Abstract:Introduction Many patients with schizophrenia are unable to achieve adequate levels of psychosocial functioning and quality of life despite of the remission of illness symptoms. According to previous reports, only one-third of patients with symptomatic remission reach functional remission. While current pharmacotherapy options seem to be relatively effective for different symptoms of schizophrenia (e.g. positive symptoms), more specific psychosocial interventions that could enable functional remission are yet to be developed. Objectives Our objective is to investigate differences in psychopathology, quality of life, functioning, and achieving functional remission before and after specific group treatments developed in our clinic. Methods We will conduct a prospective study including a consecutive cohort of female patients older than 18 years of age, which fulfilled the criteria for schizophrenia and schizoaffective disorder according to the International Classification of Disorders, 10th revision. Exclusion criteria are intellectual disabilities, mental disorders due to known physiological or neurological conditions, lactation or pregnancy, treatment with medications that can provoke psychosis, alcoholism, and other addictions. Patients will be recruited after finished hospital treatment or during individual outpatient controls. The Recovery Helm will be used at the beginning of the treatment, to make individual treatment plan and include patients in specific programs including day hospital treatment and/or outpatient group programs: psychoeducation, relaxation, metacognitive training, and social skills training. Besides collecting sociodemographic data, pre- and post-treatment assessment will include the Positive and Negative Syndrome Scale (PANSS), the Global Assessment of Functioning (GAF), the Quality of Life Scale (QLS), and the "Functional Remission of General Schizophrenia" (FROGS) scale. Results We will analyze the changes in psychopathology levels, quality of life, functioning, and achieving functional remission between the two assessment points, taking into account different treatment possibilities. Conclusions Evaluation of current available programs can help with recognition of specific needs of patients with schizophrenia and provide guidelines for further development of treatment programs that could be helpful in achieving functional remission. Abstract:Background: Clinical remission is a step towards functional remission for subjects with schizophrenia. While recovery is both a subjective personal journey and a clinical outcome to be targeted, data on patient self-rated outcomes are scarce. Objectives: (i) To determine the extent to which the association between clinical and functional remission is mediated by the subjective experience of recovery as reported by patients versus their relatives or their psychiatrist and (ii) to assess differences according to treatment, specifically with oral antipsychotics only versus long-acting injectable antipsychotics (LAIs). Design: Clinical observational study. Methods: Community-dwelling participants with schizophrenia enrolled in the EGOFORS cohort ( N = 198) were included. Clinical symptoms and remission were assessed using the Positive and Negative Syndrome Scale. Functional remission was assessed with the Functional Remission of General Schizophrenia Scale. Awareness of recovery was assessed with one question ‘What percentage of recovery do you think you have now (from 0% – no recovery – to 100% – full recovery)?’, asked of the patient, also of the patient’s close relative, and the psychiatrist. We used mediation analyses, taking into account the type of pharmacological treatment. Results: Remission criteria and perceived remission measures were significantly correlated, both within and between groups ( r > 0.330). The patient’s awareness of recovery mediated the relationship between clinical remission and level of functional remission, while the level of recovery according to psychiatrists or close relatives did not. The direct effect of clinical remission on the level of functional remission became non-significant when taking into account the mediator (patients’ awareness of recovery) in the group of patients with LAI ( t = 1.5, p = 0.150) but not in the group of patients with other treatments ( t = 3.1, p = 0.003). Conclusion: Patients with LAIs may be more efficient in reporting their level of functional remission. Higher patient awareness could be an interesting candidate to explain this. However, as the study was cross-sectional, such a proposal should be tested with a more specifically designed protocol, such as a long-term cohort.

