Abstract:OBJECTIVE:To detect the histological changes and the expression of proliferating cell nuclear antigen (PCNA) in junctional epithelialization of dental implants.METHODS:The junctional epithelium was detected by HE staining and the PCNA expression was observed by immunohistochemical staining on the st, 3rd, 6th, 9th, 12th, 15th, 20th and 30th day after implantation in Beagle dogs.RESULTS:The junctional epithelium began to form and PCNA was expressed positively 12 days after implantation. The epithelial hyperplasia tended to stabilization and the expression of PCNA weakened drastically 15 days after implantation. The epithelial attachment showed to be similar to that of natural teeth 20 days later.CONCLUSION:The gingiva epithelium can attach to titanium implants forming junctional epithelium which is similar to the natural teeth.
Flap vs. "flapless" surgical approach at immediate implants: a histomorphometric study in dogs

Abstract:Aim: To compare the remodeling of the alveolar process at implants installed immediately into extraction sockets by applying a flap or a "flapless" surgical approach in a dog model. Material and methods: Implants were installed immediately into the distal alveoli of the second mandibular premolars of six Labrador dogs. In one side of the mandible, a full-thickness mucoperiosteal flap was elevated (control site), while contra-laterally, the mucosa was gently dislocated, but not elevated (test site) to disclose the alveolar crest. After 4 months of healing, the animals were sacrificed, ground sections were obtained and a histomorphometric analysis was performed. Results: After 4 months of healing, all implants were integrated (n=6). Both at the test and at the control sites, bone resorption occurred with similar outcomes. The buccal bony crest resorption was 1.7 and 1.5 mm at the control and the test sites, respectively. Conclusions: "Flapless" implant placement into extraction sockets did not result in the prevention of alveolar bone resorption and did not affect the dimensional changes of the alveolar process following tooth extraction when compared with the usual placement of implants raising mucoperiosteal flaps.

Abstract:Aim To test whether early implant placement into the extraction socket containing an uncalcified provisional matrix leads to successful osseointegration and stable marginal bone levels. Materials and Methods In six mongrel dogs, the mandibular molars were extracted. Three weeks later, early implant placement was performed according to three experimental protocols: (i) flapless implant placement with preservation of the provisional matrix; (ii) flap elevation, socket debridement and implant placement; and (iii) flap elevation, socket debridement, implant placement and guided bone regeneration (GBR). One untreated extraction socket served as a control group. Data analyses were based on histologic slides 3 months after implant placement. Results There were no differences in bone‐to‐implant contact between the three experimental groups (66.97%, 58.89% and 60.89%, respectively) (inter‐group comparison p = .42). Marginal bone levels, first bone‐to‐implant contact as well as the thickness of the connective tissue did not reveal any significant differences between the groups (p = .85, .60 and .65, respectively). Conclusions Flapless early implant placement into posterior extraction sockets was as effective as an open flap approach in conjunction with GBR. Mineralization of the socket seems to occur irrespective of the presence of dental implants or biomaterials.

Use Of A Perforated Scaffold-Retaining Abutment To Achieve Vertical Bone Regeneration Around Dental Implants In The Minipig

