Abstract:ObjectiveThe aim of this study was to determine whether a AOI strategy on non-ventilated lung could reduce the regional and systemic proinflammatory cytokine and oxidative stress response associated with esophagectomy, and to evaluate whether AOI can be used as a novel lung protective ventilation strategy. Its impact on oxygenation after OLV, surfactant protein A, B, C (SP-A, B, C), postoperative hospital stay and postoperative pulmonary complications (PPCs) was also evaluated.MethodsFifty-four adults (ASA II-III) undergoing esophagectomy with OLV were enrolled in the study. Patients were randomly assigned into 2 groups: control group (group C) and treated group (group T). Group C was treated with traditional OLV mode,while group T was given AOI of 5 L/min oxygen on the non-ventilated lung immediately at the beginning of OLV. Arterial blood gas was analyzed before and after OLV. A bronchoalveolar lavage(BAL) was performed after OLV on the non-ventilated lung. Proinflammatory cytokine, oxidative stress markers(TNF-α, NF-κB,sICAM-1,IL-6,IL-10,SOD,MDA) and SP-A, B, C were analyzed in serum and BALF as the primary endpoint.The clinical outcome determined by PPCs was assessed as the secondary endpoint.ResultsPatients with AOI had better oxygenation in the recovery period, oxygenation index(OI) (394[367–426] and 478[440–497]mmHg, respectively) of group T at T2 and T3 were significantly higher than those (332[206–434] and 437[331–512]mmHg, respectively) of group C. OLV resulted in an increase in the measured inflammatory markers in both groups, however, the increase of inflammatory markers upon OLV in the group C was significantly higher than those of group T. OLV resulted in an increase in the measured SP-A, B, C in serum of both groups. However, the levels of SP-A, B, C of group T were lower than those of group C in serum after OLV, and the results in BALF were the opposite. The BALF levels of SOD(23.88[14.70–33.93]U/ml) of group T were higher than those(15.99[10.33–24.16] U/ml) of group C, while the levels of MDA in both serum and BALF of group T(8.60[4.14–9.85] and 1.88[1.33–3.08]nmol/ml, respectively) were all lower than those of group C (11.10[6.57–13.75] and 1.280[1.01–1.83]nmol/ml) after OLV. There was no statistical difference between the two groups in terms of postoperative hospital stay and the incidence of PPCs.ConclusionAOI on non-ventilated lung during OLV can improve the oxygenation function after OLV, relieve the inflammatory and oxidative stress response in the systemic and non-ventilated lung after OLV associated with esophagectomy.Trial registrationChiCTR-IOR-17011037. Impact of pulmonary infection after radical esophagectomy on serum inflammatory markers, pulmonary function indices, and prognosis

Abstract:Objectives: To analyze the influence of pulmonary infection after radical esophagectomy on serum inflammatory markers, pulmonary function, and prognosis. Methods: We enrolled 278 esophageal cancer patients who underwent radical esophagectomy. Patients were split into the infected (n=51) and uninfected groups (n=227). The inflammatory parameters, complications, and prognosis were compared. Results: In the infected group, interleukin (IL)-6 was 16.19±2.63 ng/L, tumor necrosis factor-α was 19.64±3.07 μg/L, and IL-1β was 22.49±5.13 ng/L at 7 days postoperatively; white blood cell counts was 12.65±2.14 x10 9 /L, percentage of neutrophils (NEU%) was 67.04±10.48%, and platelet (PLT) counts was 249.82±63.26 x10 9 /L; the increasing ranges of the above factors after the operation were much raised compared with the uninfected group ( p <0.05). Compared with the uninfected group, forced expiratory volume in one second (FEV1), forced vital capacity (FVC), and FEV1/FVC were greater declines in ranges ( p <0.05), and the arrhythmia incidence and the mortality within 60 days postoperatively were greater in the infected group ( p <0.05). Conclusion: Postoperative pulmonary infection can lead to pulmonary function damage, proinflammatory factor overexpression, and an increased risk of early death.

