Abstract:A rehabilitation exoskeleton is designed for treatment of elbowjoint anchyloses. Using the Matland Grading Standards method, a series elastic actuator (SEA)basedjoint and its compliance controller for rehabilitation training are designed in this paper. A novel torsional spring is designed with a symmetrical structure for a bidirectional load capacity. This spring was embedded in the elbow of the exoskeleton to act as the SEA activejoint and then worked as a torque sensor, too. We also designed the torque controller and the compliance controller to meet the requirements of the treatment. Successful implementation of the controller in a rehabilitation exoskeleton robot verified the feasibility and realizability of the device. The experimental validation was performed rigorously and the data from the experiments are presented and analyzed.

Abstract:Injuries and diseases such as wrist nerve injuries, stroke, neurological disorders, and other wrist-related conditions have significantly impacted people's quality of life. This study aims to develop a lightweight, affordable, and portable ForeWrist (forearm and wrist) exoskeleton. This device is intended to assist and rehabilitate individuals with wrist disabilities, mainly stroke survivors, to enhance wrist range of motion and strength. The device can offer one active degree of freedom (DOF) responsible for pronation-supination (PS) of the forearm and two passive DOFs for the wrist joint. The design of the ForeWrist PS mainly consists of a cable-driven C-shaped guide rail and stationary bearing-carriage mechanism that can be attached to the user's wrist. The simulation and experimental analysis are conducted for the design validation and performance analysis. The experimental results indicate that the designed device should demonstrate promising potential for practical applications. The root mean squared error for joint position and velocity exhibit low values, and the peak torque for an average weight of the human lower arm was found to be under 10% of the device's total capacity. The developed exoskeleton provides a full range of motion for daily activities and covers 75% of the forearm's total range of motion with a consistency error of less than . The device can be effective for both at home and outdoor assistance and rehabilitation training with its low weight of 300 g and peak velocity and torque of 70 deg/sec and 6 Nm, respectively.

Abstract:Robotic devices for rehabilitation and training is a promising and challenging research topic with a potentially huge social impact. The availability of tools for autonomously performing physiotherapy exercises increases their efficiency, provides supplementary information about results and progress, reduces physiotherapists’ efforts and the need of their physical presence during exercise sessions, and encourages autonomy and independence in people with disabilities. Nevertheless, supportive technologies developed without the inputs and feedback of the end-user throughout the design process are less likely to be adopted for their intended purpose and use case. In this article, we propose a modular hand/wrist exoskeleton that actuates the wrist flexion/extension and adduction/abduction motions and hand fingers flexion/extension motions. It is designed to be wearable and easy to control and manage and can be used by the patient in collaboration with the physiotherapist or autonomously. A user-centered design perspective has been employed in all the design and development phases. This article introduces the main features of the device and presents some tests conducted with a user having limited hand and wrist mobility.

Abstract:This paper presents a novel exoskeleton robot that can be used at home to rehabilitate the index fingers of stroke-affected patients. This exoskeleton is designed as a one-degree-of-freedom four-bar mechanism able to guide the human index finger to perform a finger curl exercise motion. The proposed device is the only lateral, stand-alone mechanism built to date that can carry the weight of the human hand, thus making the user free from wearing it. The design starts by tracing the trajectory of the index finger using 'Angulus' software. 'SALAR Mechanism Synthesizer' software is used for dimensional synthesis of the four-bar mechanism. Using additive manufacturing technology, a prototype of the proposed device is developed. Static force analysis is performed to select the most appropriate actuator for producing the required torque to manipulate the fingers effectively. The kinematics of the index finger while performing a finger curl exercise is obtained. The proposed linkage mechanism can drive the index fingers of both hands. Simulation and experimental results proved the feasibility and effectiveness of the proposed design to be used for index finger rehabilitation for a wide range of users and applications by making simple minor alterations in the design. Also, a scheme for when the device can be used for rehabilitating the middle finger together with the index finger when performing flexion and extension motions is discussed.

