Abstract:Soft materials that aim to reproduce the tribological function of the natural joint are gaining popularity as an alternative concept to conventional hard bearing materials in the hip and knee. Polyurethane (PU) elastomers, in particularly polycarbonate urethane, are among the highest performing medical-grade polymers. They have mechanical and biological properties that make them suitable for use in orthopedic implants, as they demonstrate a unique combination of toughness, durability, flexibility, biocompatibility, and biostability. As presented in this paper, newly developed implants based on polycarbonate urethane perform more similarly to the natural joint in their mechanical response to load, and in their ability to utilize a thinner structure similar to that of cartilage, without jeopardizing the integrity or stability of the implant. Several wear studies of implants based on PU demonstrate a very low damage level to the implants’ articulating surfaces following repeated loading, and provide good assurance that this material can generate a low and stable wear rate in the long term. Animal studies further provide understanding of the biological response to PU implants in the hip and knee. Short-term clinical results are now becoming available from several commercial products. These generally show good functioning of these implants in the body and no material-related complications.
Improving the Mechanical Properties and Tribological Behavior of Sulfobetaine Polyurethane Based on Hydrophobic Chains to Be Applied as Artificial Meniscus

Abstract:In the context of meniscus reconstruction in knee joints, current bulk biomaterials fail to meet the clinical demands for both excellent mechanical strength and low coefficient of friction. In this research, zwitterionic polyurethanes (PUs) with varying sulfobetaine (SB) groups were synthesized as the potential materials for artificial meniscus to investigate the relationship between the structures of SB groups and the performances of PUs. Under the saturation condition of 3 mg/mL hyaluronic acid aqueous solution, PU with long-alkyl chains and SB groups (PU-hSB4) exhibited a good tensile modulus (111.5 MPa) because the hydrophobic interaction of carbon chains was able to maintain the ordered aggregations of hard segment domains. Interestingly, hydrophobic chains in the molecular chain could also improve the tribological performance of PU-hSB4 instead of resulting from the surface roughness of samples, the components of lubricants, and the counterface of samples. A thicker and relatively stable hydration layer of noncrystal water was formed on the surface of PU-hSB4, which exhibited superior resistance to external forces compared to other PUs. Even if the hydration layer was damaged, PU-hSB4 was able to resist the compression of cartilage due to its high surface modulus, thus maintaining a similar and stable coefficient of friction (0.15-0.16) to native meniscus (0.18) and excellent wear resistance. In addition, the low cytotoxicity of PU-hSB4 further demonstrated its great potential to be applied in artificial meniscus instead.

Abstract:BackgroundConventional total knee replacement designs show high success rates but in the long term, the stiff metal components may affect bone quality of the distal femur. In this study we introduce an all-polymer total knee replacement device containing a PEEK femoral component on an UHMWPE tibial implant and study its mechanical integrity, fixation, and stress shielding of the periprosthetic femur.MethodsThe implant was analysed in finite element simulations of level gait, adopted from the ISO 14243-1 standard. Mechanical integrity of the implant and underlying cement mantle were tested, and the fixation strength of the cement-implant interface was studied. Stress shielding was assessed based on strain energy density distributions in the distal femur. We compared PEEK and CoCr implants for mechanical performance and fixation, and compared both versions against an intact case to determine the change in bone strain energy density.ResultsThe mechanical integrity of the PEEK and CoCr components was similar in magnitude, but differences in stress patterns were found. Moreover, the cement mantle was loaded more heavily in the CoCr configuration. Under similar interface properties, the CoCr-cement interface was more at risk of failure than the PEEK-cement interface. The bone strain energy density distribution of the PEEK implant was similar to the intact case, while the CoCr implant showed signs of stress shielding, and a different distribution than the intact and PEEK models.ConclusionsDuring gait, the PEEK femoral component performed similarly to CoCr, with no added risk for the cement mantle. The reduction in stress shielding for PEEK was evident and confirms the potential reduction in long-term loss of bone stock for this all-polymer knee implant.
Friction and wear mechanisms in hip prosthesis: Comparison of joint materials behaviour in several lubricants

