Abstract:As more is learned about injury mechanisms of concussion and scenarios under which injuries are sustained in football games, methods used to evaluate protective equipment must adapt. A combination of video review, videogrammetry, and laboratory reconstructions was used to characterize concussive impacts from National Football League games during the 2015-2017 seasons. Test conditions were generated based upon impact locations and speeds from this data set, and a method for scoring overall helmet performance was created. Head kinematics generated using a linear impactor and sliding table fixture were comparable to those from laboratory reconstructions of concussive impacts at similar impact conditions. Impact tests were performed on 36 football helmet models at two laboratories to evaluate the reproducibility of results from the resulting test protocol. Head acceleration response metric, a head impact severity metric, varied 2.9–5.6% for helmet impacts in the same lab, and 3.8–6.0% for tests performed in a separate lab when averaged by location for the models tested. Overall inter-lab helmet performance varied by 1.1 ± 0.9%, while the standard deviation in helmet performance score was 7.0%. The worst helmet performance score was 33% greater than the score of the best-performing helmet evaluated by this study.

Abstract:The purpose of this study was to compare the effects of wearing older, lower-ranked football helmets (LRank) to wearing newer, higher-ranked football helmets (HRank) on pre- to post-season changes in cortical thickness in response to repetitive head impacts and assess whether changes in cortical thickness are associated with head impact exposure for either helmet type. 105 male high-school athletes (NHRank = 52, NLRank = 53) wore accelerometers affixed behind the left mastoid during all practices and games for one regular season of American football to monitor head impact exposure. Pre- and post-season magnetic resonance imaging (MRI) were completed to assess longitudinal changes in cortical thickness. Significant reductions in cortical thickness (i.e., cortical thinning) were observed pre- to post-season for each group, but these longitudinal alterations were not significantly different between the LRank and HRank groups. Further, significant group-by-head impact exposure interactions were observed when predicting changes in cortical thickness. Specifically, a greater frequency of high magnitude head impacts during the football season resulted in greater cortical thinning for the LRank group, but not for the HRank group. These data provide preliminary in vivo evidence that HRank helmets may provide a buffer between the specific effect of high magnitude head impacts on regional thinning by dissipating forces more evenly throughout the cortex. However, future research with larger sample sizes, increased longitudinal measures and additional helmet technologies is warranted to both expand upon and further validate the present study findings.

Abstract:This review paper summarizes the scientific advancements in the field of concussion biomechanics in American football throughout the past five decades. The focus is on-field biomechanical data collection, and the translation of that data to injury metrics and helmet evaluation. On-field data has been collected with video analysis for laboratory reconstructions or wearable head impact sensors. Concussion biomechanics have been studied across all levels of play, from youth to professional, which has allowed for comparison of head impact exposure and injury tolerance between different age groups. In general, head impact exposure and injury tolerance increase with increasing age. Average values for concussive head impact kinematics are lower for youth players in both linear and rotational acceleration. Head impact data from concussive and non-concussive events have been used to develop injury metrics and risk functions for use in protective equipment evaluation. These risk functions have been used to evaluate helmet performance for each level of play, showing substantial differences in the ability of different helmet models to reduce concussion risk. New advances in head impact sensor technology allow for biomechanical measurements in helmeted and non-helmeted sports for a more complete understanding of concussion tolerance in different demographics. These sensors along with advances in finite element modeling will lead to a better understanding of the mechanisms of injury and human tolerance to head impact.

Abstract:In this study, to discuss the influence of concussion risk from the long-term use of American football helmets on collegiate teams, accident cases during the game are replicated based on game videos by simulations using whole-body numerical models and helmeted finite element human head models. The concussion risks caused by collisions were estimated using the mechanical parameters inside the skull obtained from finite element analyses. In the analyses, the different material properties of helmets identified by free-fall experiments using headform impactor-embedded helmets were used to represent brand-new and long-term-use helmets. After analyzing the five cases, it was observed that wearing a new helmet instead of a long-term-use one resulted in a reduction in the risk of concussion by 1 to 44%. More energy is attenuated by the deformation of the liners of the brand-new helmet, so the energy transferred to the head is smaller than that when wearing the long-term-use helmet. Thus, the long-term use of the helmet reduces its ability to protect the head.

