Abstract:A complete model of the universe needs at least three parts: (1) a complete set of physical variables and dynamical laws for them, (2) the correct solution of the dynamical laws, and (3) the connection with conscious experience. In quantum cosmology, item (1) is often called a `theory of everything,' and item (2) is the quantum state of the cosmos. Hartle and Hawking have made the `no-boundary' proposal, that the wavefunction of the universe is given by a path integral over all compact Euclidean 4-dimensional geometries and matter fields that have the 3-dimensional argument of the wavefunction on their one and only boundary. This proposal has had several partial successes, mainly when one takes the zero-loop approximation of summing over a small number of complex extrema of the action. However, it has also been severely challenged by an argument by Susskind.

Abstract:The success of an esthetic rehabilitation depends on the understanding of the patient's need and expectation. The management of patients with moderate to severe tetracycline-stained teeth is very challenging. Tooth whitening may be a valid alternative to more aggressive treatments; however, patients should be aware of the limitations of tooth whitening therapy. Clinicians may select differing treatment plans; tooth whitening can improve intrinsic discoloration in a way so that no further treatment is required. Once tooth whitening is completed, direct or indirect restorative procedures may be afforded to match the existing restoration with the bleached tooth structure. This article describes a conservative clinical approach to rehabilitate the smile of a patient with moderate to severe tetracycline-stained teeth using a combination of tooth whitening and direct composite and indirect porcelain restorations in the maxillary anterior segment. Clinical significance: The combination of tooth whitening and adhesive restorations allows clinicians a significantly more conservative approach to intrinsically stained teeth; tooth preparation for porcelain veneers and porcelain-fused-to-metal and full-ceramic crowns can be restricted to conditions in which persistent tooth discoloration or significant loss of both dentin and enamel exists.

Abstract:Gingival hyperpigmentation can significantly affect how the smile looks cosmetically, leading patients to seek treatment. This case report addresses the use of a scalpel approach combined with bur abrasion for gingival depigmentation in a 19-year-old female patient who was displeased with her "black" gums. A local anesthetic was required for the surgical removal of the pigmented epithelium and a thin layer of connective tissue. Post-operative care included antibiotics and analgesics, with chlorhexidine mouthwash use for optimal healing. Follow-up examinations showed successful depigmentation without complications. This simple method is a good choice for gingival pigmentation management since it provides satisfactory cosmetic results and patient satisfaction.

Abstract:Resume. In the modern world, the appearance of teeth is of great importance. Beautiful white teeth are not only a sign of good health, but also an element of culture and well-being. Definition, the individual natural color of the teeth is mainly determined by dentin, however, the color, transparency, thickness and degree of mineralization of the enamel affect the color of dentin. Any changes in dentin and enamel during the formation, development and after teething can cause changes in light guide properties, and accordingly, changes in color. Purpose. Analysis of the sources of scientific and medical information about the features of tooth whitening by various methods and their effectiveness with the presentation of their own clinical examples. Methods. An analytical review of the literature on various methods of teeth whitening and indications and contraindications to these methods. Results. There are different options for coloring teeth and they call as external and internal. A separate category of discolors is such as internalized staining, when external chromogens penetrate into the hard tissues of the tooth. Treatment of diseases and conditions accompanied by discoloration of the teeth is complex and may include therapeutic (occupational hygiene, whitening, restoration) and orthopedic measures. Teeth whitening is a chemical oxidation process in which the color of enamel and dentin changes from dark to light. Modern bleaching systems are based on the use of preparations of hydrogen peroxide or urea peroxide in combination with activating factors. Currently, the following main types of whitening systems are widely used in dental practice: for professional use in the clinic, for home use under the supervision of a doctor, for home use without a doctor's supervision, for example, whitening toothpastes, strips, pencils and standardized caps with gel inside. The work presents various options for teeth whitening from home to clinical and professional. Indications and contraindications to the teeth whitening procedure and its protocol are given. The analysis of the results of modern scientific research in dentistry shows that now it is possible to consider that professional whitening is effective. That is why the use of high-intensity blue diode light (LED) is quite promising, especially when eliminating deep tooth pigmentation. One of the most popular brands in the world of whitening is Philips Zoom system. The Philips Zoom WhiteSpeed lamp is a fundamentally new technology in which the manufacturer has used LEDs as a light source, which has made the teeth whitening process more comfortable and safe for the patient. Conclusion. To date, researchers and manufacturers have created various well-functioning, safe and comfortable systems for whitening teeth. It is important to find various aspects of tooth whitening and possible hazards. Understanding the operation of various systems and awareness of their advantages and disadvantages will allow you to choose the most optimal system. A professional approach to teeth whitening guarantee success.
Gonadal Dysgenesis with Ovarian Function: Clinical and Cytogenetic Findings in Six Patients