Abstract:Infectious diarrhea is seen with increasing frequency in general practice. The main reason is growing tourism in countries with conditions of public health different from ours, where the incidence of enteropathogenic parasites is higher. To obtain an overview of these parasites, the results of 23,276 stool samples from patients in general practice throughout Switzerland were analyzed. Protozoa were found in 32% of samples (4.6% pathogenic protozoa, 24% facultative pathogenic protozoa and 3.4% apathogenic protozoa). Helminths were demonstrated in 2.9% of stool samples. The helminth spectrum covered 15 types, with Trichuris trichiuria predominating (one third). 8% of the Swiss population travel every year to areas with a higher diarrhea incidence. A proportion of these subjects constitute a health risk on their return to Switzerland. They chiefly consist of people working in, or living with someone working in, the food industry. Specific parasitologic diagnosis of diarrhea contributes significantly to reducing this risk.
Analysis of Risk Factors for Acute Lung Injury/acute Respiratory Distress Syndrome after Esophagectomy

Abstract:OBJECTIVE:To explore the incidence and risk factors for the acute lung injury (ALI)/acute respiratory distress syndrome (ARDS) after resection of esophageal carcinoma.METHODS:We retrospectively analyzed 422 consecutive patients admitted to the Department of Critical Care Medicine with esophageal carcinoma undergoing esophagectomy from January 2010 to December 2016 in Peking University Cancer Hospital. ALI/ARDS were diagnosed, the patients were divided into ALI/ARDS group and control group without ALI/ARDS, the differences of clinical features were contrasted between the two groups, and the multivariate Logistic regression modeling was used to identify the independent risk factors for ALI/ARDS.RESULTS:In the study, 41 ALI/ARDS cases were diagnosed, making up 9.7% (41/422) of all the enrolled patients undergoing esophagectomy. Comparisons of the ALI/ARDS group and the control group indicated significant statistical differences in the average length of their hospital stay [(18.9±9.7) d vs. (14.8±3.6) d, P=0.011], the proportion of the patients who needed mechanical ventilation support [51.2% (21/41) vs. 9.4% (36/381), P<0.001] and in-hospital mortality [31.7% (13/41) vs. 5.0% (19/381), P<0.001]. Univariate analysis showed significant differences between the patients with ALI/ARDS and without ALI/ARDS in smoking history (P=0.064), preoperative forced expiratory volume in one second/forced vital capacity (FEV1/FVC) (P=0.020), diffusing capacity of the lung for carbon monoxide (DLCO) (P=0.011), body weight index (BMI) (P=0.044), American Society of Anesthesiologists (ASA) physical status classification (P=0.049) and one lung ventilation duration (P=0.008), while multivariate Logistic regression analysis indicated that preoperative FEV1/FVC (OR=1.053, P=0.016, 95%CI 1.010-1.098), ASA physical status classification (OR=2.392, P=0.033, 95%CI 1.073-5.335) and one lung ventilation duration (OR=0.994, P=0.028, 95%CI 0.989-0.999) were the independent risk factors for ALI/ARDS after esophagectomy.CONCLUSION:ALI/ARDS was a serious complication in patients undergoing esophagectomy associated with increment in length of hospital stay and in-hospital mortality. Multivariate Logistic regression analysis indicated that preoperative FEV1/FVC, ASA classification and one lung ventilation duration were the independent risk factors for ALI/ARDS after esophagectomy. Carefully assessing the patient before operation, shortening one lung ventilation duration were the key points in preventing ALI/ARDS after esophagectomy.