Abstract:Concussion and repeated exposure to mild traumatic brain injury are risks for athletes in many sports. While direct head impacts are analyzed to improve the detection and awareness of head acceleration events so that an athlete's brain health can be appropriately monitored and treated. However, head accelerations can also be induced by impacts with little or no head involvement. In this work we evaluated if impacts that do not involve direct head contact, such as being pushed in the torso, can be sufficient in collegiate American football to induce head accelerations comparable to direct head impacts. Datasets of impacts with and without direct head contact were collected and compared. These datasets were gathered using a state-of-the-art impact detection algorithm embedded in an instrumented mouthguard to record head kinematics. Video analysis was used to differentiate between impact types. In total, 15 impacts of each type were used in comparison, with clear video screenshots available to distinguish each impact type. Analysis of the kinematics showed that the impacts without direct head contact achieved similar levels of linear and angular accelerations during impact compared to those from direct head impacts. Finite element analyses using the median and peak kinematic signals were used to calculate maximum principal strain of the brain. Statistical analysis revealed that no significant difference was found between the two datasets based on a Bonferroni-adjusted p-value threshold of 0.017 , with the exception of peak linear acceleration. Impacts without direct head contact showed higher mean values of peak linear acceleration values of 17.6 g compared to the direct-head impact mean value of 6.1g. These results indicated that impacts other than direct head impacts could still produce meaningful kinematic loads in the head and as such should be included in athlete health monitoring.

Abstract:The purpose of this research was to examine how four different types of baseball helmets perform for baseball impacts when performance was measured using variables associated with concussion. A helmeted Hybrid III headform was impacted by a baseball, and linear and rotational acceleration as well as maximum principal strain were measured for each impact condition. The method was successful in distinguishing differences in design characteristics between the baseball helmets. The results indicated that there is a high risk of concussive injury from being hit by a ball regardless of helmet worn.

Abstract:ObjectivesThe aims were to (1) examine the rates and mechanisms of concussion and head impact in youth football (high school level or younger); (2) identify modifiable risk factors for concussion and head impact; and (3) evaluate the effectiveness of prevention strategies in tackle football at any level.MethodsNine databases (CINAHL Plus with Full Text; Cochrane Central Register of Controlled Trials; EMBASE; ERIC; Ovid MEDLINE(R) and Epub Ahead of Print, In-Process & Other Non-Indexed Citations and Daily; ProQuest Dissertations & Theses Global Database; PsycINFO; Scopus; and SPORTDiscus with Full Text) were searched using the search strategy focusing on four main concepts: concussion/head impact, tackle football, modifiable risk factors, and primary prevention. Two reviewers completed title, abstract, and full-text screening as well as risk of bias assessment (using the Downs and Black checklist), with a third author available to resolve any disagreements.Main ResultsAfter removing duplicates, 1911 articles were returned. Fifty-eight articles were included in the review and 20 in the meta-analysis. The overall combined rates of concussion (including game and practice-related concussion) based on the meta-analysis were 0.78 concussions/1000 athlete exposures [95% confidence interval (CI) 0.67–0.89] for high school football (ages 13–19) and 1.15 concussions/1000 athlete exposures (95% CI 0.89–1.41) for minor football players (ages 5–15). There is evidence that contact training and practice contact restrictions have reduced the rate of head impacts and concussion. Heads Up Football (an intervention focused on coach education and contact training) has been shown to reduce the rate of concussion by 32% and head impacts by 38% amongst high school football players. Limiting contact practices in high schools to 2 days per week reduced practice head impacts per player-season by 42%, and limiting full contact in practice to 75 min per week in the second week of the season and 60 min in week 3 and beyond resulted in a 54% decrease in the practice-related concussion rate (p = 0.003).ConclusionsThis review identified a critical need for interventions to address the high rates of concussion and head impact in youth football. To date, contact training and contact restrictions have the strongest evidence supporting their effectiveness at reducing these rates. Future research should use consistent concussion definitions and validated injury surveillance systems, and ensure complete reporting of participant characteristics and sampling details.Prospero ID CRD42020193775.

