Abstract:OBJECTIVE To detect the expression of liver-type fatty acid-binding protein (L-FABP) and vascular endothelial growth factor (VEGF) in human hepatocellular carcinoma (HCC) and its adjacent liver tissues, and investigate the correlation between the expressions of L-FABP and VEGF and their role in the occurrence and progression of HCC. METHODS Reverse transcriptase-polymerase chain reaction (RT-PCR) and immunohistochemical (IHC) staining were employed to examine the expression of L-FABP and VEGF in HCC and its adjacent liver tissues obtained from the surgical specimens of 61 HCC patients who underwent liver resections in West China hospital. RESULTS The results of RT-PCR showed that the expression level of L-FABP and VEGF in HCC was significantly higher than that in its adjacent liver tissues (L-FABP: 0.97-/+0.12, 0.83-/+0.14, t=5.21, P<0.05; VEGF: 0.92-/+0.11, 0.59-/+0.15, t=11.79, P<0.05). L-FABP tended to co-express with VEGF (P<0.05). IHC staining revealed that the expression of L-FABP and VEGF was mainly located in the cytoplasm, and the gray scale of L-FABP expression was significantly higher than that in the adjacent liver tissues (92.73-/+7.67, 82.83-/+6.90, t=7.44, P<0.05). The number of L-FABP- and VEGF-positive cells in HCC was significantly lower than that in the adjacent liver tissues (L-FABP: 92.18-/+4.44, 84.52-/+6.43, t=5.94, P<0.05; VEGF: 88.69-/+5.56, 77.64-/+5.93, t=8.72, P<0.05). Co-expression of L-FABP and VEGF observed in RT-PCR and also in IHC (P<0.05). CONCLUSION Both L-FABP and VEGF expressions are up-regulated in HCC. L-FABP gene may be involved in the carcinogenesis of human HCC. Expression of L-FABP is associated with VEGF expression, suggesting that L-FABP promotes the growth of blood vessels by taking up the fatty acids from the bloodstream, and both of them produce a marked effect on energy metabolism in HCC.
Mechanical Properties Of 7-10 Mm Bone Grafts And Small Slurry Grafts In Impaction Bone Grafting

Abstract:We compared the mechanical properties of morselized cancellous bone grafts of two sizes: 7-10 mm bone and small slurry bone (about 2 mm). The in vitro test was designed to simulate the hammer and impactor system for impaction bone grafting used in hip arthroplasty clinical practice. The 7-10 mm bone grafts showed higher height, elastic modulus, and massive extrusion strength than those of the small slurry bone grafts. No difference was found in yield strength. The bone mineral density of the 7-10 mm grafts continued to increase during impaction and became higher than that of the small slurry bone grafts after 10 impactions. Our results demonstrated that the small slurry bone grafts exhibit worse mechanical properties as compared with the 7-10 mm bone grafts, which implies that the use of this material in reconstruction of a bone defect in the acetabulum should be limited. Management of glenoid bone loss with impaction and structural bone grafting in reverse shoulder arthroplasty

