Abstract:BACKGROUND The importance of subclinical cerebrovascular disease in the elderly is increasingly recognized, but its determinants have not been fully explicated. Elevated blood pressure (BP) and fluctuation in BP may lead to cerebrovascular disease through ischemic changes and compromised cerebral autoregulation. OBJECTIVE To determine the association of BP and long-term fluctuation in BP with cerebrovascular disease. DESIGN A community-based epidemiological study of older adults from northern Manhattan. SETTING The Washington Heights-Inwood Columbia Aging Project. PARTICIPANTS A total of 686 nondemented older adults who had BP measurements during 3 study visits at 24-month intervals and underwent structural magnetic resonance imaging (corresponding temporally with the third assessment). We derived the mean (SD) of the mean BP for each participant during the 3 intervals and divided the participants into 4 groups defined as below or above the group median (<or=96.48 or >96.48 mm Hg) and further subdivided them as below or above the median SD (<or=7.21 or >7.21 mm Hg). This scheme yielded 4 groups representing the full range of BPs and fluctuations in BP. MAIN OUTCOME MEASURES Differences in white matter hyperintensity (WMH) volume and presence of brain infarctions across groups. RESULTS White matter hyperintensity volume increased across the 4 groups in a linear manner, with the lowest WMH volume in the lowest mean/lowest SD group and the highest WMH volume in the highest mean/highest SD group (F(3,610) = 3.52, P = .02). Frequency of infarction also increased monotonically across groups (from 22% to 41%, P for trend = .004). CONCLUSIONS Compared with individuals with low BP and low fluctuations in BP, the risk of cerebrovascular disease increased with higher BP and BP fluctuations. Given that cerebrovascular disease is associated with disability, these findings suggest that interventions should focus on long-term fluctuating BP and elevated BP.

Abstract:With the prevalence of electronic devices and an aging population, the number of people affected with eye disease is increasing year by year. In spite of a large number of eye drops on the market, most of them do not perform sufficiently, due to rapid clearance mechanisms and ocular barriers. To enhance drug delivery to the eye, a number of novel formulations for ocular diseases have been investigated over recent decades, aiming to increase drug retention and permeation while also allowing for sustained drug release over prolonged periods. The contact lens, initially used to correct visual acuity and beautify female eyes, is one such novel formulation with outstanding potential. Recently, contact lenses have been extensively used for ocular drug delivery to enhance ocular bioavailability and reduce side effects, and are particularly suitable for the treatment of chronic diseases, and thus are of interest to ophthalmic scientists. This review summarizes contact lens classification, methods of preparation, strategies for integrating drugs into lenses, in vitro and in vivo studies, and clinical applications. This review also discusses the current state of ocular drug therapy and provides an outlook for future therapeutic opportunities in the field of ocular drug delivery.

Abstract:Traditional ophthalmic drugs, such as eye drops, gels and ointments, are accompanied by many problems, including low bioavailability and potential drug side effects. Innovative ophthalmic drug delivery systems have been proposed to overcome the limitations associated with traditional formulations. Recently, contact lens-based drug delivery systems have gained popularity owing to their advantages of sustained drug delivery, prolonged drug retention, improved bioavailability, and few drug side effects. Various methods have been successfully applied to drug-loaded contact lenses and prolonged the drug release time, such as chemical crosslinking, material embedding, molecular imprinting, colloidal nanoparticles, vitamin E modification, drug polymer film/coating, ion ligand polymerization systems, and supercritical fluid technology. Contact lens-based drug delivery systems play an important role in the treatment of multifarious ophthalmic diseases. This review discusses the latest developments in drug-loaded contact lenses for the treatment of ophthalmic diseases, including preparation methods, application in ophthalmic diseases and future prospects.

