Abstract:Creatinine and albumin are crucial biomarkers for health monitoring and their ratio in urine is an effective approach for albuminuria assessment. Herein, to address the challenges of point-of-care and efficient analysis of the biomarkers simultaneously, we developed a fully integrated handheld smartphone-based photoelectrochemical biosensing system. A miniaturized printed circuit board included a potentiostat for photocurrent measurements and single-wavelength light-emitting diodes (LEDs) for photo-excitation, which was controlled with a Bluetooth-enabled smartphone. Graphitic carbon nitride (g-C3N4)/chitosan nanocomposites were modified on a transparent indium tin oxide (ITO) electrode as photoactive materials. Creatinine was detected through chelate formation with copper ion probes, while albumin was recognized specifically by an antigen-antibody reaction based on immunoassay. The biosensing system demonstrated good linearity and high sensitivity, with detection ranges of 100 μg mL-1 to 1500 μg mL-1 for creatinine, and 9.9 μg mL-1 to 500 μg mL-1 for albumin. Spiked artificial urine samples with different concentrations were tested to confirm the practical validity of the biosensing system, where an acceptable recovery rate ranged from 98.7% to 105.3%. This portable photoelectrochemical biosensing platform provides a convenient and cost-effective method for biofluid analysis, which has an extensive prospect in point-of-care testing (POCT) for mobile health.

Abstract:Screen-printed carbon electrodes (SPCE) functionalized with MXene-based three-dimensional nanomaterials are reported for rapid determination of creatinine. Ti3C2TX MXene with in situ reduced AuNPs (MXene@AuNP) were used as a coreactant accelerator for efficient immobilization of enzymes. Creatinine could be oxidized by chitosan-embedded creatinine amidohydrolase, creatine amidinohydrolase, or sarcosine oxidase to generate H2O2, which could be electrochemically detected enhanced by Prussian blue (PB). The enzyme@CS/PB/MXene@AuNP/SPCE detected creatinine within the range 0.03-4.0 mM, with a limit of detection of 0.01 mM, with an average recovery of 96.8-103.7%. This indicates that the proposed biosensor is capable of detecting creatinine in a short amount of time (4 min) within a ± 5% percentage error, in contrast with the standard clinical colorimetric method. With this approach, reproducible and stable electrochemical responses could be achieved for determination of creatinine in serum, urine, or saliva. These results demonstrated its potential for deployment in resource-limited settings for early diagnosis and tracking the progression of chronic kidney disease (CKD).
Design and Preparation of Centrifugal Microfluidic Chip Integrated with SERS Detection for Rapid Diagnostics.

Abstract:A microfluidic SERS chip integrated with blood separation and in-situ detection was designed and fabricated for the rapid detection of clinical blood samples. Each functional unit in the microfluidic SERS chip consist of separation-decantation cavity based on centrifugal separation principle, mixing channels and SERS detection chamber built with integrated nano-Au on Ag film microstructure. The serum creatinine was selected as a typical sample to demonstrate the capability of microfluidic SERS chip. It was found that the creatinine SERS characteristic peaks at 678 cm-1 can be effectively identified and the detection limit could be as low as 4.42 × 10-3 μmol mL-1 in water. The blood samples were also tested in microfluidic SERS chip. The whole separation and test process could be completed within 2 min, which is a significant improvement in the field of creatinine detection. The whole blood of six cases clinical blood samples were also tested, and the results were consistent with the enzymatic results. The developed microfluidic SERS chip has advantages including reduction of the required quantity of blood sample, reusable and easy to operate. It is expected to provide a new method for rapid diagnostics.

