Abstract:Introduction: The U.S. Army conducts airborne operations in order to insert soldiers into combat. Military airborne operations are physically demanding activities with a unique loading environment compared with normal duties. A significant amount of research surrounding airborne operations has focused on assessing the incidence and type of associated injuries as well as the potential risk factors for injuries. During parachute opening shock and other high-acceleration events (e.g., fixed wing flight or vehicle crashes), the neck may be vulnerable to injury if inertial loads overcome the voluntary muscular control of the cervical spine and soft tissue structures. A recent epidemiological survey of sport skydivers showed that the neck, shoulders, and back were the most frequently reported sites of musculoskeletal pain. In addition, the survey indicated that wing loading (a measure of the jumper's weight divided by the size of the parachute canopy) was a potential contributing factor for developing musculoskeletal pain. Recently, there have been efforts to measure the severity of parachute opening shock as an additional potential risk factor for injury; however, no studies have measured both head and body accelerations and no studies have measured head or body angular rate during parachute opening shock. The purpose of this study was to measure and characterize the accelerations and angular rates of both the head and body during parachute opening shock as well as investigate potential factors contributing to higher severity opening shock, which may link to the development of musculoskeletal pain or injury. Materials and methods: Data were collected from the U.S. Army Parachute Team, The Golden Knights, under an approved Medical Research and Material Command Institutional Review Board protocol. Subjects were instrumented with a helmet- and body-mounted sensor package, which included three angular rate sensors and three single-axis accelerometers each. Data were collected at 2,500 samples per second. Kruskal-Wallis tests were used to determine if helmet-mounted equipment (e.g., cameras), neck length, neck circumference, or wing loading (the ratio of jump weight to the size of the main parachute canopy) affected the accelerations or angular rates of the head or body. Results: A total of 54 jumps conducted by 19 experienced free-fall jumpers were analyzed. For the head, the mean (± SD) resultant accelerations and angular rates were 5.8 (± 1.6) g and 255.9 (± 74.2) degrees per second (deg/s), respectively. For the body, the resultant accelerations and angular rates were 4.3 (± 1.5) g and 181.3 (± 61.2) deg/s, respectively. A wing loading above 1.4 pounds per square foot (lb/ft2) was found to have a significant effect on head (P = .001) and body (P = .001) resultant acceleration as well as body angular rate about the Y-axis (P = .001). Conclusions: There is evidence to suggest that wing loading has an influence on individual head and body resultant accelerations. However, no significant effects were found for the other variables (e.g., neck length and circumference, helmet-mounted equipment, etc.). Future research should focus on identifying additional factors that result in changes in accelerations and angular rates of the head and body during parachute opening shock events.

Abstract:Summary A randomized, double-blind, crossover trial was carried out in 17 hypertensive patients to evaluate the hypotensive efficacy and safety of verapamil. Verapamil in doses of 120 mg thrice daily was compared with pindolol in doses of 7.5 mg twice daily. A thiazide diuretic was given with both drugs. Blood pressure fell about 14/11 mm Hg during treatment with either drug compared to the placebo period, and neither drug caused significant side effects. Verapamil did not affect plasma renin concentration.

Abstract:Neck injuries from parachute opening shock (POS) are a concern in skydiving and military operations. This study employs finite element modeling to simulate POS scenarios and assess cervical spine injury risks. Validated against various conditions, including whiplash, the model replicates head/neck kinematics and soft tissue responses. POS simulations capture body/head motions during parachute deployment, indicating minimal risk of severe neck injuries (Abbreviated Injury Score/AIS ≥ 2) and low risk of soft tissue tears. Vertebral stress analysis during a rougher jump highlights high stress at C5/C6 lamina, indicating fracture risk. Comparative analysis with rear impact scenarios reveals distinct strain patterns, with rear impacts showing higher ligament strain, consistent with higher soft tissue damage risk. Though POS simulations exhibit lower strain values, they emphasize similar neck deformation patterns. The model's capability to accurately simulate head and neck movements during parachute openings provides critical validation for its use in assessing injury risks. The study's findings underline the importance of considering specific loading conditions in injury assessments and contribute to refining safety standards for skydiving and military operations. By highlighting the differences in injury mechanisms between POS and rear impacts, this research offers valuable insights into tailored injury mitigation strategies. The results not only enhance our understanding of neck injury mechanisms but also inform the development of protective gear and safety protocols, ultimately aiding in injury prevention for skydivers and military personnel.

