The mentolabial angle was significantly increased with mandibular setback procedures (3.27°). Mandibular advancement procedures showed both increase (3.6°) and decrease (7.6°) in mentolabial angle. Conclusion: 3DCT showed a significant difference in perioral soft tissue changes in nasolabial and mentolabial angle but no significant change was observed in lip width. 3DCT is a reliable tool for 3D assessment. The conventional thought of changes in Nasolabial angle after surgery is changing due to the underlying factors which should be considered for prediction.

Abstract:OBJECTIVETo evaluate facial soft tissue 3-deminsion changes of skeletal Class III malocclusion patients after orthognathic surgery using structure light scanning technique.METHODSEight patients [3 males and 5 females, aged (27.08 ± 4.42) years] with Class III dentoskeletal relationship who underwent a bimaxillary orthognathic surgical procedure involving advancement of the maxilla by Le Fort I osteotomy and mandibular setback by bilateral sagittal split ramus osteotomy (BSSO) and genioplasty to correct deformity were included. 3D facial images were obtained by structure light scanner for all the patients 2 weeks preoperatively and 6 months postoperatively. The facial soft tissue changes were evaluated in 3-dimension. The linear distances and angulation changes for facial soft tissue landmarks were analyzed. The soft tissue volumetric changes were assessed too.RESULTSThere were significant differences in the sagittal and vertical changes of soft tissue landmarks. The greatest amount of soft tissue change was close to lips. There were more volumetric changes in the chin than in the maxilla, and fewer in the forehead.CONCLUSIONAfter biomaxillary surgery, there were significant facial soft tissue differences mainly in the sagittal and vertical dimension for skeletal Class III patients. The structure light 3D scanning technique can be accurately used to estimate the soft tissue changes in patients who undergo orthognathic surgery.

Abstract:The aim of this study was to generate a quantitative dynamic assessment of facial movement symmetry changes after orthognathic surgery. Twenty-five patients diagnosed with skeletal class III malocclusion with facial asymmetry who underwent bimaxillary surgery were recruited. The patients were asked to perform a maximum smile that was recorded using a three-dimensional facial motion capture system preoperatively (T0), 6 months postoperatively (T1), and 12 months postoperatively (T2). Eleven facial landmarks were selected to analyse the cumulative distance and average speed during smiling. The absolute differences for the paired landmarks between the sides were analysed to reflect the symmetry changes. The results showed that the asymmetry index of the cheilions at T2 was significantly lower than that at T0 (P = 0.004), as was the index of the mid-lateral lower lips (P = 0.006). The mean difference in cheilions was 2.13 ± 1.41 mm at T0, 1.33 ± 1.09 mm at T1, and 1.00 ± 0.98 mm at T2. The facial total mobility at T1 was significantly lower than that at T0 (P < 0.001), while the total mobility at T2 was significantly higher than that at T1 (P = 0.012). The orthognathic surgical correction of facial asymmetry was able to improve the associated asymmetry of facial movements.

Abstract:Objective: The present study aimed to determine the site and severity of maxillomandibular asymmetry before and after orthognathic surgery in asymmetric patients. Materials and methods: Presurgery and postsurgery cone beam computed tomography (CBCT) data of 21 facial asymmetry patients (7 males and 14 females, mean age: 23.0 ± 3.36 years) with soft tissue chin deviation ≥ 3 mm who had undergone bimaxillary surgery were evaluated. Seven midline and twenty bilateral hard tissue landmarks were identified for the evaluation of facial asymmetry and outcomes were assessed against age- and gender-matched control subjects. Results: In the asymmetry group, bilateral landmarks exhibited significant deviation in the mandible and midface regions. Before surgery, asymmetry was more severe at the mandibular midline and sites close to it, in the asymmetry group. Bimaxillary surgery proved to be highly effective, with a significant correction of the menton to a clinically normal value (2.90 mm, p < 0.001). After surgery, significant residual asymmetry was observed at the mental foramen (p = 0.001) in the R-L direction. Moreover, significant asymmetry persisted at the sigmoid notch (p = 0.001) in the S-I direction. Conclusions: Mandibular midline landmarks and chin peripheral regions contribute significantly to overall facial asymmetry characteristics. Despite significant correction after bimaxillary surgery, asymmetry persisted at several sites, thereby requiring secondary correction. Comprehensive 3D presurgical planning is central for asymmetry correction in a single surgery. Clinical relevance: The present study specifies the location of residual asymmetry sites and advocates the correction of those sites during initial surgery.
Assessment of Masseter Volume and Postoperative Stability After Orthognathic Surgery in Patients With Skeletal Class III Malocclusion With Facial Asymmetry