Padded Helmet Shell Covers in American Football: A Comprehensive Laboratory Evaluation with Preliminary On-Field Findings

Abstract:Protective headgear effects measured in the laboratory may not always translate to the field. In this study, we evaluated the impact attenuation capabilities of a commercially available padded helmet shell cover in the laboratory and field. In the laboratory, we evaluated the efficacy of the padded helmet shell cover in attenuating impact magnitude across six impact locations and three impact velocities when equipped to three different helmet models. In a preliminary on-field investigation, we used instrumented mouthguards to monitor head impact magnitude in collegiate linebackers during practice sessions while not wearing the padded helmet shell covers (i.e., bare helmets) for one season and whilst wearing the padded helmet shell covers for another season. The addition of the padded helmet shell cover was effective in attenuating the magnitude of angular head accelerations and two brain injury risk metrics (DAMAGE, HARM) across most laboratory impact conditions, but did not significantly attenuate linear head accelerations for all helmets. Overall, HARM values were reduced in laboratory impact tests by an average of 25% at 3.5 m/s (range: 9.7 - 39.6%), 18% at 5.5 m/s (range: -5.5 - 40.5%), and 10% at 7.4 m/s (range: -6.0 - 31.0%). However, on the field, no significant differences in any measure of head impact magnitude were observed between the bare helmet impacts and padded helmet impacts. Further laboratory tests were conducted to evaluate the ability of the padded helmet shell cover to maintain its performance after exposure to repeated, successive impacts and across a range of temperatures. This research provides a detailed assessment of padded helmet shell covers and supports the continuation of in vivo helmet research to validate laboratory testing results.

Video Analysis of Reported Concussion Events in the National Football League During the 2015-2016 and 2016-2017 Seasons

Abstract:Background: Concussions in American football remain a high priority of sports injury prevention programs. Detailed video review provides important information on causation, the outcomes of rule changes, and guidance on future injury prevention strategies. Purpose: Documentation of concussions sustained in National Football League games played during the 2015-2016 and 2016-2017 seasons, including consideration of video views unavailable to the public. Study design: Descriptive epidemiology study. Methods: All reported concussions were reviewed with all available video footage. Standardized terminology and associated definitions were developed to describe and categorize the details of each concussion. Results: Cornerbacks sustained the most concussions, followed by wide receivers, then linebackers and offensive linemen. Half (50%) of concussions occurred during a passing play, 28% during a rushing play, and 21% on a punt or kickoff. Tackling was found to be the most common activity of concussed players, with the side of the helmet the most common helmet impact location. The distribution of helmet impact source-the object that contacted the concussed player's helmet-differed from studies of earlier seasons, with a higher proportion of helmet-to-body impacts (particularly shoulder) and helmet-to-ground impacts and with a lower proportion of helmet-to-helmet impacts. Helmet-to-ground concussive impacts were notable for the high prevalence of impacts to the back of the helmet and their frequency during passing plays. Conclusion: Concussion causation scenarios in the National Football League have changed over time. Clinical relevance: The results of this study suggest the need for expanded evaluation of concussion countermeasures beyond solely helmet-to-helmet test systems, including consideration of impacts with the ground and with the body of the opposing player. It also suggests the possibility of position-specific countermeasures as part of an ongoing effort to improve safety.

