Abstract:The rising prevalence of community-acquired, methicillin-resistant Staphylococcus aureus (MRSA) has correlated with an escalating number of complex, subcutaneous abscesses in pediatric patients. The purpose of this study was to present a novel technique and early outcome results for the minimally invasive drainage of complex, subcutaneous abscesses. Methods: Patients' outcomes from the treatment of complex, subcutaneous abscesses were retrospectively reviewed under institutional review board approval from July 2006 to August 2007 at 2 independent, tertiary care pediatric hospitals. Data on patients' demographics, length of hospital stay, and length of treatment were collected, along with analysis of the isolated organisms. The operative technique uses drainage of the abscess through peripheral stab incisions. Cavity debridement and irrigation is followed by placement of a vessel-loop drain through the drainage incisions. Topical wound care without packing is performed twice a day. Drain removal follows resolution of cellulitis and drainage. Results: One hundred twenty-eight patients were treated over a 14-month period. The ratio of females to males was 1.25:1. Average patient age was 51.5 months (median, 21 months) and ranged from 5 weeks to 18 years. The average length of hospital stay was 1.5 days, though 30 patients were treated on an outpatient basis. Methicillin-resistant Staphylococcus aureus was identified in 76% of the cultured specimens. Average length of drain use was 9 days (range, 5-29 days). There were no local recurrences of subcutaneous abscesses. There was no morbidity related to the drainage procedures. Conclusion: We present a successful technique for the drainage and treatment of complex abscesses in children with limited, postoperative wound care and no morbidity or recurrence.
Loop and drain technique for subcutaneous abscess: a safe minimally invasive procedure in an adult population

Abstract:Background: This study evaluated the safety, efficacy and compliance of an emerging technique for managing complex subcutaneous abscesses in an adult population (≥16 years). Methods: A retrospective review of prospectively collected data between April and October 2015 at a rural hospital comparing conventional incision drainage (CID) and repetitive packing to the minimal incision, irrigation, loop and drain technique (LDT). LDT method was consistent with previous publications, being ≤5 mm incisions at abscess edges, irrigation and passage of a VessiLoop through the cavity and then secured above the skin. The loop remained in situ until resolution. Both groups had empirical oral antibiotics. Results: Sixty-three patients required intervention for complex subcutaneous abscess: CID group had 27 patients and LDT had 36 patients. The mean age of CID group was 43.9 years (range: 16-86 years), all required intervention in theatre with 10 patients lost to follow-up (37%) and the remaining 17 required a mean of 11.8 (range: 1-17) care visits. The mean age of LDT group was 34.9 years (range: 16-62 years), 11 completed intervention in ED under local anaesthetic (30%) and 27 required general anaesthesia. Compliance to follow-up clinic was 100%: 27 seen once at 10-14 days with loop removal and nine were seen a second time to complete care. There were no re-operations. Conclusion: This is the first LDT data in adults and proves it is safe and effective. It saves operating theatre time with 30% of LDT treated in the Emergency Department and ensures excellent follow-up compliance as patients return for VessiLoop removal. Healthcare burden is greatly reduced using LDT; most patients require only one review at 10-14 days.
A Randomized Controlled Trial of Novel Loop Drainage Technique Versus Standard Incision and Drainage in the Treatment of Skin Abscesses

Abstract:Objectives: The objective was to compare the failure rate of incision and drainage (I&D) with LOOP technique versus I&D with standard packing technique in adults and children presenting to the emergency department (ED) with subcutaneous abscess. Methods: This prospective, randomized controlled trial (NCT03398746) enrolled a convenience sample of patients presenting to two Level 1 trauma centers over 12 months with skin abscesses. Of 256 patients screened, 217 patients were enrolled, 109 randomized to I&D with packing (50%) and 108 (50%) to I&D with LOOP. The primary outcome was treatment failure defined by admission, IV antibiotics, or repeat drainage within 10-day follow-up. The secondary outcomes included ease of procedure, ease of care, pain, and satisfaction using a 10-point numeric rating scale. Results: There were no differences in patient characteristics between groups. Follow-up data were available in 196 (90%). Treatment failure occurred in 20% (range = 12%-28%) of packing patients and 13% (range = 6%-20%) of LOOP patients (p = 0.25). There were no significant differences in failure rates in adults (p = 0.82), but there was a significant difference in children (age ≤ 18 years) at 21% (range = 8%-34%) in the packing group and 0 (0%) in the LOOP group (p = 0.002). Operators reported no significant differences in ease of procedure between techniques (p = 0.221). There was significantly less pain at follow-up in the LOOP group versus packing (p = 0.004). The wound was much easier to care for over the first 36 hours in the LOOP group (p = 0.002). Patient satisfaction at 10 days postprocedure was significantly higher in the LOOP group (p = 0.005). Conclusions: The LOOP and packing techniques had similar failure rates for treatment of subcutaneous abscesses in adults, but the LOOP technique had significantly fewer failures in children. Overall, pain and patient satisfaction were significantly better in patients treated using the LOOP technique.

