Abstract:Objective To evaluate the advantages and disadvantages of coblation,ultrasonic knife and conventional ap-proach in tonsillectomy.Methods A total of 1 17 cases in need of tonsillectomy were collected and divided into 3 groups:coblation tonsillectomy group (group A,n =39),ultrasound knife tonsillectomy group (group B,n =39)and the traditional tonsillectomy group (group C,n =39).The operation time,blood loss,postoperative sore throat situa-tion,postoperative primary and secondary bleeding and so on were recorded and compared.Results The operation time was shorter,and intraoperative blood loss was less in group A and B than in group C (P <0.05).The operation time in group A [(9.92 ±4.04)min]was shorter than that [(15.86 ±1.36)min]in group B,(P <0.05).There was no difference in intraoperative blood loss between group A and B (P >0.05).The degree of the pain in the first three days after surgery in group A and in group B was significantly lower than that in group C (P <0.05),but no sta-tistically significant differences were observed after three days (P >0.05).Postoperative throat pain in group A was lighter than that in group B,but there was no significant difference (P >0.05 ).Postoperative primary hemorrhage occurred in 1 case in group A and C,while secondary hemorrhage occurred in 1,1 and 2 cases in group A,B and C, respectively.There was no significant difference in the time of pseudo-membrane defulvium among the three groups (P >0.05).Pharyngeal edema score 24 hours after operation in group A and group B was significantly lower than in group C (P <0.05),but there was no statistically significant difference between group A and group B (P >0.05). Conclusion Coblation tonsillectomy is the more safe and effective method,but its cost is relatively high.It is suitable for patients with good economic conditions and high quality of life.Ultrasonic knife tonsillectomy is suggested to be appropriate for patients with adhesive tonsillitis of long duration.

Abstract:Objectives: To compare the safety and effectiveness of tonsillectomy with three different techniques. Design: A double-blinded randomized prospective clinical trial Setting and Participants: Totally120 patients with recurrent tonsillitis between April 2018 and April 2020 were included. Main Outcome Measures: Operative time, intra and post operative bleeding loss, pseudomembrane growth time, pseudomembrane shedding time, postoperative pain, and necrosis depth of specimens were compared of harmonic scalpel(HS), coblation and cold dissection(CD) tonsillectomy. Results: The operative time and intraoperative bleeding loss was significantly less in HS and coblation group than CD group(p < 0.05). Pseudomembrane growth time and pseudomembrane shedding time were significantly longer in the HS group compared with the other groups(p < 0.05). The postoperative pain levels were significantly higher on the first postoperative day in CD group(p < 0.05), and significantly lower on the third and seventh postoperative day in coblation group(p < 0.05). There was no significant difference among three techniques in terms of postoperative bleeding loss(p > 0.05). The deepest necrosis depth in specimen was found in HS group(p > 0.05), while in CD group, only edema was observed without necrosis. Conclusion: To compared with HS and CD, coblation is a faster, safer and more painless technique for tonsillectomy in adult.
Thermal welding versus bipolar tonsillectomy: a comparative study

Abstract:Objective: To compare thermal welding tonsillectomy (TWT) with bipolar electrocautery tonsillectomy (BET) procedure. Study design and setting: A prospective randomized study was conducted on 150 consecutive adult patients undergoing tonsillectomy. Indications included chronic tonsillitis and obstructive sleep apnea syndrome. Exclusion criteria included peritonsillar abscess history, bleeding disorders, and any other procedure together with tonsillectomy. Patients were randomly assigned to TWT or BET groups. Intraoperative bleeding, operative time, postoperative pain, complication rates, and return to normal diet were evaluated. Results: In the TWT group there was no measurable intraoperative bleeding, while mean bleeding for BET group was 16 mL. No significant difference regarding mean operative time was noticed. Mean postoperative pain score and mean time for return to normal diet were significantly lower in the TWT group. Primary hemorrhage occurred in 1 subject of the BET group. Secondary postoperative hemorrhage was noticed in 1 subject of the TWT group and 3 subjects of the BET group. Conclusion: Thermal welding tonsillectomy procedure provides sufficient hemostasis, lower postoperative pain, and quick return to normal diet. Ebm rating: A-1b.

