Abstract:Objective To assess the feasibility,efficacy and safety of bladder neck preservation (BNP) in endoscopic extraperitoneal radical prostatectomy (EERP),and evaluate the influence on postoperative early continence.Methods Preoperative data,operative time,blood lose,pathological staging,margin status and early continence status from 34 BNP and 38 standard technique (ST) in EERP were retrospectively assessed.Urinary incontinence was evaluated based on the NCI Common Toxicity Criteria (version 2).Grade 0 is defined as no incontinence; Grade 1 incontinence occurs with coughing,sneezing,or laughing; Grade 2 is spontaneous incontinence with some control; and Grade 3 is no control.Number of daily pads usage were also used to measure urinary incontinence.0-1 pad/day was defined as urinary continence,2-3 pads/day was defined as mild incontinence,＞ 3 pads/day was defined as incontinence.Results Both groups were similar in age,body mass index,prostate volume,prostate-specific antigen and Gleason score (P ＞ 0.05).There was no significant difference in operative time,blood loss,catheter removal time,postoperative hospital stay,and postoperative PSA levels (P ＞ 0.05).There were 4 cases of positive margin in BNP and ST groups respectively.There were 2 cases of urine leakage in BNP group,and 6 in ST group.The cases of grade 0 in BNP group in first day,1 week,1 month and 3 months after catheter removal were3 (8.8％),9 (26.5％),16 (47.1％),24 (70.6％).The cases of 0-1 pad/day were7 (20.6％),12 (35.3％),22 (64.7％),28 (82.4％),respectively.The cases of grade 0 in ST group in first day,1 week,1 month and 3 months after catheter removal were 1 (2.6％),2 (5.3％),5 (13.2％),13 (34.2％).The eases of 0-1 pad/day were2 (5.3％),5 (13.2％),10 (26.3％),20 (52.6％),respectively.There were significant differences in postoperative early continence between BNP and ST groups (P ＜ 0.05).Conclusion EERP with BNP could be feasible and safe,with a positive impact on postoperative early continence.

Abstract:n Russian and Western debates about developments in the post–Soviet world, the term “reforms” has become a kind of magic fetish. The mere mention of reforms, like a shamanistic incantation, unleashes a storm of passions across the spectrum of public opinion. However, the specific meanings of the term are often as murky and diverse as the interests and goals of those who invoke the word. Profound and substantive differences exist over what is meant by “Russia’s reforms”—differences between Russians and Westerners, as well as between various intellectual and political camps in Russia itself. Thus, in the view of most present-day Western observers, as well as old-style Westernizers inside Russia (including most of the orthodox Marxists in the early twentieth century), reform has been deterministically linked to the idea of modernization and more generally to a belief in the linear progress of civilization. In this conception of history, all nations are perceived as developing, perhaps at different speeds, in the direction of a single universal standard. One of the popularizations of this doctrine was the much advertised essay by Francis Fukuyama on “the end of history.”1 However, it is clear that in late Soviet and post–Soviet Russia the number of proponents of the optimistic view of modernization and a linear perception of history has been shrinking. By contrast, an increasing number of Russian historians and social scientists have embraced variations of the cyclical paradigm of change. The roots of this approach go back to Heraclitus and, in modern times, to Giambatista Vico, whose teaching about the corsi e ricorsi, the ebbs and flows of history, was the first in modern Western thought to challenge the doctrine of universal and irreversible historical progress. Although few serious scholars would interpret the historical cycles as mere repetition without development, establishing parallels between distant periods of Russian history has long been characteristic of Russians’ view of their past, present, and even future. Abstract:Objective:To compare the efficacy and safety between plasma kinetic enucleation resection of the prostate(PKERP)and transurethral plasmakinetic prostate resection(PKRP)in treating benign prostatic hyperplasia(BPH).Methods:We retrospectively reviewed 29 cases treated with PKERP and 21 cases treated with PKRP by a single doctor between 2009 and 2011.Surgical time,catheterization time,vesical irrigation time,change of hemoglobin,blood transfusion rate,hospitalization,IPSS and QOL scores and postoperative complications were compared between the two groups.Results:Compared the PKRP group,surgical-time,postoperative bladder irrigation time, change of Hemoglobin,blood transfusion rate,catheterization time and hospitalization,complications were all similar.IPSS and QOL scores of 2 groups were obviously improved compared with the preoperative scores,but they were similar between the two groups.Conclusions:Both PKERP and PKRP were equally efficent and can be safe performed in patients with BPH.

