Abstract:Recent explorations have shown potential drug delivery applications of random and aligned fibrous dosage forms (e.g. patches, dressings, films and inserts). In this study, novel controlled delivery dosage forms, comprising Janus fibers, are designed and fabricated using electrohydrodynamic (EHD) engineering. Model probe and active compound release is shown from dissimilar polymer systems (non-soluble and soluble in nature) which constitute towards Janus systems. Micron scaled aligned Janus fibers comprising polycaprolactone (PCL) and polyvinyl pyrrolidone (PVP) were engineered to host rhodamine B and acriflavine, respectively. The impact of various operating parameters (e.g. flow rate, collector speed, working distance and applied voltage) on resulting mean fiber diameter and shape were optimized. Tensile strain decreased from 116.5 +/- 10.2% to 81.8 +/- 9.5% upon the inclusion of the model compounds, while both PCL/PVP and PCL/PVP/dyes samples exhibited similar tensile strength (similar to 0.65 MPa). FTIR analyses demonstrates PCL/PVP blend behavior and successful incorporation of rhodamine B and acriflavine. Acriflavine release from PCL fibers spanned over a 4-day period, and release of rhodamine B from PVP fibers was just over 6 h. The results indicate the EHD direct-jetting technique enables the precise construction of drug loaded and aligned fibers in a single-step with minimal excipient usage for potential customized drug delivery.

Abstract:Fibrous technologies (such as membranes, films, patches and filters) and their enabling engineering platforms have gained considerable interest over the last decade. In this study, novel fibrous constructs from a unique engineering platform were developed based on hybrid electrohydrodynamic (EHD) technology; incorporating functional and bioactive materials within random and aligned fibrous formulation geometries. Complex constructs were engineered using 3D printing (polycaprolactone, PCL, for sustained delivery) and electrospinning (polyvinylpyrrolidone, PVP, for rapid release) in an intercalating material layer-by-layer format using a side-by-side technological approach. Here, structure generation proceeded with deposition of ordered PCL fibers enabling well-defined void size and overall dimension, after which randomly spun PVP fibers formed a construct overcoat (as a membrane). Differences between polymer dissolution rate, hydrophilicity, mechanical properties and functional material hosting (and linked external auxiliary magnetic field trigger) provided opportunities to modulate antibiotic drug (tetracycline hydrochloride, TE-HCl) release. In vitro cell studies using human umbilical vein blood vessel cell line demonstrated device biocompatibility and Escherichia Coli (E. coli) was selected to demonstrate anti-bacterial function. Overall, a new hybrid engineering platform to prepare customizable and exciting multi-faceted drug release constructs is elucidated.

Abstract:Microfluidic technology has been used for precise drug delivery for many years, but microfluidic wearable devices have mostly been used for skin drug delivery. The application of eye drops is currently one of the most common ways to treat eye diseases. However, due to their low corneal bioavailability and short residence time in tears, topical eye drops must be applied multiple times a day. Contact lenses, as a wearable device for the eye, are a good platform for drug delivery. In this paper, we propose a type of microfluidic contact lens that integrates a microchannel and a micropump and uses a pressure source to trigger the release of a drug. Here, a flat microfluidic chip component is first fabricated by photolithography and then cast into a curved surface by secondary thermosetting. Through experiments, the outlet check valve opening pressure and liquid flow test were studied to prove that the liquid release is controllable. In addition, the microfluidic contact lens has good flexibility, light transmittance, and biocompatibility. Finally, we demonstrate through fluorescence experiments that the microfluidic contact lens can be loaded with different types of drugs in different regions. In general, liquid exchange between the eye and the contact lens can be realized through the mechanical action of blinking without using electronic components, meaning more safety and stability. The mechanical characteristics of a blink can be artificially regulated to a large extent; thus, it is also possible to achieve specific, personalized medicine.