Abstract:Objective: To evaluate the effect of perforated scaffold retainers used in conjunction with dental implants and osteoinductive scaffolds to regenerate vertical supracrestal alveolar bone in an intraoral minipig model. Materials and Methods: Three months after extraction of mandibular premolars and first molars from six adult minipigs, two titanium (Ti) custom implants were placed bilaterally in the edentulous mandibles for a total of four implants per animal. The upper 2.5 mm of the implant was left above bone level and covered with: (1) wide-neck healing caps; (2) perforated, overhanging custom scaffold retainers (umbrellas); or (3) scaffold retainers and demineralized minipig bone allograft (DBM) and nonglycosylated bone morphogenetic protein 2 (ng/rhBMP-2)-treated implants. All constructs were submerged beneath soft tissue flaps for 8 weeks. Two dental implant surfaces were compared: SLA and SLActive. Samples were retrieved after 8 weeks and analyzed by radiography, micro-computed tomography and histomorphometry. Results: All implants were stable at the end of the experiment. Histomorphometry revealed that the use of the scaffold-retaining umbrellas led to increased, but not statistically significant, vertical bone regeneration as compared to the use of wide-neck healing caps (1.0 +/- 0.4 mm vs 0.6 +/- 0.3 mm). The combination of DBM and ng/rhBMP-2 released from the surface of the SLA implant resulted in the greatest amount of vertical bone regeneration (2.1 +/- 0.2 mm). The bone-to-implant contact was similar for all groups. Mucosal dehiscence areas with healing cap or custom scaffold retainer exposures were reduced in the presence of ng/rhBMP-2. Conclusions: The combined use of custom perforated Ti scaffold retainers, DBM, and ng/rhBMP-2 regenerated a substantial amount of vertical supracrestal alveolar bone around Ti implants in an intraoral minipig model.
Histologic evaluation of implants following flapless and flapped surgery: a study in canines

Abstract:Background: Flapless surgery requires penetration of the alveolar mucosa and bone without reflection of mucoperiosteal flaps. Do these techniques force gingival tissue or foreign materials into osteotomies? If so, do such tissues or materials interfere with osseointegration? A proof-of-principle study using a canine model attempted to answer these questions. Methods: Five young adult Hound Labrador mongrel dogs received implants with a moderately roughened surface by anodic oxidation using flapless or conventional one-stage (control) surgery in contralateral jaw quadrants. The implants were placed into the osteotomies, and the international stability quotient (ISQ) was recorded using resonance frequency analysis. These measurements were repeated following a 3-month healing interval when the animals were euthanized, and implants and surrounding tissues were retrieved and processed for histologic analysis. Results: The implants were stable upon insertion and demonstrated increased stability at 3 months without significant differences between surgical protocols. The histologic evaluation showed high bone-implant contact (flapless surgery: 54.7% +/- 8.4%; control: 52.2% +/- 13.0%; P >0.05) without evidence of gingival tissue or foreign body inclusions. There were no significant differences in marginal bone levels between the surgical protocols. Post-insertion and at 3 months, ISQ values depended on the amount of torque delivered. Immediately post-insertion, for every 1-unit increase in torque value, the ISQ increased by 0.3 (95% confidence interval: 0.1 to 0.4; P = 0.0043). Three months postoperatively, for every one-unit increase in torque the ISQ value decreased 0.2 (95% confidence interval: -0.4 to -0.1; P = 0.0012). The effect of torque on ISQ values was independent of treatment effects and remained significant after adjustment for treatment. Conclusions: The results suggest that implants placed without flap reflection remain stable and exhibit clinically relevant osseointegration similar to when implants are placed with flapped procedures. Greater torque at implant placement resulted in less implant stability at 3 months.

Abstract:Purpose: To histologically examine early bone formation around transmucosal implants and to evaluate the influence of surface characteristics on early peri-implant bone healing using a miniature pig model. For this, commercially available dental implants with a rough zirconia (YTZP) surface were compared to surface-modified Ti control implants at 4 and 8 weeks after placement. Methods: Immediately following the extraction of six mandibular premolars, 20 two-piece, tissue-level, screw-shaped YTZP implants (Patent™ Standard Zirconia Implant ø4.1 × 11 mm) with a modified rough blasted before sintering surface were inserted in four adult miniature pigs. In addition, four titanium (Ti) tissue-level implants (Straumann® Standard RN ø4.1 × 10 mm Roxolid®) with a moderate surface (SLActive®), one per animal, were placed as control implants. A histological analysis was performed on the hard tissues after 4 and 8 weeks of transmucosal healing. Results: The results show a high rate of osseointegration of the test YTZP dental implants at 4 and 8 weeks following insertion. At 4 weeks, a bone-to-implant contact ratio (BIC) of 73.7% (SD ± 16.8) for the test implants (n = 10) and 58.5% for the first control implant was achieved. The second control implant had to be excluded from analysis. At 8 weeks, a BIC of 82.4% (SD ± 16.9) for the test implants (n = 9) and 93.6% (SD ± 9.1) (n = 2) for the control implant was achieved. No statistical difference was observed comparing 4 and 8 weeks YTZP data (p = 0.126). Conclusions: The results indicate a predictable osseointegration of immediate zirconia implants with a modified YTZP implant surface and a high degree of BIC present at 4 weeks following insertion. After 8 weeks of healing both the zirconia implants and the Ti implants show a BIC indicating full osseointegration. Further studies involving a larger sample size with more time points are needed to confirm these results.