Abstract:Objective: This study aimed to evaluate the effect of orthognathic surgery on masseter volume in patients with skeletal Class III malocclusion with facial asymmetry and the effect of masseter volume on stability in orthognathic surgery. Methods: This research studied 16 patients with Class III malocclusion with facial asymmetry who received combined orthodontic-orthognathic treatment and underwent craniofacial computed tomography (CT) before (T0), 2 weeks after (T1), and 6 months after (T2) surgery. Three-dimensional (3D) CT images were retrospectively analyzed, using 3D volume reconstruction to obtain the masseter volume and examine the impact of the masseter volume on stability in orthognathic surgery. Results: A statistically significant difference ( P < 0.05) in the volume of the masseter was found up to 6 months after orthognathic surgery compared with the preoperative period, and the reduction in the masticatory muscle volume on the lengthened side is greater than on the shortened side ( P < 0.05). The volume of both masseters differed according to facial asymmetry, and the difference was significantly reduced after orthognathic surgery ( P < 0.05). During the period time (T1-T2), cephalometric maxillary marker points were not significantly different ( P > 0.05), and mandibular marker points were significantly anteriorly shifted ( P < 0.05). There was an association between the masseter volume and anterior shift of point B (R > 0.5, P < 0.05), the upward and anterior shifts of the gonion point differed between the lengthened and shortened sides ( P < 0.05). Conclusion: The size of the masseter becomes smaller 6 months after orthognathic surgery, and orthognathic surgery improves both bone and soft tissue symmetry. A larger sagittal relapse of mandibular setback occurred in patients with greater masseter volume. Considering these alterations may be helpful in planning orthognathic surgery.
Evaluation of soft and hard tissue changes after bimaxillary surgery in class III orthognathic surgery and aesthetic consideration

Abstract:Aims: The aim of this study was to evaluate hard and soft tissue change after bimaxillary surgery in class III patients by focusing on sella, nasion, A point (SNA) and sella, nasion, B point (SNB) angle and aesthetic outcome. Materials and methods: The sample consisted of 96 skeletal Class III patients (42 women, 54 men) with a mean age of 25 years with standard deviation (SD) of 8.4. The youngest patient was 16-years-old and the oldest 51-years-old at the time of surgery. In total, seven skeletal parameters, eight soft tissue parameters, and two dental parameters were evaluated on the cephalograms. Result: At the beginning of the treatment 49 Patients had SNA between 80° and 84°, 34 had SNA of less than 80° and 13 had SNA of more than 84°. Post surgically, 25 patients had SNA of 78°-84°, 19 had SNA less than 78° and 52 patients had SNA of more than 84°. Out of 96 patients 22 had SNB of 78°-82° before surgery, 16 had less than 78° and 58 had SNA of more than 84°. Postoperatively, we measured SNB of 78°-80° in 42, less than 78° in 18 and of more than 82° in 36 patients. The inclination of the maxilla relative to the cranial base changed from 7.2° (SD = 4)-8° (SD = 5.1) and the mandible changed from 35.7° (SD = 6.6) to 36° (SD = 6.3) postoperatively which was not significant. The distance from upper lip to E-line increased by 2.6 mm (SD = 3.9) after surgery (P < 0.001), while, the lower lip distance to E-line decreased slightly by 0.9 mm (SD = 3.2) (P < 0.01). Nasolabial angle was decreased by 9.5° (SD = 9.4) after surgery (P < 0.001). The nose prominence also decreased from 18.2 mm (SD = 3.5) -16.5 mm (SD = 3.3). Conclusion: Although in many cases we did not have a SNA angle or SNB angle in normal range but a good aesthetic outcome have been observed. Consequently our study showed that soft tissue change and aesthetic aspects should be considered in surgical planning and achieving SNA angle or SNB angle of norm range should not be the only goal. As we could show the advancement of maxilla will result in a better lip and nose profile and this should be considered in treatment planning.
Three-dimensional analysis of hard and soft tissue changes in skeletal class II patients with high mandibular plane angle undergoing surgery