Abstract:PRIMARY OBJECTIVE:This study aims to validate the Chinese version of the Coma Recovery Scale-Revised (CRS-R). METHODS:One hundred sixty-nine patients were assessed with both the CRS-R and the Glasgow Coma Scale (GCS), diagnosed as being in unresponsive wakefulness syndrome (UWS, formerly known as vegetative state), minimally conscious state (MCS), or emergence from MCS (EMCS). A subgroup of 50 patients has been assessed twice by the same rater, within 24 h. Patient outcome was documented six months after assessment. RESULTS:The internal consistency for the CRS-R total score was excellent (Cronbach's α = 0.84). Good test-retest reliability was obtained for CRS-R total score and subscale scores (intra-class correlation coefficient [ICC] = 0.87 and ICC = 0.66-0.84, respectively). Inter-rater reliability was high (ICC = 0.719; p < 0.01). Concurrent validity was good between CRS-R total scale and GCS total scale. Diagnostic validity was excellent compared with GCS (emerged from UWS: 24%; emerged from MCS: 28%). When considering patient outcome, diagnostic validity was good. In addition, false-positive rates have been detected for both diagnoses. CONCLUSION:The Chinese version of the CRS-R is a reliable and sensitive tool and can discriminate patients in UWS, MCS, and EMCS successfully.

Abstract:In the present study, we provide evidence that IgG2a monoclonal antibody (mAb) OKT3 is able to induce reactive oxygen intermediate (ROI) formation in polymorphonuclear leukocytes (PMN) when FcγRIIIB as well as FcγRII are bound concomitantly. Inhibition of binding to either FcγR by specific mAb (3G8 or IV. 3, respectively) resulted in complete abrogation of the OKT3‐induced respiratory burst. The effect of OKT3 was independent from its specificity and thus also from its T cell‐activating property, since nonbinding IgG2a isotype controls induced similar amounts of ROI. The IgG2b mAb BMA031 as well as the respective nonbinding isotype control were only minimally effective. With regard to the potential role of PMN activation in inflammation and tissue damage, our findings offer an extended explanation for the generation of initial adverse reactions to OKT3. Thus, one might speculate that the concerted action of cytokines liberated after its administration, what may lead to margination of leukocytes, and activation of PMN via FcγR might produce first‐dose reactions to OKT3 by directing radical‐mediated damage against the endothelium.
The Reliability and Validity of a Newly Developed Self-Rating Functional Scale Compared with Personal and Social Performance Scale and World Health Organization Disability Assessment Schedule II

Abstract:A new self-rating functional scale (SRFS) was developed to assess the functionality of patients with schizophrenia, referred to the structure of Personal and Social Performance Scale (PSP). The main aim of this study was to evaluate the reliability and validity of the SRFS. A total of 210 patients with schizophrenia were recruited from outpatient clinics in six sites in China. Demographic information, SRFS, PSP, World Health Organization Disability Assessment Schedule II, and Positive and Negative Syndrome Scale were assessed. Spearman’s correlation coefficient and path analysis were used to assess the reliability and validity of the SRFS. Cronbach’s &agr; coefficient was used to assess the internal consistency reliability. Cronbach’s &agr; was 0.83, and ranged from 0.80 to 0.82 stably, indicating that the scale was reliable in internal consistency. Spearman’s correlation coefficient between split two parts was 0.68 (P<0.01), suggesting a moderate to strong split-half reliability. The standardized regression coefficients ranged from 0.48 to 0.79, indicating moderate construct validity. Spearman’s correlation coefficients between SRFS and PSP, World Health Organization Disability Assessment Schedule II, and Positive and Negative Syndrome Scale are 0.59, 0.65, and 0.47 (all P<0.01), respectively, indicating moderate criteria validities. The SRFS is a reliable and valid instrument for assessing personal and social functionality in patients with schizophrenia.
Conceptual review of measuring functional impairment: findings from the Weiss Functional Impairment Rating Scale

Abstract:Objective: This is a narrative review of validation and outcome studies using the Weiss Functional Impairment Rating Scale (WFIRS). The objective of the review is to establish a framework for understanding functional impairment and create a definition for functional response and remission. Methods: We conducted a literature search via MEDLINE, EBSCO and Google Scholar with no date restrictions and reviewed bibliographies of selected publications. Publications found in languages other than English were translated and clarification obtained from the author(s) if needed. Inclusion criteria were any manuscript that was either a WFIRS psychometric validation study or a clinical trial using the WFIRS as an outcome. There were no exclusion criteria. Results: The WFIRS has been validated in multiple cultures, and in clinical, research and control populations. The WFIRS has robust psychometric properties across ages, psychiatric status and informants. Outcome studies show variable improvement, with different response patterns between domains and among different interventions. Conclusion: Symptom improvement and remission needs to be complemented with evaluation of functional improvement and remission to obtain a full picture of clinical status over the course of treatment.
Clinical deep remission and related factors in a large cohort of patients with rheumatoid arthritis.

