Abstract:Objective To develop a computer-based model for planning and performing cleft lip repair.Methods A 3-month-old unilateral cleft lip,alveolus and plate patient was previously CT scanned.High-resolution 3D meshes of cleft lip were generated by VTK technology.The visualization model was integrated with mass spring model by Visual C++.Results The model of cleft lip was instilled with physical properties of soft tissue and the calculation was accelerated by simplifying the process.Conclusion This model combined with mass-spring model,boasting of both mechanical and geometric similarity,could provide a new method for planning and performing cleft lip repair.

Abstract:This paper presents a novel inhomogeneous volumetric mesh deformation approach by gradient field manipulation, and uses it for maxillofacial surgery simulation. The study is inspired by the state-of-the-art surface deformation techniques based on differential representations. Working in the volumetric domain instead of on only the surface can preserve the volumetric details much better, avoid local self-intersections, and achieve better deformation propagation because of the internal mesh connections. By integrating the mesh cell material stiffness parameter into our new discrete volumetric Laplacian operator, it is very convenient to incorporate inhomogeneous materials into the deformation framework. In addition, the system matrix for solving the volumetric harmonic field to handle the local transformation problem is the same used for Poisson reconstruction equation, thus it requires solving essentially only one global linear system. The system is easy to use, and can accept explicit rotational constraints, or only translational constraints to drive the deformation. One typical maxillofacial surgery case was simulated by the new methodology with inhomogeneous material estimated directly from CT data, and compared to the commonly used finite element method (FEM) approach. The results demonstrated that the deformation methodology achieved good accuracy, as well as interactive performance. Therefore, the usage of our volumetric mesh deformation approach is relevant and suitable for daily clinical practice.

Abstract:In the framework of the Lindblad theory for open quantum systems, we determine the degree of quantum decoherence of a harmonic oscillator interacting with a thermal bath. It is found that the system manifests a quantum decoherence which is more and more significant in time. We also calculate the decoherence time and show that it has the same scale as the time after which thermal fluctuations become comparable with quantum fluctuations.

Abstract:We present here calculations, based on the embedded atom method, of phonon energies and polarizations on the clean Cu (100) surface. The atomic relaxations and the dynamical matrix are obtained, without fitting to surface properties, in one consistent calculation which is no more computationally demanding than pair potentials. Excellent agreement with experiment is obtained. We find that the force constants are qualitatively different than those obtained by fitting two-body central-potential models, reflecting the many-body nature of the calculation and suggesting that fits based on central-potential models may be misleading.

Abstract:Biological swimmers often exhibit rich physics due to the hydrodynamic coupling between the fluid flow and the immersed deforming body. Complexity and nonlinearity of their behaviors have imposed significant challenges in design and analysis of robots that mimick biological swimming motions. Inspired by the recent experimental studies of soft robotic swimmers, we develop a fictitious domain/active strain method to numerically study the swimming motion of thin, light-weight robots composed of smart materials that can actively undergo reversible large deformations (e.g., liquid crystal elastomer). We assume the elastic material to be neo-Hookean, and behave like an artificial “muscle” which, when stimulated, generates a principal stretch of contraction. We adopt an active strain approach to impose contractive strains to drive elastic deformation following a multiplicative decomposition of the deformation gradient tensor. The hydrodynamic coupling between the fluid and the solid is then resolved by using the fictitious domain method where the induced flow field is virtually extended into the solid domain. Pseudo body forces are employed to enforce the interior fictitious fluid motion to be the same as the solid structure dynamics. Using the fictitious domain/active strain method, we perform a series of numerical explorations for soft robotic swimmers with both 2D and 3D geometries. We demonstrate that these robot prototypes can effectively perform undulatory swimming and jet-propulsion when active strains are appropriately distributed on the elastica.