Abstract:The prevalence of ocular chronic diseases, like those associated to diabetes (diabetic retinopathy, macular edema and glaucoma), has increased significantly in the last years. They affect quite negatively people's quality of life, leading to extremely high economic costs both for patients and society. Recent advances in pharmacotherapy have greatly improved their prognosis. However, the efficacy and safety of the treatments currently available are still a concern. Topical drug delivery (DD) is useful to treat diseases of the anterior segment of the eye, but new strategies are needed to increase drug's bioavailability, which is quite low, and to overcome the difficulties that e.g. elderly people face in topical administration. Regarding the posterior segment, the challenge is to reach the target tissues due to the eye's natural barriers. Topical treatments are generally useless in this case. Systemic administration requires large doses to achieve local therapeutic levels and may induce undesirable side effects. Intravitreal route provides direct access to the intraocular environment and is highly effective, but is rather invasive and may origin serious complications, such as endophthalmitis, retinal detachment, intravitreal haemorrhage and cataract. In BioMat Research Group of IST, and within the frame of ORBITAL and SOL projects, we are making efforts to develop and optimize new DD systems for the treatment of diabetic ocular diseases that use noninvasive routes and present sustained drug release capabilities, increasing the drug's bioavailability at the target tissues. We propose the use of soft contact lenses (SCLs) as DD vehicles, due to their high biocompatibility and prolonged contact with the eye. Being frequently replaced, SCLs may be used as drug reservoirs for continued treatments, not only of diseases of the anterior part of the eye, but also of the posterior, using innovative techniques that enhance the drug penetration in the eye. Although in the last years much research has been done on this type of devices, they are still not available in the market due to a number of reasons, such as the need of improving systems efficiency, the difficulty in transposing to industry the preparation methodologies, issues related with sterilization/storage or lack of in vivo and clinical tests. In our studies, we have used different strategies to increase the bioavailability of the drugs and achieve adequate drug release profiles. Besides applying coatings and molecular imprinting techniques, we investigated the formation of prodrugs and drug inclusion in nanostructures that protect them in the biological medium. The incorporation in SCLs of cell penetrating peptides (CPP) that increase drug's ability to cross the corneal epithelium and reach the back of the eye, also led to to promising results. Ex‐vivo tests and mathematical modelling were used to predict the biological performance of the drug‐loaded devices in vivo before performing tests with animal models. Finally, alternative sterilization techniques which allow to process such challenging systems were explored.The obtained results shall contribute for the development of new forms of treatment of ocular diseases that are more predictable, safe and effective, giving response to an unmet clinical need.

Abstract:Abstract: Contact lenses (CLs) have become an essential tool in ocular drug delivery, providing effective treatment options for specific eye conditions. In recent advancements, Therapeutic CLs (TCLs) have emerged as a promising approach for maintaining therapeutic drug concentrations on the eye surface. TCLs offer unique attributes, including prolonged wear and a remarkable ability to enhance the bioavailability of loaded medications by more than 50%, thus gaining widespread usage. They have proven beneficial in pain management, medication administration, corneal healing, and protection. To achieve sustained drug delivery from TCLs, researchers are exploring diverse systems, such as polymeric nanoparticulate systems, lipidic systems, and the incorporation of agents like vitamin E or rate-limiting polymers. However, despite breakthrough successes, certain challenges persist, including ensuring drug stability during processing and manufacturing, controlling release kinetics, and biomaterial interaction, reducing protein adhesion, and addressing drug release during packaging and storage etc. While TCLs have shown overall success in treating corneal and ocular surface disorders, careful consideration of potential issues and contraindications is vital. This review offers an insightful perspective on the critical aspects that need to be addressed regarding TCLs, with a specific emphasis on their advantages and limitations.

Abstract:Pharmaceuticals delivery to the eye sites of interest via the means of contact lenses (CLs) has attracted significant research attention in recent years. Compared with the conventional formulation in eye treatment such as eye drops, CLs administration has shown remarkable advantages in overcoming the challenges involved in ocular drug delivery such as higher bioavailability, longer drug residence and better medication compliance. This review will first detail each of the material components which have been used in the context of CLs, including HEMA, MAA, DMA, NVP, EGDMA, TRIS and PDMS. The pros and cons of each material in tailoring drug release rates of different encapsulated payloads will be discussed, with special focus on their impact on the therapeutic efficiency. In addition, the advancement of recent emerging copolymer CLs hydrogels, originated from these sophisticated monomers with distinct functions, are further summarized into several synthetic strategies in the means of copolymer architecture design and function‐performance relationship in ophthalmic applications. Finally, the possible considerations for future design of multifunctional CLs hydrogels by combing material selection rationales with biological interface science are proposed.