Abstract:SecA has been suggested to cycle on and off the cytoplasmic membrane of Escherichia coli during protein translocation. We have reconstituted 35S-SecA onto SecA-depleted membrane vesicles and followed the fate of the membrane-associated 35S-SecA during protein translocation. Some 35S-SecA was released from the membranes in a translocation-independent manner. However, a significant fraction of 35S-SecA remained on the membranes even after incubation with excess SecA. This fraction of 35S-SecA was shown to be integrated into the membrane and was active in protein translocation, indicating that SecA cycling on and off membrane is not required for protein translocation. Proteolysis experiments did not support the model of SecA insertion and deinsertion during protein translocation; instead, a major 48-kDa domain was found persistently embedded in the membrane regardless of translocation status. Thus, in addition to catalyzing ATP hydrolysis, certain domains of SecA probably play an important structural role in the translocation machinery, perhaps forming part of the protein-conducting channels.
Molecular Analysis of Peri-implant Soft Tissue Response to Different Abutment Materials in Humans. A Pilot Study

Abstract:Purpose: To evaluate the response of human peri-implant soft tissue (PIST) on different healing abutment materials 24 hours after positioning, by assessing the expression of genes related to the early connective tissue wound healing response. Materials and methods: Experimental abutments of 4 different materials (A): grade 4 titanium, (B) grade 5 titanium, (C) zirconia and (D) PEEK, were mounted on installed implants in 5 patients, four different abutments each. Before implant placement, a gingival biopsy (control-CT) was obtained using a 2 mm diameter punch (T0). After 24 hours (T24), PIST biopsies were collected using a specifically designed custom-made cutting device. Real-time PCR was performed to analyze the expression of the following genes: COL-I, COL-III, MMP-1, TIMP-1,TGF-b1, FN, ITGA4, ITGA5, ITGB1, RAC-1, COL-IV, aSMA, IL-6 and CXCL-1. Results: Gene expression analysis showed some differences between CT and abutment of different materials, although no significant differences were detected comparing the experimental groups. COL-I was significantly down-regulated in groups A and C compared to CT. MMP-1 and TIMP-1 increased in all the experimental groups, although at a lower extent in group A. FN, RAC-1, COL-IV and aSMA were down-regulated, especially in group A, in which CXCL-1 and IL-6 showed the lowest expression. Conclusions: The results of grade 4 titanium and zirconia abutments seem to be promising, since a lower expression of genes related with inflammation, myofibroblasts activation and ECM remodeling was observed when compared with grade 5 titanium and PEEK, without triggering a pro-fibrotic response in the early phases of PIST repair.

