Abstract:During the last two decades, a great amount of researches have been focused on biodegradable metals. Technologies from alloy design to melting, manufacturing and processing, from microtube to stent laser processing and drug eluting coating have been improved and optimized continuously. Biodegradable metallic stent has evolved from a concept to a real product and generated three branches of material system. A large amount of animal tests and clinical tests have been carried out to investigate biodegradable magnesium stents. Results of clinical study have indicated that the magnesium stent is feasible, with favourable safety and performance outcomes. More importantly, Biotronik won CE Mark for Magmaris bioresorbable stent in 2016. Researches of biodegradable iron stents are still in the stage of animal tests. The nitrided iron stent possesses excellent mechanical properties. Results showed a good long-term biocompatibility of nitrided iron stent in rabbit and porcine model. Biodegradable zinc stent has only been introduced in recent years. Only a few in vivo studies have been reported with zinc wires implanted in rats. Results showed a good degradation behavior and biocompatibility of zinc wires. In this paper, the current research status of biodegradable metallic stents is reviewed, and the future research and development in mechanical property optimization, drug eluting and intelligence is proposed.

Abstract:In recent years, an increase in cardiovascular diseases (CVD) has been observed, constituting a global health concern, owing to a steady escalation in patient numbers. For pro-long-life services in patients, the mechanical properties, biocompatibility, and degradation behaviors of metallic vascular stents should be significantly considered during the design and manufacturing progress. In particular, biodegradable metal-based vascular stents offer initial mechanical support for cardiovascular lesions and self-degradation during vascular remodeling, thereby obviating the necessity for subsequent surgeries. Biodegradable metallic vascular stents are regarded as a prominent direction for the future development of biodegradable vascular stents (BVS), owing to their distinctive capacity for spontaneous degradation and their superior mechanical properties, which confer significant benefits upon a broader demographic afflicted by cardiovascular disease. Currently, magnesium (Mg), iron (Fe), and zinc (Zn)-based alloys are the focus of extensive research in the field of cardiovascular biodegradable materials. This paper is dedicated to delineating the benefits and limitations of three metal alloys in cardiovascular stent applications, focusing on their mechanical performance and degradation characteristics. Furthermore, a discussion on their performance within in vitro experiments, animal trials, and clinical translation is encompassed. The performance evaluation of a prospective novel biodegradable material, molybdenum (Mo), is elucidated.

Abstract:Currently, biodegradable metals for implantation applications are widely investigated to replace biodegradable polymeric implantations, which may cause inflammatory or adverse local tissue reactions. Amongst these metals, magnesium (Mg) and zinc (Zn) alloys with good biocompatibility, mechanical properties, and corrosion resistance are being widely investigated. In this review, the criteria for biodegradable vascular stents, and the advantages and limitations of biodegradable Mg and Zn vascular stents, are summarized and evaluated. By summarizing and discussing recent research on Mg- and Zn-based alloys used as biodegradable vascular stents, this review considered many alloys that can potentially serve as biodegradable vascular stents and suggests future research directions for the development of Mg- and Zn-based alloys as biodegradable metal stents.

Abstract:With the development of new technology to treat cardiovascular disease (CVD) , cardiovascular stents have drawn more and more attention as one of the most effective device in the treatment of CVD. In this paper, we mainly fo- cused on two types of biodegradable metallic vascular stents which are mostly investigated nowadays, namely magnesium alloy stents and iron-based alloy stents. We summarized the research progresses on these biodegradable metallic alloys from the viewpoint of materials science in the present days and the animal experiments, clinic trail results of their stents as well. We overviewed about the finite element design, stress analysis, mechanical property, degradation performance, bio- compatibility of the biodegradable metallic stents. Finally, we pointed out the deficiency of current biodegradable metallic stent and their future research direction.