Abstract:Abstract Introduction Esophagectomy for esophagogastric cancer is associated with high rates of peri-operative morbidity and mortality. Little is known of the impact of one lung ventilatory (OLV) times during the thoracic phase of an esophagectomy upon perioperative outcomes. The aim of this study was to determine the impact of OLV upon post-operative complications following esophagogastric cancer resection. Methods Consecutive patients undergoing an esophagectomy at a single, U.K tertiary referral centre for esophagogastric cancer between January 2015–January 2022 were evaluated. OLV time was defined as the number of minutes there was iatrogenic occlusion of one of the main bronchi such that all ventilatory requirements were performed by one lung. Previously published OLV times were used to determine if the duration was standard (<150 minutes) or prolonged (>=150 minutes). Post-operative complications were prospectively evaluated and classified using the Clavien-Dindo (CD) classification system. Patients were grouped as either CD 0-I (none-minor complications) or CD II-V (moderate–severe complications including post-operative mortality). Results There were 361 patients included in the final analysis. OLV times of <150 minutes were recorded in 101 patients and >=150 minutes were recorded in 260 patients. Both groups were comparable in terms of development of CDII-V complications with no statistically significant intergroup differences (91/101 vs 241/260; p=0.075). Notably, 41/91 (45.1%) of patients with an OLV time <150 minutes developed a pulmonary complication versus 100/241 (41.5%) with prolonged OLV times. Conclusion Pulmonary and other peri-operative complications remain a considerable burden in the recovery of patients following esophagogastric cancer resection, independant of OLV times. Strategies to mitigate this are need in order to reduce peri-operative morbidity and improve overall quality of life into survivorship.

Abstract:BACKGROUND Currently, the primary treatment for gastroesophageal reflux is acid suppression with proton pump inhibitors, but they are not a cure, and some patients don’t respond well or refuse long-term use. Therefore, alternative therapies are needed to understand the disease and develop better treatments. Laparoscopic anti-reflux surgery (LARS) can resolve symptoms of these patients and plays a significant role in evaluating esophageal healing after preventing harmful effects. Successful LARS improves typical gastroesophageal reflux symptoms in most patients, mainly by reducing the exposure time to gastric contents in the esophagus. Amelioration of the inflammatory response and a recovery response in the esophageal epithelium is expected following the cessation of the noxious attack. AIM To explore the role of inflammatory biomolecules in LARS and assess the time required for esophageal epithelial recovery. METHODS Of 22 patients with LARS (pre- and post/5.8 ± 3.8 months after LARS) and 25 healthy controls (HCs) were included. All subjects underwent 24-h multichannel intraluminal impedance-pH monitoring and upper gastrointestinal endoscopy, during which esophageal biopsy samples were collected using endoscopic techniques. Inflammatory molecules in esophageal biopsies were investigated by reverse transcription-polymerase chain reaction and multiplex-enzyme-linked immunosorbent assay. RESULTS Post-LARS samples showed significant increases in proinflammatory cytokines [interleukin (IL)-1β, interferon-γ, C-X-C chemokine ligand 2 (CXCL2)], anti-inflammatory cytokines [CC chemokine ligand (CCL) 11, CCL13, CCL17, CCL26, CCL1, CCL7, CCL8, CCL24, IL-4, IL-10], and homeostatic cytokines (CCL27, CCL20, CCL19, CCL23, CCL25, CXCL12, migration inhibitory factor) compared to both HCs and pre-LARS samples. CCL17 and CCL21 levels were higher in pre-LARS than in HCs (P < 0.05). The mRNA expression levels of AKT1, fibroblast growth factor 2, HRAS, and mitogen-activated protein kinase 4 were significantly decreased post-LARS vs pre-LARS. CCL2 and epidermal growth factor gene levels were significantly increased in the pre-LARS compared to the HCs (P < 0.05). CONCLUSION The presence of proinflammatory proteins post-LARS suggests ongoing inflammation in the epithelium. Elevated homeostatic cytokine levels indicate cell balance is maintained for about 6 months after LARS. The anti-inflammatory response post-LARS shows suppression of inflammatory damage and ongoing postoperative recovery.