Abstract:The prevalence of ocular chronic diseases, like those associated to diabetes (diabetic retinopathy, macular edema and glaucoma), has increased significantly in the last years. They affect quite negatively people's quality of life, leading to extremely high economic costs both for patients and society. Recent advances in pharmacotherapy have greatly improved their prognosis. However, the efficacy and safety of the treatments currently available are still a concern. Topical drug delivery (DD) is useful to treat diseases of the anterior segment of the eye, but new strategies are needed to increase drug's bioavailability, which is quite low, and to overcome the difficulties that e.g. elderly people face in topical administration. Regarding the posterior segment, the challenge is to reach the target tissues due to the eye's natural barriers. Topical treatments are generally useless in this case. Systemic administration requires large doses to achieve local therapeutic levels and may induce undesirable side effects. Intravitreal route provides direct access to the intraocular environment and is highly effective, but is rather invasive and may origin serious complications, such as endophthalmitis, retinal detachment, intravitreal haemorrhage and cataract. In BioMat Research Group of IST, and within the frame of ORBITAL and SOL projects, we are making efforts to develop and optimize new DD systems for the treatment of diabetic ocular diseases that use noninvasive routes and present sustained drug release capabilities, increasing the drug's bioavailability at the target tissues. We propose the use of soft contact lenses (SCLs) as DD vehicles, due to their high biocompatibility and prolonged contact with the eye. Being frequently replaced, SCLs may be used as drug reservoirs for continued treatments, not only of diseases of the anterior part of the eye, but also of the posterior, using innovative techniques that enhance the drug penetration in the eye. Although in the last years much research has been done on this type of devices, they are still not available in the market due to a number of reasons, such as the need of improving systems efficiency, the difficulty in transposing to industry the preparation methodologies, issues related with sterilization/storage or lack of in vivo and clinical tests. In our studies, we have used different strategies to increase the bioavailability of the drugs and achieve adequate drug release profiles. Besides applying coatings and molecular imprinting techniques, we investigated the formation of prodrugs and drug inclusion in nanostructures that protect them in the biological medium. The incorporation in SCLs of cell penetrating peptides (CPP) that increase drug's ability to cross the corneal epithelium and reach the back of the eye, also led to to promising results. Ex‐vivo tests and mathematical modelling were used to predict the biological performance of the drug‐loaded devices in vivo before performing tests with animal models. Finally, alternative sterilization techniques which allow to process such challenging systems were explored.The obtained results shall contribute for the development of new forms of treatment of ocular diseases that are more predictable, safe and effective, giving response to an unmet clinical need.

Abstract:Ophthalmic drugs used for the treatment of various ocular diseases are commonly administered by eye drops. However, due to anatomical and physiological factors, there is a low bioavailability of the active principle. In order to increase the drug residence time on the cornea to adequate levels, therapeutic contact lenses have recently been proposed. The polymeric support that constitutes the contact lens is loaded with the drug; in this way, there is a direct and effective pharmacological action on the target organ, promoting a prolonged release of the active principle. The incorporation of ophthalmic drugs into contact lenses can be performed by different techniques; nowadays, the soaking method is mainly employed. To improve the therapeutic performance of drug-loaded contact lenses, innovative methods have recently been proposed, including the impregnation with supercritical carbon dioxide. This updated review of therapeutic contact lenses production and application provides useful information on the most effective preparation methodologies, recent achievements and future perspectives.

Abstract:Glaucoma, a chronic eye disease characterized by high intraocular pressure, poses challenges in drug delivery due to ocular physiological barriers. Traditional formulations have limited residence time and are quickly drained away, leading to decreased efficacy and increased adverse drug reactions. Drug-laden contact lenses have garnered attention as a potential solution for sustained action of drug delivered to the eye. However, the development of such lenses presents challenges including lens swelling, transmittance, wearability, drug loading, and drug release. Incorporation of drug molecules into contact lens material can cause lens swelling and shape change, affecting lens fit and comfort. Drug loading and release are also critical factors. To overcome these challenges, researchers have explored strategies such as the use of nanoparticles. This review provides an overview of the materials used to prepare contact lenses, including silicone hydrogel, monomers like EGDMA, MAA, and NVP, and other additives like vitamin E and hyaluronic acid. The article comprehends and evaluates the key parameters for drug-loaded contact lenses, including transmittance, swelling index, oxygen permeability, drug loading, and drug release. The review also discusses drug loading with modified nanoparticles such as micelles, coated nanoparticles, and metallic nanoparticles. By selecting appropriate contact lens materials and optimizing key parameters, drug-laden contact lenses hold promise for improved care of glaucoma patients. Further research in this field can pave the way for more effective and convenient treatment options.

Abstract:Abstract: Contact lenses (CLs) have become an essential tool in ocular drug delivery, providing effective treatment options for specific eye conditions. In recent advancements, Therapeutic CLs (TCLs) have emerged as a promising approach for maintaining therapeutic drug concentrations on the eye surface. TCLs offer unique attributes, including prolonged wear and a remarkable ability to enhance the bioavailability of loaded medications by more than 50%, thus gaining widespread usage. They have proven beneficial in pain management, medication administration, corneal healing, and protection. To achieve sustained drug delivery from TCLs, researchers are exploring diverse systems, such as polymeric nanoparticulate systems, lipidic systems, and the incorporation of agents like vitamin E or rate-limiting polymers. However, despite breakthrough successes, certain challenges persist, including ensuring drug stability during processing and manufacturing, controlling release kinetics, and biomaterial interaction, reducing protein adhesion, and addressing drug release during packaging and storage etc. While TCLs have shown overall success in treating corneal and ocular surface disorders, careful consideration of potential issues and contraindications is vital. This review offers an insightful perspective on the critical aspects that need to be addressed regarding TCLs, with a specific emphasis on their advantages and limitations.

