Abstract:Prediction of soft tissue aesthetics is important for achieving an optimal outcome in orthodontic treatment planning. Previously, applicable procedures were mainly restricted to 2-D profile prediction. In this study, a generic 3-D finite element (FE) model of the craniofacial soft and hard tissue was constructed, and individualisation of the generic model based on cone beam CT data and mathematical transformation was investigated. The result indicated that patient-specific 3-D facial FE model including different layers of soft tissue could be obtained through mathematical model transformation. Average deviation between the transformed model and the real reconstructed one was 0.47 +/- 0.77 mm and 0.75 +/- 0.84 mm in soft and hard tissue, respectively. With boundary condition defined according to treatment plan, such FE model could be used to predict the result of orthodontic treatment on facial soft tissue.

Abstract:The management of patients with dento-maxillofacial deformities is based on assessments of the dental occlusion - facial skeleton - soft tissues triad. As societal demands and surgical practices have evolved, facial soft tissues have moved to the forefront of considerations in orthognathic surgery. Techniques are therefore required to analyze facial soft tissues objectively and reproducibly, for diagnosis, preoperative planning, and follow-up. Several technologies are currently capable of providing three-dimensional (3D) models of the face, either by 3D reconstruction of traditional computed tomography or cone beam computed tomography data, or directly by stereophotogrammetry, laser scanning or structured light scanning. Multimodal image registration techniques allow bone base, dental occlusion and facial soft tissue information to be combined in a 3D virtual patient. Three-dimensional cephalometric analysis of the facial skeleton and skin is now perfectly integrated in virtual planning and is gradually gaining in automation and accuracy. Photorealistic 3D simulations allow optimal soft tissue planning and facilitate physician-patient communication. Finally, these facial modeling techniques facilitate post-operative studies of soft tissues, which generally involve comparisons of volumetric data. There are many research avenues to pursue and technical improvements are to be expected, particularly through the development of big data and artificial intelligence approaches.
Accuracy of Three-Dimensional Soft-Tissue Prediction Considering the Facial Aesthetic Units Using a Virtual Planning System in Orthognathic Surgery

Abstract:Virtual surgical planning (VSP) is commonly used in orthognathic surgery. A precise soft-tissue predictability would be a helpful tool, for determining the correct displacement distances of the maxilla and mandible. This study aims to evaluate the soft-tissue predictability of the VSP software IPS CaseDesigner® (KLS Martin Group, Tuttlingen, Germany). Twenty patients were treated with bimaxillary surgery and were included in the study. The soft-tissue simulation, done by the VSP was exported as STL files in the engineering software Geomagic Control XTM (3D systems, RockHill, SC, USA). Four months after surgery, a 3D face scan of every patient was performed and compared to the preoperative simulation. The quality of the soft-tissue simulation was validated with the help of a distance map. This distance map was calculated using the inter-surface distance algorithm between the preoperative simulation of the soft-tissue and the actual scan of the postoperative soft-tissue surface. The prediction of the cranial parts of the face (upper cheek, nose, upper lip) was more precise than the prediction of the lower areas (lower cheek, lower lip, chin). The percentage of correctly predicted soft-tissue for the face in total reached values from 69.4% to 96.0%. The VSP system IPS CaseDesigner® (KLS Martin Group; Tuttlingen, Germany) predicts the patient’s post-surgical soft-tissue accurately. Still, this simulation has to be seen as an approximation, especially for the lower part of the face, and continuous improvement of the underlying algorithm is needed for further development.

Abstract:Three-dimensional virtual simulation of orthognathic surgery is now a well-established method in maxillo-facial surgery. The commercial software packages are still burdened by a consistent imprecision on soft tissue predictions. In this study, the authors produced an anatomically detailed patient specific numerical model for simulation of soft tissue changes in orthognathic surgery. Eight patients were prospectively enrolled. Each patient underwent CBCT and planar x-rays prior to surgery and in addition received an MRI scan. Postoperative soft-tissue change was simulated using Finite Element Modeling (FEM) relying on a patient-specific 3D models generated combining data from preoperative CBCT (hard tissue) scans and MRI scans (muscles and skin). An initial simulation was performed assuming that all the muscles and the other soft tissue had the same material properties (Homogeneous Model). This model was compared with the postoperative CBCT 3D simulation for validation purpose. Design of experiments (DoE) was used to assess the effect of the presence of the muscles considered and of their variation in stiffness. The effect of single muscles was evaluated in specific areas of the midface. The quantitative distance error between the homogeneous model and actual patient surfaces for the midface area was 0.55 mm, standard deviation 2.9 mm. In our experience, including muscles in the numerical simulation of orthognathic surgery, brought an improvement in the quality of the simulation obtained.

