Abstract:Orthopaedic Journal of Sports Medicine, Volume 12, Issue 2, February 2024. Background:The presence of glenoid bone defects is indicative in the choice of treatment for patients with anterior shoulder instability. In contrast to traditional linear- and area-based measurements, techniques such as the consideration of glenoid concavity have been proposed and validated.Purpose:To compare the reliability of linear (1-dimensional [1D]), area (2-dimensional [2D]), and concavity (3-dimensional [3D]) measurements to quantify glenoid bone loss performed manually and to analyze how automated measurements affect reliability.Study Design:Cohort study (diagnosis); Level of evidence, 3.Methods:Computed tomography images of 100 patients treated for anterior shoulder instability with differently sized glenoid defects were evaluated independently by 2 orthopaedic surgeons manually using conventional software (OsiriX; Pixmeo) as well as automatically with a dedicated prototype software program (ImFusion Suite; ImFusion). Parameters obtained included 1D (defect diameter, best-fit circle diameter), 2D (defect area, best-fit circle area), and 3D (bony shoulder stability ratio) measurements. Mean values and reliability as expressed by the intraclass correlation coefficient [ICC]) were compared between the manual and automated measurements.Results:When manually obtained, the measurements showed almost perfect agreement for 1D parameters (ICC = 0.83), substantial agreement for 2D parameters (ICC = 0.79), and moderate agreement for the 3D parameter (ICC = 0.48). When measurements were aided by automated software, the agreement between raters was almost perfect for all parameters (ICC = 0.90 for 1D, 2D, and 3D). There was a significant difference in mean values between manually versus automatically obtained measurements for 1D, 2D, and 3D parameters (P < .001 for all).Conclusion:While more advanced measurement techniques that take glenoid concavity into account are more accurate in determining the biomechanical relevance of glenoid bone loss, our study showed that the reliability of manually performed, more complex measurements was moderate.

Abstract:Purpose Instability of the glenohumeral joint remains a complex clinical issue with high rates of surgical failure and significant morbidity. Advances in specific radiologic measurements involving the glenoid and the humerus have provided insight into glenohumeral pathology, which can be corrected surgically towards improving patient outcomes. The contributions of capsular pathology to ongoing instability remain unclear. The purpose of this study is to provide a systematic review of existing glenohumeral capsular measurement techniques published in the last 15 years. Methods A systematic review of multiple databases was performed following PRISMA guidelines for all primary research articles between 2008 and 2023 with quantitative measurements of the glenohumeral capsule in patients with instability, including anterior, posterior and multi‐directional instability. Results There were a total of 14 articles meeting the inclusion criteria. High variability in measurement methodology across studies was observed, including variable amounts of intra‐articular contrast, heterogeneity among magnetic resonance sequence acquisitions, differences in measurements performed and the specific approach taken to compute each measurement. Conclusion There is a need for standardization of methods in the measurement of glenohumeral capsular pathology in the setting of glenohumeral instability to allow for cross‐study analysis. Abstract:Introduction Glenoid placement is critical for successful outcomes in total shoulder arthroplasty (TSA). Preoperative templating with three-dimensional (3D) imaging has improved implant positioning, but deviations from the planned inclination and version still occur. Mixed-Reality (MR) is a novel technology that allows surgeons intra-operative access to 3D imaging and templates, capable of overlaying the surgical field to help guide component positioning. The purpose of this study was to compare the execution of preoperative templates using MR versus standard instruments (SI). Methods Retrospective review of 97 TSAs (18 anatomic, 79 reverse) from a single high-volume shoulder surgeon between January 2021 and February 2023, including only primary diagnoses of osteoarthritis, rotator cuff arthropathy, or a massive irreparable rotator cuff tear. To be included, patients needed a templated preoperative plan and then a postoperative computed tomography scan. Allocation to MR vs. SI was based on availability of the MR headset, industry technical personnel, and the templated preoperative plan loaded into the software, but preoperative or intraoperative patient factors did not contribute to the allocation decision. Postoperative inclination and version were measured by two independent, blinded physicians and compared to the preoperative template. From these measurements, we calculated the mean difference, standard deviation (SD), and variance to compare MR and SI. Results Comparing 25 MR to 72 SI cases, MR significantly improved both inclination (p<0.001) and version (p<0.001). Specifically, MR improved the mean difference from preoperative templates (by 1.9o inclination, 2.4o version), narrowed the SD (by 1.7 o inclination, 1.8 o version), and decreased the variance (11.7 to 3.0 inclination, 14.9 to 4.3 version). A scatterplot of the data demonstrates a concentration of MR cases within 5° of plan relative to SI cases typically within 10° of plan. There was no difference in operative time. Conclusion MR improved the accuracy and precision of glenoid positioning. Although it is unlikely that 2 o makes a detectable clinical difference, our results demonstrate the potential ability for technology like MR to narrow the bell curve and decrease the outliers in glenoid placement. This will be particularly relevant as MR and other similar technologies continue to evolve into more effective methods in guiding surgical execution.

