Abstract:In this article we investigate the possibility of using needles, which the interventional radiologist inserts near a deep-seated tumor during an electroporation-based therapy, to characterize the electrical conductivity of patient's tissues. Specifically, we propose to exploit voltage/current measurements and imaging that are performed prior to the application of electroporation pulses. The approach is partly based on the concepts of electrical impedance tomography; however, imaging is used to build a specific geometric model and compensate for the lack of information resulting from the small number of electrodes available. 3D canonical and clinical examples, where a few electrodes surround a tumor, demonstrate the feasibility of this method: solving the inverse problem to estimate tissues conductivity converges in a few iterations. For a given error on the measurement, it is also possible to calculate the error on the estimated conductivities. The uncertainty error with clinical data is at best 5% for one of the tissues identified, due to the limitations of the clinical device used. Various improvements to clinical devices are discussed to make the conductivity estimation more accurate but also to extract more information.

Electrical impedance characterization of normal and cancerous human hepatic tissue

Abstract:The four-electrode method was used to measure the ex vivo complex electrical impedance of tissues from 14 hepatic tumors and the surrounding normal liver from six patients. Measurements were done in the frequency range 1-400 kHz. It was found that the conductivity of the tumor tissue was much higher than that of the normal liver tissue in this frequency range (from 0.14 +/- 0.06 S m(-1) versus 0.03 +/- 0.01 S m(-1) at 1 kHz to 0.25 +/- 0.06 S m(-1) versus 0.15 +/- 0.03 S m(-1) at 400 kHz). The Cole-Cole models were estimated from the experimental data and the four parameters (rho(0), rho(infinity), alpha, f(c)) were obtained using a least-squares fit algorithm. The Cole-Cole parameters for the cancerous and normal liver are 9 +/- 4 Omega m(-1), 2.2 +/- 0.7 Omega m(-1), 0.5 +/- 0.2, 140 +/- 103 kHz and 50 +/- 28 Omega m(-1), 3.2 +/- 0.6 Omega m(-1), 0.64 +/- 0.04, 10 +/- 7 kHz, respectively. These data can contribute to developing bioelectric applications for tissue diagnostics and in tissue treatment planning with electrical fields such as radiofrequency tissue ablation, electrochemotherapy and gene therapy with reversible electroporation, nanoscale pulsing and irreversible electroporation.

Abstract:Abstract Despite being among the most common medical procedures, needle insertions suffer from a high error rate. Impedance measurements using electrode‐equipped needles offer promise for improved tissue targeting and reduced errors. Impedance visualization usually requires an extensive pre‐measured impedance dataset for tissue differentiation and knowledge of the electric fields contributing to the resulting impedances. This work presents two finite element simulation approaches for both problems. The first approach describes the generation of a multitude of impedances with Monte Carlo simulations for both, homogeneous and inhomogeneous tissue to circumvent the need to rely on previously measured data. These datasets could be used for tissue discrimination. The second method describes the simulation of the spatial sensitivity distribution of an electrode layout. Two singularity analysis methods were employed to determine the bulk of the sensitivity within a finite volume, which in turn enables consistent 3D visualization. The modeled electrode layout consists of 12 electrodes radially placed around a hypodermic needle. Electrical excitation was simulated using two neighboring electrodes for current carriage and voltage pickup, which resulted in 12 distinct bipolar excitation states. Both, the impedance simulations and the respective singularity analysis methods were compared with each other. The results show that the statistical spread of impedances is highly dependent on the tissue type and its inhomogeneities. The bounded bulk of sensitivities of both methods are of similar extent and symmetry. Future models should incorporate more detailed tissue properties such as anisotropy or changing material properties due to tissue deformation to gain more accurate predictions.

Abstract:Electrochemical and electrical characteristics have the potential to help differentiate between, and assess the health state of, different biological tissues. However, measurement and interpretation of these characteristics is non-trivial. We propose a new DC galvanostatic sensing method for application to laparoscopic cancer surgery. This presents a simple and cost-effective measurement coupled with straightforward data interpretation. This paper describes the electrochemical and electrical theory underpinning the technique. Additionally, we describe a measurement system employing this technique and present an investigation into the feasibility of using it for measuring the resistance of different tissue types. Measurements were performed on ex vivo porcine liver, colon and rectum tissues. Outputs were consistent with theory and showed a significant difference between the resistance of the different tissue types, (one-way ANOVA, F(2, 28) = 1369, p <; 0.01). These findings indicate that this novel technique may be viable as a low cost method for the discrimination and health assessment of tissues in clinical scenarios.