Abstract:Three-dimensional technologies have had a wide diffusion in several fields of application throughout the last decades; medicine is no exception and the interest in their introduction in clinical applications has grown with the refinement of such technologies. We focus on the application of 3D methodologies in maxillofacial surgery, where they can give concrete support in surgical planning and in the prediction of involuntary facial soft-tissue changes after planned bony repositioning. The purpose of this literature review is to offer a panorama of the existing prediction methods and software with a comparison of their reliability and to propose a series of still pending issues. Various software are available for surgical planning and for the prediction of tissue displacements, but their reliability is still an unknown variable in respect of the accuracy needed by surgeons. Maxilim, Dolphin and other common planning software provide a realistic result, but with some inaccuracies in specific areas of the face; it also is not totally clear how the prediction is obtained by the software and what is the theoretical model they are based on.

Abstract:In the framework of the Lindblad theory for open quantum systems, we determine the degree of quantum decoherence of a harmonic oscillator interacting with a thermal bath. It is found that the system manifests a quantum decoherence which is more and more significant in time. We also calculate the decoherence time and show that it has the same scale as the time after which thermal fluctuations become comparable with quantum fluctuations.

Abstract:This paper describes work in progress in the area of planning and prediction of maxillofacial surgery. We focus on true 3D problems which cannot be addressed by conventional 2D cephalometric radiographs. Examples are bilateral repositioning of the zygoma’s or treating facial asymmetries. The environment being developed arose from the need of patients for prediction of their expected post-op outlook and from the need of surgeons for better support to reach the desired result. Our simulation environment adheres to a scene-based approach in which CT image-derived visualizations and additional 3D photographs (showing the face surface and its natural complexion) are co-presented and manipulated. Tools for non-planar osteotomy are included. Repositioning of bone fragments is made possible. Furthermore, a voxel displacement formalism is presented to account for soft tissue implications. Combining these simulation tools, a qualitative prediction of the facial outlook is obtained. Results are shown for a case of hypoplastic zygoma repositioning.

Abstract:Preoperative planning of complex osteotomies in craniomaxillofacial surgery, in conjunction with a surgeon's expertise, is essential for achieving an optimal result. However, the soft tissue changes that accompany facial bone movements cannot yet be accurately predicted. Bony tissue, because of its greater density, can be better predicted, but it alone does not account for the final aesthetic result. A new approach using not only three-dimensional (3-D) surface models of the patient's anatomy, but also a corresponding volumetric model, is discussed. This 3-D planning software was used in the treatment of 15 patients and was found to provide a good correlation between simulation and postoperative outcome.

Abstract:The 3D prediction of post-operative changes is an inevitable tool for the surgical correction of facial asymmetry. The objective is to execute an evidence-based review answering the following question. Does the 3D virtual prediction planning draw reliable and accurate results in the surgical outcome related to the soft tissues of the face in facial asymmetry? This systematic review of the literature is based on the 3D soft tissue prediction planning of facial asymmetry correction to draw conclusions on the reliability and accuracy of these methods in the surgical outcome related to the soft tissues of the face. PubMed, Web of Science, Cochrane, and Ovid databases were adopted for the literature search. Studies published between years 2000 and 2020, aimed at the assessment of soft tissue predictions using software prediction packages for facial asymmetry, were selected. The Preferred Reporting Items for Systematic Reviews and Meta-analysis (PRISMA) was applied. Quadas-2 tool was used for the qualitative evaluation of selected studies. Initial search yielded 248 articles. Twenty articles fulfilled the inclusion and exclusion criteria and selected for qualitative analysis. Finally, 12 articles were selected for quantitative analysis. The results indicate 3D imaging prediction methods provided more accurate information with less distortion for soft tissue prediction regardless of various softwares currently available. The prediction of soft tissue accuracy in facial asymmetry was less accurate in lower face regardless of the type of surgery for facial asymmetry. The mean prediction error was less than 2 mm.

