Abstract:A 56-year-old female presented to the dermatology clinic with pigmentation on the lips, gums and tongue for 5 years. The amout and range of lesions gradually increased. The patient denied having digestive tract symptoms. The family history was unremarkable. Physical examination showed brown to black spotted patches on the lips, gums and buccal mucosa (Fig. A, B). Light brown stripes appear on multiple nails (Fig. C). Except for the mucocutaneous lesions, no other significant signs were found. No abnormality was detected in gastrointestinal endoscopy. The feces examination did not find red blood cells. No endocrine abnormal was found. Dermoscopy demonstrated brownish uniform pigmentation on the pale red background of the lips. Brown longitudinal stripes with uniform color were visible on the nails. Histopathology of a biopsy taken from the lip showed hyperpigmentation in the basal layer of the epidermis (Fig. 1). A diagnosis of Laugier-Hunziker (LH) syndrome was made. LH syndrome is a rare clinical syndrome, often manifested as pigmentation in the lips and buccal mucosa, hands, feet and nails. The diagnosis of the disease is exclusive. It is necessary to distinguish it from other diseases with mucocutaneous pigmentation, including Peutz- Jeghers syndrome, Addison syndrome, and melanoma. Unlike Peutz- Jeghers syndrome, it often occurs in middle age and has no polyps in the gastrointestinal tract. Peutz- Jeghers syndrome often occurs at childhood and accompanied with multiple polyps in the gastrointestinal tract, although they have similar mucocutaneous appearance. Addison's disease often manifests as diffuse skin and mucous membrane pigmentation, especially in the exposed parts of the face and limbs. Abstract:Here, we present two cases of oral pigmentation associated with prolonged chloroquine usage. The initial case involves a 48-year-old diabetic female with systemic lupus erythematosus (SLE) who reported symptoms of "dry mouth and eyes, and itchy skin." She was using glibenclamide, gabapentin, and hydroxychloroquine. During the physical examination, brownish spots were observed on the back and arm, along with a grayish-purple spot on the alveolar ridge and hard palate. The second case involves a 56-year-old female with arthritis and SLE, complaining of "dry mouth and sores." She was using gabapentin and hydroxychloroquine.

Abstract:Pigmented lesions in the oral cavity can arise from the accumulation of external substances or internal pigments, resulting in black or brown discoloration. The etiology can be categorized as physiologic, reactive, neoplastic, idiopathic, or indicative of systemic illness. Several systemic drugs have been linked to the development of oral and/or cutaneous pigmentation, either by stimulating the production of melanin or by the accumulation of the drug or its byproducts. The medications most commonly associated with this condition include antimalarials, hormones, oral contraceptives, phenothiazines, chemotherapeutics, amiodarone, minocycline, zidovudine, clofazimine, and ketoconazole. The aim of this case report is to illustrate the drug-induced appearance of multiple melanotic macules in an 89-year-old female patient. The patient was referred to the Department of Oral Medicine and Pathology, School of Dentistry, Aristotle University of Thessaloniki, Greece, complaining of the recent and constant appearance of black spots in her oral cavity. Her medical history revealed a multitude of prescribed drugs, with citalopram being the most recently prescribed one, approximately one year prior to the examination. The clinical examination revealed multiple melanotic macules, on the upper and lower lip as well as on the hard and soft palate. Based on these findings, a biopsy of a melanotic macule of the lip was carried out. The histopathological examination showed that the basal layer of the stratified squamous epithelium exhibited hyperpigmentation (melanin-pigmented basal cells). In addition, scattered melaninophages were noted in lamina propria. Psychotropic drugs associated with cutaneous hyperpigmentation include citalopram. Therefore, our case constitutes an exception since citalopram induced intraoral and perioral, instead of cutaneous, hyperpigmentation.

Abstract:Laugier-Hunziker syndrome is a hereditary condition characterized by pigmentation in mucosal, nail, and acral sites, without any other systemic manifestations. This study aimed to report two cases observed in an oral medicine service at a public dental specialty center. Case 1: A 64-year-old woman presented with multiple brown macules affecting the buccal mucosa, palate, and longitudinal melanonychia on her nails. Case 2: A 65-year-old woman exhibited multiple brown macules affecting her lips, palate, and nail pigmentation.