Skydiving: The audiological perspective

Abstract:Skydiving is a popular recreational sport for the young and old. There is minimal research pertaining to skydiving and its relation to the audiological system. The risks of skydiving in relation to the auditory system should be explored further. Aims: The main aim of this study was to explore the relationship between skydiving and audiology in South Africa. The sub-aims of the study focused on determining if skydivers were provided with safety precautions before they commenced with the dive, determining the middle ear pressure before and after the skydive and identifying the audiological symptoms that were present post-dive. This study also aimed at scrutinising the South African sports and recreation policy. Method: A mixed-method descriptive research design was utilised. Qualitative information pertaining to audiology was identified and recorded from the scrutiny of South Africa (SA) policy and the dropzone consent forms at two skydiving schools. Thirty-one skydivers were purposefully recruited to undergo a pre- and post-dive tympanometric assessment. Results: There is no information within the clearance forms that pertain to the audiological risks related to skydiving. There was a lack of information related to the risks of skydiving in the clearance forms at both dive schools. A statistically significant pressure change was noted in regular skydivers, regardless of the ability to equalise effectively during the skydive. Conclusion: This study identified the gaps in policy and clearance forms, highlighting the need for the inclusion of safety measures and risks in the documentation and legislation that governs the sport. Audiologists, sportspeople and medical advisors should be cognisant of the negative consequences that may be evident within the auditory system of skydivers.
The Risk Factors for Injuries in Parachuting and Load Exposure in the Training of Chinese Paratroopers.

Abstract:BACKGROUND: Parachutists are generally recognized as a "high-risk" group among military personnel. However, the findings came mostly from data analysis without soldiers as subjects. OBJECTIVE: This study aimed to investigate the injury prevalence in Chinese paratroopers on-site and determine the relationship between injury and risk factors encountered during parachuting and land-based training. METHODS: This study consisted of a field study with questionnaire and an experiment on muscle load during the simulated training exercise of platform jumping with surface electromyography (EMG), in which 7230 paratroopers and 38 soldiers were involved respectively. Chi-square test was used for the injury rate analysis, ANOVA and t-test for comparison of EMG data, and logistic regression for the analysis of multiple factors. Taking both intensity and time into consideration, jump-years (J-yrs) was used as a complex indicator for exposure to parachuting. Either injury per 1000 jumps or injured persons per 100 soldiers were calculated as injury prevalence. RESULTS: The overall injury rate among Chinese parachutists was found to be 13.9 injuries per 1000 parachute jumps and 24.5% based on personnel. The person-based injury rate increased with the exposure level significantly (.2 = 142.06, 2-sided, P < 0.05; trend test also significantly). Among the identified risk factors the uneven terrain was ranked as the most important one by logistic analysis. The EMG amplitude in MVE% increased with the platform height of all the 8 measured muscles and even reached 100% in 4 muscles, showing a high impact at landing. In addition, some characteristics of parachuting injury were also revealed by the injury type and site analysis. CONCLUSION: A dose-response relationship between parachuting and injury was observed significantly in the survey of Chinese paratroopers. Their injury rate was found to be relatively higher than the reported internationally. Landing impact as a critical point for injury seemed to be proved by the investigation and also the experiment with EMG measurement. It is suggested for future studies, to use the person-based injury rate, landing studied in work physiology and with consideration of different landing skills.