Abstract:This study aimed to study 3-dimensional (3D) changes of hard and soft tissues of skeletal class II patients after 2-jaw surgery and genioplasty. 32 adult patients diagnosed with mandibular hypoplasia who underwent 2-jaw surgery of maxillary impaction, mandibular advancement and genioplasty were enrolled. Cone-beam computed tomography and 3D stereophotogrammetry was conducted 1 week before and 6 months after surgery. Dolphin imaging software was used to establish a 3D digitizing model and 3D measurement system. Paired t-test was performed to compare the values before and after surgery. Pearson's correlation test assessed the degree of correlations between hard and soft tissue change. The mean impaction of the maxilla was 2.600 ± 3.088 mm at A. The mean advancement of the mandible was 7.806 ± 2.647 mm at B. There was a significant upward and forward movement for most landmarks of the nose and lip, while a significant decrease in nasal tip height (lateral view), upper lip height, and upper and lower vermilion height. The nose's width was significantly increased. For maxillary, Sn, Ac-r, Ac-l, and Ls demonstrated a significant correlation with A and U1 in the anteroposterior axis. However, there were no significant correlations among them in the vertical axis. For mandibular, Li demonstrated a significant correlation with L1 in the anteroposterior axis specifically for the mandible. Notably, correlations between the landmarks of the chin's hard and soft tissues were observed across all axes. The utilization of 3-D analysis facilitated a quantitative comprehension of both hard and soft tissues, thereby furnishing valuable insights for the strategic formulation of orthognathic treatment plans targeting patients with skeletal class II conditions.

Abstract:Both deep understanding and reliable prediction of postoperative soft tissue changes are crucial for planning orthognathic surgery. Instead of estimating soft tissue responses by measuring individual landmark changes, this study aimed to investigate the relationship (ratio) between soft and hard tissue movements in different facial regions through three-dimensional cone-beam computed tomography (CBCT). Preoperative and postoperative CBCT images were superimposed using the surface registration method on the basis of the cranial base, and 10 facial regions of interest were defined. Region-based volumetric subtractions between the preoperative and postoperative segments were performed. The volumetric differences and surface of each region were used to estimate the average movement. Correlation and regression analyses were performed to examine the relationships between the corresponding soft and hard tissue movements. An overall pattern of facial soft tissue movement was observed in patients with prognathism who underwent orthognathic surgery. The experiment results have shown that mean ratios for the average soft-to-hard tissue movements in the facial regions varied, which may not exactly be similar to the published reports because of the population biocharacteristics and study methods, but the trend is in agreement with the previous studies. Additionally, the prediction capability of the regression model was significantly high, ranging from 0.786 to 0.857, in upper lip, upper vermilion, and chin regions, thus demonstrating that the skin outline changes in these critical regions could be reliably predicted from the underlying bone movements. These results could likely be applied in future soft tissue simulation in orthognathic surgery.

Abstract:Background: Triple jaw surgery, inclusive of a LeFort osteotomy, bilateral sagittal split osteotomy, and genioplasty, is used to maximize functional and esthetic outcomes for patients. This is achieved through the procedure’s measurable effects on the craniofacial skeleton but is also influenced by the soft tissue changes that occur as a result of the procedure. This study aims to characterize the three-dimensional (3D) soft tissue changes of triple jaw surgery. Methods: Patient demographics were collected along with pre and postoperative 3D images (3D VECTRA photosystem, Canfield, Fairfield, NJ). Orolabial anthropometric measurements were performed using Mirror (Canfield Scientific Inc., Fairfield, NJ). Results: Forty-eight 3D data sets were included. The male/female ratio was 0.6, with a mean age of 23.4 years. Significant postoperative decreases ( P <0.001) were seen in the mean absolute distance from the lower lip to S-line and distance from lower lip to H-line. In female patients, upper vermilion fullness, lower lip height, lower vermilion fullness, distance from lower lip to S-line, and absolute distance from lower lip to H-line were found to fall within ideal norms postoperatively. In male patients, lower lip height, lower vermilion fullness, distance from lower lip to S-line, and absolute distance from lower lip to H-line were found to fall within ideal norms postoperatively. Conclusions: Triple jaw surgery has a significant soft tissue impact and can transform some orolabial measurements to conform to ideal norms.

