Abstract:Glaucoma is a chronic neuro-degenerative condition that is one of the world's leading causes of irreversible but preventable blindness. The blindness is generally caused by the lack of timely detection and treatment. Early screening is thus essential for early treatment to preserve vision and maintain life quality. Color fundus photography and Optical Coherence Tomography (OCT) are the two most cost-effective tools for glaucoma screening. Both imaging modalities have prominent biomarkers to indicate glaucoma suspects, such as the vertical cup-to-disc ratio (vCDR) on fundus images and retinal nerve fiber layer (RNFL) thickness on OCT volume. In clinical practice, it is often recommended to take both of the screenings for a more accurate and reliable diagnosis. However, although numerous algorithms are proposed based on fundus images or OCT volumes for the automated glaucoma detection, there are few methods that leverage both of the modalities to achieve the target. To fulfill the research gap, we set up the Glaucoma grAding from Multi-Modality imAges (GAMMA) Challenge to encourage the development of fundus & OCT-based glaucoma grading. The primary task of the challenge is to grade glaucoma from both the 2D fundus images and 3D OCT scanning volumes. As part of GAMMA, we have publicly released a glaucoma annotated dataset with both 2D fundus color photography and 3D OCT volumes, which is the first multi-modality dataset for machine learning based glaucoma grading. In addition, an evaluation framework is also established to evaluate the performance of the submitted methods. During the challenge, 1272 results were submitted, and finally, ten best performing teams were selected for the final stage. We analyze their results and summarize their methods in the paper. Since all the teams submitted their source code in the challenge, we conducted a detailed ablation study to verify the effectiveness of the particular modules proposed. Finally, we identify the proposed techniques and strategies that could be of practical value for the clinical diagnosis of glaucoma. As the first in-depth study of fundus & OCT multi-modality glaucoma grading, we believe the GAMMA Challenge will serve as an essential guideline and benchmark for future research.

A Structure-Related Fine-Grained Deep Learning System with Diversity Data for Universal Glaucoma Visual Field Grading

Abstract:PurposeGlaucoma is the main cause of irreversible blindness worldwide. However, the diagnosis and treatment of glaucoma remain difficult because of the lack of an effective glaucoma grading measure. In this study, we aimed to propose an artificial intelligence system to provide adequate assessment of glaucoma patients.MethodsA total of 16,356 visual fields (VFs) measured by Octopus perimeters and Humphrey Field Analyzer (HFA) were collected, from three hospitals in China and the public Harvard database. We developed a fine-grained grading deep learning system, named FGGDL, to evaluate the VF loss, compared to ophthalmologists. Subsequently, we discuss the relationship between structural and functional damage for the comprehensive evaluation of glaucoma level. In addition, we developed an interactive interface and performed a cross-validation study to test its auxiliary ability. The performance was valued by F1 score, overall accuracy and area under the curve (AUC).ResultsThe FGGDL achieved a high accuracy of 85 and 90%, and AUC of 0.93 and 0.90 for HFA and Octopus data, respectively. It was significantly superior (p < 0.01) to that of medical students and nearly equal (p = 0.614) to that of ophthalmic clinicians. For the cross-validation study, the diagnosis accuracy was almost improved (p < 0.05).ConclusionWe proposed a deep learning system to grade VF of glaucoma with a high detection accuracy, for effective and adequate assessment for glaucoma patients. Besides, with the convenient and credible interface, this system can promote telemedicine and be used as a self-assessment tool for patients with long-duration diseases.
Multi-modality Images Analysis - A Baseline for Glaucoma Grading via Deep Learning.

Abstract:Glaucoma is one of the leading causes of blindness in humans, which is not reversible, but early detection and treatment can save helpful vision. Clinicians classify glaucoma into early, moderate, and advanced stages based on the extent of the patient’s visual field deficit. The treatment of glaucoma varies with the course of the disease. With the development of deep learning technology, more and more studies focus on the automatic diagnosis of glaucoma. Most of them are based on color fundus images or OCT images. However, there are limitations in using only one modality images to analyze glaucoma due to the complexity of glaucoma. Therefore, in this paper, two modalities of images, color fundus image and 3D OCT image provided by the GAMMA Challenge, were used to design baseline algorithms for glaucoma grading. On the preliminary dataset of the GAMMA Challenge, the kappa value of the glaucoma grading results based on the two modalities of image input were improved by 0.092 and 0.075, respectively, compared with those of the model with single fundus image and single OCT image input. And on the final datasets, the corresponding improvement were 0.029 and 0.127. At the same time, considering that optic disc changes are the main features of glaucoma, we added local information of optic disc into the input module, so that the kappa values were improved respectively by 0.075 and 0.068 in the preliminary dataset and final dataset of the model based on the images of two modalities as input. In addition, this study used an ordinal regression strategy on the classification task to increase the kappa value of the results of automatic classification of glaucoma based on multi-modality images by 0.097 and 0.050 on the preliminary and final datasets of the GAMMA Challenge.

