Abstract:Aim: To study the magnitude of objective accommodative response (OAR) with account to the method of image defocusing. The latter can be realized by bringing the object nearer to the eye or by adding a minus lens in front of the viewer. Material and methods: We examined 63 patients (126 eyes) with different refraction aged from 7 to 31 years. All patients underwent a series of OAR measurements on WR-5100K Grand Seiko Binocular Open-Field Autorefkeratometer under different conditions, including negative spherical lenses of -3.0 D and -5.0 D, test optotypes corresponding to visual acuity of 0.2 and 0.7 and presented at a distance of 5 m, and a set of variably sized optotypes presented at a distance of 33 cm and 20 cm. Results: A disparity was found between the magnitude of accommodative responses evoked through different means: placing a negative lens in front of the viewer while he/she looks into the distance or bringing the object closer to the eye. In almost all cases lens-induced responses were less pronounced than distance-induced. In all myopic children, accommodative lag was longer at near than at distance. Generally, in all groups, OAR to distant objects as well as responses to a 3.0 D accommodative task at near did not depend on the size of the object. There was an insignificant and statistically unreliable difference for objects placed 20 cm away (5.0 D accommodative task): OAR to a smaller font was stronger than that to a larger one. Conclusion: In children and adults with emmetropia, hyperopic children, and myopic adults, longer accommodative lags were characteristic of lens-induced responses. In children with myopia, longer lags were observed at near. Moreover, at near, OAR to a smaller font was stronger than that to a larger one.

Abstract:Purpose To investigate the influence of phenylephrine and tropicamide on anterior segment biometry with ultralong scan depth optical coherence tomography (UL-OCT) during accommodation. Methods In this study, 20 left eyes of healthy volunteers with a mean ± standard deviation age of 31.05 ± 5.84 years and a mean refraction of −1.16 ± 1.11 diopters (range 0∼−3.0 D) were imaged using UL-OCT after instillation of artificial tears, phenylephrine, and tropicamide in three follow-up trials, respectively. At each follow-up trial, two repeated measurements were performed at states of relax and 5D accommodative stimulation. The dimensional parameters included central corneal thickness (CCT), anterior chamber depth (ACD), pupil diameter (PD), lens thickness (LT), and horizontal radii of the lens anterior and posterior surface curvatures (LAC and LPC). Results Tropicamide led to larger pupil, deeper ACD, thinner LT, and flatter crystalline lens surface (P < 0.05). Phenylephrine induced an increase in PD (P < 0.05), while no significant changes were seen in ACD, LT, LAC, and LPC (P > 0.05). CCT did not change after both phenylephrine and tropicamide instillation in this study (P > 0.05). Tropicamide induced the loss of accommodation and phenylephrine achieved pupil dilation without affecting the accommodation. PD, ACD decreased, LT increased significantly and the anterior and posterior surface of the lens in a 6.294 mm of diameter optical zone became steeper during accommodation after administration of phenylephrine (P < 0.05). Conclusion The anterior segment physiology changed after tropicamide instillation. Besides, tropicamide induced the loss of accommodation and phenylephrine preserved the accommodation with a larger pupil. And, the anterior and posterior surface of lens in a 6.294 mm of diameter optical zone became steeper during the accommodation.
Correlation analysis between myopic refraction and ocular components

Abstract:Objective To investigate the relationship between myopic refraction and ocular components.Methods Retinoscopy was performed in 533 myopic subjects (1018 eyes).K value was measured by corneal mapping,and A/B ultrasound scan to get AL,ACD and LT.VCD,AnL,RLT/AL and RAL/R were calculated.Results In myopia,AL was not associated with LT (r =0.028,P =0.364); SEQ and AL,ACD,LT,K,VCD,RAL/R,RLT/AL were all significantly correlated (P ＜0.01),and better associated with RAL/R,A1 and VCD (r =-0.815;-0.624;-0.619); ACD,LT and AnL were not statistically different between myopia groups (ANOVA:P =0.153; 0.063; 0.625),nonetheless,all the other parameter did (P ＜0.01).Conclusions Myopic severity can be featured by the significant difference of posterior segment measurement,the anterior segment is comparatively stable and statistical insignificant.

