Abstract:The sustainability of healthcare systems is challenged by the international migration of health workers in many countries. Like other Central and Eastern European (CEE) countries, a great number of health professionals from Hungary have emigrated recently, increasing the shortage of health workers in the country. The aim of this study is to explore the migration attitudes of Hungarian health workers, applying a micro-level approach of push–pull factors. For this purpose, semi-structured interviews have been conducted with practicing and inactive Hungarian health professionals living in Hungary and abroad. The interviews were subjected to a thematic analysis, and the following groups of factors were revealed and discussed: wealth and income, workplace, human capital, quality of life, family, personal network, and personality. In addition, geography and life stage (life course) as two overarching, integrative categories are also discussed. The results point to the role of income, work environment, and family in migration decisions. As a new factor, the importance of geographical characteristics (local context, distance) is also explored, which has received less attention in previous studies. A novelty of our study is the CEE post-socialist point of view, mirrored by the life-path elements of the interviews. Another novelty is the qualitative and micro-level approach, forming the basis of policy recommendations presented at the end of our study.

[Rapid inhibition of extracellular signal-regulated kinase 1/2 in a human ovarian cancer cell line by dexamethasone].

Abstract:BACKGROUND & OBJECTIVE Activation of extracellular signal-regulated kinase 1/2 (ERK1/2) plays an important role in cell proliferation of a variety of cell types. The authors had previously found that dexamethasone (Dex), a synthetical glucocorticoid, can markedly inhibit the proliferation of a human ovarian cancer cell line HO-8910. This study was designed to observe the effect of Dex on the activation of ERK1/2 in HO-8910 cells in order to explore the signal transduction pathway that mediates the anti-proliferation effect of Dex on these cells. METHODS The activation of ERK1/2 was determined by Western blot analysis, and changes of cell proliferation were examined by cell count. RESULTS Inhibition of activation of ERK1 and ERK2 by 1 x 10(-7) mol/L Dex occurred synchronously at 5 min, with maximum up to 41% and 54%, respectively at 30 min compared with the control (P < 0.001), and sustained until 4 h. The effect increased with the increasement of concentration of Dex (1 x 10(-10)-1 x 10(-6) mol/L). RU486, an antagonist of glucocorticoid receptor (GR), did not block the effect. PD98059, an inhibitor of MEK1/2, also inhibited HO-8910 cell proliferation and could enhance the growth-inhibition effect of Dex. CONCLUSION Dex can rapidly inhibit ERK1/2 activation in a GR-independent manner in HO-8910 cells, which might play a role in Dex-mediated growth inhibition.
Migration intentions among physicians working in Polish hospitals - Insights from survey research

Abstract:Background: Health workforce shortages could lead to burnout, excessive workload, dissatisfaction and consequently to migration. In Poland the exact scale of physician migration is unknown due to insufficient data. Methods: A quantitative, cross-sectional survey of physicians working in Polish hospitals was conducted between March and June 2018. 15 Polish hospitals were included in the study (7 general, 5 specialist, 3 university). The data was gathered via an on-line, self-administered questionnaire, sent to physicians working in the included hospitals. Associations between the intention to migrate and demographic characteristics as well as work-related variables and overall satisfaction were measured. Simple and multivariable logistic regression analysis was conducted to determine significant predictors of migration. Results: 1003 questionnaires were analyzed (response rate: 38%). 273 doctors declared the intention to migrate: 4.5% answering 'definitely yes' and 22.7% 'probably yes'. The main reasons were: higher earnings, better working conditions and better work-life balance abroad. Age and higher career satisfaction were negatively related to the intention to migrate: OR = 0.94 (95% CI 0,92-0,95) and OR = 0.44 (95% CI 0.34-0.56) respectively. Women were 54% less likely to intend to migrate than men (OR = 0.46, 95% CI 0.33-0.65). Almost 62% of physicians intending to migrate considered a temporary stay abroad. Conclusions: Special attention should be paid to improving working conditions, including salary, but also reducing bureaucracy, improvement of work-climate and training opportunities.