Abstract:The aim of this study was to analyze changes in soft tissue profile after mandibular advancement surgery, with special emphasis on the effect of skeletal relapse and different Class II facial patterns. The cephalometric radiographs of 30 consecutive patients (24 women and 6 men, mean age 23 years) who underwent sagittal split osteotomy were studied. The radiographs were taken immediately before operation, at one week and 14 months postoperatively. To analyze the possible influence of hyper- and hypodivergent facial patterns, the patients were classified into low- (4 patients), medium- (16 patients) and high-angle (10 patients) groups according to the magnitude of the mandibulonasal plane angle. The main movement occurred in the horizontal plane. Soft tissue pogonion and mentolabial fold were found to follow the underlying skeletal structures in a nearly 1:1 ratio. On final follow-up, skeletal relapse of 1.3 mm was measured at B-point and of 1.5 mm at pogonion. Taking the skeletal relapses into account, the ratios of both corresponding soft tissue references (alternative ratios) dropped to 60%. Soft tissue pogonion is the most reliable reference for the planning of mandibular advancement. The ratio of soft tissue movement to final skeletal position at the chin amounts to 60% for a realistic prediction. However, the low-angle group differed from other groups by showing a markedly low soft-to-hard tissue ratio of only 14% at pogonion and a high ratio of 109% at the mentolabial fold. However, these differences in ratios between the groups were statistically not significant.
Soft tissue changes during orthopedic therapy: An in vivo 3-dimensional facial scan study

Abstract:Introduction: The aim was to compare the soft tissue changes in pretreatment and posttreatment facial scans of patients who had undergone various orthopedic treatments vs a control group of untreated growing patients. Methods: Facial scans were performed before (T0) and after (T1) orthopedic treatment in 15 patients prescribed rapid palatal expander (RPE), 15 cervical headgear (HG), and 15 facemasks (FM), as well as 6 months apart in 15 untreated growing patients. After best-fit scan alignment using Geometric Control X software (3D Systems Inc, Rock Hill, SC), a 3-dimensional (3D) analysis of soft tissue changes was performed, comparing 3D reference points (total 22) and 8 areas on T0 and T1 scans. Kruskal-Wallis nonparametric tests and pairwise comparison with Bonferroni's correction were applied to identify any statistically significant differences among groups (P <0.05). All analyses were conducted with SPSS software (version 28; IBM, Armonk, NY). Results: At T1, reduced soft tissue projection was found at the nose and upper lip in the HG group, the lower lip in the HG and RPE groups, and the chin in the FM and RPE groups. The RPE group displayed a statistically significant increase in facial divergence, confirmed by gnathion position (RPE vs FM [P = 0.018] and RPE vs control [P = 0.046]), as well as an increase in the soft tissue projection of both cheeks (left cheek in range of 1-2 mm [P = 0.030] and range of 0 to -1 mm [P = 0.022]; right cheek in range of 1-2 mm [P = 0.003] and range -1 to -2 mm [P = 0.001]). There were no clinically significant differences among groups in mandibular right and left body areas. Conclusions: The 3D facial analysis revealed significant differences in soft tissues among orthopedic treatments, especially at the upper and lower lip and chin areas, as compared with untreated patients.