Abstract:A 45-year-old brown man was referred for a biopsy of asymptomatic oral lesions. He had a habit of chewing coca leaves, had been exposed to pesticides, and had a history of oral cancer. Local inspection revealed white irregular plaques with areas of erosion measuring 3x2 cm on the posterior right side of the tongue. A second lesion was identified, extending from the vestibular gingiva of tooth 47 to the vestibular gingiva of tooth 35 and involving the internal mucosa of the lower lip. The clinical appearance included white heterogeneous plaques followed by erosions and lichenoid characteristics measuring 7x2x3 cm.

Reticulate postinflammatory hyperpigmentation with band-like mucin deposition

Abstract:Background: Mucinoses of the skin are a group of disorders sharing accumulation of mucin in the skin or hair follicles. Postinflammatory hyperpigmentation, with pigmentary incontinence, is due to loss of melanin from epidermal basal cells and its accumulation in dermal macrophages. Methods: We describe clinicopathologic features of two patients presenting with the association of pigmentary incontinence with an unusual diffuse, band-like dermal deposition of mucin, clinically presenting with reticular pigmented macular lesions. Results: Two patients were observed with asymptomatic, persistent, reticular, pigmented patches located in the flexures, thighs, neck and back. Histology showed melanophages with a diffuse, band-like dermal deposition of mucin, an increased number of fibroblasts, a slight T-cell infiltrate and scattered mast cells. Blood markers of lupus erythematosus were negative. Conclusions: These findings may draw attention to pigmentary disorders such as lichen planus pigmentosus, erythema dyschromicum perstans, pigmentatio maculos eruptiva idiopathica, dermatopathia pigmentosa reticularis, prurigo pigmentosa and frictional melanosis. None of these entities, however, includes mucin deposition among its microscopic features. Macules were not preceded by erythema or any other lesions. We suggest that our cases could belong to group II of Rongioletti and Rebora's classification, i.e. they could be cases of secondary mucin deposition in postinflammatory hyperpigmentation, possibly in an unusual form of lichen planus pigmentosus or, less likely, frictional melanosis.
A Clinicopathological Study of 22 Cases of Oral Melanotic Macule

Abstract:Oral melanotic macules, or melanosis, are uncommon oral mucosal lesions. Twenty-two cases of oral melanotic macule diagnosed and treated in our institute during the period from January 1989 to June 2004 were analyzed in this study. There were 8 male and 14 female patients. The mean age of patients at the time of diagnosis was 41.2 (range, 5~80) years. The most-common sites for melanotic macule were the buccal mucosa (7 cases) and lower lip (6 cases). Multiple lesions were noted in 5 cases. The clinical appearance of the lesion was frequently a black macule (19 cases). The greatest dimension of the lesion was 1.5cm with a mean of 0.5cm. Of the 22 cases, the clinical diagnosis was oral melanosis in 12, oral melanotic macule in 6, intramucosal nevus in 2, and others in 2. Excisional biopsy was the treatment of choice in 21 cases and incisional biopsy in 1 case. Recurrence of the lesion was found in 3 cases. We found that buccal mucosal lesions had a significantly lower mean density of pigmented basal epithelial cells/mm of the basement membrane (14±26 cells/mm) than of the lower lip (89±62 cells/mm, p=0.014) or gingival lesions (76±62 cells/mm, p=0.041). In contrast, buccal mucosal lesions had a significantly higher mean density of melanophages/mm of basement membrane(52±29 cells/mm) than gingival lesions (14±2 cells/mm, p=0.031). However, there was no significant difference ii the mean density of melanophages/mm of the basement membrane between buccal mucosal lesions and lower lip lesions (28±16 cells/mm, p=0.1). We concluded that oral melanotic macules in this study occurred most often in the fifth decade. The most-commonly affected sites were the buccal mucosa and lower lip. The black pigmentation of these buccal mucosal melanotic macules might have been due to an increase in the number of melanophages in the subepithelial connective tissue, and the black pigmentation of lower lip or gingival melanotic macule might have been due to an increase in the number of melanin-pigmented basal epithelial cells.
A Case of Melanosis Duodeni Alleviated by the Discontinuation of Ferrous Sulfite