Abstract:This paper presents an accelerometer interface circuit using a vacuum packaged MEMS sensor element. In order to damp the sensor element, increase the linearity and improve the input range of the sensor, a closed-loop analog SC interface with a PD-controller is used. The theoretical input range of sensor is ±1.4 g while measured range of the sensor is from −0.7 g to +1.4 g. The interface achieves maximum non-linearity of 0.67% over temperature range of −40 to +85 °C. The sensor has bias stability of 11 µg and the noise density of 2 µg√Hz. The interface has been implemented in a 0.35 µm CMOS process with a total active area of 1.3 mm2, and it consumes 2.4 mA from a 3.6 V supply.
Two-year Outcome of High-risk Benign Prostate Hyperplasia Patients Treated with Transurethral Prostate Resection by Plasmakinetic or Conventional Procedure

Abstract:OBJECTIVE To perform a systematic comparison of transurethral plasmakinetic resection of the prostate (PKRP) to conventional transurethral resection of the prostate for treating benign prostate hyperplasia (BPH) in aged high-risk patients. METHODS Three hundred twenty-nine symptomatic patients diagnosed with BPH underwent endourological treatment by transurethral resection of the prostate (n = 136) or PKRP (n = 193). Preoperative and postoperative assessments were conducted for the International Prostate Symptom Scores, quality of life (QoL), postvoid residual urine (PVRU) volumes, maximal urine flow rates (Q(max.)), and prostate-specific antigen. Perioperative data were collected for operative time, weight of resected tissue, blood loss, cases of open surgery, duration of bladder irrigation, and duration of catheter use. Patients were re-evaluated at postoperative months 3, 6, 12, 18, and 24. Postoperative complications were recorded. RESULTS In the perioperative period, no significant differences were found between the 2 surgery groups for weight of resected tissue or cases of open surgery. However, PKRP was associated with significantly shorter operative time, duration of bladder irrigation, and duration of catheter use, as well as less blood loss. At the 2-year follow-up, both procedures were found to have significantly improved International Prostate Symptom Scores, QoL, Q(max.), PVRU, and prostate-specific antigen. In addition, each procedure was associated with some postoperative complications, some of which were significantly reduced in one over the other, such as secondary hemorrhage in PKRP. CONCLUSION The currently available endourological treatments, transurethral resection of the prostate, and PKRP, are safe and effective therapies for treating aged high-risk patients with benign prostatic hyperplasia (BPH), although PKRP is superior in many aspects, including perioperative outcomes. Abstract:Objective:To compare the short-term efficacy and safety between 1 470 nm diode laser enucleation of the prostate (DiLEP) and plasmakinetic enucleation of the prostate (PKEP) in treating benign prostatic hyperplasia (BPH).Method:From August 2016 to July 2017,a total of 180 consecutive BPH patients with lower urinary tract obstruction were randomly assigned to either 1 470 nm DiLEP group (n=90) or PKEP group (n=90)respectively in our institution.All patients enrolled in the present study have been evaluated preoperatively and followed-up at 1 and 3 months postoperatively.Preoperative characteristics,intraoperative data and postoperative outcomes of all patients were recorded and compared.Result:There was no statistical difference in the preoperative characteristics between the DiLEP and PKEP group (P＞0.05).Compared with PKEP,the DiLEP group had significantly shorter operative time,postoperative bladder irrigation time,postoperative catheterization time and less drop in postoperative serum hemoglobin and sodium levels (all P＜0.001).No severe intraoperative complications such as bladder perforation,excessive haemorrhage or transurethral resection syndrome occurred.DiLEP group was equivalent to PKEP group in International Prostate Symptom Score,quality of life scores,Qmax and post-voiding residual volume before and after surgery (all P＞0.05).The overall follow-up period ranged from 3-14 months,and the mean follow-up period (mean±SD) was (7.2±2.4) months.During the follow-up period,no incontinence or urethral stricture was observed in the DiLEP group.Only one patient had hematuresis one week after surgery and was cured by symptomatic treatment.Whereas,two patients (2.2％) suffered from incontinence in PKEP group.Conclusion:The present study verified the advantages of 1 470 nm DiLEP in treating patients with symptomatic BPH,as the efficacy of DiLEP and PKEP were similar while the former technique provides less risk of perioperative complications,shorter operative time and reduced bladder irrigation and catheterization time.