Abstract:Background: The present study is designed as a proof-of-concept study to evaluate light/chemical hardening technology and a newly formulated polymethylmethacrylate, polyhydroxyethylmethacrylate, and calcium hydroxide (PPCH) plus polyanhydride (PA) (PPCH-PA) composite graft material as a bone substitute compared to positive and negative controls in a minipig model. Methods: PPCH-PA (composite graft); PPCH alone (positive control), PA alone (positive control), and no graft (negative control) were compared. Four mandibular premolar teeth per quadrant were extracted; a total of 48 implants were placed into sockets in three minipigs. Abutments were placed protruding into the oral cavity 4 mm in height for immediate loading. Crestal areas and intrabony spaces were filled with PPCH-PA, PPCH, or PA using a three-phase delivery system in which all graft materials were hardened by a light cure. In the negative control group, implant sites were left untreated. At 12 weeks, block sections containing implants were obtained. Evaluations included periodontal probing, pullout-force load, and stability measurements to determine implant stability, radiographs to examine bone levels, and scanning electron microscopy (SEM)-energy-dispersed spectroscopy to determine bone-to-implant contact. Results: Probing measurements did not reveal any pathologic pocket formation or bone loss. Radiographs revealed that immediate implant placement and loading resulted in bone at or slightly apical to the first thread of the implant in all groups at 12 weeks. Stability test values showed a relative clinical stability for all implants (range: -7 to +1); however, implants augmented with PPCH-PA exhibited a statistically significantly greater stability compared to all other groups (P <0.05). The newly formed bone in PPCH-PA-treated sites was well organized with less marrow spaces and well-distributed osteocytes. SEM revealed a tighter implant-socket interface in the PPCH-PA group compared to other groups with reduced microfissures and implant-bone interface fractures during pullout testing, whereas implants treated with PA or no graft showed ≈ 10-μm microfissures between the implant and bone with fractures of the intrathread bone. Conclusions: The newly formulated chemically hardened graft material PPCH-PA was useful in immediate implant placement after tooth extraction and resulted in greater stability and a well-organized implant-bone interface with immediate loading, especially in those areas where cancellous bone was present. The results of this proof-of-concept study warranted further research investigating different healing times and longer durations.

Abstract:Objectives This controlled preclinical study analyzed the effect of implant surface characteristics on osseointegration and crestal bone formation in a grafted dehiscence defect minipig model. Material and Methods A standardized 3 mm × 3 mm acute‐type buccal dehiscence minipig model grafted with deproteinized bovine bone mineral and covered with a porcine collagen membrane after 2 and 8 weeks of healing was utilized. Crestal bone formation was analyzed histologically and histomorphometrically to compare three implant groups: (1) a novel, commercially available, gradient anodized (NGA) implant, to two custom‐made geometric replicas of implant "1," (2) a superhydrophilic micro‐rough large‐grit sandblasted and acid‐etched surface, and (3) a relatively hydrophobic micro‐rough large‐grit sandblasted and acid‐etched surface. Abstract:Aim To qualitatively and quantitatively evaluate the formation and maturation of peri‐implant soft tissues around 'immediate' and 'delayed' implants. Materials and Methods Miniaturized titanium implants were placed in either maxillary first molar (mxM1) fresh extraction sockets or healed mxM1 sites in mice. Peri‐implant soft tissues were evaluated at multiple timepoints to assess the molecular mechanisms of attachment and the efficacy of the soft tissue as a barrier. A healthy junctional epithelium (JE) served as positive control. Results No differences were observed in the rate of soft‐tissue integration of immediate versus delayed implants; however, overall, mucosal integration took at least twice as long as osseointegration in this model. Qualitative assessment of Vimentin expression over the time course of soft‐tissue integration indicated an initially disorganized peri‐implant connective tissue envelope that gradually matured with time. Quantitative analyses showed significantly less total collagen in peri‐implant connective tissues compared to connective tissue around teeth around implants. Quantitative analyses also showed a gradual increase in expression of hemidesmosomal attachment proteins in the peri‐implant epithelium (PIE), which was accompanied by a significant inflammatory marker reduction. Conclusions Within the timeframe examined, quantitative analyses showed that connective tissue maturation never reached that observed around teeth. Hemidesmosomal attachment protein expression levels were also significantly reduced compared to those in an intact JE, although quantitative analyses indicated that macrophage density in the peri‐implant environment was reduced over time, suggesting an improvement in PIE barrier functions. Perhaps most unexpectedly, maturation of the peri‐implant soft tissues was a significantly slower process than osseointegration.