Abstract:Introduction: Glenoid bone loss is a commonly encountered problem in complex primary and revision shoulder arthroplasty. Addressing glenoid bone loss is critical to avoid complications like early loosening, impingement, notching and instability. A large number of techniques like bone grafting using autograft or allograft, eccentric reaming, augmented base plates, patient-specific instrumentations and custom-made implants are available to tackle bone loss. Materials and methods: We prospectively collected the data of all patients with glenoid defects undergoing primary or revision reverse shoulder replacement between 2004 and 2017. This included demographic data, ranges of motion, Constant-Murley score and Subjective Shoulder Value (SSV). A pre-operative CT scan was done as well to plan the surgery and calculate the glenoid version. At each follow-up, the clinical function and shoulder scores were assessed. Additionally, the radiographs were assessed for graft incorporation, evidence of lysis and calculation of glenoid version. Results: Between 2004 and 2017, 37 patients underwent glenoid bone grafting during reverse shoulder arthroplasty. Average age was 72 years (range 46-88). Indications for surgery were cuff tear arthropathy (6 patients); revision of failed other prosthesis (23); primary osteoarthritis (4); rheumatoid arthritis (3); and second-stage revision for infection (1). The glenoid defect was contained in 24 patients, and therefore, impaction graft with a combination of bone graft substitute and/or humeral head autograft was performed. In 13 patients the glenoid defect was severe and uncontainable and therefore a graft-implant composite glenoid was implanted using humeral head autograft or allograft. Average follow-up was 3.6 years (range 1-10). Mean Constant score improved from 34 before surgery to 63 after surgery. Mean SSV score improved from 0.9/10 to 8.3/10. Active movements improved significantly with forward elevation increasing from 54° to 123°; abduction from 48° to 123°; external rotation from 24° to 38°; internal rotation from 57° to 70°. Radiographs at final follow-up showed no radiolucencies around the glenoid component and no evidence of loosening of the implant. In 2 cases there was a grade I notching. There was 100% survivorship at the last follow-up. Conclusion: Impaction bone grafting along with structural grafting when required is an effective and reproducible way of managing severe glenoid bone loss. This technique gives consistent and good clinical and radiological results.
Mechanical properties of cancellous bone from the acetabulum in relation to acetabular shell fixation and compared with the corresponding femoral head

Abstract:To gain initial stability for cementless fixation the acetabular components of a total hip replacement are press-fit into the acetabulum. Uneven stiffness of the acetabular bone will result in irregular deformation of the shell which may hinder insertion of the liner or lead to premature loosening. To investigate this, we removed bone cores from the ilium, ischium and pubis within each acetabulum and from selected sites in corresponding femoral heads from four cadavers for mechanical testing in unconfined compression. From a stress-relaxation test over 300 s, the residual stress, its percentage of the initial stress and the stress half-life were calculated. Maximum modulus, yield stress and energy to yield (resilience) were calculated from a load-displacement test. Acetabular bone had a modulus about 10-20%, yield stress about 25% and resilience about 40% of the values for the femoral head. The stress half-life was typically between 2-4 s and the residual stress was about 60% of peak stress in both acetabulum and femur. Pubic bone was mechanically the poorest. These results may explain uneven deformation of press-fit acetabular shells as they are inserted. The measured half-life of stress-relaxation indicates that waiting a few minutes between insertion of the shell and the liner may allow seating of a poorly congruent liner.

Abstract:Glenoid component loosening generates the greatest concern among surgeons performing total shoulder arthroplasty. Laboratory testing of glenoid prostheses may lead to improved design, subsequently leading to a reduction in the incidence of clinical loosening. The goals of this study were to develop a laboratory test method to address glenoid loosening and to apply this method to a variety of prosthesis designs. With use of a biaxial apparatus, glenoid components were cyclically subjected to superoinferior edge loading, mimicking the off-center rocking-horse phenomenon thought to contribute to glenoid loosening clinically. Before and after the rocking test was performed, compression and distraction of the superior and inferior edges were measured with the humeral head displaced to each edge. Rocking performance could not be predicted from initial measurements, indicating the necessity for dynamic loading to evaluate the likelihood of loosening. A roughened fixation surface for outperformed a smooth fixation surface, a curved backing showed almost half the distraction of a flat backing, and a nonconstrained prosthesis distracted less than a more constrained prosthesis.

Abstract:Objective: Our study aims to quantitatively determine the concavity of the glenoid articular surface in patients with hypermobile shoulders compared to those without. Method: We examined medical records of shoulder CTs from 2017 to 2022, selecting 50 patients with clinical signs of joint hypermobility for our case group and 54 for our control group. Two blinded readers independently assessed the glenoid morphology, calculating the glenoid concavity angle (GCA) and evaluating the articular surface shape as concave, flat, or convex. They also recorded the presence and severity of glenoid dysplasia. We compared these assessments between groups. Results: The mean GCA was significantly lower in the hypermobile group (2.3 ± 3.7° and 2.3 ± 3.8°) versus controls (6.6 ± 3.3° and 5.3 ± 3.8°) (P < 0.05). Interobserver reproducibility was high (ICC=0.76). A stark difference in glenoid morphology was noted between groups (P < 0.001), with a majority of hypermobile patients having a flat or convex glenoid. GCAs decreased with increasing shoulder laxity and dysplasia. GCA showed 77-81 % sensitivity and 55-82 % specificity for detecting shoulder hyperlaxity with a 4° cutoff. Conclusion: There is a significant association between GCA and shoulder hyperlaxity, demonstrating diagnostic efficacy and substantial interobserver agreement. Clinical relevance: GCA values lower than 4° warrant further clinical investigation for shoulder hyperlaxity and associated conditions, which is crucial for patient treatment planning.
Influence of glenosphere design and material on clinical outcomes of reverse total shoulder arthroplasty