Contact lenses as drug-delivery systems: a promising therapeutic tool

Abstract:The ocular administration of drugs using traditional pharmaceutical forms, including eye drops or ointments, results in low bioavailability, as well as requiring multiple administrations per day, with the consequent danger of therapeutic non-compliance. Although, through the use of pharmaceutical technology, attempts have been made to use various solutions in order to increase bioavailability in the most common pharmaceutical forms, it has not been entirely satisfactory. In this context, contact lenses are presented as drug delivery systems that largely remedy these two major problems and offer other additional advantages. Therefore, the use of contact lenses as drug carrying systems has been increasingly investigated in recent years, as they can increase the bioavailability of these drugs, leading to an increase in therapeutic efficacy and compliance. The main techniques used to achieve this goal are included in this review, including immersion in drug solutions, use of vitamin E barriers, molecular printing, colloidal systems, etc. The most interesting results, depending on the different eye pathologies, are presented. Although the use of contact lenses as a vehicle for the release of active ingredients is a relatively novel strategy, there are already many studies and trials that support it. In any case, further research needs to be carried out to finally reach an effective, safe, and stable product that can be marketed.

Abstract:Topical ocular instillation is the most common route of administration for ophthalmic drugs. Despite being an easy way of drug administration, the ocular drug bioavailability is low (<4% in the anterior chamber), and the duration of therapeutic effect is short, requiring frequent instillations. Controlled drug delivery systems such as contact lenses allow drug release onto the ocular surface for a longer time, which may improve ocular drug bioavailability and duration of action. Furthermore, such topical formulations may have the potential to deliver the drug to the posterior segment of the eye. Nevertheless, the ocular drug pharmacokinetics is not very well known, with a few studies published on contact lens delivery systems. In this presentation, we will take a closer look at this, describing the drug absorption and disposition processes in the eye, and clarifying some wrong assumptions. Research on drug release in tear fluid, corneal and non‐corneal absorption routes, and ocular drug disposition in the anterior and posterior segments are considered along with the quantitative pharmacokinetic data available up to now. Overall, this talk aims to provide better understanding on ocular pharmacokinetics and on the key aspects to consider when designing in vivo pharmacokinetics studies using contact lens delivery systems. Novel Contact Lenses Embedded with Drug-Loaded Zwitterionic Nanogels for Extended Ophthalmic Drug Delivery

Abstract:Therapeutic ophthalmic contact lenses with prolonged drug release and improved bioavailability have been developed to circumvent tedious eye drop instillation. In this work, zwitterionic nanogels based on poly(sulfobetaine methacrylate) (PSBMA) were easily fabricated by one-step reflux-precipitation polymerization, with the advantages of being surfactant-free and morphology controlled. Then, the ophthalmic drug levofloxacin (LEV) was encapsulated into the nanogels. A set of contact lenses with varied nanogel-loading content was fabricated by the cast molding method, with the drug-loaded nanogels dispersed in pre-monomer solutions composed of 2-hydroxyethyl methacrylate (HEMA) and N-vinyl-2-pyrrolidone (NVP). The structure, surface morphology, water contact angle (WCA), equilibrium water content (EWC), transmittance, and mechanical properties of the contact lenses were subsequently investigated, and in vitro drug release and biocompatibility were further evaluated. As a result, the optimized contact lens with nanogel-loading content of 8 wt% could sustainably deliver LEV for ten days, with critical lens properties within the range of recommended values for commercial contact lenses. Moreover, cell viability assays revealed that the prepared contact lenses were cytocompatible, suggesting their significant potential as an alternative to traditional eye drops or ointment formulations for long-term oculopathy treatment.

