Abstract:Background: Currently, the efficacy of prosthetic valves is highly dependent on the geometry of the aortic root. With the development of computational fluid dynamics, patient-specific modeling and simulation of prosthetic heart valves increasingly become the forefront of current research. Methods: In this study, we propose an approach for designing patient-specific prosthetic aortic valves using fluid-structure interaction (FSI) analysis. Firstly, the aortic root model was reconstructed from the computed tomography angiography (CTA) images. We then implemented a parametric valve design. The leaflets were determined by the attachment curve, the free edge, and the belly curve. We selected six key points, including three points located at the ends of the free edge, and three points located at the bottom of the attachment curve. This allowed for designing various leaflet geometries. Furthermore, each valve model was simulated using the FSI method. We evaluated deformation, effective orifice area (EOA), and stress to study its performance. Conclusion: The simulation results showed that patient-specific valves demonstrated a large EOA and proper stress, indicating satisfactory performance. Our study investigated an approach for designing patient-specific prosthetic aortic valves and evaluated hemodynamic parameters. This provides promising insights into the design of prosthetic aortic valves and potentially serves as a valuable tool to assist patient-specific aortic valve replacement.
Fully-coupled fluid-structure interaction simulation of the aortic and mitral valves in a realistic 3D left ventricle model

Abstract:In this study, we present a fully-coupled fluid-structure interaction (FSI) framework that combines smoothed particle hydrodynamics (SPH) and nonlinear finite element (FE) method to investigate the coupled aortic and mitral valves structural response and the bulk intraventricular hemodynamics in a realistic left ventricle (LV) model during the entire cardiac cycle. The FSI model incorporates valve structures that consider native asymmetric leaflet geometries, anisotropic hyperelastic material models and human material properties. Comparison of FSI results with subject-specific echocardiography data demonstrates that the SPH-FE approach is able to quantitatively predict the opening and closing times of the valves, the mitral leaflet opening and closing angles, and the large-scale intraventricular flow phenomena with a reasonable agreement. Moreover, comparison of FSI results with a LV model without valves reveals substantial differences in the flow field. Peak systolic velocities obtained from the FSI model and the LV model without valves are 2.56 m/s and 1.16 m/s, respectively, compared to the Doppler echo data of 2.17 m/s. The proposed SPH-FE FSI framework represents a further step towards modeling patient-specific coupled LV-valve dynamics, and has the potential to improve our understanding of cardiovascular physiology and to support professionals in clinical decision-making.

Abstract:Abstract The opening and closing dynamics of the aortic valve (AV) has a strong influence on haemodynamics in the aortic root, and both play a pivotal role in maintaining normal physiological functions of the valve. The aim of this study was to establish a subject‐specific fluid–structure interaction (FSI) workflow capable of simulating the motion of a tricuspid healthy valve and the surrounding haemodynamics under physiologically realistic conditions. A subject‐specific aortic root was reconstructed from magnetic resonance (MR) images acquired from a healthy volunteer, whilst the valve leaflets were built using a parametric model fitted to the subject‐specific aortic root geometry. The material behaviour of the leaflets was described using the isotropic hyperelastic Ogden model, and subject‐specific boundary conditions were derived from 4D‐flow MR imaging (4D‐MRI). Strongly coupled FSI simulations were performed using a finite volume‐based boundary conforming method implemented in FlowVision. Our FSI model was able to simulate the opening and closing of the AV throughout the entire cardiac cycle. Comparisons of simulation results with 4D‐MRI showed a good agreement in key haemodynamic parameters, with stroke volume differing by 7.5% and the maximum jet velocity differing by less than 1%. Detailed analysis of wall shear stress (WSS) on the leaflets revealed much higher WSS on the ventricular side than the aortic side and different spatial patterns amongst the three leaflets.