Abstract:Objectives To analyse operating time, intraoperative blood loss, postoperative bleeding rate and pain when using the relatively new BiZactTM tonsillectomy device compared to the commonly used cold steel dissection technique with bipolar cautery in adults. Design Retrospective case control study. Parameters analysed for significant association with technique were operating time, intraoperative blood loss, wound pain on postoperative days 1–4 and rate of post‐tonsillectomy bleeding (PTB). Setting Monocentric study at a department of otolaryngology and head and neck surgery at a tertiary centre in Germany. Participants A total of 183 patients who underwent a bilateral tonsillectomy with either the BiZactTM tonsillectomy device or the cold dissection technique with bipolar cautery for haemostasis. Main Outcome Measures Operating time, intraoperative blood loss, postoperative pain on the first to fourth postoperative day (numeric rating scale: 0–10) (PTB, primary bleeding ≤24 h, secondary bleeding >24 h postoperative; Stammberger scale). Results and Conclusion The BiZactTM tonsillectomy device leads to a significant shorter operating time with less intraoperative blood loss compared to cold steel dissection with bipolar haemostasis. No benefits with regards to PTB or postoperative pain could be observed. The use of the BiZactTM device provides major benefits in clinical routine and stands up to conventional tonsillectomy techniques.

Paediatric tonsillectomy: radiofrequency-based plasma dissection compared to cold dissection with sutures

Abstract:Aim of this study was to compare post-operative recovery over 14 days in children submitted to tonsillectomy using a bipolar radiofrequency-based plasma device (Coblation, Evac 70, ArthroCare Corp, Sunnyvale, CA, USA) to cold dissection. Paediatric patients (n = 42) aged 5-16 years old with chronic tonsillitis underwent tonsillectomy using cold dissection with suture ligatures or a plasma device (Evac 70, ArthroCare Corp, Sunnyvale, CA, USA). Pain intensity on the first day, use of analgesics, type of diet, and days of pain, fever, nausea, and absence from school were determined. Groups were compared using time-to-event (Kaplan-Meier) curves and statistically evaluated using the Breslow (generalized Wilcoxon) test. Children undergoing plasma tonsillectomy reported significantly less pain on the first post-operative day (1.2 +/- 0.9 vs. 3.5 +/- 1.5, p < 0.001), fewer days of pain (4.8 +/- 1.5 vs. 9.4 +/- 1.2, p <0.001), pain medication withdrawal (2.6 +/- 1.3 vs. 4.5 +/- 1.3, p <0.001) and earlier use of liquid diet (5.1 +/- 1.4 vs. 8.5 +/- 2.1, p <0.001), and fewer school days lost (5.3 +/- 1.7 vs. 8.9 +/- 1.5, p <0.001). After completing this study, plasma tonsillectomy was adopted for the majority of cases. Benefits of the plasma device include the possibility both to excise tissue and coagulate bleeding vessels using the same device whilst improving quality of post-operative recovery over cold dissection with suture ligatures.

Abstract:OBJECTIVE:Compare coblation tonsillectomy with blunt dissection tonsillectomy to see if there is any difference in intraoperative bleeding, operating time, postoperative pain, tonsillar fossae healing and return to normal diet and activities.METHOD:A randomised control trial was carried out to compare coblation tonsillectomy with blunt dissection tonsillectomy in 56 patients. The same surgeon operated on them all, using Arthro Care EVac 70 Wand for coblation tonsillectomy, and routine instruments for blunt dissection tonsillectomy.RESULT:Coblation tonsillectomy was fast and resulted in little intraoperative bleeding. Significant reduction in postoperative pain was found in the patients with coblation tonsillectomy. Patients who had coblation tonsillectomy returned to their normal diet sooner than those who underwent blunt dissection tonsillectomy. Slower healing of the tonsillar fossae was found in the coblation group. One patient in the coblation group suffered from secondary haemorrhage.CONCLUSION:Coblation tonsillectomy is easy to perform with little intraoperative bleeding. It offers significant advantages in the postoperative period, with rapid return to a normal diet and reduction in postoperative pain. But it need more time for healing of the tonsillar fossae.