Abstract:Patients who suffer from stroke have motion function disorders. They need rehabilitation training guided by doctors and trainers. Nowadays, robots have been introduced to help the patients regain their motion function in rehabilitation training. In this paper, a novel multi degree of freedom (DOF) exoskeleton robot, with light weight, including (6[Formula: see text]1) DOFs, named as Rehab-Arm, is proposed and developed for upper limb rehabilitation. The joints of the robot are equipped with micro motors which are capable of actuating each DOF respectively and simultaneously. The medial/lateral rotation of shoulder is realized by a semi-circle guide mechanism for convenience consideration and safety. The robot is used in sitting posture which is attached to a custom made chair. Hence, the robot can be used to assist patients in passive movement with 7 DOFs of the upper limb for rehabilitation. Five adult healthy male subjects participated in the experiment to test the joint movement accuracy of the robot. Finally, subjects can wear Rehab-Arm and move their upper limb, led by micro motors of the robot, to perform task assigned with specific trajectory.

Abstract:As stroke is one of the most morbidity disease, some rehabilitation devices has been developed to solve the problem, but there isn’t maturation in hand. This paper presents a new hand exoskeleton rehabilitation device to improve the result of rehabilitation and help doctors to perfect theory about hand rehabilitation. We set 8 DOF in this three fingers device, with 2 active DOF and 1 coupled DOF in index or middle finger, 1 active DOF and 1 passive DOF in thumb. The 5 active DOF is driven by motors. The transmission consists of screw nut and multi-link mechanism. We control the device through a typical secondary control system. As this device is designed to help patients not only regain the range of flexion/extension but also complete some typical simple grasping movements. To substantiate the feasibility, an experiment has been carried out. Finally, Suggestions for improvement are given.

Abstract:Wrist impairments caused by stroke or long-term immobilization should be given more attention than the rehabilitation of proximal limb segments. In addition, the physiological effect of wrist exoskeletons on the training lacks sufficient research. According to the design requirements, a novel parallel wrist rehabilitation robot (PWRR) with two electric linear actuators and a passive prismatic joint was developed to achieve the training of the flexion/extension (F/E) and radial/ulnar deviation (R/U). The constraint forces caused by the axis misalignment were significantly reduced under the passive and active training by 0.75 ± 0.22 N and 2.50 ± 0.72 N, respectively. The total work of the active training increased with the velocity, whereas it was much smaller and had no obvious linear relationship with the velocity for the passive training. The sequence and degree of muscular activation of targeted muscles were measured and analyzed at three velocities to reveal the effects of the training mode and velocity on the physiological effects. PWRR could achieve the isometric training of wrist muscles at any angle of F/E and R/U validated by the comparative experiments of this device and weights. At each interactive force, the median frequency (MNF) of the EMG power spectrum and the average rectified value (ARV) of its amplitude decreased with increasing training time. The MNF slope increased with interactive thresholds, where the maximum slope was (−29.5±2.42) ×10−3%/s at 15 N. Conversely, the ARV slope gradually decreased with the thresholds, where the maximum slope was (−61.0±8.55) ×100−3%/s at 5 N.

Abstract:This paper describes the mechanical design, actuation and sensing of an exoskeleton for hand function training after stroke. The frame is 3D-printed in one piece including the joints. Apart from saving assembly time, this enables parametrization of the link sizes in order to adapt it to the patient's hand and reduce joint misalignment. The joint angles are determined using Hall effect sensors. They measure the change of the magnetic field of in the joints integrated magnets achieving an average accuracy of 1.25 °. Tendons attached to the finger tips transmit forces from motors. The armature current, which is proportional to the force transmitting tendons is measured using a shunt and controlled by a custom-made current-limiter circuit. Preliminary experiments with a force/torque-sensor showed high linearity and accuracy with a root mean square error of 0.5937 N in comparison to the corresponding forces derived from the motor torque constant.

Abstract:For more than a decade, many countries have been actively developing robotic assistive devices to assist in the rehabilitation of individuals with limb disability to regain function in the extremities. The exoskeleton assistive device in this study has been designed primarily for hemiplegic stroke patients to aid in the extension of fingers to open up the palm to simulate the effects of rehabilitation. This exoskeleton was designed as an anterior-support type to achieve palmar extension and acts as a robotic assistive device for rehabilitation in bilateral upper limb task training. Testing results show that this wearable exoskeleton assistive device with human factor consideration using percentile dimensions can provide comfortable wear on patients as well as adequate torque to pull individual fingers into flexion towards the palm for rehabilitation. We hope this exoskeleton device can help stroke patients with loss of function in the upper extremities to resume motor activities in order to maintain activities of daily living.

