Abstract:In Sri Lanka, and across the world, ethics is becoming an ever increasingly integral part of clinical practice and biomedical research. Comprehensible and transparent ethics standards are essential to public trust in scientific research studies. In the pursuit of ethical research, Ethics Review Committees play a momentous role in the review and monitoring of biomedical research, especially in the case of clinical studies. National legislation and specific legal guidelines which systematically regulate the establishment, functioning, registration, and/or accreditation of such committees in Sri Lanka are yet to be developed. The National Medicines Regulatory Authority, an independent authority within the Sri Lankan Ministry of Health, plays an important role in the regulation of clinical trials in Sri Lanka. This review, albeit briefly, highlights the history of bioethics, the current state of Ethics Review Committees in Sri Lanka, and the recent efforts taken to assess their operations to ensure they meet international standards. In addition, this manuscript critically analyzes the membership composition and quorum requirements of selected Ethics Review Committees in Sri Lanka, as well as the challenges faced by these committees today.

Abstract:A REVIEW WAS CONDUCTED OF ENGLISH-language peer-reviewed and gray literature on health and ethics written by authors from Bangladesh, China, India, and Pakistan. This was supplemented by the knowledge of co-authors who are involved in bioethics capacity building in these countries. Of the identified literature that focused on the application of Western principles, it largely discussed informed consent and revealed norms in clinical decision-making that include physician paternalism, family involvement in decision-making, and reluctance to provide information that might upset patients. It appears that Western ethical principles may be interpreted and applied in unexpected ways. The literature further indicates that, although there is some consistency with Western ideas, Islamic, Confucian, and Indian religious and philosophical traditions contain concepts not reflected in international guidance. Findings suggest scholars from these countries seek to enter into a bioethics dialogue with the potential to enrich and inform "international" frameworks.

Abstract:Giant unilamellar vesicles (GUVs of diameter 5-25 μm) of soy phosphatidylcholine (PC), resistant to intense light exposure (400-440 nm, ~15 mW·mm(-2)), underwent budding when containing chlorophyll a (Chla) in the lipid bilayer ([PC]:[Chla] = 1500:1). On the basis of image heterogeneity analysis using inverted microscopy, a dimensionless entropy parameter for the budding process was shown to increase linearly during an initial budding process. Lipophilic β-carotene (β-Car, [PC]:[β-Car] = 500:1) reduced the initial budding rate by a factor of 2.4, while the hydrophilic glycoside rutin ([PC]:[rutin] = 500:1) had no effect. Chla photosensitized oxidation of PC to form linoleoyl hydroperoxides, further leading to domains of higher polarity in the vesicles, is suggested to trigger budding. The average dipole moment (μ) of linoleic acid hydroperoxides was calculated using density functional theory (DFT) to have the value of 2.84 D, while unoxidized linoleic acid has μ = 1.86 D. β-Carotene as a lipophilic antioxidant and singlet-oxygen quencher seems to hamper oxidation in the lipid bilayers and delay budding in contrast to rutin located in the aqueous phase. The effect on budding of GUVs as a detrimental process for membranes is suggested for use in assays for evaluation of potential protectors of cellular integrity and functions under oxidative stress.

Abstract:Objectives: Increasingly (but not exclusively), terrorist activity and the use of explosive devices have enjoyed the focus of the global media. This paper aims to bring a range of issues to attention, to highlight how burn injuries are sustained in such incidents and why burn injuries (and thus burn disasters) are so complicated to manage. Materials and Methods: The author's experience with burn injury caused during explosions and his involvement in burn disaster situations has been summarized to form the basis of the article. This has been expanded upon with discussion points which provide a strategy for planning for such events and by a broad sample of the literature. Results: Several strategies are suggested to facilitate planning for burn disasters and to illustrate to those not directly involved why forward planning is pivotal to success when these incidents occur. Conclusions: Disasters generating large numbers of burn-injured are relatively frequent. Explosive devices are widespread in their use both in military and increasingly in civilian fields. Encompassing a large range of aetiologies, geographical sites, populations, and resources; burn disaster management is difficult and planning essential.

Abstract:Clinical trials are crucial in developing safe medical treatments and combating diseases. However, the ethical considerations surrounding research involving human subjects have been an ongoing topic of debate. Existing ethical policies aim to ensure the accuracy of research findings and protect the well-being of participants. Nevertheless, these policies have been rooted in Western scientific and medical systems, which historically exploited communities for the benefit of privileged individuals and capital accumulation, perpetuating domination and settler colonialism. While ethical policies alone cannot erase these harmful legacies, they have globally failed at holding researchers, companies, and institutions accountable for their impact on communities. This failure has led to exploitation and unintended harm in disenfranchised communities with under-resourced health systems and limited access to healthcare resources. To address these challenges, this literature review proposes integrating stricter regulations, transparent disclosure of trial results, and comprehensive post-trial care. Additionally, it advocates for including community partnerships in clinical trial ethics policies to prioritize community needs and promote accountability. By examining qualitative studies on the perspectives of patients, researchers, and clinicians involved in clinical trials, as well as the current state of clinical trial ethics policies, this paper suggests a partnership-based approach that can facilitate the development of new treatments while addressing historical legacies of exploitation and harm in disenfranchised communities worldwide.

