Abstract:AIM: To study the changes and effect factors of posterior corneal surface after small incision lenticule extraction (SMILE) with different myopic diopters. METHODS: Ninety eyes of 90 patients who underwent SMILE were included in this retrospective study. Patients were allocated into three groups based on the preoperative spherical equivalent (SE): low myopia (SE≥-3.00 D), moderate myopia (-3.00 D>SE>-6.00 D) and high myopia (SE≤-6.00 D). Posterior corneal surfaces were measured by a Scheimpflug camera preoperatively and different postoperative times (1wk, 1, 3, 6mo, and 1y). Posterior mean elevation (PME) at 25 predetermined points of 3 concentric circles (2-, 4-, and 6-mm diameter) above the best fit sphere was analyzed. RESULTS: All surgeries were completed uneventfully and no ectasia was found through the observation. The difference of myopia group was significant at the 2-mm ring at 1 and 3mo postoperatively (1mo: P=0.017; 3mo: P=0.018). The effect of time on ΔPME was statistically significant (2-mm ring: P=0.001; 4-mm ring: P<0.001; 6-mm ring: P<0.001). The effect of different corneal locations on ΔPME was significant except 1wk postoperatively (1mo: P=0.000; 3mo: P=0.000; 6mo: P=0.001; 1y: P=0.001). Posterior corneal stability was linearly correlated with SE, central corneal thickness, ablation depth, residual bed thickness, percent ablation depth and percent stromal bed thickness. CONCLUSION: The posterior corneal surface changes dynamically after SMILE. No protrusion is observed on the posterior corneal surface in patients with different degrees of myopia within one year after surgery. SMILE has good stability, accuracy, safety and predictability.

Abstract:Abstract Background: To study the 5-year changes in the posterior corneal surface after small incision lenticule extraction (SMILE) for high myopia.Methods: Eighty eyes received SMILE was included in this prospective study. They were allocated into two groups based on the spherical equivalent: high myopia (40eyes, -7.49±0.70D) and moderate myopia (40eyes, -4.43±0.87D). Certain points of posterior corneal elevation (the central point (PCE), thinnest point (PTE), maximal point (PME), and in various corneal areas) were evaluated using a Scheimpflug camera (Pentacam; Oculus GmbH, Germany) preoperatively and at 6 months and 5 years after surgery. Results: All surgeries were completed uneventfully and no ectasia was developed throng the observation. The safety index and efficacy index were 1.14 and 1.03 in the high myopia group, and 1.16 and 1.06 in the moderate myopia group, respectively. Most of the calculated values in the high myopia group showed a slight increase at 6 months but decreased at 5 years. At 5 years postoperatively, the value of the PTE was significantly lower than at baseline in both groups (P≤0.047); a statistical difference was also revealed in the PME in the moderate group with slight changes (10.15±3.01μm vs. 11.60±4.33μm, P=0.002); no statistical significance was observed in other calculated values (P≥0.067). Similarly, no significant linear correlation was noted between changes in all values and the residual bed thickness either (P≥0.057).Conclusions: SMILE causes no protrusion in posterior corneal surface for correction of high myopia at the follow-up visit of 5 years.

Abstract:PURPOSE: To investigate long-term changes in posterior corneal elevation after small incision lenticule extraction (SMILE).METHODS: Thirty-six eyes of 20 patients (7 male, 13 female) who underwent SMILE were recruited for this prospective consecutive study. The mean preoperative manifest spherical equivalent was -6.24 +/- 1.47 diopters (D) (range: -3.88 to -8.75 D). Posterior corneal surfaces were measured by a Scheimpflug camera (Pentacam; Oculus Optikgerate GmbH, Wetzlar, Germany) preoperatively and at 1 month, 6 months, 12 months, 2 years, and 3 years after surgery. Posterior central elevation (PCE), posterior maximal elevation (PME), and posterior elevation at the thinnest corneal point (PTE) in the central 4 mm area above the best fit sphere (BFS) were analyzed. The BFS was the same across all followup periods and determined by the preoperative data. A P value of less than.05 was considered statistically significant.RESULTS: All surgeries were executed without complications and no case of keratectasia was observed during the follow-up. An uncorrected distance visual acuity of 20/20 or better was achieved in 100% of eyes at the last visit. The mean change of PCE, PME, and PTE at 3 years after the operation was -2.39 +/- 2.85, 0.50 +/- 3.33, and -2.33 +/- 2.90 mu m, respectively. There were significant differences in the measurements of PCE and PTE before surgery and 3 years after surgery (P =.009); however, there was no significant difference in PME before surgery and 3 years after surgery (P = 1.000). No correlation was found between changes in posterior corneal elevation and residual bed thickness, ablation depth, and preoperative thinnest corneal thickness.CONCLUSIONS: The posterior corneal surface, measured using the Pentacam, was stable after SMILE in the long-term follow-up. The cause of the slight backward change of PCE and PTE needs further study.