Abstract:This paper presents a new method for modeling the mechanics of the aortic valve and simulates its interaction with blood. As much as possible, the model construction is based on first principles, but such that the model is consistent with experimental observations. We require that tension in the leaflets must support a pressure, then derive a system of partial differential equations governing its mechanical equilibrium. The solution to these differential equations is referred to as the predicted loaded configuration; it includes the loaded leaflet geometry, fiber orientations and tensions needed to support the prescribed load. From this configuration, we derive a reference configuration and constitutive law. In fluid-structure interaction simulations with the immersed boundary method, the model seals reliably under physiological pressures and opens freely over multiple cardiac cycles. Further, model closure is robust to extreme hypo- and hypertensive pressures. Then, exploiting the unique features of this model construction, we conduct experiments on reference configurations, constitutive laws and gross morphology. These experiments suggest the following conclusions: (1) The loaded geometry, tensions and tangent moduli primarily determine model function. (2) Alterations to the reference configuration have little effect if the predicted loaded configuration is identical. (3) The leaflets must have sufficiently nonlinear material response to function over a variety of pressures. (4) Valve performance is highly sensitive to free edge length and leaflet height. These conclusions suggest appropriate gross morphology and material properties for the design of prosthetic aortic valves. In future studies, our aortic valve modeling framework can be used with patient-specific models of vascular or cardiac flow.

Abstract:Given the complexity of human left heart anatomy and valvular structures, the fluid–structure interaction (FSI) simulation of native and prosthetic valves poses a significant challenge for numerical methods. In this review, recent numerical advancements for both fluid and structural solvers for heart valves in patient-specific left hearts are systematically considered, emphasizing the numerical treatments of blood flow and valve surfaces, which are the most critical aspects for accurate simulations. Numerical methods for hemodynamics are considered under both the continuum and discrete (particle) approaches. The numerical treatments for the structural dynamics of aortic/mitral valves and FSI coupling methods between the solid Ωs and fluid domain Ωf are also reviewed. Future work toward more advanced patient-specific simulations is also discussed, including the fusion of high-fidelity simulation within vivo measurements and physics-based digital twining based on data analytics and machine learning techniques.

Abstract:Introduction: After diagnosis and treatment of aortic dissections, there is the potential for future complications such as the growth or rupture of pre-existing aneurysms or further dissection of the aorta. This susceptibility to future complications is what we refer to as “aortic fragility”. Current computational models of aortic dissection rely upon computational fluid dynamics (CFD) or finite element analysis (FEA); however, there are limitations to using either method alone. Using a healthy patient aorta as our control case, we developed a novel method of coupling CFD with FEA in a fluid surface interaction (FSI) model for studying aortic dissection. Methods: We developed this FSI model by first segmenting aortas from CT scans to generate patient-specific models. With these meshes, we used a cardiac cycle fluid velocity profile to perform CFD and extracted static pressure values on the aortic wall at systole and diastole. We applied these pressures to our FEA model as the mechanical load driving wall deformation. Results: We successfully completed the CFD portion of our FSI simulation with results consistent with our expectations. Velocity data at the ascending and descending aorta, branches and arch correctly reflect our input cardiac cycle velocity profile. Histograms of static pressure values on the aortic wall at systole and diastole show that the mean systolic pressure is greater than the mean diastolic pressure, thus reflecting our expectations. Heat maps of this data reveal the aortic arch and branches to be areas of highest pressure, with pressure gradually decreasing in the distal aorta. Conclusion: With our healthy aorta control, we have successfully validated the CFD component of our FSI model. Our methods improve upon previous models by coupling CFD with FEA, allowing the use of realistic pressure loads derived from fluid analysis. We plan to further improve by validating the FEA component of our model and applying our methodology to dissection geometries. With this FSI workflow, we hope to more accurately model and predict the deformation patterns of aortic dissection in fragile aortas.

Abstract:Heart valves are essential for maintaining unidirectional blood flow, and their failure can severely affect cardiac functions. The use of artificial heart valves as replacement has proven to be a reliable and effective solution. Computational fluid dynamics has emerged as a powerful numerical tool for investigating the design, performance, and malfunctioning of mechanical heart valves without the need for invasive procedures. In this study, we employed smoothed particle hydrodynamics (SPH) in an open-source code “DualSPHysics,” to study the hemodynamics of a bi-leaflet mechanical heart valve (BMHV). The proposed SPH method was validated against the traditional finite volume method and experimental data, highlighting its suitability for simulating the heart valve function. The Lagrangian description of motion in SPH is particularly advantageous for fluid–structure interaction (FSI), making it well-suited for accurately modeling the heart valve dynamics. Furthermore, the SPH/FSI technique was applied to investigate the hemodynamic abnormalities associated with BMHV dysfunction. This work represents the first attempt to use SPH to model flow through a realistic BMHV by incorporating FSI. The normal and altered flow behavior and the movement dynamics of the BMHV under various blockage scenarios have also been investigated along with the potential risks of the blocked mechanical valve. The findings demonstrate that this SPH/FSI approach provides a unique, effective, and valuable tool for accurately capturing the transient hemodynamic behavior of bi-leaflet heart valves and its versatility enables the application to more complex patient-specific issues related to cardiovascular diseases.