Abstract:Topical ophthalmic solutions (eye drops) are becoming increasingly popular in treating and preventing ocular diseases for their safety, noninvasiveness, and ease of handling. However, the static and dynamic barriers of eyes cause the extremely low bioavailability (<5%) of eye drops, making ocular therapy challenging. Thus, drug-eluting corneal contact lenses (DECLs) have been intensively investigated as a drug delivery device for their attractive properties, such as sustained drug release and improved bioavailability. In order to promote the clinical application of DECLs, multiple aspects, i.e., drug release and penetration, safety, and biocompatibility, of these drug delivery systems were thoroughly examined. In this review, we systematically discussed advances in DECLs, including types of preparation materials, drug-loading strategies, drug release mechanisms, strategies for penetrating ocular barriers, in vitro and in vivo drug delivery and penetration detection, safety, and biocompatibility validation methods, as well as challenges and future perspectives.

Abstract:Pharmaceuticals delivery to the eye sites of interest via the means of contact lenses (CLs) has attracted significant research attention in recent years. Compared with the conventional formulation in eye treatment such as eye drops, CLs administration has shown remarkable advantages in overcoming the challenges involved in ocular drug delivery such as higher bioavailability, longer drug residence and better medication compliance. This review will first detail each of the material components which have been used in the context of CLs, including HEMA, MAA, DMA, NVP, EGDMA, TRIS and PDMS. The pros and cons of each material in tailoring drug release rates of different encapsulated payloads will be discussed, with special focus on their impact on the therapeutic efficiency. In addition, the advancement of recent emerging copolymer CLs hydrogels, originated from these sophisticated monomers with distinct functions, are further summarized into several synthetic strategies in the means of copolymer architecture design and function‐performance relationship in ophthalmic applications. Finally, the possible considerations for future design of multifunctional CLs hydrogels by combing material selection rationales with biological interface science are proposed.

Abstract:The interface between material science and ophthalmic medicine is witnessing significant advances with the introduction of biopolymers in medical device fabrication. This review discusses the impact of biopolymers on the development of ophthalmic devices, such as intraocular lenses, stents, and various prosthetics. Biopolymers are emerging as superior alternatives due to their biocompatibility, mechanical robustness, and biodegradability, presenting an advance over traditional materials with respect to patient comfort and environmental considerations. We explore the spectrum of biopolymers used in ophthalmic devices and evaluate their physical properties, compatibility with biological tissues, and clinical performances. Specific applications in oculoplastic and orbital surgeries, hydrogel applications in ocular therapeutics, and polymeric drug delivery systems for a range of ophthalmic conditions were reviewed. We also anticipate future directions and identify challenges in the field, advocating for a collaborative approach between material science and ophthalmic practice to foster innovative, patient-focused treatments. This synthesis aims to reinforce the potential of biopolymers to improve ophthalmic device technology and enhance clinical outcomes.

Abstract:<p>Contact lenses are ocular prosthetic devices used by over 150 million people worldwide. Primary applications of contact lenses include vision correction, therapeutics, and cosmetics. Contact lens materials have significantly evolved over time to minimize adverse effects associated with contact lens wearing, to maintain a regular corneal metabolism, and to preserve tear film stability. This article encompasses contact lens technology, including materials, chemical and physical properties, manufacturing processes, microbial contamination, and ocular complications. The function and the composition of the tear fluid are discussed to assess its potential as a diagnostic media. The regulatory standards of contact lens devices with regard to biocompatibility and contact lens market are presented. Future prospects in contact lens technology are evaluated, with particular interest given to theranostic applications for in situ continuous monitoring the ocular physiology.</p>