Abstract:Electrical Impedance Spectroscopy has already demonstrated the ability to distinguish different tissues types, or tumors from normal tissue, or tissues displaying diverse degrees of pathology. When applying the technique, however, the necessity to make contact with the tissue often constitutes a practical limitation. Electrical Impedance Imaging (EIT), or in a broader sense, regional impedance assessment, struggle to assess different tissue conditions out of measurements from the surface of the body. But sensitivity is very small even for tissue a few centimeters under the skin, and in-vivo measurements are often not viable.The lung offer a third approximation by introducing a catheter though a bronchoscope, which is a routine clinical procedure. Measurements have been obtained by using 3 or 4-electrode techniques and allow us to distinguish, at least, fibrotic, emphysema or neoplastic regions from normal parenchyma. New instrumental developments, clinical measurements and preliminary results are presented and discussed.
Characterization of Nonlinearity and Dispersion in Tissue Impedance During High-Frequency Electroporation

Abstract:Objective: The use of high-voltage, high-frequency bipolar pulses (HFBPs) is an emerging electroporation-based therapy for the treatment of solid tumors. In this study, we quantify the extent of nonlinearity and dispersion during the HFBP treatment. Methods: We utilize flat-plate electrodes to capture the impedance of the porcine liver tissue during the delivery of a burst of HFBPs of widths 1 and 2 $\mu$s at different pulse amplitudes. Next, we fit the impedance data to a frequency-dependent parallel RC network to determine the conductivity and permittivity of the tissue as a function of frequency, for different applied electric fields. Finally, we present a simple model to approximate the field distribution in the tissue using the conductivity function at a frequency that could minimize the errors due to approximation with a nondispersive model. Results: The conductivity/permittivity of the tissue was plotted as a function of frequency for different electric fields. It was found that the extent of dispersion reduces with higher applied electric field magnitudes. Conclusion: This is the first study to quantify dispersion and nonlinearity in the tissue during the HFBP treatment. The data have been used to predict the field distribution in a numerical model of the liver tissue utilizing two needle electrodes. Significance: The data and technique developed in this study to monitor the electrical properties of tissue during treatment can be used to generate treatment-planning models for future high-frequency electroporation therapies as well as provide insights regarding treatment effect.

Abstract:In recent years, the structural analysis of tissue elements has gained significant importance in biomedical research. Advancements in imaging technologies have created a pressing need to quantify tissue and cell properties accurately. This paper introduces a MATLAB-based analytical tool designed to measure a spectrum of properties from 2D images of tissues and cells. Our software efficiently computes parameters such as eccentricity, orientation, density, co-localization, size, and perimeter. The algorithm’s precision in evaluating these characteristics has broad implications for enhancing the understanding of various biological processes and diseases. The code’s flexibility allows for application across different tissue types and experimental conditions, providing researchers with a robust method for quantitative analysis. Abstract:Dielectric characterization is extremely promising in medical contexts because it offers insights into the electromagnetic properties of biological tissues for the diagnosis of tumor diseases. This study introduces a promising approach to improve accuracy in the dielectric characterization of millimeter-sized biopsies based on the use of a customized electromagnetic characterization system by adopting a coated open-ended coaxial probe. Our approach aims to accelerate biopsy analysis without sample manipulation. Through comprehensive numerical simulations and experiments, we evaluated the effectiveness of a metal-coating system in comparison to a dielectric coating with the aim for replicating a real scenario: the use of a needle biopsy core with the tissue inside. The numerical analyses highlighted a substantial improvement in the reconstruction of the dielectric properties, particularly in managing the electric field distribution and mitigating fringing field effects. Experimental validation using bovine liver samples revealed highly accurate measurements, particularly in the real part of the permittivity, showing errors lower than 1% compared to the existing literature data. These results represent a significant advancement for the dielectric characterization of biopsy specimens in a rapid, precise, and non-invasive manner. This study underscores the robustness and reliability of our innovative approach, demonstrating the convergence of numerical analyses and empirical validation.

