Abstract:Objective The effect of renal ischemia-reperfusion injury on the function of renal allograft was investigated after donation after cardiac death (DCD) renal transplantion. Methods We retrospectively analyzed the clinical data of renal transplant donors and recipients at the Kidney Disease Center of the First Affiliated Hospital of Zhejiang University from May 2011 to June 2015. One year after transplantation, the recipients were classified into high renal function group [estimated glomerular filtration rate (eGFR) ] ≥60 mL·min-1·(1.73 m2)-1) and low renal function group (eGFR <60 mL·min-1·(1.73 m2)-1. According to the DCD donor surgery record, the systolic blood pressure (SBP) and blood oxygen saturation (SpO2) of the donors during different periods of warm ischemia in two groups were analyzed. The normal distribution measurement data were showed as the mean±standard deviation (±s), and the t-test was used to compare the basic characteristic data, donor SBP and SpO2 in the different warm ischemia periods between the two groups, except for gender. The Wilcoxon Signed Rank test was used to compare the pathological scores of glomerular and glomerular of donor kidneys between two groups when organ procurement. Count data were expressed as a percentage. Chi-square test was used to compare the donor gender and DGF incidence in the two groups. P<0.05 was considered statistically significant. Results By June 2016, all patients were followed up (28.4±2.8) months, ranged from 13.1 to 62.5 months. 340 recipients were eventually included, including 259 recipients in the high renal function group and 81 recipients in the low renal function group. The incidence of DGF was 14.7% (38/259) and 22.2% (18/81) in high and low renal function groups within 1 year after transplantation, with no significant difference (χ2=2.557, P>0.05). The average age and BMI of donors in the high renal function group were lower than those in the low renal function group, and the percentage of males and eGFR at the time of donation was higher than that in the low renal function group (t=-6.363, -2.049, 4.190, χ2=4.863, P all <0.05). The renal pathology scores of donors with high renal function were lower than those with low renal function, and the difference was statistically significant (Z=-2.606, P<0.05). The recipients of the high renal function group were younger than the low renal function group, and the proportion of males was higher than the low renal function group, with significant difference (t=-2.790, χ2=9.658, P<0.05). In the high and low renal function groups, the initial SpO2 decreased by 40%, the average time from life support device withdrawal, different initial SpO2 with 90%, 80%, 70%, 60% of to SpO2 to SpO2 could not be measured, were (5.9±4.3) and (4.8±3.3), (8.0±5.2) and (6.1±4.4), (4.5±3.6) and (3.5±2.8), (4.0±3.7) and (2.9±2.4), (4.0±3.6) and (2.8±2.7), (3.6±3.5) and (2.4±2.5) minutes, with significant difference (t=2.088, 2.983, 2.328, 2.622, 2.557, 2.759, all P<0.05). In the high and low renal function groups, the average change rate of initial SpO2 declined by 10%, 40%, life support device withdrawal to SpO2 not detected, initial 60% SpO2 to SpO2 not detected were (2.40±1.78) and (2.90±1.70), (8.71±6.96) and (15.01±12.97), (19.60±17.49) and (25.80±22.85), (22.41±15.94) and (29.93±19.36) percentage per minute, with significant difference (t=-2.230, -5.647, -2.577, -3.514, P all <0.05). Conclusions The prognosis of DCD renal transplantation was influenced by the general factors such as donor and recipient age and BMI. The high and low renal function recipients had different high and low SpO2 time intervals and different rates of change derived from the DCD donors. During DCD renal transplantation, optimized surgical procedures should be taken to minimize the low SpO2 duration, so that the warm ischemia damage of the donor would be alleviated to acheive a better prognosis of tranplant. Abstract:MerC, encoded by merC in the transposon Tn21 mer operon, is a heavy metal transporter with potential applications for phytoremediation of heavy metals such as mercuric ion and cadmium. In this study, we demonstrate that MerC also acts as a transporter for methylmercury. When MerC was expressed in Escherichia coli XL1-Blue, cells became hypersensitive to CH3Hg(I) and the uptake of CH3Hg(I) by these cells was higher than that by cells of the isogenic strain. Moreover, transgenic Arabidopsis plants expressing bacterial MerC or MerC fused to plant soluble N-ethylmaleimide-sensitive factor attachment protein receptors (SNAREs) accumulated CH3Hg(I) effectively and their growth was comparable to the wild-type plants. These results demonstrate that when the bacterium-derived merC gene is ectopically introduced in genetically modified plants, MerC expression in the transgenic plants promotes the transport and sequestration of methylmercury. Thus, our results show that the expression of merC in Arabidopsis results in transgenic plants that could be used for the phytoremediation and elimination of toxic methylmercury from the environment.
Postreperfusion Renal Allograft Biopsy Predicts Outcome of Single-Kidney Transplantation: A 10-Year Observational Study in China