Abstract:Recently, significant advancements have been achieved in the field of biomedical research, including the improvement of new and effective diagnostic tools, novel treatments, as well as a better knowledge of illnesses and their causes. Major favourable progress is promised by current biomedical research, which also undoubtedly presents substantial difficulties. In the multi-layered, dispersed, and complicated structure of research governance, issues like personal or institutional conflicts of interest and risks to the individual, community, and society are not appropriately handled. The current study captures the important milestones in the guideline of biomedical research and advancement in India in terms of public support, government programme initiatives, and global and national regulatory frameworks. As one of the primary systems for regulating the ethical and legal aspects of biomedical research, this essay also examines the duties and responsibilities of the ethics committee.

Decentralised clinical trials in rural Australia: Opportunities and challenges

Abstract:Aims: To present opportunities and a model to redress the under-representation of rural communities and people in Australian clinical trials. Context: Clinical trials are essential for building and understanding the health evidence base. The lack of representation of rural people in clinical trials is evident in other countries. Examining the Australian New Zealand Clinical Trial Registry (ANZCTR) suggests this is also the case in Australia. Approach: We propose an approach that empowers rurally based academics and clinicians to co-design clinical trials and increase rural Australians' participation in clinical trials to address this inequality of access. A case study of a decentralised, co-designed clinical trial is presented to support this approach. Conclusion: Decentralising clinical trials could improve access to clinical trials, strengthen the social capital of rural communities and help address the health inequalities that exist between rural and metropolitan communities.
A comparative ethical analysis of the Egyptian clinical research law

Abstract:Background: In this study, we examined the ethical implications of Egypt's new clinical trial law, employing the ethical framework proposed by Emanuel et al. and comparing it to various national and supranational laws. This analysis is crucial as Egypt, considered a high-growth pharmaceutical market, has become an attractive location for clinical trials, offering insights into the ethical implementation of bioethical regulations in a large population country with a robust healthcare infrastructure and predominantly treatment-naïve patients. Methods: We conducted a comparative analysis of Egyptian law with regulations from Sweden and France, including the EU Clinical Trials Regulation, considering ethical human subject research criteria, and used a directed approach to qualitative content analysis to examine the laws and regulations. This study involved extensive peer scrutiny, frequent debriefing sessions, and collaboration with legal experts with relevant international legal expertise to ensure rigorous analysis and interpretation of the laws. Results: On the rating of the seven different principles (social and scientific values, scientific validity, fair selection of participants, risk-benefit ratio, independent review, informed consent and respect for participants) Egypt, France, and EU regulations had comparable scores. Specific principles (Social Value, Scientific Value, and Fair selection of participants) were challenging to directly identify due to certain regulations embodying 'implicit' principles more than explicitly stated ones. Conclusion: The analysis underscores Egypt's alignment with internationally recognized ethical principles, as outlined by Emanuel et al., through its comparison with French, Swedish, and EU regulations, emphasizing the critical need for Egypt to continuously refine its ethical regulations to safeguard participant protection and research integrity. Key issues identified include the necessity to clarify and standardize the concept of social value in research, alongside concerns regarding the expertise and impartiality of ethical review boards, pointing towards a broader agenda for enhancing research ethics in Egypt and beyond.

Abstract:With the increasing number of clinical trials being undertaken in China over the last few years, stakeholders in human subjects protections (HSP) are facing many new challenges; currently, these challenges in HSP are largely taken up by the ethical review committees and study investigators. This article identifies a number of issues with the present system and suggests ways to resolve these issues at the institutional level. Though the Chinese government has developed a basic regulatory framework for HSP, further work still needs to be done.

Abstract:Every human research creates generalizable information which is an integral part of health improvement & understanding of disease. It also causes some risk or inconvenience to research participants. Codes of conduct for physicians have existed since ancient period. Declaration of Helsinki ICMR, Human subjects’ protection; Animal care guidelines etc provide a policy for conduct of research involving research participants in India. The Why & the How in research are vital questions. The ‘Why’ is very clear to define as improvement of public health. What matters more is the ‘How’: while performing it is necessary to adhere to research ethics for promotion of knowledge, truth and avoidance of errors. Research being team work, involves principles of team work such as trust, mutual respect, even handedness, and accountability. Upholding confidentiality, data sharing policies, guidelines of authorship, peer review policies, copyright and patenting policy promotes security of intellectual property and gives confidence for collaborations. Ethical research outlines standard guidelines for institutions and professionals for their behavior towards the specified goals and helps to develop the trust of community further promoting funding.