Abstract:In six patients with gonadal dysgenesis, ovarian function was evidenced by menses, which persisted in one patient for as long as seven years after the onset of menarche. Three patients had abnormal sex chromosome karyotypes in peripheral blood. Four subjects, with leukocyte cultures of 45/XO, 46/XX, 46/XX and 46/XX, respectively, had similar karyotypes in skin biopsy material. Those patients with abnormal sex chromosomal complements tended to be somewhat shorter in stature. No correlations could be seen between the sex idiogram and the duration of ovarian activity. The importance of considering gonadal dysgenesis as a differential diagnosis in all patients with secondary amenorrhea, especially when the menstrual life has been brief, is stressed.
A randomized clinical trial investigating three vital tooth bleaching protocols and associated efficacy, effectiveness and participants' satisfaction

Abstract:Objectives: To compare tooth colour change and participant's satisfaction following: home (HB), in-office (IOB) and combined (CB) bleaching treatments. Methods: A group of 105 participants received HB, IOB and CB treatments. HB was performed using custom-made trays and 10 % carbamide peroxide for 14 days. IOB was performed using 37.5 % hydrogen peroxide applied in 3 cycles. CB bleaching treatment involved IOB followed by HB. Tooth colour change was assessed visually (VC-ΔVC) and using a digital spectrophotometry device (ES-ΔeVS). Participant's perception of oral health, smile and straightness and whiteness of teeth were evaluated using self-reported questionnaire. Parameters/responses were evaluated/collected prior bleaching and at recalls. Linear mixed models were used to estimate between- and within-group differences. Results: CB resulted in significantly higher shade difference at 15 days recall (ΔVC and ΔeVS, all p ≤ 0.046). At 6 months recall, CB group demonstrated higher ΔeVS compared to IOB (p = 0.018) but the difference was not significant between the same groups when using VC (p = 0.051). Significant colour improvement was observed among all groups at 6 months recall (all within-group p < 0.001) except older participants (≥40 years) who received HB (ΔeVS: within-group p = 0.060). Overall, self-perception of oral health and satisfaction with smile and whiteness of teeth were significantly improved in all groups (all within-group p ≤ 0.001). Satisfaction levels with straightness of teeth were significantly improved for CB and IOB overall and in younger (<40 years) participants (all within-group p ≤ 0.013). Conclusion: CB treatment resulted in a pronounced colour improvement when compared to both techniques used individually. All bleaching protocols resulted in significant improvement of participants' perceived oral health and satisfaction with smile and whiteness of teeth. Clinical significance: Accelerated whitening can be achieved using a combination of in-office and home bleaching. Patients who may accept gradual whitening of teeth can be treated effectively using a cost-effective protocol and less concentrated bleaching agents.