Abstract:Introduction: Biopsy findings often lead to the discard of many donor kidneys although their clinical value is not fully understood. We investigated the predictive value of postreperfusion biopsy on long-term allograft outcome after single-kidney transplantation. Methods: We retrospectively evaluated the significance of histologic findings, read by experienced renal pathologists, in 461 postreperfusion biopsy specimens collected from 2010 to 2017 after deceased donor renal transplant; and performed time-to-event analyses to determine the association between histology and hazard of death-censored graft failure. Recipients were followed-up with over a median time of 6.8 (range, 0.2-11.9) years. We assessed specimens using the Remuzzi score (scale of 0-12) and categorized them into low-score (<= 3) and high-score (>3) groups. Kappa coefficients were calculated to assess agreement in procurement versus reperfusion biopsies. Results: High Remuzzi score kidneys came from older donors with a higher incidence of hypertension, higher final creatinine, death from cerebrovascular disease, expanded criteria donor, and a higher kidney donor risk index (KDRI) (all P < 0.001). In adjusted analyses, Remuzzi score was independently associated with death-censored graft failure (hazard ratio [HR] 1.389 for each 1 score rise in Remuzzi score, 95% confidence interval 1.181-1.633, P < 0.001). Overall histologic agreement (procurement biopsy versus reperfusion biopsy) was kappa = 0.137. Conclusion: Our findings suggest that postreperfusion biopsy is associated with long-time graft outcomes after transplant from a deceased donor. Agreement between procurement and reperfusion biopsy was found to be low. Prospective trials are necessary to optimize procurement biopsy practices.

Abstract:Purpose: The clinical outcomes of kidney transplantation from deceased donors have seen significant improvements with the use of machine perfusion (MP), now a standard practice in transplant centers. However, the use of perfusate biomarkers for assessing organ quality remains a subject of debate. Despite this, some centers incorporate them into their decision-making process for donor kidney acceptance. Recent studies have indicated that lactate dehydrogenase (LDH), glutathione S-transferase, interleukin-18, and neutrophil gelatinase-associated lipocalin (NGAL) could predict post-transplant outcomes. Materials and methods: Between August 2016 and June 2017, 31 deceased-donor after brain death were included and stroke was the main cause of death. Pediatric patients, hypersensitized recipients were excluded. 43 kidneys were subjected to machine perfusion. Perfusate samples were collected just before the transplantation and stored at -80ºC. Kidney transplant recipients have an average age of 52 years, 34,9% female, with a BMI 24,6±3,7. We employed receiver operating characteristic analysis to investigate associations between these perfusate biomarkers and two key clinical outcomes: delayed graft function and primary non-function. Results: The incidence of delayed graft function was 23.3% and primary non-function was 14%. A strong association was found between NGAL concentration and DGF (AUC=0.766, 95% CI, P=0.012), and between LDH concentration and PNF (AUC=0.84, 95% CI, P=0.027). Other perfusate biomarkers did not show significant correlations with these clinical outcomes. Conclusion: The concentrations of NGAL and LDH during machine perfusion could assist transplant physicians in improving the allocation of donated organs and making challenging decisions regarding organ discarding. Further, larger-scale studies are required.
PO2 21% oxygenated hypothermic machine perfusion in kidney transplantation: Any clinical benefit?