Abstract:Objectives: Pulmonary complication remains common morbidity after thoracoscopic minimally invasive oesophagectomy (MIE), partially due to lung injury caused by one-lung ventilation (OLV). We attempted two-lung ventilation (TLV) with low tidal volume in semi-prone position assisted by artificial CO2 pneumothorax. This study aimed to compare the impact of two ventilation strategies on postoperative pulmonary complications. Methods: From March 2011 to August 2013, 200 patients underwent thoracoscopic MIE. The initial 100 patients performed with OLV (succeeded in 99, with 1 case conversion), and the later half performed with TLV (succeeded in 99, with 1 case conversion). During thoracic stage, patients in the OLV group were intubated with a double-lumen endotracheal tube and given 8 ml/kg tidal volume; while in the TLV group, patients were intubated with a single-lumen endotracheal tube and also given 8 ml/kg tidal volume. An artificial CO2 pneumothorax was achieved at pressure of 8 mmHg. Meanwhile, a pre-placed bronchial blocker was used in case of conversion to thoracotomy. During the abdominal and cervical stage, all patients performed two-lung ventilation. Patients' demographics and clinical outcomes were recorded and statistically compared. Results: No significant difference was observed between the two groups in demographics, blood loss, lymph nodes harvested, postoperative hospital stay, total complications or 30-day mortality. The thoracic operative time was longer in the TLV group than in OLV (85.4 ± 13.4 min vs 74.8 ± 11.2 min, P = 0.000). The occurrence of pulmonary complications in the TLV group was lower (9.1% vs 20.2%, P = 0.027). Reintubation for ARDS/ALI was given in 12 patients (3 in TLV and 9 in OLV). The oxygenation index was significantly higher in the TLV group than OLV group on POD 1 (319.2 ± 31.3 vs 298.2 ± 29.9, P = 0.000) and POD 2 (330.8 ± 33.3 vs 318.0 ± 32.4, P = 0.006). Conclusions: Two-lung ventilation with low tidal volume during MIE in semi-prone position appears to be feasible, safe and effective. It may attenuate postoperative pulmonary complications. Additional RCT studies are required to confirm this conclusion. Disclosure: No significant relationships.

Abstract:Background: Minimally invasive esophagectomy (MIE) has been advantageous for lowering pulmonary complications compared with open approaches.(1) However, pulmonary complications remain the most common morbidity after surgical resection of esophageal cancer.(2,3) The aim of this prospective, randomized, controlled, clinical trial was designed to see whether low tidal volume (VT) could further minimize pulmonary complications after MIE.Methods: Between June 2011 and July 2012, a total of 101 patients who underwent MIE received left-lung ventilation during thoracoscopic esophagectomy. All patients received left-lung ventilation during thoracoscopic esophagectomy. Patients were randomly assigned to a low VT (5 mL/kg + 5 cm H2O positive end-expiratory pressure) preserved ventilation (PV) group (n = 53) and a conventional VT (8 mL/kg) controlled ventilation (CV) group (n = 48) in the thoracic stage. Alveolar lavage fluid was harvested from the ventilated lung at intubation and at 18 hours after surgery for analysis of interleukin (IL)-1 beta, IL-6, and IL-8 levels. Clinical characteristics, including patient demographics, operation features, and changes in oxygenation index, were recorded and analyzed. Pulmonary complications were identified and statistically compared between the 2 groups.Results: The clinical characteristics and operation features were comparable between the 2 groups. IL-1 beta, IL-6, and IL-8 expressions in preoperative alveolar lavage fluid were similar between the 2 groups. Significantly lower IL expressions were observed in the PV group than those in the CV group at 18 hours after MIE (IL-1 beta, 25.42 +/- 31.01 vs 94.96 +/- 118.24 pg/mL; IL-6, 30.86 +/- 75.78 vs 92.99 +/- 72.90 pg/mL; IL-8, 258.75 +/- 188.24 vs 403.95 +/- 151.44 pg/mL; all P < .05). The 18-hour postoperative oxygenation index was lower in the CV group than that in the PV group (292.85 +/- 28.74 vs 326.35 +/- 34.43; P = .046). Pulmonary complications were observed in 18 cases of our series, occurring more frequently on the ventilation side (right, 6 cases; and left, 12 cases). All patients were cured by conservative therapy without severe sequelae. The occurrence of pulmonary complications in the PV group was lower than that in the CV group (9.43% vs 27.08%; P = .021).Conclusions: Lung injury due to intraoperative single-lung ventilation may contribute to pulmonary complications after MIE. Low VT ventilation could decrease ventilation-associated lung inflammation, thus minimizing pulmonary complications after MIE. Further studies, based on a larger volume of populations, are required to confirm these findings.
Clinical Analysis of Patients with Respiratory Failure after Esophageal Cancer Operation