Abstract:Rapid, efficient, and point-of-care (POC) detection still presents challenges for bioanalysis. With the increasing demand for biological analysis, various real-time and automated batch analysis techniques are particularly popular. Compared with traditional methods, microfluidic chip has the advantages of miniaturization, automation and low sample consumption. Advanced enzyme-like nanomaterials combined with microfluidic technology can provide various novel portable devices for real-time, fast, efficient, and sensitive bioanalysis. This paper reviews the recent progress of enzyme-like materials-integrated microfluidic platforms for various bioanalysis. The types of enzyme-like nanomaterials and various applications are summarized. In the end, the current challenge and perspective are also discussed. Overall, this review provides a detailed illustration and potential guidance for developing microfluidic-based bioanalysis methods with the assistance of enzyme-like materials.
A Low-cost Creatinine Biosensor by Differential Optical Signal Readout for the Whole Blood Analysis

Abstract:This study developed a low-cost paper-based biosensor for point-of-care (POC) detection of blood creatinine by using differential optical signal readout. Dual-channel photochemical paper-based test strips were fabricated with stackable multilayer films containing pre-immobilized enzymes and reagents for the identification and conversion of creatinine and creatine. Enzyme-linked reactions generated hydrogen peroxide (H 2 O 2 ), which formed a blue oxidized condensate with aniline derivatives. The color depth was quantified via the differential optical signal of the two channels and positively correlated with the concentration of the analyte. This method was first proposed to address the issue of endogenous interferences in the enzymatic assay of creatinine, greatly improving the detection accuracy. The proposed biosensor was calibrated with spiked blood samples, and achieved a wide detection range of 31-1483 μmol/L, showing superior detection performance to general enzymatic methods, especially in the low concentration range. Creatine interference testing demonstrated that the biosensor could resist the interference of ≤ 300 μmol/L endogenous creatine. It is believed that the proposed optical differential biosensor for blood creatinine could enable to pave the way for a daily monitoring system for renal diseases.Clinical Relevance— This stackable multilayer paper-based biosensor provides an enzymatic colorimetric assay of creatinine in whole blood, which can be read out by the differential optical signal to exclude interference from endogenous creatine.

Abstract:The level of creatinine in the human body has clinical implications with regard to a potential association with kidney, muscle, and thyroid dysfunction, hence necessitating fast and accurate detection, especially at the point-of-care (POC) level. This paper presents the design, fabrication, and feasibility of a compact, low-cost and reliable POC photochemical biosensor connected to a smartphone for the determination of whole blood creatinine by differential optical signal readout. Disposable, dual-channel paper-based test strips were fabricated using stackable multilayer films pre-immobilized with enzymes and reagents for the identification and conversion of creatinine and creatine, resulting in dramatic colorimetric signals. A handheld optical reader was integrated with dual-channel differential optical readout to address endogenous interferences in the enzymatic assay of creatinine. We demonstrated this differential concept with spiked blood samples, obtaining a wide detection range of 20–1483 μmol/L and a low detection limit of 0.03 μmol/L. Further interference experiments displayed the differential measuring system's excellent performance against endogenous interference. Furthermore, the sensor's high reliability was confirmed through comparison with the laboratory method, with the results of 43 clinical tests consistent with the bulky automatic biochemical analyzer, with its correlation coefficient R 2 = 0.9782. Additionally, the designed optical reader is Bluetooth-enabled and can connect to a cloud-based smartphone to transmit test data, enabling active health management or remote monitoring. We believe the biosensor has the potential to be an alternative to the current creatinine analysis conducted in hospitals and clinical laboratories, and it has promising prospects for contributing to the development of POC devices.

Abstract:Creatinine has become an important indicator for the early detection of uremia. However, due to the disadvantages of external power supply and large volume, some commercial devices for detecting creatinine concentration have lost a lot of popularity in everyday life. This paper describes the development of a self-powered biosensor for detecting creatinine in sweat. The biosensor can detect human creatinine levels in real time without the need for an external power source, providing information about the body’s overall health. The piezoelectric output voltage of creatininase/creatinase/sarcosine oxidase-modified ZnO nanowires (NWs) is significantly dependent on the creatinine concentration due to the coupling effect of the piezoelectric effect and enzymatic reaction (piezo-enzymatic-reaction effect), which can be regarded as both electrical energy and biosensing signal. Our results can be used for the detection of creatinine levels in the human body and have great potential in the prediction of related diseases.
Zwitterion-Functionalized Cuprous Oxide Nanoparticles for Highly Specific and Enzymeless Electrochemical Creatinine Biosensing in Human Serum