Abstract:Background Tonsillitis is a vastly prevalent disease, accounting for the majority of outpatient visits. The dissection and snare method has been the predominant approach for tonsillectomy for centuries. Coblation-assisted tonsillectomy offers advantages such as faster healing, shorter surgery duration, minimal blood loss, and fewer postoperative complications. Therefore, a study was conducted to evaluate the distinctions and compare the efficacy of traditional dissection and coblation-assisted tonsillectomy. Materials and methods Patients were divided into two groups: Group I was operated on using the conventional method, and Group II was operated on using the coblation method. Both groups were then assessed for intraoperative time, intraoperative bleeding, postoperative pain, and postoperative complications. Results Coblation-assisted tonsillectomy patients had a significantly shorter mean duration for the procedure and significantly lower blood loss in comparison to the conventional method. There were no statistically significant variations in the incidence of postoperative complications. Coblation-assisted tonsillectomy patients experienced considerably higher pain scores on various postoperative days. Conclusion Coblation-assisted tonsillectomy had the added advantage of reduced intraoperative blood loss, shorter surgical time, and faster recovery postoperatively. Coblation-assisted tonsillectomy can be considered an effective alternative to conventional methods. However, it's important to consider factors such as cost-effectiveness and surgeon experience. Further research involving larger sample sizes and longer follow-up periods could yield more insightful knowledge of the outcomes of these two tonsillectomy techniques.

Abstract:Objective: Tonsillectomy has been described using a number of techniques. Recently Coblation Technology has been used to remove tonsils with anecdotal evidence of a reduction in post-operative morbidity. In this study we aim to see if there is any difference in post-operative pain, tonsillar fossae healing and return to a normal diet performing tonsillectomy, using tissue coblation compared with standard bipolar dissection. Methods: A double blind randomised control trial to compare the technique of tissue coblation with standard bipolar dissection to remove tonsils in 38 children on the waiting list for tonsillectomy, with a history of chronic tonsillitis or obstructive tonsils. Results: A significant reduction in post-operative pain was found in the children whose tonsils were removed by tissue coblation (P<0.0001). More rapid healing of the tonsillar fossae was found in the coblation group. Children who had their tonsils removed by coblation were found to return to their normal diet far sooner than those who underwent bipolar dissection. There were no episodes of primary or secondary haemorrhage in either group. Conclusions: This new technique using tissue coblation for tonsil removal offers significant advantages in the post-operative period, with rapid return to a normal diet and a drastic reduction in analgesic requirements following the surgery.