Abstract:Adenovirus and reovirus are nonenveloped viruses that engage cell-surface receptors using filamentous attachment proteins with head-and-tail morphology. The coxsackievirus and adenovirus receptor (CAR) and reovirus receptor junctional adhesion molecule 1 (JAM1) are immunoglobulin superfamily members that form homodimers stabilized by ionic and hydrophobic contacts between their N-terminal immunoglobulin-like domains. Both proteins are expressed at regions of cell-cell contact and contain sequences in their cytoplasmic tails that anchor the proteins to the actin cytoskeleton. Like CAR and JAM1, the attachment proteins of adenovirus and reovirus, fiber and sigma1, respectively, also share key structural features. Both fiber and sigma1 have defined regions of flexibility within the tail, which is constructed in part using an unusual triple beta-spiral motif. The head domains of both proteins are formed by an 8-stranded beta-barrel with identical beta-strand connectivity. Strikingly, both adenovirus fiber and reovirus 1 engage their receptors by interacting with sequences that also mediate formation of receptor homodimers. Therefore, while adenovirus and reovirus belong to different virus families and have few overall properties in common, the observed similarities between the receptors and attachment proteins of these viruses suggest a conserved mechanism of attachment and an evolutionary relationship.

Abstract:Introduction: It is difficult to overestimate the personal and socioeconomic impact of chronic low back pain (CLBP). It is the leading cause of years lost to disability and poses the highest economic toll among chronic illnesses. Despite the strong need for extensive research efforts, few drugs have consistently demonstrated effectiveness for this condition. Areas covered: In this review, the epidemiology, rationale for mechanism-based treatment, competitive environment and market trends, and the preclinical and clinical evidence supporting over 15 different classes of analgesic medications studied for CLBP or related pain conditions are discussed. Treatments are divided by drug category, type of CLBP they are likely to treat (e.g., neuropathic or mechanical), and whether they are new formulations of existing treatments, new indications for existing treatments or represent novel mechanisms of action. Databases searched included MEDLINE, Embase, Pharmaprojects and various clinical trial registries. Expert opinion: Many barriers exist for the development of medications for CLBP including difficulties in identifying pathophysiological mechanisms, biologic resiliency secondary to multiple concurrent pain pathways and off-target and sometimes serious side effects. Nevertheless, the volume and diversity of novel molecular entities has continued to surge and includes possible disease-modifying therapies such as gene and stem cell therapy.

Abstract:INTRODUCTION: Incision and drainage (I&D) of abscess is the gold standard treatment. The purpose of this study was to compare Loop Drainage Technique (LDT) of subcutaneous Head and Neck abscess with I&D in terms of primary outcome like efficacy and change in abscess diameter and secondary outcome like pain, duration of procedure, duration of healing and patient satisfaction. MATERIALS AND METHODS: A prospective cohort study was conducted in a tertiary care hospital for one year. Patients were consecutively allocated to I&D and LDT. All patients underwent ultrasonic evaluation of abscess size pre-procedure and 7th day post-procedure. Both groups received antibiotics at discharge and followed up at regular interval. RESULTS: A total of 70 patients were selected (35 in each group). Mean(sd) change in abscess diameters between I&D and LDT was not significant (p=0.83). In terms of efficacy or healing both procedures were equivalent (p=0.72). The secondary outcome like Pain (VAS score), duration of procedure was significantly lower in LDT than I&D (p<0.0001). The patient satisfaction score measured by Likert scale was significantly better in LDT than I&D (p<0.0001). However duration of healing was same in both groups (p=0.49). CONCLUSION: Loop drainage is equally efficacious to standard I&D in abscess resolution but the duration of procedure and pain experienced was less, with better compliance.
Loop Drainage Is Noninferior to Traditional Incision and Drainage of Cutaneous Abscesses in the Emergency Department