Abstract:Introduction: Although transurethral resection of prostate (TURP) remains the reference standard for benign prostate hyperplasia (BPH), the concern about complications promotes researchers to develop alternative surgical methods with fewer complications. In this study, we compared the safety and efficacy between the transurethral plasma kinetic enucleation of prostate (TUPKEP) and transurethral plasma kinetic resection of prostate (TUPKRP), mainly including absorption of irrigation fluid, the operation time, the weight of prostate tissue removed and severe complications. Methods: Sixty BPH patients were randomly and evenly assigned to the TUPKEP or TUPKRP group. The irrigation fluid used in both groups was 1% ethanol-containing saline solution. The ethanol concentrations in the subjects' end expiration were measured during operation. The volume of irrigation fluid absorbed was calculated accordingly. Results: No significant difference was found in operation time between two groups, whereas the weight of prostate tissue resection was significantly higher in the TUPKEP than that in the TUPKRP group. Conclusion: The study provides evidence for the safety, feasibility and effectiveness of both bipolar transurethral techniques. Further, compared to the TUPKRP group, the TUPKEP group has more efficient for resection of prostatic hyperplasia tissue, even though in terms of fluid absorption, no difference has been found in both groups. Ethanol monitoring is simple, safe and effective, which is beneficial for enhancing safety procedures.

Abstract:Objective To evaluate the clinical effect in the near future of transurethral plasma kinetic resection versus transurethral resection of the prostate for the treatment of symptomatic benign prostatic hyperplasia(BPH).Methods 80 patients who were diagnosised BPH actually were divided into two groups and treated by either TUPKRP or TURP. The perioperative marker and theraputic results were recorded and analyzed.Results There was no significant difference of preoperative factors between the two groups.6 months postoperatively, IPSS,QOL and Q_(max) were significantly improved.Nosignicicant difference in the improvement of subjective sympotoms and objective results has been noted with the different procedures.The incidence of blood transfusion and TRUS occuring,the mean bladder irrigating time,catheration time,hospital stay time were significant shorter in TUPKRP than in TURP.Conclusions Although TUPKRP is as effective as TURP for the treatment of symptomatic BPH.TUPKRP has less adverse side effect and recovering time than TURP.Thus TUPKRP is a better treatment option for BPH.
Impact of Plasmakinetic Resection of Prostate on Sexual Function in Patients with Benign Prostatic Hyperplasia

Abstract:Objective To investigate the efficacy of plasmakinetic resection of the prostate (TKRP) in patients with benign prostatic hyperplasia (BPH), and its impact on sexual function. Methods 130 patients with BPH in our hospital from January 2009 to June 2011 were equally divided into the TKRP group ,and the transurethral prostate electric resection (TURP) group. The international prostate symptom score (IPSS),quality of life (QOL) score, maximum urinary flow rate (Qmax), operative time, weight of resected prostate tissue ,blood loss, indwelling catheter time, postoperative hospital stay, the incidence of erectile dysfunction (ED) and retrograde ejaculation (RE) were observed. Results The intraoperative blood loss, catheterization time and postoperative hospital stay were decreased significantly in the TKRP group than those in TURP group (P0.01). The surgery time and the removal of tissue were not statistically significant between the two groups (P0.05). The levels of postoperative IPSS, QOL and residual urine were significantly decreased. The level of Qmax after surgery was increased significantly in both groups compared with the preoperative (P 0.01). After treatment, there was no difference between the two groups (P 0.05). The incidence of ED in the TKRP group at 6th and 9th month after the treatment was lower than those in the TURP group (P0.05), but the RE was not significant (P0.05). Conclusion The advantage of TKRP is little trauma, quicker recovery, and little impact on sexual function.