Abstract:Introduction Tooth color is a key factor in the perception of an attractive smile, making bleaching a widely sought-after cosmetic dental procedure. Valued for its minimally invasive approach, this technique effectively lightens teeth to achieve desired aesthetic outcomes. As the desire for immediate results grows among patients, various bleaching agents, and energy sources have been developed to expedite the bleaching process. The study aims to evaluate and compare the efficacy of 35% hydrogen peroxide activated by light-emitting diode (LED) and laser on artificially stained teeth, assessing color changes at different time intervals. Methodology Sixty maxillary central incisors with intact crowns were selected for the study. An artificial staining solution was prepared, and the samples were immersed in it for 15 days to simulate staining. After this period, the teeth were removed from the solution, rinsed with water, and dried. Baseline photographs of each tooth were taken for comparison. The teeth were then divided into two groups, one treated with LED activation and the other with laser activation of 35% hydrogen peroxide as the bleaching agent. The color changes were measured and recorded at different time intervals. Data were presented as mean and standard deviation, with statistical analysis conducted using independent sample t-test and repeated measures ANOVA to compare the efficacy of the two methods. Results Both LED and laser activation of 35% hydrogen peroxide demonstrated significant color changes in the stained teeth. The initial color improvement was noticeable in both groups, with LED and laser treatments producing comparable whitening effects. The mean color change values indicated that both activation sources were effective in enhancing the bleaching process, providing immediate and substantial results. Statistical analysis showed no significant difference between the two methods, suggesting their equivalency in clinical efficacy for tooth bleaching. Conclusion The study concludes that both LED and laser activation of 35% hydrogen peroxide are efficient methods for in-office tooth bleaching, providing immediate and enhanced whitening effects. The comparable results between the two energy sources indicate that can be effectively used to meet the increasing demand for rapid aesthetic improvements in a clinical setting. This finding supports the adoption of both LED and laser bleaching techniques as viable options for dental practitioners aiming to achieve optimal patient satisfaction with minimal invasiveness.

Abstract:We report on a 42-year-old male patient who has been treated over a period of five years because of acute relapsing pancreatitis. The diagnoses of viral pancreatitis, pancreatic pseudocyst, and choledochal cyst type III (choledochocele) have been erroneously made. The clinical and laboratory findings over this period are presented, compared with one another, and analyzed. The correct diagnosis of a duodenal duplication cyst filled with multiple calculi and lying within the lumen of the duodenum could not be established until intraoperative findings and histological examinations were completed. The pathogenesis of relapsing acute pancreatitis associated with duodenal duplication cyst filled with multiple calculi is proposed, based on the analysis of these findings and the literature review.

Abstract:As a safe, effective, economical, and convenient technique, tooth whitening is one of the most popular treatments for improving tooth discoloration. This review summarizes the theoretical and recent research developments in the classification and mechanisms of tooth discoloration, as well as the principles, agents, effects, and side effects of tooth whitening techniques. The aim is to provide a basis for the clinical treatment of tooth whitening techniques and to suggest possible new ideas for further research. The accepted mechanism of whitening is the redox reaction of oxides in the whitening reagent, and the whitening effect is remarkable. However, side effects such as tooth sensitivity and irritation of gum and other oral soft tissues can still occur. It is recommended that more monitoring be carried out in the clinic to monitor these side effects, and care should be taken to protect the soft tissues in the mouth during office whitening procedures. Furthermore, there is a need to develop new additives or natural whitening products to reduce the occurrence of side effects.

Abstract:Smile designing primarily involves an enhancement towards appealing and elegant smile through appropriate dental restorations. Todayrsquos modern society gives a lot of precedence to a beautiful and healthy smile making it crucial for dental specialists to devise a comprehensive plan for fulfilling patientrsquos desires. Oral rehabilitation of patients in need of an appealing smile necessitates an integrative approach that involves a modification not only in the soft tissue architecture but also in the form and configuration of teeth. This clinical report describes a successful interdisciplinary approach for the treatment of an aesthetically and functionally compromised anterior dentition of a young female patient. Treatment plan included extraction of teeth with poor prognosis intentional root canal treatment and crown lengthening of maxillary and mandibular anterior teeth followed by conventional porcelain-fused-to metal fixed prosthesis. The final treatment outcome not only restored satisfactory function and aesthetics of the patient but also led to an improvement in the quality of life with a positive mind set towards her appearance.nbsp