Abstract:To measure by ultralong scan depth optical coherence tomography (UL-OCT) dimensional changes in the anterior segment of human eyes during accommodation.Evaluation of diagnostic test or technology.Forty-one right eyes of healthy subjects with a mean age of 34 years (range, 22-41 years) and a mean refraction of -2.5 ± 2.6 diopters were imaged in 2 repeated measurements at minimal and maximal accommodations.A specially adapted and designed UL-OCT instrument was used to image from the front surface of the cornea to the back surface of the crystalline lens. Custom software corrected the optical distortion of the images and yielded the biometric measurements. The coefficient of repeatability and the intraclass correlation coefficient were calculated to evaluate the repeatability and reliability.Anterior segment parameters and associated repeatability and reliability upon accommodation. The dimensional results included central corneal thickness (CCT), anterior chamber depth and width (ACD, ACW), pupil diameter (PD), lens thickness (LT), anterior segment length (ASL = ACD + LT), lens central position (LCP = ACD + 1/2LT), and horizontal radii of the lens anterior and posterior surface curvatures (LAC, LPC).Repeated measurements of each variable within each accommodative state did not differ significantly (P>0.05). The coefficients of repeatability (CORs) and intraclass correlation coefficients for CCT, ACW, ACD, LT, LCP, and ASL were excellent (1.2%- 3.59% and 0.998-0.877, respectively). They were higher for PD (18.90%-21.63% and 0.880-0.874, respectively) and moderate for LAC and LPC (34.86%-42.72% and 0.669-0.251, respectively) in the 2 accommodative states. Compared with minimal accommodation, PD, ACD, LAC, LPC, and LCP decreased and LT and ASL increased significantly at maximal accommodation (P<0.05), whereas CCT and ACW did not change (P>0.05).The UL-OCT measured changes in anterior segment dimensions during accommodation with good repeatability and reliability. During accommodation, the back surface of the lens became steeper as the lens moved forward.The authors have no proprietary or commercial interest in any of the materials discussed in this article.
[Wavefront and accommodation parameters under different conditions of correction in myopia and hyperopia]

Abstract:Purpose: To compare the wavefront and accommodation parameters without correction and in soft contact lenses (SCL) in natural and cycloplegic conditions in eyes with myopia and hyperopia. Material and methods: A total of 142 myopic (mean -5.6±1.4 D) and 48 hyperopic (mean +3.5±1.1 D) eyes were examined in 95 patients aged 5-32 years (mean age 16.9±0.9 years) to compare the wavefront aberrations without correction and with different SCL before and after cycloplegia (two drops of cyclopentolate hydrochloride 1%). The device was set up for 4 mm zone for both narrow and wide pupils. To compare the accommodation parameters under different correction conditions, 85 patients aged 8-23 years (mean age 14.9±0.6 years) with average myopia of (-)5.27±1.4D (123 eyes) and average hyperopia of +3.53±1.2 D (46 eyes) were chosen from the study group. Among the measured parameters are objective accommodative response (OAR), relative accommodation reserves (RAR), pseudoaccumulation amplitude (PA), higher-order aberrations: RMSHOAs, 6-9 Trefoil, 7-8 Coma, spherical aberration (SA). Results: In myopic eyes with SCL Coma 7 decreases, Coma 8 increases with transition to positive values, and Trefoil 9 increases. In hyperopic eyes, trefoil 6 decreases, Coma 7-8 go negative. In myopic or hyperopic eyes with SCL, SA goes from positive to negative. In both myopia and hyperopia, accommodation and PA rates are higher in SCL than in glasses. Conclusion: SCL change certain wavefront parameters for myopia and hyperopia in different ways. The accommodation parameters in SCL are elevated in both myopia and hyperopia. The negative spherical aberration induced by contact lenses improves the accommodative response. The revealed features should be considered in the development of correction methods that target refractogenesis.