Abstract:Fibro-calcific aortic valve (AV) diseases are characterized by calcium growth or accumulation of fibrosis in the AV tissues. Fibrocalcific aortic stenosis (FAS) rises specifically in females, like calcification-induced aortic stenosis (CAS), may eventually necessitate valve replacement. Fluid-structure-interaction (FSI) computational models for severe CAS and FAS patients were developed using lattice Boltzmann method and multi-scale finite elements (FE). Three parametric AV models were introduced: pathology-free of non-calcified tri-and-bicuspid AVs with healthy collagen fibre network (CFN), a FAS model incorporated a thickened CFN with embedded small calcification volumes, and a CAS model employs healthy CFN with embedded high calcification volumes. The results indicate that the interaction between calcium deposits, adjacent tissue and fibres crucially influences haemodynamics and structural reactions. A fourth model of transcatheter aortic valve replacement (TAVR) post-procedure outcomes was created to study both CAS and FAS. TAVR-CAS had a higher maximum contact pressure and lower anchoring area than TAVR-FAS, making it prone to aortic tissue damage and migration. Finally, although the TAVR-CAS offered a larger opening area, its paravalvular leakage was higher. This may be attributed to a similar thrombogenicity potential characterizing both models. The computational framework emphasizes the significance of mechanobiology in FAS and underscores the requirement for tissue modelling at multiple scales.

Abstract:The failure of the aortic heart valve is common, resulting in deterioration of the pumping function of the heart. For the end stage valve failure, bi-leaflet mechanical valve (most popular artificial valve) is implanted. However, due to its non-physiological behaviour, a significant alteration is observed in the normal haemodynamics of the aorta. While in-vivo experimentation of a human heart valve (native and artificial) is a formidable task, in-silico study using computational fluid dynamics (CFD) with fluid structure interaction (FSI) is an effective and economic tool for investigating the haemodynamics of natural and artificial heart valves. In the present work, a haemodynamic model of a natural and mechanical heart valve has been developed using meshless particle-based smoothed particle hydrodynamics (SPH). In order to further enhance its clinical relevance, this study employs a patient-specific vascular geometry and presents a successful validation against traditional finite volume method and 4D magnetic resonance imaging (MRI) data. The results have demonstrated that SPH is ideally suited to simulate the heart valve function due to its Lagrangian description of motion, which is a favourable feature for FSI. In addition, a novel methodology for the estimation of the wall shear stress (WSS) and other related haemodynamic parameters have been proposed from the SPH perspective. Finally, a detailed comparison of the haemodynamic parameters has been carried out for both native and mechanical aortic valve, with a particular emphasis on the clinical risks associated with the mechanical valve.

A spatiotemporal analysis of the left coronary artery biomechanics using fluid–structure interaction models

Abstract:Biomechanics plays a critical role in coronary artery disease development. FSI simulation is commonly used to understand the hemodynamics and mechanical environment associated with atherosclerosis pathology. To provide a comprehensive characterization of patient-specific coronary biomechanics, an analysis of FSI simulation in the spatial and temporal domains was performed. In the current study, a three-dimensional FSI model of the LAD coronary artery was built based on a patient-specific geometry using COMSOL Multiphysics. The effect of myocardial bridging was simulated. Wall shear stress and its derivatives including time-averaged wall shear stress, wall shear stress gradient, and OSI were calculated across the cardiac cycle in multiple locations. Arterial wall strain (radial, circumferential, and longitudinal) and von Mises stress were calculated. To assess perfusion, vFFR was calculated. The results demonstrated the FSI model could identify regional and transient differences in biomechanical parameters within the coronary artery. The addition of myocardial bridging caused a notable change in von Mises stress and an increase in arterial strain during systole. The analysis performed in this manner takes greater advantage of the information provided in the space and time domains and can potentially assist clinical evaluation.