Abstract:Aim: Germany's growing shortage of doctors is a current topic of numerous discussions in healthcare policy. The objective of this study is to show the medical supply situation and migratory movement of doctors in the outpatient and inpatient physician care sector, with particular emphasis on the immigration of foreign doctors to Germany and its consequences. Subjects and methods: The annual statistical reports from the Bavarian Medical Association and the Federal Medical Association were investigated with regard to foreign doctors and migratory movements. To establish Bavaria's situation regarding outpatient physician care, unpublished case reports and planning sheets of the Statutory Health Insurance Physicians in Bavaria (KVB) were analysed. A survey amongst Bavarian hospitals shows the current situation of the inpatient care sector. Results: The trend of emigration by German doctors continues unabated, especially to Switzerland, UK, USA and Austria. In Bavaria, outpatient care by GPs or specialists is still standard or in oversupply. However, the survey was able to confirm a considerable lack of doctors for inpatient care. So far, it has been possible to compensate existing staff shortages in hospitals by employing foreign doctors, despite significant language deficits. Discussion and conclusions: To resolve the shortage of doctors in future, a reform of the Medical Requirements Planning in combination with structural improvements and measures to integrate immigrant doctors is essential.
Physician emigration from Germany: insights from a survey in Saxony, Germany

Abstract:Background: Physician migration has been gaining attention worldwide. In Germany, physician migration became a topic of interest in the context of the discussion about a shortage of physicians, for which one contributing factor may be physicians leaving the country. However, there is a lack of literature on "push" factors causing German physicians to leave. The present study seeks to provide current data in an effort to promote the identification of "push" factors motivating German physicians to emigrate. Methods: In a cross-sectional survey, all physicians ≤40 years of age registered with the State Chamber of Physicians of Saxony, Germany (n = 5956) were sent a paper-pencil questionnaire examining socio-demographics, job satisfaction, the wish to emigrate, and the likelihood of moving abroad in the near future. Variables associated with the wish to emigrate were assessed with multivariate logistic regression models. Results: Approximately 30% of participants wished to emigrate. The favourite destination countries were Switzerland, Scandinavian countries, and Australia or New Zealand. Of participants wishing to emigrate, approximately 52% thought it likely to emigrate for a limited, and 15% for an unlimited period of time. Participants with the wish to emigrate were significantly less satisfied with their job situation as compared to physicians without the wish to emigrate, the one exception being their "relationship with patients". The three aspects with the highest difference in satisfaction were the overall work situation, followed by work load, and time for family, friends, and leisure activities. Being a woman, being in a relationship, and having children were associated with a lower chance for wishing to emigrate. Higher satisfaction with the factors "work load", "patient care", and "structural aspects" was also associated with a lower chance for wishing to emigrate. Conclusions: Emigration seems to be a viable option for at least a subset of physicians. Preventive measures should address modifiable determinants associated with an increased chance for wishing to emigrate, such as job satisfaction. Especially satisfaction with the factor "work load" seems to play a crucial role as a "push" factor for physician emigration.