Abstract:Purpose: To investigate the effects of lenticule creation and subsequent corneal lenticule extraction on corneal deformation parameters during small incision lenticule extraction (SMILE) procedure.Materials and Methods: In this prospective study, 18 eyes of 10 patients (27.90 +/- 7.11 years, -5.64 +/- 2.45 diopters) scheduled for SMILE procedure were enrolled. Changes in the corneal deformation parameters, including deformation amplitude (DA), applanation time(AT1 and AT2), applanation length(AL1 and AL2), corneal velocity(CV1 and CV2), peak distance(P.Dist.), radius and intraocular pressure values were measured preoperatively, immediately after lenticule creation and subsequent to corneal lenticule extraction in all eyes with the Corvis Scheimpflug Technology (Corvis ST, OCULUS, Wetzlar, Germany). Repeated measures analysis of variance (ANOVA) with bonferroni-adjusted post hoc comparisons was performed to investigate changes following each step of the procedure.Results: All surgical procedures were uneventful. A significant difference was detected among the three time points (preoperation, post-lenticule creation and post lenticule extraction) for AT1 (P<0.001), AT2 (P=0.001), DA(P<0.001), and IOP(P=0.002). Bonferroni-adjusted post hoc comparisons indicated that there was no significant change in AT1, AT2, DA, or IOP after lenticule creation (post hoc P>0.05), but there was a significant change in these parameters following subsequent corneal lenticule extraction (post hoc P<0.01), when compared to values obtained pre-operatively. The scheimpflug camera of the Corvis ST demonstrated the intralamellar small gas bubbles formed from the vaporisation of tissue after lenticule creation and a gray zone was observed between the cap and the residual stromal bed after lenticule extraction.Conclusions: There is a significant change in corneal deformation parameters following SMILE procedure. The changes may be caused predominantly by stromal lenticule extraction, while lenticule creation with femtosecond laser may not have an obvious effect on corneal deformation properties.

Abstract:PurposeTo evaluate the changes in posterior corneal elevation in thin corneas after small incision lenticule extraction (SMILE).MethodsIn this prospective study, 97 eyes of 97 patients undergoing SMILE were recruited. Eyes were categorized into the following groups based on the preoperative minimum central corneal thickness (CCT): group A (37 eyes, 480–499 μm), group B (30 eyes, 500–529 μm), and group C (30 eyes, 530–560 μm). The posterior corneal surface was measured with a Pentacam over a 3-year follow-up period. Changes in the posterior corneal elevation at the central point (PCE), thinnest point (PTE), and predetermined area were measured.ResultsNo iatrogenic keratectasia was observed during the follow-up period. The mean changes in PCE, PTE, and the inferior area in group A were 1.14 ± 3.40 μm, −0.11 ± 3.20 μm, and −0.26 ± 1.23 μm, respectively (P ≥ 0.125). Although statistically significant change in the central-4 mm area was noted, the value was quite small (0.98 ± 1.67 μm) and was not higher than that in the other two groups (P = 0.003). For all three groups, the elevation remained stable or showed a backward change in the central annulus, while there was a small forward displacement in the 6-mm optical zone. In group A, changes in elevation values yielded negative statistical correlations with residual bed thickness and CCT (P ≤ 0.006) (except for the inferior area, the 4-mm and 6-mm optical zone).ConclusionsWith a strict preoperative assessment, SMILE achieved good safety and efficacy in correcting myopia in thin corneas and enabled a stable posterior corneal surface over a 3-year follow-up period.SynopsisCareful preoperative assessment and suitable surgical design should be taken to ensure posterior corneal stability after SMILE in thin corneas.