Abstract:Background: In deceased donor kidney transplantation (KT), the use of hypothermic machine perfusion (HMP) has been acquiring the status of best practice in the pre-transplant management of kidney grafts. Two types of HMP are currently available, oxygenated HMP and non-oxygenated HMP. However, data on the real clinical impact of oxygenation on KT outcome are still heterogeneous. Methods: Retrospective study on a cohort of 103 patients transplanted with a single kidney graft that was managed either with end-ischemic oxygenated (O2 group, Waves Machine, n = 51, 49.5%) or non-oxygenated HMP (no-O2 group, Life Port Kidney Transporter Machine, n = 52, 50.5%), during the period January 2016-December 2020. Oxygenation was performed at pO2 21%. Results: The median cold ischemia time was 29 h:40 min [IQR 26 h:55 min-31 h:10 min] and the prevalence of grafts from extended criteria donors (ECD) was 46.7%, with a median kidney donor profile index (KDPI) of 72 [41-94]. The study groups were homogeneous in terms of recipient characteristics, ischemia times and donor characteristics. O2 and no-O2 groups showed comparable outcomes in terms of delayed graft function (O2 vs no-O2, 21.5% vs 25%, p = 0.58), vascular (0.2% vs 0.2%, p > 0.99) and urologic (13.7% vs 11.5%, p = 0.77) complications, and episodes of graft rejection (11.7% vs 7.7%, p = 0.52). At 1 year follow up, even creatinine serum levels were comparable between the groups (1.27 mg/dL [1.09 and 1.67] vs 1.4 mg/dL [1.9-1.78], p = 0.319), with similar post-transplant trend (p = 0.870). No significant benefit was either observed in ECD or KDPI > 60 subgroups, respectively. Conclusions: Oxygenation at pO2 21% during HMP seems not to significantly enhance the KT outcomes in terms of postoperative complications or graft function.
Assessment of Oxidative Stress Markers in Hypothermic Preservation of Transplanted Kidneys

Abstract:Ischemia-reperfusion injury (IRI) after renal transplantation is a complex biochemical process. The first component is an ischemic phase during kidney storage. The second is reperfusion, the main source of oxidative stress. This study aimed to analyze the activity of enzymes and concentrations of non-enzymatic compounds involved in the antioxidant defense mechanisms: glutathione (GSH), glutathione peroxidase (GPX), catalase (CAT), superoxide dismutase (SOD), glutathione reductase (GR), glutathione transferase (GST), thiobarbituric acid reactive substances (TBARS), malondialdehyde (MDA), measured in preservation fluid before transplantation of human kidneys (KTx) grafted from brain dead donors. The study group (N = 66) was divided according to the method of kidney storage: Group 1-hypothermic machine perfusion (HMP) in LifePort perfusion pump, n1 = 26, and Group 2-static cold storage (SCS), n2 = 40. The measurements of kidney function parameters, blood count, and adverse events were performed at constant time points during 7-day hospitalization and 3-month follow-up. Kidney perfusate in Group 2 was characterized by significantly more acidic pH (p < 0.0001), higher activity of GPX [U/mgHb] (p < 0.05) and higher concentration of MDA [μmol/L] (p < 0.05). There was a statistically significant improvement of kidney function and specific blood count alterations concerning storage method in repeated measures. There were aggregations of significant correlations (p < 0.05) between kidney function parameters after KTx and oxidative stress markers: diuresis & CAT, Na+ & CAT, K+ & GPX, urea & GR. There were aggregations of significant correlations (p < 0.05) between recipient blood count and oxidative stress markers: CAT & MON, SOD & WBC, SOD & MON. Study groups demonstrated differences concerning the method of kidney storage. A significant role of recipient's gender, gender matching, preservation solution, and perfusate pH was not confirmed, however, basing on analyzed data, the well-established long-term beneficial impact of HMP on the outcome of transplanted kidneys might partially depend on the intensity of IRI ischemic phase and oxidative stress, reflected by the examined biomarkers.

Diagnosis and Prediction of Early Acute Renal Transplant Rejection with Blood Oxygen Level Dependent Magnetic Resonance Imaging

Abstract:Objective To assess the value of blood oxygen level-dependent magnetic resonance imaging (BOLD-MRI)in diagnosis and prediction of early acute renal transplant rejection.Methods BOLD-MRI was performed in a cohort of 103 patients undergoing cadaver renal transplantation between Dec 2005 and March 2007.Among them,82 recipients had nomlal renal function,21 had biopsy-proved acute rejection.R2* (1/s)measurements were obtained in the medulla and cortex of grafted kidneys. Results R2* values of the medulla were significantly lower in the acute rejection group[R2*=(14.02±2.68)/s]than that in the normally functioning transplants group [R2*=(16.66+2.82)/s],the difference between these two groups was significant (P＜0.01);ROC curve analyses suggested that medullary MR2* values could accurately identify acute rejection in the early post-transplantation period.In the normal functioning transplant group,those with lower medullary R2* values (MR2*＜14.9/s,n=23) had higher acute rejection rates than those with higher medullary R2* values (MR2*＞14.9/s,n=59) in the first 6 months following transplantation,but the difference between these two groups was not significant (17.39% vs 8.47%,P=0.259). Conclusions Mean R2* values in the medullary regions of grafted kidneys with BOLD-MPd may be a non-invasive diadynamic criteria with good sensitivity and specificity,and may be a valuable predictor of early acute renal transplant rejection.