Abstract:The International Classification of Functioning, Disability and Health (ICF) short-version checklist was used to assess the impairments, activity limitations and participation restrictions experienced by a sample of HIV-positive in-patients admitted to Chris Hani Baragwanath Hospital in Johannesburg, South Africa. Laboratory tests, observation and review of patients' medical records were used to complete the ICF Checklist. Eighty patients were assessed (23 males and 57 females). Common impairments related to the following functions: digestive, metabolic and endocrine systems (83.9%); sensory (83.5%); haematological, immunological and respiratory systems (82.5%); neuromusculoskeletal movement (73.8%); mental (72.6%); energy and drive (75%); sleep (71%); emotional (62%); and muscle power (75%). Activity limitations were present in the area of mobility (56.4%), major life areas (55.1%), and community, social and civic life (50%). Associations found among impairments, activity limitations and participation restrictions were that patients with sensory problems were five-times more likely to have problems in self-care than people without sensory problems. Patients with impairments in the digestive, genitourinary and neuromusculoskeletal systems experienced problems with general tasks (confidence interval [CI]: 4.05–103.03; p < 0.01). Patients with cardiovascular, haematological, immunological and respiratory system problems were 14-times more likely to have problems with execution of general tasks (odds ratio [OR] 14.06, CI: 2.75–71.94; p = 0.002). Activities of participation restriction, difficulties with general tasks and demands (OR 9.68, CI: 1.20–77.92), interpersonal relationships (OR 3.62, CI: 1.09–12.00), domestic life (OR 3.97, CI: 1.12–14.16), and community, social and civic life (OR 4.13, CI: 1.05–16.20) were closely associated with barriers in obtaining products for personal use and using technology. Understanding the prevalence and associations of disability and function in the course of HIV disease may serve as a baseline for developing appropriate and context-sensitive rehabilitation interventions and management strategies for people living with HIV or AIDS.

Abstract:Objective Providing a general overview of the European Union's health workforce mobility under the challenges facing health systems regarding the supply of health workers. Method We use a descriptive method , based on the analysis of secondary data, qualitative and quantitative, concerning the European Semester from the European Union, complemented with statistical data from both the Union and some international organisations. Results The mobility of health professionals in the Union, associated to strong reliance on recruiting abroad and shortages due to emigration, was identified as a challenge in the European Semester process in a significant number of times during 2017-2023. The pandemic aggravated pre-existing shortages and the need to strike a balance between maintaining the resolution capacity of health systems while abiding by the free movement of health professionals. The information shows that Romania, Slovakia, Spain, Lithuania, Latvia, Portugal, Bulgaria, Greece, Croatia, Hungary, Italy, and Slovenia could be flagged with an "issuer profile". Luxembourg, Ireland, Malta, and Sweden could be flagged with a "recipient profile". We benefited from improvements in the information system concerning the Union's health workforce. Further advances regarding the harmonisation of health professions' definition are needed, especially for nurses. Conclusions The European Union faces internal migrations of health professionals. Mobility is used as a solution to shortages. The pandemic aggravated pre-existing shortages bringing to the forefront the need to strike a balance between health objectives and internal market objectives. Member States are immersed in health reforms, some financed with European Funds. Promoting health workforce planning and forecasting would emerge as a necessary action, including improving harmonised information. Drawing in a systematic way on the available information from the European Semester reports may provide some clues to give answers to policymaking concerning health professionals' mobility. Resumen Objetivo Proporcionar una visión general de la movilidad del personal sanitario de la Unión Europea ante los desafíos a los que se enfrentan los sistemas sanitarios relacionados con la oferta de profesionales. Método Utilizamos un método de investigación descriptivo, basado en el análisis de información secundaria, cualitativa y cuantitativa, relativa al Semestre Europeo de la Unión Europea, complementada con datos estadísticos tanto de la Unión Europea como de algunas organizaciones internacionales. Resultados La movilidad de los profesionales sanitarios en la Unión Europea, asociada a problemas de fuerte dependencia de la contratación de profesionales extranjeros y escasez debido a la emigración, se identificó como un desafío en el proceso del Semestre Europeo en un número significativo de veces durante el periodo 2017-2023. La pandemia agravó la escasez preexistente, enfatizando la necesidad de lograr un equilibrio entre mantener la capacidad resolutiva de los sistemas sanitarios y respetar al mismo tiempo la libre circulación de los profesionales sanitarios. Abstract:We recently developed a new group of folate-conjugated Vinca alkaloids, one of which, EC145, emerged as a candidate for clinical development. Brief treatment of nude mice bearing approximately 100 mm(3) folate receptor-positive human xenografts led to complete response (CR) in 5/5 mice and cures (i.e., remission without a relapse for >90 days post-tumor implantation) in 4/5 mice. Multiple CRs and cures were also noted when EC145 was used to treat mice initially bearing tumors as large as 750 mm(3). Likewise, complete cures (5/5) resulted following the treatment of an aggressive folate receptor-positive J6456 lymphoma model. The activity of EC145 was not accompanied by noticeable weight loss or major organ tissue degeneration. Furthermore, no significant antitumor activity (0/5 CR) was observed in EC145-treated animals that were co-dosed with an excess of a benign folate ligand, thus demonstrating the target-specific activity of EC145. The enhanced therapeutic index due to folate conjugation was also evidenced by the fact that the unconjugated drug (desacetylvinblastine monohydrazide) was found to be completely inactive when administered at nontoxic dose levels and only marginally active when given at highly toxic dose levels. Subsequent dose regimen studies confirmed that EC145 given on a more frequent, qdx5 schedule resulted in the most effective antitumor response as compared with an equivalent total dose given on thrice- or single-injection-per-week schedule. Taken together, these studies show that EC145 has significant antiproliferative activity and tolerability, thus lending support to an ongoing phase 1 trial for the treatment of advanced malignancies.
Physician Workforce in Lithuania: Changes during Thirty Years of Independence