Abstract:Abstract: A case of melanosis duodeni is presented. The patient was an 80‐year‐old Japanese woman who had received oral ferrous sulfite for 19 months. Endoscopic examination of the duodenum showed marked pigmentation of the duodenal mucosa. Histological and electron microscopic examinations revealed that the pigment had histochemical features compatible with hemosiderin and was located mainly within macrophage lysosomes in the lamina propria. We discontinued the administration of ferrous sulfite and commenced follow‐up endoscopy. The pigmentation disappeared within 7 months of discontinuing treatment. Thus, the ferrous sulfite was speculated to be the causative agent of the pigmentation in the present case. The patient had concomitant hypertension, rheumatoid arthritis, anemia and chronic renal failure. The possible associations of these complications with the development of duodenal pigmentation were also considered.

Abstract:Pseudomelanosis duodeni is a rare condition characterized by the endoscopic appearance of diffuse dark pigmentation of the duodenal mucosa. It is typically seen in older women and has been reported to be associated with hypertension, chronic renal disease, diabetes mellitus, gastrointestinal hemorrhage, and the use of medications such as oral iron, furosemide, thiazide, hydralazine, and propranolol. We present a case of pseudomelanosis duodeni appearing after 2 years of oral iron therapy in an 85-year-old woman. Although oral iron supplementation seemed the strongest possible etiology, our patient had multiple comorbidities and was on other medications that have been described as associations. The majority of individuals taking oral iron or under these clinical conditions do not develop this entity; some other factors in patients may be responsible for its occurrence.
Cancer Chemotherapy-associated Pigmentation of the Oral Mucosa

Abstract:Background/aim: Oral adverse events caused by anticancer drugs are diverse, but few reports have examined pigmentation of the oral mucosa. The aim of this study was to clarify the prevalence of oral mucosal pigmentation caused by anticancer drugs. Patients and methods: This single-centre retrospective study investigated patients who underwent oral examination in our hospital during cancer chemotherapy for 3 years from April 1, 2019 to March 31, 2021. Inclusion criteria were patients who could be followed-up for ≥3 months after completing chemotherapy with drugs that caused pigmentation. The primary predictive variable was the cancer chemotherapeutic agent used. The primary outcome variable was pigmentation of the oral mucosa. Collected data were statistically analysed using the χ2 test or Fisher's exact test, with the level of significance set at p<0.05. Results: A total of 388 patients were enrolled in the study. Eleven patients (2.8%) showed oral mucosal pigmentation. Drugs causing pigmentation [deposition rate (number of patients with deposits/users)] were TS-1 (combination of tegafur, gimeracil, and oteracil potassium) [12.2% (5/41)], paclitaxel [4.0% (2/50)], gemcitabine [5.0% (1/20)], cyclophosphamide [2.3% (1/42)], carboplatin [1.6% (1/64)], fluorouracil [2.3% (1/43)], and capecitabine [3.4% (1/29)]. Conclusion: Oral pigmentation due to cancer chemotherapy was found in 2.8% of patients. TS-1, carboplatin, cyclophosphamide, capecitabine, fluorouracil, gemcitabine, and paclitaxel caused pigmentation of the oral mucosa. Among these, TS-1 was the most likely to cause pigmentation, affecting 12.2% of users.

Abstract:A 47-year-old Caucasian woman was referred due to the presence of multiple brown macular lesions on her gum that had been persisting for over a year. Her medical history included the use of mouthwash and hair loss therapy. She denied smoking and alcohol consumption but was taking escitalopram. Additionally, her sister and aunt had previously been diagnosed with pituitary adenoma. To investigate these oral lesions, the patient underwent a lip biopsy, and the histopathological diagnosis revealed fixed pigmentary erythema.