Abstract:Background: Clinical remission is the treatment target in rheumatoid arthritis (RA). This study aimed to investigate clinical remission and related factors in a large cohort of patients with RA. Methods: This study composed of 342 patients with RA. Data were collected by face-to-face interview of 1049 patients with RA who visited the Department of Rheumatology of three teaching hospitals from September 2015 to May 2016. The patients with RA were clinically assessed by rheumatologists and a four-page questionnaire was completed on site. Subsequently, patients fulfilled remission criteria were further analyzed. The practicability of different definitions of remission of RA was rated by a panel of rheumatologists. Sustained intensive disease modifying anti-rheumatic drug (DMARD) treatment was defined as a combination treatment with two or more DMARDs for at least 6 months. Results: In this cohort of 342 patients with RA, the proportions of patients achieving remission were 38.0%, 29.5%, 24.9%, 21.1%, 19.0%, 18.1%, and 17.0%, based on criteria of disease activity score in 28 joints (DAS28) using CRP (DAS28-CRP), DAS28 using ESR (DAS28-ESR), routine assessment of patient index data 3 (RAPID-3), Boolean, simplified disease activity index (SDAI), clinical disease activity index, and the newly described clinical deep remission (CliDR), respectively. Boolean and CliDR are the best in practicability scored by rheumatologists (7.5 and 8.0, respectively). Compared with the non-sustained intensive group, sustained intensive treatment with DMARDs yielded higher remission rates of 25.6%, 23.8%, and 21.3% in patients with RA based on Boolean (chi(2) = 3.937, P = 0.047), SDAI (chi(2) = 4.666, P = 0.031), and CliDR criteria (chi(2) = 4.297, P = 0.038). The most commonly prescribed conventional synthesized DMARDs (csDMARDs) in patients with RA was leflunomide, followed by methotrexate, and hydroxychloroquine. Compared with the non-remission group, patients achieving remission had a longer median duration of DMARDs (45.0 [22.8-72.3] months, Z = -2.295, P = 0.022). Conclusions: The findings in this study indicated that clinical deep remission is achievable in patients with RA. Sustained intensive DMARD treatment is needed to achieve a better outcome in RA.
Which Subgroup of First-Episode Schizophrenia Patients Can Remit During the First Year of Antipsychotic Treatment?

Abstract:OBJECTIVE:To investigate the persistent remission rate (PRR) and its predictors within the first year of antipsychotic treatment in first-episode schizophrenia (FES) patients.METHODS:In a sample of 301 FES patients who remained in antipsychotic treatment for 1 year, we assessed symptoms with the Positive and Negative Syndrome Scale (PANSS), cognition in six domains and functioning with the Personal and Social Performance Scale (PSP).RESULTS:In total, 75.4% (227/301) of FES patients remaining in antipsychotic treatment reached persistent remission (PR) in one year. The PSP score was higher in remitters than non-remitters at the endpoint of the 1-year follow-up (P <0.0001). The PANSS negative score-but not the PANSS total score, positive score or general psychopathological score; PSP score; or cognitive performance at baseline-was negatively associated with PR. Lower scores for "abstract thinking" and "stereotyped thinking" were independent predictors of PR.CONCLUSIONS:In FES, nearly 3/4 patients could achieve PR with 1 year of antipsychotic treatment, and having fewer negative symptoms, especially thinking and volition symptoms, can predict PR.CLINICAL TRIAL REGISTRATION:www.ClinicalTrials.gov, identifier NCT01057849.
Effects of cognitive remediation therapy on improving clinical symptoms and social functions in patients with chronic schizophrenia