Abstract:Healthcare human resource planning is one of the biggest challenges facing the healthcare systems in many countries. Inadequate decisions in human resource planning can lead to an insufficient number of healthcare professionals then healthcare inequalities. One of the components of resource planning in the healthcare system is long-term data monitoring and the identification of potential trends. Since 1990, the number of physicians in Lithuania has decreased by 15.3% (−2266), but the decrease in the population has led to a 13.61% increase in the number of physicians per 10,000 inhabitants (5.32). During the analyzed period, the largest decrease in the number of physicians workforce by specialty was the number of medical physicians (−73.08%), epidemiology and hygiene (−69.30%), children's diseases (−49.08%), the most increased number was of family/general practitioners (GPs), geneticists, physical medicine, and rehabilitation specialists. Since 1992, the number of visits to physicians in Lithuania, which has been decreasing for a long time, began increasing, and in 2022 (9.3 visits) it has almost reached the number of visits (9.5) per capita as in 1991. The aim of this research was to collect long-term data from various databases, summarize them, and identify possible trends and the reasons for data changes. The study analyzed data from the Lithuanian healthcare system from the Declaration of Independence of Lithuania to the last 30 years. The data includes or affects the indicators of the healthcare system, changes in population and doctors, the number of visits to doctors, the number of medical students and residents, and data determining inequalities in the healthcare system. Long-term data analysis is useful for developing a model of healthcare human resource planning and for planning healthcare resources.

Forecasting the Early Impact of COVID-19 on Physician Supply in EU Countries

Abstract:Background: Many countries faced health workforce challenges even before the pandemic, such as impending retirements, negative population growth, or sub-optimal allocation of resources across health sectors. Current quantitative models are often of limited use, either because they require extensive individual-level data to be properly calibrated, or (in the absence of such data) because they are too simplistic to capture important demographic changes or disruptive epidemiological shocks such as the SARS-CoV-2 pandemic. Method: We propose a population-dynamic and stock-flow-consistent approach to physician supply forecasting that is complex enough to account for dynamically changing behaviour, while requiring only publicly available time-series data for full calibration. We demonstrate the utility of this model by applying it to 21 European countries to forecast the supply of generalist and specialist physicians to 2040, and the impact of increased health care utilisation due to Covid on this supply. Results: Compared with the workforce needed to maintain physician density at 2019 levels, we find that in many countries there is indeed a significant trend towards decreasing generalist density at the expense of increasing specialist density. The trends for specialists are exacerbated by expectations of negative population growth in many Southern and Eastern European countries. Compared to the expected demographic changes in the population and the health workforce, we expect a limited impact of Covid on these trends, even under conservative modelling assumptions. Finally, we generalise the approach to a multi-professional, multi-regional and multi-sectoral model for Austria, where we find an additional suboptimal distribution in the supply of contracted versus non-contracted (private) physicians. Conclusion: It is therefore vital to develop tools for decision-makers to influence the allocation and supply of doctors across specialties and sectors to address these imbalances.

