Abstract:Background: Teeth bleaching is a common practice performed by dental hygienists and dentists to correct aesthetic problems due to dental discoloration. Patients' opinions on this procedure may be different according to their personal experiences, expectations and individual sensitivities. The aim of this study is to assess the degree of satisfaction and related sensitivity of patients to professional and home bleaching treatment. Methods: A questionnaire consisting of 12 multiple-choice questions was submitted to 625 subjects among general population. These were related to: 1) patient's age and whether the patient is a smoker; 2) personal satisfaction; and 3) post-treatment tooth sensitivity. Descriptive analyses of all variables in the questionnaire were performed, and statistical analyses between variables were carried out by Chi-square test and Fisher's Exact test. Results: Of the people surveyed, 66% performed home bleaching, while the remaining performed the treatment in a dental office. Thirty-three percent of patients had previously undergone teeth bleaching and the 95.7% stated that they were satisfied with the treatment. Of these, only 36.4% reported experiencing postoperative hypersensitivity. Most patients (91.3%) received professional scaling before undergoing bleaching treatment. Conclusions: Patient satisfaction with tooth whitening treatment can vary from patient to patient and represents a strong indicator of treatment efficacy and suitability. A proper communication, careful management and individualized patient assessment are key to maximizing satisfaction and ensuring a positive and fulfilling patient experience.
The effect of a 10% carbamide peroxide home bleaching system on the gingival health

Abstract:Esthetics plays an important role in dentistry today. Because of an increased emphasis on beauty and health, cosmetic dentistry has been the thrust to the forefront of many practices. Many health products are used for bleaching teeth, but all side effects are not known. Tooth sensitivity and gingival irritation are the most common side effects, but they are typically mild and transient in nature. The aim of this investigation was to evaluate objectively the effect of 10% carbamide peroxide gel (Opalescence Utradent Inc, USA) on gingival health by measuring changes in Bleeding Index, Plaque Index, and Gingival Index. Eighteen subjects, 11 female and 7 male, age range 15-30 years (mean 24 years), were selected for the study as a convenient sample. Their teeth had either fluorosis, dental stains, smoking, or tetracycline staining. The Opalescence system was used as office monitored, at-home bleaching for three weeks. There was a statistically significant reduction in bleeding on probing (1% - 37%, p < or = 0.003), Plaque Index (4% - 50%, p < or = 0.000), and Gingival Index (2.5% - 34%, p < or = 0.002). Only two subjects reported tooth hypersensitivity, and none of the subjects complained about gingival irritation. There was a positive change in tooth color as far as staining was concerned. Further research including randomized controlled, double blind clinical trials is needed to confirm these findings and to examine other factors related to bleaching of teeth.

Abstract:A complete model of the universe needs at least three parts: (1) a complete set of physical variables and dynamical laws for them, (2) the correct solution of the dynamical laws, and (3) the connection with conscious experience. In quantum cosmology, item (1) is often called a `theory of everything,' and item (2) is the quantum state of the cosmos. Hartle and Hawking have made the `no-boundary' proposal, that the wavefunction of the universe is given by a path integral over all compact Euclidean 4-dimensional geometries and matter fields that have the 3-dimensional argument of the wavefunction on their one and only boundary. This proposal has had several partial successes, mainly when one takes the zero-loop approximation of summing over a small number of complex extrema of the action. However, it has also been severely challenged by an argument by Susskind.