Abstract:Eye disorders affect a substantial portion of the global population, yet the availability of efficacious ophthalmic drug products remains limited. This can be partly ascribed to a number of factors: (1) inadequate understanding of physiological barriers, treatment strategies, drug and polymer properties, and delivery systems; (2) challenges in effectively delivering drugs to the anterior and posterior segments of the eye due to anatomical and physiological constraints; and (3) manufacturing and regulatory hurdles in ocular drug product development. The present review discusses innovative ocular delivery and treatments, encompassing implants, liposomes, nanoparticles, nanomicelles, microparticles, iontophoresis, in situ gels, contact lenses, microneedles, hydrogels, bispecific antibodies, and gene delivery strategies. Furthermore, this review also introduces advanced manufacturing technologies such as 3D printing and hot-melt extrusion (HME), aimed at improving bioavailability, reducing therapeutic dosages and side effects, facilitating the design of personalized ophthalmic dosage forms, as well as enhancing patient compliance. Abstract:Contact lens is the transparent hemisphere-shaped ophthalmic product worn by a large population all across the globe for improving vision. It is applied to the cornea of the eye not only for improving vision but also for the delivery of drugs. The contact lens has many benefits but it also causes several ophthalmic infections which include amoebic keratitis, fungal, bacterial, and viral keratitis. The development of bacterial biofilm in lens cases exacerbates the ocular infection after the application of contact lenses in the cornea. Contact lens often associated with discomfort and dry eye. Abstract:Contact lenses (CL) have become an immensely popular means of vision correction, offering comfort to millions worldwide. However, the persistent issue of biofilm formation on lenses raises significant problems, leading to various ocular complications and discomfort. The aim of this review is to develop safer and more effective strategies for preventing and managing microbial biofilms on CL, improving the eye health and comfort of wearers. Taking these into consideration, the present study investigates the intricate mechanisms of biofilm formation, by exploring the interplay between microbial adhesion, the production of extracellular polymeric substances, and the properties of the lens material itself. Moreover, it emphasizes the diverse range of microorganisms involved, encompassing bacteria, fungi, and other opportunistic pathogens, elucidating their implications within lenses and other medical device-related infections and inflammatory responses. Going beyond the challenges posed by biofilms on CL, this work explores the advancements in biofilm detection techniques and their clinical relevance. It discusses diagnostic tools like confocal microscopy, genetic assays, and emerging technologies, assessing their capacity to identify and quantify biofilm-related infections. Finally, the paper delves into contemporary strategies and innovative approaches for managing and preventing biofilms development on CL. In Conclusion, this review provides insights for eye care practitioners, lens manufacturers, and microbiology researchers. Contact lenses for antifungal ocular drug delivery: a review

Abstract:Introduction: Fungal keratitis, a potentially blinding disease, has been difficult to treat due to the limited number of approved antifungal drugs and the taxing dosing regimen. Thus, the development of a contact lens (CL) as an antifungal drug delivery platform has the potential to improve the treatment of fungal keratitis. A CL can serve as a drug reservoir to continuously release drugs to the cornea, while limiting drug loss through tears, blinking, drainage and non-specific absorption. Areas covered: This review will provide a summary of currently available methods for delivering antifungal drugs from commercial and model CLs, including vitamin E coating, impregnated drug films, cyclodextrin-functionalized hydrogels, polyelectrolyte hydrogels and molecular imprinting. This review will also highlight some of the main factors that influence antifungal drug delivery with CLs. Expert opinion: Several novel CL materials have been developed, capable of extended drug release profiles with a wide range of antifungal drugs lasting from 8 h to as long as 21 days. However, there are factors, such as first-order release kinetics, effectiveness of continuous drug release, microbial resistance, ocular toxicity and potential complications from inserting a CL in an infected eye, that still need to be addressed before commercial applications can be realized.

Abstract:A wide-angle laser Doppler spectrometer has been combined with a number of high-field-strength cytopherometer designs for the purpose of studying the electrophoretic mobility (EPM) of biological particle and cells in non-physiological and physiological media. This produces a typical Doppler shift of 100 Hz for an EPM of 1.0 micrometers s-1 V-1 cm, and a shift-to-width ratio of greater than 10 for human erythrocytes in physiological saline. The unit uses two equal-intensity beams of identical pathlength, crossing in the measuring volume at an angle of 40--90 degrees in air and can function in either a fringe or coherent mode. The resolution for human cells and bioparticles is comparable with conventional cytopherometry but the data collection rate is many times higher.

Abstract:Notable advances in materials science and in surgical techniques make the management of cataract by replacement of the opaque crystalline with an intraocular lens (IOL), one of the most cost-effective interventions in current healthcare. The usefulness and safety of IOLs can be enhanced if they are endowed with the ability to load and to sustain drug release in the implantation site. Drug-eluting IOLs can prevent infections and untoward reactions of eye tissues (which lead to opacification) and also can act as drug depots for treatment of several other ocular pathologies. Such a myriad of therapeutic possibilities has prompted the design of drug-IOL combination products. Several approaches are under study, namely combination of the IOL with an insert in a single device, soaking in drug solutions, impregnation using supercritical fluids, coating with drug/polymer layers, and covalent grafting of the drug. The advantages/limitations of each technique are discussed in the present review on selected examples. Although more in vivo data are required, the information already available proves the interest of some approaches in ocular therapeutics.