Abstract:To investigate changes in anterior chamber depth (ACD) and the distance between the Visian implantable Collamer lens (ICL) and the crystalline lens during pharmacologic accommodation in high myopia.Prospective, comparative case series.Thirty-three phakic eyes of 18 high myopic patients (range, -8.63 to -23.86 diopters) with a mean age of 29 years (range, 20-44 years) were examined at least 1 year after ICL implantation.Ultrasound biomicroscopy was used to measure distance changes between the corneal endothelium, the ICL, and the crystalline lens after inducing pharmacologic accommodation with topical pilocarpine in 1 eye. The contralateral eye served as the control.Mean changes of ACD measured from the posterior corneal surface to the crystalline lens (ACD-L), from the posterior corneal surface to the anterior surface of the ICL (ACD-ICL), and the distance between the ICL and the crystalline lens (ICL-L) at the central and peripheral regions of the eye.For each eye, the ICL was in contact with the iris, but it was never in contact with the crystalline lens. At baseline, the mean distance between the ICL and the crystalline lens was 0.609 ± 0.165 mm at the central horizontal meridian, 0.588 ± 0.157 mm at the central vertical meridian, 0.281 ± 0.106 mm at the peripheral temporal sulcus, and 0.290 ± 0.098 mm at the peripheral nasal sulcus. After instillation of pilocarpine, a significant decrease in ICL-L was accompanied by a significant reduction in ACD-L and an increase in ACD-ICL (P<0.01). There were no significant changes in the control eyes (P>0.05). The central ICL-L reduction in the study group was significantly larger than that in the control group (P<0.01), but the peripheral ICL-L changes in the study group were not significantly different from those in the controls (P>0.05).During pharmacologic accommodation, the ICL and the crystalline lens came closer as the ICL was pushed backward by the iris as a result of pupillary constriction. Simultaneously, the anterior surface of the crystalline lens became more convex and moved forward. Pelvic organ prolapse surgery: changes in perioperative management improving hospital pathway.

Abstract:BACKGROUND There is growing interest in improving operating theatre efficacy and shortening stay in hospital. We made a retrospective patient record review looking at changes in the perioperative management of pelvic organ prolapse surgery during the period 2012 to 2014 in four hospitals in the Gothenburg area in Sweden. METHODS We studied 440 pelvic organ prolapse surgery records; procedures performed during 2012, 2013 and 2014. Anterior colporrhaphy, posterior colporrhaphy and combined procedures were included. Patient characteristics, surgery, anesthetic technique, theatre time, surgical time, time in recovery room, and hospital stay was analyzed. RESULTS We found major changes in perioperative management at the same time as local anesthesia and sedation (LAS) increased. Operating room efficiency was increased; increased number of patients bypassed the recovery room and hospital stay was overall shortened, an increasing number of patients were discharged day of surgery. Patients discharge day of surgery increased from 24% to 72% between 2012 and 2014. Surgery under LAS increased from 87% to 96% of the procedures in the private hospital and from 18% to 83% in the university hospital between 2012 and 2014. CONCLUSIONS There is a clear change in perioperative care of patients undergoing pelvic organ prolapse surgery to a more enhanced perioperative management. Routine and practices differs however between hospitals. The use of local anesthesia and sedation improve theatre turn around and shortens hospital stay.

Abstract:Medical research continues to illuminate the origins of many human diseases. Gene therapy has been widely proposed as a novel strategy by which this knowledge can be used to deliver new and improved therapies. Viral gene transfer is relatively efficient but there are concerns relating to the use of viral vectors in humans. Conversely, nonviral vectors appear safe but inefficient. Therefore, the development of an efficient nonviral vector remains a highly desirable goal. This review focuses on the numerous challenges preventing efficient nonviral gene transfer in vivo and discusses the many technologies that have been adopted to overcome these problems.