Abstract:Background: There is limited research on loop drainage (LD) compared to incision and drainage (I&D) for treatment of cutaneous abscesses. We investigated whether LD was noninferior to I&D for abscess resolution and whether there was any difference in repeat ED visits or complication rates between these techniques. Methods: We performed a prospective randomized controlled trial, using a convenience sample at an urban academic emergency department (ED). Subjects over 18 years who presented for first-time management of an abscess were eligible. Patients requiring specialist drainage or hospital admission or had previous treatment for the abscess were excluded. Enrolled subjects were seen 2 weeks after treatment for blinded reevaluation of abscess resolution, and the electronic medical record was reviewed for return ED visits/abscess complications. Results: Of 2,889 patients screened, 238 subjects consented and were randomized to LD or I&D. Abscess resolution was achieved in 53/65 (81.5%) of patients in the I&D arm, compared to 66/75 (88%) in the LD arm. Fewer patients in the LD group compared to the I&D group returned to the ED for abscess-related management during the following 14 days (37.3% vs 67.1%, p = 0.002). Among returning subjects, there was a significant difference in mean visits per subject between LD and I&D groups (0.5 vs. 1.2, p = 0.001). There were fewer complications among LD than I&D subjects (9.3% vs. 24.6%, p = 0.01). Conclusion: Our study provides evidence that LD is noninferior to I&D in achieving complete abscess resolution at 14 days and is associated with fewer return ED visits and fewer complications. This makes it an attractive alternative treatment option for abscesses.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Background</h3><p>Cutaneous abscesses are common presentations to the Emergency Department. While the primary treatment for most abscesses is conventional incision and drainage (CID), this is painful and can lead to multiple return visits. The loop drainage technique (LDT) has been proposed as an alternate, less‐invasive approach to abscess management. The primary outcome of this study was to compare LDT with CID for skin and soft tissue abscesses.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>PubMed, Scopus, CINAHL, LILACS, Google Scholar, the Cochrane Database of Systematic Reviews, the Cochrane Central Register of Controlled Trials, and bibliographies of selected articles were assessed for all retrospective, prospective observational, and randomized controlled trials comparing treatment failures between LDT and CID among patients with skin and soft tissue abscesses. Data were dual extracted into a predefined worksheet and quality analysis was performed using the Cochrane Risk of Bias tool or the Newcastle Ottawa scale. Data were summarized and presented as odds ratios (OR) with 95% confidence intervals (CI). Subgroup analyses were performed for adult and pediatric patients.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>1,374 studies were identified with eight studies (n=910 patients) selected for inclusion. Overall, CID failed in 69 out of 487 patients (14.17%), while LDT failed in 35 out of 423 patients (8.27%). There was an OR of 2.02 (95% CI 1.29 to 3.18) in favor of higher failures in the CID group. This finding remained consistent with only randomized controlled trials (OR: 1.75; 95% CI 1.07 to 2.86), but no difference was identified in the adult or pediatric subgroups.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusion</h3><p>The LDT was associated with reduced treatment failures when compared with CID. Future studies should further assess the impact on pain, cosmetic outcomes, and healthcare costs.</p></section>

Abstract:Introduction: Perianal abscesses are commonly treated using the traditional cruciate incision and drainage method, considered the standard universal therapy. However, a minimally invasive approach involving a small incision for draining uncomplicated perianal abscesses is gaining attention. In this comparative analysis, we explore the efcacy of this minimally invasive technique against the conventional approach. Methods: Our research investigates the postoperative outcomes of two different surgical methods: traditional cruciate incision and drainage versus a minimally invasive approach. This study was conducted at the Dr. Scott's Laser Piles Fistula Daycare Centre in South India over a 42-month period from December 2019 to June 2023. The study involved 36 patients and was designed as a retrospective descriptive study. Statistical analysis primarily involved calculating the mean. Key parameters under scrutiny encompass pain levels, dressing requirements, recurrence rates, readmissions, operative time, length of hospital stay, complications, wound healing progress, and associated costs. We have employed the Result: Clavien-Dindo classication system, a widely accepted standard in surgical practice, to categorize adverse complications resulting from surgical procedures. Our study outcomes demonstrate clear benets associated with the minimally invasive approach. Patients who underwent this procedure reported lower levels of postoperative pain, reduced demand for dressing changes, and quicker recuperation. While recurrence rates were similar between the two methods, the minimally invasive approach led to signicantly fewer readmissions. The procedure's shorter operative time contributed to a decreased duration of hospital stay. Notably, complications occurred less frequently, and wound healing progressed more positively in this group. Based on the comprehensive comparative analysis, we rec Conclusion: ommend the minimally invasive approach, such as transcutaneous drainage, as an ideal strategy for managing uncomplicated perianal abscesses. This alternative technique demonstrates comparable efcacy and offers several advantages regarding patient comfort, resource utilization, and healthcare costs.