Abstract:Resume. In the modern world, the appearance of teeth is of great importance. Beautiful white teeth are not only a sign of good health, but also an element of culture and well-being. Definition, the individual natural color of the teeth is mainly determined by dentin, however, the color, transparency, thickness and degree of mineralization of the enamel affect the color of dentin. Any changes in dentin and enamel during the formation, development and after teething can cause changes in light guide properties, and accordingly, changes in color. Purpose. Analysis of the sources of scientific and medical information about the features of tooth whitening by various methods and their effectiveness with the presentation of their own clinical examples. Methods. An analytical review of the literature on various methods of teeth whitening and indications and contraindications to these methods. Results. There are different options for coloring teeth and they call as external and internal. A separate category of discolors is such as internalized staining, when external chromogens penetrate into the hard tissues of the tooth. Treatment of diseases and conditions accompanied by discoloration of the teeth is complex and may include therapeutic (occupational hygiene, whitening, restoration) and orthopedic measures. Teeth whitening is a chemical oxidation process in which the color of enamel and dentin changes from dark to light. Modern bleaching systems are based on the use of preparations of hydrogen peroxide or urea peroxide in combination with activating factors. Currently, the following main types of whitening systems are widely used in dental practice: for professional use in the clinic, for home use under the supervision of a doctor, for home use without a doctor's supervision, for example, whitening toothpastes, strips, pencils and standardized caps with gel inside. The work presents various options for teeth whitening from home to clinical and professional. Indications and contraindications to the teeth whitening procedure and its protocol are given. The analysis of the results of modern scientific research in dentistry shows that now it is possible to consider that professional whitening is effective. That is why the use of high-intensity blue diode light (LED) is quite promising, especially when eliminating deep tooth pigmentation. One of the most popular brands in the world of whitening is Philips Zoom system. The Philips Zoom WhiteSpeed lamp is a fundamentally new technology in which the manufacturer has used LEDs as a light source, which has made the teeth whitening process more comfortable and safe for the patient. Conclusion. To date, researchers and manufacturers have created various well-functioning, safe and comfortable systems for whitening teeth. It is important to find various aspects of tooth whitening and possible hazards. Understanding the operation of various systems and awareness of their advantages and disadvantages will allow you to choose the most optimal system. A professional approach to teeth whitening guarantee success.
Effect of bleaching treatments on the adhesion of orthodontic brackets: a systematic review

Abstract:Background: Nowadays bleaching procedures have gained popularity in orthodontic patients. Peroxide and Carbamide acids are the common agents which are used in in-office and at home bleaching techniques. Consequently, the Bonding adhesion to the enamel can be influenced by the orthodontic phase and the residual peroxide might interfere with the polymerization and the adhesion of the brackets. Frequent debonding of the brackets from teeth after the bleaching procedure could cause the lengthening of the therapy and promote irregularities on enamel surface derived from an additional bonding phase of the brackets. The aim of this systematic review is to appraise the influence regarding the effect of the bleaching procedure on the bond strength of orthodontic brackets. Methods: An electronic database search was performed. Search terms included: bleaching, brackets, adhesion; data were extracted and summarized. Risk of bias was assessed using the Chocrane risk of bias tool, adapted for in vitro studies. Results: A total of 8689 articles were screened and 11 studies met the inclusion criteria of this systematic review. 1000 teeth of human and bovine origin were analyzed for the shear bond strength (SBS) of stainless and ceramic brackets after the bleaching treatments. All the authors divided the groups in different subgroups with different bleaching agents and in different concentration. The SBS value allowed to demonstrate the necessity to delay the bonding of the brackets for two weeks after a bleaching treatment and its improvement when tooth mousse or antioxidants agents are used. Conclusions: The SBS values and the delay of the bonding procedure must be considered in dental practice and clinical strategies are necessary in order to avoid drawbacks which could cause the debonding of the brackets after bleaching due to the alterations of the dental substrate, thus interfering with the orthodontic treatments.
Effect of Various Vital Bleaching Systems on Clinical Outcomes and Patient Satisfaction