Clinically Unusual Pigmented Lesion of the Buccal Mucosa: A Case Report

Abstract:We present a rare clinical case of a 64-year-old patient with a pigmented lesion localized in the left buccal mucosa. Subjective complaints of slight pain and discomfort in the process of eating and increased sensitivity when consuming hot food were reported. According to the information provided by the patient, the lesion had progressively increased in size. A history of previous dental manipulations was reported, namely, the extraction of teeth with amalgam obturations in the left half of the maxilla as per relevant indications. During the intraoral examination, a flat, black-colored lesion, 0.4 cm in diameter, with well-defined borders was observed in the buccal mucosa. Teeth 25, 26, and 27 were previously extracted five to seven years ago. An orthopantomography was performed as a routine procedure. It did not show any presence of X-ray contrast areas that could explain the symptoms of the patient. The symptomatic nature of the lesion as well as the negative radiological findings prompted surgical treatment and excisional biopsy with subsequent histological evaluation to rule out oral malignancy. An excision was performed. During the follow-up examination in the next eight days, all the symptoms of the patient were gone. The conclusion of the pathology report was "histological findings and clinical data consistent with amalgam tattoo". The amalgam tattoo is the most frequent iatrogenic pigmented lesion of the oral mucosa, which results from the implantation of amalgam particles in the soft tissues and it is usually asymptomatic. In this case, no surgical treatment is needed. However, in some rare cases, like the one we are presenting, some symptoms can occur and complicate the diagnostic process. In these cases, the complete excision of the lesion is to be performed with subsequent histological evaluation. The atraumatic intervention of teeth, obturated with definitive amalgam fillings, is a main factor for preventing this kind of pigmentation of the oral mucosa.

The patient is a 69-year-old melanodermic woman with a medical history of high blood pressure and chronic kidney disease. She presented with complaints of skin lesions that initially developed on her arms, subsequently spreading to her body and mouth, accompanied by painful symptoms. Hyperpigmented patches were observed on her neck, dorsum, arms, and legs, and white striae were noted in her nails.

A Case of Incontinentia Pigmenti Achromians Associated with Vitiligo

Abstract:A case of incontinentia pigmenti achromians is reported. A 10-year-old girl presented with macular hypopigmentation of special patterns over the face, trunk and left leg, which appeared one year after birth. Skin examination: bizarre and patterned whitish patches were found over the face, trunk and left leg, some lesions manifested as vortex or splash-ink. Two totally white macules of coin size with sharp margin were seen on the right chest and left leg. Histological examination of hypopigmented patch showed that much melanin granule and very few melanocytes were found in the basal layer of the epidermis, no obvious inflammatory infiltrates were seen in the dermis.

Abstract:A 16-year-old girl born from a nonconsanguineous marriage presented to the dermatology out-patient department with innumerable hyperpigmented macules predominantly over the photoexposed parts of her body involving the face, neck, chest, back, of forearms, hands, and legs with a history of photosensitivity and photophobia (Figure 1). These were interspersed with multiple depigmented macules over her arms and legs since, she was three years old. She also had a small pigmented mass involving the left eye present for the last 3 months. She had a family history of similar cutaneous lesions in her elder sister. There was no history of hearing loss, seizures, spasticity, or cognitive impairment.
Oral pigmentation in physiologic conditions, post-inflammatory affections and systemic diseases