Abstract:In this work, the most commonly used joint materials for substitution of hip joints, ultra high molecular weight polyethylene (UHMWPE) for the cup, and alumina, stainless steel or CoCrMo alloy for the head, were submitted to pin-on-disk tribological tests. The interfacial behaviour prosthetic material/lubricant was investigated through wettability measurements. Four lubricants were used: Hanks’ balanced salt solution (HBSS) and solutions of bovine serum albumin (BSA), of hyaluronic acid (HA) and of both components (BSA+HA) in HBSS. It was observed that the friction coefficient increased significantly with time, in a random way, when the lubricant did not contain albumin. The addition of HA induced a slight reduction in the initial value of the friction coefficient but did not affect qualitatively its behaviour for longer times. Observation of the worn metallic surfaces revealed parallel grooves typical of abrasive wear and also lumpy transfer film of UHMWPE, while the transfer of polymer to alumina was incipient. Wear was particularly intense on the steel surface where delamination and fatigue wear mechanisms were also found. In the presence of albumin, the friction coefficient remained constant and small for the metallic surfaces and increased in the case of alumina. The observation of the worn surfaces showed that the presence of albumin in the lubricant avoided the transfer of polymer for the metallic surfaces but not for the alumina surfaces. These results, together with the wettability measurements, strongly indicate that the presence of albumin in the lubricant avoids the adhesion and transfer of UHMWPE only for the least hydrophilic surfaces, which are the metallic ones.

Abstract:Hydration lubrication by the hydrated surface-grafted macromolecules covering the joint cartilage is crucial for the proper functioning of natural synovial joints. However, when considering artificial replacements, similar surface-grafted polymers on artificial materials are usually insufficient for the long-term lubrication due to lack of stability and durability. In this work, an overall material modification through embedding polyelectrolyte brushes was performed for prolonged lifetime. Method-wise, hydrophilic monomer 3-sulfopropyl methacrylate potassium salt (SPMK) was grafted onto ultrahigh molecular weight polyethylene (UHMWPE) powders through photoinduced radical graft polymerization, followed by hot-press forming of the modified powders to afford homogeneous modified material. Trials revealed significantly increased surface wettability and negative charge density, giving rise to strong hydration lubrication that led to a stable and low friction coefficient of 0.009, and an ultralow wear rate of 9.1 x 10(-8) mm(3) (N m)(-1) under physiological conditions, which could be consistently maintained even after the surface was sheared off with polishing equipment. This overall material modification on UHMWPE through polyelectrolyte embedment, applied to obtain long-lasting, highly lubricated materials, therefore presents a promising method for the preparation of artificial joints.
Comparison of wear on articular cartilage by titanium alloy, ultra-high-molecular-weight polyethylene, and carbon fibre reinforced polyether-ether-ketone: A pilot study

Abstract:Artificial implant materials may articulate against native articular cartilage in certain clinical scenarios and the selection of an implant material that results in the least wear on articular cartilage is preferred to maintain normal joint architecture and function. This project compared the wear on porcine femoral condyles induced by articulation against porcine patellae, titanium alloy (Ti6Al4V), ultra high molecular weight polyethylene (UHMWPE), and carbon fibre reinforced polyether-ether-ketone (CFR-PEEK) through an ex vivo experimental setup. A sinusoidal compressive load of 30-160 N, representing an approximate joint pressure of 0.19-1 MPa at a frequency of 3 Hz coupled with a rotational displacement of +/- 10⁰ at 3 Hz was used to simulate physiological joint motion. Wear was characterized via gross examination and histologically using the OARSI scoring system after 43,200 cycles. CFR-PEEK resulted in the most significant wear on articular cartilage compared to titanium alloy and UHMWPE whereas titanium alloy and UHMWPE resulted in similar levels of wear. All materials caused more wear compared to cartilage-on-cartilage testing. The wear mechanism was characterized by progressive loss of proteoglycan content in cartilage in histology samples.