Abstract:We consider a generic reaction-diffusion system in which one of the parameters is subjected to dichotomous noise by controlling the flow of one of the reacting species in a continuous-flow-stirred-tank reactor (CSTR) -membrane reactor. The linear stability analysis in an extended phase space is carried out by invoking Furutzu-Novikov procedure for exponentially correlated multiplicative noise to derive the instability condition in the plane of the noise parameters (correlation time and strength of the noise). We demonstrate that depending on the correlation time an optimal strength of noise governs the self-organization. Our theoretical analysis is corroborated by numerical simulations on pattern formation in a chlorine-dioxide-iodine-malonic acid reaction-diffusion system.

Abstract:It is determined that in order to create a reliable modern scientific foundation for understanding the essence of somatic human rights in the process of biomedical research, a historical digression into the specifics of research of a legal issue of constitutional and legal bases of consolidation of certain somatic rights, selection of certain research priorities and previously existing and current legislation was formed. This will highlight certain trends in the development of both legal doctrine and rule-making activities, the result of which is a radical improvement of current legislation of Ukraine, including codified content, to outline certain issues while clarifying ways to solve them in different historical periods. to avoid negative in the future and borrow positive experiences. Taking into account the historical experience of doctrinal research of certain legal phenomena, scientists say, allows to form a perfect modern scientific basis for improving the constitutional and legal status of a legal institution. It is pointed out that the history of formation and development of the mechanism of realization and protection of somatic human rights is an integral part of the historical and legal process. The study of positive experiences in this field, which has deep historical roots and is closely linked to socio-economic and political processes, is important for both theory and practice. An integral element of scientific intelligence of any level and direction is a thorough source base, critical analysis and systematization of which is the primary task of a true scientist. This is what makes it possible to carry out objective and impartial research in the field of jurisprudence, as modern scholars emphasize.

Abstract:Abstract Background Medical ethics deals with the ethical obligations of doctors to their patients, colleagues and society. The annual reports of Sri Lanka Medical Council indicate that the number of complaints against doctors has increased over the years. We aimed to assess the level of knowledge, attitude and practice regarding medical ethics among doctors in three teaching hospitals in Sri Lanka. Methods A hospital-based cross-sectional study was conducted among doctors ( n = 313) using a pre-tested self-administered, anonymous questionnaire. Chi Squared test, and ANOVA test were used to identify the significance of association between level of knowledge and selected factors. Results Most doctors (81.2%) had a poor level of knowledge on medical ethics, with postgraduate trainees showing significantly ( p = 0.023, Chi square) higher level of knowledge. The average knowledge on medical ethics among doctors was significantly different between the three hospitals ( p = 0.008, ANOVA). Over 95% had a favourable attitude towards gaining knowledge and advocated the need for training. The majority (69.3%) indicated awareness of unethical practices. 24.6% of respondents stated that they get a chaperone ‘sometimes’ during patient examination while 3.5% never do. The majority (54%) responded that they never accept gifts from pharmaceutical companies in recognition of their prescribing pattern. 12–41% of doctors participated in the study acknowledged that they ‘sometime’ engaged in unethical practices related to prescribing drugs, accepting gifts from pharmaceutical companies and when obtaining leave. Conclusion Most doctors had a poor level of knowledge of medical ethics. Postgraduate trainees had a higher level of knowledge than other doctors. The majority showed a favourable attitude towards gaining knowledge and the need of training. Regular in-service training on medical ethics for doctors would help to improve their knowledge on medical ethics, as well as attitudes and ethical conduct.

Abstract:We evaluated the prevalence and severity of occult coronary artery disease (CAD) and cerebrovascular disease (CeVD) in patients with abdominal aortic aneurysm (AAA). We studied 100 consecutive patients with no history of CAD, normal electrocardiogram, normal systolic function, and no angina or dyspnea. All patients underwent carotid Doppler study and invasive coronary angiography. Significant CAD was observed in 61% of patients. In all, 51% of patients with significant CAD showed either left main (n = 7), 3-vessel (n = 17), or proximal left anterior descending (n = 7) CAD, corresponding to 31% of the total cohort. Cerebrovascular disease was detected in 53% of patients, and in 38% of them was significant (peak systolic flow velocity ≥125 <230 cm/s). In 36% of patients with CeVD either left main (n = 5), 3-vessel (n = 11), or proximal left anterior descending (n = 3) CAD was observed. Severe asymptomatic CAD is prevalent in AAA, and 31% of patients fulfill indications for coronary revascularization.

Abstract:There are good practical reasons to use electronic consent (e-consent) in randomised trials, especially when conducting large-scale clinical trials to answer population-level health research questions. However, determining ethical reasons for e-consent is not so clear and depends on a proper understanding of what e-consent means when used in clinical trials and its ethical significance. Here we focus on four features of ethical significance which give rise to a range of ethical considerations relating to e-consent and merit further focused ethics research.