Abstract:Melanocytes are found throughout the oral mucosa but usually go unnoticed because of their relatively low level of pigment production. When focally or generally active in pigment production or proliferation they may be responsible for several affections in the oral mucosae ranging from physiologic pigmentation, systemic diseases to malignant neoplasms. The diagnosis of oral pigmentations (OP) is usually challenging for the physician, but a careful examination of the oral cavity may reveal the first manifestation of underlying systemic diseases. Therefore, a full medical history (including drug assumption and smoking) together with a general dermatological examination are mandatory and represent the first approach to OPs. When the diagnosis cannot be reached clinically with enough certainty, a biopsy for histological examination is needed, also in order to exclude possible life threatening conditions such as melanoma. Dermoscopy is another reliable diagnostic tool to make a differential diagnosis between melanocytic lesions and other conditions and then to manage the follow-up of patients. Few papers on the subject have been published in the dermatological literature and the oral cavity is often poorly investigated during routine dermatological examinations. We therefore decided to perform a review of benign OPs, classifying them into diffuse (physiological/racial pigmentations, smoker's melanosis, drug-induced hyperpigmentation, post-inflammatory hyperpigmentation, black hairy tongue, OPs associated to systemic diseases) and localized (amalgam tattoo, melanocytic nevi, melanoacanthoma, melanosis) lesions.
Prevalence and clinical features of pigmented oral lesions

Abstract:Background: To examine the relative prevalence, types, and clinical features of pigmented lesions of the oral mucosa in 1275 patients attending a university hospital for dental care. Methods: Patients attending dental clinics at The University of Jordan Hospital over a 1-year period were examined for the presence of oral pigmentations. Histopathological examination was performed on focally pigmented lesions with a suspicious or uncertain clinical diagnosis. Results: A total of 386 (30.2%) patients were found to have oral pigmentations. Of these, racial pigmentation (39.9%) and smokers' melanosis (32.9%) were the most common causes of oral pigmentations. Other causes included amalgam tattoo (18.9%), focal melanotic macules (5.7%), postinflammatory pigmentation (1.6%), pigmentation due to medications or systemic disease (0.52%), heavy metal deposits (0.26%), and oral nevus (0.26%). Gingivae and buccal mucosae were the most common sites for oral pigmentations. Conclusion: Pigmentations of the oral mucosa are common. Gingivae and buccal mucosae are the most common sites for oral pigmentations. Proper history and recognition of clinical features are important for effective management.

Abstract:Xeroderma pigmentosum (XP) is a rare autosomal recessive genetic disorder characterized by intense skin photosensitivity that is often associated with corneal ulceration, erythema, malignant lesions in sun-exposed areas, and neurological damage in severe cases. XP is due to alterations in the nucleotide excision repair system which could eliminate DNA fragments damaged by ultraviolet radiation. We report a case of a 14-year-old admitted for photophobia and a conjunctival mass. He underwent laboratory tests, including a complete blood count (CBC), which was unremarkable, and serological tests such as rapid plasma reagin (RPR) and human immunodeficiency virus (HIV) test were negative. A consultation in Ophthalmology was requested, concluding in bilateral corneal dystrophy. A few months later he developed two masses, one on the distal border of the tongue and the other at the level of the parotid region. He underwent two excisional biopsies; the parotid mass revealed an ulcerated squamous cell carcinoma on a background of xeroderma pigmentosum, and the tongue tip mass revealed a well-differentiated infiltrating verrucous carcinoma with a smooth margin. Xeroderma pigmentosum is a rare genodermatosis affecting the skin, eyes and oral cavity. It is sometimes associated with cancers of internal organs and rarely of the tongue. This study reports a case of XP associated with verrucous carcinoma of the tongue and ocular complications. Currently, there is no curative treatment for XP, and the only treatments available are symptomatic and preventive.

Abstract:Labial and oral melanotic macules are commonly encountered in a broad range of conditions ranging from physiologic pigmentation to a sign of an underlying life-threatening disease. Although Laugier-Hunziker syndrome (LHS) shares some features of labial and oral pigmentation with a variety of conditions, it is a benign and acquired condition, frequently associated with longitudinal melanonychia. Herein, the demographic, clinical, dermoscopic, and pathological aspects of LHS were reviewed comprehensively. The important differential diagnoses of mucocutaneous and nail pigmentation are provided. An accurate diagnosis is crucial to design a reasonable medical strategy, including management options, malignant transformation surveillance, and psychological support. It is important that clinicians conduct long-term follow-up and surveillance due to the potential risks of malignant transformation and local severe complications in some conditions.