Abstract:Highlights • The research background, interests and challenges of Fe-based bioresorbable stents has been introduced. • The design of Fe-based materials to accelerate corrosion rate, via surface and structural modification, has been reviewed. • The impact of degradation products on in vitro and in vivo biosafety has been introduced. Fe-based materials have received more and more interests in recent years as candidates to fabricate bioresorbable stents due to their appropriate mechanical properties and biocompatibility. However, the low degradation rate of Fe is a serious limitation for such application. To overcome this critical issue, many efforts have been devoted to accelerate the corrosion rate of Fe-based stents, through the structural and surface modification of Fe matrix. As stents are implantable devices, the released corrosion products (Fe 2+ ions) in vessels may alter the metabolism, by generating reactive oxygen species (ROS), which might in turn impact the biosafety of Fe-based stents. These considerations emphasize the importance of combining knowledge in both materials and biological science for the development of efficient and safe Fe-based stents, although there are still only limited numbers of reviews regarding this interdisciplinary field. This review aims to provide a concise overview of the main strategies developed so far to design Fe-based stents with accelerated degradation, highlighting the fundamental mechanisms of corrosion and the methods to study them as well as the reported approaches to accelerate the corrosion rates. These approaches will be divided into four main sections, focusing on (i) increased active surface areas, (ii) tailored microstructures, (iii) creation of galvanic reactions (by alloying, ion implantation or surface coating of noble metals) and (iv) decreased local pH induced by degradable surface organic layers. Recent advances in the evaluation of the in vitro biocompatibility of the final materials and ongoing in vivo tests are also provided. Abstract:Biodegradable magnesium (Mg) alloys are expected to be promising materials for cardiovascular stents (CVS), which can avoid the long-term clinical problems of current CVS, such as in-stent restenosis, late stent thrombosis, etc. Mg alloy stents exhibit superior biocompatibility and tunable biodegradability, compared with conventional permanent metallic stents. However, the poor formability and non-uniform corrosion of Mg alloy stents hinder their clinical application of CVS. This review focuses on the development of Mg alloys for CVS in recent years. According to the results of bibliometric analysis, we analyzed different biodegradable Mg alloy systems. Moreover, the structural design strategies for Mg alloy stents that can reduce the stress concentration, as well as the surface modification methods to control the corrosion behavior and biological performance of Mg alloy stents are also highlighted. At last, this review systematically discussed the potential directions and challenges of biodegradable magnesium stents (BMgS) in cardiovascular fields.

In contrast to polymer bioresorbable stents (BRS) that exhibited suboptimal performance in clinical trials due to their deficient mechanical properties, metallic BRS with improved mechanical strength have made their way into the clinic and have demonstrated more promising results. In the roadmap of research and development of metallic BRS, magnesium and iron based biodegradable metal stents had been clinically used, and the zinc based biodegradable metal stents had been trailed in Mini-Pigs. In this mini-review paper, we demonstrate the current technology levels and point out the future R&D direction of metallic BRS. Magnesium based BRS should target for decreasing struct thickness meanwhile balancing with enough supporting strength. Iron based BRS should move towards high efficient absorption, conversion, metabolism, elimination of its degradation products. Zn based BRS should strive to improve mechanical stability, creep resistance and biocompatibility. Abstract:A stent is a medical device designed to serve as a temporary or permanent internal scaffold to maintain or increase the lumen of a body conduit. The researchers and engineers diverted to investigate biodegradable materials due to the limitation of metallic materials in stent application such as stent restenosis which requires prolonged anti platelet therapy, often result in smaller lumen after implantation and obstruct re-stenting treatments. Biomedical implants with temporary function for the vascular intervention are extensively studied in recent years. The rationale for biodegradable stent is to provide the support for the vessel in predicted period of time and then degrading into biocompatible constituent. The degradation of stent makes the re-stenting possible after several months and also ameliorates the vessel wall quality. The present article focuses on the biodegradable materials for the cardiovascular stent. The objective of this review is to describe the possible biodegradable materials for stent and their properties such as design criteria, degradation behavior, drawbacks and advantages with their recent clinical and preclinical trials.
[Ileal artery aneurysm: a case report and review of literature].