Hydrogel-Embedded Polydimethylsiloxane Contact Lens for Ocular Drug Delivery

Abstract:Topical administration is the commonly preferred method of administering ophthalmic formulations, with the majority of available medications in the form of eye drops or ointments. However, the topical application of ophthalmological medications has less bioavailability and a short residence time because of the physiological and anatomical constraints of the eye, making efficient ophthalmic drug delivery a challenging task. Microfluidic contact lenses have the advantage of delivering drugs into the eye in a controlled and on-demand manner. Here, we showcase the use of hydrogel-embedded microcavities on PDMS-based contact lenses for ocular drug delivery applications. The fabrication technique adopted here is the spontaneous formation of the spherical cavity by hydrogel monomer droplet, followed by the simultaneous thermal curing of hydrogel and PDMS, creating a spherical cavity as small as 150 μm. The spherical cavity is embedded with pH-responsive hydrogel for on-demand drug delivery. The drug loaded in the hydrogel matrix is released into the ocular environment by diffusion. The spherical cavity with a narrow opening restricts the diffusion to a minimum under normal ocular pH conditions(pH > 6). When the ocular pH reduces (pH < 6), the pH-responsive hydrogel inside the spherical cavity deswell and accelerates the drug release.

Abstract:Traditional ophthalmic drugs, such as eye drops, gels and ointments, are accompanied by many problems, including low bioavailability and potential drug side effects. Innovative ophthalmic drug delivery systems have been proposed to overcome the limitations associated with traditional formulations. Recently, contact lens-based drug delivery systems have gained popularity owing to their advantages of sustained drug delivery, prolonged drug retention, improved bioavailability, and few drug side effects. Various methods have been successfully applied to drug-loaded contact lenses and prolonged the drug release time, such as chemical crosslinking, material embedding, molecular imprinting, colloidal nanoparticles, vitamin E modification, drug polymer film/coating, ion ligand polymerization systems, and supercritical fluid technology. Contact lens-based drug delivery systems play an important role in the treatment of multifarious ophthalmic diseases. This review discusses the latest developments in drug-loaded contact lenses for the treatment of ophthalmic diseases, including preparation methods, application in ophthalmic diseases and future prospects.

Abstract:The design and development of a sustained-release drug delivery system targeting the administration of active pharmaceutical ingredients (APIs) to the eye could overcome the limitations of topically administered eye drops. Understanding how to modify or design new materials with specific functional properties that promote the attachment and release of specific drugs over longer time periods, alongside understanding clinical needs, can lead to new strategic opportunities to improve treatment options. In this paper we discuss two approaches to the design or modification of materials to produce a sustained therapeutic effect. Firstly, we discuss how the synthesis of a peptide hydrogel from a naturally-derived antimicrobial material led to the design of a bandage contact lens which may be able to be used prophylactically to reduce post-surgery infection. Secondly, we discuss how silicone oil tamponade agents used to treat retinal detachments can have adjunctive behaviour to enhance the solubility of the anti-proliferative drug retinoic acid and produce a sustained release over several weeks. These studies are the result of close partnerships between clinical ophthalmologists, materials scientists, and chemists, and illustrate how these partnerships can lead to comprehensive understandings that have the potential to change patient outcomes.

Contact lenses for antifungal ocular drug delivery: a review

Abstract:Introduction: Fungal keratitis, a potentially blinding disease, has been difficult to treat due to the limited number of approved antifungal drugs and the taxing dosing regimen. Thus, the development of a contact lens (CL) as an antifungal drug delivery platform has the potential to improve the treatment of fungal keratitis. A CL can serve as a drug reservoir to continuously release drugs to the cornea, while limiting drug loss through tears, blinking, drainage and non-specific absorption. Areas covered: This review will provide a summary of currently available methods for delivering antifungal drugs from commercial and model CLs, including vitamin E coating, impregnated drug films, cyclodextrin-functionalized hydrogels, polyelectrolyte hydrogels and molecular imprinting. This review will also highlight some of the main factors that influence antifungal drug delivery with CLs. Expert opinion: Several novel CL materials have been developed, capable of extended drug release profiles with a wide range of antifungal drugs lasting from 8 h to as long as 21 days. However, there are factors, such as first-order release kinetics, effectiveness of continuous drug release, microbial resistance, ocular toxicity and potential complications from inserting a CL in an infected eye, that still need to be addressed before commercial applications can be realized.