Marginal bone stability using 3 different flap approaches for alveolar split expansion for dental implants: a 1-year clinical study

Abstract:Purpose: Width augmentation for the alveolar process using alveolar split procedures has not been studied in a comparative study with regard to marginal bone stability. Most research in this regard has used implant osseointegration as an endpoint for the success of the bone grafting procedure. The purpose of the present clinical trial was to retrospectively evaluate the stability of the buccal crestal bone around dental implants placed into alveolar split graft sites using 3 different approaches: a minimal flap, a partial-thickness flap, and a full mucoperiosteal flap. Implants were placed simultaneously or delayed to alveolar split grafting. The aim of the present study was to do a comparative analysis of these 3 techniques by clinical detection for the presence or absence of marginal bone using a periodontal probe 1 year after implant restoration. The working hypothesis was that detachment or disturbance of the blood supply of the buccal plate would be influenced by the flap procedure used and would therefore affect the late marginal bone stability around the implants. Materials and methods: A total of 40 consecutive patients were treated in 2 different practice locations (20 at each office) with alveolar split procedures and simultaneous implant placement, using 3 different flap approaches and were seen again after 1 year at the 2 private office locations. The patients were in general good health, without active periodontal disease. All patients were nonsmokers. Patients with diabetes mellitus were excluded from the study. The aim of the present study was to establish the stability of the mobilized buccal bone plate, which could only be observed indirectly. The method used to detect the presence or absence of buccal marginal bone was a blunt periodontal probe used in the sulcus on the facial surface of the restored implant or a sharp explorer used transgingivally to detect marginal bone presence. However, the bone thickness could not be established using either of these methods. This was done in both practice locations by the surgeon who had performed the procedure (in a few cases, open flap procedures were done up to 1 year after grafting, which permitted direct observation of the original treatment site). The 3 different flap approaches studied were full-thickness flap reflection, partial-thickness flap reflection, and minimal flap reflection of the osteoperiosteal flap approach to crestally split and then widen the alveolus. Results: A total of 40 consecutive patients treated with 65 alveolar split expansion procedures done in 2 practice locations during a 2-year period were statistically analyzed retrospectively for buccal bone augmentation presence and implant restorability after 1 year of healing. Facial bone loss of 2 mm or more was seen in 11 sites, 10 of which were full flap reflections and 1 an osteoperiosteal flap site. Implant osseointegration was 92.5% for the osteoperiosteal flaps, 93.3% for the partial-thickness flaps, and 94.4% for the full-thickness flaps. Conclusions: The 3 flap approaches to alveolar widening by crest splitting with implant placement had a sustained increased alveolar width after 1 year. However, most full flap alveolar split cases had facial bone loss and gingival recession. The osteoperiosteal flaps (book flap) and partial-thickness flaps showed stable buccal bone patterns. The results of the present clinical study of relatively early osseous remodeling suggest that full mucoperiosteal flaps should not be reflected when an alveolar split is done. However, to further elucidate the marginal bone vitality, a longer study period must be undertaken to more fully validate the alveolar split procedure and verify the best flap approach.

Abstract:The role of nonchylomicron very low density lipoproteins (VLDL, S(f) 20-400) in the transport of triglyceride and cholesterol was studied during lipid absorption. Various long chain fatty acids were infused intraduodenally in the form of mixed fatty acid-mono-olein-taurocholate micelles; control animals received saline or taurocholate. As compared with controls, all fatty acids (palmitic, oleic, linoleic) resulted in significant increases in chylomicron (S(f) > 400) triglyceride. In addition, palmitic acid resulted in a twofold increase in VLDL triglyceride, whereas with the absorption of oleic or linoleic acid VLDL triglyceride did not change significantly. Differences in triglyceride fatty acid composition between chylomicrons and VLDL were observed during lipid absorption. Although the absolute amount of endogenous cholesterol in intestinal lymph was not significantly affected by lipid absorption under these conditions, its lipoprotein distribution differed substantially among the lipid-infused groups. During palmitate absorption, VLDL cholesterol was similar to that in the taurocholate-infused controls, and was equal to chylomicron cholesterol. In contrast, during oleate and linoleate absorption the VLDL cholesterol fell markedly, and was less than half of the chylomicron cholesterol in these groups. The half-time of plasma survival of VLDL cholesterol-(14)C was found to be twice that of chylomicron cholesterol-(14)C. These studies demonstrate that dietary long chain fatty acids differ significantly in their effects upon the transport of triglyceride and cholesterol by lipoproteins of rat intestinal lymph. These findings, together with the observed differences in rates of removal of chylomicrons and VLDL from plasma, suggest that variations in lipoprotein production at the intestinal level may be reflected in differences in the subsequent metabolism of absorbed dietary and endogenous lipids.
Kinetic disposition of flunixin meglumine in the camel (Camelus dromedarius).