Abstract:Background Monitoring of renal function after kidney transplantation (KT) is performed by measuring serum creatinine (SCr), urine volumes (UV), and glomerular filtration rate (GFR). Other methods based on oxygen metabolism, such as the renal venous oxygen pressure (P rv O 2 ), may be useful. The aim of this study was to explore the correlation between P rv O 2 and SCr, UV, and GFR 5 days after KT (SCr5, UV5, and GFR5, respectively). Methods We conducted a prospective cohort study in adults scheduled for living donor KT. A venous blood sample was taken from the renal vein after declamping the renal artery, and blood gas determinations were made. Correlation analyses between P rv O 2 and SCr5, UV5, and GFR5 were done by calculating Spearman's correlation coefficient with generalized linear models (GLM). A Spearman's correlation analysis was performed between the percentage decrease in SCr (%ΔSCr) and P rv O 2 . A GLM was also performed to determine the association of P rv O 2 with slow graft function (SGF). Results The study included 42 patients, of whom 67% were men. The median age was 31 years (IQR, 27-43.5). P rv O 2 was negatively correlated with SCr5 (ρ = -0.53, P = .003), and positively correlated with GFR5 (ρ = 0.49, P = .001) and %ΔSCr (ρ = 0.47, P = .002). A higher P rv O 2 was associated with an increase in GFR in univariable (β = 1.24, 95% CI, 0.56-1.93, P = .001) and multivariable (β = 1.24, 95% CI, 0.53-1.94, P = .001) analyses. No association was found between P rv O 2 and SGF. Conclusion P rv O 2 could be used to monitor renal function in the first 5 days after related living-donor KT, given its good correlation with SCr and GFR.

Abstract:Due to the shortage of kidneys donated for transplantation, surgeons are forced to use the organs with an elevated risk of poor function or even failure. Although the existing methods for pre-transplant quality evaluation have been validated over decades in population cohort studies across the world, new methods are needed as long as delayed graft function or failure in a kidney transplant occurs. In this study, we explored the potential of utilizing photoacoustic (PA) imaging during normothermic machine perfusion (NMP) as a means of evaluating kidney quality. We closely monitored twenty-two porcine kidneys using 3D PA imaging during a two-hour NMP session. Based on biochemical analyses of perfusate and produced urine, the kidneys were categorized into 'non-functional' and 'functional' groups. Our primary focus was to quantify oxygenation (sO2) within the kidney cortical layer of depths 2 mm, 4 mm, and 6 mm using two-wavelength PA imaging. Next, receiver operating characteristic (ROC) analysis was performed to determine an optimal cortical layer depth and time point for the quantification of sO2 to discriminate between functional and non-functional organs. Finally, for each depth, we assessed the correlation between sO2 and creatinine clearance (CrCl), oxygen consumption (VO2), and renal blood flow (RBF). We found that hypoxia of the renal cortex is associated with poor renal function. In addition, the determination of sO2 within the 2 mm depth of the renal cortex after 30 min of NMP effectively distinguishes between functional and non-functional kidneys. The non-functional kidneys can be detected with the sensitivity and specificity of 80% and 85% respectively, using the cut-off point of sO2 < 39%. Oxygenation significantly correlates with RBF and VO2 in all kidneys. In functional kidneys, sO2 correlated with CrCl, which is not the case for non-functional kidneys. We conclude that the presented technique has a high potential for supporting organ selection for kidney transplantation.