Abstract:A 12-channel wear screening device was used to compare the wear properties of a variety of prosthetic joint materials. Two types of tests were run: (1) Ultrahigh molecular weight (UHMW) polyethylene bearing against metal or ceramic counterfaces and (2) various polymers bearing against 316 stainless steel as a standard counterface. Wear was quantified by weighing the polymer specimens, with presoaking and control-soak specimens used to minimize the error due to fluid absorption. The specimens were lubricated with bovine blood serum. Friction and polyethylene wear was very low with each of the metals (316 stainless steel, cobalt-chrome alloy, multiphase alloy, and titanium 6-4 alloy) such that the differences in wear rate would not be significant in terms of choosing a material for clinical application. However, titanium 6-4 alloy was found to be especially susceptible to abrasive wear by particles of acrylic cement. Nitrided titanium 6-4 counterfaces were impervious to acrylic abrasion. Polyethylene wear against highly polished, fully dense ceramics (Sialon, Alumina, Macor, and pyrolytic graphite) was as low as that with the metal counterfaces. Wear increased slightly with increasing ceramic surface roughness. The coefficient of friction of polyethylene against pyrolytic graphite was two to three times higher than with the metals or other ceramics. All of the alternate polymers underwent more wear than UHMW polyethylene. Teflon and polyester, two polymers that have proven unsuccessful in prior clinical use, had wear rates 1,600 and 830 times greater than polyethylene, respectively, an indication that the laboratory wear test provided a quantitative prediction of the behavior of the materials in vivo. However, it was difficult to assess the clinical significance of the less extreme wear rates since the ability of the tissues encapsulating a prosthesis to accomodate wear debris is not known on a quantitative basis.

Abstract:An efiective series RLC model for the electromagnetic response of weakly absorbing dielectric sphere near the flrst magnetic dipole resonance was developed, and the efiective magnetic properties of Mie resonance-based dielectric metamaterials were obtained in terms of this model. In comparison with traditional efiective medium theory such as extended Maxwell-Garnett (EMG) theory based on Mie model, this approach is more intuitive and can give an analytical dependence of the magnetic properties of the composite on the electromagnetic and geometric parameters of the constituting dielectric particles.
Prior invasive fungal infection is not a contraindication for subsequent allogeneic bone marrow transplantation in adult patients with hematologic malignancies.

Abstract:Background: Hemiarthroplasty has clear advantages over alternative procedures and is used in 20% of all shoulder joint replacements. Because of cartilage wear, the clinical outcome of hemiarthroplasty is unreliable and controversial. This paper suggests that the optimal choice of prosthetic material may reduce cartilage degeneration and improve the reliability of the procedure. The specific objectives were to assess 3 materials and assess how the severity of arthritis might affect the choice of prosthetic material. Methods: A CoCr alloy, an AL2O3 ceramic, and a polycarbonate urethane polymer (PCU) were mechanically tested against 5 levels of human osteoarthritic cartilage (from intact to severely arthritic, n = 45). A high friction coefficient, a decrease in Young's modulus, an increase in permeability, a decrease in relaxation time, an increase in surface roughness, and a disrupted appearance of the cartilage after testing were used as measures of cartilage damage. The biomaterial that caused minimal cartilage damage was defined as superior. Results: The CoCr caused the most damage. This was followed by the AL2O3 ceramic, whereas the PCU caused the least amount of damage. Although the degree of arthritis had an effect on the results, it did not change the trend that CoCr performed worst and PCU the best. Discussion and conclusion: This study indicates that ceramic implants may be a better choice than metals, and the articulating surface should be as smooth as possible. Although our results indicate that the degree of arthritis should not affect the choice of prosthetic material, this suggestion needs to be further investigated.

Abstract:Tribological behaviors of four polymers (conventional and cross-linked UHMWPE, conventional and glass fiber-reinforced PEEK) articulated with Ti6Al4V ball were studied under both dry sliding and 25% bovine serum lubrication. The hardness, static contact angle, surface damage topography, and wear parameter of wear scar were tested. Both cross-linked process of UHMWPE and glass fiber-reinforced treatment of PEEK improved wettability while they did not increase hardness. PEEK revealed higher surface hardness and better wettability than UHMWPE. The dominant wear mechanisms for UHMWPE were plastic deformation and fatigue wear while the failure mechanisms were severe adhesive and abrasive wear for PEEK. Cross-linked process of UHMWPE could form multi-molecular arrangement and reduce stratification, also decreasing friction coefficient and wear rate in both dry sliding and lubrication conditions. However, glass fiber-reinforced treatment of PEEK only decreased its friction coefficient and wear rate in dry condition, which was closely related to the function and wear mechanism of glass fiber. Cross-linked UHMWPE revealed the lowest friction coefficient and wear rate under lubrication condition, which was attributed to the cross-linked treatment and the formation of both protein adsorption film and lubrication film. Hence, cross-linked UHMWPE may be an alternative polymer for use as artificial cervical disc bearing material when it articulated with Ti6Al4V.