Abstract:Objective To investigate the changes in accommodative response and anterior segment biometry during accommodation in young progressive myopes who wear overnight orthokeratology lenses.Methods Eighteen children (age:14.4±2.6 years) were enrolled in this prospective experimental study and were tested before wearing orthokeratology (OK) lenses and after wearing them for three months.Using a Badal optical system,the refractive error of each subject was corrected before and after lens wear,and then accommodative stimuli of 3 D and 5 D were presented.A custom-built optical coherence tomographer (OCT) with a long scan depth was used to image the entire anterior segment through the pupil,and to obtain the anterior segment biometry including pupil diameter (PD),anterior chamber depth (ACD),lens thickness (LT) and the lens anterior surface's radius of curvature (LAC).An auto-refractometer (WAM-5500) was used to measure ocular refraction during accommodation,which was used to calculate the accommodative response.The measurements of anterior segment biometry and accommodative response were repeated twice under each accommodative state.Results After wearing orthokeratology lenses for one to three months,an improvement in the accommodative response was evident under both the 3 D (1.72±0.59 D vs.2.42±0.84 D) and 5 D (3.09±0.63 D vs.3.61±0.86 D) accommodative states (paired t-test,t=2.84,2.12,P＜0.05).During accommodation,△PD,△ACD,△LT,and △LAC changed obviously.Compared with baseline (before wearing OK lenses),accommodation-related changes in △ACD (3 D:-0.11±0.04 mm vs.-0.16±0.06 mm,t=3.88,P＜0.01;5 D:-0.15±0.05 mm vs.-0.20±0.07 mm,t=2.37,P＜0.05),△LAC (3 D:-2.60±0.79 mm vs.-3.81±1.08 mm,t=3.96,P＜0.01;5 D:-3.57±1.14 mm vs.-4.32±1.36 mm,t=2.08,P＜0.05) and △LT (3 D:0.22±0.13 mm vs.0.27±0.06 mm,t=-1.94,P＜0.05;5 D:0.26± 0.09 mm vs.0.30±0.10 mm,t=-1.99,P＜0.05) showed statistically significant increases.Conclusion After wearing orthokeratology lenses for 3 months,the accommodative response and changes in the anterior segment biometry increase in myopic children.
The Influence of Accommodation on Retinal Peripheral Refraction Changes in Different Measurement Areas

Abstract:Background. The change in refraction caused by accommodation inevitably affects the peripheral defocus state and thus may influence the effect of retinal peripheral myopic defocus measures in myopia control. This study investigated accommodation changes in different peripheral retinas under cycloplegia to help improve myopia control. Methods. Fifty-six eyes of fifty-six myopic subjects were recruited for this prospective study. The center and peripheral retina refractions were measured using multispectral refractive topography. The subjects were divided into low-to-moderate myopia group (range: −1.25 D to −6.00 D) and high myopia group (range: −6.25 D to −9.75 D) according to spherical equivalent (SE). The compound tropicamide (0.5% tropicamide and 0.5% phenylephrine) was used to relax the accommodation. The difference between cycloplegia and non-cycloplegia peripheral retinal refraction was analyzed using the t-test. The correlation between eccentricity and changes in peripheral refraction was analyzed using Pearson's correlation analysis. Results. The manifest refraction of the retina significantly decreased with an increase in eccentricity after cycloplegia. The annular refraction difference value at 50°–53° (ARDV 50–53) showed the largest refraction decrease of 1.31 D compared with the central retinal refraction decrease of 0.84 D. The inferior quadrantal refraction difference value had the least change compared to the other quadrants. The relative peripheral refraction (RPR) changes in refraction difference value (RDV) at 15° (RDV-15), RDV-30, and RDV-45 were less than 0.15 D. When the range of annulus narrowed to 5°, the narrower annulus showed faster change with eccentricity increase in ARDV 30–35, ARDV 35–40, ARDV 40–45, ARDV 45–50, and ARDV 50–53. The RPR was highly correlated with eccentricity (R = 0.938 and P < 0.001 ). The high myopia group had a greater hyperopic shift in the periphery than the low-to-moderate group after cycloplegia. Conclusions. Peripheral refraction showed a significant hyperopic shift after cycloplegia with an increase in eccentricity. The RPR became more hyperopic than the central refraction. The high myopia group showed more hyperopic shifts in the peripheral region. Accommodation should be taken into consideration in peripheral defocus treatment.