Abstract:Wrist exoskeletons are increasingly being used in the rehabilitation of stroke and hand dysfunction because of its ability to assist patients in high intensity, repetitive, targeted and interactive rehabilitation training. However, the existing wrist exoskeletons cannot effectively replace the work of therapist and improve hand function, mainly because the existing exoskeletons cannot assist patients to perform natural hand movement covering the entire physiological motor space (PMS). Here, we present a bioelectronic controlled hybrid serial-parallel wrist exoskeleton HrWr-ExoSkeleton (HrWE) which is based on the PMS design guidance, the gear set can carry out forearm pronation/supination (P/S) and the 2-DoF parallel configuration fixed on the gear set can carry out wrist flexion/extension (F/E) and radial/ulnar deviation (R/U). This special configuration not only provides enough range of motion (RoM) for rehabilitation training (85F/85E, 55R/55U, and 90P/90S), but also makes it easier to provide the interface for finger exoskeletons and be adapted to upper limb exoskeletons. In addition, to further improve the rehabilitation effect, we propose a HrWE-assisted active rehabilitation training platform based on surface electromyography signals.

Abstract:Neurorehabilitation research suggests that not only high training intensity, but also somatosensory information plays a fundamental role in the recovery of stroke patients. Yet, there is currently a lack of easy-to-use robotic solutions for sensorimotor hand rehabilitation. We addressed this shortcoming by developing a novel clinical-driven robotic hand rehabilitation device, which is capable of fine haptic rendering, and that supports physiological full flexion/extension of the fingers while offering an effortless setup. Our palmar design, based on a parallelogram coupled to a principal revolute joint, introduces the following novelties: (1) While allowing for an effortless installation of the user's hand, it offers large range of motion of the fingers (full extension to 180° flexion). (2) The kinematic design ensures that all fingers are supported through the full range of motion and that the little finger does not lose contact with the finger support in extension. (3) We took into consideration that a handle is usually comfortably grasped such that its longitudinal axis runs obliquely from the metacarpophalangeal joint of the index finger to the base of the hypothenar eminence. (4) The fingertip path was optimized to guarantee physiologically correct finger movements for a large variety of hand sizes. Moreover, the device possesses a high mechanical transparency, which was achieved using a backdrivable cable transmission. The transparency was further improved with the implementation of friction and gravity compensation. In a test with six healthy participants, the root mean square of the human-robot interaction force was found to remain as low as 1.37 N in a dynamic task. With its clinical-driven design and easy-to-use setup, our robotic device for hand sensorimotor rehabilitation has the potential for high clinical acceptance, applicability and effectiveness.

Abstract:Home-based hand rehabilitation has excellent potential as it may reduce patient dropouts due to travel, transportation, and insurance constraints. Being able to perform exercises precisely, accurately, and in a repetitive manner, robot-aided portable devices have gained much traction these days in hand rehabilitation. However, existing devices fall short in allowing some key natural movements, which are crucial to achieving full potential motion in performing activities of daily living. Firstly, existing exoskeleton type devices often restrict or suffer from uncontrolled wrist and forearm movement during finger exercises due to their setup of actuation and transmission mechanism. Secondly, they restrict passive metacarpophalangeal (MCP) abduction–adduction during MCP flexion–extension motion. Lastly, though a few of them can provide isolated finger ROM, none of them can offer isolated joint motion as per therapeutic need. All these natural movements are crucial for effective robot-aided finger rehabilitation. To bridge these gaps, in this research, a novel lightweight robotic device, namely “Flexohand”, has been developed for hand rehabilitation. A novel compliant mechanism has been developed and included in Flexohand to compensate for the passive movement of MCP abduction–adduction. The isolated and composite digit joint flexion–extension has been achieved by integrating a combination of sliding locks for IP joints and a wire locking system for finger MCP joints. Besides, the intuitive design of Flexohand inherently allows wrist joint movement during hand digit exercises. Experiments of passive exercises involving isolated joint motion, composite joint motions of individual fingers, and isolated joint motion of multiple fingers have been conducted to validate the functionality of the developed device. The experimental results show that Flexohand addresses the limitations of existing robot-aided hand rehabilitation devices.