Abstract:This paper addresses an important issue raised for the clinical relevance of Computer-Assisted Surgical applications, namely the methodology used to automatically build patient-specific Finite Element (FE) models of anatomical structures. From this perspective, a method is proposed, based on a technique called the Mesh-Matching method, followed by a process that corrects mesh irregularities. The Mesh-Matching algorithm generates patient-specific volume meshes from an existing generic model. The mesh regularization process is based on the Jacobian matrix transform related to the FE reference element and the current element. This method for generating patient-specific FE models is first applied to Computer-Assisted maxillofacial surgery, and more precisely to the FE elastic modelling of patient facial soft tissues. For each patient, the planned bone osteotomies (mandible, maxilla, chin) are used as boundary conditions to deform the FE face model, in order to predict the aesthetic outcome of the surgery. Seven FE patient-specific models were successfully generated by our method. For one patient, the prediction of the FE model is qualitatively compared with the patient's post-operative appearance, measured from a Computer Tomography scan. Then, our methodology is applied to Computer-Assisted orbital surgery. It is, therefore, evaluated for the generation of eleven patient-specific FE poroelastic models of the orbital soft tissues. These models are used to predict the consequences of the surgical decompression of the orbit. More precisely, an average law is extrapolated from the simulations carried out for each patient model. This law links the size of the osteotomy (i.e. the surgical gesture) and the backward displacement of the eyeball (the consequence of the surgical gesture).

Derivation and simulation of a two-phase fluid deformable surface model

Abstract:We consider two-phase fluid deformable surfaces as model systems for biomembranes. Such surfaces are modeled by incompressible surface Navier-Stokes-Cahn-Hilliard-like equations with bending forces. We derive this model using the Lagrange-D'Alembert principle considering various dissipation mechanisms. The highly nonlinear model is solved numerically to explore the tight interplay between surface evolution, surface phase composition, surface curvature and surface hydrodynamics. It is demonstrated that hydrodynamics can enhance bulging and furrow formation, which both can further develop to pinch-offs. The numerical approach builds on a Taylor-Hood element for the surface Navier-Stokes part, a semi-implicit approach for the Cahn-Hilliard part, higher order surface parametrizations, appropriate approximations of the geometric quantities, and mesh redistribution. We demonstrate convergence properties that are known to be optimal for simplified sub-problems.

Fast porous visco-hyperelastic soft tissue model for surgery simulation: application to liver surgery

Abstract:Understanding and modeling liver biomechanics represents a significant challenge due to its complex nature. In this paper, we tackle this issue in the context of real-time surgery simulation where a compromise between biomechanical accuracy and computational efficiency must be found. We describe a realistic liver model including hyperelasticity, porosity and viscosity that is implemented within an implicit time integration scheme. To optimize its computation, we introduce the Multiplicative Jacobian Energy Decomposition (MJED) method for discretizing hyperelastic materials on linear tetrahedral meshes which leads to faster matrix assembly than the standard Finite Element Method. Visco-hyperelasticity is modeled by Prony series while the mechanical effect of liver perfusion is represented with a linear Darcy law. Dynamic mechanical analysis has been performed on 60 porcine liver samples in order to identify some viscoelastic parameters. Finally, we show that liver deformation can be simulated in real-time on a coarse mesh and study the relative effects of the hyperelastic, viscous and porous components on the liver biomechanics.

Abstract:Angioplasty with stent placement is a widely used treatment strategy for patients with stenotic blood vessels. However, it is often challenging to predict the outcomes of this procedure for individual patients. Image-based computational fluid dynamics (CFD) is a powerful technique for making these predictions. To perform CFD analysis of a stented vessel, a virtual model of the vessel must first be created. This model is typically made by manipulating two-dimensional contours of the vessel in its pre-stent state to reflect its post-stent shape. However, improper contour-editing can cause invalid geometric artifacts in the resulting mesh that then distort the subsequent CFD predictions. To address this limitation, we have developed a novel shape-editing method that deforms surface meshes of stenosed vessels to create stented models.

Volume Preserving Simulation of Soft Tissue with Skin

Abstract:Simulation of human soft tissues in contact with their environment is essential in many fields, including visual effects and apparel design. Biological tissues are nearly incompressible. However, standard methods employ compressible elasticity models and achieve incompressibility indirectly by setting Poisson's ratio to be close to 0.5. This approach can produce results that are plausible qualitatively but inaccurate quantatively. This approach also causes numerical instabilities and locking in coarse discretizations or otherwise poses a prohibitive restriction on the size of the time step. We propose a novel approach to alleviate these issues by replacing indirect volume preservation using Poisson's ratios with direct enforcement of zonal volume constraints, while controlling fine-scale volumetric deformation through a cell-wise compression penalty. To increase realism, we propose an epidermis model to mimic the dramatically higher surface stiffness on real skinned bodies. We demonstrate that our method produces stable realistic deformations with precise volume preservation but without locking artifacts. Due to the volume preservation not being tied to mesh discretization, our method also allows a resolution consistent simulation of incompressible materials. Our method improves the stability of the standard neo-Hookean model and the general compression recovery in the Stable neo-Hookean model.