Abstract:Background and Aim:The goal of this study was to analyze the validity and prediction accuracy of a newly-developed procedure for three-dimensional soft tissue prediction based on Finite Element Method, and to compare the results with prediction produced using an existing two-dimensional prediction program (Dentofacial Planner Plus™).Patients and Methods:In twelve patients who underwent combined surgical-orthodontic treatment, profile prediction was generated using both procedures preoperatively and then compared at predefined measurement points with the patient’s actual postoperative soft tissue status.Results:The deviations observed depended on the facial region, whereby the prediction errors for both procedures were much greater in the lower facial third than in the midfacial third. Calculating in all the measurement points, the mean horizontal prediction error was 0.32 mm for the Finite Element Method and 0.75 mm for the Dentofacial Planner Plus™. Overall, we were able to demonstrate the new procedure’s superior validity and quality of visualization. In addition to profile prediction, the procedure allows a differentiated three-dimensional assessment of esthetically important regions such as the cheeks, nasolabial folds and the nasal wings. Additional X-radiation is not necessary in this risk-free and stress-free procedure.Conclusion:Three-dimensional soft tissue prediction employing finite element modeling is a useful aid for implementing esthetically-optimized treatment planning.

Abstract:Three dimensional imaging systems whether in the form of digital models, facial scanning or dolphin software play an important role in proper diagnosis and treatment planning of the orthosurgical cases. This case report highlights importance of three-dimensional treatment planning in the ortho-surgical management of the patient with skeletal Class III malocclusion involving atypical extraction. On clinical examination, patient had concave profile, prominent chin, deficient midface, flat cheekbone contour and prominent nasal projection. He had Class III molar and canine relation bilaterally with missing 12 (previously extracted), negative overbite and overjet with discordant maxillomandibular midlines. After 3-dimensional digital model set-up for occlusion planning and dolphin simulation for soft tissue prediction, extraction of left maxillary lateral incisor was planned followed by maxillary advancement and mandibular setback surgery and finish the occlusion in Class II molar relationship. After the orthodontic and orthognathic surgical treatment, the skeletal and dental imbalance was corrected, and most of the treatment objectives were achieved and patient was extremely satisfied with his facial profile. The treatment results were stable at the 3 years follow-up.

Abstract:Since the introduction of dental and maxillofacial CBCT imaging in the United States in 2001, volumetric reconstructions providing three-dimensional (3-D) visualizations of complex craniofacial skeletal, dental, and soft tissue conditions have been applied to assess the effects of dental tooth loss, dentofacial deformities, craniofacial anomalies, and airway obstructions associated with obstructive sleep apnea/hypopnea syndrome. In particular, 3-D assessment of facial morphology provides the diagnostic basis for rational clinical decisions for the treatment of patients that require combined orthodontic and surgical correction. Surgical and orthodontic treatment planning involving multiple procedures is now based on computer-assisted assessment and virtual simulations using volumetric images, virtual models, and 3-D printed splints rather than two-dimensional (2-D) radiographic images and plaster-based surgical simulations. Studies on the 3-D bone remodeling, surgical simulations, and comparisons with postoperative outcomes have helped clarify clinical questions on the variability of outcomes of surgery (Cevidanes et al. 2005; Carvalho et al. 2010; Tucker et al. 2010).

Abstract:Objectives: Restoring complex craniofacial deformities presents numerous challenges. Recent years have seen the development of new surgical techniques aimed at improving operation quality and reducing difficulty. However, designing the reduction volume for the affected region and achieving precise contouring remain difficult tasks. The use of three-dimensional (3D) solid models can provide significant assistance at all stages of the operations. This study aimed to investigate the validity of prototype 3D solid models for complex craniofacial operations. Methods: Simulated 3D models of the patients were used. Conventional surgical instruments were employed for the planning of the operations. Patients had follow-up periods ranging from 6 to 18 months. Results: Operations have been planned and performed using three-dimensional solid models. Intraoperative steps were executed as simulated during planning. No major complications were developed. All bone and soft-tissue reconstructions healed without major infection. Conclusion: Stereolithographic models provide: 1) a better understanding of anatomy, 2) presurgical simulation, 3) intraoperative accuracy in lesion localization, 4) accurate fabrication of implants, and 5) improved education for trainees. An accurate method for intraoperative navigation and preoperative planning is clearly useful. This technology offers a direct representation of the patient's anatomy through computer reconstruction, allowing for the efficient and precise transfer of information in complex facial restorative procedures.
Interdisciplinary 3D digital treatment simulation before complex esthetic rehabilitation of orthodontic, orthognathic and prosthetic treatment: workflow establishment and primary evaluation