Abstract:PurposeTo investigate the changes of posterior corneal elevation after small incision lenticule extraction (SMILE) for moderate and high myopia.MethodsIn this prospective study, fifty consecutive eyes of thirty patients (10 male, 20 female) who underwent SMILE for myopia and myopic astigmatism were included. Eyes were divided in two groups based on the preoperative spherical equivalent refraction: high myopia group (32 eyes, range -6.25D to -10.00D) and moderate myopia group (18 eyes, range -3.00D to -6.00D). Posterior corneal surfaces were measured by a Scheimpflug camera (Pentacam, Oculus Germany) preoperatively and 1 month, 3 months, 6 months and 12 months post-operatively. Posterior central elevation (PCE) and posterior mean elevation (PME) at 17 predetermined points in the central-4mm area above the best-fit sphere were analyzed.ResultsNo significant difference in the amount of posterior corneal elevation changes in the high myopia group was noted over time (P = 0.23 and P = 0.94 for PCE and PME, respectively). Similarly, the changes in the moderate myopia group before and after SMILE were not significant either (P = 0.34 and P = 0.40 for PCE and PME). A statistically significant correlation was found between the residual bed thickness and the shift of the PCE in the high myopia group at 12 months postoperatively (r = 0.53, P = 0.01).ConclusionsThe results of this study suggest that the posterior corneal surface remain stable within one year after SMILE for both moderate and high myopia. The changes of PCE correlate to the residual bed thickness for high myopia. Long-term changes of posterior corneal surface need further investigation.

Abstract:Background To compare the changes in posterior corneal surface after small-incision lenticule intrastromal keratoplasty (SMI-LIKE) and femtosecond laser-assisted lenticule intrastromal keratoplasty (FS-LIKE) for hyperopia correction. Methods In this prospective comparative randomized study, 23 eyes with hyperopia were recruited. Eyes were categorized into two groups—SMI-LIKE group (11 eyes) and FS-LIKE group (12 eyes). Lenticules from myopia small incision lenticule extraction were implanted into a pocket (SMI-LIKE group) or at a depth of 100 µm under a flap (FS-LIKE group). Posterior corneal elevations in the center, mid-periphery, and periphery, as well as mean keratometry of the posterior corneal surface (Kmb) were measured using a Pentacam over a three-month follow-up. Results All surgeries were completed successfully and no complications occurred. At one day postoperatively, there was a slight backward change with SMI-LIKE and a forward change with FS-LIKE in the central region of the posterior corneal elevation. Conversely, the peripheral area showed forward displacement in SMI-LIKE and an apparent backward change in FS-LIKE. The mid-peripheral regions manifested a backward change after the procedure throughout the entire follow-up in both groups. Kmb exhibited flattening at one month postoperatively and subsequently returned to its original level at three months after SMI-LIKE while in FS-LIKE, Kmb steepened after lenticule implantation with a significant change noted at one day postoperatively ( P = 0.001). Conclusions Posterior corneal surface after SMI-LIKE and FS-LIKE exhibited different change patterns in various corneal regions, with the most prominent change occurring at one day postoperatively during the three-month follow-up. Long-term Changes in Corneal Densitometry and Associated Factors Following Small Incision Lenticule Extraction for Moderate and High Myopia

Abstract:PurposeTo investigate long-term trends in corneal densitometry and associated influencing factors following small incision lenticule extraction (SMILE).MethodsA comparative study was performed among 72 eyes of 38 patients undergoing SMILE. Eyes were divided into moderate myopia [mean spherical equivalent (SE),−4.22 ± 0.68D] and high myopia (mean SE,−7.63 ± 1.09 D) groups. Visual acuity, manifest refraction, corneal topography and corneal densitometry (CD) were evaluated preoperatively and 3 years postoperatively.ResultsThe efficacy indices at last postoperative follow-up (42.47 ± 0.51 months) were 0.92 ± 0.21 and 0.97 ± 0.22, the safety indices were 1.12 ± 0.17 and 1.14 ± 0.21 for high and moderate myopia, respectively (all P > 0.05). CD values in the 0–6 mm zone of the posterior corneal layer was statistically significantly lower than the preoperative values in both groups. Postoperative CD values (0–2 mm zone) in the posterior layer of the high myopia group were statistically significantly lower than in the moderate myopia group (P = 0.025); CD values (2–6 mm zone) in the anterior layer were higher in the high myopia group (P = 0.026). Correlation analyses showed that CD values in the 0–2 mm middle layer were negatively correlated with lenticule thickness in high myopia (r = −0.411, P = 0.016); there was a negative correlation between the CD values (0–6 mm) and corneal oblique trefoil in this group (P < 0.05).ConclusionCorneal transparency in the 0–6 mm zone of the posterior cornea increased following SMILE in moderate and high myopia. Long-term CD values in high myopia may be correlated with oblique trefoil and lenticule thickness.
Influence of optical zone on myopic correction in small incision lenticule extraction: a short-term study