Abstract:Objective To evaluate the clinical behavior of different bleaching products - the hydrogen peroxide (H2O2), carbamide peroxide (CP), and titanium dioxide bleaching systems. Methods Three bleaching systems with different concentrations (H2O2 15%, 38%, CP 15%, 35%, and titanium dioxide 20% H2O2) were used. Sixty participants with discolored teeth were enrolled and equally divided into six groups. Each group was rendered the assigned bleaching protocol against the control group, which received plain dentifrice polishing. Each case was evaluated immediately, two weeks, three months, six months, and one year after the bleaching treatment. The clinical evaluation was made for color change by (shade guide and digital images with L*a*b* parameters), tooth sensitivity, gingival irritation, and participants' satisfaction. Results All bleaching systems showed color improvement after bleaching regimens with significant effect showed by in-office titanium dioxide 20% H2O2 followed by 38% H2O2 with Δ=10.26 and Δ=6.52, respectively, when compared to other bleaching techniques. Higher sensitivity was recorded in group III with 50% of the participants reporting postoperative sensitivity. Higher gingival irritation was recorded in group IV (15% CP) where 60% of the patients reported gingival irritation. Thirty-five of the 60 participants (58.5%) recorded that the treatment whitened their teeth "moderately" and "a lot" while seven participants recorded "a slight" difference. There was a highly significant difference in participants' satisfaction between all bleaching groups (p < 0.05). Conclusion It is evident that there is a development of dental sensitivity and gingival irritation irrespective of the bleaching system used. The color assessment showed that the desired result can be achieved with variable levels of patient satisfaction with excellent overall results with the titanium dioxide system.
ATR-FTIR, EDS and SEM evaluations of enamel structure after treatment with hydrogen peroxide bleaching agents loaded with nano-hydroxyapatite particles

Abstract:Background Tooth whitening is one of the most requested dental treatments, but it still presents some side effects. Indeed, the bleaching agent can generate patients’ discomfort and dental hard tissue damages, not achieving an efficient and long-lasting treatment with optimum whitening effect. To overcome these limitations, the bleaching agents containing nano-hydroxyapatite can represent a reliable solution to avoid these detrimental effects. Methods In this study, human third molars were treated with commercial bleaching agents, containing nano-hydroxyapatite (nHA) and 6% (at-home treatment), 12% and 18% (in-office treatments) of hydrogen peroxide (HP), named respectively G1, G2 and G3. The results were evaluated descriptively and analytically using Attenuated Total Reflectance-Fourier Transform Infrared Spectroscopy (ATR-FTIR), Scanning Electron Microscopy (SEM) and Energy Dispersive X-ray spectroscopy (EDS), comparing the treated groups with a commercial gel containing 10% carbamide peroxide (CONV) and with a non-treated control group (CTRL). Results ATR-FTIR analysis revealed a similar composition in carbonates and phosphates for CTRL, G1 and G2 groups, which showed no significant differences in their spectral profiles; an increased amount of organic matter was found in G3, while CONV displayed an intermediate behavior. SEM analysis did not highlight significant changes in the enamel microstructure of G1 and CONV when compared to CTRL; the pattern observed in G2 presented a slight increase of enamel irregularities, while G3 displayed a partial removal of the aprismatic layer and microporosities. No evident effects due to nHA were observed in the structure of the hydroxyapatite component of G1, G2 and CONV, if compared to CTRL, while G3 showed a slight loss of crystallinity. In all groups, EDS identified slight changes in the concentration of chemical elements O and Ca, while the Ca/P ratio was similar when compared to CTRL. Conclusion The obtained results suggest that the application of the tested commercial bleaching agents, with a concentration of HP up to 12%, does not alter the morphological and chemical composition of the enamel surface and maintains its crystallinity.