Changes of the Anterior Segment of the Eyes with Minus Lens-Induced Accommodation in Children with Myopia

Abstract:Objective To observe the changes of anterior segment of the eyes in children with low or moderate degrees of myopia and investigate the relationship between defocus induced by different diopters and the variations in anterior chamber depth(ACD) and pupil diameter(PD) when accommodation is induced by single extraocular lenses.Methods Thirty myopic subjects of children were enrolled in the study.ACD and PD were induced of every subject with Visante optical coherence tomography(OTC) under different levels of accommodation(0-6 D).Results With the increase of accommodative stimulation,both ACD and PD decreased at the rate of-1.1817% and-2.8478%,respectively.The variations of the above indicators reached their peaks at the level of 6 D accommodation.Conclusion For childhood myopia,both ACD and PD decrease with the increasing accommodation caused by shortened reading distance,and the changes reach their maximum under an accommodation induced at 6 D.

Abstract:Objective: To explore the interocular differences in both the increase of corneal higher-order aberrations (HOAs) and axial length elongation, and the correlation between them after orthokeratology lens (OK lens) wear in children with myopic anisometropia. Methods: It was a prospective cohort study. From 2018 to 2019, 31 children with myopic anisometropia who were fitted with OK lenses initially in the Department of Ophthalmology of Peking University Third Hospital were enrolled, including 15 males (30 eyes) and 16 females (32 eyes). The age was (10.7±1.4) years. Before (baseline) and 12 months after wearing OK lenses, corneal aberrations, corneal relative refractive power (corneal RRP, the difference between peripheral corneal refraction and central corneal refraction) and axial length were measured using the Sirius combining a corneal topographer and a tomographer and an IOL-Master intraocular lens biometer in two eyes. Interocular differences in corneal aberrations, corneal RRP and axial length were analyzed by the paired t test and Wilcoxon rank sum test. The Pearson and Spearman correlation analyses were used to explore the correlation between the increase in corneal HOAs, corneal RRP and axial length elongation. Results: At 12 months after wearing OK lenses, 6 children were lost for follow-up, and the data of the remaining 25 children (50 eyes) were evaluated. With 3.0- and 6.0-mm measuring zones of the total cornea and the anterior surface, the increases in total corneal HOAs, coma, spherical aberration of higher myopic eyes were significantly more than fellow lower myopic eyes (all P<0.05). The corneal RRP with 2.0-mm and 2.5-mm radius in the upper cornea and 2.5-mm radius in the inferior cornea of higher myopic eyes were more than fellow lower myopic eyes (t=-2.06, -2.07, -2.42; P=0.05, 0.049, 0.023), but there was no significant interocular difference in corneal RRP in the horizontal meridian direction of the cornea (both the nasal and temporal sides of the cornea) (both P>0.05). The axial length elongation in higher myopic eyes was 0.14 (0.08, 0.20) mm, which was significantly less than that in the contralateral eyes [0.18 (0.14, 0.35) mm] (Z =-3.21, P= 0.001). The interocular difference in axial length decreased from 0.40 (0.34, 0.54) mm at baseline to 0.33 (0.24, 0.54) mm (Z =-3.21, P = 0.001). The increases in HOAs, coma and spherical aberration of the anterior corneal surface with 3.0-mm measuring zone and the increase in coma of the anterior corneal surface with 6.0-mm measuring zone were all negatively correlated with axial length elongation (r=-0.493, -0.606, -0.500, -0.307; P<0.05). But the corneal RRP had no significant correlation with axial length elongation (r=-0.092, -0.104, -0.230, -0.061; P>0.05). Conclusion: After 12 months of OK lens wear, interocular difference in axial length reduced significantly in children with myopic anisometropia, which was related to the larger increase in corneal HOAs of higher myopic eyes than fellow lower myopic eyes, especially coma.