Abstract:Antigen dependent cellular cytotoxity (ADCC) and non-killer cell activities of haematological healthy donors were investigated in the 51Cr release test. Attempts of cell fraction reveal that lymphocytes are active as killer and non-killer cells. Granulocytes were efficient effector cells of antigen dependent cellular cytotoxity (ADCC), however, they had no natural-killer activity. In testing leukocyte fractions of 11 donors, killer cell would only be found in the non-T-fraction. In contrast to that, three types could be observed in the distribution on non-killer cells: Distribution on T-lymphocyte fraction (27.3%), distribution on non-T-lymphocyte fraction (9.1%) and approximately equal distribution on T- and non-T-lymphocyte fraction 63.7%). Without any treatment patients with acute lymphocytic leukemia showed an antigen dependent cellular cytotoxity and non-killer activity only in exceptional cases. Normal activities were reached in remission, with chemotherapy having a depressive effect on non-killer activity.

Abstract:Introduction: Neck pain in fighter pilots has been studied by several air forces and found to be relatively common. The aim of this project was to study the incidence, characteristics, possible associated causative factors, and operational impact of neck pain in Royal Norwegian Air Force (RNoAF) fighter pilots. Methods: The study was designed as a retrospective anonymous questionnaire survey, collecting data on age, aircraft type, flying hours, and physical activity. Any experience of spinal symptoms related to flying was included, as well as detailed questions on operational factors. Estimates regarding how neck symptoms influenced flying performance were established using visual analogue scales (VAS). Pilots also described their own in-flight techniques to avoid neck symptoms. Results: Of respondents, 72% had experienced neck pain in relation to flying, while 35% had experienced back pain. Of these episodes, 93% were related to neck rotation. Mean G level for acute incidents of in-flight pain was 6.7 G. Total training time is on average higher in pilots who have no neck pain compared to those who have had neck pain events in the last 12 mo; the mean training time being 3.9 h in the "no pain-group" vs. 2.7 h in the "pain group". "Checking six" was the most affected in-flight function. Discussion: New technologies such as night-vision goggles and helmet-mounted displays increase helmet weight, thereby adding a higher strain to the neck even in moderate G environments. More research on specific prevention strategies is warranted in order to improve the in-flight working environment of fighter pilots.

Abstract:OBJECTIVE: We investigated the incidence and injury patterns of acute parachuting injuries evaluated at a level II military facility during a 5-year period to compile medical information that may assist in improving the safety of parachuting.METHODS: A retrospective chart review of all parachuting injuries that presented to the Madigan Army Medical Center emergency department in Tacoma, Washington, during a 5-year period from February 2005 to June 2011.RESULTS: A total 110 patient charts met inclusion criteria. Lower extremity injuries made up 65% of total injuries, followed by 22% head injuries, 22% neck or back injuries, and 19% upper extremity injuries. One patient died in the emergency department of head injuries, and 1 patient was transferred for an open-book pelvic fracture. The most common phase of the jump in which to sustain injury was at landing. A total of 15 patients were admitted to the hospital. It is of note that some patients had combined injuries. The mechanism of injury documented in 96 of 110 (87%) patients and 10 of 15 (66%) admitted patients was a difficult landing.CONCLUSIONS: The rate of injury for each area of the body is within previously reported rates, with lower extremity injuries making up the largest category and leading to the majority of admissions. The most common time to sustain an injury was during landing, which represented a majority of both emergency room visits and hospitalizations.

Low-back problems in recreational self-contained underwater breathing apparatus divers: prevalence and specific risk factors