Abstract:Background: Girls’ lacrosse headgear that met the ASTM International performance standard (ASTM F3137) became available in 2017. However, the effects of headgear use on impact forces during game play are unknown. Purpose: To evaluate potential differences in rates, magnitudes, and game-play characteristics associated with verified impacts among players with and without headgear during competition. Study Design: Cohort study; Level of evidence, 3. Methods: A total of 49 female high school participants (mean age, 16.2 ± 1.2 years; mean height, 1.66 ± 0.05 m; mean weight, 61.2 ± 6.4 kg) volunteered for this study, which took place during the 2016 (no headgear; 18 games) and 2017 (headgear; 15 games) seasons. Wearable sensors synchronized with video verification were used. Descriptive statistics, impact rates, and chi-square analyses described impacts and game-play characteristics among players with and without headgear. Differences in mean peak linear acceleration (PLA) and peak rotational velocity (PRV) between the no headgear and headgear conditions were evaluated using a linear generalized estimating equation regression model to control for repeated within-player measurements. Results: Overall, 649 sensor-instrumented player-games were recorded. A total of 204 impacts ≥20 g recorded by the wearable sensors were verified with video analysis (102 no headgear; 102 headgear). Most impacts were imparted to the player’s body (n = 152; 74.5%) rather than to the player’s head (n = 52; 25.5%). Impact rates per player-game did not vary between the no headgear and headgear conditions (0.30 vs 0.34, respectively; impact rate ratio, 0.88 [95% CI, 0.37-2.08]). There was no association between impact frequency by mechanism or penalties administered between the no headgear and headgear conditions for overall or direct head impacts. The generalized estimating equation model estimated a significant reduction in mean impact magnitudes overall (PLA: –7.9 g [95% CI, –13.3 to –2.5]; PRV: –212 deg/s [95% CI, –359 to –64]) with headgear relative to no headgear. No game-related concussions were reported during this study. Conclusion: Lacrosse headgear use was associated with a reduction in the magnitude of overall impacts but not a significant change in the rate of impacts, how they occur, or how penalties were administered for impacts sustained during competition. Further research is needed with a larger sample and different levels of play to evaluate the consequences of headgear use in girls’ lacrosse.

Abstract:Opinion statementHead and spinal injuries in collision sports such as American football, ice hockey, and lacrosse affect millions of youth and adolescent athletes in the USA. Strategies to reduce sport-related concussion and catastrophic spinal cord injuries have increased over the past few decades, with rule changes limiting body checking at younger levels of youth ice hockey and bans on spear tackling in American football. Until recently, sports medicine communities have focused their efforts on providing consensus statements and guidelines pertaining to the detection, evaluation, and management of sport-related concussion. Many of these recommendations have extrapolated data from adult studies as limited research is available on youth athletes. There is a movement among sports medicine clinicians and researchers to collaborate with youth sports organizations to provide scientific data and answers to questions pertaining to youth collision sport injury. Changes and advances in protective equipment have not resulted in a dramatic decline in collision sport head injuries. But several strategies hold promise, such as teaching proper body checking and tackling technique; establishing and enforcing rules that limit head contact and intent-to-harm hits to vulnerable players; implementing participation strategies determined by age, size, and physical maturity level; and introducing injury prevention strategies to reduce head injuries. By administering these interventions collectively, a safer environment for our youth and adolescent collision sport athletes to engage in healthy competition can be provided. In addition, evaluation of our collective intense sport culture is needed as societal pressures are also likely contributing to injuries and unintended consequences. Pediatricians play a vital role in advocating for the safety of youth collision sport athletes as medical providers, parents, coaches, and as board members and in advisory positions of youth sports organizations.
Evaluation of a laboratory model of human head impact biomechanics