Abstract:Glaucoma causes irreversible vision loss due to damage to the optic nerve, and there is no cure for glaucoma. OCT imaging modality is an essential technique for assessing glaucomatous damage since it aids in quantifying fundus structures. To promote the research of AI technology in the field of OCT-assisted diagnosis of glaucoma, we held a Glaucoma OCT Analysis and Layer Segmentation (GOALS) Challenge in conjunction with the International Conference on Medical Image Computing and Computer Assisted Intervention (MICCAI) 2022 to provide data and corresponding annotations for researchers studying layer segmentation from OCT images and the classification of glaucoma. This paper describes the released 300 circumpapillary OCT images, the baselines of the two sub-tasks, and the evaluation methodology. Abstract:As one of the leading causes of irreversible blindness worldwide, glaucoma is characterized by structural damage and functional loss. Glaucoma patients often have a long follow-up and prognosis prediction is an important part in treatment. However, existing public glaucoma datasets are almost cross-sectional, concentrating on segmentation on optic disc (OD) and glaucoma diagnosis. With the development of artificial intelligence (AI), the deep learning model can already provide accurate prediction of future visual field (VF) and its progression with the support of longitudinal datasets. Here, we proposed a public longitudinal glaucoma real-world appraisal progression ensemble (GRAPE) dataset. The GRAPE dataset contains 1115 follow-up records from 263 eyes, with VFs, fundus images, OCT measurements and clinical information, and OD segmentation and VF progression are annotated. Two baseline models demonstrated the feasibility in prediction of VF and its progression. This dataset will advance AI research in glaucoma management.

Abstract:The early detection of glaucoma is essential in preventing visual impairment. Artificial intelligence (AI) can be used to analyze color fundus photographs (CFPs) in a cost-effective manner, making glaucoma screening more accessible. While AI models for glaucoma screening from CFPs have shown promising results in laboratory settings, their performance decreases significantly in real-world scenarios due to the presence of out-of-distribution and low-quality images. To address this issue, we propose the Artificial Intelligence for Robust Glaucoma Screening (AIROGS) challenge. This challenge includes a large dataset of around 113,000 images from about 60,000 patients and 500 different screening centers, and encourages the development of algorithms that are robust to ungradable and unexpected input data. We evaluated solutions from 14 teams in this paper, and found that the best teams performed similarly to a set of 20 expert ophthalmologists and optometrists. The highest-scoring team achieved an area under the receiver operating characteristic curve of 0.99 (95% CI: 0.98-0.99) for detecting ungradable images on-the-fly. Additionally, many of the algorithms showed robust performance when tested on three other publicly available datasets. These results demonstrate the feasibility of robust AI-enabled glaucoma screening.

Self-Adaptive Transfer Learning for Multicenter Glaucoma Classification in Fundus Retina Images

Abstract:The early screening of glaucoma is important for patients to receive treatment in time and maintain eyesight. Deep learning (DL) based models have been successfully used for computer-aided diagnosis (CAD) of glaucoma. However, a DL model pre-trained on certain dataset from one hospital may have poor performance on other hospital data, therefore its applications in the real scene are limited. In this paper, we propose a self-adaptive transfer learning (SATL) strategy to fill the domain gap between multi-center datasets. Specifically, the encoder of a DL model that is pre-trained on the source domain is used to initialize the encoder of a reconstruction model. Then, this reconstruction model is trained using only unlabeled image data from the target domain, which makes the encoder in the model adapt itself to extract useful features both for target domain images encoding and glaucoma classification, simultaneously. Experimental results on a private and two public glaucoma diagnosis datasets demonstrate that the proposed SATL strategy is effective. Also, it meets the real scene application and the privacy protection policy due to its independence from the source domain data.