Abstract:Background To explore the clinical profiles and outcomes of patients with acute respiratory failure (ARF) after esophagectomy. Methods We retrospectively analyzed cases of patients who had been diagnosed with ARF after esophagectomy and compared survivors with non-survivors to explore the risks that may affect their outcomes. Results In total, 62 patients were admitted to the intensive care unit (ICU) with ARF after esophagectomy between January 1, 2010, and December 31, 2017. Of these patients, 69.4% needed mechanical ventilation, with an average time on the ventilator of 304 hours (304.33±374.37 hours). The average length of stay in the ICU and in the hospital were 14 days (14.48±17.64 days) and 50 days (50.15±37.28 days), respectively. Mortality in the ICU and 90 days after the operation was 6.5% and 16.1%, respectively. Compared with the survivors, the 90-day post-operative non-survivors had a poorer N stage in the TNM classification system. The causes of ARF included anastomotic leakage, pneumonia, vocal cord paralysis, sputum plugging, pulmonary embolism (PE), and acute respiratory distress syndrome (ARDS). ARF induced by different factors occurred at different times and had different outcomes. The three most common reasons for mortality in the ICU were ARDS (33.33%), anastomotic leakage (11.76%), and pneumonia (10%). The three most common reasons for mortality in the 90-day post-operative period were pneumonia (40%), anastomotic leakage (23.53%), and ARDS and acute exacerbations of chronic obstructive pulmonary disease (AECOPD) (33.33%). Conclusions Anastomotic leakage, pneumonia, ARDS, and AECOPD were the main causes of death in ARF patients after esophagectomy. We found that the N stage in the TNM classification system may affect 90-day post-operative mortality in these patients.
Lung Aeration and Ventilation after General Anesthesia in Left Lateral Position: a Prospective Observational Study Using Electrical Impedance Tomography.

Abstract:BACKGROUND Endoscopic submucosal dissection (ESD) under general anesthesia in left lateral position may lead to transient impairment of pulmonary function. We used electrical impedance tomography (EIT), an increasingly implied non-invasive instrument for bedside real-time monitoring regional changes in ventilation, to assess the changes of regional lung aeration and ventilation in patients undergoing ESD. METHODS Twenty-two patients scheduled for elective ESD under mechanical ventilation in left lateral position were studied. We acquired 2-min EIT records at four time points: (M1) baseline, before induction of anesthesia, (M2) after the start of mechanical ventilation and before surgery, (M3) after surgery and before extubation, and (M4) after extubation and before leaving operation room, respectively. To quantify regional changes in lung ventilation, we calculated the ventilation proportion of left and right lung regions. Possible changes in lung aeration were detected by changes in end-expiratory lung impedance (EELI). Global inhomogeneity index (GI) was also analyzed. RESULTS After tracheal intubation in the left lateral position, left lung showed a lower ventilation proportion (M1, 49.6% vs. M2, 36.2% P<0.05), a reduction in EELI {∆EELI -87 [-809; 253]} and a higher GI index value (M1, 0.29±0.09 vs. M2, 0.41±0.12, P<0.05), while right lung showed a higher ventilation proportion (M1, 50.4% vs. M2, 63.8%, P<0.05) and an increase in EELI {∆EELI 161 [-952; 1,905]}. During ESD operation, no changes in either regional ventilation distribution or GI index were observed. After extubation, the GI values in right and left lung were both returned to the level before anesthesia. CONCLUSIONS In patients with left lateral position undergoing ESD, left lung was characterized by decreased ventilation and more inhomogeneity while right lung was opposite after intubation. ESD procedure with carbon dioxide insufflation did not lead to significant changes in either regional ventilation or homogeneity. And the change of lung inhomogeneity during ESD procedure is transient.
Omental transposition to mediastinum improves the outcome of postoperative intra-thoracic infections of Ivor-Lewis surgery]