Abstract:Each year, approximately 300,000 heart valve repair or replacement procedures are performed worldwide, including approximately 70,000 aortic valve replacement surgeries in the United States alone. Computational platforms for simulating cardiovascular devices such as prosthetic heart valves promise to improve device design and assist in treatment planning, including patient-specific device selection. This paper describes progress in constructing anatomically and physiologically realistic immersed boundary (IB) models of the dynamics of the aortic root and ascending aorta. This work builds on earlier IB models of fluid-structure interaction (FSI) in the aortic root, which previously achieved realistic hemodynamics over multiple cardiac cycles, but which also were limited to simplified aortic geometries and idealized descriptions of the biomechanics of the aortic valve cusps. By contrast, the model described herein uses an anatomical geometry reconstructed from patient-specific computed tomography angiography (CTA) data, and employs a description of the elasticity of the aortic valve leaflets based on a fiber-reinforced constitutive model fit to experimental tensile test data. The resulting model generates physiological pressures in both systole and diastole, and yields realistic cardiac output and stroke volume at physiological Reynolds numbers. Contact between the valve leaflets during diastole is handled automatically by the IB method, yielding a fully competent valve model that supports a physiological diastolic pressure load without regurgitation. Numerical tests show that the model is able to resolve the leaflet biomechanics in diastole and early systole at practical grid spacings. The model is also used to examine differences in the mechanics and fluid dynamics yielded by fresh valve leaflets and glutaraldehyde-fixed leaflets similar to those used in bioprosthetic heart valves. Although there are large differences in the leaflet deformations during diastole, the differences in the open configurations of the valve models are relatively small, and nearly identical hemodynamics are obtained in all cases considered.

Abstract:This study describes an integrated approach biomechanics to modeling of a frameless prosthetic heart valve based on the analysis of medical graphic data (multispiral computed tomography), solid modeling (Abaqus/CAE) and numerical analysis of blood flow (OpenFOAM). The object of the study was the clinical case of patient D. (56 years old), who was surgically implanted with the “TiAra” aortic valve prosthesis. In this work, we reconstructed three-dimensional computer models of the functioning of the prosthesis for 10 segments of one cardiac cycle, followed by numerical experiments reproducing the movement of the key points of the product –commissural racks, leaflets. High asymmetric mobility of the bioprosthesis elements was shown during deformations of the heart cycle – one of the racks is much more mobile than the other two (up to 32% in movement). Solid modeling of the prosthesis deformation did not reveal significant malfunctioning or exceeding the stress-strain state of the components: Mises stress maximum reached 0.8 MPa. The distribution of stress indicators on the diagrams of three-dimensional models revealed the main concentration in the leaflet bell and commissural racks. In addition, qualitatively, the operation of the cusp apparatus corresponded to the functioning conditions – the dynamics of pressure changes in the structure of the cardiac cycle “systole diastole”, area of cusps contact, and the elastic properties of the materials. The analysis of the flow simulation results determined the presence of several sections with a turbulent structure in the region of the Valsalva sinuses, which, however, did not demonstrate critical quantitative characteristics – wall shear stresses (up to 74 MPa), flow velocities (0.1–1.0 m/s).

Abstract:Aortic lesions, exemplified by bicuspid aortic valves (BAVs), can complicate congenital heart defects, particularly in Turner syndrome patients. The combination of BAV, dilated ascending aorta, and an elongated aortic arch presents complex hemodynamics, requiring detailed analysis for tailored treatment strategies. While current clinical decision-making relies on imaging modalities offering limited biomechanical insights, integrating high-performance computing and fluid–structure interaction algorithms with patient data enables comprehensive evaluation of diseased anatomy and planned intervention. In this study, a patient-specific workflow was utilized to biomechanically assess a Turner syndrome patient's BAV, dilated ascending aorta, and elongated arch. Results showed significant improvements in valve function (effective orifice area, EOA increased approximately twofold) and reduction in valve stress (~ 1.8-fold) following virtual commissurotomy, leading to enhanced flow dynamics and decreased viscous dissipation (~ twofold) particularly in the ascending aorta. However, increased viscous dissipation in the distal transverse aortic arch offset its local reduction in the AAo post-intervention, emphasizing the elongated arch's role in aortic hemodynamics. Our findings highlight the importance of comprehensive biomechanical evaluation and integrating patient-specific modeling with conventional imaging techniques for improved disease assessment, risk stratification, and treatment planning, ultimately enhancing patient outcomes.