Abstract:Acute stimulation of cardiac beta1-adrenergic receptors (beta1ARs) by norepinephrine represents the strongest endogenous mechanism for increasing cardiac function, but long-term stimulation induces cardiomyocyte apoptosis and contributes to cardiac disease. These effects have been attributed to coupling of the beta1AR to the stimulatory G protein (Gs) and classical cAMP-mediated signaling. In this issue of the JCI, Noma and colleagues report that cardiomyocyte beta1ARs may in addition deliver an antiapoptotic signal through transactivation of EGFRs (see the related article beginning on page 2445). Their findings provide a perspective for a novel class of receptor ligands that may direct beta1AR signaling toward alternative signaling pathways.
Management of glenoid bone loss with impaction and structural bone grafting in reverse shoulder arthroplasty

Abstract:Introduction: Glenoid bone loss is a commonly encountered problem in complex primary and revision shoulder arthroplasty. Addressing glenoid bone loss is critical to avoid complications like early loosening, impingement, notching and instability. A large number of techniques like bone grafting using autograft or allograft, eccentric reaming, augmented base plates, patient-specific instrumentations and custom-made implants are available to tackle bone loss. Materials and methods: We prospectively collected the data of all patients with glenoid defects undergoing primary or revision reverse shoulder replacement between 2004 and 2017. This included demographic data, ranges of motion, Constant-Murley score and Subjective Shoulder Value (SSV). A pre-operative CT scan was done as well to plan the surgery and calculate the glenoid version. At each follow-up, the clinical function and shoulder scores were assessed. Additionally, the radiographs were assessed for graft incorporation, evidence of lysis and calculation of glenoid version. Results: Between 2004 and 2017, 37 patients underwent glenoid bone grafting during reverse shoulder arthroplasty. Average age was 72 years (range 46-88). Indications for surgery were cuff tear arthropathy (6 patients); revision of failed other prosthesis (23); primary osteoarthritis (4); rheumatoid arthritis (3); and second-stage revision for infection (1). The glenoid defect was contained in 24 patients, and therefore, impaction graft with a combination of bone graft substitute and/or humeral head autograft was performed. In 13 patients the glenoid defect was severe and uncontainable and therefore a graft-implant composite glenoid was implanted using humeral head autograft or allograft. Average follow-up was 3.6 years (range 1-10). Mean Constant score improved from 34 before surgery to 63 after surgery. Mean SSV score improved from 0.9/10 to 8.3/10. Active movements improved significantly with forward elevation increasing from 54° to 123°; abduction from 48° to 123°; external rotation from 24° to 38°; internal rotation from 57° to 70°. Radiographs at final follow-up showed no radiolucencies around the glenoid component and no evidence of loosening of the implant. In 2 cases there was a grade I notching. There was 100% survivorship at the last follow-up. Conclusion: Impaction bone grafting along with structural grafting when required is an effective and reproducible way of managing severe glenoid bone loss. This technique gives consistent and good clinical and radiological results.