Abstract:Objective To improve the successful rate of operation, shorten the operation and decrease the complication rate of surgery. Methods The patients who needed to receive transurethral resection of prostate were randomly separated into two groups:In group 1 the patients received the suprapubic needle aspiration of bladder before the transurethral bipolar plasmakinetic resection of the prostate; in group 2 the patients didn't receive the suprapubic needle aspiration of bladder. The volume of resected prostate, the operation time, the average time of urethral catheterization, the average length of stay, the IPSS post operation and the complication were compared. Results The operation time, the average length of stay and the complication rate reduced in Group 1, however there were no significant difference in the IPSS post operation and the maxi mum flow rate between the two groups. Conclusion The advantages of the suprapubic needle aspiration of bladder combined with transurethral bipolar plasmakinetic resection of the prostate are clear field of view during operation, shortening operation time and decreasing complication of PKRP.
Bipolar Transurethral Enucleation and Resection of the Prostate Versus Bipolar Resection of the Prostate for Prostates Larger Than 60gr: A Retrospective Study at a Single Academic Tertiary Care Center

Abstract:OBJECTIVE:To evaluate the efficacy and safety of bipolar transurethral enucleation and resection of the prostate (B-TUERP) versus bipolar transurethral resection of the prostate (B-TURP) in the treatment of prostates larger than 60g.MATERIAL AND METHODS:Clinical data for 270 BPH patients who underwent B-TUERP and 204 patients who underwent B-TURP for BPH from May 2007 to May 2013 at our center were retrospectively analyzed. Outcome measures included operative time, decreased hemoglobin level, total prostate specific antigen (TPSA), International Prostate Symptom Score (IPSS), maximal urinary flow rate (Qmax), quality of life (QoL) score, post void residual urine volume (RUV), bladder irrigation duration, hospital stay, and the weight of resected prostatic tissue. Other measures included perioperative complications including transurethral resection syndrome (TURS), hyponatremia, blood transfusion, bleeding requiring surgery, postoperative acute urinary retention, urine incontinence and urinary sepsis. Patients in both groups were followed for two years.RESULTS:Compared with the B-TURP group, the B-TUERP group had shorter operative time, postoperative bladder irrigation duration and hospital stay, a greater amount of resected prostatic tissue, less postoperative hemoglobin decrease, better postoperative IPSS and Qmax, as well as lower incidences of hyponatremia, urinary sepsis, blood transfusion requirement, urine incontinence and reoperation (P<0.05 for all).CONCLUSIONS:B-TUERP is superior to B-TURP in the management of large volume BPH in terms of efficacy and safety, but this finding needs to be validated in further prospective, randomized, controlled studies.

Abstract:The aim of this study was to compare the clinical results of plasmakinetic resection of the prostate (PRP) with standard transurethral resection (TUR) of the prostate (TURP). A total of 240 patients (mean age 63.5; age range 52–90 years), with symptomatic benign prostatic hyperplasia were randomized into two groups and treated with two different techniques (TURP and PRP). We evaluated pre-operative, per-operative and post-operative (first and 12th months) findings of all patients. The mean catheterization time was 3 and 4.5 days in the PRP and standard TURP groups, respectively (P<0.001). We observed the improvements in maximum flow rates in PRP group were significantly higher than TURP group (P<0.001). TUR of the prostate using plasmakinetic energy seems to be a promising treatment alternative to conventional TURP. It has the advantages of low intraoperative and post-operative complications, short convalescence, excellent intraoperative hemostasis, absence of fluid absorption and TUR syndrome.

Abstract:Theoretical accounts of post-traumatic stress disorder (PTSD) suggest that memory for a precipitating event is crucial for its development. Indeed, Sbordone and Liter (1995) have recently argued that mild traumatic brain injury and PTSD are mutually exclusive disorders. A case is described who sustained a severe head injury in a road traffic accident. He had a retrograde amnesia of two days and a post-traumatic amnesia of four weeks. Six months after his accident he was found to be suffering from a number of anxiety symptoms, including nightmares and intrusive thoughts, consistent with a diagnosis of PTSD. The implications of this case for theories of PTSD are discussed.