Abstract:We study the effect of the noise distribution on the error probability of the detection test when a class of randomly rotated spherical fingerprints is used. The detection test is performed by a focused correlation detector, and the spherical codes studied here form a randomized orthogonal constellation. The colluders create a noise-free forgery by uniform averaging of their individual copies, and then add a noise sequence to form the actual forgery. We derive the noise distribution that maximizes the error probability of the detector under average and almost-sure distortion constraints. Moreover, we characterize the noise distribution that minimizes the decoder's error exponent under a large-deviations distortion constraint.

Abstract:Objective: To provide an update on tray bleaching for various tooth discoloration conditions, including a complete examination form as well as an information and consent form. Clinical considerations: Since the bleaching process was first documented in 1989, it has become a safe, successful, and conservative treatment for consistently whitening the color of patients' natural teeth. Though initially used on a limited basis, the process has expanded to include bleaching nicotine and tetracycline stains, single dark teeth, brown spots, reducing white spots, caries control as well as color change from aging. Ten percent carbamide peroxide is the material most used in research and has shown to be the most effective with the least amount of adverse side effects, including sensitivity or gingival irritation. Bleaching overnight using a smooth nonscalloped, nonreservoir vacuum-formed tray has been shown to be the method of choice for most clinicians, leading to greater patient compliance and an overall successful treatment. When possible, conservative bleaching treatment should be considered prior to more invasive, irreversible procedures such as veneers, or crowns to meet patients' esthetic requirements. Because of its basic pH, and potential for caries inhibition, complete restorative treatment does not have to be performed prior to initiating bleaching, making it an extremely flexible treatment. Conclusion: With a thorough bleaching analysis, proper treatment of appropriate discolorations over an ideal timeframe, tray bleaching is a powerfully predictable tool in restorative dentistry. Clinical significance: Tray bleaching with 10% carbamide peroxide should be the first consideration for treatment of discolorations of any type, with varying times of treatment, even in the presence of mild decay.

Abstract:Background: Teeth bleaching is a common practice performed by dental hygienists and dentists to correct aesthetic problems due to dental discoloration. Patients' opinions on this procedure may be different according to their personal experiences, expectations and individual sensitivities. The aim of this study is to assess the degree of satisfaction and related sensitivity of patients to professional and home bleaching treatment. Methods: A questionnaire consisting of 12 multiple-choice questions was submitted to 625 subjects among general population. These were related to: 1) patient's age and whether the patient is a smoker; 2) personal satisfaction; and 3) post-treatment tooth sensitivity. Descriptive analyses of all variables in the questionnaire were performed, and statistical analyses between variables were carried out by Chi-square test and Fisher's Exact test. Results: Of the people surveyed, 66% performed home bleaching, while the remaining performed the treatment in a dental office. Thirty-three percent of patients had previously undergone teeth bleaching and the 95.7% stated that they were satisfied with the treatment. Of these, only 36.4% reported experiencing postoperative hypersensitivity. Most patients (91.3%) received professional scaling before undergoing bleaching treatment. Conclusions: Patient satisfaction with tooth whitening treatment can vary from patient to patient and represents a strong indicator of treatment efficacy and suitability. A proper communication, careful management and individualized patient assessment are key to maximizing satisfaction and ensuring a positive and fulfilling patient experience.

Abstract:This study addresses the synchronization of N different hyperchaotic systems. Simple robust controllers with state variable error feedback and nonlinear compensating mechanisms are designed, such that the states of N different hyperchaotic systems are asymptotically synchronized. According to the Lyapunov stability theorem, the asymptotic stability of the solutions of the state error system is studied. The hyperchaotic Lorenz system, the hyperchaotic Liu system and the hyperchaotic Rössler system are taken as examples, numerical simulations and theoretical analysis show that the controller is effective and feasible.