Abstract:Purpose The purpose of the present scoping review is to identify sources of heterogeneity in reporting domains that have the potential to improve surgical decision-making in reverse shoulder arthroplasty associated with glenoid bone grafting. Methods A scoping review was conducted according to the JBI and PRISMA ScR guidelines. Articles covering glenoid bone grafting at the time of reverse shoulder replacement, published between 2012 and 2022 in MEDLINE, Scopus, Epistemonikos, Web of Science, and Cochrane Database of Systematic Reviews, were searched. Reporting of core outcome measures, as well as prosthesis-related variables, rehabilitation protocols, radiographic evaluation methods, and bone graft incorporation assessments, were collected. Results For the final analysis, a total of 14 articles were considered. There were 649 operated shoulders with a mean patient age of 72 years, ranging from 48 to 85 years. Analysis of preoperative ranges of motion and patient-reported outcome measures revealed increased heterogeneity in patient selection between studies. Reporting of implant-related parameters was the most inconsistent. Rehabilitation protocols were scarcely reported. Assessment of graft incorporation and radiographic evaluation of choice revealed that there is no consistent method or measurement and the clinical relevance of radiolucent lines at the graft-glenoid bone interface is unclear. Conclusion Addressing implant-related reporting gaps has the highest potential to improve surgical decision-making and provide orthopedic surgeons with a more comprehensive understanding of published results on glenoid bone grafting at the time of primary reverse shoulder arthroplasty.

Abstract:Background Primary gleno-humeral osteoarthritis is associated with both excessive posterior humeral subluxation (PHS) and excessive glenoid retroversion in 40% of cases. These morphometric abnormalities are a particular issue since they may be responsible for a deterioration in long term clinical and radiological outcomes. The aim of this study was to perform a CT-analysis of patients who underwent Total Shoulder Arthroplasty (TSA) for primary osteoarthritis (OA) with B2-, B3- or C-type glenoids in whom an attempt was made to correct for excessive glenoid retroversion and excessive posterior humeral subluxation intraoperatively. Material We performed a retrospective, single-center study including 62 TSA patients with a pre-operative PHS of the glenohumeral joint (31 men, 31 women, 70±9 years) between January 2000 and January 2014. Glenoids were classified as B2 (32 cases), B3 (13 cases) or C (17 cases). Glenoid retroversion was corrected by anterior asymmetric reaming. Patients were reviewed for clinical and CT-scan assessment with a mean follow-up of 8.3 years (minimum 5 years). At final follow-up the CT-images were reconstructed in a scapular plan. A PHS index of >65% defined persistence. Results The revision free rate was estimated at 93%. Correlation between PHS and retroversion was moderate preoperatively (ρ=0.58) and strong at final follow-up (ρ=0.73). Postoperative CT-scans on average showed a surgical correction of PHS compared to preoperatively (79% vs 65% respectively, p<0.05) and retroversion (20° vs 10° respectively, p<0.05). At final follow-up, 25/62 patients had a persistence in 2D model and 41/62 in 2D corrected model. Persistence of PHS had no influence on clinical outcomes but did demonstrate a significantly higher glenoid loosening rate (20% vs 59%, p<0.05). Conclusion Correlation between PHS and retroversion was moderate preoperatively and strengthened at long-term follow up. Anterior asymmetric reaming allowed for a surgical improvement of both PHS and retroversion, but it was not sufficient to maintain a correction over time. Glenoid loosening was more frequent in case of PHS persistence but seemingly without clinical relevance.