Abstract:This work describes methodology for evaluating laboratory models of head impact biomechanics. Using this methodology, we investigated: how closely does twin-wire drop testing model head rotation in American football impacts? Head rotation is believed to cause mild traumatic brain injury (mTBI) but helmet safety standards only model head translations believed to cause severe TBI. It is unknown whether laboratory head impact models in safety standards, like twin-wire drop testing, reproduce six degree-of-freedom (6DOF) head impact biomechanics that may cause mTBI. We compared 6DOF measurements of 421 American football head impacts to twin-wire drop tests at impact sites and velocities weighted to represent typical field exposure. The highest rotational velocities produced by drop testing were the 74th percentile of non-injury field impacts. For a given translational acceleration level, drop testing underestimated field rotational acceleration by 46% and rotational velocity by 72%. Primary rotational acceleration frequencies were much larger in drop tests (~100 Hz) than field impacts (~10 Hz). Drop testing was physically unable to produce acceleration directions common in field impacts. Initial conditions of a single field impact were highly resolved in stereo high-speed video and reconstructed in a drop test. Reconstruction results reflected aggregate trends of lower amplitude rotational velocity and higher frequency rotational acceleration in drop testing, apparently due to twin-wire constraints and the absence of a neck. These results suggest twin-wire drop testing is limited in modeling head rotation during impact, and motivate continued evaluation of head impact models to ensure helmets are tested under conditions that may cause mTBI.

Abstract:Because of the relatively rigid coupling between the upper dentition and the skull, instrumented mouthguards have been shown to be a viable way of measuring head impact kinematics for assisting in understanding the underlying biomechanics of concussions. This has led various companies and institutions to further develop instrumented mouthguards. However, their use as a research tool for understanding concussive impacts makes quantification of their accuracy critical, especially given the conflicting results from various recent studies. Here we present a study that uses a pneumatic impactor to deliver impacts characteristic to football to a Hybrid III headform, in order to validate and compare five of the most commonly used instrumented mouthguards. We found that all tested mouthguards gave accurate measurements for the peak angular acceleration (mean relative error, MRE < 13%), the peak angular velocity (MRE < 8%), brain injury criteria values (MRE < 13%) and brain deformation (described as maximum principal strain and fiber strain, calculated by a convolutional neural network based brain model, MRE < 9%). Finally, we found that the accuracy of the measurement varies with the impact locations yet is not sensitive to the impact velocity for the most part.

Incorporating neck biomechanics in helmet testing: Evaluation of commercially available WaveCel helmets

Abstract:Background: Cycling helmets often incorporate elements aimed to dissipate rotational energies, which is widely acknowledged to play a key role in concussion mechanics. In this study, we investigated the mechanics of an oblique helmet test protocol that induced helmet rotation while using it to evaluate the effectiveness of three helmet models: two standard expanded polystyrene helmets and a commercially-available helmet equipped with a liner designed to mitigate linear and rotational energies. Methods: Helmets equipped with WaveCel were tested against two expanded polystyrene helmet models through guided drops using a Hybrid III (HIII) head-and-neck surrogate. The three helmet models were tested across four impact conditions (n = 5) of different speeds and impact surface angles. Findings: Across all tests, a similar sequence of head motion was observed - first a flexion phase followed by an extension phase. The extension phase lacked evidence of biofidelity and was likely attributable to the energy stored in the neckform during the flexion phase; it was therefore neglected from analysis. Results showed WaveCel reduced the probability of AIS2 head injury across all tests (3 to 27% reductions in 4.8 m/s impacts; 36 to 37% reductions in 6.2 m/s impacts). Interpretation: The two-phased response of the HIII suggests that boundary condition selection can influence results and should thus be reported in studies using similar methods. While this protocol involved both axial and tangential impact components and were thus representative of real-world collisions, the efficacy of WaveCel should be further investigated through additional laboratory studies and tracking real-world cycling injury statistics.
Toward reducing impact induced brain injury: Lessons from a computational study of army and football helmet pads