Abstract:Background An interdisciplinary treatment simulation and smile design before a complex esthetic rehabilitation is important for clinicians' decision-making and patient motivation. Meanwhile, intervention and interaction are necessary for dental specialists in these complex rehabilitations. However, it is difficult to visualize an interdisciplinary treatment plan by using the conventional method, especially when orthognathic surgery is involved, thus hindering communication between dental specialists. This research aims to establish a 3D digital workflow of interdisciplinary treatment simulation to solve this problem. Methods An interdisciplinary 3D digital workflow of simulated treatment plan for complex esthetic rehabilitation was established. Eleven patients were enrolled and illustrated with their treatment plans using 3D treatment simulation, as well as 2D digital smile design (DSD) plus wax-up. Visual analogue scales (VAS) were used to rate the intuitiveness, understanding, and satisfaction or help between the two methods by patients and dental specialists. Results According to the ratings from the patients, 3D treatment simulation showed obvious advantages in the aspects of intuitiveness (9.7 +/- 0.5 vs 6.4 +/- 1.4) and treatment understanding (9.1 +/- 0.8 vs 6.6 +/- 1.5), and the satisfaction rates were also higher (9.0 +/- 0.6 vs 7.1 +/- 1.8). Dental specialists regarded the 3D digital plans as more intuitive (8.9 +/- 0.8 vs 5.9 +/- 1.0) and useful to understand the plans from the other specialists (8.9 +/- 0.7 vs 6.1 +/- 1.0) and helpful to their own treatment plans (8.7 +/- 0.9 vs 5.9 +/- 1.4). Conclusions The interdisciplinary 3D digital treatment simulation helps both patients and dental specialists to improve treatment understanding, and facilitates dental specialists for decision-making before complex esthetic rehabilitation.

Abstract:Introduction: Orthodontists and surgeons have been looking for more accurate methods to plan and predict surgical outcomes in patients with skeletal discrepancies. Methods: The sample consisted of 20 subjects from the surgical clinic of a graduate orthodontic program who had been treated with Le Fort I maxillary movement, bisagittal split osteotomy, with or without genioplasty. All subjects had to have preoperative (T0) and at least 6 months postoperative (T1) cone-beam computed tomographies that were imported to Dolphin three-dimensional (3D) software version 11.9 in digital imaging and communications in medicine format. Three-dimensional voxel-based superimposition on the cranial base was performed for T0 and T1 to accurately measure the skeletal surgical movements. A virtual orthognathic surgery was performed on T0 to mimic the actual skeletal osteotomies using the treatment simulation tool in Dolphin 3D. A prediction 3D soft tissue image (Tp) was generated based on the Dolphin virtual skeletal planning. The differences between Tp and T1 for all patients were measured using linear and angular measurements visualized by surface mapping. Results: Significant differences were found between Tp and T1 in Nasolabial angle, Soft tissue A point, and Subalar area. Conclusions: The soft tissue prediction accuracy after double jaw surgery using Dolphin 3D is limited in some areas, especially upper lip and base of the nose.

Abstract:The need to measure alpha particle emissivities at levels below 0.005 /spl alpha//cm/sup 2/-hr is becoming increasingly important in fundamental physics experiments (e.g. neutrino and rare decay measurements), environmental monitoring, nuclear activities monitoring and semiconductor packaging materials. Present counters can barely reach this level, being limited both by cosmic ray events and by their own alpha emissions. Here we report a detector capable of measurements at 0.00005 /spl alpha//cm/sup 2/-hr using a large electrode ionization chamber with digital pulse shape analysis to locate the point of emission of each alpha particle. Filled with 1 atm N/sub 2/, the counter is essentially blind to both environmental gamma-rays and cosmic ray muon showers, so its background becomes limited by the ability of the pulse shape analysis to distinguish different points of alpha particle origin. The counter's geometry intentionally exaggerates differences between signals originating from its different surfaces, with an inter-electrode separation D over 3 times the alpha particle range L. Since signal risetimes equal charge drift times, anode events have 8-10 /spl mu/s risetimes, while sample event risetimes are 30-35 /spl mu/s and readily distinguished. Integrated charge also increases with drift length, producing a 2 to 1 difference between sample and anode events. Applying both risetime and amplitude cuts distinguishes between sample and anode emitted alpha particles at about 1 part in 1000. A guard electrode surrounding the anode allows alpha particles from the counter's sidewalls to be rejected at a similar ratio, so that essentially only alpha particles emanating from the sample are finally counted.