Abstract:Background: Tonsillectomy is a very common operation and is performed using various surgical methods. Coblation is a popular method because it purportedly causes less pain than other surgical methods. However, the superiority of coblation is unproven. Objectives: To compare the effects of coblation tonsillectomy for chronic tonsillitis or tonsillar hypertrophy with other surgical techniques, both hot and cold, on intraoperative morbidity, postoperative morbidity and procedural cost. Search methods: The Cochrane ENT Information Specialist searched the ENT Trials Register; Central Register of Controlled Trials (CENTRAL 2017, Issue 3); PubMed; Ovid Embase; CINAHL; Web of Science; ClinicalTrials.gov; ICTRP and additional sources for published and unpublished trials. The date of the search was 20 April 2017. Selection criteria: Randomised controlled trials (RCTs) of children and adults undergoing tonsillectomy with coblation compared with any other surgical technique. This review is limited to trials of extracapsular (traditional) tonsillectomy and excludes trials of intracapsular tonsil removal (tonsillotomy). Data collection and analysis: We used the standard Cochrane methods. Our primary outcomes were: patient-reported pain using a validated pain scale at postoperative days 1, 3 and 7; intraoperative blood loss; primary postoperative bleeding (within 24 hours) and secondary postoperative bleeding (more than 24 hours after surgery). Secondary outcomes were: time until resumption of normal diet, time until resumption of normal activity, duration of surgery and adverse effects including blood transfusion and the need for reoperation. We used GRADE to assess the quality of the evidence for each outcome; this is indicated in italics. Main results: We included 29 studies, with a total of 2561 participants. All studies had moderate or high risk of bias. Sixteen studies used an adequate randomisation technique, however the inability to mask the surgical teams and/or provide adequate methods to mitigate the risk of bias put nearly all studies at moderate or high risk of detection and measurement bias for intraoperative blood loss, and primary and secondary bleeding. In contrast most studies (20) were at low risk of bias for pain assessment. Most studies did not report data in a manner permitting meta-analysis.Most studies did not clearly report the participant characteristics, surgical indications or whether patients underwent tonsillectomy or adenotonsillectomy. Most studies reported that tonsillitis (infection) and/or tonsillar hypertrophy (obstruction) were the indication for surgery. Seven studies included only adults, 16 studies included only children and six studies included both. Pain At postoperative day 1 there is very low quality evidence that patients in the coblation group had less pain, with a standardised mean difference (SMD) of -0.79 (95% confidence interval (CI) -1.38 to -0.19; 538 participants; six studies). This effect is reduced a SMD of -0.44 (95% CI -0.97 to 0.09; 401 participants; five studies; very low-quality evidence) at day 3, and at day 7 there is low quality evidence of little or no difference in pain (SMD -0.01, 95% CI -0.22 to 0.19; 420 participants; five studies). Although this suggests that pain may be slightly less in the coblation group between days 1 and 3, the clinical significance is unclear. Intraoperative blood loss Methodological differences between studies in the measurement of intraoperative blood loss precluded meta-analysis. Primary and secondary bleeding The risk of primary bleeding was similar (risk ratio (RR) 0.99, 95% CI 0.48 to 2.05; 2055 participants; 25 studies; low-quality evidence). The risk of secondary bleeding was greater in the coblation group with a risk ratio of 1.36 (95% CI 0.95 to 1.95; 2118 participants; 25 studies; low-quality evidence). Using the median of the control group as the baseline risk, the absolute risk in the coblation group was 5% versus 3.6% in the control group. The difference of 1.3% has a 95% CI of 0.2% lower in the coblation group to 3.5% higher. Secondary outcomes Differences in study design and data reporting precluded the identification of differences in the time to resumption of normal diet or activity, or whether there was a difference in the duration of surgery.Although we could not feasibly compare the costs of equipment or operative facility, anaesthetic and surgical fees across different healthcare systems we used duration of surgery as a proxy for cost. Although this outcome was commonly reported in studies, it was not possible to pool the data to determine whether there was a difference.Adverse events other than bleeding were not well reported. It is unclear whether there is a difference in postoperative infections or the need for reoperation. Authors' conclusions: The coblation technique may cause less pain on postoperative day 1, but the difference is small and may be clinically meaningless. By postoperative day 3, the difference decreases further and by postoperative day 7 there appears to be little or no difference. We found similar rates of primary bleeding but we cannot rule out a small increased risk of secondary bleeding with coblation. The evidence supporting these findings is of low or very low quality, i.e. there is a very high degree of uncertainty about the results. Moreover, for most outcomes data were only available from a few of the 29 included studies.The current evidence is of very low quality, therefore it is uncertain whether or not the coblation technique has any advantages over traditional tonsillectomy techniques. Despite the large number of studies, failure to use standardised or validated outcome measures precludes the ability to pool data across studies. Therefore, well-conducted RCTs using consistent, validated outcome measures are needed to establish whether the coblation technique has a benefit over other methods. In the included studies we identified no clear difference in adverse events. However, given the rarity of these events, randomised trials lack the power to detect a difference. Data from large-scale registries will provide a better estimate of any difference in these rare outcomes.

Abstract:Objective: To compare the clinical outcomes of minimally invasive right subaxillary vertical thoracotomy and traditional median stemotomy through right atrium in treatment of common congenital heart diseases. Methods: Clinical data of 59 cases of common congenital heart diseases treated with minimally invasive right axillary vertical thoracotomy from May, 2011 to February, 2013 and 77 cases of same diseases with traditional median stemotomy in the past three years were retrospectively analyzed, including atrial septal defect, membranous ventricular septal defect and partial endocardial cushion defect. The results were compared from the two groups, including the time for operation and cardiopulmonary bypass, amount of blood transfusion, postoperative drainage, ventilation time, hospital stay, and prognosis. Results: No severe complications happened in both groups, like deaths or secondary surgery caused by bleeding. No significant differences were in CPB time and postoperative ventilator time between groups (P>0.05), while for all of the operative time, the length of incision, postoperative drainage and hospital stay, minimally invasive right axillary vertical thoracotomy was superior to median stemotomy, with statistically significant differences (P<0.05). In six month followup after operation, no complications of residual deformity and pericardial effusion were found in both groups by doing echocardiography, but mild pectus carinatum was found in 8 patients in the traditional median sternotomy group (traditional group), whereas patients in another group were well recovered. Conclusions: Minimally invasive right subaxillary vertical thoracotomy for common congenital heart diseases is as safe as traditional median stemotomy, without the increasing incidence of postoperative complications. Additionally, compared with traditional median stemotomy, minimally - invasive right subaxillary vertical thoracotomy is better in the aspects of hidden incision, appearance, and postoperative recovery.