Abstract:The effect of polyvinylpyrrolidone (PVP) on tribological properties of joint prostheses is still unclear despite its good lubricity and biocompatibility. PVP solutions with five concentrations were prepared with two different solvents. The viscosity, surface tension, and static contact angle of the solutions were measured as well as the friction coefficient and wear volume of artificial joint pairs. With PVP concentration increasing, the friction coefficient firstly decreased significantly but subsequently tapered off after a high level. The inclusion of PVP also improved the wear resistance of the pair. It implies that PVP could be a promising lubrication additive for artificial joints.
Torsion Wear Behavior of PEEK-on-UHMWPE as an All-Polymer Joint Combination in Total Knee Replacement

Abstract:Polyether ether ketone (PEEK) has been considered as an alternative material to cobalt chrome in the femoral component of a total knee replacement. In this article, the experimental simulation of torsion wear for a PEEK and CoCr femoral head against an ultra-high-molecular-weight polyethylene (UHMWPE) tibial pad was carried out using a two-station knee simulator. The testing results showed that T-θ curves of torsion wear for PEEK-on-UHMWPE bearing couples showed three types of morphological characteristics, such as linear, elliptical, and parallelogram shapes. With an increase in torsion angular displacement, T-θ curves changed from the linear to the parallelogram type, and the relative motion state changed from the partial slip regime coordinated by the elastic deformation to the slip regime with plastic deformation. With an increase in normal load, the T-θ curve changed from the parallelogram to the elliptical type, and the relative motion state changed from the slip regime to the mixed slip regime. Under intermediate torsion wear conditions, the wear factors of UHMWPE were 11.25 ± 2.08 × 10−6 mm3/C and 14.65 ± 2.49 × 10−6 mm3/C against PEEK and CoCr. Under high torsion wear conditions, the wear factors were 20.51 ± 3.23 × 10−6 mm3/C and 23.26 ± 3.01 × 10−6 mm3/C, respectively. The wear surface of UHMWPE against PEEK showed slight adhesive wear and plastic deformation in the center, and abrasive wear with local fatigue spalling was found at the edges. The wear topographies presented characteristics similar to the CoCr bearing combination.

Abstract:<p>At present, the main frictional components of artificial hip joint (AHJ) are mainly the combinations of hard materials (metal, ceramic) and soft materials (polymer), such as, metal‐on‐metal (MoM), ceramic‐on‐ceramic (CoC), metal‐on‐polymer (MoP), and ceramic‐on‐polymer (CoP). As a new type of AHJ, the polymer‐on‐polymer (PoP) combination has gradually begun to be explored and studied. In this paper, PoP AHJ of CF/PEEK composite are studied, and elastohydrodynamic lubrication analysis based on the solution of the Reynolds equation is performed, and the results are compared with the MoM and MoP AHJs. The comparison shows that under the same steady‐state condition, the PoP AHJ has better lubrication performance. The influence of the key structural parameters of the CF/PEEK PoP AHJ is also analyzed in this paper, and the results show that the femoral head size and radial clearance have great influence on the thickness and pressure distribution of the lubricating film. The research results in this paper further confirm the application potential of the PoP AHJ, and are helpful to its practical application and structural design.</p>

Abstract:The tribological behavior between orthopedic implants and cortical bone is important but usually neglected. Poly(ether-ether-ketone) (PEEK) is a promising material for orthopedic applications. To further understand and improve the interfacial tribological properties between PEEK implant and host bone tissue, a PEEK-cortical bone tribo-pair is designed and fabricated. The frictional and wear performance of such tribo-pair is investigated under different lubricants, i.e., simulated body fluid (SBF), calf serum (CS), hyaluronic acid (HA), and mucin (MUC). The results suggest that MUC solution can be utilized as an artificial natural synovial fluid to improve the tribological properties of PEEK-based implants.
Influence of Cross-Shear and Contact Pressure on Wear Mechanisms of PEEK and CFR-PEEK in Total Hip Joint Replacements