Abstract:Objective: To compare thulium laser enucleation of the prostate (ThuLEP) with plasma kinetic resection of the prostate (PKRP) in the treatment of BPH. Methods: We retrospectively analyzed the medical records of 160 cases of BPH treated by ThuLEP (the observation group, n = 80) or PKRP (the control group, n = 80) in our hospital from January 2021 to December 2023. We recorded the operation time, bladder irrigation time, catheter retention time, hospitalization time, postoperative complications, and pre- and postoperative maximum urinary flow rate (Qmax), residual urine volume (PVR), prostate-specific antigen (PSA) and prostate volume, followed by comparison of the data obtained between the two groups of patients. Results: Compared with the controls, the patients of the observation group showed significantly shorter operation time (［67.25 ± 7.24］ vs ［60.10 ± 5.15］ min, P< 0.05), bladder irrigation time (［46.90 ± 10.77］ vs ［43.24 ± 6.65］ h, P< 0.05), catheterization time (［5.60 ± 1.31］ vs ［5.03 ± 1.24］ d, P< 0.05) and hospitalization time (［7.31 ± 2.00］ vs ［6.55 ± 1.67］ d, P< 0.05), higher Qmax (［18.50 ± 1.24］ vs ［20.68 ± 1.45］ ml/s, P< 0.05), lower PVR (［12.10 ± 3.53］ vs ［10.82 ± 3.10］ ml, P< 0.05), PSA (［4.60 ± 0.78］ vs ［3.38 ± 0.40］ μg/L, P< 0.05) and prostate volume (［25.35 ± 6.46］ vs ［20.12 ± 5.13］ ml, P< 0.05) at 3 months after surgery, but no statistically significant difference in the total incidence of postoperative complications (7.50% ［6/80］ vs 5.00% ［4/80］, P > 0.05). Conclusion: ThuLEP, with its advantages of notable effect, short operation and hospitalization time, significant improvement of urinary flow dynamics and prostate function, deserves clinical promotion for the treatment of BPH.

Abstract:Objective: We conducted a study to assess the effectiveness and safety of three different surgical instruments for enucleation in treating high-risk benign prostatic hyperplasia (BPH). These instruments include red laser, green laser, and plasma surgical equipment for enucleation of the prostate. Method: In a retrospective analysis, 237 patients diagnosed with high-risk benign prostatic hyperplasia (BPH) underwent prostate enucleation using three different groups of surgical instruments at the Department of Urology, the First Affiliated Hospital of Jinzhou Medical University. These groups included the red laser device group (n = 67), the green laser device group (n = 61), and the plasma device group (n = 109). The study evaluated changes in prostate mass, blood loss, operation time, and postoperative efficacy at 1- and 6-month intervals, as well as any associated complications. Results: The red and green laser surgical instrument groups demonstrated superiority over the plasma group in several aspects. (1) Operation Time: *1* Red laser group: 87.9 ± 14.7 minutes; *1* Green laser group: 86.1 ± 15.3 minutes. (2) Blood Loss: *1* Red laser group: 30.1 ± 5.9 mL; *2*Green laser group: 30.9 ± 6.1 mL. (3) Temporary Urinary Incontinence: *1*Red laser group: 3 cases (4.48%); *2*Green laser group: 2 cases (3.28%). These differences were statistically significant. Additionally, postoperative indicators such as IPSS(International Prostate Prostate Symptom Score), QoL(Quality of Life), Q max(The Maximum Flow Rate), and RUV(Residual Urine Volume) showed significant improvement in all three groups compared to the preoperative state. Conclusion: Red laser, green laser and plasma surgical devices were safe and effective in the treatment of high critical benign prostatic hyperplasia; laser surgical devices showed better performance in terms of operation time, intraoperative bleeding and complications.