Abstract:Regular electrical impedance tomography (EIT) is based on the surface electrodes which results in long application times, long stabilization times, and high ESEI. Clinically relevant information in electrical impedance tomography is diluted by the electrical properties of stratum corneum. Eliminating stratum corneum would improve the signals from the tissue. In this paper, a new concept and possible technology - minimally invasive electrical impedance tomography based on micromachined needle electrodes is described to decrease diagnostics uncertainty. Microneedle electrodes are penetrated into the skin, and compared to regular EIT, can obtained minimal ESEI required. Otherwise, the object of tomography would be extended to the local body or the micro tissue, such as a cell, to realize micro imaging. Abstract:Monitoring bone healing after osteosynthesis in the lower limb provides insight into the need for additional medical intervention. Electrical impedance spectroscopy (EIS), via an implantable device, has recently been studied as a remote and radiation-free surveillance method for fracture patients. To address ambiguities in conventional EIS characterizations of physiological and pathological features, this article presents a machine learning (ML)-assisted EIS interpretation method, tailored specifically for cases where an intramedullary nail (IM nail) is applied. Building on the equivalent circuit model method, we propose modeling the electrical properties of the tissue under test (TUT) using generic distributed elements. A convolutional neural network (CNN) is then deployed to prefit the measured EIS with the aim of identifying the dominating elements, addressing the issue of initial guesswork, and revealing their physiological meaning. This method is demonstrated through the analysis of in vivo EIS measurements of the rabbit tibia postsurgery. By aligning fitting curves to closely match the measured spectra, the relevant components in the equivalent circuit model can be consistently identified, and their local correlation with the bone states is characterized. Our method exhibits potential for the quantitative analysis of EIS and could pave the way for its future research and application.

Electrical Characterization of Human Biological Tissue for Irreversible Electroporation Treatments

Abstract:Irreversible electroporation (IRE) is a cancer therapy that uses short, high-voltage electrical pulses to treat tumors. Due to its predominantly non-thermal mechanism and ability to ablate unresectable tumors, IRE has gained popularity in clinical treatments of both liver and pancreatic cancers. Existing computational models use electrical properties of animal tissue that are quantified a priori to predict the area of treatment in three dimensions. However, the changes in the electrical properties of human tissue during IRE treatment are so far unexplored. This work aims to improve models by characterizing the dynamic electrical behavior of human liver and pancreatic tissue. Fresh patient samples of each tissue type, both normal and tumor, were collected and IRE pulses were applied between two parallel metal plates at various voltages. The electrical conductivity was determined from the resistance using simple relations applicable to cylindrical samples. The results indicate that the percent change in conductivity during IRE treatments varies significantly with increasing electric field magnitudes. This percent change versus applied electric field behavior can be fit to a sigmoidal curve, as proposed in prior studies. The generic conductivity data from human patients from this work can be input to computational software using patient-specific geometry, giving clinicians a more accurate and personalized prediction of a given IRE treatment.

Abstract:Molecular medicine involves the introduction of macromolecules, such as drugs or gene constructs, into specific cells of the body. Electroporation, which uses electric pulses to permeate cell membranes, is a method for achieving this. However, as with other molecular medicine procedures, it lacks a real-time mechanism to detect and control which cells have been affected. We propose and demonstrate, via computer simulation, that electrical impedance tomography has the potential for detecting and imaging electroporation of cells in tissue in real-time, thereby providing feedback for controlling electroporation.

Abstract:The present study proposed competitive adaptive reweighted sampling (CARS) algorithm to be used to select the key variables from near-infrared hyperspectral imaging data of "Ya" pear. The performance of the developed model was evaluated in terms of the coefficient of determination(r2), and the root mean square error of prediction (RMSEP) and the ratio (RPD) of standard deviation of the validation set to standard error of prediction were used to evaluate the performance of proposed model in the prediction process. The selected key variables were used to build the PLS model, called CARS-PLS model. Comparing results obtained from CARS-PLS model and results obtained from full spectra PLS, it was found that the better results (r(2)pre = 0. 908 2, RMSEP=0. 312 0 and RPD=3. 300 5) were obtained by CARS-PLS model based on only 15. 6% information of full spectra. Moreover, performance of CARS-PLS model was also compared with PLS models built by using variables got by Monte Carlo-uninformative variable elimination (MC-UVE) and genetic algorithms (GA) method. The result found that CARS variable selection algorithm not only can remove the uninformative variables in spectra, but also can reduce the collinear variables from informative variables. Therefore, this method can be used to select the key variables of near-infrared hyperspectral imaging data. This study showed that near-infrared hyperspectral imaging technology combined with CARS-PLS model can quantitatively predict the soluble solids content (SSC) in "Ya" pear. The results presented from this study can provide a reference for predicting other fruits quality by using the near-infrared hyperspectral imaging.