Abstract:Background Although several studies indicate the harmful effects of bleaching on pulp tissue, the demand for this procedure using high concentrations of hydrogen peroxide (HP) is high. Objectives To investigate the influence of bleaching on the pulp tissue. Methods Electronic searches were conducted (PubMed/MEDLINE, Scopus, Cochrane Library and grey literature) until February 2021. Only in vivo studies that evaluated the effects of HP and/or carbamide peroxide (CP) bleaching gels on the inflammatory response in the pulp tissue compared with a non‐bleached group were included. Risk of bias was performed according to a modified Methodological Index for Non‐Randomized Studies scale for human studies and the Systematic Review Centre for Laboratory Animal Experimentation's RoB tool for animal studies. Meta‐analysis was unfeasible. Results Of the 1311 studies, 30 were eligible. Of these, 18 studies evaluated the inflammatory response in animal models. All these studies reported a moderate‐to‐strong inflammatory response in the superficial regions of pulp, characterized by cell disorganization and necrotic areas, particularly during the initial periods following exposure to 35%–38% HP, for 30–40 min. In the evaluation of human teeth across 11 studies, seven investigated inflammatory responses, with five observing significant inflammation in the pulp of bleached teeth. In terms of tertiary dentine deposition, 11 out of 12 studies noted its occurrence after bleaching with 35%–38% HP in long‐term assessments. Additionally, three studies reported significant levels of osteocalcin/osteopontin at 2 or 10 days post‐treatment. Other studies indicated an increase in pro‐inflammatory cytokines ranging from immediately up to 10 days after bleaching. Studies using humans' teeth had a low risk of bias, whereas animal studies had a high risk of bias. Discussion Despite the heterogeneity in bleaching protocols among studies, High‐concentrations of HP shows the potential to induce significant pulp damage. Conclusions High‐concentrations of bleaching gel increases inflammatory response and necrosis in the pulp tissue at short periods after bleaching, mainly in rat molars and in human incisors, in addition to greater hard tissue deposition over time. However, further well‐described histological studies with long‐term follow‐up are encouraged due to the methodological limitations of these studies. Registration PROSPERO (CRD42021230937).

Abstract:Introduction Tooth color is a key factor in the perception of an attractive smile, making bleaching a widely sought-after cosmetic dental procedure. Valued for its minimally invasive approach, this technique effectively lightens teeth to achieve desired aesthetic outcomes. As the desire for immediate results grows among patients, various bleaching agents, and energy sources have been developed to expedite the bleaching process. The study aims to evaluate and compare the efficacy of 35% hydrogen peroxide activated by light-emitting diode (LED) and laser on artificially stained teeth, assessing color changes at different time intervals. Methodology Sixty maxillary central incisors with intact crowns were selected for the study. An artificial staining solution was prepared, and the samples were immersed in it for 15 days to simulate staining. After this period, the teeth were removed from the solution, rinsed with water, and dried. Baseline photographs of each tooth were taken for comparison. The teeth were then divided into two groups, one treated with LED activation and the other with laser activation of 35% hydrogen peroxide as the bleaching agent. The color changes were measured and recorded at different time intervals. Data were presented as mean and standard deviation, with statistical analysis conducted using independent sample t-test and repeated measures ANOVA to compare the efficacy of the two methods. Results Both LED and laser activation of 35% hydrogen peroxide demonstrated significant color changes in the stained teeth. The initial color improvement was noticeable in both groups, with LED and laser treatments producing comparable whitening effects. The mean color change values indicated that both activation sources were effective in enhancing the bleaching process, providing immediate and substantial results. Statistical analysis showed no significant difference between the two methods, suggesting their equivalency in clinical efficacy for tooth bleaching. Conclusion The study concludes that both LED and laser activation of 35% hydrogen peroxide are efficient methods for in-office tooth bleaching, providing immediate and enhanced whitening effects. The comparable results between the two energy sources indicate that can be effectively used to meet the increasing demand for rapid aesthetic improvements in a clinical setting. This finding supports the adoption of both LED and laser bleaching techniques as viable options for dental practitioners aiming to achieve optimal patient satisfaction with minimal invasiveness.

Abstract:The effect of commonly used bleaching materials on the dental hard tissues was tested in extracted human premolars. In each tooth, the apical two-thirds of the root was removed, and the remaining tooth stump was cut longitudinally into two equal segments. The segments were cleaned, dried, and divided into six experimental groups. Each group was treated with one of the following bleaching materials: 30% hydrogen peroxide (HP), 10% carbamide peroxide (CP), sodium perborate (SP), Nu-Smile (NS), Opalescence (Op), and DentlBright (DB). Treatment consisted of immersing the specimens in the respective test material followed by incubation at 37 degrees C for 7 days. The levels of calcium, phosphorus, sulfur, and potassium were measured in the enamel, dentin, and cementum. In the enamel, a significant reduction in the calcium/phosphorus (Ca/P) ratio was found following treatment with HP. In the dentin, a significant reduction in Ca/P ratio was found following treatment with HP,CP, DB, and Op. In the cementum, a significant reduction in the Ca/P ratio was found following treatment with HP, CP, NS, and Op. Changes in sulfur and potassium levels also occurred, but were usually not statistically significant. Significant reduction in sulfur levels occurred only in the cementum following treatment with CP and SP. Sulfur levels increased significantly following treatment with NS. Significant reduction in potassium levels occurred only in the dentin following treatment with CP. It is concluded that bleaching materials may adversely affect the dental hard tissues and should be used with caution.