Abstract:Background: To evaluate the influence of preoperative optical zone on myopic correction in small incision lenticule extraction. Methods: In this retrospective clinical study, 581 eyes from 316 patients underwent SMILE were selected, including 117 eyes in the small optical zone group (range from 6.0 to 6.4 mm) and 464 eyes in the large optical zone group (range from 6.5 to 6.8 mm). The measurements included uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), spherical, and cylinder were measured preoperatively and 3 months postoperatively. Propensity score match (PSM) analysis was performed with age, gender, eye (right/left), keratometry and preoperative spherical equivalent between two different groups. The influence of optical zones on postoperative refractive outcomes were evaluated using univariate regression analysis. Results: In total, 78 pairs of eyes were selected by PSM (match ratio 1:1). There were no differences in the age, gender, eye (right/left), keratometry or preoperative spherical equivalent between the small and large optical zone groups. However, the difference of postoperative spherical equivalent was significantly between groups. Patients with larger optical zones had a trend towards less undercorrection ( P = 0.018). Univariate linear regression model analysis found that each millimeter larger optical zone resulted in 8.13% or 0.39D less undercorrection (P < 0.001). The dependency between the optical zones and postoperative spherical equivalent was significant in the higher preoperative myopia group ( r = 0.281, P < 0.001), but not significant in the lower myopia group ( r = 0.028, P = 0.702). Conclusion: The diameter of optical zones would affect postoperative refractive outcomes in small incision lenticule extraction. This study indicated that larger optical zones induced less undercorrection, especially in patients with high myopia.
Effective Optical Zone Following Small Incision Lenticule Extraction: a Review.

Abstract:Small incision lenticule extraction (SMILE) is a "flapless" keratorefractive surgery with excellent safety, efficacy, stability, and predictability for myopia correction. A recent global multicenter study also reported good refractive outcomes for hyperopic SMILE. SMILE has shown advantages including improved biomechanical strength, fewer dry eye symptoms, less corneal denervation, and fewer surgery-induced higher-order aberrations over laser in situ keratomileusis (LASIK). However, night vision complaints, including glare, halos, and starbursts, could still occur after SMILE. These symptoms have been proven to be closely related to the effective optical zone (EOZ), which is defined as the achieved area of corneal ablation. A larger postoperative EOZ may indicate better visual quality, making EOZ an important safety parameter for keratorefractive surgeries. As SMILE has gained wider application globally, the EOZ following SMILE has also been increasingly studied in the field of refractive surgery. This review provides an update on topics related to the EOZ after SMILE, including its measurement and influencing factors, aiming to benefit the personalization of the surgical algorithm and ultimately improve the visual quality after the SMILE procedure.
Effective Optical Zone Following Small Incision Lenticule Extraction for Myopia Calculated with Two Novel Methods