Abstract:Introduction: Optimal management of retroversion in anatomic total shoulder arthroplasty (aTSA) remains controversial and limited attention has been directed to the impact of glenoid inclination. Prior biomechanical study suggest that residual glenoid inclination generates shear stresses that may lead to early glenoid loosening. Combined biplanar glenoid deformities may complicate anatomic glenoid reconstruction and affect outcomes. The goal of this matched-cohort analysis was to assess the relationship between biplanar deformities and mid-term radiographic loosening in aTSA. Methods: The study cohort was identified via an institutional repository of 337 preoperative CT scans from 2010-2017. Glenoid retroversion, inclination, and humeral head subluxation were assessed via 3D-planning software. Patients with retroversion ≥ 20˚ and inclination ≥ 10˚ who underwent aTSA with eccentric reaming and non-augmented components were matched by age, sex, retroversion, and Walch classification to patients with retroversion ≥ 20˚ only. Primary outcome was glenoid component Lazarus radiolucency score. Results: Twenty-eight study subjects were matched to 28 controls with retroversion only. No difference in age (61.3 vs. 63.6 years, p=0.26), sex (19 [68%] vs. 19 [68%] male, p=1.0), or follow-up (6.1 vs. 6.4 years, p=0.59). Biplanar deformities had greater inclination (14.5˚ versus 5.3˚, p<0.001), retroversion (30.0˚ versus 25.6˚, p=0.01) and humeral subluxation (86.3% versus 82.1%, p=0.03). Biplanar patients had greater postoperative implant superior inclination (5.9 [4.6] vs. 3.0 [3.6] degrees, p=0.01) but similar rate of complete seating 24 [86%] vs. 24 [86%] p=1.0). At final follow-up, biplanar subjects had higher Lazarus radiolucent scores (2.4 [1.7] vs. 1.6 [1.1], p=0.03) and higher proportion of patients with glenoid radiolucency (19 [68%] vs. 11 [39%], p=0.03). No difference in complete component seating (86% versus 86%, p=0.47) or initial radiolucency grade (0.21 versus 0.29, p=0.55) on immediate postop radiographs. Biplanar patients demonstrated a greater amount of posterior subluxation at immediate postop(3.5% [1.3%] versus 1.8% [0.6%]; p=0.03) and final follow-up (7.6% [2.8%] versus 4.0% [1.8%]; p=0.04). At final radiographic follow-up, biplanar subjects had higher Lazarus radiolucent scores (2.4 [1.7] vs. 1.6 [1.1], p=0.03; ICC=0.82). Bivariate regression analysis demonstrated biplanar deformity was the only significant predictor (OR 3.3, p=0.04) of glenoid radiolucency. Conclusion: Biplanar glenoid deformity resulted in time-zero glenoid implant superior inclination and increased mid-term radiographic loosening and posterior subluxation. Attention to glenoid inclination is important for successful anatomical glenoid reconstruction. Future research is warranted to understand the long-term implications of these findings and impact of utilizing augmented implants or reverse shoulder arthroplasty to manage biplanar deformities.

Abstract:Background: Glenoid component positioning is an important and challenging aspect of total shoulder arthroplasty. The use of freehand technique with standard instrumentation or preoperative planning based on 2-dimensional computed tomography (CT) scans provides an opportunity for improvement in terms of component accuracy, precision, and deformity correction. These techniques have produced varying outcomes. Methods: Preoperative planning software (PPS), patient specific instrumentation (PSI), and intraoperative navigation (NAV) have been developed to improve the accuracy of implant placement and deformity correction with the ultimate goals of improved patient outcomes and implant longevity. Literature search was conducted on published and available studies comparing the accuracy of glenoid component placement and improvements in surgical and patient outcomes amongst the aforementioned techniques. Results: PPS, PSI, and NAV have demonstrated improved accuracy over freehand techniques with standard instrumentation. However, data demonstrating the clinical benefit and cost effectiveness of these new technologies are lacking. Discussion: In this paper, we reviewed the evidence available to answer the question of whether or not advanced shoulder arthroplasty technologies have been beneficial and reviewed future technologies in development such as virtual/mixed-reality and robotic assisted shoulder surgery. Measurement of acetabular version based on biplanar radiographs with 3D reconstructions in comparison to CT as reference standard in cadavers

Abstract:To quantify acetabular version using 3 D reconstructions based on biplanar radiographs (BPR) with CT as reference standard. No institutional review board approval was needed. Nine dry-bone pelvises underwent BPR in five different positions (rotation/tilt). The 3 D models of each pelvis were reconstructed by two radiologists on the basis of anatomical landmarks using semi-automated software. Automated software was used to assess the 3 D models and to calculate acetabular versions perpendicular to the anterior pelvic plane on all levels in the craniocaudal direction in 1 mm steps. Transverse CT images perpendicular to the anterior pelvic plain were reconstructed through the acetabulum in 1 mm steps. Both readers measured acetabular version on each image. Inter-reader agreement was calculated. Measurements based on BPR and CT were compared. Inter-reader agreement was almost perfect for BPR-based acetabular version measurements (ICC (intraclass correlation coefficient) = 0.920, P < 0.0005) and CT (ICC = 0.990, P < 0.0005). Correlation of acetabular versions between the five BPR-positions was substantial/almost perfect (ICC = 0.722-0.887 and 0.749-0.872 for readers 1 and 2, respectively; most P < 0.0005). The acetabular version measurements between the ap-positioning from BPR and CT showed moderate agreement (mean CCC (concordance correlation coefficient) = 0.733 for reader 1, CCC = 0.755 for reader 2). Acetabular version on multiple levels can be measured using BPR and dedicated post-processing software and is relatively independent of pelvic rotation and tilt. Clin. Effects of Sound on the Marine Environment