Abstract:As a safe, effective, economical, and convenient technique, tooth whitening is one of the most popular treatments for improving tooth discoloration. This review summarizes the theoretical and recent research developments in the classification and mechanisms of tooth discoloration, as well as the principles, agents, effects, and side effects of tooth whitening techniques. The aim is to provide a basis for the clinical treatment of tooth whitening techniques and to suggest possible new ideas for further research. The accepted mechanism of whitening is the redox reaction of oxides in the whitening reagent, and the whitening effect is remarkable. However, side effects such as tooth sensitivity and irritation of gum and other oral soft tissues can still occur. It is recommended that more monitoring be carried out in the clinic to monitor these side effects, and care should be taken to protect the soft tissues in the mouth during office whitening procedures. Furthermore, there is a need to develop new additives or natural whitening products to reduce the occurrence of side effects.
Edge influences on suprathreshold white brightness perception.

Abstract:Edge plays special role in spatial perception and in determining the lightness information of the surface within borders. The aim of our study was to measure suprathreshold brightness in different levels of edges hardness. Steven's Power Law for circles modulating in luminance were estimated for 10 subjects (mean age 25 SD 3, 5 female). Stimuli and psychophysical procedures were built using Psyknematix software (v1.4.3, KiberVision, Montreal, Canada) installed in iMAC OS 10.8 X (Apple Inc., USA). Stimuli were presented on the iMAC display using 11 bit graphic board and consisted of two circles of 3 degrees of visual angle, separated by 10 degrees. We tested 7 levels of Michelson contrast: 7, 8, 10, 15, 26, 50, 100. Three edge filtering was tested ( 0.3, 0.8 and 1.5 degrees of smoothing). The subject task were to judge the brightness of the edge filtered circle compared with the circle of hard edge which was considered the Modulus and received an arbitrary level of 50, representing the amount of brightness perception. If the bright was perceived as two times more intense, the subject had to give a number 100. . If the bright was perceived as two times less intense, the subject had to give a number 25 and so on. In each trial, the same contrast level was presented in both circles. Five judgments were performed for each contrast level in each edge filtering. We found an increase in the Power Law exponent as the increase of filtering (for sigma of 0.3 = 0.37, sigma of 0.8 = 0.55, and sigma 1.5 = 1.03). All power function fitting had a correlation coefficient higher than 0.95. We conclude that there is a progressively increase in brightness perception as increase the edge filtering and a full performance in suprathreshold contrast perception is achieved with a half size stimulus smoothing. Abstract:ObjectivesThis paper aims to assess the whitening effectiveness and toxicity of tooth-bleaching protocols applied to enamel/dentin disks simulating mandibular incisors (ICs) and premolars (PMs).Materials and methodsA 10% hydrogen peroxide (H2O2) gel was applied for 3 × 15, 1 × 15, or 1 × 5 min to enamel/dentin disks simulating mandibular ICs and PMs, and the trans-enamel and trans-dentinal diffusion products were applied to human dental pulp cells (1 h). Professional therapy (35% H2O2—3 × 15 min) was used as positive control, and non-bleached samples were used as negative control. Cell viability and morphology, oxidative stress generation, and odontoblastic marker expression were assessed. The H2O2 diffusion and enamel color change (ΔE) were also analyzed.ResultsThe 10% H2O2 gel induced significant cell viability reduction only when applied 3 × 15 min, with the intensity of oxidative stress and down-regulation of odontoblastic markers being higher in the IC group. The other experimental bleaching protocols caused slight alterations regarding the cell parameters evaluated, with intensity being related to enamel/dentin thickness. These effects were also correlated with higher H2O2 diffusion in the IC group. ΔE values similar as positive control were found for the 10% 3 × 15 and 1 × 15 protocols on IC group, after 4 and 6 sessions.ConclusionApplication of a 10% H2O2 bleaching gel for 15 or 45 min to thin dental substrate significantly minimizes cell toxicity in comparison with highly concentrated gels associated with similar esthetic outcomes by increasing the number of bleaching sessions.Clinical relevanceBleaching gels with 10% H2O2 applied in small teeth for short periods may be an interesting alternative to obtain whitening effectiveness without causing toxicity to pulp cells, which may be able to reduce the tooth hypersensitivity claimed by patients.