A reduced 3D-0D FSI model of the aortic valve including leaflets curvature

Abstract:In the present work, we propose a novel lumped-parameter model for the description of the aortic valve dynamics, including elastic effects associated to the leaflets' curvature. The introduction of a lumped-parameter model based on momentum balance entails an easier calibration of the parameter models, that are instead typically numerous in phenomenological-based models. This model is coupled with 3D Navier-Stokes equations describing the blood flow, where the valve surface is represented by a resistive method, and valve leaflets velocity is taken into consideration. The resulting reduced fluid-structure interaction problem has a computational cost that is comparable with the solution of a prescribed-motion fluid dynamics problem. A SUPG-PSPG stabilized finite element scheme is adopted for the discretization of the coupled problem, and the computational results show the suitability of the system in representing the leaflets motion, the blood flow in the ascending aorta, and the pressure jump across the leaflets.

Abstract:Finite Element simulations are a robust way of investigating cardiac biomechanics. To date, it has only been performed with the left ventricle (LV) alone for fetal hearts, even though results are likely different with biventricular (BiV) simulations. In this research, we conduct BiV simulations of the fetal heart based on 4D echocardiography images to show that it can capture the biomechanics of the normal healthy fetal heart, as well as those of fetal aortic stenosis better than the LV alone simulations. We found that performing LV alone simulations resulted in overestimation of LV stresses and pressures, compared to BiV simulations. Interestingly, inserting a compliance between the LV and right ventricle (RV) in the lumped parameter model of the LV only simulation effectively resolved these overestimations, demonstrating that the septum could be considered to play a LV-RV pressure communication role. However, stresses and strains spatial patterns remained altered from BiV simulations after the addition of the compliance. The BiV simulations corroborated previous studies in showing disease effects on the LV, where fetal aortic stenosis (AS) drastically elevated LV pressures and reduced strains and stroke volumes, which were moderated down with the addition of mitral regurgitation (MR). However, BiV simulations enabled an evaluation of the RV as well, where we observed that effects of the AS and MR on pressures and stroke volumes were generally much smaller and less consistent. The BiV simulations also enabled investigations of septal dynamics, which showed a rightward shift with AS, and partial restoration with MR. Interestingly, AS tended to enhance RV stroke volume, but MR moderated that down.

Abstract:<h3 class="Heading">Purpose</h3><p class="Para">Transcatheter aortic valve replacement (TAVR) is a minimally invasive treatment for high-risk patients with aortic diseases. Despite its increasing use, many influential factors are still to be understood and require continuous investigation. The best numerical approach capable of reproducing both the valves mechanics and the hemodynamics is the fluid–structure interaction (FSI) modeling. The aim of this work is the development of a patient-specific FSI methodology able to model the implantation phase as well as the valve working conditions during cardiac cycles.</p><h3 class="Heading">Methods</h3><p class="Para">The patient-specific domain, which included the aortic root, native valve and calcifications, was reconstructed from CT images, while the CAD model of the device, metallic frame and pericardium, was drawn from literature data. Ventricular and aortic pressure waveforms, derived from the patient's data, were used as boundary conditions. The proposed method was applied to two real clinical cases, which presented different outcomes in terms of paravalvular leakage (PVL), the main complication after TAVR.</p><h3 class="Heading">Results</h3><p class="Para">The results confirmed the clinical prognosis of mild and moderate PVL with coherent values of regurgitant volume and effective regurgitant orifice area. Moreover, the final release configuration of the device and the velocity field were compared with postoperative CT scans and Doppler traces showing a good qualitative and quantitative matching.</p><h3 class="Heading">Conclusion</h3><p class="Para">In conclusion, the development of realistic and accurate FSI patient-specific models can be used as a support for clinical decisions before the implantation.</p>