Abstract:This paper describes work in progress in the area of planning and prediction of maxillofacial surgery. We focus on true 3D problems which cannot be addressed by conventional 2D cephalometric radiographs. Examples are bilateral repositioning of the zygoma’s or treating facial asymmetries. The environment being developed arose from the need of patients for prediction of their expected post-op outlook and from the need of surgeons for better support to reach the desired result. Our simulation environment adheres to a scene-based approach in which CT image-derived visualizations and additional 3D photographs (showing the face surface and its natural complexion) are co-presented and manipulated. Tools for non-planar osteotomy are included. Repositioning of bone fragments is made possible. Furthermore, a voxel displacement formalism is presented to account for soft tissue implications. Combining these simulation tools, a qualitative prediction of the facial outlook is obtained. Results are shown for a case of hypoplastic zygoma repositioning.

Abstract:Abstract This paper presents an intuitive rhinoplasty planning and simulation system, to provide high quality prediction of postoperative appearance, and design patient specific nose prosthesis automatically. The key component is a novel volumetric Laplacian deformation tool inspired by the state‐of‐the‐art differential surface deformation techniques. Working on the volumetric domain and incorporating inhomogeneous material from CT data make the new approach suitable for soft tissue simulation. In particular, the system employs a special sketch contour driving deformation interface, which can provide realistic 3D rhinoplasty simulation with intuitive and straightforward 2D manipulation. When satisfied with the appearance, the change of soft tissue before and after simulation is utilized to generate the individual prosthesis model automatically. Clinical validation using post‐operative CT data demonstrated that the system can provide prediction results of high quality. And the surgeons who used the system confirmed that this planning system is attractive and has potential for daily clinical practice. Copyright © 2010 John Wiley & Sons, Ltd.

Abstract:Errors in biomechanics simulations arise from modeling and discretization. Modeling errors are due to the choice of the mathematical model whilst discretization errors measure the impact of the choice of the numerical method on the accuracy of the approximated solution to this specific mathematical model. A major source of discretization errors is mesh generation from medical images, that remains one of the major bottlenecks in the development of reliable, accurate, automatic and efficient personalized, clinically-relevant Finite Element (FE) models in biomechanics. The impact of mesh quality and density on the accuracy of the FE solution can be quantified with \emph{a posteriori} error estimates. Yet, to our knowledge, the relevance of such error estimates for practical biomechanics problems has seldom been addressed, see [25]. In this contribution, we propose an implementation of some a posteriori error estimates to quantify the discretization errors and to optimize the mesh. More precisely, we focus on error estimation for a user-defined quantity of interest with the Dual Weighted Residual (DWR) technique. We test its applicability and relevance in two situations, corresponding to computations for a tongue and an artery, using a simplified setting, i.e., plane linearized elasticity with contractility of the soft-tissue modeled as a pre-stress. Our results demonstrate the feasibility of such methodology to estimate the actual solution errors and to reduce them economically through mesh refinement.

Biomechanics and Modeling in Mechanobiology

Abstract:This work explores the use of an embedded computational fluid dynamics method to study the type B aortic dissection. The use of the proposed technique makes it possible to easily test different intimal flap configurations without any need of remeshing. To validate the presented methodology, we take as reference test case an in vitro experiment present in the literature. This experiment, which considers several intimal flap tear configurations (number, size and location), mimics the blood flow in a real type B aortic dissection. We prove the correctness and suitability of the presented approach by comparing the pressure values and waveform. The obtained results exhibit a remarkable similarity with the experimental reference data. Complementary, we present a feasible surgical application of the presented computer method. The aim is to help the clinicians in the decision making before the type B aortic dissection surgical fenestration. The capabilities of the proposed technique are exploited to efficiently create artificial reentry tear configurations. We highlight that only the radius and center of the reentry tear need to be specified by the clinicians, without any need to modify neither the model geometry nor the mesh. The obtained computational surgical fenestration results are in line with the medical observations in similar clinical studies.