Comparison of ropivacaine, bupivacaine and lidocaine in the management of post-tonsillectomy pain

Abstract:Objectives: To compare the efficacy of preoperative peritonsillar injection of ropivacaine, bupivacaine and lidocaine for pediatric tonsillectomy. Patients and methods: Between March 2009 and April 2012, 120 patients (66 males and 54 females) between ages of 4 and 13 years were included to the study. After informed consent was obtained from the parents, the patients were randomized into four groups. In Group 1 (31 patients, mean age 8.40 ± 4.05 years) received topical lidocaine hydrochloride with 1:100,000 epinephrine was applied to surgical bed following tonsillectomy. Group 2 (29 patients, mean age 8.15 ± 4.20 years) and group 3 (31 patients, mean age 7.75 ± 3.95 years) were administered 0.25% bupivacaine hydrochloride with 1:200,000 epinephrine and 0.5% ropivacaine respectively. In Group 4 (29 patients, mean age 8.15 ± 4.20 years) topical 0.9% saline was used. The operation time, postoperative pain, amount of intraoperative blood loss, postoperative hemorrhage and complications were assessed. The intensity of pain was scored on a visual analogue scale. The patients were followed up for 3 weeks after surgery. Results: The difference between mean operative time of the three groups against saline injected group was statistically significant (p<0.001). The difference between mean intraoperative blood loss of the lidocaine group against three groups was statistically significant (p<0.001). None of the patients in four groups experienced primary and secondary hemorrhage. The difference between mean pain score between ropovacaine and bupivacaine groups was not statistically significant (p>0.001). The difference between mean pain score of the two groups against lidocaine and saline groups was statistically significant (p<0.001). Conclusion: Ropivacaine infiltration is as effective as bupivacaine for post-tonsillectomy pain management in children. In view of potential side effects of bupivacaine-epinephrin combination, ropivacaine is a safer choice, for post-tonsillectomy pain relief.

Abstract:This study aims to compare the clinical efficacy of coblation tonsillectomy with that of conventional tonsillectomy for the treatment of chronic tonsillitis (CT). The clinical data of 87 patients with CT treated in our hospital from November 2020 to October 2022 were retrospectively analyzed, and patients were divided into two groups based on the surgical protocol. Among them, 43 patients treated with traditional tonsillectomy were set as control group, and 44 patients treated with coblation tonsillectomy were set as study group. We compared perioperative indexes, serum stress indicators (PCT, Cor, PGE2, PGF2α), inflammatory mediator levels (CRP, TNF-α, WBC, NLR), pain level (VAS score), immune function (T lymphocyte subpopulation) at different time points, and complications between the two groups. The study group had significantly lower intraoperative bleeding, shorter operation time, feeding and hospitalization time, and longer time for complete regression of yellow-white scabs than the control group ( P < 0.05). At 1 day postoperatively, the study group had significantly lower levels of serum PCT, Cor, CRP, TNF-α, WBC, and NLR than the control group, while PGE2 and PGF2α levels decreased in both groups ( P < 0.05). At 1, 3, 5, and 7 day after surgery, the VAS scores were significantly lower in the study group than in the control group ( P < 0.05). At 1 week after surgery, the study group had significantly higher levels of CD3+, CD4+, and CD4+/CD8+ and significantly lower levels of CD8+ than the control group ( P < 0.05). The complication rate in the study group (4.55%) was significantly lower than that in the control group (20.93%) ( P < 0.05). Coblation tonsillectomy is less traumatic for CT patients, effectively reduces the degree of postoperative pain, decreases stress response, suppresses inflammatory response, improves immune function, reduces complications, and promotes postoperative recovery compared with traditional tonsillectomy.