Abstract:Soft-switching PWM combined three-level (TL) converters are very suitable for high input voltage with wide range applications due to the reduced voltage stress on power switches and reduced filter requirements. The converters include two transformers and have two operation modes, which provide possibilities to optimize the performance of the converters. This paper proposes some optimization strategies for the combined TL converters based on the control strategies and turns-ratios to achieve a better performance. Two prototype converters have been tested for the verification of the theoretical analysis.

Abstract:AIM: To compare clinical results between toric and spherical periphery design orthokeratology (ortho-k) in myopic children with moderate-to-high corneal astigmatism.METHODS: This retrospective study enrolled 62 eyes of 62 subjects using toric ortho-k lenses. These subjects were assigned to the toric group. Based on the one-to-one match principle (same age, proximate spherical equivalence and corneal astigmatism), 62 eyes of 62 subjects were enrolled and included in the spherical group. At one-year follow-up visit, visual acuity, corneal astigmatism, treatment zone decentration, axial elongation and adverse reaction were compared between these two groups.RESULTS: At the one-year visit, corneal astigmatism was significantly lower in the toric group (1.22 +/- 0.76 D) than in the spherical group (2.05 +/- 0.85 D) (P=0.012). The mean magnitude of the treatment zone decentration was 0.62 +/- 0.42 mm in the toric group and 1.07 +/- 0.40 mm in the spherical group (P=0.004). Axial elongation was significantly slower in the toric group (0.04 +/- 0.13 mm) than in the spherical group (0.09 +/- 0.13 mm) (P=0.001). The one-year axial elongation was significantly correlated with initial age (r=-0.487, P<0.001) and periphery design of ortho-k lens (r=0.315, P<0.001). The incidence of corneal staining was lower in the toric group (8.1%) than in the spherical group (19.4%) (P<0.001).CONCLUSION: Toric periphery design ortho-k lenses may provide lower corneal astigmatism, better centration, slower axial elongation and lower incidence of corneal staining in myopic children with moderate-to-high corneal astigmatism.
Association Between Corneal Biomechanical Properties and Myopia in Chinese Subjects

Abstract:To examine the relationship between corneal biomechanical properties and the degree of myopia. Chinese subjects (n=172, age: 11−65 years) were divided into diagnostic groups with non-myopia (spherical equivalence (SE)>−0.50 D), low (−3.00≤SE≤−0.50 D), moderate (−6.00≤SE<−3.00 D), and high myopia (SE<−6.00 D). Only the right eye of each subject was analyzed. Central corneal thickness (CCT) was measured by optical coherence tomography. An ocular response analyzer was used to measure corneal hysteresis (CH), corneal resistance factor (CRF), intraocular pressure (IOP), and corneal compensated IOP (IOPcc). Refraction was measured by both automated and subjective refractometry and expressed as SE. CH was significantly lower in high myopia compared with both low and non-myopia (P≤0.002). CCT was 1.5 times more correlated to CH variation compared with refraction. Similarly, CRF was four times more dependent on CCT than refraction. CH (P<0.001) or CRF (P=0.005) was positively correlated to refraction. Both IOP and IOPcc were negatively correlated to refraction (P<0.001), respectively. CH decreases only in high myopia. Refraction is positively correlated to both CH and CRF but negatively correlated to both IOP and IOPcc. These results indicate that the mechanical strength in anterior segment of the eye is compromised in high myopia. In addition, high myopia may increase the risk of glaucoma.
The Relationship Between Corneal Biomechanical Parameters and Treatment Outcomes of Orthokeratology Lenses