Abstract:Low-back problems (LBP) are one of the most common musculoskeletal disorders in the general population, with reported lifetime prevalences of 50% to 80%. Also, certain sports participants (e.g. gymnasts, alpine skiers, runners) are at risk of LBP and its repercussions. This epidemiological study was undertaken to examine the lifetime and 1-year prevalence of LBP among recreational Flemish self-contained breathing apparatus (scuba) divers and to identify general and sport-specific risk factors associated with the occurrence of LBP. A retrospective self-assessment questionnaire was developed and assessed for validity and reliability, to gather data concerning demographics, LBP prevalence and associated risk factors and injuries among active scuba divers. A total of 181 recreational scuba divers (men: n = 138, mean age 40.3 +/- 12.8 years; women: n = 43, mean age 35.0 +/- 10.9 years) from 10 randomly selected internationally recognised scuba diving clubs participated in the study. Lifetime and 1-year prevalence of LBP among recreational Flemish scuba divers were 55.8% and 50.3%, respectively. General risk factors for LBP were found to include prior history of LBP, structural abnormalities, heavy workload, pregnancy and parturition, general fatigue and bending forwards and backwards. Scuba divers suffering from LBP generally had a significantly higher dive certificate than those without LBP (p = 0.007). Symptomatic scuba divers also used significantly more weights on their weight belts during indoor training (p = 0.003) and during outdoor dives with a dry suit (p = 0.044) as compared to asymptomatic scuba divers. In scuba diving, reliable sport-specific risk factors for LBP were found to be scarce. Further biomechanical research is required to point out whether or not scuba diving characteristics actually contribute to LBP.

Abstract:Objective The popularity of sport climbing has been growing since its inclusion in the Olympic Games program, which led to more people practicing it on recreational, amateur, and professional levels. Strenuous climbing training sessions and competitions might lead to frequent and serious musculoskeletal injuries and complaints among competitive climbers. This study aimed to investigate the prevalence of low back pain (LBP) and to explore the influence of various risk factors on LBP in adolescent climbers. Methods The sample included 180 competitive climbers (46.6% males) aged 13–19 years competing in under-16 (48.3%) or under-20 categories. Data collection was carried out using the Nordic Musculoskeletal Questionnaire (NMQ) and the Graded Chronic Pain Scale (GCPS). Results A total of 74.4% of the entire sample of participants (male = 75%; female = 74%) reported lower back complaints throughout the past twelve months, and only 15.5% during the last seven days. A major part of complaints was classified as low intensity-low disability (Grade I, 62.8%; male = 72.6%; female = 54.2%). Under-20 competitors reported a small but significantly higher percentage of almost all NMQ measures compared to under-16 athletes. Conclusions This study found a relatively high prevalence of LBP, although complaints were of limited severity and did not affect climbers' regular training practice. Moreover, climbers did not differ in LBP prevalence according to sex, while climbers from the older age group reported higher complaints and seeking medical attention than younger climbers. Future studies should prospectively monitor the influence of climbing on LBP in youth climbers.
Prevalence of Musculoskeletal Injuries in Swedish Elite Track and Field Athletes

Abstract:Background: Little is known of injury patterns in track and field (athletics). Injury prevalence has been proposed as the most appropriate measure of the injury rate in sports where athletes are at risk for overuse problems. Purpose: To ascertain 1-year retrospective and current prevalence of injury in elite track and field athletes to help plan injury prevention programs for this sport. Study Design: Descriptive epidemiology study. Methods: Two hundred seventy-eight youth (16 years old) and adult athletes from an eligible study population of 321 athletes were included. Results: The 1-year retrospective injury prevalence was 42.8% (95% confidence interval [CI], 36.9%-49.0%); the point prevalence was 35.4% (95% CI, 29.7%-41.4%). The diagnosis group displaying the highest injury prevalence was inflammation and pain in the gradual onset category (1-year prevalence, 20.9%; 95% CI, 16.2%-26.2%; and point prevalence, 23.2%; 95% CI, 18.4%-28.7%). A strong tendency for higher 1-year prevalence of 16.5% (95% CI, 12.2%-21.4%) than point prevalence of 8.5% (95% CI, 5.5%-12.5%) was recorded for sudden onset injuries in the diagnosis group sprain, strain, and rupture. The body region showing the highest injury prevalence was the knee and lower leg with 15.0% (95% CI, 11.0%-19.8%) 1-year prevalence and 13.7% (95% CI, 9.8%-18.3%) point prevalence, followed by the Achilles tendon, ankle, and foot/toe with 11.7% (95% CI, 8.2%-16.1%) 1-year prevalence and 11.4% (95% CI, 7.9%-15.8%) point prevalence. Conclusion: The injury prevalence is high among Swedish elite track and field athletes. Most of the injuries affect the lower extremities and are associated with a gradual onset. Although it is associated with a potential recall bias, the 1-year retrospective prevalence measure captured more sudden onset injuries than the point prevalence measure. Future prospective studies in track and field are needed to identify groups of athletes at increased risk.