Abstract:Context: Orthognathic surgery brings about a harmonious relationship between jaws, resulting in improved facial aesthetics. It is key to analyze if satisfactory results can be attained by using virtual surgical planning for orthognathic procedures so as to recommend it for routine clinical practice. Aims: The aims of this study were to evaluate the various soft tissue changes that take place following orthognathic surgery using three-dimensional (3D) imaging and virtual surgical planning software and quantify the accuracy of virtual surgical planning software on patients undergoing orthognathic surgery. Settings and design: This is an observational prospective study with a sample size of 12. Methods and materials: In this prospective study, 12 patients undergoing orthognathic surgery were included following the inclusion and exclusion criteria. A usual pre-surgical work-up was done and a 3D replica of the facial skeleton was formulated using the software with the DICOM data acquired from the patient i.e., CT and scans of patient's dentition. Virtual surgical planning was done and splints were manufactured according to the desired newly achieved position. Patients were operated following the virtual surgical plan guided by the computer-aided design and computer-aided manufacturing (CAD-CAM) splints. Post-operative evaluation was done. As the study is more of a descriptive study to obtain detailed knowledge of a new procedure, only one group is being studied and hence there is no statistical testing included in this study. Results: The mean discrepancy noticed after superimposition of soft tissue points was 0.92 with a standard deviation of 0.3. Conclusions: 3D CT virtual surgical planning is a reliable tool to achieve predictable and reliable post-operative results in orthognathic surgical cases.
Accuracy of 3-dimensional soft tissue prediction for orthognathic surgery in a Chinese population

Abstract:Objectives: This study aims to determine the validity of a 3D planning software in predicting the soft tissue outcome of Chinese patients undergoing orthognathic surgery for correction of Skeletal III dentofacial deformity. Methods: Pre- and post-operative 3D facial stereophotogrammetric scans and cone beam computed tomography were taken for 10 Chinese patients who had underwent orthognathic surgery. The pre-operative 3D facial scan was integrated with the pre-operative CBCT using the ProPlan CMF software. The simulated soft tissue 3D face was then compared with the actual 3D facial scan obtained at least 6 months postoperatively. Two outcome measures were computed as follows (i) mean absolute difference between meshes (ii) percentage of points where the distance between the two meshes is 2mm or less. Results: The mean absolute difference between the predicted and actual soft tissue surface meshes for the full face and the 6 anatomic regions ranged from 0.72mm to 1.42 mm. The mean absolute distance between the meshes for all the anatomic regions were within 2 mm (p<0.05). The percentage of mesh points with less than 2mm error ranged from 72.5% to 92.5%. The accuracy of soft tissue prediction, assessed using mean absolute distance for the full face, was significantly correlated to the amount of sagittal surgical movement (r=0.707, p=0.022). The lower lip was also found to be the least accurate. Conclusions: Using ProPlan CMF, the accuracy of 3D soft tissue predictions for bimaxillary orthognathic surgery in Chinese Skeletal III patients were clinically satisfactory.
Correlation of Orthognathic Surgical Movements to Perception of Facial Appearance in Patients With Cleft Lip and Palate

Abstract:Study design: Cephalometric scans were compared before and after surgery to assess the degree of correction. Correlations between skeletal movements and survey outcomes were determined using multivariate regression analysis. Objective: This study aims to identify relationships between subjective observer-reported improvements in esthetics and emotional appearance with specific surgical movements. Methods: Ten patients at a single tertiary institution (average age: 18.1 ± 0.8), 9 males and 1 female, underwent orthognathic repair and had comprehensive cephalometric records. Standardized anterior posterior and lateral pre and postoperative photographs of patients were included in a survey to clinicians to assess noncognitive domains on a Likert Scale (1-10). CLEFT-Q was administered to gauge patient satisfaction in categories of appearance, speech, and quality of life. Results: Per clinicians, multiple domains increased including facial attractiveness (4.1 ± 0.7 versus 7.3 ± 0.7, P < 0.001), friendliness (4.5 ± 0.4 versus 7.3 ± 0.5, P < 0.001), confidence (4.1 ± 0.4 versus 7.1 ± 0.4, P < 0.001), and recommendation for surgery decreased (8.9 ± 0.1 versus 3.6 ± 0.5, P < 0.001). Speech distress decreased with increased SNA and convexity, whereas Psychological and Social scores decreased with an increased ANB. Functional eating and drinking scores increased with maxillary depth. Conclusions: Orthognathic surgery improves many noncognitive domains in patients with cleft lip and palate as assessed by both patients and clinicians on all aspects of facial attractiveness and perception. These findings demonstrate objective bases of skeletal adjustments for perceived improvements in facial appearance and emotion.