Abstract:Background To evaluate changes in corneal biomechanical properties after long-term orthokeratology (OK) treatment and the factors affecting treatment outcomes. Methods Twenty-four myopic teenagers who wore OK lenses for more than 1 year were included. Twenty-three individuals of the same age and with the same spherical equivalent wearing single-vision spectacles (SVS) were enrolled as controls. After routine eye examinations, corneal biomechanical properties and axial length were measured. Parameters were compared between groups. Results Less axial elongation (AE) occurred in the OK group ( P = 0.021). The OK group experienced a statistically significant decrease in the A1 deformation amplitude ( P = 0.02), whole eye movement maximum ( P = 0.026), and Ambrósio’s relational thickness to the horizontal profile (ARTh) ( P < 0.001), and a statistically significant increase in the pachyslope ( P < 0.001) and Corvis biomechanical index ( P < 0.001). Smaller ARTh and a larger highest concavity deflection area resulted in a better refractive state. The inhibitory effect of AE was better for older patients with smaller ARTh. Conclusions Long-term OK treatment slowed myopia progression by reshaping the cornea. Smaller ARTh after OK lens wear indicated a better refractive state and slower AE and could predict OK lens treatment outcomes.

Abstract:Purpose: To determine the biometry of anterior segment dimensions of the human eye on both horizontal and vertical meridians with extended scan depth optical coherence tomography (OCT) during accommodation.Methods: Twenty pre-presbyopic volunteers, aged between 24 and 30, were recruited. The ocular anterior segment of each subject was imaged using an extended scan depth OCT under non-and 3.0 diopters (D) of accommodative demands on both horizontal and vertical meridians. All the images were analyzed to yield the following parameters: pupil diameter (PD), anterior chamber depth (ACD), anterior and posterior surface curvatures of the crystalline lens (ASC and PSC) and the lens thickness (LT). Two consecutive measurements were performed to assess the repeatability and reproducibility of this OCT. They were evaluated by calculating the within-subject standard deviation (SD), a paired t-test, intra-class correlation coefficients (ICC) and the coefficient of repeatability/reproducibility (CoR).Results: There were no significant differences between two consecutive measurements on either horizontal or vertical meridians under both two different accommodative statuses (P>0.05). The ICC for all parameters ranged from 0.775 to 0.998, except for the PSC (0.550) on the horizontal meridian under the non-accommodative status. In addition, the CoR for most of the parameters were excellent (0.004% to 4.89%). In all the parameters, only PD and PSC were found different between the horizontal and vertical meridians under both accommodative statuses (P<0.05). PD, ACD, ASC and PSC under accommodative status were significantly smaller than those under the non-accommodative status, except that the PSC at the vertical meridian did not change. In addition, LT was significantly increased when accommodation.Conclusion: The extended scan depth OCT successfully measured the dimensions of the anterior eye during accommodation with good repeatability and reproducibility on both horizontal and vertical meridians. The asymmetry of lens posterior surface and oval-shaped pupil were found during accommodation.