Bipolar plasma enucleation of the prostate vs open prostatectomy in large benign prostatic hyperplasia cases - a medium term, prospective, randomized comparison

Abstract:WHAT'S KNOWN ON THE SUBJECT? AND WHAT DOES THE STUDY ADD?: According to the EAU Guidelines 2012, large size benign prostatic hyperplasia (BPH) cases (>80 mL) continue to have open prostatectomy as the first line treatment alternative, despite the substantial peri-operative morbidity and extended catheterization and convalescence periods related to this undoubtedly invasive approach. During the past two decades, holmium laser enucleation of the prostate was constantly described as a successful choice for this category of patients. According to rather numerous studies, the technique displayed superior results in terms of surgical safety and postoperative recovery compared with the open procedure. On the other hand, the concept of electrosurgical enucleation of the prostate, using either a monopolar or bipolar cutting current, materialized into several technical applications that eventually failed to gain general acknowledgement as reliable alternatives to the BPH transurethral approach. While keeping in mind the already proved advantage of enucleating substantial quantities of BPH tissue, bipolar plasma enucleation of the prostate was introduced as a novel endoscopic approach in cases of large prostates. The present trial represents the first prospective, medium-term, randomized comparison to be published of this innovative technique with standard open prostatectomy. Basically, the premises for a viable alternative relied on the practical advantages provided by the 'button' electrode, mainly the large surface creating the conditions for a fast enucleation process, continuous vaporization and concomitant haemostasis. Eventually, it was concluded that the plasma enucleation procedure distinguished itself as a successful treatment option in large BPH patients, characterized by good surgical efficiency, significantly reduced complications, faster postoperative recovery, similar prostatic tissue ablation capabilities and satisfactory follow-up results compared with the open technique. Most importantly, plasma-button enucleation patients benefited from a similar 12 months' outcome from the perspectives of symptom scores and voiding parameters when drawing a parallel with open surgery results, thus underlining the reliable viability of this type of endoscopic approach. Objectives: To evaluate the viability of bipolar plasma enucleation of the prostate (BPEP) by comparison with open transvesical prostatectomy (OP) in cases of large prostates with regard to surgical efficacy and peri-operative morbidity. To compare the medium-term follow-up parameters specific for the two methods. Patients and methods: A total of 140 benign prostatic hyperplasia (BPH) patients with prostate volume >80 mL, maximum flow rate (Qmax ) <10 mL/s and International Prostate Symptom Score (IPSS) >19 were randomized in the two study arms. All cases were assessed preoperatively and at 1, 3, 6 and 12 months after surgery by IPSS, Qmax , quality of life score (QoL) and post-voiding residual urinary volume (PVR). The prostate volume and prostate specific antigen (PSA) level were measured at 6 and 12 months. Results: The BPEP and OP techniques emphasized similar mean operating durations (91.4 vs 87.5 min) and resected tissue weights (108.3 vs 115.4 g). The postoperative haematuria rate (2.9% vs 12.9%) as well as the mean haemoglobin drop (1.7 vs 3.1 g/dL), catheterization period (1.5 vs 5.8 days) and hospital stay (2.1 vs 6.9 days) were significantly improved for BPEP. Recatheterization for acute urinary retention was more frequent in the OP group (8.6% vs 1.4%), while the rates of early irritative symptoms were similar for BPEP and OP (11.4% vs 7.1%). During the follow-up period, no statistically significant difference was determined in terms of IPSS, Qmax , QoL, PVR, PSA level and postoperative prostate volume between the two series. Conclusions: BPEP represents a promising endoscopic approach in large BPH cases, characterized by good surgical efficiency and similar BPH tissue removal capabilities compared with standard transvesical prostatectomy. BPEP patients benefited from significantly reduced complications, shorter convalescence and satisfactory follow-up symptom scores and voiding parameters.
Comparison of holmium laser enucleation of prostate and plasmakinetic enucleation of prostate for the treatment of benign prostatic hyperplasia