Abstract:Abstract : The long-term goals are to develop novel techniques to measure and predict, through modeling, the effect of sound on the marine environment. Modeling includes acoustic sources, propagation and the interaction of sound with animal behavior models. Determining the necessary environmental information such as bathymetry, sound speed and seabed properties for accurate modeling is also an essential component of this work. The objective of this research is to develop modeling tools for estimating the impact of sound on marine life. The goal is to provide state-of-the-art, open source codes to model sound sources, sound propagation and animal behavior. We are also assembling open source environmental databases for quantities such as seabed properties, bathymetry and ocean sound speed. Together, these tools will provide the best estimate of the impact of various sonar systems on the marine environment. These tools are bundled with a simple user interface in the One Navy Model (ONM) for Acoustic Effects Software (formerly known as the Effect of Sound on the Marine Environment (ESME) Workbench) which is intended to be a type of gold standard for estimating the impact of anthropogenic sound on marine mammals. Currently, Navy environmental impact statements are prepared at the Naval Undersea Warfare Center (NUWC) and by several government contractors. The software and databases being used are often either classified or proprietary. ONR has put together a team consisting Boston University (David Mountain), Biomimetica (Dorian Houser), HLS Research (Michael Porter) and Portland State University (Martin Siderius) to build the ESME Workbench, which will make the needed calculations for assessing environmental impact without using classified or proprietary components.

Abstract:Background Optimal glenosphere positioning in a lateralized reverse shoulder arthroplasty (RSA) to maximize functional outcomes has yet to be clearly defined. Center of rotation (COR) measurements have largely relied on AP radiographs which allow assessment of lateralization and inferior position, but ignore scapular Y radiographs which may provide an assessment of posterior and inferior position relative to the acromion. The purpose of this study was to evaluate the COR in the sagittal plane and assess the effect of glenosphere positioning with functional outcomes utilizing a 135° inlay stem with a lateralized glenoid. Methods A retrospective review was performed on a prospectively maintained multicenter database on patients who underwent primary RSA from 2015-2021 with a 135° inlay stem. The COR was measured on minimum 2-year postoperative sagittal plain radiographs using a perfect-circle fit method. A perfect circle was made on the glenosphere and the center was marked. From there, four measurements were made: 1) center to the inner cortex of the coracoid, 2) center to the inner cortex of the anterior acromion, 3) center to the inner cortex of the middle acromion, 4) center to the inner cortex of the posterior acromion. Regression analysis was performed to evaluate any association between the position of the COR relative to bony landmarks with functional outcomes. Results A total of 136 RSAs met the study criteria. There was no relation with any of the distances with outcome scores (ASES, VAS). In regards to range of motion (ROM), each distance had an effect on at least one parameter. The COR to coracoid distance had the broadest association with ROM with improvements in forward flexion (FF), external rotation (ER0), and internal rotation with arm at 90° (IR90) ( p = <0.001, 0.031, <0.001; respectively). The COR to coracoid distance was also the only distance to affect the final FF and IR90. For every 1 mm increase in this distance, there was a 1.8° increase in FF and 1.5° increase in IR90 (ß = 1.78; 95% CI 0.85 – 2.72, p = <0.001, ß = 1.53; 95% CI 0.65 – 2.41, p = <0.001; respectively). Conclusion Evaluating the COR following RSA in the sagittal plane suggests that posteroinferior glenosphere position may improve ROM when using a 135° inlay humeral component and a lateralized glenoid.