Abstract:AIM: To explore the efficiency of cognitive remediation therapy (CRT) on improving the clinical symptoms and social functions in patients with chronic schizophrenia. METHODS: A total of 104 chronic schizophrenic inpatients with stable clinical state were recruited from Beijing Huilongguan Hospital from January to August in 2003. They, aged 25-55 years, all accorded with the diagnosis standard of schizophrenic by CCMD-Ⅲ and DSM-Ⅳ. Their course of disease was more than 2 years at protraction, residue or partial remission. The curative effect of drug treatment was stable and unvaried, informed consents were obtained from the subjects or their relatives. All the patients were divided into CRT group and control group at random (n =52), and were pretreated with antipsychotic agent therapy. CRT manual (Chinese version) derived from Neurocognitive Remediation Manual 2000 designed by Ann Delahunty and Rodney Morice et al, which included three models of cognitive flexibility, working memory and planning execution. In control group, the subjects received occupation recreational therapy of operatic music therapy and dancing therapy instructed by therapists. Pre-and post-treatment, positive and negative syndrome scale (PANSS), social functional disability scale for psychiatric inpatients, and nurse observation scale for inpatient evaluation (NOSIE) were administered to all subjects. RESULTS: Totally 104 chronic schizophrenia patients were involved in the study, and 44 ones in CRT group and 46 one in control group entered the result analysis, except 14 lost.①A significant decrease occurred in PANSS total score, negative symptom score, compound scale score, general psychopathology score and reaction retardation score in both groups (t =2.12-4.59, P < 0.05). But there was no significant difference in these decreases between two groups (P > 0.05).②The total score of social functional disability scale for psychiatric inpatients was significantly declined in both groups after treatment (t =3.89, 2.04, P < 0.05), and no significant difference appeared between two groups (P > 0.05). NOSIE general illness score, general negative score, and retardation score all decreased significantly after treatment in CRT group (t =1.49, 1.19, 2.81, P < 0.05), with retardation score decreased significantly between two groups (F=4.97, P < 0.05).③There was a positive correlation between social functional improvement and verbal fluency improvement (R 2 =0.36, P < 0.05). Another positive correlation occurred between two NOSIE subscales (general illness and positive score) and verbal fluency (R 2=0.37, 0.34, P < 0.05). CONCLUSION: CRT can improve the social function of patients with schizophrenia moderately, and is correlated with some cognitive function enhancements. There is no obvious prompting effect of CRT on clinical symptoms.
Comparison of Clinical Remission Criteria of Rheumatoid Arthritis and Discussion about the Practicability in Clinical Practice

Abstract:Objective To get the message of clinical remission rate of rheumatoid arthritis(RA) patients in China,and to evaluate the advantage and practicability of remission criteria.Methods The questionnaires of patients with RA,who were receiving usual care at the two hospitals,were used for the study.We got clinical symptom and sign,laboratory data by the questionnaire.The database was analyzed according to nine criteria of remission.The remission rate of RA patients and the association,difference of the nine criteria for clinical pratice were evaluate and compared.Results Among 310 cases,240 effective questionnaires were gotten.The remission rate ranged from 15.4% to 47.2%,the lowest was ACR criterion(15.4%) and the highest was DAS28-CRP criterion(47.2 %),mACR was 22.1 %,Clin42 was 22.6 %,CDAI was 23.8%,Clin28 was 26%,SDAI was 26.4 %,DAS28 was 32.1 %,RAPID3 was 32.4 %.The differences among the nine criterias were significant(P = 0.00).Conclusion ACR criteria is the most strict,which can be considered as real remission.DAS28 criteria is the liberal criterion,which partly still has residual disease activity.RAPID3 criteria is feasible and maybe a useful remission criterion for busy clinical practice.
Interview-based assessment of cognition in schizophrenia: applicability of the Schizophrenia Cognition Rating Scale (SCoRS) in different phases of illness and settings of care