In the present work we present a 26 min protocol for stroke imaging at 9.4 T using a 1H surface cryo coil [1]. Furthermore, the infarcted regions of mice with a permanent occlusion of the right middle cerebral artery (MCA) were compared to those of mice which underwent recombinant tissue plasminogen activator (rt-PA) therapy.

Abstract:Background: Neck pain (NP) is a common musculoskeletal disorder among fighter pilots and has become a rising concern due to its detrimental impact on military combat effectiveness. The occurrence of NP is influenced by a variety of factors, but less attention has been paid to the association of NP with demographic, occupational, and cervical sagittal characteristics in this group. This study aimed to investigate the prevalence and risk factors of NP in Chinese male fighter pilots using a questionnaire and cervical sagittal measurements. Methods: Demographic and flight-related data, as well as musculoskeletal pain information, were gathered from Chinese male fighter pilots via a self-report questionnaire. Cervical sagittal parameters were measured and subtypes were classified using standardized lateral cervical radiographs. Differences in various factors between the case and control groups were analyzed using t -tests or chi-square tests. Binary logistic regressions were conducted to explore potential risk factors contributing to NP. Predictors were presented as crude odds ratios (CORs) and adjusted odds ratios (AORs), along with their respective 95% confidence intervals (CIs). Results: A total of 185 male fighter pilots were included in this cross-sectional study. Among them, 96 (51.9%) reported experiencing NP within the previous 12 months. The multivariate regression analysis revealed that continuous flight training (AOR: 4.695, 95% CI: 2.226–9.901, p < 0.001), shoulder pain (AOR: 11.891, 95% CI: 4.671–30.268, p < 0.001), and low back pain (AOR: 3.452, 95% CI: 1.600–7.446, p = 0.002) were significantly associated with NP. Conclusion: The high 12-month prevalence of NP among Chinese male fighter pilots confirms the existence of this growing problem. Continuous flight training, shoulder pain, and low back pain have significant negative effects on pilots' neck health. Effective strategies are necessary to establish appropriate training schedules to reduce NP, and a more holistic perspective on musculoskeletal protection is needed. Given that spinal integrated balance and compensatory mechanisms may maintain individuals in a subclinical state, predicting the incidence of NP in fighter pilots based solely on sagittal characteristics in the cervical region may be inadequate.

Musculoskeletal Pain in Adolescents Engaged in Impact and Nonimpact Sports-ABCD Growth Study

Abstract:Purpose: This study aimed to identify, through a 28-week follow-up, the association between pain and engagement in different sports among adolescents. Methods: In total, 63 adolescents reported the occurrence of pain, which was tracked weekly. Participants were categorized into 2 groups based on their sports participation: "swimming group" and "other sports group." Researchers directly measured the time spent in different intensities during sports participation for 1 week using a heart rate monitor. For the covariates, body mass, body fatness, and whole-body lean soft tissue were assessed using a dual-energy X-ray absorptiometry scanner. Kaplan-Meier plots and risk of pain were calculated for the absence of pain during the follow-up. Results: The association between the sport type and pain occurrence remained significant even after adjusting the models for potential confounders, showing that swimmers had an increased risk of reporting pain in the neck (HR = 3.33; 95% CI, 1.53-7.25), shoulders (HR = 2.21; 95% CI, 1.17-4.21), and trunk (HR = 5.60; 95% CI, 2.28-13.70). Conclusion: The association between the sport type and pain occurrence is evident even when controlling for confounding factors.