Abstract:Herein, we report the development of a novel enzymeless electrochemical biosensor for highly specific detection of creatinine utilizing zwitterion-functionalized cuprous oxide nanoparticles (Cu2O NPs). We utilized a simple yet effective alternative to traditionally used cover layers based on the surface engineering of Cu2O NPs with N-hexadecyl-N,N dimethyl-3-ammonio-1-propanesulfonate zwitterion. This surface modification generates a pseudo-proton-exchange membrane which electrostatically hinders interfering agents from reaching the electrode surface, thus resulting in highly specific creatinine detection without loss in sensitivity. To fabricate the enzymeless biosensor, single-crystalline Cu2O NPs were synthesized via a sulfonate ion-directed seed aging protocol and were simply drop-cast onto screen-printed carbon electrodes. The shape directional effect of sulfonate ions to induce truncation in the final morphologies of the synthesized Cu2O NPs is also reported for the first time. The creatinine biosensor demonstrated fast response time (<50 s), good reproducibility (RSD = 2.8%, n = 10), and high specificity against interferents like ascorbic acid, acetic acid, glucose, urea, and uric acid. A linear response to creatinine concentration from 10 to 200 μM (R 2 = 0.9876 and LOD = 5.0 μM) was observed, which covers the entire range of physiological creatinine in human serum. Moreover, robust storage stability with a negligible decrease in signal strength over an extended storage period of 6 months was achieved, thus highlighting the practical feasibility for point-of-care testing of creatinine.

Abstract:Herein, this paper initially reports a new colorimetric Tyndall effect-inspired assay (TEA) for simple, low-cost, sensitive, specific, and point-of-care detection of creatinine (an important small biomolecule) by making use of silver nanoparticles (AgNPs) as model colloidal nanoprobes for visual light scattering signaling. The naked-eye TEA method adopts negatively-charged citrate-capped AgNPs (Cit-AgNPs) prepared by sodium citrate reduction. In the presence of alkaline conditions, the creatinine analyte can form carbanion/oxoanion amino tautomers which in turn crosslink with carboxylate groups on the Cit-AgNPs via a hydrogen bonding network to mediate the aggregation of such colloidal nanoprobes showing a significantly-enhanced TE signal that was created and quantified by a hand-held laser pointer pen and a smartphone, respectively. The results demonstrate that the resulting equipment-free method with the TE readout could enable the portable quantification of creatinine with a detection limit of ∼55 nM, which was ∼90–2334 times lower than that obtained from AgNP-based colorimetric approaches with the most common localized surface plasma resonance signaling. Moreover, it shows a larger analytical sensitivity up to ∼580.8227 signal per nM, offering ∼2.4–232-fold improvement in comparison with many of the recent instrumental creatinine nanosensors. The accuracy and practicality of the developed nanosensing system was additionally confirmed with satisfactory recovery results ranging from ca. 98.52 to 100.36% when analyzing a set of real complex human urine samples.

An Automatic Whole Blood Analyzer for Renal Function Analysis with a Centrifugal Microfluidic Device

Abstract:In recent years, chronic kidney disease (CKD) has received widespread attention as one of the fastest growing non-communicable diseases (NCD) worldwide. Here, a clinical biochemical detection system based on a centrifugal microfluidic chip was designed to simplify the rapid detection of renal function indices. A photosensor was used to design an optical signal acquisition structure that can detect products or substrates after enzymatic reactions of uric acid, creatinine, and urea. The weak optical signals collected from this structure were processed using a pre-designed amplifying circuit and a software algorithm to calculate absorbance. The relationship between absorbance and concentration was established according to the Beer-Lambert law. The results indicated good stability and accuracy of the system, which is 21.3 cm × 16.5 cm × 19 cm in size as compared to other detection systems due to the adoption of a centrifugal microfluidic chip. It was portable and easy to operate, in addition to its ability to rapidly detect renal function indices. This system exhibits great potential for the detection of highly integrated point-of-care testing in the future.