Abstract:In a 70-year-old Japanese male who had a history of a radical operation of ruptured descending thoracic aortic aneurysm, the ileal artery aneurysm developed asymptomatically during postoperative follow up period. The aneurysm was about 10 by 4.5 by 4 cm in size and atherosclerosis microscopically. This case seemed to be the 20th case of superior mesenteric artery aneurysm and the 4th case of ileal artery aneurysm reported in Japan. Furthermore, this may be the first report of ileal artery aneurysm associated with thoracic aortic aneurysm in Japanese and English literature.

Abstract:Magnesium alloy stents have been extensively studied in the field of biodegradable metal stents due to their exceptional biocompatibility, biodegradability and excellent biomechanical properties. Nevertheless, the specific in vivo service environment causes magnesium alloy stents to degrade rapidly and fail to provide sufficient support for a certain time. Compared to previous reviews, this paper focuses on presenting an overview of the development history, the key issues, mechanistic analysis, traditional protection strategies and new directions and protection strategies for magnesium alloy stents. Alloying, optimizing stent design and preparing coatings have improved the corrosion resistance of magnesium alloy stents. Based on the corrosion mechanism of magnesium alloy stents, as well as their deformation during use and environmental characteristics, we present some novel strategies aimed at reducing the degradation rate of magnesium alloys and enhancing the comprehensive performance of magnesium alloy stents. These strategies include adapting coatings for the deformation of the stents, preparing rapid endothelialization coatings to enhance the service environment of the stents, and constructing coatings with self-healing functions. It is hoped that this review can help readers understand the development of magnesium alloy cardiovascular stents and solve the problems related to magnesium alloy stents in clinical applications at the early implantation stage.

Abstract:The new principle and technique to tune biodegradation rates of biomaterials is one of the keys to the development of regenerative medicine and next-generation biomaterials. Biodegradable stents are new-generation medical devices applied in percutaneous coronary intervention, etc. Recently, both corrodible metals and degradable polymers have drawn much attention in biodegradable stents or scaffolds. It is, however, a dilemma to achieve good mechanical properties and appropriate degradation profiles. Herein, we put forward a metal-polymer composite strategy to achieve both. Iron stents exhibit excellent mechanical properties but low corrosion rate in vivo. We hypothesized that coating of biodegradable aliphatic polyester could accelerate iron corrosion due to the acidic degradation products, etc. To demonstrate the feasibility of this composite material technique, we first conducted in vitro experiments to affirm that iron sheet corroded faster when covered by polylactide (PLA) coating. Then, we fabricated three-dimensional metal-polymer stents (MPS) and implanted the novel stents in the abdominal aorta of New Zealand white rabbits, setting metal-based stents (MBS) as a control. A series of in vivo experiments were performed, including measurements of residual mass and radial strength of the stents, histological analysis, micro-computed tomography, and optical coherence tomography imaging at the implantation site. The results showed that MPS could totally corrode in some cases, whereas iron struts of MBS in all cases remained several months after implantation. Corrosion rates of MPS could be easily regulated by adjusting the composition of PLA coatings.

Abstract:Over the past two decades, biodegradable metals (BMs) have emerged as promising materials to fabricate temporary biomedical devices, with the purpose of avoiding potential side effects of permanent implants. In this review, we first surveyed the current status of BMs in neuroscience, and briefly summarized the representative stents for treating vascular stenosis. Then, inspired by the convincing clinical evidence on the in vivo safety of Mg alloys as cardiovascular stents, we analyzed the possibility of producing biodegradable cerebrovascular Mg alloy stents for treating ischemic stroke. For these novel applications, some key factors should also be considered in designing BM brain stents, including the anatomic features of the cerebral vasculature, hemodynamic influences, neuro-cytocompatibility and selection of alloying elements. This work may provide insights into the future design and fabrication of BM neurological devices, especially for brain stents.