Abstract:PURPOSE: To evaluate the effective optical zone (EOZ) following small incision lenticule extraction in myopic eyes using two novel methods and investigate factors influencing postoperative EOZ. METHODS: In this prospective study, 45 patients (45 eyes) with a mean spherical equivalent of -5.82 +/- 1.58 diopters underwent SMILE and were observed during a 6-month follow-up. Postoperative EOZ was calculated using custom software that automatically distinguishes EOZ on the tangential curvature difference map (EOZc) and total corneal refractive power map (EOZp) of the Pentacam HR (Oculus Optikgerate GmbH). The agreement between the two methods, the difference between postoperative EOZ and programmed optical zone (POZ), and its relationship with parameters including corrected spheres, cylinders, ablation ratio (ablation depth/central cornea thickness), and Q-value change were investigated. RESULTS: The EOZc area was 20.76 +/- 2.43 mm(2) (diameter: 5.04 +/- 0.60 mm) and the EOZp area was 20.22 +/- 4.70 mm(2) (diameter: 5.13 +/- 0.30 mm). Both were significantly smaller than POZ (P <.001). Bland-Altman plots showed 4.44% (2/45) points located outside the 95% limits of agreement. EOZc and EOZp reductions were negatively related to corrected cylinders (r = -0.631, P <.001 and r = -0.594, P <.001, respectively). EOZp reduction was positively correlated with corrected spheres (r = 0.336, P =.024). Subgroup analysis revealed significant differences in EOZc and EOZp reduction between low and high astigmatism groups despite myopia degree. CONCLUSIONS: EOZ after incision lenticule extraction, measured using two novel methods, was smaller than POZ. The reduction of EOZ was negatively correlated with the corrected cylinders.
Corneal morphological changes after small incision lenticule extraction for myopic anisometropia

Abstract:Purpose This research aims to study the corneal morphological changes in adult patients with myopic anisometropia after small incision lenticule extraction (SMILE) and the safety, efficacy, and predictability of clinical outcomes. Methods This was a prospective cohort study. Patients with myopic anisometropia [refractive difference >2.0 diopters (D)] were included in this study who underwent SMILE at our hospital from September 2019 to March 2021. For the two eyes of each patient, the one with higher myopia was defined as group A, and the fellow eye was group B. The follow-up time points were set as 1 week, 1 month, 3 months, and 6 months after the surgery. The data collected were uncorrected and best-corrected distance visual acuity (UDVA and CDVA), spherical equivalent (SE), efficacy and safety indexes, posterior corneal elevation (PCE), anterior and posterior corneal radius of curvature in the 3 mm area at the center of the thinnest point of the cornea (ARC and PRC), and higher-order aberrations (HOAs). Results The study included 36 patients (72 eyes), and the mean age was 25.2 ± 6.4 years. The preoperative SEs were −6.45 ± 1.25 D in group A and −3.76 ± 1.29 D in group B. Six months after surgery, the SEs in groups A and B were −0.09 ± 0.50 D and 0.07 ± 0.47 ( P = 0.059), respectively. The efficacy indexes were 1.06 ± 0.16 in group A and 1.07 ± 0.14 in group B ( P = 0.750). The safety indexes were 1.08 ± 0.14 in group A and 1.12 ± 0.15 in group B ( P = 0.173). The PCE was significantly reduced at 6 months after surgery in pagebreak both groups ( P < 0.05). The ARC was significantly higher than before the surgery ( P < 0.05) in the two groups. The two groups showed significant increases in total HOAs, coma 90°, and spherical aberrations ( P < 0.05). Conclusion SMILE is predictable, effective, and safe in correcting myopic anisometropia. The postoperative changes in HOAs are characteristic.

Abstract:Background: To compare the changes in posterior corneal elevation after small incision lenticule extraction (SMILE) and femtosecond laser-assisted laser in situ keratomileusis (FS-LASIK) in correcting myopia over -9 diopters (D). Methods: In this prospective comparative study, 82 eyes of 82 patients scheduled for refractive correction were recruited. Eyes were randomly assigned to the SMILE group (45 eyes, -10.43 +/- 0.92 D) or FS-LASIK group (37 eyes, -10.97 +/- 1.37 D). The posterior corneal surface was measured using a Scheimpflug camera (Pentacam, Oculus, Germany) preoperatively and at 1 day, 1 month, and 6 months after surgery. Posterior corneal elevation in the central point and central 4-mm area, and in various optical zones above the best-fit sphere, was analyzed. A P value of less than 0.05 was considered statistically significant. Results: All surgeries were completed successfully. The safety index and efficacy index were 1.20 and 1.00, respectively, in the SMILE group, and was 1.10 and 0.90, respectively, in the FS-LASIK group. No significant difference existed in all analyzed data before and at 6 months after surgery in both the SMILE group and the FS-LASIK group. Changes in posterior corneal elevation after FS-LASIK were greater than after SMILE, with no statistical significance (P >= 0.07). In the SMILE group, residual bed thickness was found to be moderately negatively correlated with changes in the elevation in the central area (P <= 0.045); whereas it was positively correlated in the peripheral area (P=0.002). Conclusions: SMILE and FS-LASIK presented stable posterior corneal surface in correction of myopia over -9.0 D at the follow-up visit of 6 months.