Abstract:In this study, we investigate the integration of the enzyme creatinine deiminase into solid-state nanopore walls through electrostatic assembly for the development of creatinine sensors. In these asymmetric single nanochannels, ionic transport is determined by the surface charge inside the channel, resulting in diode-like behavior that rectifies ionic current. The efficiency of such rectification depends on the surface charge density. In the presence of creatinine, the enzymatic reaction generates ammonium, leading to an increase in local pH near the channel, which can be detected through changes in transmembrane ionic transport response. Changes in rectification efficiency can be well correlated with the analyte concentration, allowing for a detection limit of 5 nM creatinine. Furthermore, this solid-state nanopore-based device is capable of sensing in diluted urine samples, showing a good linear correlation between the response and the logarithm of the creatinine concentration over a wide range of concentrations (50 nM-100 μM). These results demonstrate the potential of systems based on the integration of enzymes that induce pH changes and solid-state nanopores for the development of biomarker sensors capable of operating in complex real samples.

Abstract:Microfluidic platforms capable of automated, rapid, sensitive, and quantitative detection of biomarkers from patient samples could make a major impact on clinical or point-of-care (POC) diagnosis. In this work, we realize an automated diagnostic platform composed of two main components: (1) a disposable, self-contained, and integrated microfluidic chip and (2) a portable instrument that carries out completely automated operations. To demonstrate its potential for real-world application, we use injection molding for mass fabrication of the main components of disposable microfluidic chips. The assembled three-layered chip with on-chip mechanical valves for fluid control consists of (1) a top silicone fluidic layer with embedded zigzag microchannels, reagent reservoirs and a negative pressure port, (2) a middle tinfoil layer with patterned antibody/antigen stripes, and (3) a bottom silicone substrate layer with waste reservoirs. The versatility of the microfluidics-based system is demonstrated by implementation of a chemiluminescence immunoassay for quantitative detection of C-reactive protein (CRP) and testosterone in real clinical samples. This lab-on-a-chip platform with features of quantitation, portability and automation provides a promising strategy for POC diagnosis.
Target-responsive DNA hydrogel with microfluidic chip smart readout for quantitative point-of-care testing of creatine kinase MB

Abstract:Swift and effective diagnosis of acute myocardial infarction (AMI) is critical to patient survival due to its serious life-threatening effects and increasing incidence. Creatine kinase MB (CK-MB) is one of the markers of AMI. In this work, we enabled a portable visual quantitative assay of CK-MB by incorporating target-responsive DNA hydrogel with a microfluidic chip. The CK-MB aptamer and the complementary short DNA strand were grafted onto the polyacrylamide strand separately and formed the hydrogel by base-paired linkage. Upon introduction of CK-MB, the aptamer bound to CK-MB. This led to hydrogel dissociation and subsequent release of pre-trapped gold nanoparticles (AuNPs), which is proportional to the concentration of CK-MB. To achieve portable on-site detection, we further combined the hydrogel with a microfluidic chip and utilized the color change caused by the released AuNPs to take picture and analyze the average gray value. Then, as low as 0.027 nM CK-MB could be detected by cell phone. With good portability, visualization, and simple sample handling, this method has great potential for quantitative point-of-care testing (POCT) of targets in resource-constrained settings.