Abstract:Relevance. Pathological myopia leads to development of myopic maculopathy, which results in impaired visual functions. Formation of retinal pathological changes in the posterior pole of the eye in myopia is based on axial elongation of the eye and the formation of myopic staphyloma. Research methods aimed to clinical assessment of the strength properties of the corneoscleral capsule of the eye are interest to predict the risk of developing myopic staphylomas. Purpose. To conduct a comparative analysis of corneal stromal thickness and its biomechanics properties in pathological and uncomplicated high myopia. Material and methods. 42 patients (67 eyes) with high myopia (axial length of the eye (AL) – 26 mm or more) were examined and divided into 2 groups: the main group – 25 patients (40 eyes) with pathological myopia and presence of myopic staphyloma; the comparison group – 17 patients (27 eyes) with uncomplicated myopia and no staphyloma. Results. Statistically significantly lower values were found: max stromal corneal thickness, central stromal corneal thickness (2 mm), in segments S5, ST5, T5, IT5, I5, IN5, N5, SN5, S6, T6, IT6, I6, IN6, N6, HC radius (p=0.016), as well as an increase in indexes A2-Velocity (p=0.011) and DA (p<0.001) in the group of patients with pathological myopia. A direct correlation was established between the DA parameters and AL of the eye (p=0.008). Conclusion. Stromal corneal thickness in patients with pathological myopia in most segments was significantly lower than in patients with uncomplicated high myopia. The presence of significant differences in pathological myopia in indexes: A2-Velocity, DA, HC radius indicates a greater deformability and softness in cornea, and the lengthening of the DA parameter and its direct correlation with the values of the AL indicates to increased risk of axial elongation of the eye. Abstract:CONTEXT - Fluorescence in situ hybridization (FISH) is a well-established method for detection of genomic aberrations in diagnostic, prognostic, and predictive marker testing. OBJECTIVE - To review common applications of FISH in cytology. DATA SOURCES - The published literature was reviewed. CONCLUSIONS - Cytology is particularly well suited for all kinds of FISH applications, which is highlighted in respiratory tract cytology with an increasing demand for predictive FISH testing in lung cancer. Fluorescence in situ hybridization is the gold standard for detection of predictive anaplastic lymphoma kinase gene (ALK) rearrangements, and the same evaluation criteria as in histology apply to cytology. Several other gene rearrangements, including ROS proto-oncogene 1 receptor tyrosine kinase (ROS1), are becoming clinically important and share the same underlining cytogenetic mechanisms with ALK. MET amplification is one of the most common mechanisms of acquired resistance to epidermal growth factor receptor (EGFR) tyrosine kinase inhibitors and can be targeted by crizotinib. As genomic aberrations are a hallmark of malignant cells, FISH is a valuable objective ancillary diagnostic tool. In urinary tract cytology, atypical urothelial cells equivocal for malignancy are a common diagnostic dilemma and multitarget FISH can help clarify such cells. Diagnosis of malignant mesothelioma remains one of the most challenging fields in effusion cytology, and ancillary FISH is useful in establishing the diagnosis. Fluorescence in situ hybridization is a morphology-based technique, and the prerequisite for reliable FISH results is a targeted evaluation of the cells in question (eg, cancer or atypical cells). Cytopathologists and cytotechnicians should therefore be involved in molecular testing in order to select the best material and to provide their morphologic expertise.
Central And Peripheral Corneal Power Change In Myopic Orthokeratology And Its Relationship With 2-Year Axial Length Change

Abstract:PURPOSE. We assessed the relationship between axial length (AL) change and corneal refractive power change induced by orthokeratology (ortho-k) contact lens wear.METHODS. A total of 88 myopic children aged 7 to 12 years were fitted with ortho-k lenses for overnight wear. Axial length was measured before enrollment and every 6 months after initiation of lens wear up to 24 months. Corneal apical refractive power and the mean power of each concentric ring from 0.5 to 7.2 mm in diameter at 0.1-mm intervals were measured using the Pentacam HR, and compared before and after 3 months of ortho-k lens wear. Summed corneal power change (SCPC) was defined as the sum of relative corneal power change within the central 7.2-mm diameter corneal region. Age, baseline spherical equivalent refractive error (SERE), and SCPC were tested against 2-year AL growth in a stepwise multiple linear regression model.RESULTS. Of the 88 myopic children who were successfully fitted with ortho-k lenses, 64 completed the 2-year follow-up period. These completed test subjects experienced a mean axial growth of 0.37 +/- 0.27 mm. Spherical equivalent refractive error and corneal apical refractive power decreased from baseline values of -2.88 +/- 0.96 diopters (D) and 43.47 +/- 1.03 D to -0.20 +/- 0.85 D and 40.61 +/- 1.23 D, respectively, after 3 months of lens wear. The mean SCPC was 10.84 +/- 5.28 D.mm after 3 months. Axial elongation was significantly correlated with SCPC (standardized beta = -0.573, P < 0.001) and baseline age (standardized beta = -0.403, P < 0.001), but not with baseline SERE (beta = 0.081, P = 0.396). The regression model using SCPC and baseline age to predict axial elongation was acceptable (adjusted R-2 = 0.56) and statistically significant (F-2,F-61 = 41.09, P < 0.001).CONCLUSIONS. Axial elongation in children undergoing ortho-k therapy is negatively correlated with the age of the wearer and summed corneal power change from the central to the mid-peripheral cornea. We speculate that axial elongation is influenced by the extent to which the retinal defocus profile is altered with ortho-k lens wear. (http://www.chictr.org number, ChiCTR-TNRC-11001210.)