Abstract:Abscess is a common ailment presenting to the Surgical OPD and Emergency on a daily basis. Conventionally, incision and drainage(ID) by making an incision in the most fluctuant part and later packing the cavity with a wick has been practised(1). We postulate a newer method of loop drainage(LD) of the cavity should be the new Gold Standard. This study aims to compare clinical outcome on conventional incision and drainage versus loop drainage of subcutaneous abscess. Study design was a prospective, observational study. Study was conducted in Silchar Medical College, from 23/12/2022 - 23/06/2023 , with a total of 120 patients. Of the 120 patients followed up, 48 underwent ID and 72 underwent LD. LD patients had a better pain profile post operatively on FLACC and Likert scales (-7.62 D1, 5.46 D3, 4.05 D5), lower treatment failure rate(-10.8%), more favourable scar formation(POSAS patient and observer -17.52, - 10.30). Latex loops had better scores than PVC loops(POSAS - 15.30, - 9.45 vs -18.34, - 12.45). Loop drainage of abscess, especially with Latex Loops have lower failure rates, better compliance and better clinical outcomes. Especially in paediatric patients where ID is more difficult and cumbersome and loss to follow up more common, LD should no doubt be considered the new Gold Standard in surgical practice.

Abstract:Background Treatment of lacrimal sac abscess of the traditional surgical approach may result in complications from cutaneous fistula formation, damage the sac, cause skin scarring and even have the potential for inducing cicatricial ectropion. We designed a new treatment scheme that is expected to achieve internal drainage with the use of lacrimal probe and crawford silicon tube. Methods A prospective study was performed for the management of lacrimal sac abscesses. All suitable patients from January 2011 to June 2014 were managed by lacrimal probe and crawford tube insertion. Postoperatively, patients received 0.5% Levofloxacin eye drops four times per day and oral Levofloxacin tablets 0.5 g once per day for four days. Follow-up times were for more than three months after removing the Crawford tube. The condition of the lacrimal sac and the patient’s symptoms were carefully evaluated. Results Fourteen patients suffering from lacrimal sac abscesses were included in this study. A history of chronic dacryocystitis was found in six patients, after acute dacryocystitis was found in three patients, and nasolacrimal occlusion with epiphora was found in other five patients. Resolution of signs and symptoms of lacrimal sac abscesses in all fourteen patients. No recurrence of lacrimal sac abscesses occurred during the median follow up period of 20.9 ± 7.8 months (range 6–36 months). Epiphora reoccurred in four patients. Conclusions Lacrimal probe and crawford silicon tube is successful as a procedure of choice for lacrimal sac abscesses. The insertion of a Crawford tube also offers potential advantages over standard treatment with the lack of recurrence of dacryocystitis or infection in post-surgical patients.
Turning behavior in Drosophila larvae: a role for the small scribbler transcript

Abstract:The Drosophila larva is extensively used for studies of neural development and function, yet the mechanisms underlying the appropriate development of its stereotypic motor behaviors remain largely unknown. We have previously shown that mutations in scribbler (sbb), a gene encoding two transcripts widely expressed in the nervous system, cause abnormally frequent episodes of turning in the third instar larva. Here we report that hypomorphic sbb mutant larvae display aberrant turning from the second instar stage onwards. We focus on the smaller of the two sbb transcripts and show that its pan‐neural expression during early larval life, but not in later larval life, restores wild type turning behavior. To identify the classes of neurons in which this sbb transcript is involved, we carried out transgenic rescue experiments. Targeted expression of the small sbb transcript using the cha‐GAL4 driver was sufficient to restore wild type turning behavior. In contrast, expression of this sbb transcript in motoneurons, sensory neurons or large numbers of unidentified interneurons was not sufficient. Our data suggest that the expression of the smaller sbb transcript may be needed in a subset of neurons for the maintenance of normal turning behavior in Drosophila larvae.