Abstract:A creatinine nanosensor has been developed by PANI (polyaniline) and PDMS (polydimethylsiloxane). The mechanism is attributed to the coupling of the triboelectric nanogenerator (TENG) and the creatinine enzymatic reaction. After enzymatic modification, the triboelectric output from PANI and PDMS carries the information of the ambient creatinine concentration, which is due to the change of PANI electroconductivity in the enzymatic reactions. The nanosensors demonstrate high sensitivity at room temperature. The response reaches 51.42 % when creatinine concentration is 10-3 mol/L. Meanwhile, its good selectivity to creatinine is demonstrated compared with NaCl, glucose, and urea. The nanosensor also supports large flexibility in bending angle measurements (10?-40?), thereby potentially allowing creatinine detection in wearable sensing applications. Experimental results show that the flexible nanosensor can offer continuous and non-invasive detection of creatinine, paving the way for electronic skin and self-powered healthcare systems. Abstract:Enzyme-linked immunosorbent assay (ELISA) is a popular laboratory technique for detection of disease-specific protein biomarkers with high specificity and sensitivity. However, ELISA requires labor-intensive and time-consuming procedures with skilled operators and spectroscopic instrumentation. Simplification of the procedures and miniaturization of the devices are crucial for ELISA-based point-of-care (POC) testing in resource-limited settings. Here, we present a fully integrated, instrument-free, low-cost and portable POC platform which integrates the process of ELISA and the distance readout into a single microfluidic chip. Based on manipulation using a permanent magnet, the process is initiated by moving magnetic beads with capture antibody through different aqueous phases containing ELISA reagents to form bead/antibody/antigen/antibody sandwich structure, and finally converts the molecular recognition signal into a highly sensitive distance readout for visual quantitative bioanalysis. Without additional equipment and complicated operations, our integrated ELISA-Chip with distance readout allows ultrasensitive quantitation of disease biomarkers within 2h. The ELISA-Chip method also showed high specificity, good precision and great accuracy. Furthermore, the ELISA-Chip system is highly applicable as a sandwich-based platform for the detection of a variety of protein biomarkers. With the advantages of visual analysis, easy operation, high sensitivity, and low cost, the integrated sample-in-answer-out ELISA-Chip with distance readout shows great potential for quantitative POCT in resource-limited settings.

In this paper, an enzyme-nanozyme cascade sensing platform was developed for visual creatinine detection. Perovskite oxide BiFeO 3 synthesized by a sol–gel method was applied as a nanozyme, showing excellent peroxidase-like activity. During detection, creatinine was oxidized in turn by three natural enzymes (creatinase, creatininase, and sarcosine oxidase) to produce H 2 O 2 , and H 2 O 2 was then catalyzed by the BiFeO 3 nanozyme, resulting in the change of achromatous 3,3′,5,5′-tetramethylbenzidine (TMB) into blue oxidized TMB (oxTMB). Based on this principle, visual quantification of creatinine was realized. Due to the high stability and catalytic efficiency of nanozyme, the cascade sensing platform can be used to detect creatinine in a broad range of 0.5–150 μM with a detection limit of 0.09 μM. Meanwhile, thanks to the specificity of the natural enzymes, the platform exhibited admirable selectivity for creatinine determination despite the existence of a variety of interfering substances, which were successfully adopted to measure the level of creatinine in human serums. The cascade sensing platform is expected to serve the determination of a large number of biomarkers by simply alternating the natural enzymes.

Abstract:A paper-based analytical device (PAD) with an integrated composite electrode for non-enzymatic creatinine sensing was developed. The electrode was produced and optimization was efficiently accomplished using a rapid digital dispensing approach. The electrochemical sensor was fabricated using an HP D300 digital dispenser to deliver a copper oxide and ionic liquid composite onto an electrochemically reduced graphene modified screen-printed carbon electrode (CuO/IL/ERGO/SPCE) on a PAD. The modified electrode was characterized using electrochemical and microscopic techniques. Electrochemical detection of creatinine was performed on the SPCE using amperometry at a constant potential. Under optimized conditions, the paper-based sensor exhibited a linear range of 0.01-2.0 mM (R2 = 0.99) and the limit of detection was 0.22 μM (S/N = 3, IUPAC definition) for creatinine. The simple fabrication process, low cost, and clinically appropriate creatinine sensitivity make this device applicable for point-of-care use.

Abstract:A rapid, simple and sensitive electrochemical assay of creatinine based on disposable screen-printed carbon electrode mixed with ferrocenemethanol (Fc-SPCE) was developed. The Fc-SPCE was covered with the enzyme ink containing creatinine amidinohydrolase, creatine amidinohydrolase, sarcosine oxidase and horseradish peroxidase to form creatinine biosensor. Then the electrocatalytic behavior of creatinine at creatinine biosensor was investigated employed electrochemical methods. Under optimized conditions, the catalytic current at creatinine biosensor exhibited a linear relationship with creatinine in the range from 5 to 1000 uM with a detection limit of 2.4 uM. Furthermore, the creatinine biosensor showed excellent storage stability for more than 6 months when kept dry at room temperature.