Abstract:Glaucoma leads to irreversible vision impairment due to optic nerve damage, and there is currently no cure available. The visual field (VF) test is a reference standard examination to assess visual function and determine glaucomatous optic nerve damage. However, the VF test is time-consuming, requires patient cooperation, and may have poor repeatability. Optical Coherence Tomography (OCT) is widely used for eye structure examination. OCT images provide objective cross-sectional information of the fundus structure, aiding glaucoma diagnosis. Moreover, monocular OCT imaging is significantly faster than monocular visual field test. Therefore, we aim to use structural OCT images for predicting VF test indicators. We have organized the STAGE challenge, in conjunction with the International Conference on Medical Image Computing and Computer Assisted Intervention (MICCAI) 2023. The challenge involves three tasks: 1) predicting mean deviation (MD) values; 2) forecasting sensitivity maps; and 3) estimating pattern deviation probability maps. Participants will access to a dataset of 400 volume OCT data samples with corresponding MD values, sensitivity maps, and pattern deviation probability map labels from VF test reports. Abstract:Glaucoma is the leading causes of blindness in the world. We develop a convolutional neural network for glaucoma diagnosis based on visual fields (VF), which is the gold standard to show functional damages of optic nerve. However, we have to deal with two major problems common in medical imaging domains. 1) It is difficult and expensive to label a large amount of data, while most modern deep learning methods require it. 2) Severe data imbalance makes the classifier easily over-fitting. In this work, for the first problem, we train an AutoEncoder with all the data (labeled and unlabeled) to obtain good features and introduce an active learning (AL) scheme to select and annotate a few most valuable samples from the unlabeled date set for model training. Then, we address the second problem by augmenting negative samples generated by a deep convolutional generative adversarial network (DCGAN). Experiments on our dataset (738 Samples) suggest the effectiveness of the proposed approach.
Unifying Structure Analysis and Surrogate-driven Function Regression for Glaucoma OCT Image Screening

Abstract:Optical Coherence Tomography (OCT) imaging plays an important role in glaucoma diagnosis in clinical practice. Early detection and timely treatment can prevent glaucoma patients from permanent vision loss. However, only a dearth of automated methods has been developed based on OCT images for glaucoma study. In this paper, we present a novel framework to effectively classify glaucoma OCT images from normal ones. A semi-supervised learning strategy with smoothness assumption is applied for surrogate assignment of missing function regression labels. Besides, the proposed multi-task learning network is capable of exploring the structure and function relationship from the OCT image and visual field measurement simultaneously, which contributes to classification performance boosting. Essentially, we are the first to unify the structure analysis and function regression for glaucoma screening. It is also worth noting that we build the largest glaucoma OCT image dataset involving 4877 volumes to develop and evaluate the proposed method. Extensive experiments demonstrate that our framework outperforms the baseline methods and two glaucoma experts by a large margin, achieving 93.2%, 93.2% and 97.8% on accuracy, F1 score and AUC, respectively.

DeepGF: Glaucoma Forecast Using the Sequential Fundus Images.

Abstract:Disease forecast is an effective solution to early treatment and prevention for some irreversible diseases, e.g., glaucoma. Different from existing disease detection methods that predict the current status of a patient, disease forecast aims to predict the future state for early treatment. This paper is a first attempt to address the glaucoma forecast task utilizing the sequential fundus images of a patient. Specifically, we establish a database of sequential fundus images for glaucoma forecast (SIGF), which includes an average of 9 images per eye, corresponding to 3,671 fundus images in total. Besides, a novel deep learning method for glaucoma forecast (DeepGF) is proposed based on our SIGF database, consisting of an attention-polar convolution neural network (AP-CNN) and a variable time interval long short-term memory (VTI-LSTM) network to learn the spatio-temporal transition at different time intervals across sequential medical images of a person. In addition, a novel active convergence (AC) training strategy is proposed to solve the imbalanced sample distribution problem of glaucoma forecast. Finally, the experimental results show the effectiveness of our DeepGF method in glaucoma forecast.
Deep Learning with HM-VGG: AI Strategies for Multi-modal Image Analysis