Abstract:AbstractIntroductionEmergency department visits for cutaneous abscesses are increasing. It is important for healthcare professionals to be proficient in identifying and treating abscesses. Loop drainage technique (LDT) is a newer technique which has been described in several articles but limited resources for teaching have been studied. The objective of this study was to compare 3 models for learning and teaching the LDT.MethodsThis was a prospective survey study of a convenience sample of emergency medicine residents at a large urban academic center. Residents volunteered to participate during a scheduled cadaver and simulation session. After a self-directed review of the LDT, each participant performed ultrasound visualization and then the LDT on 3 simulated abscesses: a cadaveric model, a commercial abscess pad, and a homemade phantom. Participants completed pre- and post-simulation surveys.ResultsOf 57 residents, 28 participated in the 1-day simulation. The majority (57.1%, p < 0.009) preferred the cadaver model for learning the LDT, and 78.6% reported it to have the most realistic physical examination for an abscess (p = 0.001). Prior to participation, 0% of residents felt proficient performing LDT. After participation, 46.4% of residents felt proficient and 78.6% reported intent to use in clinical practice (p < 0.001).ConclusionsSimulation is an effective educational tool for both learning new skills and improving procedural competency. Residents found cadavers provided the most realistic physical examination, and the majority preferred it for learning the LDT. However, cadavers are not always accessible, an important factor when considering various educational settings.

Abstract:IntroductionThe standard treatment for an abscess since time immemorial has been Incision and Drainage [I&D] without primary closure at most sites. Post-I&D, pain and ugly scar are the most common complaints in these patients. In this study, we have evaluated the role of minimal access surgery [MAS] in the management of superficial surgical infections. Materials and methodsThis was a prospective study, comparing the clinical and cosmetic outcomes of patients undergoing conventional I&D of superficial abscesses and those undergoing MAS [ 5mm incision for a port of 5mm diameter] drainage. ResultsA total of 50 patients were recruited, out of which 2 patients had a spontaneous rupture. 25 patients underwent I&D and 23 underwent MAS drainage. The mean age was 35.8 ±13.5 years, mean size of the abscess was 6 ± 1.9 cm. Baseline characteristics like age, sex distribution, duration of symptoms, size of the abscess and volume of the pus drained were comparable between the two groups. Resolution of pain and redness was achieved earlier in MAS drainage compared to conventional I&D. Resolution of induration was statistically insignificant. Duration of the need for dressing and duration of return to daily activity was less with MAS drainage. Scar size was significantly less in the MAS drainage group. Staphylococcus aureus was the most common organism isolated, and both groups had comparable rates of complications and recurrences. ConclusionOur study has shown that MAS drainage offers earlier recovery along with better cosmetic outcomes.
Ambulatory Pathway for the Management of Superficial Abscesses: Criteria for Safe Provision of Care

Abstract:Background Superficial abscesses are common surgical emergencies and unless complicated, are considered a low-priority emergency often leading to delayed primary treatment. Patients who are clinically stable benefit more from an ambulatory-care approach to their treatment. Aim of study This is a retrospective study to investigate the current practice of treatment of superficial abscesses at West Suffolk Hospital, United Kingdom, and evaluate the potential impact of implementing care via the ambulatory pathway to improve patients' experience, optimise the use of hospital resources and identify areas for service improvement. Patients and methods A total of 76 patients who required inpatient care for drainage of their superficial abscess under general anaesthesia over six months were a part of the study. Admission, procedure, and hospital stay characteristics were evaluated as well as our proposed superficial abscess ambulatory-care pathway (SAAP) criteria. Results The mean age was 39 ± 18 years. Perianal and pilonidal abscesses accounted for 53% of the procedures. Only 24% of the patients had a C-reactive protein (CRP) >100 at admission. The average waiting time before intervention was 19 hours (± 16.25) with patients spending two days on average in the hospital for a procedure that lasted an average of 31 minutes. Overall, 39.5% of the patients received drainage of their abscess on the same day of presentation to the hospital with only five discharges on the same day of admission. Of the total patients, 52.6% met our SAAP criteria for ambulatory care and could have avoided inpatient care and benefitted from same-day discharges. Conclusion Ambulatory care of a superficial abscess is a safe, feasible approach. We recommend establishing an ambulatory care pathway for the management of superficial abscess cases with our SAAP criteria serving as a useful objective guide for effective and safe triage of patients with reduced hospital stay and more efficient utilisation of resources.
A Randomized Controlled Trial Comparing Ultrasonography-Guided Needle Aspiration and Surgical Drainage for the Management of Breast Abscess