Abstract:To study the efficacy of pedicled greater omentum transposed to mediastinum in prevention against postoperative in-hospital intrathoracic complications after esophagectomy.Clinical data of 148 consecutive patients with esophageal cancer undergoing Ivor-Lewis surgery in our department from January 2010 to May 2014 were retrospectively reviewed. Among them, 84 patients with omental transposition(transposition group) and 64 patients without omental transposition(non-transposition group) were compared.Postoperative hospital stay was shorter in patients with omental transposition compared to those without omental transposition(P<0.05). Intrathoracic infection rate was significantly lower in transposition group(33/84, 39.3%) than that in non-transposition group(36/64, 56.2%), and as was the combined sepsis rate[19/33, 57.6% vs. 31/36, 86.1%, P<0.05]. No significant differences were found in the morbidity of anastomotic leakage, wound infection, cardiac complication and mortality during hospitalization. The median postoperative hospital stay was significantly shorter in transposition group than that in non-transposition group(13.0 vs. 16.5 days, P<0.05).Omental transposition to mediastinum can reduce the development and severity of intrathoracic infection and shorten hospital stay in patients undergoing esophagectomy.

Abstract:Acute respiratory distress syndrome (ARDS) has a significant impact on post-operative morbidity and mortality following oesophagectomy. Smoking is a risk factor for the development of ARDS, although the mechanism is unclear. We examined the effect of smoking on alveolar and systemic inflammation, in addition to alveolar-capillary permeability, leading to ARDS in patients undergoing oesophagectomy. We compared clinical, biomarker and PiCCO system data between current smokers (n=14) and ex-smokers (n=36) enrolled into a translational substudy of the BALTI-P (Beta Agonist Lung Injury Trial Prevention) trial. Current smokers compared with ex-smokers had significantly higher numbers of circulating neutrophils, elevated bronchoalveolar lavage (BAL) interleukin (IL)-1 receptor antagonist (IL-1ra), soluble tumour necrosis factor receptor-1 and pre-operative plasma soluble intercellular adhesion molecule-1, and lower BAL vascular endothelial growth factor and post-operative plasma IL-17 (p<0.05). On post-operative day 1, current smokers had higher extravascular lung water index (9.80 versus 7.90; p=0.026) and pulmonary vascular permeability index (2.09 versus 1.70; p=0.013). Current smokers were more likely to develop ARDS (57% versus 25%; p=0.031) and had a significantly reduced post-operative median survival (421 versus 771 days; p=0.023). Smoking prior to oesophagectomy is associated with dysregulated inflammation, with higher concentrations of inflammatory mediators and lower concentrations of protective mediators. This translates into a higher post-operative inflammatory alveolar oedema, greater risk of ARDS and poorer long-term survival.
Mediastinal Transposition of the Omentum Reduces Infection Severity and Pharmacy Cost for Patients Undergoing Esophagectomy.

Abstract:BACKGROUNDThe greater omentum has been found to be immunologically competent in protecting abdominal organs from inflammation. Anastomotic omentoplasty has been used and proven effective in preventing anastomotic leaks after an esophagectomy. However, pulmonary complications are still a substantial problem after an esophagectomy. This study investigated the benefits of mediastinal transposition of the omentum, a modification of the conventional omental wrapping technique, in controlling overall postoperative intrathoracic complications.METHODSFrom January 2010 to March 2015, 208 consecutive patients receiving an open Ivor-Lewis esophagectomy at our institution were retrospectively reviewed. One hundred twenty-one patients with omentum mediastinal transposition were assigned to the transposition group and 87 patients without omental transposition were placed in the non-transposition group. The patients' demographics, postoperative short-term outcomes, and in-hospital cost were documented and analyzed.RESULTSMediastinal transposition of the omentum led to a shorter postoperative hospital stay (14 vs. 16 d, P=0.038) and a lower intrathoracic infection rate (30.6% vs. 48.3%, P=0.009). Intrathoracic infection was milder in the transposition group (P=0.005), though a non-significant was found in overall complications (P=0.071). The multivariate logistic regression analyses identified omentum mediastinal transposition (P=0.007, OR=0.415) as an independent protective factor for postoperative intrathoracic infection. The total in-hospital cost was comparable in both groups (P>0.05), whereas the pharmacy cost was lower in the transposition group than in the non-transposition group (¥21,668 vs. ¥27,012, P=0.010).CONCLUSIONSMediastinal transposition of the omentum decreases the rate and severity of postoperative intrathoracic infection following an open Ivor-Lewis esophagectomy. This result in decreased pharmacy costs, rather than resulting in an increased economic burden sustained by surgical patients.
[Cytokine Responses after Lobectomy for Early Non-Small Cell Lung Cancer: a Prospective Randomized Comparison of Video-Assisted Thoracic Surgery and Open Thoracotomy].