Abstract:A cardiovascular stent is a small mesh tube that expands a narrowed or blocked coronary artery. Unfortunately, current stents, regardless metallic or polymeric, still largely fall short to the ideal clinical needs due to late restenosis, thrombosis and other clinical complications. Nonetheless, metallic stents are preferred clinically thanks to their superior mechanical property and radiopacity to their polymeric counterparts. The emergence of bioresorbable metals opens a window for better stent materials as they may have the potential to reduce or eliminate late restenosis and thrombosis. In fact, some bioresorbable magnesium (Mg)-based stents have obtained regulatory approval or under trials with mixed clinical outcomes. Some major issues with Mg include the too rapid degradation rate and late restenosis. To mitigate these problems, bioresorbable zinc (Zn)-based stent materials are being developed lately with the more suitable degradation rate and better biocompatibility. The past decades have witnessed the unprecedented evolution of metallic stent materials from first generation represented by stainless steel (SS), to second generation represented by Mg, and to third generation represented by Zn. To further elucidate their pros and cons as metallic stent materials, we systematically evaluated their performances in vitro and in vivo through direct side-by-side comparisons. Our results demonstrated that tailored Zn-based material with proper configurations could be a promising candidate for a better stent material in the future.

Abstract:The new‐generation coronary stents are expected to be biodegradable, and then the biocompatibility along with biodegradation get to be more challenging. It is a critical issue to choose appropriate biomimetic conditions to evaluate biocompatibility. Compared with other candidates for biodegradable stents, iron‐based materials are of high mechanical strength, yet have raised more concerns about biodegradability and biocompatibility. Herein, a metal‐polymer composite strategy was applied to accelerate the degradation of iron‐based stents in vitro and in a porcine model. Furthermore, we found that serum, the main environment of vascular stents, ensured the safety of iron corrosion through its antioxidants. This work highlights the importance of serum, particularly albumin, for an in vitro condition mimicking blood‐related physiological condition, when reactive oxygen species, inflammatory response and neointimal hyperplasia are concerned. The resultant metal‐polymer composite stent was implanted into a patient in a clinical research via interventional treatment, and the follow‐up confirmed its safety, efficacy and appropriate biodegradability. Abstract:This article overviews the development course and present situation of Mg-based stents.The crucial issues of limiting the clinical application of Mg-based stents are proposed in this article.This article summarizes the recent research progress in the surface modification tactic of Mg-based stents.The future directions of modification strategies of Mg-based stents are discussed in this article.Magnesium (Mg) and its alloys, as potential biodegradable materials, have drawn wide attention in the cardiovascular stent field because of their appropriate mechanical properties and biocompatibility. Nevertheless, the occurrence of thrombosis, inflammation, and restenosis of implanted Mg alloy stents caused by their poor corrosion resistance and insufficient endothelialization restrains their anticipated clinical applications. Numerous surface treatment tactics have mainly striven to modify the Mg alloy for inhibiting its degradation rate and enduing it with biological functionality. This review focuses on highlighting and summarizing the latest research progress in functionalized coatings on Mg alloys for cardiovascular stents over the last decade, regarding preparation strategies for metal oxide, metal hydroxide, inorganic nonmetallic, polymer, and their composite coatings; and the performance of these strategies in regulating degradation behavior and biofunction. Potential research direction is also concisely discussed to help guide biological functionalized strategies and inspire further innovations. Abstract:Interest in the research of bioabsorbable metallic stents (BMSs) has been growing in the last decades. In contrast to the traditional permanent metallic stents, BMSs can provide a transient opening into narrowing vessel and be metabolized by human body with appropriate host response after accomplishing the mission to assist tissue healing. Meanwhile, the absence of the foreign metallic cage rescues the stented segment from risks associated with their permanent counterparts such as vascular inflammation, late thrombosis, and neoatherosclerosis. In addition, it also facilitates the restoration of vasomotion, mechanotransduction, adaptive shear stress, late luminal enlargement, and late expansive remodeling. This chapter comprehensively reviews the recently developed representative Mg-based, Fe-based, and Zn-based BMSs with emphasis on the degradation behavior, mechanical properties, and in vitro and in vivo biocompatibility. Finally, the challenges and directions for the future development of BMSs are also discussed.