Abstract:Background A breast abscess is a localized collection of inflammatory exudate in breast tissue. It is more common in lactating women. Treatment of the breast abscess is usually by incision and drainage, which is accompanied by extensive trauma, lengthy surgical incisions, psychological distress, surgical scar, and discomfort during dressing changes. Recent research has shown that ultrasonography-guided needle aspiration is an alternative to incision and drainage for breast abscess treatment, has superior cosmetic outcomes, and has fewer costs. However, there are no clear guidelines for the same. The primary objective of this study was to assess and contrast the outcomes and efficacy of two approaches in the treatment of breast abscesses: ultrasonography-guided needle aspiration and conventional incision and drainage. Methods This was a single-center, prospective, randomized, controlled, non-inferiority trial. Patients with breast abscesses were randomized either to receive needle aspiration or incision drainage. Patients with lactational breast abscesses were encouraged to breastfeed from either breast. The antibiotic was given for 10 days to all the patients. The study's main focus was on the primary outcome, which was the failure rate. Additionally, several secondary outcomes were examined, including postoperative pain, time required for healing, presence of residual abscess or recurrence, formation of fistulas, and the assessment of scar appearance. Results A total of 44 patients were randomized to 23 and 21 in each arm. There was no statistical difference in treatment failure (p-value=0.862), fistula formation (p-value=1.00), and recurrence (p-value=1.00). There was a significant statistical difference in healing period (p-value <0.001), scar formation (p-value <0.0001), continuation of breastfeeding (p-value=0.005), and clinical resolution (p-value=0.002). There was a significant reduction in post-intervention pain in the needle aspiration group than in the other group (p-value <0.001). Conclusion This study showed a significant difference in postoperative pain, healing time, the continuation of breastfeeding, and scar formation in the needle aspiration group, favouring minimally invasive treatment for breast abscess. However, due to the small sample size, we could not conclude that ultrasonography-guided needle aspiration of the breast abscess is non-inferior to the conventional incision and drainage.

Abstract:Acute soft tissue abscesses are common conditions in our environment. For the treatment of skin and soft tissue abscesses, options include repeated aspiration, incision and drainage, incision and drainage with primary closure and conservatively treated by giving antibiotics. The established principle of surgical management of abscesses has been incision and free drainage; this permits healing by secondary intention or treatment by secondary closure. This modality of treatment has been challenged with the introduction of antibiotics. Ellis taught that the abscess wall prevented access of blood-borne antibiotics to the abscess cavity and that if this wall was curetted away the cavity could ll with antibiotic-laden blood clot, permitting safe primary closure. The primary closure technique is supported by many surgeons who showed its effectiveness in the treatment of breast, anorectal, axillary abscesses. Advantages of primary closure technique include faster healing rate, less hospital stay and early return to work, no greater recurrence than the conventional method, better scar formation and reduced cost of labour and material. It may be recommended as an alternative treatment that is superior to the orthodox technique
Purulent Skin and Soft Tissue Infections, Challenging the Practice of Incision and Drainage: A Scoping Review

Abstract:Aim. To generate a landscape of the current knowledge in the interventional management and outcomes of purulent skin and soft tissue infections. Design. This study is a scoping review. Methods. Electronic searches were undertaken using CINAHL, Medline, Cochrane Library, British Nursing Index, Science Direct, the National Health Service knowledge and library hub, ClinicalTrials.gov, and MedNar. The population, concept, context framework, and the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews were utilised, supporting a rigorous appraisal and synthesis of literature. Data Sources. The initial search and synthesis of literature were completed in January 2022 with repeat searches completed in March 2022 and July 2023. There were no imposed chronological parameters placed on the returned literature. Results. Nineteen papers were reviewed. Incision and drainage with primary closure, needle aspiration, loop drainage, catheter drainage, and suction drainage are viable adjuncts or alternatives to the traditional surgical management of skin and soft tissue abscesses. Conclusion. Despite the empirically favourable alternatives to the incision and drainage technique demonstrated, this does not appear to be driving a change in clinical practice. Future research must now look to mixed and qualitative evidence to understand the causative mechanisms of incision and drainage and its ritualistic practice. Implications. Ritual surgical practices must be challenged if nurses are to improve the treatment and management of this patient group. This will lead to further practice innovation. Impact: This study explored the challenges posed to patients, clinicians, nurses, and stakeholders, resulting from the ritualistic practice of the incision and drainage technique in purulent skin or soft tissue abscesses. Empirically and holistically viable alternatives were identified, impacting all identified entities and recommending a wider holistic study. Reporting Method. Adherence to EQUATOR guidance was achieved through the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews.