Abstract:Electrical impedance spectroscopy (EIS) has been proposed as a promising noninvasive method to differentiate healthy thyroid from parathyroid tissues during thyroidectomy. However, previously reported similarities in the in vivo measured spectra of these tissues during a pilot study suggest that this separation may not be straightforward. We utilise computational modelling as a method to elucidate the distinguishing characteristics in the EIS signal and explore the features of the tissue that contribute to the observed electrical behaviour. Firstly, multiscale finite element models (or 'virtual tissue constructs') of thyroid and parathyroid tissues were developed and verified against in vivo tissue measurements. A global sensitivity analysis was performed to investigate the impact of physiological micro-, meso- and macroscale tissue morphological features of both tissue types on the computed macroscale EIS spectra and explore the separability of the two tissue types. Our results suggest that the presence of a surface fascia layer could obstruct tissue differentiation, but an analysis of the separability of simulated spectra without the surface fascia layer suggests that differentiation of the two tissue types should be possible if this layer is completely removed by the surgeon. Comprehensive in vivo measurements are required to fully determine the potential for EIS as a method in distinguishing between thyroid and parathyroid tissues.

Abstract:Abstract The present study determines the effect of compression over bioimpedance of healthy soft tissue ( in-vitro and in-vivo) . Electrical impedance spectroscopy (EIS) is a promising tissue characterization and tumor detection technique that uses tissue impedance or admittance to characterize tissue and identify tissue properties as well as cell structure. Variation in EIS measurements while applying pressure suggests that compression tends to affect soft tissue bioimpedance. Moreover, the displacements in tissue caused by applied compression may provide useful information about the structure and state of the tissue. Thus combining the changes to the electrical properties of tissue resulted by applied compression, with the changes in tissue displacements caused by applied compression, and consequently measuring the effect that electrical and mechanical properties have on each other, can be useful to identify tissue structure. In this study, multifrequency bioimpedance measurements were performed on in-vitro and in-vivo soft tissue at different pressure levels. Increasing compression on the in-vitro tissue results in an increase in both extracellular resistance and membrane capacitance while it causes a reduction in the intracellular resistance. However, as the compression over the in-vivo samples increases, the intracellular and extracellular resistance increase and the membrane capacitance decreases. The in-vivo measurements on human body are also tested on contra-lateral tissue sites and similar tissue impedance variation trends are observed in the contra-lateral sites of human body. The evidence from these tests suggests the possibility of using this EIS-Pressure combined measurement method to improve tumor detection in soft tissue. Based upon the observations, the authors envision developing an advanced model based upon the Cole model, which is dependent on tissue displacements.
Modeling of electric field distribution in tissues during electroporation

Abstract:Background: Electroporation based therapies and treatments (e.g. electrochemotherapy, gene electrotransfer for gene therapy and DNA vaccination, tissue ablation with irreversible electroporation and transdermal drug delivery) require a precise prediction of the therapy or treatment outcome by a personalized treatment planning procedure. Numerical modeling of local electric field distribution within electroporated tissues has become an important tool in treatment planning procedure in both clinical and experimental settings. Recent studies have reported that the uncertainties in electrical properties (i.e. electric conductivity of the treated tissues and the rate of increase in electric conductivity due to electroporation) predefined in numerical models have large effect on electroporation based therapy and treatment effectiveness. The aim of our study was to investigate whether the increase in electric conductivity of tissues needs to be taken into account when modeling tissue response to the electroporation pulses and how it affects the local electric distribution within electroporated tissues. Methods: We built 3D numerical models for single tissue (one type of tissue, e.g. liver) and composite tissue (several types of tissues, e.g. subcutaneous tumor). Our computer simulations were performed by using three different modeling approaches that are based on finite element method: inverse analysis, nonlinear parametric and sequential analysis. We compared linear (i.e. tissue conductivity is constant) model and non-linear (i.e. tissue conductivity is electric field dependent) model. By calculating goodness of fit measure we compared the results of our numerical simulations to the results of in vivo measurements. Results: The results of our study show that the nonlinear models (i.e. tissue conductivity is electric field dependent: σ(E)) fit experimental data better than linear models (i.e. tissue conductivity is constant). This was found for both single tissue and composite tissue. Our results of electric field distribution modeling in linear model of composite tissue (i.e. in the subcutaneous tumor model that do not take into account the relationship σ(E)) showed that a very high electric field (above irreversible threshold value) was concentrated only in the stratum corneum while the target tumor tissue was not successfully treated. Furthermore, the calculated volume of the target tumor tissue exposed to the electric field above reversible threshold in the subcutaneous model was zero assuming constant conductivities of each tissue.Our results also show that the inverse analysis allows for identification of both baseline tissue conductivity (i.e. conductivity of non-electroporated tissue) and tissue conductivity vs. electric field (σ(E)) of electroporated tissue. Conclusion: Our results of modeling of electric field distribution in tissues during electroporation show that the changes in electrical conductivity due to electroporation need to be taken into account when an electroporation based treatment is planned or investigated. We concluded that the model of electric field distribution that takes into account the increase in electric conductivity due to electroporation yields more precise prediction of successfully electroporated target tissue volume. The findings of our study can significantly contribute to the current development of individualized patient-specific electroporation based treatment planning.
Electromechanical Characterization of Human Brain Tissues: A Potential Biomarker for Tumor Delineation