Abstract:Objectives: Low back pain (LBP) has negative implications for the military's combat effectiveness. This study was conducted to determine the prevalence and risk factors of LBP among pilots through a questionnaire and physical function assessments. Methods: Data on the demographic and occupational characteristics, health habits, physical activity, and musculoskeletal injuries of 217 male pilots (114 fighter, 48 helicopter, and 55 transport pilots) were collected using a self-reported questionnaire and physical function assessments. Results: LBP prevalence was 37.8% in the total cohort and 36.0, 45.8, and 34.5% among fighter, helicopter, and transport pilots, respectively. Multivariate regression analysis revealed that the risk factors significantly associated with LBP were neck pain [odds ratio (OR): 3.559, 95% confidence interval (CI): 1.827–6.934], transversus abdominis activation (OR: 0.346, 95% CI: 0.172–0.698), and hip external rotator strength (OR: 0.001, 95% CI: 0.000–0.563) in the total cohort; neck pain (OR: 3.586, 95% CI: 1.365–9.418), transversus abdominis activation (OR: 0.268, 95% CI: 0.094–0.765), hip external rotator strength (OR: 0.000, 95% CI: 0.000–0.949), and weekly flying hours (OR: 3.889, 95% CI: 1.490–10.149) in fighter pilots; irregular strength training (OR: 0.036, 95% CI: 0.003–0.507) and hip external rotator strength (OR: 0.000, 95% CI: 0.000–0.042) in helicopter pilots; and neck pain (OR: 6.417, 95% CI: 1.424–28.909) in transport pilots. Conclusions: High volume flight schedules and weak core muscle functions have significant negative effects on pilots' back health. LBP is commonly associated with high weekly flying hours, worsening neck pain, transversus abdominis insufficient activation, and reduced hip extensor/rotator strength. Risk factors vary among pilots of different aircraft. Thus, specific core muscle training would be especially important for military pilots.

Abstract:Abstract Background Musculoskeletal pain and its risk factors are rarely assessed in studies on adolescent athletes. The aim was to identify risk factors at baseline that were associated with the persistence or development of musculoskeletal pain at a two-year follow-up in adolescent sport school students, and to study cross-sectional associations at follow-up between musculoskeletal pain and sports performance. Methods Sport school students (79 boys and 52 girls, aged 14 years at baseline) were divided into infrequent (never–monthly) or frequent (weekly–almost daily) pain groups, based on frequency of pain using a pain mannequin. Logistic regression analyses were performed to study longitudinal associations between frequent pain at follow-up and baseline variables: pain group, number of regions with frequent pain, health status by EQ-5D, maturity offset (pre, average, or post peak height velocity), and sports (contact or non-contact). Linear regression analyses were used to study cross-sectional associations between pain groups and 20-m sprint, agility T-test, counter-movement jump, and grip strength at follow-up. Results were stratified by sex. Results A higher percentage of girls than boys reported frequent pain at follow-up (62% vs. 37%; p = 0.005). In boys, frequent pain at follow-up was associated with being pre peak height velocity at baseline (OR 3.884, CI 1.146–13.171; p = 0.029) and participating in non-contact sports (OR 3.429, CI 1.001–11.748; p = 0.050). In girls, frequent pain at follow-up was associated with having frequent pain in two or more body regions at baseline (OR 3.600, CI 1.033–12.542; p = 0.044), having a worse health status at baseline (OR 3.571, CI 1.026–12.434; p = 0.045), and participating in non-contact sports (OR 8.282, CI 2.011–34.116; p = 0.003). In boys, frequent pain was associated with worse performances in 20-m sprint and counter-movement jump, but not in agility T-test and grip strength. Conclusions Baseline risk factors for having frequent pain at follow-up were late maturation in boys, frequent pain and worse health status in girls, and participation in non-contact sports in both sexes. Boys with pain performed worse in sports tests. Coaches and school health-care services should pay attention to the risk factors and work towards preventing pain from becoming persistent.