Abstract:Hand rehabilitation is critical to the recovery of post-stroke patients. However, designing a modular mechanism of the hand to improve the human-machine compatibility and precision of operation is still a challenge. This paper proposes a new type of hand exoskeleton rehabilitation robot with nine degrees of freedom. With the flexible rods, the passive range of motion for finger adduction/abduction is extended under the premise of independent flexion/extension of the metacarpophalangeal and proximal interphalangeal joint. Based on hand anatomy, the relationship between the offset of the metacarpophalangeal joint and the body height in the process of flexion and extension is discussed, and it is applied to the structure optimization and control system. The genetic algorithm is employed to achieve the size optimization, and the kinematics is analyzed. Finally, a prototype is built and preliminary experiments are carried out, including the range of motion and the grasping ability of the robot. The experimental results show that the robot can realize the patients' hand rehabilitation function and has certain adaptability.

Abstract:Upper limb rehabilitation robot can effectively help patients recover motor ability. Existing rehabilitation robots are usually driven by rigid motors and the mechanical structures cannot adapt to the different patients with the different physical parameters and different rehabilitation needs. This paper designs a reconfigurable upper limb rehabilitation exoskeleton for elbow and wrist joints driven by pneumatic muscle actuators (PMAs). The exoskeleton can assist patients to achieve elbow flexion/extension, wrist flexion/ extension and adduction/abduction by integrating soft elbow and wrist joint modules which can work separately or together. The wrist joint can realize two degrees of freedom (2-DoF) movement via adjustable modules. To conquer the dynamic model errors and load disturbances when reconstructing the modular joints, a non-singular fast terminal sliding mode control method based on nonlinear disturbance observer (NFTSMC-NDO) is proposed, and a position/force hierarchical control method is formed to ensure the controllability of the soft modular robot. Experimental results show that the proposed method can achieve high-precision motion control of soft modular joints and provide reconfigurable assistance for patients, improving the adaptability and compliance of rehabilitation training.

Abstract:Stroke and related complications such as hemiplegia and disability create huge burdens for human society in the 21st century, which leads to a great need for rehabilitation and daily life assistance. To address this issue, continuous efforts are devoted in human–machine interaction (HMI) technology, which aims to capture and recognize users’ intentions and fulfil their needs via physical response. Based on the physiological structure of the human hand, a dimension-adjustable linkage-driven hand exoskeleton with 10 active degrees of freedom (DoFs) and 3 passive DoFs is proposed in this study, which grants high-level synergy with the human hand. Considering the weight of the adopted linkage design, the hand exoskeleton can be mounted on the existing up-limb exoskeleton system, which greatly diminishes the burden for users. Three rehabilitation/daily life assistance modes are developed (namely, robot-in-charge, therapist-in-charge, and patient-in-charge modes) to meet specific personal needs. To realize HMI, a thin-film force sensor matrix and Inertial Measurement Units (IMUs) are installed in both the hand exoskeleton and the corresponding controller. Outstanding sensor–machine synergy is confirmed by trigger rate evaluation, Kernel Density Estimation (KDE), and a confusion matrix. To recognize user intention, a genetic algorithm (GA) is applied to search for the optimal hyperparameters of a 1D Convolutional Neural Network (CNN), and the average intention-recognition accuracy for the eight actions/gestures examined reaches 97.1% (based on K-fold cross-validation). The hand exoskeleton system provides the possibility for people with limited exercise ability to conduct self-rehabilitation and complex daily activities.