Effect of perioperative recipient management on initial renal allograft function

Abstract:To study the effect of perioperative management on initial renal allograft function, we identified 22 consecutive cadaver donor kidney pairs in which one kidney was transplanted at one local institution (group I) while the contralateral kidney was transplanted at a second local hospital (group II). Donor‐derived and harvest parameters were uniform for each pair. The two institutions employed similar immunosuppressive protocols, but markedly different perioperative recipient management protocols including: a) maximal volume repletion, perioperative hemodynamic monitoring, and a parameter‐determined volume replacement protocol in group I; b) donor organ chilling during revascularization in group I; c) different intraoperative diuretics; d) different volume replacement solutions; and e) different immediate postoperative nursing settings. Rewarm times were significantly longer for group I grafts than for their group II contralateral counterparts (51 ± 10 min versus 29 ± 8 min; p < 0.001). Despite this, group I grafts produced significantly larger initial 24‐hour urine volumes than group II (6.4 ± 4.2 l versus 3.7 ± 2.9 l; p = 0.02). Mean change in serum creatinine in the initial 24 h was also significantly greater in group I than group II (–6.0 ± 2.6 mg/dl versus –2.6 ± 3.1 mg/dl; p = 0.003). None of the 22 grafts in group I manifested poor initial function while 9 of their 22 (45%) counterparts in group II did (p = 0.001). A total of 5 group II recipients required dialysis during the first 5 post‐transplant days while no group I patient was dialysed during this period (p = 0.05). We conclude that, given uniform donor organs, perioperative recipient management protocols appear to significantly influence initial urine volume, rate of fall of serum creatinine, and the incidence of ischemic injury in cadaveric renal allografts. In addition, the potentially deleterious effects of rewarm times greater than 45 min may be surmountable, in many cases, by perioperative interventions such as those utilized in group I recipients in the present study.

To assure accuracy, anti-PnPs EIA for serotype 14 antibodies, a previously assigned serotype, was performed concurrently. This method assures consistency of the new microgram-per-microliter assignments with previous antiserotype assignments to lot 89-S. The sum of the experimentally derived isotype assignments for anti-PnPs serotypes in lot 89-S agrees well with the separately determined total Ig assignment for each serotype. The lot 89-S assignments for serotypes 1, 5, 6B, 14, 18C, 19F, and 23F were used for pneumococcal conjugate vaccine clinical trial evaluation and to generate data in efficacy trials where serological correlates for protection have been proposed. The assignment of antibody concentrations to additional pneumococcal serotypes in this reference reagent facilitates the consistent and accurate comparison of serum antibody concentrations across clinical trials.
Effect of recipients' peri‐operative parameters on the outcome of kidney transplantation

Abstract:In this prospectively collected data base multifactorial study, the influence of the peri‐operative recipients' parameters on the transplanted kidney function were examined. One hundred and twenty‐one patients who underwent first cadaver kidney transplantation in our unit were involved and were followed up for another 2 yr. The recipients were divided into three groups. Group I, patients with non‐functioning grafts, group II, with delayed graft function, and group III. with good graft function. All routinely measured parameters were involved in the examination. The pre‐operative haematocrit level differed significantly among the three groups. Significant intra‐operative differences were found among the three groups in mean arterial pressure, (group I: 81±31 mmHg, group II: 90±24 mmHg, vs. group III: 108±26 mmHg, p <0.01) at 5 min before revascularisation. The patients who had better post‐operative kidney function, had lower pre‐operative haematocrit level, and higher blood pressure in the different phase of operation, than those who had post‐operative kidney function problems. The second warm ischaemic time in Group III was significantly shorter than in group I. (Group I: 51.5 min vs. group II: 47 min. vs. group III: 46.3 min, p<0.001.) Rejection rate was higher during the first 5 post‐operative days in patients with non‐functioning grafts. (Group 1: 53% and group II: 24% vs. group III: 12% p < 0.001.) The other examined parameters had not differed significantly among the three groups. The kidney function differences observed in the early (first 5 d) post‐operative period, remained similar at the end of the next 2 yr. According to our results the peri‐operative fluid‐balance is one of the most important factors which may influence the success of the kidney transplantation.

Abstract:Background: With the rising prevalence of end-stage kidney disease, the use of expanded criteria donor allografts, seen as essential for meeting organ demand, still proves challenging due to their higher risk of graft loss, delayed function, and rejection. Machine perfusion, a technique in preserving allografts, offers improved allograft outcomes compared to static cold storage while allowing for the noninvasive measurement of kidney injury biomarkers in the perfusate solution. This offers an objective method to assess graft function at various preservation stages. Materials and methods: We conducted a narrative review of the databases PubMed and Scopus, including studies written in the English language and published after 2010. Results: In this narrative review, we identified biomarkers, like 4-hydroxyproline, taurine, and glutathione transferase, as predictive markers of delayed graft function. Additionally, biomarkers, like extracellular histone h3, vascular cell adhesion protein, and matrix metalloprotease protein, have shown correlation with decreased graft function, although their predictive ability remains inconclusive. Discussion: The review outlines various suggestions for potential areas of research focus to enhance future expanded criteria donor allograft utilization. However, limitations exist, including the absence of a singular reliable biomarker and the challenges of validating biomarker effectiveness across diverse outcomes.