Abstract:Background The aim of this multicentre retrospective study was to compare reverse total shoulder arthroplasty clinical outcomes with glenospheres of different diameters, designs and materials. Methods Between 2003 and 2008, 133 patients were divided into three groups: 60 (45%) with 36-mm standard CoCrMo (group A), 21 (16%) with 36-mm eccentric cobalt-chromium-molybdenum (CoCrMo) (group B) and 52 (39%) with 44-mm cross-linked ultra-high molecular weight polyethylene (X-UHMWPE) (group C) glenospheres. Mean (SD) follow-up was 38.7 (17.4) months. Clinical evaluation included Constant score and range of motion. Radiographic analysis included radiolucent lines, instability, loosening and assessment of scapular notching. Results Mean Constant score significantly increased for all groups (Wilcoxon test, p < 0.001). Group C allowed a higher and stable increase in range of motion. After 12 months and 24 months, groups C and B showed less pain than group A (Mann–Whitney U-test, p < 0.05). Group C had significantly lower scapular notching than group B (Mann–Whitney U-test, p = 0.001) and A (Mann–Whitney U-test, p = 0.009) at 12 months, 24 months and 36 months. Groups A and C presented 5 (8.3%) and 4 (7.6%) early complications, respectively. Conclusions The present study reported good results for all groups, although groups C and A presented better clinical outcomes, significantly lower notching and instability. A 44-mm X-UHMWPE glenosphere allowed a faster and more stable functional recovery, despite poorest pre-operative conditions. Additional long-term studies are needed to evaluate survivorship.

Background:The treatment of bony glenoid defects after anteroinferior shoulder dislocation currently depends on the amount of glenoid bone loss (GBL). Recent studies have described the glenoid concavity as an essential factor for glenohumeral stability. The role of glenoid concavity in the presence of soft tissue and muscle forces is still unknown.Hypothesis:Glenoid concavity would have a major impact on glenohumeral stability in an active-assisted biomechanical model including soft tissue and the rotator cuff's compression forces.Study Design:Controlled laboratory study.Methods:In 8 human shoulder specimens, individual coordinate systems were calculated based on anatomic landmarks. The glenoid concavity was measured biomechanically and based on computed tomography. Static load was applied to the rotator cuff tendons and the deltoid muscle. In a robotic test setup, anteriorly directed force was applied to the humeral head until translation of 5 mm (Nant) was achieved. Nant was used as a parameter indicating shoulder stability. This was performed in the following testing stages: (1) intact joint, (2) labral lesion, (3) 10% GBL, and (4) 20% GBL. The 8 specimens were divided equally into 2 subgroups (low concavity [LC] versus high concavity [HC]), with 4 specimens each, according to the previously measured concavity.Results:Anterior glenohumeral stability was highly correlated with the native glenoid concavity (R2 = 0.8). In the testing stages 1 to 3, we found a significantly higher mean stability in the HC subgroup compared with the LC subgroup (P≤ .0142). The HC subgroup still showed higher absolute Nant values with 20% GBL; however, there was no significant difference from the LC subgroup. The loss of stability in 20% GBL was correlated with the initial concavity (R2 = 0.86). Thus, a higher loss of Nant in the HC subgroup was observed (P = .0049).Conclusion:In an active-assisted model with intact soft tissue surrounding and muscular compression forces, the glenoid concavity correlates with shoulder stability. In bony defects, loss of concavity is an essential factor causing instability. Due to their significantly higher native stability, glenoids with HC can tolerate a higher amount of GBL.Clinical Relevance:Glenoid concavity should be considered in an individualized treatment of bony glenoid defects. Further studies are required to establish reference values and develop therapeutic algorithms.