Abstract:Physical therapy plays a crucial role in the motor recovery. Rehabilitation robots have emerged as significant advancement, enabling repetitive therapeutic interventions in both clinical and home settings. In this context, the development of a highly functional, reliable, portable, and cost-effective mechatronic systems for wrist and hand rehabilitation represents a significant step in the field. This article focused on the design and implementation of the M3Rob device, a 3-DoF wrist exoskeleton equipped with a force sensor, allowing for the execution of active therapies, recognized for their effectiveness in motor recovery. Hence, a close-loop admittance control utilizing a joint-space target trajectory as input is presented and experimentally evaluated across three distinct levels of assistance. Moreover, to address the need for rehabilitation targeting activities of daily living, the device enables the incorporation of a hand exoskeleton for simultaneously performing hand and wrist rehabilitation. Featuring a range of motion of 180° for pronation/supination, 120° for flexion/extension, and 75° for ulnar/radial deviation, in combination with joint torques spanning from 7.85 to 43.86 Nm, the device covers the required motions and forces essential for daily activities. The presented device offers a comprehensive solution for wrist and hand rehabilitation, effectively addressing critical challenges in motor recovery.

Abstract:Voluntary hand movements are usually impaired after a cerebral stroke, affecting millions of people per year worldwide. Recently, the use of hand exoskeletons for assistance and motor rehabilitation has become increasingly widespread. This study presents a novel hand exoskeleton, designed to be low cost, wearable, easily adaptable and suitable for home use. Most of the components of the exoskeleton are 3D printed, allowing for easy replication, customization and maintenance at a low cost. A strongly underactuated mechanical system allows one to synergically move the four fingers by means of a single actuator through a rigid transmission, while the thumb is kept in an adduction or abduction position. The exoskeleton’s ability to extend a typical hypertonic paretic hand of stroke patients was firstly tested using the SimScape Multibody simulation environment; this helped in the choice of a proper electric actuator. Force-myography was used instead of the standard electromyography to voluntarily control the exoskeleton with more simplicity. The user can activate the flexion/extension of the exoskeleton by a weak contraction of two antagonist muscles. A symmetrical master–slave motion strategy (i.e., the paretic hand motion is activated by the healthy hand) is also available for patients with severe muscle atrophy. An inexpensive microcontroller board was used to implement the electronic control of the exoskeleton and provide feedback to the user. The entire exoskeleton including batteries can be worn on the patient’s arm. The ability to provide a fluid and safe grip, like that of a healthy hand, was verified through kinematic analyses obtained by processing high-framerate videos. The trajectories described by the phalanges of the natural and the exoskeleton finger were compared by means of cross-correlation coefficients; a similarity of about 80% was found. The time required for both closing and opening of the hand exoskeleton was about 0.9 s. A rigid cylindric handlebar containing a load cell measured an average power grasp force of 94.61 N, enough to assist the user in performing most of the activities of daily living. The exoskeleton can be used as an aid and to promote motor function recovery during patient’s neurorehabilitation therapy.

Abstract:Rehabilitation robots are used to promote structural and functional recovery of the nervous system with repetitive, task-oriented training and have been gradually applied to clinical rehabilitation training. This paper proposes an upper limb exoskeleton rehabilitation robot system that could realize shoulder-elbow-wrist joint rehabilitation training. Firstly, a motion equivalent model was established based on the upper limb movement mechanism, the robot mechanism configuration was designed, and the optimization algorithm and spatial mechanism theory were used to optimize and analyze the structural parameters and human–machine compatibility of the robot, which will guide the design of the robot's model. Then, the robot kinematics were solved, and its maximum motion range, dexterity distribution, and daily motion trajectory were simulated. Finally, a system prototype was built to test the maximum range of robot-assisted human upper limb training by laser tracker, while the pressure of human–machine interaction during training was captured and analyzed by flexible sensors. The results show that the proposed rehabilitation robot could nearly completely cover the range of motion of upper limb joints and meet the needs of trajectory training, and the linear velocity dexterity and angular velocity dexterity in the motion space are maximum 0.55 and 0.89, and the human–machine interaction pressures during the training process are all less than 10 kPa. Besides, this paper also conducted a system evaluation based on the fuzzy comprehensive evaluation model, and the evaluation result was 0.39, with an excellent evaluation grade, it indirectly indicates that the robot's overall performance was good.