Abstract:OBJECTIVE To compare video-assisted thoracic surgery (VATS) and open thoracotomy (OT) on acute inflammatory responses and immunosuppression after lobectomy for early non-small cell lung cancer (NSCLC). METHODS Present prospective randomized study. OT or VATS lobectomy was performed in patients who met enter criteria and clinical data was collected. Plasma concentration of IL-6, IL-8 and IL-10 were measured before surgery and at postoperative day (POD) 1 and POD 3. There were 271 patients underwent lobectomy for early NSCLC, including of 133 patients in group VATS and 138 patients in group OT from January 2007 to June 2008. There were 132 males and 139 females, aging from 19 ∼ 70 years with a mean of (56 ± 8) years. RESULTS Compared with OT group, shorter postoperative hospital stay [(8.2 ± 2.5) d vs. (9.8 ± 6.2) d, P = 0.03], lower morbidity rate (11.3% vs. 21.7%, P = 0.02) and lower increase of plasma concentration of IL-6 at POD 1 [(35 ± 25)% vs. (65 ± 43)%, P = 0.00], IL-6 at POD 3 [(14 ± 22)% vs. (55 ± 44)%, P = 0.00] and IL-10 at POD 1 [(25 ± 20)% vs. (43 ± 35)%, P = 0.00] were observed in patients of VATS group. CONCLUSION VATS lobectomy for early NSCLC is associated with less acute inflammatory responses and less immunosuppression when compared with OT.

Abstract:ObjectivesSystemic inflammation is a potentially debilitating complication of thoracic surgeries with significant physical and economic morbidity. There is compelling evidence for the role of the central nervous system in regulating inflammatory processes through humoral mechanisms. Activation of the afferent vagus nerve by cytokines triggers anti-inflammatory responses. Peripheral electrical stimulation of the vagus nerve in vivo during lethal endotoxemia in rats inhibited tumor necrosis factor synthesis and prevented shock development. However, the vagal regulatory role of systemic inflammation after lung lobectomy is unknown.MethodsOne hundred patients who underwent lobectomy via thoracotomy were recruited and equally randomized to treated group or controls. Intermittent stimulation of the auricular branch of vagus nerve in the triangular fossa was applied in the treated group using neurostimulator V (Ducest®, Germany), starting 24 h preoperatively and continued till the 4th postoperative day (POD). Inflammatory interleukins (IL) were analyzed using ELISA preoperatively, on the 1st and 4th POD.ResultsOn the 1st POD, patients who underwent neurostimulation had reduced serum concentrations of CRP (p = 0.01), IL6 (p = 0.02) but elevated IL10 (p = 0.03) versus controls. On the 4th POD, serum concentrations of CRP, IL6 and IL10 were similar in both groups. Moreover, the treated group was associated with lower incidence of pneumonia (p = 0.04) and shorter hospitalization time (p = 0.04) versus controls.ConclusionsModulations in the brain stem caused by noninvasive transcutaneous stimulation of the vagus nerve after lung lobectomy attenuate the acute postsurgical inflammatory response by the regulation of IL6 and IL10, resulting in reduced incidence of postoperative pneumonia and short hospitalization time.Clinical Trial Registry NumberNCT03204968.