Abstract:Introduction: The Schizophrenia Cognition Rating Scale (SCoRS), an interview-based assessment of cognition, has proved to be a valid measure of cognitive performance in patients with schizophrenia. Objective: The aims of this study were to analyze the validity of this scale in a naturalistic setting representative of the Italian system of psychiatric care, and to test whether the SCoRS could be appropriately used in different phases of illness and contexts of care. Methods: Eighty-six patients with schizophrenia (DSM-IV-TR criteria) (N = 59 clinically stabilized patients; N = 27 recently hospitalized patients) were administered the SCoRS. The reliability of SCoRS was assessed and global ratings were correlated with neurocognitive, clinical, and psychosocial functioning measures. Results: SCoRS inter-rater and test-retest reliability were high. In clinically stabilized patients, SCoRS global ratings were significantly correlated with composite scores of cognitive performance (global cognitive index: r = -0.570, P<0.001), symptoms (Positive and Negative Syndrome Scale (PANSS) total score: r = 0.602, P < 0.001), and psychosocial functioning (Global Assessment of Functioning (GAF): r = -0.532, P<0.001; Health of the Nation Outcome Scale (HoNOS): r = 0.433, P < 0.001). On the other hand, no such correlations were found in recently hospitalized patients. Correlations with neuropsychological and functional measures were less significant as the severity of the patients' symptoms, especially positive symptoms, increased. Conclusion: The SCoRS is a valid measure of cognitive performance and is related to psychosocial functioning, especially in clinically stable patients with schizophrenia. The usefulness of the SCoRS in patients recently admitted to hospital for an acute phase of illness is uncertain.

Abstract:PURPOSE:The study examined the rate of remission in individuals experiencing a first episode of schizophrenia (FES) in China and explored predictors of remission in the acute phase of the illness.DESIGN AND METHODS:Fifty-five FES patients were randomly treated with risperidone, olanzapine, or aripiprazole at therapeutic doses for 8 weeks, and their clinical profiles and cognition were assessed using standardized assessment instruments at entry and the end of the study.FINDINGS:Of the 55 patients, 30 (54.5%) remitted by the end of the 8-week study. In univariate analyses, shorter duration of untreated psychosis, higher scores on both the time-based prospective memory (TBPM) and event-based prospective memory tasks and the Hopkins Verbal Learning Test-revised, and less severe negative symptoms were significantly associated with remission. In stepwise multiple logistic regression analyses, only higher scores on the TBPM significantly predicted remission. Individuals having higher scores reflecting better TBPM at baseline were more likely to achieve remission after 8 weeks of optimized antipsychotic treatment.PRACTICE IMPLICATIONS:TPBM may be useful in helping clinicians identify those FES patients most likely to achieve a favorable treatment response.

Abstract:Introduction: Expert consensus operationalized treatment response and remission in obsessive-compulsive disorder (OCD) as a Yale-Brown Obsessive-Compulsive Scale (Y-BOCS) reduction ≥35% and score ≤12 with ≤2 on Clinical Global Impressions Improvement (CGI-I) and Severity (CGI-S) scales, respectively. However, there has been scant empirical evidence supporting these definitions. Methods: We conducted a systematic review and an individual participant data meta-analysis of randomized-controlled trials (RCTs) in adults with OCD to determine optimal Y-BOCS thresholds for response and remission. We estimated pooled sensitivity/specificity for each percent reduction threshold (response) or posttreatment score (remission) to determine response and remission defined by a CGI-I and CGI-S ≤ 2, respectively. Results: Individual participant data from 25 of 94 eligible RCTs (1235 participants) were included. The optimal threshold for response was ≥30% Y-BOCS reduction and for remission was ≤15 posttreatment Y-BOCS. However, differences in sensitivity and specificity between the optimal and nearby thresholds for response and remission were small with some uncertainty demonstrated by the confidence ellipses. Conclusion: While the empirically derived Y-BOCS thresholds in our meta-analysis differ from expert consensus, given the predominance of data from more recent trials of OCD, which involved more refractory participants and novel treatment modalities as opposed to first-line therapies, we recommend the continued use of the consensus definitions.
Replication of a neuroimaging biomarker for striatal dysfunction in psychosis