Abstract:The kinetic disposition of flunixin meglumine was investigated in 5 camels after a single intravenous administration (1.1 mg/kg bwt). The plasma clearance was 1.012 ml.min-1.kg-1, the steady-state volume of distribution 0.168 l/kg and the elimination half-life 2.51 h. The results are discussed and compared with those reported in cattle and horses.
Impact of labially inclined implant axes on immediate implant placement and provisionalization in anterior maxilla: A prospective cohort study

Abstract:Objectives: To investigate whether a labially inclined implant axis compromises the clinical outcomes of immediate implant placement and provisionalization (IIPP) in the anterior maxilla. Materials and methods: Patients with unsalvageable central or lateral maxillary incisors were enrolled. IIPP with simultaneous connective tissue graft (CTG) was performed in all participants. In the control group, the alveolar ridge had a long axis aligned with the tooth, which ensured that the immediate implant was aimed at the incisor edge or the cingulum of future restoration. The test group had a large angle between the axes of the ridge and tooth. To avoid bone fenestration, the implants were placed labially inclined and emerged from the labial side of future restoration. Intra-oral scanning and cone-beam computed tomography were performed to record soft and hard tissue profiles at baseline and 1 year later. Soft tissue stability, bone remodeling, and pink esthetic score (PES) were evaluated and compared between two groups. Results: Thirty-nine participants (19 tests and 20 controls) completed the study. At 1-year post-surgery, the mid-facial gingival margin migrations were 0.85 ± 0.37 mm (test) and 0.81 ± 0.33 mm (control), without significant differences. No differences were identified in buccal profile alteration, linear ridge reduction, buccal bone thickness, or PES scores. The test group demonstrated thinner buccal soft tissue at the crestal level than the control group. Conclusions: When large tooth-ridge angulation presented, labially inclined implant, avoiding buccal ridge fenestration in IIPP with CTG, did not compromise the clinical outcome in short term.

Abstract:Exercise induced asthma (EIA) was described as long as 2000 years ago in Cappadocia and is a well recognised characteristic of asthma, especially in childhood and adolescence (l), indeed it is a major distinctive feature of childhood asthma, especially without anti-inflammatory treatment. It has been maintained that 70-80 per cent of untreated asthmatics suffer from EIA (2). Backer and Ulrich reported that 16 per cent of a random sample of 494 Danish children and adolescents experienced a more than ten per cent reduction in FEVl after a standardised exercise test (3). EIA may cause limitations in the daily life for asthmatic children, and it has been documented that many asthmatic children suffer such limitations (4, 5). It is important for children's self-esteem to participate in play and sports, therefore mastering EIA is considered an essential part of successful management of asthma (6). Furthermore, EIA may be regarded as a measure of bronchial hyperresponsiveness (7, S), and quantifying EIA in a standardised way may help in monitoring asthma.

Abstract:Background and Objectives: Dental implants are recognized as an effective treatment in the management of edentulous patients; controversies surround the connection between the sufficiency of keratinized gingiva (KG) and peri-implant health. Maintaining an ample amount of peri-implant KG is crucial for minimizing gingival inflammation, highlighting the need for regular consideration of soft-tissue augmentation. Among the diverse periodontal plastic surgical procedures, the apically positioned flap (APF) is notable for its ability to enhance the width of keratinized tissue while minimizing patient morbidity. The aim of this study was to evaluate the effects of L-PRF on palatal wound healing and patient discomfort after surgery. Materials and Methods: Twenty patients with two adjacent submerged fixtures in the maxilla and buccal keratinized gingiva widths < 2 mm were treated with APF and L-PRF. Clinical evaluations were performed at 1, 2, 3, and 4 weeks post-surgery, focusing on parameters such as complete wound epithelialization (CWE), postoperative discomfort (D), changes in feeding habits (CFH), alteration of sensitivity (AS) around the wound area, and the consumption of analgesics. Results: Our data revealed CWE in 5 patients by the end of the second week, with the remaining 15 achieving CWE by the end of the third week. For D and CHF, a statistically significant improvement was recorded for all cases between the first and second weeks, as well as AS, although less substantial, by the third week. No significant changes were noted for AS over the initial two weeks. Conclusions: These findings suggest that L-PRF may enhance wound healing and decrease patient discomfort following APF for fixture uncovering.