Abstract:Biodegradable stents can provide scaffolding and anti-restenosis benefits in the short term and then gradually disappear over time to free the vessel, among which the Mg-based biodegradable metal stents have been prosperously developed. In the present study, a Mg-8.5Li (wt.%) alloy (RE- and Al-free) with high ductility (> 40%) was processed into mini-tubes, and further fabricated into finished stent through laser cutting and electropolishing. In-vitro degradation test was performed to evaluate the durability of this stent before and after balloon dilation. The influence of plastic deformation and residual stress (derived from the dilation process) on the degradation was checked with the assistance of finite element analysis. In addition, in-vivo degradation behaviors and biocompatibility of the stent were evaluated by performing implantation in iliac artery of minipigs. The balloon dilation process did not lead to deteriorated degradation, and this stent exhibited a decent degradation rate (0.15 mm/y) in vitro, but divergent result (> 0.6 mm/y) was found in vivo. The stent was almost completely degraded in 3 months, revealing an insufficient scaffolding time. Meanwhile, it did not induce possible thrombus, and it was tolerable by surrounding tissues in pigs. Besides, endothelial coverage in 1 month was achieved even under the severe degradation condition. In the end, the feasibility of this stent for treatment of benign vascular stenosis was generally discussed, and perspectives on future improvement of Mg-Li-based stents were proposed.

Abstract:Hospital mortality due to myocardial infarction has decreased markedly since the 1970's. Intravenous thrombolysis is certainly one of the reasons for this decrease but its precise role cannot be defined. In practice, major therapeutic trials enable definition of the broad lines. It is nevertheless essential to combine our personal experience with their analysis. It is not yet possible at present to say which is the best thrombolytic. Use of the association streptokinase-rtPA has the advantages of both agents and the same risks at a lower cost than that of rtPA alone.
Nanophasic Biodegradation Enhances the Durability and Biocompatibility of Magnesium Alloys for the Next-Generation Vascular Stents.

Abstract:Biodegradable metal alloys emerge as a new class of biomaterials for tissue engineering and medical devices such as cardiovascular stents. Deploying biodegradable materials to fabricate stents not only obviates a second surgical intervention for implant removal but also circumvents the long-term foreign body effect of permanent implants. However, these materials for stents suffer from an un-controlled degradation rate, acute toxic responses, and rapid structural failure presumably due to a non-uniform, fast corrosion process. Here we report that highly uniform, nanophasic degradation is achieved in a new Mg alloy with unique interstitial alloying composition as the nominal formula Mg-2.5Nd-0.2Zn-0.4Zr (wt%, hereafter, denoted as JDBM). This material exhibits highly homogeneous nanophasic biodegradation patterns as compared to other biodegradable metal alloy materials. Consequently it has significantly reduced degradation rate determined by electrochemical characterization. The in vitro cytotoxicity test using human vascular endothelial cells indicates excellent biocompatibility and potentially minimal toxic effect on arterial vessel walls. Finally, we fabricated a cardiovascular stent using JDBM and performed in vivo long-term assessment via implantation of this stent in an animal model. The results confirmed the reduced degradation rate in vivo, excellent tissue compatibility and long-term structural and mechanical durability. Thus, this new Mg-alloy with highly uniform nanophasic biodegradation represents a major breakthrough in the field and a promising material for manufacturing the next generation biodegradable vascular stents.

Abstract:Implantation of cardiovascular stents is an important therapeutic method to treat coronary artery diseases. Bare-metal and drug-eluting stents show promising clinical outcomes, however, their permanent presence may create complications. In recent years, numerous preclinical and clinical trials have evaluated the properties of bioresorbable stents, including polymer and magnesium-based stents. Three-dimensional (3D) printed-shape-memory polymeric materials enable the self-deployment of stents and provide a novel approach for individualized treatment. Novel bioresorbable metallic stents such as iron- and zinc-based stents have also been investigated and refined. However, the development of novel bioresorbable stents accompanied by clinical translation remains time-consuming and challenging. This review comprehensively summarizes the development of bioresorbable stents based on their preclinical/clinical trials and highlights translational research as well as novel technologies for stents (e.g., bioresorbable electronic stents integrated with biosensors). These findings are expected to inspire the design of novel stents and optimization approaches to improve the efficacy of treatments for cardiovascular diseases.