Abstract:Computational hemodynamics has become increasingly important within the context of precision medicine, providing major insight in cardiovascular pathologies. However, finding appropriate compromise between speed and accuracy remains challenging in computational hemodynamics for an extensive use in decision making. For example, in the ascending thoracic aorta, interactions between the blood and the aortic wall must be taken into account for the sake of accuracy, but these fluid structure interactions (FSI) induce significant computational costs, especially when the tissue exhibits a hyperelastic and anisotropic response. The objective of the current study is to use the Small On Large (SOL) theory to linearize the anisotropic hyperelastic behavior in order to propose a reduced-order model for FSI simulations of the aorta. The SOL method is tested for fully-coupled FSI simulations in a patient-specific aortic geometry presenting an Ascending Thoracic Aortic Aneurysm (aTAA). The same model is also simulated with a fully-coupled FSI with non-linear material behavior, without SOL linearization. Eventually, the results and computational times with and without the SOL are compared. The SOL approach is demonstrated to provide a significant reduction of computational costs for FSI analysis in the aTAA, and the results in terms of stress state distribution are comparable. The method is implemented in ANSYS and will be further evaluated for clinical applications.

Abstract:Computational modelling of whole-heart function is a useful tool to study heart mechanics and haemodynamics. Many existing heart models focus on electromechanical aspect without considering physiological valves and use simplified fluid models instead. In this study we develop a four-chamber heart model featuring realistic chamber geometry, detailed valve modelling, hyperelasticity with fibre architecture and fluid–structure interaction analysis. Our model is used to investigate heart behaviours with different modelling assumptions including restricted/free valve annular dynamics, and with/without heart-pericardium interactions. Our simulation results capture the interactions between valve leaflet and surrounding flow, typical left ventricular flow vortices, typical venous and transvalvular flow waveform, and physiological heart deformations such as atrioventricular plane movement. The improvement of ventricular filling and atrial emptying at early diastole is evident with free annulus. In addition, we find that the added pericardial forces on the heart have a predominant effect on atrial wall deformation especially during atrial contraction, and further help with the atrial filling process. Most importantly, the current study provides a framework for comprehensive multi-physics whole-heart modelling considering all heart valves and fluid–structure interactions.

Immersed boundary model of aortic heart valve dynamics with physiological driving and loading conditions

Abstract:The immersed boundary (IB) method is a mathematical and numerical framework for problems of fluid-structure interaction, treating the particular case in which an elastic structure is immersed in a viscous incompressible fluid. The IB approach to such problems is to describe the elasticity of the immersed structure in Lagrangian form, and to describe the momentum, viscosity, and incompressibility of the coupled fluid-structure system in Eulerian form. Interaction between Lagrangian and Eulerian variables is mediated by integral equations with Dirac delta function kernels. The IB method provides a unified formulation for fluid-structure interaction models involving both thin elastic boundaries and also thick viscoelastic bodies. In this work, we describe the application of an adaptive, staggered-grid version of the IB method to the three-dimensional simulation of the fluid dynamics of the aortic heart valve. Our model describes the thin leaflets of the aortic valve as immersed elastic boundaries, and describes the wall of the aortic root as a thick, semi-rigid elastic structure. A physiological left-ventricular pressure waveform is used to drive flow through the model valve, and dynamic pressure loading conditions are provided by a reduced (zero-dimensional) circulation model that has been fit to clinical data. We use this model and method to simulate aortic valve dynamics over multiple cardiac cycles. The model is shown to approach rapidly a periodic steady state in which physiological cardiac output is obtained at physiological pressures. These realistic flow rates are not specified in the model, however. Instead, they emerge from the fluid-structure interaction simulation.

Abstract:Fluid–structure interactions (FSI) are very common in many natural processes and engineering applications. However, modeling FSI with large structural deformations and turbulence flows under high Reynolds number (Re) is still a challenging task. Here, we present a hybrid 3D model that combines the advantages of Lattice Boltzmann Method (LBM) in solving complex flow problems and the capability of Material Point Method (MPM) in handling large structural deformations. A sharp interface coupling scheme is presented in detail and a LES sub-grid model is adapted for turbulent flows. The structure solver is validated by comparing with Euler beam theory and the fluid solver is tested for fluid round circle and cube obstacles under a large range of Re. The coupled model is validated by simulations of a flexible plate deformation in shear flows. Good agreements are found comparing with experimental results. Finally some applications of importing realistic geometries from Computer Assisted Design (CAD) software, are shown to demonstrate the future applications of the method.