Abstract:To bring biomarkers closer to clinical application, they should be generalizable, reliable, and maintain performance within the constraints of routine clinical conditions. The functional striatal abnormalities (FSA), is among the most advanced neuroimaging biomarkers in schizophrenia, trained to discriminate diagnosis, with post-hoc analyses indicating prognostic properties. Here, we attempt to replicate its diagnostic capabilities measured by the area under the curve (AUC) in receiver operator characteristic curves discriminating individuals with psychosis ( n = 101) from healthy controls ( n = 51) in the Human Connectome Project for Early Psychosis. We also measured the test-retest (run 1 vs 2) and phase encoding direction (i.e., AP vs PA) reliability with intraclass correlation coefficients (ICC). Additionally, we measured effects of scan length on classification accuracy (i.e., AUCs) and reliability (i.e., ICCs). Finally, we tested the prognostic capability of the FSA by the correlation between baseline scores and symptom improvement over 12 weeks of antipsychotic treatment in a separate cohort ( n = 97). Similar analyses were conducted for the Yeo networks intrinsic connectivity as a reference. The FSA had good/excellent diagnostic discrimination (AUC = 75.4%, 95% CI = 67.0–83.3%; in non-affective psychosis AUC = 80.5%, 95% CI = 72.1–88.0%, and in affective psychosis AUC = 58.7%, 95% CI = 44.2–72.0%). Test-retest reliability ranged between ICC = 0.48 (95% CI = 0.35–0.59) and ICC = 0.22 (95% CI = 0.06–0.36), which was comparable to that of networks intrinsic connectivity. Phase encoding direction reliability for the FSA was ICC = 0.51 (95% CI = 0.42–0.59), generally lower than for networks intrinsic connectivity. By increasing scan length from 2 to 10 min, diagnostic classification of the FSA increased from AUC = 71.7% (95% CI = 63.1–80.3%) to 75.4% (95% CI = 67.0–83.3%) and phase encoding direction reliability from ICC = 0.29 (95% CI = 0.14–0.43) to ICC = 0.51 (95% CI = 0.42–0.59). FSA scores did not correlate with symptom improvement. These results reassure that the FSA is a generalizable diagnostic – but not prognostic – biomarker. Given the replicable results of the FSA as a diagnostic biomarker trained on case-control datasets, next the development of prognostic biomarkers should be on treatment-response data.

Abstract:Introduction Drivers for remission, relapse and violence-related behaviour among patients with schizophrenia are the most complicated issue. Methods and analysis This study aims to recruit a longitudinal cohort of patients with schizophrenia. Two suburban districts and two urban districts were randomly selected according to health service facilities, population, geographical region and socioeconomic status. Individuals (>18 years old) who received a diagnosis of schizophrenia following the International Classification of Diseases (10th edition) criteria within the past 3 years will be invited as participants. Assessments will be carried out in local community health centres. Data will be used to (1) establish a community-based schizophrenia cohort and biobank, (2) prospectively determine the course of multidimensional functional outcomes of patients with schizophrenia who are receiving community-based mental health treatment, and (3) map the trajectories of patients with schizophrenia and prospectively determine the course of multidimensional outcomes based on the differential impact of potentially modifiable moderators. Ethics and dissemination The study has been reviewed and approved by the Human Research Ethics Committee of Shanghai Mental Health Center (2021-67). Results of the study will be disseminated through peer-reviewed journals. If effective, related educational materials will be released to the public.

Abstract:events as they are usually present in chronically sun-exposed skin. It has been suggested that immunohistochemical p53 expression is helpful in the diagnosis of d-VIN and the evaluation with p53 may identify those LS cases with a likelihood of evolving into SCC. Therefore, it could be proposed that a decrease of p53 by pimecrolimus treatment is beneficial in moderating the risk of developing SCC. Nonetheless, p53 expression may indicate stress responses, e.g. ischaemic cell injury but also inflammation, as previously described in the uterine cervix. p53 staining in LS could therefore be interpreted as a stress-related phenomenon rather than an indicator for preinvasive neoplasia. Besides decreased p53 staining, pimecrolimus treatment increased the number and staining intensity of Bcl-2-positive basal keratinocytes. The expression of anti-apoptotic Bcl-2 protein decreases in carcinogenesis of some carcinomas coinciding with frequent p53 overexpression. The decline of Bcl-2-positive cells is considered to promote tumorigenesis, but overexpression of Bcl-2 has been observed in premalignant conditions. Whether the decrease of p53 and increase of Bcl2 expression provide protection from malignant progression warrants long-term follow-up. Subsequently, careful periodic gynaecological check-ups with adjunctive biopsies are needed.