Alveolar regeneration of the post-extraction site with cortical deficiency using the Lamina Socket Sealing technique: A retrospective study with clinical, radiographic and histomorphometric analysis. Part 1/2

Abstract:Purpose: This retrospective observational study aims to evaluate the outcome of a new alveolar ridge preservation (ARP) technique, "Lamina Socket Sealing" (LSS) technique, using a porcine barrier. Methods: 36 subjects with maxillary premolars to be extracted and extensive alveolar wall defects were enrolled and treated. Porcine-derived barriers and mesenchymal membrane were used to seal the extraction socket with alveolar particulate graft. The outcome variables were: radiographical bone changes and clinical outcomes. Results: 36 subjects, 21 females and 15 males were treated. The change in mm (mean + SE) of horizontal ridge width from baseline to 4 months after surgery at the coronal level of the alveolus was +1.25 ± 0.20 mm (P< 0.05), mean level was +0.34 ± 0.1 mm (P< 0.05) and at the apical level was +0.08 ± 0.17 mm (P> 0.05) which was not statistically significant. The buccal and palatal height changes in mm (mean + SE) from baseline to 4 months after surgery were respectively 1.21 ± 0.17 mm for the buccal wall (P< 0.05) and 1.06 ± 0.24 mm for the palatal wall (P< 0.05). The bone volume of all sites allowed for the placement of the planned implants. Clinical significance: The alveolar ridge preservation technique (Lamina Socket Sealing) using resorbable heterologous cortical lamina with flapless approach allowed not only the preservation of the damaged post-extraction socket, but also minimal ridge augmentation, according to the principles of guided bone regeneration.

Abstract:ObjectivesTo compare two ridge preservation techniques and spontaneous healing in terms of hard and soft tissue changes 2 months after tooth extraction. Material and MethodsThe study was designed as a randomized controlled trial and included 75 patients. After single tooth extraction in the maxillary incisor/premolar area, patients were randomly allocated to one of the following groups: 1) ridge preservation with a xenogeneic bone substitute covered with a collagen matrix (CM‐group), 2) ridge preservation with a xenogeneic bone substitute covered with a free palatal graft (PG‐group) or 3) spontaneous healing (control). Eight weeks after tooth extraction, implants were placed and clinical, profilometric and radiographic evaluations were performed. In addition, the need for further guided bone regeneration (GBR) at implant placement was assessed. The differences between the treatment groups were compared with the One‐way ANOVA or Kruskal‐Wallis test with the corresponding post hoc analysis. The proportions of the categorical parameters were compared with the Fisher´s exact test. ResultsSeventy‐five patients underwent early implant placement 8 weeks after tooth extraction and were evaluated. CM‐group (‐0.9 SD 0.6 mm) and PG‐group (‐1.0 SD 0.8 mm) showed less horizontal bone resorption compared to the control group (‐3.2 SD 2.1 mm) (p<.001). Moreover, the necessity of GBR at implant placement was significantly less in CM‐group (32%) and PG‐group (24%) when compared to control group (72%) (p=.001). Patients in CM‐group experienced less pain than PG‐group, one week after tooth extraction (p=.042). No significant differences were found regarding graft evaluation, post‐operative complications and soft tissue contour. ConclusionsRidge preservation using a xenogeneic bone substitute covered with a collagen matrix or a palatal graft, results in less bone resorption and fewer GBR procedures at early implant placement compared to spontaneous healing.