Abstract:Purpose: Our purpose was to quantify stresses in the bone surrounding stemless implants in various configurations. Methods: A detailed finite element model of the glenohumeral joint was used to simulate abduction kinematics before and after arthroplasty and to measure bone stresses around the implants. Two digital patients were simulated: one healthy and one with supraspinatus muscle impairment (deficiency). Two anatomic total shoulder arthroplasty (TSA) configurations were placed in a 135° cutting plane. Two reverse shoulder arthroplasty (RSA) configurations with cutting angles of 135° and 145° were simulated with asymmetrical and symmetrical polyethylene cups, respectively, to obtain humeral neck-shaft angles of 145°. Results: Compared with preoperative models, TSA preserved and RSA restored abduction kinematics. The bone mechanical stresses were located mainly around the central stud of the TSA and were more peripheral to the RSA humeral components. The RSA configuration with the 145° cutting angle and symmetrical cup generated the lowest maximal bone stress and bone volume involvement. Stresses in the scapular cortical bone were highest in the supraspinatus fossa for TSA and the crest of the acromion for RSA. Conclusion: Early stability and glenohumeral bone stress change with implant configuration and should not be extrapolated from anatomic clinical data to reverse configurations. Material properties of femoral cancellous bone in axial loading. Part I: Time independent properties

Abstract:The time independent material behavior of cylindrical specimens obtained from the cancelous bone of 20 cadaveric human femora were determined. In this part of the publication, the nominal values for compressive strength, limits of elasticity (yield point), strain, elastic modulus and apparent density are being reported for the cancellous bone of the femoral head and condyle. The correlations between the various parameters are analysed. A positive linear correlation between the four parameters compressive stength, limit of elasticity, modulus of elasticity and apparent density could not be excluded. The material properties vary considerably both within one single bone and between individuals. Compressive strength, modulus of elasticity and apparent density found for cancellous bone of the femoral head are greater than those found in the condyles. Within the condyles, compressive strength, elastic modulus and apparent density increase from the proximal parts to the parts closer to the joint. The medial femoral condyle showed higher compressive strength than the lateral one. Relating each of the three other parameters to the apparent density of the individual specimen did not result in equalizing the data for the material properties. This indicates that the mechanical properties of cancellous bone are strongly related to the direction of loading.
Anatomic measurement of osseous parameters of the glenoid

Abstract:The angle and position of the scapular glenoid are important in shoulder mechanics, the interpretation of diseases, and planning shoulder replacement surgery. In total shoulder replacement, understanding the bony parameters of the glenoid is also of considerable guiding significance for designing implant size and improving material adaptability. To compare glenoid parameters measured from skeletal scapula specimens with those measured by 3D modeling of CT scanning images, analyze correlations between these data, and draw conclusions to guide clinical treatment of shoulder joint injury and total shoulder joint replacement. The data of manual and CT measurements from the same Chinese dry glenoid was compared. Three-dimensional measurement data were collected from the Japanese population and compared with the Chinese population data generated in this study. There were no significant differences between manual measurement and CT measurement in the inclination angle, glenopolar angle, anteroposterior transverse diameter, upper to lower vertical diameter, and depth of the glenoid (P = 0.288, 0.524, 0.111, 0.194, and 0.055, respectively). Further, there were no significant differences between Japanese and Chinese glenoid bones in the upper and lower vertical diameters or anteroposterior transverse diameters (P > 0.05). There were no significant differences between CT and manual measurements, suggesting that the CT method may provide measurements very close to the actual specimen size. This result, however, indicated that the measurer should be careful when measuring the depth of the glenoid.