Abstract:Objective:To compare the safety and efficacy of the holmium laser enucleation of prostate(HOLEP)and plasmakinetic enucleation of prostate (PKERP)for the treatment of benign prostatic hyperplasia(BPH).Methods:A total of 80 patients with BPH were selected and randomly divided into two groups,with 40 cases in each group,and one group was received HOLEP treatment,while the other group was received PKERP treatment.The operation time,surgery quality, change of hemoglobin before and after surgery,bladder irrigation time,hospitalization time,surgical complications and short-term efficacy of two methods were compared.Results:The operation time in HOLEP group and PKERP group was (75.60± 16.95)min and (95.30±21.57)min,respectively.The bladder irrigation time was (16.4±4.67)h and (18.65±3.67)h, respectively.The average hospital stay time was (7.66±0.87)d and (8.19±1.2)d,respectively.The change of hemoglobin was (7.35±6.14)g/L and (10.25±6.43)g/L,respectively (P<0.05).There was no statistical significance of surgery quality,average catheter indwelling time and incidence rate of complications found in two groups.The statistical significances were found between the IPSS,QOL,Qmax and PVR before surgery and the 3rd and 6th months after surgery (P<0.05), while nostatistical difference was found between two groups.Conclusions:The HOLEP group and PKERP group had relatively equal short-term treatment efficacy,while the HOLEP group with less bleeding,short operation time,short bladder irrigation time,and short hospitalization time.Therefore,the HOLEP can be the golden standard of BPH surgery,while PKERP can be applied in surgery without holmium laser.

Abstract:BACKGROUND:Transurethral resection of prostate (TURP) has been considered as the standard treatment for benign prostatic hyperplasia (BPH). However, issues that have not yet been overcome for TURP include bleeding and absorption of irrigation fluid. Thus, novel improvement of the surgery is necessary. This study aimed to evaluate the efficacy and safety of bipolar plasma vaporization of the prostate (BPVP) with "button-type" electrode against standard TURP for BPH.METHODS:From January 2009 to January 2012, 30 patients who scheduled for surgical treatment of BPH surgical treatment were enrolled in the trial with provided consent for the study. Patients were prospectively randomized 1:1 to undergo either BPVP or TURP. Participants were blinded to the randomization scheme. All cases were assessed preoperatively and followed at 1, 3, and 6 months postoperatively by indwelling catheter time, blood loss, hospital stays, International Prostate Symptom Score (IPSS), quality of life (QOL), and Qmax.RESULTS:BPVP was significantly superior to TURP in terms of indwelling catheter time ((4.1 ± 4.1) days vs. (6.8 ± 6.8) days, P = 0.000), blood loss ((64.7 ± 103.8) ml vs. (254.7 ± 325.4) ml, P = 0.040), hospital stay ((8.7 ± 1.0) days vs. (11.7 ± 1.5) days, P = 0.000), IPSS ((4.2 ± 8.0) vs. (9.3 ± 3.7), P = 0.049), QOL ((1.5 ± 0.8) vs. (2.6 ± 1.4), P = 0.027), Qmax ((16.3 ± 5.7) ml/s vs. (12.5 ± 3.1) ml/s, P = 0.038), hemoglobin ((130.7 ± 9.4) g/L vs. ((122.1 ± 11.9) g/L, P = 0.047), Na(+) level ((138.6 ± 2.1) mmol/L vs. ((137.2 ± 2.0) mmol/L, P = 0.046) and operation time ((39.0 ± 15.5) minutes vs. ((69.3 ± 24.8) minutes, P = 0.004). And there were no statistical differences between BPVP group and TURP group in preoperatively assessment: patient's age ((70.9 ± 7.1) years vs. (71.9 ± 6.1) years, P = 0.736), IPSS ((24.6 ± 4.7) vs. (27.3 ± 5.9), P = 0.100), QOL ((5.1 ± 0.8) vs. (5.1 ± 1.0), P = 0.940), Qmax ((4.4 ± 2.7) ml/s vs. (5.3 ± 2.6) ml/s, P = 0.314), hemoglobin ((137.4 ± 8.7) g/L vs. (139.2 ± 10.4) g/L, P = 0.623), Na(+) level ((140.5 ± 1.8) mmol/L vs. (141.3 ± 1.4) mmol/L, P = 0.192) and prostate volume ((59.0 ± 17.4) ml vs. (70.1 ± 28.8) ml, P = 0.276).CONCLUSIONS:Compared with TURP, BPVP with "button-type" electrode shows superior efficacy and safety. Therefore, BPVP with "button-type" electrode represents a valuable endoscopic treatment alternative for BPH patients.