Abstract:Introduction In mental health prevention, person-centered, and rights-based approaches, the role of recovery is highlighted (WHO, 2021). Various evaluation tools are used in rehabilitation objectives and programs, including the Specific Levels of Functioning Scale (SLOF) (Mucci et al. Schizophr Res 2014;159 144-50) and the Recovery Assessment Scale – Domains and Stages (RAS-DS), a self-measure of mental health recovery. It includes 38 items clustered into four recovery domains and meets two functions. In addition to measuring self-reported outcomes, it increases service-user control towards objectives and recovery action plans (Honey et al. BMC Psychiatry 2023;23 500). Objectives To evalue the efficacy of RAD-DS in a psychiatric rehabilitation facility to be used as a routine tool in daily rehabilitation activity. Methods In our observational study, we recruited 103 inpatients (total: 103 patients, females: 38 patients, males: 65 patients) in a psychiatric rehabilitation facility. The patient presented with psychiatric disorders that met the diagnostic criteria of DMS-5 (schizophrenia, bipolar disorder, MDD, personality borderline disorder). Epidemiological data are shown in Table 1. All patients were undergoing a psychiatric rehabilitation program and were observed during a one-year evaluation. In all patients, the following rating scales were administered at baseline (T0) and after a year (T1): For the evaluation of social measures, life outcomes, and functioning and recovery: - Recovery Assessment Scale – Domains and Stages RAS-DS - Specific Levels of Functioning Scale (SLOF) - Global Assessment of Functioning (GAF) For psychopathological evaluation: - Brief Psychiatric Rating Scale (BPRS) The data were statistically analyzed with the EZAnalyze 3.0 software for the Excel platform. Results The RAS-DS total score results (Table 2) show a not significant difference between T0 vs. T1 (mean: 101.80 vs. 104.37, p. 0.193). An improvement in the score was observed after one year of rehabilitation treatment in the subgroup "Doing things I value" (T0 vs. T1: mean 16.15 vs. 18.77, p 0.001). Statistically significant differences were observed in the subgroups "Mastering my illness" (T0 vs. T1: mean 18.3 vs. 20.85, p. 0.021). In the other subgroups, the differences were not statistically significant. Interestingly, these results are comparable to those found with SLOF and GAF (respectively, p. 0.972 and p. 0.873). Image: Image 2: Conclusions The current trend of research and clinical practice is to give more importance to psychiatric rehabilitation treatment (Franza Psychiatr Danub 2022;34(Suppl 8) 9-13). The results obtained with our observational study indicate the possible usefulness of indicators of patient well-being, as well as the RAS-DS in the management of psychiatric rehabilitation programs. Criterion-related validity in a sample of migraine outpatients: the diagnostic criteria for psychosomatic research

Abstract:Objective: The Diagnostic Criteria for Psychosomatic Research (DCPR) are those of psychosomatic syndromes that did not find room in the classical taxonomy. More recently, the DCPR were updated, called DCPR-revised (DCPR-R). The present study was conducted to test the criterion-related validity of the DCPR-R. Methods: Two hundred consecutive subjects were enrolled at the Headache Center of Careggi University Hospital (Italy): 100 subjects had a diagnosis of chronic migraine (CM) and 100 had a diagnosis of episodic migraine (EM). Participants received a clinical assessment, which included the DCPR-revised Semi-Structured Interview (DCPR-R SSI), the Structured Clinical Interview for DSM-5 (SCID-5), and the psychosocial index (PSI). Results: Forty-seven subjects (23.5%) had at least one DSM-5 diagnosis: major depressive disorder (8.5%; n = 17) and agoraphobia (7.5%; n = 15) were the most frequent. One hundred and ten subjects (55%) reported a DCPR-R diagnosis: allostatic overload (29%; n = 58) and type A behavior (10.5%; n = 21) were the most frequent. When the incremental validity of the DCPR system over the DSM system was tested using PSI subscales as the criterion variable, the DCPR-R increased up to 0.11-0.24 the amount of explained variance. Subjects with at least one DCPR-R diagnosis showed lower PSI well-being scores (p = .001), higher PSI stress scores (p < .001), and higher PSI psychological distress scores (p = .008) than subjects without a DCPR-R diagnosis. Conclusion: The DCPR-R showed a good criterion-related validity in migraine outpatients. Thus, they might be implemented, together with the DSM-5, in the assessment of migraine subjects.