Abstract:One of the major experimental achievements in the past decades is the ability to control quantum systems to high levels of precision. To quantify the level of control we need to characterize the dynamical evolution. Full characterization via quantum process tomography is impractical and often unnecessary. For most practical purposes, it is enough to estimate more general quantities such as the average fidelity. Here we use a unitary 2-design and twirling protocol for efficiently estimating the average fidelity of Clifford gates, to certify a 7-qubit entangling gate in a nuclear magnetic resonance quantum processor. Compared with more than 10^{8} experiments required by full process tomography, we conducted 1656 experiments to satisfy a statistical confidence level of 99%. The average fidelity of this Clifford gate in experiment is 55.1%, and rises to at least 87.5% if the signal's decay due to decoherence is taken into account. The entire protocol of certifying Clifford gates is efficient and scalable, and can easily be extended to any general quantum information processor with minor modifications.
Effect of protective ventilation on postoperative lung injury during one-lung ventilation

Abstract:[Objective] To evaluate the efficacy of individual protective ventilation strategy on the postoperative lung injury during one-lung ventilation (OLV). [Methods] Thirty-two patients undergoing pulmonary resection were randomly divided into two groups. Traditional ventilation group (TG): tidal volume 8 mL/kg and peak inspiratory pressure (PIP) <30 cmH2O. Protective ventilation group (PG): tidal volume 1/2VT (half-tidal-volume from preoperative pulmonary function test) and PIP<25 cmH2O. The arterial blood samples were collected for blood gases analysis immediately 20 minutes after two-lung ventilation (TLV) in the supine position after anesthesia (T1) and the end of operation (T2). The values of PaO2, PaCO2, pH and the inspired oxygen fraction (FiO2) were recorded and the values of oxygenation index (OI) were calculated respectively. The venous blood samples were collected for measurement of plasma interleukin-6 (IL-6) level at two time points. [Results] In all the patients, a decrease in OI and increase in plasma IL-6 level occurred at the end of operation (P <0.05). There were no significant differences of changes between the TG and the PG (P >0.05). For the overweight patients (difference between body weight and predicted body weight higher 10 kg), the decreases in OI were larger in traditional ventilation strategy than those in protective ventilation strategy (P <0.05). [Conclusion] During OLV, the individual protective ventilation strategy moderates the postoperative lung injury for the overweight patients.

Abstract:BACKGROUND:Pulmonary complications after orthotopic liver transplantation (OLT) include high morbidity and mortality. Experimental data have suggested hepatic ischemia and reperfusion are induced by pro-inflammatory cytokines. The high level of inflammatory cytokines might additionally influence pulmonary capillary fluid filtration. The objectives of this study were to measure the concentrations of tumor necrotic factor-alpha (TNF-alpha), interleukin-6 (IL-6) and interleukin-8 (IL-8) during OLT and to investigate the relationship between these cytokines and postoperative pulmonary complications.METHODS:Twenty-two patients undergoing OLT were divided into two groups according to whether they had postoperative pulmonary complications: group A consisting of 8 patients with postoperative pulmonary complications, and group B consisting of 14 patients without postoperative pulmonary complications. Enzyme-linked immunoassay (ELISA) was used to determine serum TNF-alpha, IL-6 and IL-8. Blood samples were taken at the beginning of operation (T0), clamping and cross-clamping of the inferior cava and portal vein (T1, T2), 90 minutes and 3 hours after reperfusion (T3, T4) and 24 hours after operation (T5).RESULTS:The level of PaO2/FiO2 in group A was lower than that in group B (P<0.05). The concentrations of TNF-alpha, IL-6 and IL-8 in the two groups increased rapidly at T2, peaked at T3, decreased rapidly after T3 until 24 hours after operation. The concentrations of TNF-alpha, IL-6 and IL-8 in group A were higher than those in group B at T2, T3, and T4 (P<0.05).CONCLUSION:After un-clamping of the inferior cava and portal vein, the serum concentrations of TNF-alpha, IL-6 and IL-8 increased may be related to pulmonary injury after hepatic ischemic reperfusion.

Abstract:The period has long since passed when it was considered reasonable to expect the diverse information handling requirements of a medical institution to be managed in a cost-effective manner by a “total hospital information system”. Instead, the computing power and economy of sale of minicomputers, the successful application of dedicated systems to a number of hospital information management functions, and the oversize investment and uncertain benefit of large scale systems efforts in the past have together caused many hospitals to make various commitments to information systems functional in particular areas in which the need has been felt to be strongest. It now appears unlikely that any single system will issue forth which has the combined capabilities of the best financial system, clinical laboratory information system, pharmacy inventory control system, and medical record management system currently available, to cite a few examples of functional areas which have met with relatively successful application.