Through analysis of biomechanics property of soft tissue, this paper applies the visco-elastic model in the study of soft tissue deformation in surgery simulation. Combining deformation accuracy with computer efficiency, the algorithm achieves more realistic soft tissue deformation based on visco-elastic model.
Real-to-Sim Deformable Object Manipulation: Optimizing Physics Models with Residual Mappings for Robotic Surgery

Abstract:Accurate deformable object manipulation (DOM) is essential for achieving autonomy in robotic surgery, where soft tissues are being displaced, stretched, and dissected. Many DOM methods can be powered by simulation, which ensures realistic deformation by adhering to the governing physical constraints and allowing for model prediction and control. However, real soft objects in robotic surgery, such as membranes and soft tissues, have complex, anisotropic physical parameters that a simulation with simple initialization from cameras may not fully capture. To use the simulation techniques in real surgical tasks, the "real-to-sim" gap needs to be properly compensated. In this work, we propose an online, adaptive parameter tuning approach for simulation optimization that (1) bridges the real-to-sim gap between a physics simulation and observations obtained 3D perceptions through estimating a residual mapping and (2) optimizes its stiffness parameters online. Our method ensures a small residual gap between the simulation and observation and improves the simulation's predictive capabilities. The effectiveness of the proposed mechanism is evaluated in the manipulation of both a thin-shell and volumetric tissue, representative of most tissue scenarios. This work contributes to the advancement of simulation-based deformable tissue manipulation and holds potential for improving surgical autonomy.

Abstract:We implement full, three-dimensional constrained mixture theory for vascular growth and remodeling into a finite element fluid-structure interaction (FSI) solver. The resulting "fluid-solid-growth" (FSG) solver allows long term, patient-specific predictions of changing hemodynamics, vessel wall morphology, tissue composition, and material properties. This extension from short term (FSI) to long term (FSG) simulations increases clinical relevance by enabling mechanobioloigcally-dependent studies of disease progression in complex domains.

Abstract:Background: Preoperative planning for liposuction is vital to ensure safe practice and patient satisfaction. However, current standards of fat assessment before surgery are guided by subjective methods such as visual inspection, skin-pinch tests, and waist circumference measurements. Objectives: This study aimed to develop an inexpensive software-based tool that utilizes ultrasound (US) imaging and an online platform to accurately simulate regional subcutaneous adipose tissue (SAT) distribution and safe volume estimation for liposuction procedures. Methods: The authors present a web-based platform with integrated 2-dimensional (2D) and 3-dimensional (3D) simulations of SAT to support liposuction planning and execution. SAT-Map was constructed using multiple sub-applications linked with the python framework programming language (Wilmington, DE). Results: The SAT-Map interface provides an intuitive and fluid means of generating patient-specific models and volumetric data. To further accommodate this, an operational manual was prepared to achieve consistent visualization and examination of estimated SAT content. The system currently supports static 2D heatmap simulation and 3D interactive virtual modelling of the SAT distribution. Supplementary clinical studies are needed to evaluate SAT-Map's clinical performance and practicality. Conclusions: SAT-Map revolutionizes the concept of preoperative planning for liposuction by developing the first combined web-based software that objectively simulates fat distribution and measures safe liposuction volume. Our software approach presents a cost-efficient, accessible, and user-friendly system offering multiple advantages over current SAT assessment modalities. The immediacy of clinically accurate 3D virtual simulation provides objective support to surgeons towards improving patient conversation, outcomes, and satisfaction in liposuction procedures.

Abstract:In this paper, a boundary element method and a finite element method with the static condensation technique are respectively applied to real-time animation of deformable objects. First, a finite element model in conjunction with the static condensation technique for eliminating internal nodes of the deformable object is introduced to simulate in real time deformation of the objects. Then, a Boundary Element Method (BEM) is given to accelerate solution to the dynamic deformation problem and to simplify data preparation. Finally, the above methods have respectively been applied to the anatomical or surgical simulation. A pituitary model in the human brain, which is reconstructed from a set of anatomical sections, is selected to be a deformable object under the action of a virtual tool such as scalpel or probe. It produces fair graphic realism and high speed performance. The results show that the BEM not only has less computational expense than the FEM, but also can be convenient to combine with the 3D reconstruction and surface modeling as it enables the reduction of the dimensionality of the problem by one and simplifies data preparation.