Abstract:With the increasing market demand for artificial hip joints, total hip joint replacement has gradually become an effective means of treating a series of hip joint diseases. In order to improve the service life of artificial hip joints, some new artificial hip joint materials, including polyetheretherketone (PEEK) and carbon fiber reinforced polyetheretherketone (CFR-PEEK), have been developed. In this paper, pin-on-plate wear tests under different cross-shear ratios and contact pressures were carried out to study the wear mechanism and worn surface topography of PEEK and CFR-PEEK. The experimental results showed that the wear of PEEK was associated with cross-shear, while CFR-PEEK was not. When the cross-shear ratio was 0.039 and contact pressure was 3.18 MPa, PEEK had poor wear resistance and its wear factor was about eight times that of ultra-high molecular weight polyethylene (UHMWPE). The wear resistance of CFR-PEEK had a significant advantage, since its wear factor was about 30% of that of PEEK. The wear factors of PEEK and CFR-PEEK increased as the contact pressure increased. The arithmetic average of the height amplitude of the surface, Sa, also increased gradually according to the topography of the worn surface. The wear mechanisms of PEEK and CFR-PEEK were scratching, plough cutting, and abrasion Since CFR-PEEK had good wear resistance and insensitivity to cross-shear motion, it is suitable for making artificial hip joints under low contact pressure condition.
Simvastatin inhibits apoptosis of endothelial cells induced by sepsis through upregulating the expression of Bcl-2 and downregulating Bax.

Abstract:BACKGROUND Many studies have showed that apoptosis of endothelial cells plays a curial role in the progress of sepsis. But the role of simvastatin in apoptosis of endothelial cells induced by sepsis is not clear. The present study aimed to investigate the role of simvastatin in apoptosis of endothelial cells induced by sepsis and its mechanism. METHODS Human umbilical vein endothelial cells (HUVECs) were randomly divided into three groups: control group, sepsis serum intervention group (sepsis group) and simvastatin+sepsis serum intervention group (simvastatin group). After 24-hour incubation with corresponding culture medium, the relative growth rate of HUVECS in different groups was detected by MTT assay; the apoptosis of HUVECs was detected by Hoechst33258 assay and flow cytometry; and the expression of the Bcl-2 and Bax genes of HUVECs was detected by PCR. RESULTS Compared with the sepsis group, HUVECs in the simvastatin group had a higher relative growth rate. Apoptotic HUVECs decreased significantly in the simvastatin group in comparison with the sepsis group. Expression of the Bcl-2 gene in HUVECs decreased obviously, but the expression of the Bax gene increased obviously after 24-hour incubation with sepsis serum; however, the expression of the Bcl-2 and Bax genes was just the opposite in the simvastatin group. CONCLUSIONS Our study suggests that simvastatin can inhibit apoptosis of endothelial cells induced by sepsis through upregulating the expression of Bcl-2 and downregulating Bax. It may be one of the mechanisms for simvastatin to treat sepsis.

Torsional friction behavior of the contact interface between the materials of an artificial knee joint replacement

Abstract:The torsional friction behavior of the materials (CoCrMo alloy and ultra-high-molecular-weight polyethylene (UHMWPE)) that are used in artificial knee joint replacement (AKJR) were investigated under dynamic loading. The torsional friction torque, wear loss and scars and stress distribution were analyzed and compared. The results show the following: (1) the friction torque declines rapidly at the beginning of the experiments, and meanwhile, the maximum normal load and the torsional angle amplitude show very little effect on the steady value; (2) the wear loss decreases with the increased torsional angle amplitude but increases with the increased maximum normal load; additionally, as the cycle times increase, the wear rate decreases, and the wear is minimal; (3) unloading and secondary loading significantly reduce the friction and wear of the materials, and thus, patients should rest after prolonged exercise to reduce wear; (4) the margins of UHMWPE and CoCrMo are worn, and the wear mechanism is abrasive wear; (5) around the center of the UHMWPE, the surface is peeled off, and the wear mechanism is fatigue wear; and (6) in terms of the compressive stress and shear stress, the calculated results from a finite element model match the experimental results well.