Abstract:Background Attention to the healthcare workforce has increased, yet comprehensive information on migrant healthcare workers is missing. This study focuses on migrant healthcare workers' experiences and mobility patterns in the middle of a global health crisis, aiming to explore the capacity for circular migration and support effective and equitable healthcare workforce policy. Methods Romanian physicians working in Germany during the COVID‐19 pandemic served as an empirical case study. We applied a qualitative explorative approach; interviews (n = 21) were collected from mid of September to early November 2022 and content analysis was performed. Results and Discussion Migrant physicians showed strong resilience during the COVID‐19 crisis and rarely complained. Commitment to high professional standards and career development were major pull factors towards Germany, while perceptions of limited career choices, nepotism and corruption in Romania caused strong push mechanisms. We identified two major mobility patterns that may support circular migration policies: well‐integrated physicians with a wish to give something back to their home country, and mobile cosmopolitan physicians who flexibly balance career opportunities and personal/family interests. Health policy must establish systematic monitoring of the migrant healthcare workforce including actor‐centred approaches, support integration in destination countries as well as health system development in sending countries, and invest in evidence‐based circular migration policy.

Abstract:Physician migration, which maintains global importance in terms of health ethics, is a phenomenon that has gained momentum in Türkiye throughout the recent years. However, there are limited studies on this subject, and no research has been found that addresses the issue in terms of health ethics and rights. In this regard, this study is likely to contribute to a better understanding of Türkiye’s physician migration. Furthermore, draw some attentions for solutions and health policies through the lens of physician and patients’ rights. The population of the retrospectively designed study consists of research proposals related to physician migration in the Turkish Grand National Assembly and the statements of members of parliament in the general assembly proceedings during the same period. Data obtained from transcripts that were searched using keywords such as “physician,” “doctor,” “health,” “migration,” and “foreign countries” were coded and subjected to content analysis using a qualitative method. Multifaceted findings emerged in the context of physician and patient rights in the categories of “reasons,” “consequences,” and “solutions”. It was observed that certain rights were more affected by the process, that rights were interdependent, and that structural regulations in the healthcare system were necessary for their fulfillment. There is a need for legislation that would concretely demonstrate physician rights in a legal status. In the context of physician rights, the prominence of “reasons” and the emphasis on patient rights in the “consequences” category also point to a significant ethical dilemma. The dilemma between the autonomy of the physician and the principle of justice, which is central to the ethical debate about physician migration, has been confirmed, indicating a need for further in-depth research on this topic. The purpose of this study is to evaluate how physician migration, which has recently increased in Türkiye, is addressed by members of the Turkish Grand National Assembly in terms of physician and patient rights.
Passing through - reasons why migrant doctors in Ireland plan to stay, return home or migrate onwards to new destination countries