Abstract:Objectives: The objective of this study was to investigate the efficacy of tooth whitening and the effects on satisfaction with whitening treatment and esthetic self-perception of over-the-counter tooth whitening products. Materials and methods: Fifty-six participants were randomly allocated to three groups based on the whitening product they used (toothpaste, pencil, or a combination of both). In this prospective study, alterations in tooth color were assessed using a spectrophotometer at three distinct time intervals (initial measurement, seven days, and 14 days after commencing the use of the whitening product). Simultaneously, the respondents were given the PIDAQ questionnaire to complete. Patient satisfaction with treatment characteristics (5-point Likert scale), and perception of side effects were assessed at the end of the study. Results: Participants who used the whitening pen either alone or in combination with whitening toothpaste showed significant improvements in tooth color and whiteness index at seven and 14 days compared to those who used whitening toothpaste alone (p≤0.001). No significant differences were found between products in overall satisfaction with treatment, perception of final tooth color, and treatment comfort. Furthermore, with the exception of the psychological influence factor after 14 days (p≤0.001), there were no significant differences in the PIDAQ questionnaire scores between the whitening products at the different time points. However, many respondents (16.6%) reported oral mucosal sensitivity and a higher percentage (27.8%) reported tooth sensitivity during whitening pen application. Conclusion: The combination of whitening toothpaste and whitening pen, as well as the whitening pen alone, showed effective objective color change results, but home whitening procedures did not significantly affect participants' self-perceived satisfaction or psychosocial outcomes depending on the product used.
Blinded, parallel and randomized clinical evaluation of in-office dental bleaching with violet LED (405-410nm)

Abstract:This clinical study evaluated the effect of bleaching performed with violet LED light (405-410 nm), either combined with hydrogen peroxide (HP) gel, or not, on color change, dental sensitivity, participants' satisfaction and impact on their quality of life. A hundred participants were divided into one of the groups (n = 25): G1 - 35% HP (4 sessions, 1x/week); G2 - violet LED (4 sessions, 1x/week); G3 - violet LED (4 sessions, 2x/week); G4 - hybrid technique (violet LED + 35% HP; 4 sessions, 1x/week). Color evaluation was performed with colorimetric tests (objective and subjective), before, 14 days and 3 months after completion. Additionally, satisfaction with treatment, impact on quality of life (OHIP-14) and dental sensitivity were recorded. The data were submitted to statistical analysis, considering a significance level of 5%, with the exception of the data from the questionnaire on satisfaction (descriptive analysis). Two-way ANOVA and Tukey tests showed that there was no difference between color variation resulting from techniques used in G1 and G4; those used in G2 and G3 did not differ and were less effective for bleaching than those of the other groups, in both subjective and objective evaluations. Regarding tooth sensitivity, subjects in G2 and G3 experienced no sensitivity, while those in G4 showed lower sensitivity values than those in G1. With respect to quality of life, only subjects in G1 and G2 showed a significant positive impact. Among the evaluated techniques, the hybrid type seemed to be a good alternative, showing effective bleaching with less tooth sensitivity.
Biological action of bleaching agents on tooth structure: A review