Clinical Outcomes and Graft Resorption After Metal-Free Bone Block Suture Tape Cerclage Fixation for Recurrent Anterior Shoulder Instability: A Computed Tomography Analysis

Abstract:The American Journal of Sports Medicine, Ahead of Print. Background:Glenoid reconstruction with a bone block for anterior glenoid bone loss (GBL) has shown excellent outcomes. However, fixation techniques that require metal implants are associated with metal-related complications and bone graft resorption.Hypothesis:Arthroscopic glenoid reconstruction using a tricortical iliac crest bone graft (ICBG) and metal-free suture tape cerclage fixation can safely and effectively restore the glenoid surface area in patients with recurrent anterior shoulder instability and anterior GBL.Study Design:Case series; Level of evidence, 4.Methods:Adult patients (≥18 years) of both sexes with recurrent anterior shoulder instability and anterior GBL ≥15% were enrolled. These patients underwent arthroscopic glenoid reconstruction with ICBGs and metal-free suture tape cerclage fixation. The effectiveness and clinical outcomes with this technique were evaluated at 24 months using functional scores. Resorption of the graft articular surface was assessed by computed tomography, with the graft surface divided into 6 square areas aligned in 2 columns. Descriptive analysis was conducted.Results:A total of 23 consecutive patients met inclusion criteria (22 male, 1 female; mean age, 30.5 ± 7.9 years). The mean preoperative GBL was 19.7% ± 3.4%, and there were 15 allograft and 8 autograft ICBGs. All patients exhibited graft union at 3 months. The median follow-up was 38.5 months (interquartile range, 24-45 months). The Western Ontario Shoulder Instability Index, Rowe, Constant-Murley, and Subjective Shoulder Value scores improved from preoperatively (35.1%, 24.8, 83.1, and 30.9, respectively) to postoperatively (84.7%, 91.1, 96.0, and 90.9, respectively) (P < .001). No differences in clinical scores were observed between the graft types. One surgical wound infection was reported, and 2 patients (8.7% [95% CI, 2.4%-26.8%]) required a reoperation. The mean overall glenoid surface area increased from 80.3% ± 3.5% to 117.0% ± 8.3% immediately after surgery before subsequently reducing to 98.7% ± 6.2% and 95.0% ± 5.7% at 12 and 24 months, respectively (P < .001). The mean graft resorption rate was 18.1% ± 7.9% in the inner column and 80.3% ± 22.4% in the outer column. Additionally, 3 patients treated with an allograft (20.0% [95% CI, 7.1%-45.2%]), including the 2 with clinical failures, exhibited complete graft resorption at the last follow-up.Conclusion:Arthroscopic glenoid reconstruction using an ICBG and metal-free suture tape cerclage fixation was safe and effective, yielding excellent clinical outcomes. Resorption of the graft articular surface predominantly affected the nonloaded areas beyond the best-fit circle perimeter.

Abstract:ABSTRACT Cyber security has emerged as an established discipline for computer systems and infrastructures with a focus on protecting information stored on those systems from adversaries who want to obtain, damage, corrupt, modify, destroy, or prohibit access to it. Several information security techniques are available to protect information systems against unauthorized use, duplication, modification, destruction, and virus attacks. An Intrusion Detection System (IDS) is a program that analyzes what happens or has happened during an execution and finds indications that the computer has been misused. In this paper, we have proposed an effective IDS in which a local agent present in every node collects data from its own system and classifies anomaly behaviors using SVM classifier. The local agent uses the mobile agent to gather information from the neighboring node to check its integrity before it allows the system to send data to its neighboring node. The local agent is also capable of removing the local system from network if the system is found to be under attack, thereby providing a global secure environment. Our system identifies successful attacks from the anomaly behaviors.