Abstract:Background: International recruitment is a common strategy used by high-income countries to meet their medical workforce needs. Ireland, despite training sufficient doctors to meet its internal demand, continues to be heavily dependent on foreign-trained doctors, many of whom may migrate onwards to new destination countries. A cross-sectional study was conducted to measure and analyse the factors associated with the migratory intentions of foreign doctors in Ireland. Methods: A total of 366 non-European nationals registered as medical doctors in Ireland completed an online survey assessing their reasons for migrating to Ireland, their experiences whilst working and living in Ireland, and their future plans. Factors associated with future plans - whether to remain in Ireland, return home or migrate to a new destination country - were tested by bivariate and multivariate analyses, including discriminant analysis. Results: Of the 345 foreign doctors who responded to the question regarding their future plans, 16 % of whom were Irish-trained, 30 % planned to remain in Ireland, 23 % planned to return home and 47 % to migrate onwards. Country of origin, personal and professional reasons for migrating, experiences of training and supervision, opportunities for career progression, type of employment contract, citizenship status, and satisfaction with life in Ireland were all factors statistically significantly associated with the three migratory outcomes. Conclusion: Reported plans may not result in enacted emigration. However, the findings support a growing body of evidence highlighting dissatisfaction with current career opportunities, contributing to the emigration of Irish doctors and onward migration of foreign doctors. Implementation of the WHO Global Code, which requires member states to train and retain their own health workforce, could also help reduce onward migration of foreign doctors to new destination countries. Ireland has initiated the provision of tailored postgraduate training to doctors from Pakistan, enabling these doctors to return home with improved skills of benefit to the source country.
Exploring public sector physicians' resilience, reactions and coping strategies in times of economic crisis; findings from a survey in Portugal's capital city area

Abstract:Background: Evidence is accumulating on the impact of the recent economic crisis on health and health systems across Europe. However, little is known about the effect this is having on physicians - a crucial resource for the delivery of healthcare services. This paper explores the adaptation to the crisis of public sector physicians and their ability to keep performing their functions, with the objective of gaining a better understanding of health workers' resilience under deteriorating conditions. Methods: We conducted a survey among 484 public primary care and hospital physicians in Portugal's capital city area and explored their perceptions of the crisis, adaptation and coping strategies. We used ordinal and logistic regression models to link changes in hours worked and intentions to migrate with physicians' characteristics and specific answers. Results: We found little evidence of physicians changing their overall allocation of working time before and after the crisis, with their age, types of specialisation, valuation of job flexibility and independence significantly associated with changes in public sector hours between 2010 and 2015. Being divorced, not Portuguese, of younger age, and working a high number of hours per week, were found to increase the probability of physicians considering migration, the same as having a poor opinion of recent government health policies. On the other hand, enjoying their current working environment, not wanting to disrupt provision of service, and leisure time were found to protect against scaling down public sector hours or considering migration. Conclusions: Our work on Portuguese physicians contributes to the debate on health workers' resilience, showing the value of understanding the influence of personal characteristics and opinions on their adaptation to changing circumstances, before designing policies to improve their working conditions and retention.
Exploring Satisfaction and Migration Intentions of Physicians in Three University Hospitals in Poland

Abstract:Introduction: University hospitals constitute a unique group of health care organizations which traditionally link three functions: (1) providing highly specialized services, (2) teaching activities, and (3) conducting research. Objectives: To assess the level of carrier satisfaction among physicians working in three university hospitals in Poland (1); to assess whether the physicians have the intention to migrate and what the main reasons for migration are (2); and to identify the actions that might be taken at the hospital level to mitigate physicians' intentions to migrate (3). Methods: Cross-sectional study with both quantitative and qualitative components. In the quantitative part, an online questionnaire was distributed among physicians working in three university hospitals. A total number of 396 questionnaires were analyzed. In the qualitative part, in-depth interviews with six hospital managers were conducted and analyzed using thematic analysis. Results: On a scale from one "very dissatisfied" to six "very satisfied", the mean career satisfaction of physicians was 4.0 (SD = 0.74). The item with the lowest mean concerned salary level (2.8, SD = 1.41). In the sample, 34% of physicians declared intentions to migrate from Poland. The main reasons for the intention to migrate were: Better working conditions abroad, higher earnings, the ability to maintain better work-life balance, better training opportunities abroad, and problems due to a stressful current workplace. Hospital managers considered the actions that can be taken at the hospital level to mitigate physicians' migration to be specific to those focused on the working environment. Conclusions: Career development opportunities and features related to the working environment are the main factors influencing physicians' satisfaction and migration intentions that can be modified at the university hospital level.