Abstract:The use of bleaching agents to remove stains is one of the main dental procedures to improve the aesthetics of teeth. This review presents the main agents used for tooth whitening, existing clinical protocols, and the structural changes that may occur through their use. The main bleaching agents consist of hydrogen peroxide and carbamide peroxide, which are used in bleaching techniques for vital teeth. These techniques can be performed in the office by a professional or by the individual in a home en-vironment under professional guidance. Bleaching agents come in a variety of concentrations and there are over-the-counter products available on the market with lower concentrations of hydrogen peroxide. Due to the chemical characteristics of the agents, changes in the organic and inorganic content of the tooth structure can be observed. These changes are related to morphological changes characterized by in-creased permeability and surface roughness, such changes compromise the mechanical resistance of the tooth. Furthermore, bleaching agents can promote molecular changes after reaching the dental pulp, resulting in oxidative stress of pulp cells and the release of pro-inflammatory mediators. Despite the bleaching effectiveness, tooth sensitivity is considered the main side effect of use. Therefore, among the heterogeneity of protocols, those that used the bleaching agent for a prolonged time and in lower con-centrations presented more harmful effects on the tooth structure.
Comparison of in-office and at-home bleaching techniques: An umbrella review of efficacy and post-operative sensitivity

Abstract:Objectives: The objective of this umbrella review is to evaluate the efficacy and adverse effects of different teeth whitening techniques in-office (IO) and at-home (AH), regarding chromatic changes and teeth sensitivity. Materials and methods: The search was carried out from several databases. The included studies were all systematic reviews with or without meta-analysis of RCT or quasi-RCT. The participants were patients that underwent external dental bleaching in permanent vital teeth. The interventions were in-office (IO) bleaching techniques and at-home (AT) bleaching techniques with different bleaching agents and concentrations. Results: The search resulted in a total of 257 articles, and 28 SR were included in the qualitative analysis and nine in the quantitative analysis. There is no difference between in-office and at-home techniques in terms of color change (p = 0.95) and post-treatment sensitivity (p = 0.85). There is similarity risk and intensity of teeth sensitivity between AH and IO bleaching. IO bleaching with light-activated systems with low concentrations of bleaching agent showed similar results to IO bleaching techniques with high concentrated bleaching gels. With the application of the criteria of the AMSTAR 2 tool, the reviews were considered critically low to high. Conclusions: There are no significant differences in terms of color change between the different bleaching techniques compared. Teeth sensitivity is always present regardless of the technique used. The use of light activation systems did not increase the intensity and risk of post-operative sensitivity.

Abstract:Objective To evaluate the influence of air‐abrasion of enamel with three different desensitizing powders on the whitening effect of a bleaching gel containing 40% H2O2, which was used for in‐office tooth bleaching. Materials and Methods Forty human incisors, extracted and prepared, were acquired for this study and subsequently randomized into four groups (n = 10). The control group specimens underwent no pretreatment prior to the bleaching procedure, whereas the remaining three groups underwent air abrasion using distinct desensitizing powders; (a) Sylc, which contains bioglass 45S5; (b) BioMinF, which contains calcium phospho‐fluoro‐silicate glass; and (c) MI Pearls, which contains nano‐hydroxyapatite, 1 h preceding the Opalescence Boost PF 40% bleaching procedure. Color measurements were conducted using a double‐beam UV–Vis spectrophotometer at four distinct time points (prior to bleaching, 24 h, 15 days, and 30 days post‐bleaching). Results Tooth color change outcomes revealed that there were no statistically significant results with respect to the interaction of the two criteria (treatments and time) (p = 0.990). Additionally, there were no statistically significant results with respect to the main effects of treatments (p = 0.385), while there were statistically significant effects with respect to the time criterion (p = 0.013). Conclusions The use of the tested desensitizing powders prior the bleaching procedure did not affect the tooth color change induced by the tested bleaching agent. Clinical Significance Tooth color change and whiteness are not affected by air‐abrasion desensitizing treatments when applied prior to in‐office bleaching procedures.