Abstract:Background: Preoperative planning for anatomic total shoulder arthroplasty (aTSA) and reverse total shoulder arthroplasty (rTSA) is becoming increasingly common. While preoperative planning allows surgeons to determine individualized implant types, utilization of intraoperative navigation improves the accuracy of implant placement and may increase confidence in the preoperative plan. The purpose of this study was to evaluate and compare the rate at which surgeons use a glenoid implant different than their preoperative plan with and without the use of computer navigation. Methods: A retrospective review of a multicenter prospectively collected shoulder arthroplasty database was conducted between 2016 and 2022. Inclusion criteria were primary aTSA or rTSA with an available preoperative plan and record of the actual implant used. Change in glenoid implant was defined as a deviation in the final implant from the preoperative plan in regard to backside shape (nonaugmented vs augment or differing augment shape). Results: We included 1,915 shoulder arthroplasties (525 aTSA, 1,390 rTSA) performed with preoperative planning and intraoperative navigation and 110 shoulder athroplasties (37 aTSA, 73 rTSA) performed with preoperative planning alone. Overall, the final glenoid implant deviated from the preoperative plan less frequently when intraoperative navigation was used compared with preoperative planning alone (1.9% [n = 36] versus 7.3% [n = 8], P = 0.002). When stratified by procedure, deviation from the preoperative plan occurred significantly less for rTSA when preoperative planning was used with intraoperative navigation versus planning alone (2% [n = 29] versus 11% [n = 8], P < 0.001; OR = 0.17 [95% CI = 0.07 to 0.46]), but not aTSA (1% [n = 7] versus 0% [n = 0], P = 1). Use of intraoperative navigation was independently associated with lower odds of deviation from the preoperative plan on multivariable logistic regression (OR = 0.25 [95% CI = 0.11 to 0.56], P = 0.001). Conclusion: Use of intraoperative navigation is associated with increased adherence to the preoperative plan for primary rTSA. Use of navigation may increase surgeon confidence despite known limitations of glenoid visualization during this procedure. This may offer advantages in outpatient surgery centers and smaller hospitals where inventory space may be limited. Level of evidence: III, retrospective cohort study.

Abstract:Objective To establish the standardized anterior view of the glenoid cavity of the shoulder using 3D reconstruction of 64-slice spiral CT scans, and provide radiological features for correct evaluation of glenoid cavity defect in patients with recurrent anterior shoulder dislocation. Methods The glenoid cavity of 70 adult shoulders from 35 healthy volunteers were scanned using 64-slice spiral CT. 3D images of the glenoid cavity (humeral head removed image) were reconstructed on the 3D workstation. On three multiplane reconstruction images and one 3D volume reconstruction image, the color reference lines were adjusted to render a standardized anterior view image of the glenoid cavity. To verify reproducibility of the technique, three radiologists were asked to use the above-mentioned technique to acquire standardized 3D anterior view and coordinates of the glenoid cavity from the workstation and output the data in 2D format. AutoCAD software was applied for analysis. Results Measurements taken from the anterior view images of the glenoid cavity acquired by the three radiologists from the 3D workstation were: the distance between the most anterior protrusion and most posterior protrusion of the glenoid cavity was (28.1 ± 3.3) mm, and the distance between the upper protrusion and lower protrusion was (38.7 ± 3.7) mm. There were no significant differences among the data obtained by the three radiologists (P > 0.05). Conclusion Our method of acquiring standardized anterior view of the glenoid cavity from the 3D workstation is easy, practical and reproducible. Standardizing anterior views of the glenoid cavity is clinically significant in that it facilitates the precise assessment of bone defect of the glenoid cavity.

Abstract:The angle and position of the scapular glenoid are important in shoulder mechanics, the interpretation of diseases, and planning shoulder replacement surgery. In total shoulder replacement, understanding the bony parameters of the glenoid is also of considerable guiding significance for designing implant size and improving material adaptability. To compare glenoid parameters measured from skeletal scapula specimens with those measured by 3D modeling of CT scanning images, analyze correlations between these data, and draw conclusions to guide clinical treatment of shoulder joint injury and total shoulder joint replacement. The data of manual and CT measurements from the same Chinese dry glenoid was compared. Three-dimensional measurement data were collected from the Japanese population and compared with the Chinese population data generated in this study. There were no significant differences between manual measurement and CT measurement in the inclination angle, glenopolar angle, anteroposterior transverse diameter, upper to lower vertical diameter, and depth of the glenoid (P = 0.288, 0.524, 0.111, 0.194, and 0.055, respectively). Further, there were no significant differences between Japanese and Chinese glenoid bones in the upper and lower vertical diameters or anteroposterior transverse diameters (P > 0.05). There were no significant differences between CT and manual measurements, suggesting that the CT method may provide measurements very close to the actual specimen size. This result, however, indicated that the measurer should be careful when measuring the depth of the glenoid.