Abstract:In the advent of an increasingly aging population and due to the popularity of electronic devices, ocular conditions have become more prevalent. In the world of medicine, accomplishing eye medication administration has always been a difficult task. Despite the fact that there are many commercial eye drops, most of them have important limitations, due to quick clearance mechanisms and ocular barrers. One solution with tremendous potential is the contact lens used as a medication delivery vehicle to bypass this constraint. Therapeutic contact lenses for ocular medication delivery have attracted a lot of attention because they have the potential to improve ocular bioavailability and patient compliance, both with minimal side effects. However, it is essential not to compromise essential features such as water content, optical transparency, and modulus to attain positive in vitro and in vivo outcomes with respect to a sustained drug delivery profile from impregnated contact lenses. Aside from difficulties like drug stability and burst release, the changing of lens physico-chemical features caused by therapeutic or non-therapeutic components can limit the commercialization potential of pharmaceutical-loaded lenses. Research has progressed towards bioinspired techniques and smart materials, to improve the efficacy of drug-eluting contact lenses. The bioinspired method uses polymeric materials, and a specialized molecule-recognition technique called molecular imprinting or a stimuli-responsive system to improve biocompatibility and support the drug delivery efficacy of drug-eluting contact lenses. This review encompasses strategies of material design, lens manufacturing and drug impregnation under the current auspices of ophthalmic therapies and projects an outlook onto future opportunities in the field of eye condition management by means of an active principle-eluting contact lens.

Abstract:Ophthalmic drugs used for the treatment of various ocular diseases are commonly administered by eye drops. However, due to anatomical and physiological factors, there is a low bioavailability of the active principle. In order to increase the drug residence time on the cornea to adequate levels, therapeutic contact lenses have recently been proposed. The polymeric support that constitutes the contact lens is loaded with the drug; in this way, there is a direct and effective pharmacological action on the target organ, promoting a prolonged release of the active principle. The incorporation of ophthalmic drugs into contact lenses can be performed by different techniques; nowadays, the soaking method is mainly employed. To improve the therapeutic performance of drug-loaded contact lenses, innovative methods have recently been proposed, including the impregnation with supercritical carbon dioxide. This updated review of therapeutic contact lenses production and application provides useful information on the most effective preparation methodologies, recent achievements and future perspectives.

Abstract:With the prevalence of electronic devices and an aging population, the number of people affected with eye disease is increasing year by year. In spite of a large number of eye drops on the market, most of them do not perform sufficiently, due to rapid clearance mechanisms and ocular barriers. To enhance drug delivery to the eye, a number of novel formulations for ocular diseases have been investigated over recent decades, aiming to increase drug retention and permeation while also allowing for sustained drug release over prolonged periods. The contact lens, initially used to correct visual acuity and beautify female eyes, is one such novel formulation with outstanding potential. Recently, contact lenses have been extensively used for ocular drug delivery to enhance ocular bioavailability and reduce side effects, and are particularly suitable for the treatment of chronic diseases, and thus are of interest to ophthalmic scientists. This review summarizes contact lens classification, methods of preparation, strategies for integrating drugs into lenses, in vitro and in vivo studies, and clinical applications. This review also discusses the current state of ocular drug therapy and provides an outlook for future therapeutic opportunities in the field of ocular drug delivery.

Abstract:As a noninvasive eye disease detection and drug delivery device, contact lenses can improve eye bioavailability and enable continuous drug delivery. In order to monitor the release of drugs in real time, molecularly imprinted contact lenses (MICLs) based on photonic crystals (PCs) were prepared for the treatment of diabetes-related diseases. The specific adsorption of molecularly imprinted polymers on dexamethasone sodium phosphate (DSP) increased the drug loading and optimized the drug release behavior. At the same time, the drug release ensures the rapid color report during the loading and releasing of drugs due to the volume and refractive index change of the hydrogel matrix. The continuous and slow release of DSP by MICLs in artificial tears was successfully monitored through structural color changes, and the cytotoxicity test results showed that the MICL had good biocompatibility. Therefore, MICLs with a PC structure color have great biomedical potentiality in the future.
Laser-Enabled Surface Treatment of Disposable Endoscope Lens with Superior Antifouling and Optical Properties