Abstract:Cervical pregnancy occurs in approximately 1 in 16,000 pregnancies. The majority of these cases left untreated will abort spontaneously in the first trimester, apparently because of the unfavorable implantation site. The incidence of cervical pregnancies is rising and may reflect the steady increase in ectopic gestations and cesarean sections. 2 In the past, this diagnosis was made late in the first trimester, usually following heavy vag·inal bleeding (many times leading to emergency hys· terectomy). Currently, the diagnosis is based on ultrasound findings, permitting efforts at conservative treatment. We compare the transabdominal and endovaginal modes of sonography in a case of cervical pregnancy that was subsequently treated with metho· trexate.
Intraoperative Cerebral And Renal Tissue Oxygenation To Predict Postoperative Acute Kidney Injury In Pediatric Cardiac Surgery: A Prospective Observational Study

Abstract:Objective Infants undergoing surgery for congenital heart disease can develop postoperative acute kidney injury (AKI) due to several intraoperative insults including renal hypoperfusion and ischemia. Near-infrared spectroscopy is a non-invasive technique which allows the continuous monitoring of tissue oxygenation (StO2). There is inconclusive evidence that perioperative renal and cerebral tissue oxygen desaturation might predict postoperative AKI. We hypothesized that intraoperative renal and cerebral tissue oxygen desaturation are associated with the development of postoperative AKI in children undergoing cardiac surgery. Design and method This prospective observational study was conducted in the University Medical Center Groningen. The study was registered at the Netherlands Trial Register (NL9852). We included pediatric patients undergoing corrective cardiac surgery for a non-cyanotic congenital heart defect with a left-to-right shunt. Intraoperative cerebral and renal StO2 were continuously monitored with the O3® regional oximeter (Masimo Corporation, Irvine, California, USA). Baseline renal and cerebral StO2 values were recorded at surgical incision. Two different renal and cerebral tissue oxygen desaturation thresholds were investigated, i.e. a StO2 decrease greater than 10% or 20% from the respective baseline value. The area under the threshold (AUT10%, AUT20%) and the time-weighted average (TWA10%, TWA20%, calculated dividing the AUT by the duration of surgery) were computed to quantify the extent of desaturation. Serum creatinine values and urine output were recorded daily up to seven days postoperatively, and the development of AKI was defined according to the KDIGO criteria. The AUT and TWA of renal and cerebral StO2 were compared between patients who developed postoperative AKI and those who did not. Results are presented as median [interquartile range]. The Wilcoxon test was used to compare non-normal continuous variables. We planned to include 50 patients expecting a 40% AKI incidence. Results and conclusions Fifty consecutive patients were enrolled from October 2021 to January 2024. One patient was excluded from the analysis because of missing StO2 values. The median age and weight were 8.5 [6-30] months and 7.4 [6.1-12.4] kg. The incidence of AKI was 18.4%.The AUT10% and TWA10% for renal and cerebral StO2 did not differ between patients who developed AKI and those who did not (renal AUT10%: 0 [0-2] vs 0 [0-5] %min, p=0.50; renal TWA10%: 0 [0-0.01] vs 0 [0-0.03] %, p=0.47; cerebral AUT10%: 37 [12-420] vs 10 [1-93] %min, p=0.12; cerebral TWA10%: 0.16 [0.08-2.18] vs 0.07 [0-0.62] %, p=0.12).Similarly, the AUT20% and TWA20% for cerebral StO2 did not differ between patients who developed AKI and those who did not (cerebral AUT20%: 3 [0-42] vs 0 [0-6] %min, p=0.32; cerebral TWA20%: 0.01 [0-0.17] vs 0 [0-0.04] %, p=0.32). Relative decreases in renal StO2 of >20% from baseline were observed in 6 patients in the non-AKI group, and in none of the patients in the AKI group.In conclusion, intraoperative renal and cerebral tissue oxygen desaturation were not associated with the development of postoperative AKI in children undergoing cardiac surgery for a non-cyanotic congenital heart defect with left-to-right shunt. However, since the incidence of AKI was lower than expected, our study might be underpowered to detect such an association.