Abstract:PURPOSE: To investigate changes in corneal backscatter after small incision lenticule extraction (SMILE).METHODS: One eye was randomly selected from 34 patients who underwent SMILE procedures. Corneal backscatter was analyzed using Scheimpflug technology before surgery and at postoperative 1 day, 1 week, 1 month, 3, 6, and 12 months, and 3 years. Three different corneal depths (the anterior 120 mu m, the center, and the posterior 60 mu m) and three concentric radial zones (0 to 2, 2 to 6, and 6 to 10 mm) were analyzed. Another 40 eyes of 40 patients who underwent femto-second laser-assisted LASIK (FS-LASIK) were examined before surgery and at postoperative 1 day and 3 years as the control group.RESULTS: At postoperative day 1, a significant increase in corneal backscatter was detected in the SMILE group compared to preoperative values in the central 0 to 6 mm zone of the anterior layer and the 0 to 2 mm zone of the central layer (all P < .01). This increase was higher than the increase in the FS-LASIK group in the central 0 to 6 mm zone of the anterior and central layers (all P < .01). No significant differences in corneal backscatter were detected between preoperative values and the 1-week, and 1-, 3-, 6-, and 12-month follow-up visits in the SMILE group (all P > .05). Moreover, a significant decrease in corneal backscatter was detected in the three zones of all three layers in the two groups at 3 years postoperatively compared to preoperative values (all P < .001) and no significant difference was found between the two groups (all P < .05).CONCLUSIONS: Corneal backscatter peaks and declines to the baseline in the early period after SMILE and corneal transparency achieves an improvement within 3 years postoperatively.
Achieved and Functional Optical Zone in Myopic Eyes with High Astigmatism after Small Incision Lenticule Extraction

Abstract:PURPOSE: To investigate the achieved optical zone and func- tional optical zone in myopic eyes with high astigmatism after small incision lenticule extraction (SMILE). METHODS: Seventy-five eyes of 75 consecutive patients with myopia before SMILE were included and divided into two groups (no astigmatism group: without astigmatism vs high astigmatism group: with astigmatism > 2.00 diopters). The achieved optical zone, functional optical zone, and corneal aberrations were compared using Scheimpflug imaging at 6 months postoperatively. Correlations between corneal aber- rations and functional optical zone were analyzed. RESULTS: The mean achieved optical zone diameter was smaller in the no astigmatism group than in the high astigma- tism group. There were no significant differences in achieved optical zone among the different meridians in each group. The mean functional optical zone diameter was significantly smaller in the no astigmatism group than in the high astig- matism group. Compared with the programmed optical zone, significant reduction of the functional optical zone was found in the two groups. The reduction of the functional optical zone was significantly smaller in the high astigmatism group than in the no astigmatism group, except for the 60 degrees, 90 degrees, and 120 degrees meridians. There were significant correlations between the functional optical zone area and the induced corneal aber- rations in the high astigmatism group, except for horizontal coma, whereas significant correlations of functional optical zone with total higher order aberrations and spherical aber- ration were detected in the no astigmatism group.

Abstract:AIM: To investigate the long-term changes of corneal densitometry (CD) and its contributing elements after small incision lenticule extraction (SMILE). METHODS: Totally 31 eyes of 31 patients with mean spherical equivalent of -6.46±1.50 D and mean age 28.23±7.38y were enrolled. Full-scale examinations were conducted on all patients preoperatively and during follow-up. Visual acuity, manifest refraction, axial length, corneal thickness, corneal higher-order aberrations, and CD were evaluated. RESULTS: All surgeries were completed successfully without complications or adverse events. Ten-year safety index was 1.17±0.20 and efficacy 1.04±0.28. CD value of 0–6 mm zones in central layer was statistically significantly lower 10y postoperatively, compared with preoperative values (0–2 mmΔ=-1.62, 2–6 mmΔ=-1.24, P<0.01). There were no correlations between CD values and factors evaluated. CONCLUSION: SMILE is a safe and efficient procedure for myopia on a long-term basis. CD values get lower 10y postoperatively, whose mechanism is to be further discussed.