Abstract:Objective To assess the effectiveness of partial versus total tonsillectomy in children. Data Sources MEDLINE, EMBASE, and Cochrane Library from January 1980 to June 2016. Review Methods Two investigators independently screened studies and extracted data. Investigators independently assessed risk of bias and strength of evidence of the literature. Heterogeneity precluded quantitative analysis. Results In 16 eligible randomized controlled trials (RCTs), definitions of “partial” tonsillectomy varied. In addition to comparing partial with total tonsil removal, 11 studies compared surgical techniques (eg, coblation). In studies comparing the same technique, return to normal diet or activity was faster with partial removal (more favorable outcomes in 4 of 4 RCTs). In studies with differing surgical techniques, return to normal diet and activity was faster with partial versus total tonsillectomy (more favorable outcomes in 5 of 6 studies). In 3 of 4 RCTs, partial tonsillectomy was associated with more throat infections than total tonsillectomy. Differences between groups were generally not statistically significant for obstructive symptom persistence, quality of life, or behavioral outcomes. Across all studies, 10 (6%) of roughly 166 children had tonsillar regrowth after partial tonsillectomy. Conclusions Data do not allow firm conclusions regarding the comparative benefit of partial versus total removal; however, neither surgical technique nor extent of surgery appears to affect outcomes markedly. Partial tonsillectomy conferred moderate advantages in return to normal diet/activity but was also associated with tonsillar regrowth and symptom recurrence. Effects may be due to confounding given differences in populations and surgical approaches/techniques. Heterogeneity and differences in the operationalization of “partial” tonsillectomy limited comparative analyses.
Application of Bipolar Coagulation Forceps in Tonsillectomy under General Anaesthesia

Abstract:Objective To explore the effect of bipolar coagulation forceps in tonsillectomy under general anaesthesia.Methods 65 patients were randomly divided into three groups:the tonsillectomy with bipolar coagulation forceps surgery(TE-BCFS)group,the tonsillectomy with monopolar electrosurgery(TE-MES)group and the traditional blunt dissection tonsillectomy(TE-TBDT)group.The surgery duration and intra-operative blood loss were determined and compared.The postoperative pain scale,time of returning to a normal diet,and postoperative complications were also evaluated.Results Surgical time was significantly shorter,and intra-operative blood loss and postoperative complication occurrence were statistically less in the TE-BCFS group,especially in patients with obvious adhesions between tonsils and the surrounding tissues,and patients with embedded tonsil.Conclusion Under general anaesthesia,TE-BCFS offers a better effect than TE-MES and TE-TBDT do,on those patients with obvious adhesions between tonsils and the surrounding tissues,and patients with embedded tonsil.
Effect of the BiZact™ Low-Temperature Dissecting Device on Intra- and Postoperative Morbidities Related to Tonsillectomy-A Systematic Review and Meta-Analysis

Abstract:Background and Objectives: We investigated the effects of using a BiZact™ device for tonsillectomy on operating time, intraoperative blood loss, postoperative bleeding rate, and pain through a meta-analysis of the relevant literature. Materials and Methods: We reviewed studies retrieved from the databases of PubMed, SCOPUS, Google Scholar, Embase, Web of Science, and Cochrane up to March 2024. The results were analyzed following PRISMA guidelines. Six studies that compared the outcomes of patients receiving perioperative BiZact™ tonsillectomy with those in control groups (cold steel dissection or bipolar tonsillectomy) were included for this analysis of the outcomes, which included intraoperative bleeding and time, postoperative pain, and frequency of postoperative bleeding. Results: The operative time (SMD -11.5985, 95%CI [-20.3326; -2.8644], I2 = 99.5%) in the treatment group was significantly reduced compared to the control group. However, BiZact™ showed no significant efficacy in reducing intraoperative bleeding when compared with the control group (SMD -0.0480, 95%CI [-1.8200; 1.7240], I2 = 98.6%). Postoperative pain on day 1 (SMD -0.0885, 95%CI [-0.4368; 0.2598], I2 = 98.9%), day 3 (SMD -0.2118, 95%CI [-0.6110; 0.1873], I2 = 99.5%), and later than day 7 (SMD 0.0924, 95%CI [-0.2491; 0.4338], I2 = 98.6%) in the treatment group was not significantly reduced relative to the control group. When compared to the control group, BiZact™ did not reduce the incidence of secondary postoperative bleeding control in the operation room (OR 0.5711, 95%CI [0.2476; 1.3173], I2 = 32.1%), primary bleeding (OR 0.4514, 95%CI [0.0568; 3.5894], I2 = 0.0%), or all postoperative bleeding events (OR 0.8117, 95%CI [0.5796; 1.1368], I2 = 26.3%). Conclusions: This study demonstrated that using the BiZact™ device for tonsillectomy significantly decreased the operative time but could not effectively reduce intraoperative bleeding or postoperative pain and bleeding.