Abstract:Objective: Accurate identification of surgical margins in brain tumors is of significant prognostic importance. Despite the availability of methods such as 5-ALA and image guidance, recognizing tumor boundary is highly subjective, dependant on recognizing subtle changes in tissue characteristics including texture and color to aid distinction. Method: Design and development of a semi-automated system integrated with MEMS-based electromechanical sensors to enable an objective and reliable method of distinguishing tumors from normal brain tissue. Simultaneous electrical impedance and viscoelastic characterization of three types of freshly excised gliomas (glioblastoma (GBM), astrocytoma (AST), and oligodendroglioma (OLI)) (N = 8 each) and seventeen different normal brain regions (N = 6 each) obtained postmortem. Results: The electrical impedance of gliomas (462±56Ω) was found to be significantly lower than corresponding normal (1267±515Ω) regions at 100kHz (p = 7.46e-11). The difference in the impedance between individual tumor types and corresponding normal regions was also statistically significant (p = 1e-8), suggesting accurate tumor delineation. There were distinct differences in the viscoelastic relaxation responses of high-grade and low-grade gliomas. White matter regions demonstrated higher impedance and faster stress relaxation compared to grey matter regions as a characteristic of their structural composition. Conclusion: We demonstrate that simultaneous electromechanical characterization of brain tumors and normal brain tissues can be an effective biomarker for tumor delineation, grading, and studying heterogeneity between the brain regions. Significance: The observations suggest the potential use of the technology in a clinical setting to achieve gross total resection and improve treatment outcomes by helping surgeons perform real-time risk evaluation during surgery.
Towards Estimating Arterial Diameter Using Bioimpedance Spectroscopy: A Computational Simulation and Tissue Phantom Analysis

Abstract:This paper improves the accuracy of quantification in the arterial diameter-dependent impedance variance by altering the electrode configuration. The finite element analysis was implemented with a 3D human wrist fragment using ANSYS Electronics Desktop, containing fat, muscle, and a blood-filled radial artery. Then, the skin layer and bones were stepwise added, helping to understand the dielectric response of multi-tissues and blood flow from 1 kHz to 1 MHz, the current distribution throughout the wrist, and the optimisation of electrode configurations for arterial pulse sensing. Moreover, a low-cost wrist phantom was fabricated, containing two components: the surrounding tissue simulant (20 wt % gelatine power and 0.017 M sodium chloride (NaCl) solution) and the blood simulant (0.08 M NaCl solution). The blood-filled artery was constricted using a desktop injection pump, and the impedance change was measured by the Multi-frequency Impedance Analyser (MFIA). The simulation revealed the promising capabilities of band electrodes to generate a more uniform current distribution than the traditional spot electrodes. Both simulation and phantom experimental results indicated that a longer spacing between current-carrying (CC) electrodes with shorter spacing between pick-up (PU) electrodes in the middle could sense a more uniform electric field, engendering a more accurate arterial diameter estimation. This work provided an improved electrode configuration for more accurate arterial diameter estimation from the numerical simulation and tissue phantom perspectives.