Abstract:Purpose: Accurate glenoid component placement in total shoulder arthroplasty (TSA) remains challenging even with preoperative planning, especially for variable glenoid erosion patterns in the coronal plane. Methods: We retrospectively reviewed 170 primary TSAs in which preoperative planning software was used. After registration of intraoperative bony landmarks, surgeons were blinded to the navigation screen and attempted to implement their plan by simulating placement of a central-axis guide pin: 230 screenshots of simulated guide pin placement were included (aTSA = 66, rTSA = 164). Displacement, error in version and inclination, and overall malposition from the preoperatively-planned target point were stratified by the Favard classification describing superior-inferior glenoid wear: E0 (n = 89); E1 (n = 81); E2 (n = 29); E3(n = 29); E4(n = 2). Malposition was considered > 10° for version/inclination errors or > 4 mm displacement from the starting point. Results: Mean displacement error was 3.5 ± 2.7 mm (aTSA = 2.7 ± 2.3 mm, rTSA = 3.8 ± 2.9 mm), version error was 5.7 ± 4.7° (aTSA = 5.8 ± 4.4°, rTSA = 5.7 ± 4.8°), inclination error was 7.1 ± 5.6 (aTSA = 4.8 ± 4.8°, rTSA = 8.1 ± 5.7°), and malposition rate was 53% (aTSA = 38%, rTSA = 59%). When compared by Favard classification, there were no differences in any measure; when stratified by TSA type, version error differed for rTSAs (P = .038), with E1 having the greatest version error (6.9 ± 5.2°) and E3 the least (4.2 ± 3.4°). When comparing glenoids without wear (E0) and glenoids with superior wear (E2 and E3), the only difference was greater version error in glenoids without wear (6.0 ± 4.9° vs. 4.6 ± 3.7°, P = .041). Conclusions: Glenoid malposition did not differ based on coronal glenoid morphology. Although, malposition was relatively high, suggesting surgeons should consider alternate techniques beyond preoperative planning and standard instrumentation in TSA. Abstract:BACKGROUND: Descriptions of glenoid deformities in glenohumeral osteoarthritis (GHOA) have focused on the axial plane. Less is known regarding arthritic glenoids with higher amounts of superior inclination and little evidence exists to guide management of inclination or combined version-inclination deformity when performing anatomic total shoulder arthroplasty (aTSA). We hypothesized that biplanar deformities (BD) would be present in a higher proportion of GHOA patients than previously appreciated, and these deformities would be difficult to adequately reconstruct with contemporary aTSA implants.METHODS: A retrospective query was performed of GHOA patients indicated for TSA 2012-2017 with a computed tomography (CT) scan within three months of surgery. Images were uploaded to three-dimensional software for automated measurements. Glenoids with superior inclination ≥10˚, retroversion ≥20˚ were considered to have BD. Walch classification was determined, C-type glenoids were excluded. Rotator-cuff muscle cross-sectional area (CSA) was measured and fatty infiltration graded. Glenoids with BD were virtually planned for aTSA with correction to neutral inclination and version, then 5˚ superior inclination and 10˚ retroversion.RESULTS: Two-hundred sixty-eight shoulders in 250 patients were included; average age 65 years, 67% male. There were no differences in inclination between Walch types (p=0.25). Twenty-nine shoulders with BD were identified (11%). These deformities were not associated with age (p=0.47) or gender (p=0.50) but were skewed towards Walch B-type, specifically B2 (p=0.03). Acromial index and posterior humeral head subluxation were higher in BD patients (p=0.04, p<0.001, respectively). Biplanar deformities had similar cuff CSA compared to those without but were less frequently associated with fatty infiltration of the subscapularis (p=0.05). When correcting to neutral version and inclination, 41% BD could not be reconstructed. Of those that could, 94% required augmented implants. When correcting to 5˚ superior inclination and 10˚ retroversion, 10% could not be reconstructed. Of those that could, 58% required augmented implants. With partial correction, augment use was predicted by retroversion >26˚ (p=0.009). Inclination did not predict augment use (p=0.90). Final implant position commonly involved unseating in the posterosuperior quadrant and cancellous exposure in the anteroinferior quadrant.CONCLUSIONS: This retrospective CT-based study of 268 shoulders with GHOA found a 11% prevalence of BD. These deformities were commonly associated with Walch B2 wear patterns. Virtual aTSA planning showed a high failure rate (41%) when correcting to neutral version and inclination. Posteriorly augmented implants were frequently required, and often still involved unseating in the posterosuperior quadrant, increased cancellous exposure in the anteroinferior quadrant, and vault perforation.