Abstract:Background: Recreational fitness sports are popular worldwide and rank first among organised sports. This study aims to bridge a knowledge gap by examining which exercises are most prone to causing pain symptoms, as a possible precursor for injury, and analysing the body regions that are most frequently affected. Methods: Using an online questionnaire, 20 demographic and training-specific items and 49 sport-specific exercises were recorded. Frequent exercises as well as the incidence and distribution of pain symptoms that the athletes experienced during or in relation to their training were evaluated. Results: The study assessed common exercises and documented the frequency and distribution of pain symptoms experienced by athletes during or in relation to their training. A total of 1387 respondents were included in this study. Of these, 732 (53.1%) experienced pain during their fitness training, with 333 (24.2%) being female and 397 (22.3%) being male. The method of creating a training plan showed a significant influence (p < 0.001): athletes who devised their own plans reported pain or instability more frequently than those in the comparison groups. Guided exercises on machines resulted in the lowest frequency of pain (11.54%), while exercises with free weights were associated with the highest pain rate among respondents (19.94%). Specifically, exercises such as the back squat, deadlift, bench press, and triceps dips were identified as the exercises most commonly associated with pain. The most frequently reported pain region was the shoulder, followed by the lower back and knees. Conclusion: The findings reveal a significant number of unreported pain symptoms. The disparity between rigorous training volumes and the absence of professional care frequently leads to injuries and pain. It is incumbent upon sports medicine to investigate the root causes of these complaints (pain or instability) to implement preventive measures against potential injuries.

Abstract:Parachuting injuries among cadet pilots often occur because they undergo extensive training within a short period of time. To explore the injury mechanism and to prevent injuries, this paper investigated the injury risk factors for the military parachuting of cadet pilots. A prospective study of static line parachute descents by Chinese Air Force cadet pilots was performed. The study indicates that 153 cadet pilots were injured, with a total of 168 injuries. The injury rates associated with extrinsic and intrinsic risk factors were compared and analyzed by using orthogonal multivariate analysis. Landing is the most dangerous part of parachuting, causing 88% of injuries. Extrinsic factors associated with increased injury rates are higher surface wind speed and third-generation protective boots. Intrinsic factors associated with reduced injury rates are excellent mental qualities, excellent parachuting movements, and female cadet pilots. Strengthening parachuting movements and mental qualities, approving jumps only when basic parachuting conditions are met, and improving protective equipment are recommended strategies for preventing parachuting injuries.

Abstract:Abstract Introduction In a previous study, we found that morning circadian preference and increased fatigue were associated with increased injuries during a nocturnal parachute jump. The purpose of the present study was to replicate this finding, and expand upon it by determining whether, and the extent to which, subjective sleep ratings are also predictive of injury during parachute jumps. Methods Participants included 27 male active duty Soldiers aged 23.7 ± 2.9 years (M ± SD). Participants completed a series of questionnaires at baseline (1 week prior to jump), pre-jump (1 hour before), and post-jump (1 hour after). Questionnaires about sleep; insomnia symptoms; circadian preference (morning vs. evening); injuries; and subjective experiences of sleepiness, fatigue, and jump quality were administered. Results Circadian preference was correlated with ‘sleep quality over the previous month’ (r = -.537, p < .01) and week (r = -.502, p < .01) with those indicating evening preference reporting poorer sleep quality. There was no correlation between circadian preference and average total sleep time. No participants had Insomnia Severity Index (ISI) scores above the clinical threshold for insomnia. However, those indicating an evening circadian preference had higher ISI scores (r = .635, p < 0.001). No significant correlations between circadian preference and previous jump injuries, subjective ratings of parachute landing fall (PLF) quality, or landing hardness were found. Likewise, momentary sleepiness and subjective fatigue were not significantly correlated with PLF or landing hardness. Conclusion In this sample of young Soldiers, evening circadian preference was associated with poorer subjective sleep quality and more insomnia-related symptoms. However, neither circadian preference nor fatigue level was significantly correlated with number of previous jump injuries or subjective ratings of jump performance. Given the small sample size and the generally high skill level of the population of parachutists, it is possible that the present n was not sufficient to detect an effect of circadian preference on injuries or jump quality. In future studies, objective measures of sleep should be added, the n should be increased, and the subject sample should include parachutists with a broader range of experience and training.