Abstract:This study introduces the Hybrid Multi-modal VGG (HM-VGG) model, a cutting-edge deep learning approach for the early diagnosis of glaucoma. The HM-VGG model utilizes an attention mechanism to process Visual Field (VF) data, enabling the extraction of key features that are vital for identifying early signs of glaucoma. Despite the common reliance on large annotated datasets, the HM-VGG model excels in scenarios with limited data, achieving remarkable results with small sample sizes. The model's performance is underscored by its high metrics in Precision, Accuracy, and F1-Score, indicating its potential for real-world application in glaucoma detection. The paper also discusses the challenges associated with ophthalmic image analysis, particularly the difficulty of obtaining large volumes of annotated data. It highlights the importance of moving beyond single-modality data, such as VF or Optical Coherence Tomography (OCT) images alone, to a multimodal approach that can provide a richer, more comprehensive dataset. This integration of different data types is shown to significantly enhance diagnostic accuracy. The HM- VGG model offers a promising tool for doctors, streamlining the diagnostic process and improving patient outcomes. Furthermore, its applicability extends to telemedicine and mobile healthcare, making diagnostic services more accessible. Finding New Diagnostic Information for Detecting Glaucoma using Neural Networks

Abstract:We describe a new approach to automated Glaucoma detection in 3D Spectral Domain Optical Coherence Tomography (OCT) optic nerve scans. First, we gathered a unique and diverse multi-ethnic dataset of OCT scans consisting of glaucoma and non-glaucomatous cases obtained from four tertiary care eye hospitals located in four different countries. Using this longitudinal data, we achieved state-of-the-art results for automatically detecting Glaucoma from a single raw OCT using a 3D Deep Learning system. These results are close to human doctors in a variety of settings across heterogeneous datasets and scanning environments. To verify correctness and interpretability of the automated categorization, we used saliency maps to find areas of focus for the model. Matching human doctor behavior, the model predictions indeed correlated with the conventional diagnostic parameters in the OCT printouts, such as the retinal nerve fiber layer. We further used our model to find new areas in the 3D data that are presently not being identified as a diagnostic parameter to detect glaucoma by human doctors. Namely, we found that the Lamina Cribrosa (LC) region can be a valuable source of helpful diagnostic information previously unavailable to doctors during routine clinical care because it lacks a quantitative printout. Our model provides such volumetric quantification of this region. We found that even when a majority of the RNFL is removed, the LC region can distinguish glaucoma. This is clinically relevant in high myopes, when the RNFL is already reduced, and thus the LC region may help differentiate glaucoma in this confounding situation. We further generalize this approach to create a new algorithm called DiagFind that provides a recipe for finding new diagnostic information in medical imagery that may have been previously unusable by doctors.

MTCLF is a teacher-student framework. The teacher network is used to generate the label evidence map. The student network can diagnose glaucoma and predict the evidence map at the same time with the well-designed dual-branch CNN structure and collaborative learning module. We design two curriculum coefficients theta and sigma to guide the training process of the student network in the sample space so that the student network can adaptively balance the sample contribution, reduce the prediction bias and mine hard samples. Results: The experimental results show that the accuracy, sensitivity, specificity, AUC and F-2-score of MTCLF based on the LAG dataset for glaucoma diagnoses are 0.967, 0.961, 0.970, 0.996, and 0.958, respectively. These results are superior to those of the state-of-the-art methods. Conclusion: MTCLF not only achieves the best performance for unbiased glaucoma diagnoses but also generates a reliable evidence map to help clinicians explore fine lesion areas. Abstract:A difficult challenge in the realm of biomedical engineering is the detection of physiological changes occurring inside the human body, which is a difficult undertaking. At the moment, these irregularities are graded manually, which is very difficult, time-consuming, and tiresome due to the many complexities associated with the methods involved in their identification. In order to identify illnesses at an early stage, the use of computer-assisted diagnostics has acquired increased attention as a result of the requirement of a disease detection system. The major goal of this proposed work is to build a computer-aided design (CAD) system to help in the early identification of glaucoma as well as the screening and treatment of the disease. The fundus camera is the most affordable image analysis modality available, and it meets the financial needs of the general public. The extraction of structural characteristics from the segmented optic disc and the segmented optic cup may be used to characterize glaucoma and determine its severity. For this study, the primary goal is to estimate the potential of the image analysis model for the early identification and diagnosis of glaucoma, as well as for the evaluation of ocular disorders. The suggested CAD system would aid the ophthalmologist in the diagnosis of ocular illnesses by providing a second opinion as a judgment made by human specialists in a controlled environment. An ensemble-based deep learning model for the identification and diagnosis of glaucoma is in its early stages now. This method's initial module is an ensemble-based deep learning model for glaucoma diagnosis, which is the first of its kind ever developed. It was decided to use three pretrained convolutional neural networks for the categorization of glaucoma. These networks included the residual network (ResNet), the visual geometry group network (VGGNet), and the GoogLeNet. It was necessary to use five different data sets in order to determine how well the proposed algorithm performed. These data sets included the DRISHTI-GS, the Optic Nerve Segmentation Database (DRIONS-DB), and the High-Resolution Fundus (HRF). Accuracy of 91.11% for the PSGIMSR data set and the sensitivity of 85.55% and specificity of 95.20% for the suggested ensemble architecture on the PSGIMSR data set were achieved. Similarly, accuracy rates of 95.63%, 98.67%, 95.64%, and 88.96% were achieved using the DRIONS-DB, HRF, DRISHTI-GS, and combined data sets, respectively.