Abstract:This study assesses tonsillotomy (TT) versus traditional tonsillectomy (TE) for treating children with obstructive sleep‐disordered breathing (oSDB), highlighting TT's benefits in reducing postoperative complications and enhancing recovery but noting the potential for recurrent infections due to residual tonsil tissue. Despite TT's advantages in improving children's postoperative recovery and quality of life, it necessitates a balanced consideration of its short‐term benefits against the possibility of long‐term risks, including the need for reoperation. The evaluation aims to determine the efficacy and safety of TT as a viable alternative to TE, taking into account both immediate postoperative outcomes and longer‐term health implications. Objectives To evaluate the potential of tonsillotomy (TT) as an alternative to tonsillectomy (TE) for treating children with obstructive sleep‐disordered breathing (oSDB). Data Sources Two independent reviewers searched PubMed, the Cochrane Library, Embase, and additional sources to identify controlled studies comparing TT versus TE in pediatric patients with oSDB. Review Methods We evaluated post‐surgery symptom relief, and short‐ and long‐term postoperative benefits for children. Fixed‐effect meta‐analysis, sensitivity analysis, and subgroup analysis were conducted. Results In our analysis of 32 studies with 9,430 children, the analysis revealed TT group had a similar therapeutic effect on improvement of OSA‐18 as TE group (MD = 5.20, 95% CI: −32.67 to 43.07, p = 0.96). Abstract:This study aimed to assess the impact of varying monopolar diathermy power settings on postoperative pain, hemorrhage, and wound healing following tonsillectomy. A single-center, prospective, randomized, double-blinded, controlled clinical study was conducted. During bilateral tonsillectomy procedures, one tonsil received low-power settings (15 W, cutting/blend) while the other tonsil received high-power settings (35 W, cutting/blend). Postoperative pain scores (0–10) and wound healing scores (0–3) were evaluated immediately after surgery and at 1, 2, and 4 weeks postoperatively using the visual analog scale. Additionally, histological analysis was performed on electrically resected tonsil tissues to assess tissue damage in the tonsil bed. The allocation of high and low power settings to each side was randomized. Results showed that 1 week after the surgery, the high-power group experienced significantly higher pain scores (mean ± standard deviation: 4.84 ± 2.21) compared to the low-power group (3.56 ± 2.24, p = 0.049). Moreover, the high-power side exhibited slower wound healing during the initial 1–2 weeks postoperatively, as indicated by lower wound scores at 2 weeks (high-power: 1.96 ± 0.64; low-power: 2.43 ± 0.59, p = 0.008). Furthermore, histological analysis revealed significantly deeper tissue degradation on the high-power side compared to the low-power side (p < 0.001), with mean depths of 565.2 ± 291.0 μm and 156.0 ± 36.8 μm, respectively. In conclusion, these findings suggest that when employing monopolar diathermy in tonsillectomy, lower power settings can lead to improved outcomes in terms of postoperative pain, wound healing, and tissue damage.

Environmental Impact of Adult Tonsillectomy: Life Cycle Assessment and Cost Comparison of Techniques

Abstract:Objectives: To quantify and compare the cost and environmental impact of different techniques for adult tonsillectomy surgery, and to identify target areas for impact reduction. Methods: Fifteen consecutive adult tonsillectomy surgeries were prospectively randomized to one of three tonsillectomy techniques: cold, monopolar electrocautery, or low-temperature radiofrequency ablation (Coblation). Life cycle assessment was used to comprehensively evaluate the environmental impact of study surgeries. Outcomes assessed included multiple measures of environmental impact, including greenhouse gas (GHG) emissions, and cost. Environmental impact measures were analyzed to identify highest-yield areas for improvement, and outcomes were compared between surgical techniques using statistical analysis. Results: GHG emissions for cold, monopolar electrocautery, and Coblation techniques were 157.6, 184.5, and 204.7 kilograms of carbon dioxide equivalents (kgCO2 -eq) per surgery, respectively, with costs totaling $472.51, $619.10, and $715.53 per surgery, respectively. Regardless of surgery technique, anesthesia medications and disposable equipment contributed most to environmental harm. Cold technique demonstrated reduced environmental impact related to disposable surgical equipment in the categories of greenhouse gas emissions, acidification of soil and water, eutrophication of air, ozone depletion, release of carcinogenic, and non-carcinogenic toxic substances, and respiratory pollutant production (p < 0.05 for all comparisons with other techniques). Conclusion: Within the boundaries of operating room processes, cold technique minimizes cost and environmental impact of adult tonsillectomy surgery, with statistical significance noted in the impact of disposable surgical equipment. Areas of highest potential for improvement identified include reducing use of disposable equipment and collaboration with the Anesthesiology care team to streamline medication use.