Abstract:We use computational simulations to compare the impact response of different football and U.S. Army helmet pad materials. We conduct experiments to characterize the material response of different helmet pads. We simulate experimental helmet impact tests performed by the U.S. Army to validate our methods. We then simulate a cylindrical impactor striking different pads. The acceleration history of the impactor is used to calculate the Head Injury Criterion for each pad. We conduct sensitivity studies exploring the effects of pad composition, geometry, and material stiffness. We find that: (1) The football pad materials do not outperform the currently used military pad material in militarily-relevant impact scenarios; (2) Optimal material properties for a pad depend on impact energy; and (3) Thicker pads perform better at all velocities. Our analysis suggests that by using larger helmet shells with correspondingly thicker pads, impact-induced traumatic brain injury may be significantly reduced. A comparison of sub-concussive impact attenuating capabilities of ice hockey helmets with and without XRD foam

Abstract:Objective To compare the impact attenuating capabilities between ice hockey helmets manufactured with and without XRD impact protection foam, worn with and without a XRD skullcap, at reducing sub-concussive head accelerations. Design Quasi-experimental laboratory. Methods Ice hockey helmets were fit onto a Hybrid III 50th Head Form Head and dropped 25 times onto the left temporal side for each condition: XRD foam helmet, XRD foam helmet with XRD skullcap adjunct, non XRD foam helmet, and non XRD foam helmet with XRD skullcap adjunct. The helmets were dropped from a height that resulted in sub-concussive linear accelerations (25–80 g's). Using a tri-axial accelerometer, peak linear accelerations (g) were measured, and the average was used to compare impact attenuation properties across the four conditions. Results The highest linear accelerations were observed in the XRD foam helmet without skullcap (32.97 ± 0.61 g) and were significantly greater ( p < 0.001) than the XRD helmet with skullcap (21.38 ± 0.76 g). The helmet without XRD foam elicited the lowest peak linear accelerations (16.10 ± 0.73 g) which were significantly lower than the XRD foam helmet regardless of whether the skullcap was added ( p < 0.001). Conclusions Although sub-concussive loads are potentially just as dangerous, much of the research regarding helmet and skullcap efficacy appears to be at high concussive impacts; < 70 g's. The findings suggest that helmets with incorporated XRD foam, either within the design or added as an adjunct, are less effective at attenuating linear accelerations at sub-concussive levels than the low-density foam helmet.

Head Impact Exposures Among Youth Tackle and Flag American Football Athletes

Abstract:Background: Promoted as a safer alternative to tackle football, there has been an increase in flag football participation in recent years. However, examinations of head impact exposure in flag football as compared with tackle football are currently limited. Hypothesis: Tackle football athletes will have a greater number and magnitude of head impacts compared with flag football athletes. Study Design: Cohort study. Level of Evidence: Level 4. Methods: Using mouthguard sensors, this observational, prospective cohort study captured data on the number and magnitude of head impacts among 524 male tackle and flag football athletes (6-14 years old) over the course of a single football season. Estimates of interest based on regression models used Bayesian methods to estimate differences between tackle and flag athletes. Results: There were 186,239 head impacts recorded during the study. Tackle football athletes sustained 14.67 (95% CI 9.75-21.95) times more head impacts during an athletic exposure (game or practice) compared with flag football athletes. Magnitude of impact for the 50th and 95th percentile was 18.15 g (17.95-18.34) and 52.55 g (51.06-54.09) for a tackle football athlete and 16.84 g (15.57-18.21) and 33.51 g (28.23-39.08) for a flag football athlete, respectively. A tackle football athlete sustained 23.00 (13.59-39.55) times more high-magnitude impacts (≥40 g) per athletic exposure compared with a flag football athlete. Conclusion: This study demonstrates that youth athletes who play tackle football are more likely to experience a greater number of head impacts and are at a markedly increased risk for high-magnitude impacts compared with flag football athletes. Clinical Relevance: These results suggest that flag football has fewer head impact exposures, which potentially minimizes concussion risk, making it a safer alternative for 6- to 14-year-old youth football athletes.