Abstract:Glenoid implants used in anatomic total shoulder arthroplasties typically incorporate peripheral pegs as a design feature to support eccentric loads. These peripheral pegs and the implant-cement-bone interface undergo substantial cyclic tensile-compressive loads during normal activity. Therefore, these pegs are of interest in translating the micromechanics of local implant fixation failure to the biomechanics of gross anatomic failure of the glenoid implant after total shoulder arthroplasty. This study used an in vitro peg-cement-bone construct which incorporated bone tissue acquired from osteoarthritic patients undergoing total shoulder arthroplasty. Strain distributions in the peripheral peg-cement-bone interfaces were analyzed under loading conditions emulating glenoid implant edge displacements. It was found that tensile strains in the interfaces were highest near the backside-peg junction and were greater in magnitude than compressive strains. Notably, strains near the peg's fixation channels were relatively low. These results suggest that cracks may initiate around the peg near the backside and travel downward to cause broader fixation failure.
In-vitro evaluation of a polyurethane compliant-layer glenoid for use in shoulder arthroplasty

Abstract:A polyurethane glenoid component has been designed and manufactured as part of a total shoulder arthroplasty (TSA) system based on compliant-layer (CL) technology. Compared with conventional TSA designs, this biomimetic approach offers reduced friction and wear and potentially improved longevity. In-vitro evaluation of the glenoid system has included loosening and stability tests, and wear measurement using a specially constructed wear simulator. The results obtained support the hypothesis that a CL glenoid design may provide improved resistance to dynamic loosening and rim erosion, and demonstrate superior wear performance over a standard ultra-high molecular weight polyethylene design. This study not only confirms the feasibility of a CL glenoid component but also highlights the potential to increase implant longevity, thereby allowing earlier surgical intervention before poor glenoid bone stock and soft tissue compromise the outcome of TSA.

Abstract:BACKGROUND: A leading cause of anatomic total shoulder arthroplasty failure continues to be glenoid component loosening. Historically, metal-backed glenoids (MBG) have shown decreased performance compared to polyethylene-backed glenoid (PEG) components. Previous studies have shown that MBG components have higher revision rates and shorter time to prosthetic failure compared with PEG components. However, the negative outcomes of many of these studies may have been linked to an examination of older generations of MBG implants, and do not account for newer technology. Thus, our study aims to assess the short-term radiographic and clinical outcomes of patients treated with anatomic total shoulder arthroplasty using the current and more modern generation of implants with a universal metal-backed glenoid component.METHODS: We conducted a retrospective chart review of patients who had their primary anatomic shoulder replacement performed from 2017 to 2022 by a single surgeon, and used one of two currently available implants that incorporated a metal-backed glenoid component with a convertible baseplate. After the exclusion of those lost to follow-up, we included 26 patients in this study, 3 of whom underwent bilateral shoulder replacement surgery at staggered time intervals. The primary outcome measure was evidence of radiographic change via in-office X-ray. Analysis of radiographic changes was conducted for each postoperative visit, which was aimed for 2 weeks, 6 weeks, 12 weeks, 1 year, and annually thereafter. Secondary analysis tracked clinical progression and utilized DASH scores, range of motion evaluation, strength testing, and instability examination using evidence of apprehension and sulcus signs at each time interval.RESULTS: In our study, radiographic changes at each time interval and the longest follow-up appointment demonstrated no evidence of hardware loosening of either the glenoid or the humeral component and no evidence of periprosthetic fracture or hardware failure of any kind for MBG implants. There were no revisions performed due to polyethylene wear. The overall average range of motion, strength, and DASH scores were improved at each postoperative visit.CONCLUSION: Previous studies have suggested that TSA implants with MBG components may be inferior to PEG implants. However, the results of many of these studies may not be as representative of the current implant designs available today. While additional studies are necessary to assess the long-term survivability of the implant and its effects on glenoid bone stock, our findings suggest that university MBG components have minimal complications, do not demonstrate loosening, and have acceptable outcomes in short-term follow-up.