Abstract:Endoscopes are ubiquitous in minimally invasive or keyhole surgeries globally. However, frequent removal of endoscopes from the patient's body due to the lens contaminations results in undesirable consequences. Therefore, a cost-effective process chain to fabricate thermoplastic-based endoscope lenses with superior antifouling and optical properties is proposed in this research. Such multifunctional surface response was achieved by lubricant impregnation of nanostructures. Two types of topographies were produced by femtosecond laser processing of metallic molds, especially to produce single-tier laser-induced periodic surface structures (LIPSS) and two-tier multiscale structures (MS). Then, these two LIPSS and MS masters were used to replicate them onto two thermoplastic substrates, namely polycarbonate and cyclic olefin copolymer, by using hot embossing. Finally, the LIPSS and MS surfaces of the replicas were infiltrated by silicone oils to prepare lubricant-impregnated surfaces (LIS). Droplet sliding tests revealed that the durability of the as-prepared LIS improved with the increase of the lubricant viscosity. Moreover, the single-tier LIPSS replicas exhibited longer-lasting lubricant conservation properties than the MS ones. Also, LIPSS-LIS replicas demonstrated an excellent optical transparency, better than the MS-LIS ones, and almost match the performance of the reference polished ones. Furthermore, the LIPSS-LIS treatment led to superior antifouling characteristics, i.e., regarding fogging, blood adhesion, protein adsorption, and microalgae attachment, and thus demonstrated its high suitability for treating endoscopic lenses. Finally, a proof-of-concept LIPSS-LIS treatment of endoscope lenses was conducted that confirmed their superior multifunctional response.

Abstract:More is demanded from ophthalmic treatments using contact lenses, which are currently used by over 125 million people around the world. Improving the material of contact lenses (CLs) is a now rapidly evolving discipline. These materials are developing alongside the advances made in related biomaterials for applications such as drug delivery. Contact lens materials are typically based on polymer- or silicone-hydrogel, with additional manufacturing technologies employed to produce the final lens. These processes are simply not enough to meet the increasing demands from CLs and the ever-increasing number of contact lens (CL) users. This review provides an advanced perspective on contact lens materials, with an emphasis on materials science employed in developing new CLs. The future trends for CL materials are to graft, incapsulate, or modify the classic CL material structure to provide new or improved functionality. In this paper, we discuss some of the fundamental material properties, present an outlook from related emerging biomaterials, and provide viewpoints of precision manufacturing in CL development.

Abstract:To improve drug bioavailability, eye drops can be replaced by drug-eluting contact lenses. However, issues of drug leaching from lenses during manufacture and storage, and sterilization, currently limit their commercial application. To address the issues, stimuli-(lysozyme)-sensitive chitosan nanoparticles were developed to provide controlled ocular drug delivery. Nanoparticles were prepared by ionic gelation and characterized by TEM, X-ray diffraction, DSC, and FTIR. In the flux study, conventional-soaked contact lenses (SM-TM-CL) showed high-burst release, while with direct drug-only laden contact lenses (DL-TM-CL) the drug was lost during extraction and sterilization, as well as having poor swelling and optical properties. The nanoparticle-laden contact lenses (TM-Cht-NPs) showed controlled release of timolol for 120 h in the presence of lysozyme, with acceptable opto-physical properties. In the shelf-life study, the TM-Cht-NPs contact lenses showed no leaching or alteration in the drug release pattern. In animal studies, the TM-NPs-CL lenses gave a high drug concentration in rabbit tear fluid (mean = 11.01 μg/mL for 56 h) and helped maintain a low intraocular pressure for 120 h. In conclusion, the chitosan nanoparticle-laden contact lenses demonstrated the potential application to treat glaucoma with acceptable opto-physical properties and addressed the issues of drug-leaching during sterilization and storage.