Abstract:Glaucoma is one of the leading causes of irreversible but preventable blindness in working age populations. Color fundus photography (CFP) is the most cost-effective imaging modality to screen for retinal disorders. However, its application to glaucoma has been limited to the computation of a few related biomarkers such as the vertical cup-to-disc ratio. Deep learning approaches, although widely applied for medical image analysis, have not been extensively used for glaucoma assessment due to the limited size of the available data sets. Furthermore, the lack of a standardize benchmark strategy makes difficult to compare existing methods in a uniform way. In order to overcome these issues we set up the Retinal Fundus Glaucoma Challenge, REFUGE (https://refuge.grand-challenge.org), held in conjunction with MICCAI 2018. The challenge consisted of two primary tasks, namely optic disc/cup segmentation and glaucoma classification. As part of REFUGE, we have publicly released a data set of 1200 fundus images with ground truth segmentations and clinical glaucoma labels, currently the largest existing one. We have also built an evaluation framework to ease and ensure fairness in the comparison of different models, encouraging the development of novel techniques in the field. 12 teams qualified and participated in the online challenge. This paper summarizes their methods and analyzes their corresponding results. In particular, we observed that two of the top-ranked teams outperformed two human experts in the glaucoma classification task. Furthermore, the segmentation results were in general consistent with the ground truth annotations, with complementary outcomes that can be further exploited by ensembling the results.

Abstract:With the rapid development of artificial intelligence (AI) in medical image processing, deep learning in color fundus photography (CFP) analysis is also evolving. Although there are some open-source, labeled datasets of CFPs in the ophthalmology community, large-scale datasets for screening only have labels of disease categories, and datasets with annotations of fundus structures are usually small in size. In addition, labeling standards are not uniform across datasets, and there is no clear information on the acquisition device. Here we release a multi-annotation, multi-quality, and multi-device color fundus image dataset for glaucoma analysis on an original challenge -- Retinal Fundus Glaucoma Challenge 2nd Edition (REFUGE2). The REFUGE2 dataset contains 2000 color fundus images with annotations of glaucoma classification, optic disc/cup segmentation, as well as fovea localization. Meanwhile, the REFUGE2 challenge sets three sub-tasks of automatic glaucoma diagnosis and fundus structure analysis and provides an online evaluation framework. Based on the characteristics of multi-device and multi-quality data, some methods with strong generalizations are provided in the challenge to make the predictions more robust. This shows that REFUGE2 brings attention to the characteristics of real-world multi-domain data, bridging the gap between scientific research and clinical application.

Abstract:Glaucoma is one of the ophthalmic diseases that may cause blindness, for which early detection and treatment are very important. Fundus images and optical coherence tomography (OCT) images are both widely-used modalities in diagnosing glaucoma. However, existing glaucoma grading approaches mainly utilize a single modality, ignoring the complementary information between fundus and OCT. In this paper, we propose an efficient multi-modality supervised contrastive learning framework, named COROLLA, for glaucoma grading. Through layer segmentation as well as thickness calculation and projection, retinal thickness maps are extracted from the original OCT volumes and used as a replacing modality, resulting in more efficient calculations with less memory usage. Given the high structure and distribution similarities across medical image samples, we employ supervised contrastive learning to increase our models' discriminative power with better convergence. Moreover, feature-level fusion of paired fundus image and thickness map is conducted for enhanced diagnosis accuracy. On the GAMMA dataset, our COROLLA framework achieves overwhelming glaucoma grading performance compared to state-of-the-art methods.