Background:The presence of glenoid bone defects is indicative in the choice of treatment for patients with anterior shoulder instability. In contrast to traditional linear- and area-based measurements, techniques such as the consideration of glenoid concavity have been proposed and validated.Purpose:To compare the reliability of linear (1-dimensional [1D]), area (2-dimensional [2D]), and concavity (3-dimensional [3D]) measurements to quantify glenoid bone loss performed manually and to analyze how automated measurements affect reliability.Study Design:Cohort study (diagnosis); Level of evidence, 3.Methods:Computed tomography images of 100 patients treated for anterior shoulder instability with differently sized glenoid defects were evaluated independently by 2 orthopaedic surgeons manually using conventional software (OsiriX; Pixmeo) as well as automatically with a dedicated prototype software program (ImFusion Suite; ImFusion). Parameters obtained included 1D (defect diameter, best-fit circle diameter), 2D (defect area, best-fit circle area), and 3D (bony shoulder stability ratio) measurements. Mean values and reliability as expressed by the intraclass correlation coefficient [ICC]) were compared between the manual and automated measurements.Results:When manually obtained, the measurements showed almost perfect agreement for 1D parameters (ICC = 0.83), substantial agreement for 2D parameters (ICC = 0.79), and moderate agreement for the 3D parameter (ICC = 0.48). When measurements were aided by automated software, the agreement between raters was almost perfect for all parameters (ICC = 0.90 for 1D, 2D, and 3D). There was a significant difference in mean values between manually versus automatically obtained measurements for 1D, 2D, and 3D parameters (P < .001 for all).Conclusion:While more advanced measurement techniques that take glenoid concavity into account are more accurate in determining the biomechanical relevance of glenoid bone loss, our study showed that the reliability of manually performed, more complex measurements was moderate.

Effects of Sound on the Marine Environment

Abstract:Abstract : The long-term goals are to develop novel techniques to measure and predict, through modeling, the effect of sound on the marine environment. Modeling includes acoustic sources, propagation and the interaction of sound with animal behavior models. Determining the necessary environmental information such as bathymetry, sound speed and seabed properties for accurate modeling is also an essential component of this work. The objective of this research is to develop modeling tools for estimating the impact of sound on marine life. The goal is to provide state-of-the-art, open source codes to model sound sources, sound propagation and animal behavior. We are also assembling open source environmental databases for quantities such as seabed properties, bathymetry and ocean sound speed. Together, these tools will provide the best estimate of the impact of various sonar systems on the marine environment. These tools are bundled with a simple user interface in the One Navy Model (ONM) for Acoustic Effects Software (formerly known as the Effect of Sound on the Marine Environment (ESME) Workbench) which is intended to be a type of gold standard for estimating the impact of anthropogenic sound on marine mammals. Currently, Navy environmental impact statements are prepared at the Naval Undersea Warfare Center (NUWC) and by several government contractors. The software and databases being used are often either classified or proprietary. ONR has put together a team consisting Boston University (David Mountain), Biomimetica (Dorian Houser), HLS Research (Michael Porter) and Portland State University (Martin Siderius) to build the ESME Workbench, which will make the needed calculations for assessing environmental impact without using classified or proprietary components.

Abstract:Abstract Prior to shoulder replacement, the quality of cancellous bone stock of humerus head should be evaluated. Thermogravimetric analysis (TGA) has already been utilized to assess thermal stability of cancellous bone mineral, collected from the femoral head of patients with osteoarthritis and osteoporosis. Our workgroup has recently examined the thermal parameters of rotator cuff of patients undergoing reversed shoulder replacement. We hypothesized that TGA of humerus head would indicate difference in the bone quality of orthopedic and trauma patients. We also hypothesized that the calorimetric data could correlate with the grade of degenerative changes. Cylindrical subchondral humeral head samples were collected from patients subjected to reversed shoulder replacement due to orthopedic or trauma indications. Then, calorimetric parameters were measured using DTA/TG analysis. Radiological evaluation was also performed to classify the grade of osteoarthritis. In case of orthopedic samples, the calorimetric parameters indicated a moderate to severe degree of bone damage and loss of mineralization, because of the progressed osteoarthritis. Meanwhile, the trauma samples exhibited only moderate or minimal subchondral bone degeneration. DTA curves showed different patterns and indicated shifts in transition temperatures, comparing control and pathologic samples. In addition, correlation was found between the degree of osteoarthritis and calorimetric enthalpy. DTA/TG analysis of humerus head samples indicated marked differences in bone quality of orthopedic and trauma patients. Further investigation is needed to differentiate the calorimetric parameters of different layers of subchondral bone.
